Isn’t it Great Being a Veterinarian?... Dr. John T. Been

In this month’s edition of my message I’d like to bring attention to the subject of workplace safety. We have, by the very nature of our profession, a very real exposure to a unique and sometimes dangerous workplace environment. We deal continuously with animals, equipment, biologic agents, travel requirements and long hectic hours that can place us at risk for both physical and psychological injury. We’re all familiar with war stories and battle scars from veterinarians we know of who have suffered injury or worse from workplace incidents. My purpose in this message is to re-state the importance of awareness and prevention as a vital component of a safe workplace environment. One of the joys of my practice life as a food animal veterinarian was my interaction with fourth year veterinary medical students who came to our clinic each year as part of the highly recognized ambulatory rotation from University of Wisconsin School of Veterinary Medicine. These students stayed on for two week visits and observed/assisted in our daily rounds. As the student supervisor at our clinic I had a basic orientation speech where I’d highlight my basic expectations, but #1 was always think safety – for yourselves, others around you, and for the patients we would be dealing with. Two other points of emphasis were that they needed to rewind my halter properly and, of course, they’d better laugh at my jokes. Nothing, however, is more important than preventing injury and nearly all workplace injuries are preventable. The National Institute for Occupational Safety and Health (NIOSH), a division of the Center for Disease Control and Prevention (CDC), has specific recommendations for employers. Employers have the responsibility to provide a safe workplace environment and effective safety and health programs. These will reduce workplace injuries, illnesses, and associated costs. Adaptation of a structured workplace safety program creates an environment of attention to the welfare of the clinic family. Key elements of an effective safety and health program include: – Management leadership – Hazard prevention and control – Worker participation – Education and training – Hazard identification – Program evaluation Another important aspect to consider is prevention through design.

Efforts should be made to ‘design out’ hazards and risk points in existing facilities and in new construction projects. Always consider safety in the design and construction of animal handling, restraint, and housing facilities and in processes such as anesthetic gas control systems and toxic or infectious waste management. Control measures to reduce risk after a safety evaluation in a veterinary facility will benefit from then developing a ‘hierarchy of controls’ to prevent injury in the workplace. – Elimination – remove the hazard. – Substitution – switch to a less risky hazard, ie, a safer chemical or method. – Engineering control – prevent exposure with effective barriers. – Administrative control – implement changes in practices or policies. – Personal protective equipment (PPE) – use gloves, eye or ear protection, respirators, etc. Worker training and setting a good example regarding safe workplace practices can’t be overemphasized. As veterinarians, we are looked up to as leaders by both our employees and our clients and are emulated in our actions so we want to be excellent examples of safe and ethical practice. With this in mind, the WVMA Executive Board has approved the formation of a task force which will address the topic of workplace safety. The WVMA office staff receives frequent inquiries from our membership regarding Occupational Safety and Health Administration (OSHA) rules and requirements for their facilities. Our hope is to form a working alliance with OSHA to develop materials that would be available to our members that are specific to veterinary practice regarding workplace hazards and safety. We believe that would be a valuable resource and a practical member service. Megan Senatori, Dewitt, Ross & Stevens s.c. Dear Legal Briefs, I am struggling with a situation and would really appreciate some guidance. I have a client who I am concerned may be misusing prescription medications for the client’s animal. I contacted local law enforcement and have been advised to file a report. However, I seem to recall you once saying that veterinary records are confidential. If so, how can I file a report without getting myself in trouble with the client about confidentiality obligations? I am really in a bind. Can you help? Sincerely, Concerned Veterinarian Trying To Do The Right Thing

Dear Concerned Veterinarian: Wow, being a veterinarian is not as easy as simply helping animals is it? I appreciate your question and your concerns. You are definitely on the right track with respect to your obligations when it comes to veterinary records, but, the confidentiality of veterinary records is not absolute, given recent changes in the law. Some background may be helpful.

Since the law now includes the above provision, it implicitly means that the veterinarian may (in fact, must) share confidential information in the situations described in VE § 7.06(26). The language in VE § 7.06(26) is meaningless if the veterinarian is bound by Wis. Stat. § 89.075, which does not contain the exceptions included in the regulation.

