WVMA Member Presents at UW Department of Pediatrics Grand Rounds By Bailey Quam, WVMA communications and project specialist On March 9, Wisconsin Veterinary Medical Association (WVMA) Member, Dr. Keith Poulsen spoke at the University of Wisconsin School of Medicine and Public Health Department of Pediatrics Grand Rounds. His talk titled, “Is ‘I grew up on a farm’ Enough to Protect your Patients from Zoonotic Disease?”, was a part of the WVMA’s Public Health and Food Safety Committee’s goal to educate human doctors on the One Health model. “The talk was relevant for the group because it focused on zoonotic disease” says Dr. Poulsen, who has a split appointment at the School of Veterinary Medicine and Wisconsin Veterinary Diagnostic Laboratory. “The pediatrics department is a great place to start sharing information because children are at a higher risk for infections of zoonotic and foodborne disease compared to adults.” Grand Rounds is held weekly and the audience consists of people from the Department of Pediatrics at UW Health including medical students, residents, fellows, and attending clinicians from UW Hospital, St. Mary’s Hospital, and Meriter Hospital. Dr. Poulsen’s presentation focused on two specific diseases, salmonella and listeria. “Many times physicians see these diseases and know they are related to contact with animals or animal products, but they don’t always totally understand the background of why this occurs,” says Dr. Poulsen. “I spent a significant amount of time explaining why kids are exposed and what things like dairy bull calves and silage are.” According to the American Academy of Pediatrics (AAP), “pediatricians are asked to consider animal-related issues in daily practice by taking a history of animal contact and consulting and collaborating with local veterinarians.” Speaking at Grand Rounds is one way to encourage relationships and contacts within the two medical fields. There were three take home messages for Dr. Poulsen’s presentation: 1) Just because you live on a farm, doesn’t mean you are immune from zoonotic pathogens; 2) Less than 10 percent of children grow up on farms compared to over 50 percent of their grandparents and great grandparents, so they don’t have exposure immunity; and 3) Use the WVMA and Wisconsin veterinarians as a resource for your practice. Dr. Poulsen’s presentation has opened the door for future One Health interactions and the WVMA has been invited to present again in the future l

November 4, 1980, I distinctly remember the first Presidential election I participated in. Yes, 1980 seems like a long time ago, because it is; 37 years is a long time. Today, 1980 seems like a different time; my point of reference has certainly changed; has the process changed? The 2016 election cycle was unlike any previously witnessed. The United States electorate has so many platforms to engage in the political process, so many voices are now being heard. The news cycle relentlessly grinds on 24 hours a day, seven days a week. It’s easy to feel like our voices aren’t heard, like we have no input, like we’re irrelevant. WVMA members have a valuable platform amplifying our voices, a resource advocating on our behalf. We need to be aware of it, we need to be involved with it, we need to financially support it. The Wisconsin Legislature’s actions directly effects many WVMA members’ professional and business activities. WVMA political advocacy asks the legislature to take action on behalf of Wisconsin veterinarians and veterinary practices during each session. During every election cycle, legislative candidates ask veterinarians to support campaign activities. The VetMed PAC, a state Political Action Committee, was formed by the WVMA, enabling membership to easily support candidates for Wisconsin state elective offices. Unlicensed practice of veterinary medicine undermines our profession, jeopardizing the health and welfare of our patients. The Veterinary Examining Board oversees the veterinary practice act and was formerly housed in the Wisconsin Department of Professional Services. The WDPS was ineffective at investigating cases of unlicensed practice. The WVMA and VetMed PAC, under Attorney Jordan Lamb’s extraordinary leadership, lobbied the Wisconsin Legislature to move the Veterinary Examining Board from the WDPS to Wisconsin Department of Agriculture, Trade and Consumer Protection. This was accomplished in 2015, the investigation of unlicensed veterinary practice is actively progressing, numerous cases have reached successful conclusions. What does VetMed PAC do? • Contributes to campaigns of candidates who recognize and understand the critical roles veterinary medicine plays in communities and in managing animal and human health issues. • The WVMA Political Action Committee selects candidates from both sides of the aisle to receive VetMed PAC financial support. • The PAC committee solicits WVMA members’ financial contributions to the VetMed PAC. The contributions must be personal contributions, no corporate contributions can be accepted. How can you help? • The WVMA Political Action Committee welcomes member input regarding candidates understanding of veterinary medicine. This is your platform, an opportunity for your voice to be heard. • Contributions of $25 or $50 made by numerous WVMA members goes a very long way. Reflections from Haiti: A life-changing experience

