WVMA Welcomes New Executive Board Members District 1 – Katrina Geitner, DVM

Dr. Katrina Geitner graduated from Iowa State University College of Veterinary Medicine in 1991. Currently, she is doing relief work in the Milwaukee metropolitan area. Dr. Geitner has been a member of the WVMA since 2000 and has also been active in the Waukesha VMA where she served as president in 1997. Dr. Geitner believes there are many important issues facing veterinary medicine. “Continuing to grow a veterinary clinic and remain profitable. To reap a reward for hard work and give employees Katrina Geitner, DVM Chris Keim, DVM a decent wage for all of their contributions for animal health and business,” says Dr. Geitner. “I am a positive person who has experience as an associate veterinarian and an owner,” says Dr. Geitner. “I am very approachable and like to network to solve problems.” Outside of veterinary medicine, Dr. Geitner enjoys traveling, running, reading and shopping. Dr. Chris Keim graduated from the University of Illinois College of Veterinary Medicine in 1976. Currently, he is a partner at Sunshine Genetics, an international embryo technologies business and has been a member of the WVMA since 2004. Within the embryo transfer specialty, Dr. Keim served four years on the American Embryo Transfer Association Board of Directors. For two years he served as treasurer. Dr. Keim is eager to learn how he can help protect the veterinary medical profession from unlicensed practice by serving on the WVMA executive board. “I would like to learn more about the political and legal process, and the inter-relationship of the legislature, veterinary examining board, and the Department of Safety and Professional Services,” says Dr. Keim. “It seems that emphasis has been placed on protecting the consumer, while overlooking the welfare of the animals, and the welfare of the veterinarian who is educated, trained and licensed.” Currently, he believes the most important issue facing veterinary medicine is the rapid growth of the profession. “The number of new graduates has increased by about 1,000 compared to the year I graduated,” says Dr. Keim. “It is difficult for young veterinarians to obtain a high paying full time position to compensate. Consequently, many will not easily buy a home or purchase part of a veterinary practice.” Outside of veterinary medicine, Dr. Keim enjoys playing golf, fast cars and pretty cows. n

Personal Improvement and Development – a Life Long Process Dr. Jane Clark

I am writing this column on January 8th and we left Madison at 1:30 pm for Chicago. It is snowing, the wind is blowing 20 mph, the temperature is below zero and the wind chill makes it feel like 20-30 below. The weather continued like this for the entire four hours it took to arrive at our destination – the AVMA Leadership Conference. I attended this conference last year as WVMA president-elect not knowing what to expect or exactly how beneficial it would be. I left totally impressed. I learned how to speak to legislators, the importance of strategic planning, how to create meaningful agendas and run efficient meetings to name a few. I networked with leaders in the AVMA and met executives of other State VMAs, learning best practices that work for their respective associations. So when the opportunity knocked on my door to attend the Leadership Conference this year, I couldn’t wait. The WVMA executive board created a new strategic plan in May of 2014 and now we need to start implementing it. How do we do this? Our vision is to be “The premier state organization advancing veterinary medicine.” How will we know if we have attained this lofty vision? Well, this conference addresses these questions and more by focusing on benchmarks and performance measures, assessing organizational structure and conducting governance reviews. It will show us how to create an annual action plan, which details the delegation of assignments, committee tasks and deadlines needed to advance the strategic plan.

I wasn’t born with the knowledge of how to be the president of a statewide organization or how to lead such an organization into the future, so I appreciate the professional development opportunity to improve myself and grow as a leader. I have attended many continuing education meetings during my career, all aimed at improving my diagnostic, therapeutic and technical skills and knowledge so I can be the best veterinarian I can be, but that doesn’t necessarily help me as president. Attending this conference takes me out of my comfort zone a little bit and pushes me. Pushing the limits and trying new things out of your normal modus operandi is what helps you change, improve and grow as a person and professionally. So my challenge to you this month is to try something new that will grow you as a person or enhance your professional career in some way. It could be assessing the behaviors of your coworkers and deciding which behaviors you like and which ones annoy you. Then analyze yourself to see if you have these same behaviors and actively try to incorporate the ones you like and eliminate the ones you don’t. It could be learning how to perform cardiac ultrasound when all you have done before is abdominal. It could be as simple as making a list of all the things you would like to accomplish that day, and then cross them off one-by-one. There are so many ways one could improve that the possibilities are really endless and unique to each person. Personal improvement makes me feel better about myself and I hope it will make you happier as well. n

