50-Year Membership Award Recipients Named The Wisconsin Veterinary Medical Association (WVMA) is pleased to announce the ten members who received 50-year membership recognition during the WVMA Convention in Madison on October 11, 2018. They include: James Andrews, DVM – Dr. Andrews graduated from the University of Minnesota College of Veterinary Medicine in 1964. He accepted a commission in the U.S. Army until 1966, then attended graduate school at the University of Minnesota from 1967-1968. Dr. Andrews was at a mixed practice in Durand, Wis., from 1969-1982, then moved to a mixed practice in St. Croix Falls, Wis., where he stayed until he retired in 2003. Dr. Andrews and his wife, Fay, currently reside in Shafer, Minn. Raymond Backe, DVM – Dr. Backe is a 1966 graduate of the Purdue University College of Veterinary Medicine. He practiced at a small animal practice in New Berlin, Wis. Dr. Backe was a member of the state advisory committee that initiated the veterinary tech program through Madison Area Technical College in the late ’60 and early ‘70s and was the small animal chair of the Program Committee. He feels very blessed to have had the opportunity to practice veterinary medicine for 40 years, and found the experience to be extremely rewarding. He and his wife, Margaret, reside in Amherst Junction, Wis. Victor Eggleston, DVM – Dr. Eggleston is a 1966 graduate of Michigan State University’s College of Veterinary Medicine. Upon graduation he purchased a solo, mixed practice in Stockbridge, Mich. After about two years, he sold that practice and moved to Elkhart Lake, Wis., to be a partner at the St. Anna Veterinary Clinic, a primarily dairy practice. Dr. Eggleston remained there for 32 years, until he retired in 2000. Following retirement, he supervised a Johnes field research project for the UW Veterinary School from 2002-2009. Dr. Eggleston served on the Insurance and Legislative Committees, and spent time on the Executive Board, where he served as president in 2001. He and his wife, Joyce, now reside in New Glarus, Wis. Morris Link, DVM – Dr. Link graduated from the University of Minnesota College of Veterinary Medicine in 1968. He practiced at a small animal clinic in Madison, Wis. Early in his practice, he did extensive orthopedic surgeries for local practitioners. Dr. Link was part of a heavy breeder practice, working with breeders from surrounding states. During his 45 years of practicing veterinary medicine, he developed a real expertise in OFA certification of dogs’ hips. Dr. Link still resides in Madison. CONTINUED ON PAGE 10

I am writing this column on my return trip from the World Buiatric Congress (WBC) that was held in Sapporo, Japan. My wife, Penny, and I enjoy the travel experiences that these conferences provide. Arriving in a distinctly different culture with a foreign language presents a challenge for communication. Thank goodness the official language of the conference was English, but elsewhere it was not. No man is an island unto himself, so communicate we did as best we could outside the friendly confines of the convention center. While we have familiarity with the English of primary Spanish speakers here in Wisconsin, we had no familiarity with Japanese. Thank goodness for universal non-verbal body language to facilitate the transfer of ideas and information. Head nods or shakes and pointing and gesturing go a long way helping bridge the communication gap. There is also picture communication (restaurant menus had pictures of each entrée), a very useful tool to make a dinner selection which doesn’t surprise you. These challenging experiences have given me cause to reflect on communication in general. My old practice in Bear Creek was one of the clinics of the ambulatory rotation for the veterinary school in Madison. I always stressed the importance of good communication to students for successful practice and maintaining good client relationships. Doctor-client communication was one of the topics at this year’s WBC.
Motivational interviewing is a counseling approach that helps individuals overcome ambivalence to change. Ambivalence is the state of indecision because of conflicting concerns. Motivational interviewing is a counseling approach that was developed to help people overcome substance abuse. The study was to see if the principles of motivational interviewing could be applied and effective for veterinarian-client interactions. Herd health consultation and recommendations are in essence a counseling service to help farmers make changes to reduce disease and improve profitability. Veterinarians make a wide range of recommendations on an ongoing basis, from drug and reproductive protocols to footbath strategies, calf health management and many others. Sometimes recommendations are quickly embraced, other times rejected or ignored. We all have been frustrated at times by farmers’ ambivalence to some of our suggestions. Farmers are indecisive, often because they question: “Will the return be of more value than the time, effort and expense of the change?” Or simply: “Is the change worth the risk?”
The study provided motivational interview training to a group of dairy veterinarians with a range of experience. They recorded an audio file of advisory communication before and after the training to determine whether communication approaches were altered post-training. A Bold Step Forward – Part 2