You are correct that Wisconsin law requires veterinarians to keep records A veterinarian who intends to share otherwise confidential information should always, however, be careful to ensure that the situation falls confidential, subject to some exceptions when the client consents to within one of the exceptions contained within VE § 7.06(26). For sharing information. Wis. Stat. § 89.075 states as follows: example, a veterinarian could not rely on VE § 7.06(26) to share 89.075 Access to health care records. That is always a risk, but, if the client ever made such a claim, the veterinarian could defend by arguing that he or she was complying with a directive from local law enforcement. It is hard to envision a civil claim being successful under those circumstances. It is likewise hard to envision the Veterinary Examining Board taking action against a veterinarian who has shared confidential information with law enforcement under those circumstances. n

New Members Elected to the WVMA Executive Board District 7 – Karl Solverson, DVM

Dr. Karl Solverson graduated from the University of Wisconsin School of Veterinary Medicine in 1999. He began practicing in Wittenburg, Wis., then moved to Westby where he currently owns Solverson Veterinary Services and primarily practices dairy medicine.

Dr. Kevin Landorf graduated from the University of Illinois College of Veterinary Medicine in 1989. He began practicing in Illinois at a mixed animal practice. In 1990, he moved to Eau Claire, Wis. and worked predominantly as a dairy veterinarian for 10 years. For the past 15 years, he has worked at Oakwood Hills Animal Hospital, a small animal exclusive practice in Eau Claire.

A member of the WVMA since 1999, he has participated in the Food Armor® program and the WVMA Annual Convention. “I really enjoy my work as a veterinarian and I want to work with other board members to identify problems and continue to promote how animal health and human health are linked,“ he says. Dr. Karl Solverson

There are two main issues facing the veterinary profession that concern Dr. Solverson. “First is the consumer focus on animal welfare and needing to keep ahead of those trends. There is a growing concern on the part of organizations and the public on the treatment of animals in regards to pets as well as livestock. We need to help our clients understand these concerns and help promote proper protocols that address animal welfare areas. We need to be proactive in addressing these concerns,” he says. “The other is student debt of veterinary students coming out of college compared to the starting salaries for these recent graduates,” says Dr. Solverson. “I feel we as a profession need to look for ways to keep the students interested in the veterinary profession by undertaking projects that focus on growing scholarship dollars to help defray the cost while in school or look at expanding programs for debt relief for veterinarians entering high-need fields or geographic areas.” Outside of veterinary practice, Dr. Solverson has been involved in his local community and says, “I am used to working with diverse groups of people to solve problems. I am the past vice president of our local Westby Syttende Mai board, past director Vernon County Cattlemans, past director Vernon County Humane Society, and I am a member of the Vernon County Fair Meat Animal Sale committee.” Dr. Solverson and his wife have three children and enjoys traveling when he has the chance, reading non-fiction books and attending at least one Badger football game a year.

A member of the WVMA since 1996, Dr. Landorf is currently serving as president of the Northwest Veterinary Medical Association.

“I am at a point in my career where I feel that I have the time necessary to get involved at the state level. I am interested in learning more about what the WVMA does and am excited to bring the information back to our local association. I am still proud and enthusiastic about our profession and want to be a part of maintaining the integrity of veterinary medicine,” he says. Dr. Landorf believes there are many important issues facing veterinary medicine but worries about the increasing debt that veterinary medical students are having to carry as they begin their careers. “I am afraid that it will limit their options in following their passion or becoming practice owners. What will happen to underserved areas that cannot support the income needed to service the large debts?” he says. Dr. Landorf grew up outside of Naperville, Ill., which gave him his love of animals large and small. Outside of veterinary medicine, he enjoys boating with family and friends, playing tennis, woodworking, and reading. n

Eliminate Yellow Pages Advertising: When was the last time you opened a Yellow Pages Directory? Even the promise of a presence on their website and app is empty. Most consumers avoid directory type websites because they often lack the information like hours and location. Tender Your Annual Contracts: Next time your IT, cleaning and maintenance contracts are up for renewal, tell your provider to submit a bid because you are shopping it around. Often, you can get a more competitive arrangement from existing providers.