Wisconsin State Veterinarian Continues Ban on Spring Fair Pig Weigh-ins Wisconsin Department of Agriculture, Trade and Consumer Protection

To continue protecting Wisconsin’s pork industry from the spread of a deadly, communicable virus among pigs, Dr. Paul McGraw, state veterinarian at the Wisconsin Department of Agriculture, Trade and Consumer Protection (DATCP) is reissuing a ban on the spring pig weigh-ins that usually take place in preparation for Wisconsin’s numerous county fairs. McGraw also recommends only terminal swine shows be held given the concerns regarding the spread of Porcine Epidemic Diarrhea virus (PEDv), Porcine Reproductive Respiratory Syndrome (PRRS), and influenza.

Swine farmers are encouraged to use proper biosecurity methods, including washing trucks and trailers between loads, washing boots and clothing, and establishing a line of separation between clean and dirty areas. The National Pork Board has developed a wide variety of biosecurity information that is free and available at www.pork.org. For more information about animal diseases, visit datcp.wi.gov. “The precautions we took last year helped keep our infection rates down in Wisconsin, so I want to make sure that continues,” McGraw said. For example, Wisconsin has had 7 known positive cases of PEDv since the virus was first found in the U.S. pig population in 2013. Only two cases have been reported since the mandatory reporting requirement went into effect in June 2014. Meanwhile, industry analysts estimate more than eight million swine nationwide have died from PEDv alone in the same time period. Banning spring weigh-ins is a crucial step toward minimizing the effect of PEDv, PRRS and influenza on Wisconsin pork producers. In addition, McGraw still recommends that fairs hold terminal shows, where pigs go to slaughter after the show. It is completely up to the counties to develop plans locally with locker plants and slaughter facilities to make this work for them. “We have a very active fair season here in Wisconsin, so it’s important that we take all the disease transmission risks into consideration—this is not just about PEDv,” McGraw says. “The only safe way to control these diseases is to ensure that the pigs comingled at fairs and shows are sent directly to slaughter.”

Continuing Education Requirements: The Legal Duties of Veterinarians Megan Senatori, DeWitt, Ross & Stevens, s.c.

Like many other professionals, veterinarians who are licensed in Wisconsin have a legal obligation to take continuing education courses. Many veterinarians forget that their continuing education (“CE”) requirement is broader than merely acquiring a specified number of credits or courses, they must also maintain records sufficient to demonstrate CE compliance. This article provides an overview of Wisconsin’s legal requirements for CE for veterinarians, including recordkeeping requirements related to CE compliance. To begin, Wisconsin’s legal requirement relating to CE for veterinarians (and veterinary technicians, though not covered in this article) can be located in Chapter VE 7, VE 10, and Wis. Stat. § 453.062. Veterinarians licensed in Wisconsin must complete at least 30 hours of CE pertinent to veterinary medicine in each biennial renewal period. Such courses must include at least 25 hours of scientific topics pertinent to veterinary medicine. To be approved, the subject matter of a CE program or course must be pertinent to veterinary medicine or veterinary technology. Additionally, the program or course sponsor must agree to record registration and furnish a certificate of attendance to each participant. A CE hour consists of 50 minutes of contact time. The courses must be completed during the preceding 2-year licensure or certification period. There are only two exceptions to the CE requirement discussed above. First, the Veterinary Examining Board (the “Board”) may waive the CE requirements if it finds that exceptional circumstances, such as prolonged illness, disability or other similar circumstances, have prevented an applicant from meeting the requirements. Second, the CE requirements do not apply to an applicant who applies to renew a license or certification that expires on the first expiration date after initial issuance of the license or certification. In Wisconsin, veterinarians are not responsible for merely obtaining the requisite CE, they are also responsible for providing a certification of compliance and for maintaining records sufficient to demonstrate compliance. All 30 continuing education hours must be documented. A minimum of 25 hours of continuing education must be documented by an approved provider.