In August, I introduced the changes the WVMA Executive Board is implementing in the association -- the how and the why. This month, I will address some of the internal (office) changes that are being executed. Over the past few months, I have looked at three major areas: IT structure and security, how we interact when handling registrations and payments, and staff roles and responsibilities. As a result, the following changes have been implemented. 1. We hired an outside IT company to complete a comprehensive analysis of our systems -- security, efficiencies, costs, and making sure we are meeting best practices for IT systems. Some deficiencies were found, so changes were made to allow us to meet all best practices. As a side note, with the speed that IT changes, I would recommend business owners or managers perform these “health” checks every few years. 2. I’ve received a lot of questions as to why we have transitioned to 100% online registration. This change was driven by the need to be PCI compliant. While some of you are comfortable faxing or mailing your registration forms with credit card numbers on them, we are not comfortable receiving that confidential information through those means. By handling registrations online, no credit card information is viewed or stored in house. All payments run through Authorize.net, a leading processing gateway. 3. I reviewed the human resources skills needed to implement the changes to the WVMA that have been approved by the Executive Board. Adjustments to our staff have been made. Jodi and Tracie are both part-time employees that bring a level of experience and expertise to the WVMA that will take us to the next level! Dr. Katie has transitioned into the Executive Director role of the Food Armor® Foundation, and we are leveraging Bailey’s strengths and passion by transitioning her into the new Continuing Education Model while also continuing her student services efforts. Kelsey will use her business administration and leadership education, as well as her internship and volunteer experiences, to keep things organized and running efficiently in the office. There has been and will continue to be change. I appreciate your patience as we work through and implement the new WVMA model, which will better serve all of our members, bring the entire veterinary medical community in Wisconsin closer together and position us for long-term value and success. In closing, we must lead for transformation, not transaction, and from vision, not mission. These changes are about moving forward, rather than adhering to the status quo, to justify continued existence. The organizations that change the world are those that become experts at listening to their target clients’ real needs and develop customized, innovative solutions to solve them. Wisconsin Administrative Rules Update By Jordan Lamb, JD, DeWitt Ross & Stevens S.C. The WVMA advocacy team has been involved in the development of several administrative rule changes over the past legislative session. Below is a summary of two recent rule changes. Referrals of Patients to Complementary and Alternative Medicine Practitioners, Wis. Admin. Code § VE1 & VE 2 (pending) The Wisconsin Veterinary Examining Board (VEB) has developed proposed revisions to ch. VEB 7, Wis. Adm. Code, to clarify the circumstances under which a veterinarian may make a referral of a veterinary client to another licensed professional, or supervise a certified veterinary technician, for the provision of complementary, alternative, or integrative therapies on the client’s animal. Under current law, there is a definition of veterinary complementary, alternative, and integrative therapies (“CAITs”) in VE 1.02 (3m). However, that definition does not specify how these CAITs are to be treated within the practice of veterinary medicine. There have been numerous requests made to the VEB from veterinarians, certified veterinary technicians, and members of other licensed professions to clarify the referral relationship that a veterinarian may have with these other professionals, and the delegation to certified veterinary technicians, for the provision of CAITs upon a veterinarian’s animal patients.
In addition, the VEB proposes to create an additional provision within this rule to allow a veterinarian to delegate to a certified veterinary technician, who is not a licensed professional in one of these disciplines, but who has the education, training, experience, and demonstrated abilities, the performance of a CAIT on the animal patient under the supervision of the veterinarian and within the VCPR. This rule has had a public hearing before the Department of Agriculture, Trade and Consumer Protection and is now awaiting legislative review when the Legislature resumes its session in 2019. It is anticipated that this rule will become effective in 2019. Veterinary Professional Assistance Program for Wisconsin Veterinarians and Veterinary Technicians, Wis. Admin. Code § VE 11 (enacted)
The legislature directed the Veterinary Examining Board to create a professional assistance program for Wisconsin veterinarians and veterinary technicians in order to provide a resource for those struggling with substance abuse and mental health issues. The first step toward creating the program was the creation of new Wisconsin Administrative Rule Ch. VE 11, “Veterinary Professional Assistance Program,” which will establish a procedure for addressing allegations that a person licensed or certified by the VEB has practiced as a veterinarian or veterinary technician while impaired Accordingly, in this proposed rule change, the VEB proposes to by alcohol or other drugs or that his or her ability to practice is create a rule that specifies that a veterinarian may make such a referral, so long as the veterinarian confirms the other professional impaired by alcohol or other drugs, and for assisting a person licensed by the VEB under this chapter who requests to participate is licensed in good standing with the Wisconsin Department of in the procedure or who requests assistance in obtaining mental Safety and Professional Services, to perform the type of CAIT health services. Create Loyal Customers with Customer Experience By Jessica Eversoll, Botham, INK. Customer Experience is your customer’s perception of your brand based on any and all