Send Back Unused Product: Most veterinary hospitals have too much inventory. Call Bill Westwood from Veterinary Purchasing and he will tell you exactly how much of each product you need for your hospital. Bill will look at your purchasing history and tell you what you need to keep and what you can send back. This is a free service offered by Veterinary Purchasing and can move thousands of dollars from inventory on your shelf to money in your pocket.

Pot Luck Meetings: A good team building exercise and even better way to save money, pot luck lunches for staff meetings can save the practice thousands of dollars a year on staff meals. In advance of your next staff meeting, dole out the cooking responsibilities so each person brings part of the meal. You can introduce themes, have contests and make it a fun distraction.

Reduce Office Supplies: Figure out where your office supplies cost is now and then offer your staff in charge of purchasing a 10% of everything he or she can save in a year. If they save you a $1,000, you pay them $100. This will provide them with a strong incentive to come up with creative ways to save money. One hospital moved from Post-it Notes to scraps of paper and saved over a $1,000 in one year alone. n

I’d also like to tell you about our 50th Anniversary celebration. We’ve put together a collection of communication programs and special events to commemorate the occasion, and more importantly, shine some light on our profession. We’ve produced a special edition of the Journal of Veterinary Medical Education and commissioned former dean Dr. Don Smith from Cornell to write our history book. We’ve developed a major video as well as a snappy television PSA, and encouraged our deans to share them with you during state veterinary medical association meetings. We’ve invited our allied organizations to recognize our anniversary benchmark with a resolution or letter of commendation, and we’re working with the Veterinary Medicine Caucus in the U.S. Congress on other recognition programs. Finally, to address our commitment to the future, we’re planning an anniversary “Grand Initiative,” which will be announced during our March 2016 annual meeting.

Why is all this important to you? Because our anniversary celebration is magnifying the value and importance of what veterinarians do every day in communities across the nation. By raising awareness among consumers, policy-makers, and government officials we’re trying to build support for our entire profession. We sincerely hope what we’re doing on the national and international level is creating a better operating environment for you. The veterinary college serving your region is there for you in many ways: as a referral center, providing telephone consults, CE programs, pet memorial programs for your clients and other programs, it contributes to your success. In turn, many of you are involved with mentoring programs or work with lawmakers to support legislation and budget activity that helps your college. We are grateful for all you do. On this anniversary and always, we are reminded of the critical importance of this most essential relationship. Thanks for letting me share a little about what we are doing. n

New WVMA Student Member Activities Introduced Several new programs have been created to add benefits for student members. WVMA Student Member Representatives, Brandon Sharping and Holly Hovanec, along with the Student Advisory Committee helped to create and implement these activities throughout the 2015-2016 school year. New student activities include: - Student Wetlabs – New for the 2015 WVMA Convention were student wetlabs held on Saturday, October 11. The wetlabs included Small Animal UItrasound, Goat Medicine, Poultry Medicine and Embryo Transfer. This was such a success that it will be continued at the 2016 WVMA Annual Convention. - Student Networking Dinner – The first Student Networking Dinner was held in November at Lodi Veterinary Care in Lodi. Students were able to talk with clinic staff about specific cases, discuss hiring protocols, tour the clinic and ask any other questions that interested them. Another Student Networking Dinner is planned for spring. - Student Member Tour of American Foods Group – On January 10 student members were able to tour the American Foods Group

processing facility to learn the role veterinarians play in the meat industry. This is the first time student members have participated in a tour like this with the WVMA. After the tours, students visited Waupun Veterinary Services to learn what the clinic is doing to stay up-to-date on the latest technology related to food animal medicine. - Student Member Resume Book - For the second time, student members have submitted their resumes to the WVMA for distribution to member veterinarians and placement on the WVMA website. Along with this book, students are also given the option to advertise in the classified section of the WVMA Voice and online. This is a great way to connect veterinarians with their future colleagues.