To obtain credit for completion of CE hours, a licensee or certificate holder must, at the time of each renewal, sign a statement certifying that he or she has completed, during the preceding 2-year licensure or certification period, the required CE programs. For auditing purposes, veterinarians must maintain records of CE hours for at least 5 years from the date the required certification statement is signed. The Board may audit for compliance by requiring a veterinarian to submit evidence of compliance to the Board for the biennium immediately preceding the biennium in which the audit is performed. Documentation of completion of CE hours must include one of the following: (a) A certificate of attendance from an approved course provider. (b) Complete references from journal articles read. (c) A grade report or transcript from an accredited college or university. (d) A copy of a published work authored or co-authored by the licensee or certificate holder. (e) A copy of a meeting syllabus, announcement, abstract or proceeding for a presentation. (f) A signed document from an internship or residency institution certifying enrollment in a program. A veterinarian who fails to complete the CE requirements by the renewal date is prohibited from practicing as a veterinarian until his or her license or certificate is renewed. Therefore, if any complaint is made against a veterinarian, the Board may require the veterinarian to submit proof of the CE programs or courses that he or she has completed during the preceding 2-year licensure or certification period. The Board takes CE requirements seriously. A veterinarian commits an act of unprofessional conduct if he or she falsely certifies to the Board that the veterinarian has completed the 30 hours of CE required or is exempt from having to complete the 30 hours of CE. As a result, it is important for veterinarians to review and understand CE requirements, including the certification and recordkeeping aspects of CE. As a best practice, we recommend that veterinarians review their compliance with CE requirements on at least an annual basis, to ensure that they are obtaining the requisite number of CE, and to confirm that the veterinarian is maintaining the required documentation. Due to the importance placed on CE requirement, including that a veterinarian’s certification as to CE be accurate, it is important to take time to carefully ensure compliance. n

Second WVMA Member Elected to Wisconsin State Assembly Wisconsin Veterinary Medical Association (WVMA) member Rep. Joel Kitchens was elected to represent Assembly District 1 in the Wisconsin State Assembly during the last election. There, he will join Rep. Dean Knudson, also a WVMA member, who was elected to his third term representing Assembly District 30. Rep. Joel Kitchens graduated from the Ohio State University College of Veterinary Medicine in 1983 and currently practices large animal medicine at Door County Veterinary Associates. Before serving in the Wisconsin State Assembly, Rep. Kitchens served 15 years on his local school board, of which he was president for 13 years. “I truly love Wisconsin and the area in which I live and believe that I can provide the representation our people deserve and help make this an even better place to live,” says Rep. Kitchens. “I want to give something back to the community that has been so good to me and my family.” Being a veterinarian has helped to greatly prepare Rep. Kitchens for his new position due to the analytical skills he developed from the educational process and diagnostic approach taught during school. “I deal with farmers working a second job trying to hold on to their small farm and I deal with multimillionaires who own horse farms or very large dairy farms,” says Rep. Kitchens. “I think that gives me an understanding of the perspectives of a wide section of our community and it has taught me to communicate effectively with all types of people.” The first step Rep. Kitchens plans to take in the Wisconsin State Assembly is to become acquainted with the procedures and building relationships within the Capital. During this, he will be keeping in close contact with the veterinary community in order to stay abreast of issues that could

directly affect the profession. “We are unique in that we deserve the same privileges and respect afforded to other healthcare professions yet many of the legal requirements and restrictions placed on them would create a financial hardship for veterinarians” says Rep. Kitchens. “I will help assure that my colleagues understand the differences as well as the similarities between us and those other professions.” Rep. Kitchens also has a strong passion for Wisconsin agriculture and hopes to be a resource when animal rights issues are presented. “As a large animal veterinarian and member of the Agriculture Committee I will fight to assure that Wisconsin continues to be a leader in agriculture,” says Rep. Kitchens. In everything Rep. Kitchens hopes to accomplish during his first term, his greatest hope is to represent the true characteristics of the veterinary medical profession and community. “Veterinarians as a group are hardworking, self-sufficient and family oriented,” says Rep. Kitchens. “More than anything, I hope to represent those core values that we all share.” n