interactions with you, your team, your website, your products, your marketing and your services. One negative interaction with any aspect of your brand will be a negative customer experience and a negative perception of your brand. One positive interaction with any aspect of your brand can create a loyal customer. Your brand has defined what your customer can consistently expect when they interact with your business (e.g. how the customer is greeted or the checkout experience). While your business has defined its promise as a brand, it is the customer who decides whether or not your business has delivered its promise. This is defined by the customer experience; how much did you exceed or fall short of the customer’s expectations? The customer experience is the determining factor of whether or not a customer will continue doing business with a brand. From the customer’s perspective, a good customer experience (as defined by Forrester) is made up of three things: 1. Useful -- Did we deliver value? 2. Usable -- Was the value easy to find and engage with? 3. Enjoyable -- Was the experience emotionally engaging? While we know it’s important to keep our interactions in line with our brand’s message, how can we improve customer experience? 1. Quality of Care. Define your ideal customer experience and begin to implement this overall experience. 2. Customer Journey. Review every touch point with your customers and determine whether you met these three criteria. This starts from recommendations and continues all the way through post-service follow up. 3. Educate your team on the importance of customer experience, building a customer-centric culture. 4. Review and improve the environments in which customers experience your brand, including: a. In clinic b. Online c. Digital d. Phone and email 5. Review and improve direct (in-clinic) relationships and indirect relationship materials including reviews, advertising, press, journals and word of mouth (recommendations). Use every phone call, in-clinic visit and social media account as an opportunity to connect and nurture the relationships with your customers. With so many touch points, it seems like creating a powerful, memorable and consistent customer experience is more complicated than ever. Technically, that’s true. However, customers want the same thing they’ve always wanted, and that’s good customer service. They may be more demanding. They may want problems resolved faster. But that’s understandable because technology has given us the tools to provide that kind of speed. Use your technology to help improve the customer experience, not replace customer interaction. Once you align customer expectations of your brand with an ability to consistently deliver on these expectations – across channels, touchpoints, and environments – you’ll be able to enjoy meaningful differentiation and reap the rewards of fierce customer loyalty. Ronald Niedfeldt, DVM – Dr. Niedfeldt is a 1966 graduate of the Iowa State University College of Veterinary Medicine. His career began while he was still in veterinary school, when he entered the Army Veterinary Corps. Following veterinary school, Dr. Niedfeldt spent nearly 10 years in mixed animal practice at the Whitewater Veterinary Clinic. He then started the Prairie Animal Hospital in Beloit, Wis., where he practiced until retiring in 2008. Dr. Niedfeldt is a past Rock Valley VMA president and a past WVMA Executive Board member. He and his wife, Janet, reside in Lake Mills, Wis. Thomas Rutz, DVM – Dr. Rutz is a 1968 graduate of the University of Minnesota College of Veterinary Medicine. He is still practicing today at a dairy practice in Brussels, Wis. Dr. Rutz’s fondest memories of his career thus far include working during a time when solo practice was commonplace and having the opportunity to work with four generations of a dairy farm family. He resides in Brussels. John Samuelson, DVM, MS – Dr. Samuelson graduated from Kansas State University’s College of Veterinary Medicine in 1961. Following graduation, he practiced in a mixed practice in Horton, Kan., for two months before entering the US Air Force, where he spent 2 ½ years as a base veterinarian. Following his service, Dr. Samuelson attended graduate school at Cornell University. He received his MS degree in 1966. He then spent 30 years practicing at a general veterinary practice in Dodgeville, Wis., until he retired in 1997. Dr. Samuelson is a past member of the WVMA Insurance Committee. He and his wife, LaVerna, reside in Blue Mounds, Wis. Richard Schmidt, DVM – Dr. Schmidt graduated from the Iowa State University College of Veterinary Medicine in 1964. During his career, he was part of a mixed practice in Lodi, Wis. He was active in the WVMA, serving on the Executive Board, Finance Committee and as treasurer. While practicing veterinary medicine, Dr. Schmidt saw a shift in the focus of the large animal practice. Early on, he said, large animal practice was directed to the individual animal. Later, the emphasis moved to solving herd problems. Dr. Schmidt retired in 1994. He and is wife, Vonda, reside in Lodi. Geraldine Schumann, DVM – Dr. Geraldine Schumann is a 1967 graduate of the Iowa State University College of Veterinary Medicine. She began her veterinary career in Aurora, Ill., where she was a relief veterinarian. She then started a small animal practice, the Hartland Animal Hospital, with her husband, Dr. Kenneth Schumann. Dr. Geraldine has fond memories of their six children spending time at their animal hospital. As they got older, they all worked there too. While none of them became veterinarians, she said they are all successful in the professions they chose. Dr. Geraldine retired in 2004. Kenneth Schumann, DVM – Dr. Kenneth Schumann graduated from the Iowa State University College of Veterinary Medicine in 1965. After graduation, he worked in Whitewater, Wis., until he received his draft notice and volunteered for the Army Corp for two years. In 1968, he started a small animal practice, the Hartland Animal Hospital, with his wife, Dr. Geraldine Schumann. Dr. Kenneth retired from his practice in 2004 but continues to stay involved. He has been doing volunteer work in Haiti for 24 years, including projects such as training veterinary technicians and goat husbandry. The Schumanns reside in Oconomowoc, Wis. l