In February, I had the opportunity to travel to Haiti as part of a veterinary mission trip. Haiti is the second poorest country in the western hemisphere and it was a life-changing experience that I have reflected on every day since returning. Instead of sharing what we did every day, I will share what I found to be the most eye opening. We were in the northern in part of the country near Cap Haitien where we worked with animals and their owners. Here are my reflections based on my experiences with the culture, people and animals we had the opportunity to provide care to. • Haiti is in desperate need of jobs. People spend their days just standing around and burning time because jobs are so hard to come by. When it comes to countries that are less off, you might think you are helping the people by giving them “free stuff” but in reality we aren’t helping that much. Aid providers need to help establish skills that jobs and businesses can be built from. This is important for their long-term sustainability. We all have our days when work doesn’t go as we had hoped and we get frustrated or down. After Haiti, I am blessed I have a job and can provide for my family. • The role of animals in Haiti is significantly different than in the U.S. For example, the horses are small and used to carry people and product up and down the mountains and to markets. Some have saddle sores and it puts tourists in the dilemma of do I pay for the service of the horse to carry me up to the Citadel or not? While it is natural instinct to say, “No, I will walk”, the reality is, if the horse isn’t working, then the family won’t eat that day or week. For many families, the horse is their only source of income. It comes down to whether a child is going to eat or not, which makes those decisions even more difficult. Thanks to Dr. Judy Batker, the Citadel horses receive veterinary care at least twice a year through her donation of time. She records their health status and shared with me that the horses look significantly better than they did a few years ago. • Government is something most of us complain about, but after this trip, I am thankful for the government system we have. Haiti’s roads are horrible. Dirt roads with non-stop holes the size of vehicles. There is no garbage disposal and the people believe it is the government’s job to collect it. Since the government doesn’t, garbage is thrown anywhere and everywhere. CONTINUED ON PAGE 9

VFD Rule and Implementation – What we know to-date By DATCP staff Liz Meils, state apiarist, and Heather Bartley, feed program specialist As of January 1, 2017 the Veterinary Feed Directive (VFD) Rule is now in effect. This means that in order to purchase and apply medically important antibiotics to your beehive, you need either a VFD Order or a Prescription from a licensed veterinarian—depending on the treatment you require. Before a vet will issue one of the above, you will need to establish a Veterinarian Client Patient Relationship (VCPR). A “veterinarianclient-patient relationship” is defined in Title 21, the Food, Drug & Cosmetic Act, Section 530.3(i), and means a relationship between a veterinarian, a client and the patient in which all of the following apply: 1. A veterinarian has assumed the responsibility for making medical judgments regarding the health of (an) animal(s) and the need for medical treatment, and the client (the owner of the animal or animals or other caretaker) has agreed to follow the instructions of the veterinarian; 2. There is sufficient knowledge of the animal(s) by the veterinarian to initiate at least a general or preliminary diagnosis of the medical condition of the animal(s); and 3. The practicing veterinarian is readily available for follow-up in case of adverse reactions or failure of the regimen of therapy. Such a relationship can exist only when the veterinarian has recently seen and is personally acquainted with the keeping and care of the animal(s) by virtue of examination of the animal(s), and/or by medically appropriate and timely visits to the premises where the animal(s) are kept. The VCPR is the prerequisite that establishes an important professional supervision over the beekeeper by the veterinarian. Start by asking your current vet for other animals in your care if he/she will work with your bees, otherwise check out http://www. honeybeeveterinaryconsortium.org/ or http://beevets.com/ to see if there are any vets in your area willing to work with bees. Another option includes local bee clubs; several around Wisconsin have identified vets in their club’s area willing to work with all the member beekeepers.
(NADA N138-938, N008-804, N095-143 ). While a VFD is not required to purchase a Type A medicated article (like Terramycin concentrate), in order for a beekeeper to apply the terramycin/powdered sugar mix to a hive, that beekeeper is still required to acquire a VFD Order issued by a vet prior to manufacturing a medicated feed using the Type A medicated article. VFD orders can be filled by manufacturers/ distributors of VFD medicated feeds. Lists of approved mills and distributors by state can be found on FDA.gov, keyword search: “VFD distributor list by state”. A distributor can manufacture/distribute/sell a “reasonable” amount of medicated feed to fulfill the VFD order. That means, if the VFD calls for 0.8 lbs. they can sell the whole 2 lb. package. That being said, a beekeeper will need to be aware that they cannot use the remaining 1.2 lbs. without a second VFD. A VFD is for a ONE-TIME treatment, and typically does not require a diagnosis. That, again, is the purpose of the professional supervision established by the VCPR. Work with your veterinarian to make sure the VFD is filled out completely and identifies the number of hives and the duration/ intervals of treatments. Currently there are no approved VFD drugs that allow refills or reorders. VFDs have an expiration date, to be determined by the veterinarian, not to exceed 6 months. If the VFD order will expire before completing the duration of use on the order, the beekeeper will need to contact the vet to request a new VFD order. Prescriptions are written for oral dosage medications with active ingredients Tylosin tartrate or Lincomycin hydrochloride such as Tylan or Lincomix. Prescriptions will typically be filled by the veterinarian. Another option would be to identify a pharmacy that is an authorized compounding pharmacy.
In Wisconsin, it will be the Federal Food & Drug Administration, DATCP Feed Program and the Department of Safety and Professional Services (DSPS) who will inspect for compliance with this rule. Immigration Concerns Affecting Veterinary Medicine By Raluca Vais-Ottosen, Esq., immigration attorney, DeWitt Ross & Stevens, s.c. Much has been discussed about immigration in recent months. With increased emphasis on enforcement of existing laws and proposals for changes in the law, it is timely to highlight the requirements of current law, and comment on how a proposed change might affect veterinary medicine practices.