Before Participating in CE, Research the Source In recent years there has been an increasing trend of veterinarians and veterinary technicians participating in CE not offered by approved providers. This issue was the most discussed topic during the last Veterinary Examing Board meeting. It is the responsibility of the veterinarian and veterinary technician to know if the CE is valid. For more information on which providers are approved, refer to VE 10.03(04) below. (4) APPROVED PROGRAM PROVIDERS. Subject to compliance with the requirements set forth in subs. (1) to (3), the board shall approve attendance at and completion of one or more continuing education programs approved by any one of the following approved program providers as fulfilling the continuing education hours required under this chapter: (a) A national, regional, state, or local veterinary medical or veterinary technician association. (b) A federal or state agency. (c) An accredited college or university. (d) An association listed in the AVMA or the National Association of Veterinary Technicians in America directory. (e) An AVMA accredited veterinary school or veterinary technician program. (f) A program approved by the American Association of Veterinary State Boards through its Registry of Approved Continuing Education approval program. (g) A foreign veterinary medical or veterinary technician association, an accredited college or university, or a governmental agency that is, as determined by the board, comparable to a program provider listed under pars. (a) to (f). To ensure your time and money is spent wisely, research the CE provider prior to registration. It is the responsibility of the veterinarian or veterinary technician to ensure they receive the highest quality CE possible and stay in compliance with Wisconsin licensing laws. n

What Veterinarians Should Tell Clients About Pain Control and Their Pets

by Michele Sharkey, DVM, MS, Office of New Animal Drug Evaluation; Margarita Brown, DVM MS, Office of Surveillance and Compliance; and Linda Wilmot, DVM, Office of New Animal Drug Evaluation – U.S. Food and Drug Administration Non-steroidal anti-inflammatory drugs (NSAID) are commonly prescribed and are effective pain control drugs for pets. Like most drugs, they do cause side effects, some serious. Veterinarians are in the best position to inform their clients about these side effects, so the clients can take better care of their pets. And, pet owners expect veterinarians to explain all potential risks of medications. Dogs are living longer and healthier lives thanks to advances in veterinary medicine and pharmaceuticals. With active lifestyles that extend into advanced ages, dogs are often diagnosed with osteoarthritis or undergo surgical procedures and are treated for postoperative pain. NSAIDs are among the most common analgesics prescribed in these cases. NSAIDs are used to control signs of arthritis, including inflammation, swelling, stiffness, and joint pain. Inflammation—the body’s response to irritation or injury—is characterized by redness, warmth, swelling, and pain. NSAIDs work by blocking the production of prostaglandins, the body chemicals that cause inflammation. Other NSAIDs are available in the United States for human uses, but have not been approved for use in dogs or cats. Sometimes there may not be an approved animal drug available for a specific indication or dosage form. However, the Animal Medicinal Drug Use Clarification Act of 1994 gives veterinarians the same kind of discretionary authority available to physicians, allowing veterinarians, under certain conditions, to prescribe drugs for “extralabel” uses, which are uses not listed on the label. As with any medication, veterinarians should discuss the benefits as well as the risks of the drugs with their clients when prescribing an NSAID. Every year millions of doses of medications are prescribed for dogs with good reason—but many adverse reactions occur. Most adverse reactions are mild, but some result in permanent impairment or even death. If the client can recognize a possible reaction and stop the medication while seeking veterinary attention for the dog, quick action may make the difference between a good outcome and a disaster. The most common side effects from NSAIDs include vomiting, loss of appetite, depression/lethargy, and diarrhea. Some side effects can be serious, especially if the drug is not used according to labeled directions, resulting in the need for medical care. Serious adverse reactions include gastric ulcers and perforations, kidney and liver problems. Death may result in some instances. All NSAIDs approved for oral use in dogs and cats come with a Client Information Sheet (also known as the Information for Dog (Cat) Owner Sheet) that describes the drug’s side effects. Dog and cat owners should ask veterinarians for the Client Information Sheet when an NSAID is prescribed. These Client Information Sheets provide the pet owner with important information in a user-friendly manner regarding what can be expected from use of the drug, potential side effects, and the need to seek veterinary attention if problems occur. By accompanying each NSAID prescription with an Information for Dog Owner Sheet, a handy reference of valuable safety information and drug company contact information is readily available to the owner. Not all side effects can be predicted