Dr. Reyher’s second presentation was a qualitative review of the veterinarian’s effectiveness to inspire client behavior change. The data set for this review was the same group who went through the motivational interview training. Here they found that farmer confidence in the veterinarian and the perception of shared understanding and values are important factors toward improving effectiveness of advisory success. Dr. Reyher shared some testimonial comments from the participating veterinarians. To paraphrase one of the comments: “I can’t believe how well this worked; I wish I’d known about this earlier.”

Effective communication is one of the pillars of successful practice. I think that motivational interview training would be very valuable to veterinarians for improving their ability to motivate clients to change for the better. These techniques would be especially valuable for young veterinarians who are in the initial stages of building rapport and trust with clients early in their careers. WVDL Welcomes Dr. Lorelei Clarke to the Madison Laboratory Dr. Clarke joined the Pathology service in June of 2018. Lorelei was born and raised in Hawaii and earned her DVM (2012) from Colorado State University. She recently completed a combined residency-PhD program in anatomic pathology at the University of Georgia. Her doctoral work focused on arboviral diseases of white-tailed deer, and she has broader interests in infectious diseases of wildlife and livestock. Outside of the lab, she enjoys hiking, camping, marathon running, water sports, and other outdoor activities. Welcome to the WVDL Dr. Clarke!