And if the employer takes an adverse action against a new hire triggered by an over-documented practice, despite the employee being authorized to work, then the penalties may range from $445 to $3,563 per each instance. The penalty may increase to $17,816 per instance for repeat violators.
Comply with the I-9 Employment Eligibility Verification Process
In sum, there is a fine line between doing your due diligence to avoid hiring undocumented workers, and avoiding document discrimination and over-documenting. As noted above, I-9 mistakes can be very costly. However, many I-9 mistakes can be corrected and, if done properly, may shield the employer from liability in the event of a government audit.
Employers have one universal obligation under immigration law: to complete the I-9 Employment Eligibility Verification Process for each new hire. This applies to all employers and all new employees, including U.S. citizens. The purpose of the I-9 process is to spot new hires who are not authorized to work in the U.S. At the same time, the process is designed to avoid discrimination against foreign nationals who are authorized to work. It is reasonable to anticipate that I-9 audits will receive greater emphasis. Liability Exposure For I-9 Violations Many employers mistakenly believe that so long as all their workers are U.S. citizens or otherwise authorized to work, it does not matter if their I-9 verifications are done correctly. In fact, failure to properly conduct the I-9 verification exposes the employer to civil liability that can range from $216 to $2,156 per each form found to be incorrect. This is even if the employee to whom the incorrect I-9 pertains is a U.S. citizen. If the employee turns out to lack employment authorization, then the liability increases and it may range from $539 to $4,313 per unauthorized worker, and it can go up to $21,563 per unauthorized worker in the case of repeat violators. This can quickly add up to six or seven-digit fines against employers. For instance, a Washington orchard was recently fined $2.25 million for improper I-9 practices and hiring undocumented workers. In addition, no company is too small to be audited or penalized. A mom and pop convenience store with only two employees, both U.S. citizens, was investigated and fined because neither employee had an I-9 on file. “Better Safe Than Sorry� Not A Good I-9 Strategy For fear of hiring undocumented workers, many employers request additional documents from job applicants, especially those who appear to be foreign. But over-documenting may constitute employment discrimination with penalties ranging from $178 to

There is no quota on the number of TN visas that can be issued each year, so eligible applicants do not have to wait in line or be subject to a lottery to get this type of visa. However, the TN visa is a product of the NAFTA agreement, which the current administration has under review. If the United States pulls out of the NAFTA agreement, then the TN visa classification will disappear. If the NAFTA agreement is renegotiated, the status of TN visas will depend on the terms of the revised agreement. If the TN visa is eliminated, then Veterinarians and Animal Scientists from Mexico and Canada will have to compete with their colleagues from the rest of the world for H-1B visas, a category that has a quota and has been so oversubscribed in recent years that the government subjects all applications to a lottery. There are 85,000 H-1B visas issued annually, and 20,000 of those are reserved to individuals with a graduate degree from a U.S. university. For fiscal year 2017, there were over 240,000 H-1B applications filed in the first five days of the filing season. Eliminating the TN visa classification will have a significant impact on veterinary clinics, dairy farms and animal breeders who rely on Veterinarians and Animal Scientists from Mexico and Canada to fill these positions. If your practice relies on the TN visa, you should begin assessing alternative options early on, to avoid being left without legal status if the TN visa is eliminated or is modified. l