All approved medications indicated for pets are subject to extensive evaluation by a drug company using stringentstandards set by the CVM before they are marketed. Every effort is made to ensure safe and effective treatments. However, every drug has the potential for side effects. Pre-testing by the animal drug manufacturer and review of the data from those tests by the government ensure that the animal drug is safe and effective. Because of the relatively low frequency of some adverse events, some adverse effects are recognized only after the marketing of the product in a large population of animals. NSAID therapy can also unmask hidden disease, previously undiagnosed due to the absence of apparent clinical signs. Dogs with underlying kidney disease, for example, may experience worsening of that disease while on NSAIDs. Dogs at greatest risk for kidney problems are those that are dehydrated, on concomitant diuretic therapy, or have kidney, heart, and/or liver dysfunction. Unexpected reactions to a drug are reported to the drug manufacturer, and every reaction reported to a pharmaceutical manufacturer must by law be reported to the FDA. Advice given to owners

We recommend that pet owners work with their veterinarians to make medication decisions including the use of over-the-counter drugs, vitamins, herbal supplements, flea control products, and other medications. Giving certain medications and other over-the-counter products at the same time could be detrimental to a dog’s health. Many reactions due to NSAIDs may be lessened if owners are aware of potential side effects, and with appropriate use many can be minimized or avoided. First and foremost, if an owner suspects a reaction to an NSAID, the owner should stop administering the drug immediately and should contact a veterinarian. Some reactions are mild and go away after stopping the drug. The veterinarian is in the best position to advise the dog owner on using an NSAID. Before administering an NSAID to a dog, the veterinarian often recommends blood tests. The knowledge gained from these tests could be critical in deciding if the drug is safe to use in a dog. If a dog is prescribed an NSAID for the control of pain associated with osteoarthritis, regular veterinarian check-ups and blood tests are recommended to evaluate the continued use of the drug.

Pain control in response to the use of an NSAID varies between dogs (just as it does in people). Because the response to pain medication is individualized, no one NSAID is considered more effective than another, and because every NSAID can cause adverse reactions, including stomach/ intestinal ulcers and death, none is considered safer than others. But selecting the best NSAID is important. With advances in the recognition and definition of animal pain and the many NSAID choices available, much benefit can be gained from the appropriate and careful use of these drugs. Sometimes, the process of finding the best NSAID for a specific patient can mean changing the prescription. Only one brand of NSAID should be administered to a dog at any given time. If at some time the owner and the veterinarian decide to try a different NSAID, a wash-out period is recommended. A wash-out period is a period of time recommended by the veterinarian during which the dog does not receive any NSAID. This allows one NSAID to be cleared from the body before starting another NSAID. Then the dog can be switched to another NSAID. NSAIDs should not be combined with the use of a corticosteroid, either. The pain associated with osteoarthritis may wax and wane. The lowest effective dose should be used for the shortest duration depending on the dog’s response. If the dog seems to improve to the point of not needing the drug, the owner should discuss continued use of the NSAID with a veterinarian. The key to making any transition or change work well is good veterinarianclient communications. An informed dog owner is the best defense against serious side effects from NSAIDs. The veterinarian is the most qualified source for information regarding NSAID use and a dog’s care. Owners should not hesitate to ask questions and inquire about possible side effects or signs to watch for when treating a dog. A Client Information Sheet, which a veterinarian should give the pet owner whenever an NSAID is prescribed, serves as a reminder of this information for use at home.

What starts out as a minor problem can readily progress to an emergency. An owner should be encouraged to call his or her veterinarian with any concerns about the NSAID the dog is receiving.