Health Saving Accounts By Marilyn Cook, Professional Insurance Programs A health savings account (HSA) is a tax-exempt savings account used for out-of-pocket health expenses, and can only be used by those enrolled in a high deductible health plan (HDHP). While an HDHP generally has lower monthly premiums than traditional plans, the deductible is also considerably higher and must be met (with the exception of preventive care) before the plan pays any benefits. Using pretax funds for out-of-pocket expenses before reaching the deductible can reduce overall health care costs. Both employers and employees may contribute money to the employee’s HSA. Employee contributions through payroll deductions are withheld on a pretax basis, and other employee HSA contributions are tax deductible. Employer HSA contributions are a tax-deductible employee benefit expense. Additionally, unlike flexible spending accounts, the employee is not required to use up or subsequently lose (“use it or lose it”) the money deposited into an HSA at the end of the plan year. Employees own and control the money in the HSA. Decisions on how to spend the money are made by the employee without relying on a third party or a health insurer. Here are some of the frequently asked questions we get about HSAs: Can HSA funds be used to pay health expenses incurred by a spouse or a dependent?
How much do employers have to contribute to their employees’ HSAs?
Yes. HSA funds can be withdrawn tax free for the qualified medical expenses for a spouse (including same-sex spouse) or tax-exempt dependent, even if they are not covered by the HDHP.
Employers may contribute as much or as little as they want as long as the total employer and employee contributions do not exceed the legal limits (in 2018, $3,450 if self-only HDHP coverage or $6,900 if family HDHP coverage; in 2019, $3,500 if self-only HDHP coverage or $7,000 if family HDHP coverage).
What happens if there is money left in the HSA at the end of the year? Individuals may keep unused funds in their HSA accounts from one year to the next and may use HSA funds to pay qualified expenses from a previous year, as long as they were incurred after the HSA was established. Can employees take HSAs with them when they retire or change employment? Yes. Individuals may take HSA funds with them when retiring or changing employment and may designate a beneficiary to receive the funds upon the individual’s death. Do employers own their employees’ HSAs? Can employers control how employees spend the money in HSAs? Employers do not own their employees’ HSAs. The employee fully owns the contributions to the account as soon as they are deposited, just as with a personal checking or savings account to which an employer would deposit the employee’s compensation.

Do HSA contributions have to be made in equal amounts each month? No. Employers can contribute in a lump sum or in any frequency they wish. However, employers must keep in mind that the funds belong to the employee after they are deposited. Background: I attended the University of Minnesota - Twin Cities for my undergraduate studies, earning a BS in Chemistry in 1997. After college I worked as a research and development chemist for a pharmaceutical company.
My passion for veterinary science began when I started raising and breeding English Springer Spaniels and training them for field trials. In 2006, I entered the Iowa State University College of Veterinary Medicine. I graduated cum laude with a Doctorate of Veterinary Medicine in 2010. After graduation, I practiced at a mixed animal practice. In 2011, I purchased Northwoods Veterinary Clinic in Gillett, Wis. My professional interests include small animal medicine and surgery, including orthopedic surgery.

Veterinary Medicine Outreach: I am a member of Christian Veterinary Mission and have been on several short-term veterinary mission trips to Nicaragua and Ghana, Africa. Also, I am the 2018 president of the Northeastern Wisconsin Veterinary Medicine Association (NEWVMA). In addition, we have successfully placed four surrendered pets with forever homes through our clinic. Family Life and Hobbies: I am married and have two children, ages 17 and 13. We have two English Springer Spaniels, Jack and Orla, that I enjoy hunting with, as well as a Cane Corso rescue dog named Caspian. I enjoy spending time with my family, turkey hunting and fishing on the Bay. I have also started training for triathlons. l

Current Events and Emerging Outbreaks Reported by WVMA’s One Health Committee
July, 2018: Wisconsin Department of Health Services (WDHS) and La Crosse County Health officials reported the first documented death from Rocky Mountain Spotted Fever (RMSF) in Wisconsin. RMSF is a tick-borne, bacterial disease that commonly causes a distinctive spotted rash in humans. Early symptoms also include fever, headache, nausea and stomach pain. RMSF is rare in Wisconsin and is most commonly found in central and southeastern U.S. regions. For more information, visit the WDHS RMSF webpage: https://www.dhs.wisconsin.gov/tickborne/rmsf.htm August, 2018: Wisconsin state and local health officials reported the first 2018 human case of West Nile Virus (WNV). The patient is a resident of Dane County. WNV is a mosquito-borne virus that typically causes fever, headache and rash, although more severe neurologic symptoms are possible. The majority of human WNV cases reported in Wisconsin occur during August and September.

They accept submissions from residents in Wisconsin and throughout the Midwest (including Iowa, Illinois, Michigan and Minnesota). The UW is also the designated Midwest Center of Excellence for vector-borne diseases and has developed a “Tick App” for smartphones. This free app allows people to report tick encounters, learn about tick bite prevention, and will help researchers better understand which activities carry the biggest risk of tick bites and tickborne disease.