Ionizing Radiation Protection for Veterinary Medical Clinics – Part II By Leslie Ptak, Department of Labor/OSHA compliance assistance specialist ALARA is an acronym for “as low as reasonably achievable.” Long a principle of radiation safety, it means making every reasonable effort to maintain exposures to ionizing radiation as far below the dose limits as practical. Every reasonable effort means that a combination of engineering and work practice controls are used every time an exposure is taken to reduce the potential for employee exposure. The four elements of ALARA are to (1) generate the lowest radiation beam needed and collimate (and address scatter) (2) increase shielding (3) decrease durations of exposure and (4) increase distance from the beam/scatter. This articles discusses ALARA as they pertain to veterinary medical clinics. Generate the lowest radiation beam needed (and address scatter) An x-ray device that is working optimally will emit primary radiation beams that deliver appropriate levels of radiation with minimal scatter. How do you know your device is working properly? In Wisconsin, the WDHS RPS tells you. Every registered x-ray machine is tested by the WDHS RPS on a regular basis, usually every three or four years. Radiation meters (ionization chambers and solid state detection meters) are used to evaluate the primary beam and any scatter radiation.
• Aprons must be 0.5 millimeter (mm) of lead equivalence. There are 0.25 mm lead equivalence aprons on the market but they are not appropriate for veterinary use. • Gloves, worn when the employee’s hands get close to the primary beam while holding the patient, must contain 1.58 mm of lead equivalence covering both the back and palm of the hand. There are no circumstances where half gloves, which have lead shielding only on the back of the hand, can be used when getting close to the beam.

Other than the small garbage cans in the facility we slept at, I don’t recall seeing any garbage containers. I don’t think the people of Haiti even see the garbage anymore, it has simply become a part of the norm. • There is always discussion on whether or not we have too many environmental laws in the U.S. I am not going to debate that issue, but the point I want to make is, some laws are good and help the future generations. Haiti is a country that has been stripped down environmentally and has significant erosion issues. The country used to be a major grower of mahogany lumber. Today, it is non-existent. It has all been consumed and nothing replaced. There is no planning for the future, survival from day-to-day is the goal of their people. Water is limited and the safety of the water is questionable. We only drank bottled water and even used bottled water to brush our teeth. Coke is more abundant and cheaper for them to consume than water. • Health care in Haiti is extremely limited, if people can even afford it. We witnessed a lady laying on a log bench while we were driving down the mountain one day. We stopped to see what was wrong and discovered that her large toe was rotting off. She couldn’t afford medical care and had never had her foot medically looked at. Those I was with told me she will end up losing her life to Gangrene. That image will never leave my mind.
all families can afford schooling, so they must make the decision of what child to send or not send any. I look at some of the kids that attend school with my three children, they don’t care about learning or showing up. While in Haiti, the kids are asking tourists for money to use for their schooling and are always trying to find ways to get to school. There are a few government run schools. However, the government is typically very late in paying or doesn’t pay the teachers. So, teacher motivation to show up is an obstacle. • In the U.S. we are happy when we grow our export markets. It helps sustain and grow our commodity prices. However, I saw the other side of that equation in Haiti. While Haiti grows rice and chickens, it is more cost effective for the country to import U.S. rice, eggs and grade B chicken (meat) for consumption. Those imports do nothing to help build up businesses in Haiti. Short-term benefit versus long-term sustainability. Another not so easy dilemma. • Animals are used for meat. Bovine are not milked, there is very little dairy products consumed. All animals are significantly smaller than their American counterparts. For the most part, they roam freely looking for food or have a rope around their neck or horns and are staked out daily and walked home at night. They don’t receive supplemental feeds. What they can find to eat on the landscape or in a garbage pile is what they eat. Parasites are a big issue and we treated several animals with screw worms in Haiti.

Ionizing Radiation Protection for Veterinary Medical Clinics – Part II
work shift when x-rays are taken. There are a variety of badge types, with various price points. One current price is $6.25/ quarter, including the cost of analyzing the badge. • Pregnant employees should have a second badge worn (1) at their waist, but (2) inside the apron. • DHS 157 includes a “Top Ten Dosimeter Do’s and Don’ts” in its X-Ray Regulatory Guide Radiation Safety Program for all industries including veterinary. For legal, historical and badge analytical reasons, these “Top Tens” should always be followed. • Employees have a right to their own dosimetry exposure reports. They also can see the results of other employees’ exposure results as long as there is no personally identifiable information included. • The WDHS RPS recommends that employees track their own dosimetry results over time. They have created a form (Form No. F-45003, dated 05/09) with which to do so, and it is available on the WDHS RPS web site. Distance The farther an individual is from the beam and scatter radiation, the less exposure they have. This is an ALARA principle. The WDHS
RPS has set a distance of 6.5 feet (two meters) as the approximate distance from the beam with which to stand (if you can). In veterinary care, it might not always be possible to stand 6.5 feet back. A well communicated policy about “keeping your distance when possible” shows concern to employees and gives them permission to protect themselves, just as they take care of their patients. • If an employee does not need to be in the x-ray room when the exposure is taken, the employee should step out. • If the employee can move 6.5 feet from the beam when the exposure is taken, they should. • Remember the inverse square law. For example, doubling distance from the source will decrease radiation exposure by a factor of four. Keep good records– Radiation surveys and dosimetry results must be maintained for at least thirty years according to OSHA’s Access to Medical and Exposure Records rule, 1910.1020. WDHS RPS goes further than that and requires in DHS 157.31 that personnel dosimetry reports be retained for as long as the site is a registered x-ray user. Employees must have access to your site specific Radiation Safety Policy, the Notice to Employees and their dosimetry records. l

Reflections from Haiti
These are some of the reflections I have thought about every day since returning. None of these issues are easy to solve. The people are extremely kind and appreciative of the care provided for their animals. (They are required to pay a small fee for the veterinary service, so they value the service they receive). I want to thank Dr. Judy Batker for her passion and dedication to making a difference in the lives of the animals in Haiti. Through the

regular animal care she provides, I witnessed the friendships she has made, trust she has developed and attitudinal shift she has created in the lives of the Haitiens. I encourage everyone to take the opportunity to engage in one missionary trip in your lifetime. It will leave you appreciative and thankful for the small problems we have in our everyday lives. l
AVMA Update
By Dr. Douglas Kratt, WVMA House of Delegates delegate and Dr. Steve Erickson, WVMA House Delegates alternate delegate • On January 13, the AVMA House of Delegates (HOD) held its second Veterinary Information Forum, with three new topics. o The HOD discussed issues related to unauthorized practice of veterinary medicine. It recommended that the Board of Directors refer review of the AVMA Model Veterinary Practice Act to appropriate entities, potentially leading to the addition of language specific to unlicensed practice and/or development of an official policy on unlicensed practice. o The House approved recommending that the Board of Directors consider developing a policy to encourage collaborative work with the U.S. government regarding the proper utilization and employment of veterinarians to ensure food safety and security decisions are made with the highest consideration to public health. o The House also discussed possible ethical issues of veterinarians participating in programs which bundle products, provide for volume discounts, or result in cash rebates. It was concluded that the current Principles of Veterinary Medical Ethics addresses these situations adequately. • The HOD amended and approved a new policy on responsible breeding of companion animals. o The proposed policy was amended during discussion in reference committees and was passed unanimously by the House of Delegates. o The purpose of this policy is to support responsible breeding practices that reduce or eliminate the health and welfare concerns associated with inherited conditions, not to condemn or stigmatize specific breeds. o The policy reads: • To maximize the health and welfare of companion animals, the AVMA supports research in genetic and inherited disorders to better educate the profession and breeders on identifying and minimizing inherited disorders in companion animal breeding programs. To assist with this, the AVMA encourages veterinarians to pursue continuing education in the emerging area of genetic disease in companion animals. The AVMA also encourages veterinarians to educate breeders, companion animal owners, and the public on the responsibilities involved with breeding and selecting companion animals. Drs. Erickson and Kratt are currently seeking future topics to discuss in the Veterinary Information Forum to be held this summer. Hantavirus Infections Prompt Addition to 10-Day Reportable Disease List By Department of Agriculture, Trade and Consumer Protection Recently confirmed cases of human illness caused by Seoul hantavirus have prompted the Wisconsin State Veterinarian, Dr. Paul McGraw, to add the disease to the list of diseases that should be reported to the Department within 10 days. Human cases have occurred in Wisconsin among residents who cared for domestic rats in Wisconsin ratteries and among Illinois residents exposed to ratteries in their state. Considerable movement of these rats has taken place via sales and swapping of breeding stock among the Wisconsin facilities and to other ratteries in both Illinois and Wisconsin. Seoul virus is an old world hantavirus whose reservoir is rats. Seoul virus can only be transmitted by rats and not by other rodents or animals. Rats infected with Seoul virus will not show any signs of illness, but the infection spreads readily between rats. Humans become infected by direct exposure to urine, droppings, or saliva from infected rats, or exposure to dust from their bedding. Transmission may also result from bites from infected rats. Seoul virus is not transmitted person-to-person. Human infections with this virus are rarely reported in the United States.
