New Graduates for Hire! Dr. Jane Clark

I just attended my younger son’s college graduation ceremony this past weekend and we discussed how to get his foot in the door as a new graduate. He was lamenting that all of the jobs in his field required a minimum of one year of experience. How was he to get this experience if no one would hire him without it? So this month’s column will discuss the benefits of hiring a new graduate for your business.

New graduates have board certified contacts they know and can access for help when you are uncertain of what to do. Most new graduates are young, flexible and adaptable to quickly changing work schedules, like the emergency that comes in at closing time, or on the weekend. They are generally healthy so time off for illness is minimal. They may not have children yet, so they don’t need to leave early to see that soccer game or pick them up from daycare.

New graduates bring high energy and endless enthusiasm to the work environment. Things that you may have seen hundreds of times are all new and interesting to new graduates. They bounce around your practice full of wonder and awe at all the wonderful and strange things they New graduates have experienced teamwork in college for years so they are seeing. It reminds us older veterinarians of why we went into this profession in the first place. This exuberance is not lost on our clients and are well schooled in the need for a functional motivated team, where all members are valued regardless of the specific job they perform. Hiring Minors to Work at the Clinic: What is Required and What is Prohibited? Gregory E. Scallon, DeWitt Ross & Stevens, s.c.

As we head into summer, you may be planning to hire some high school students to help out at the clinic. Or, perhaps you have students who also work part-time during the school year. If your practice hires minors (defined as someone under age 18) any time of the year, be aware that specific federal and Wisconsin regulations limit the type of work that minors can perform and the hours which they can work. It seems timely to review these regulations. Please note, there are various restrictions on the types of tasks minors of different ages can perform. This discussion addresses only those which seem most likely applicable to a veterinary medicine practice. You should seek additional guidance for your unique circumstances. Almost everyone loves animals. You may have a situation where a minor simply likes to “hang-out” at the clinic with the animals, and during this time the minor helps to care for the animals. Neither you nor the minor may consider this a “job”. However, as a general rule, a minor cannot be considered a “volunteer” for a “for profit” business. If the minor is performing productive labor, the minor must be considered an “employee” and must be compensated for the services provided, and the employer must comply with all of the applicable child labor laws. The situation is different for a “non-profit”, such as a humane shelter, where the minor may provide voluntary, uncompensated services. As an employer, you are required to have a work permit on file for a minor before allowing the minor to work in the practice. Among the requirements to obtain a work permit is a letter from the employer, on the employer’s letterhead, stating the intent to hire the minor and describing the job duties and hours of work. If the duties change, a new work permit must be obtained. In most instances, minors must be at least 14 years of age to be employed. No minor may work during regularly scheduled school hours. Fourteen and fifteen year olds may work no more than 3 hours on a school day, 18 hours in a school week, 8 hours on a non-school day, or 40 hours in a non-school week. No person under 16 years of age may work more than 6 days in any workweek.

Persons 17 years of age and younger may not work longer than 6 consecutive hours without a duty free 30 minute lunch break. Overtime rules are the same for minors as for adults when the minors work more than 40 hours in a week. Work may not begin before 7:00 a.m., nor end after 7:00 p.m., except from June 1 through Labor Day, when evening hours are extended to 9:00 p.m. The operation of any machinery, including a power mower or trimmer, is prohibited under age 16. Youths 16 and 17 may perform any non-hazardous jobs, for unlimited hours. There are restrictions under the child labor laws on the ability of minors to drive a motor vehicle in the course of the employment. Even if otherwise allowed, you are also likely to encounter difficulty in obtaining insurance for a minor to drive a motor vehicle. Restrictions do not apply to minors who are high school graduates or student learners employed under certain conditions. Excluded from the restrictions applicable to minors employed by the practice are minors who are at the clinic in the role of an unpaid intern or trainee. The question of whether a person is considered an employee or an intern was discussed in a Legal Briefs article in the September, 2012, issue of the WVMA Voice. The distinctions discussed in that earlier Legal Briefs would also be applicable to determining whether a minor is an employee or an intern. A minor engaged in “job shadowing” is generally also not considered to be an employee. Offering a minor a part-time job can be a win-win situation for both the clinic and for the youth. Knowing and following the laws governing employment of a minor will assure you don’t inadvertently subject the clinic to penalties. Exposing the next generation to the practice of veterinary medicine may instill the drive that will lead to a career in the field of veterinary medicine. n

This presentation provides the practitioner with a widely accepted approach to the management of the most important cardiovascular disease in veterinary medicine. Controversies in the management of mitral regurgitation will be addressed; however the practitioner will be left with an agreed-upon approach to managing this important syndrome. General Audience Treating Congestion – the Use and Misuse of Diuretics and ACE-Inhibitors The possible ill effects of diuretics will be discussed against the backdrop of their utility in virtually all cases of heart failure. Alternative drugs and methods of administration will also be covered. This will be contrasted with the use of drugs which blunt the RAAS (ACE-Inhibitors and Mineralacorticoid Receptor Blockers, such as spironolactone). The benefits of each and how to use together will be emphasized. Cases will be used to supplement the lecture, as time permits. When to Use and NOT to Use Inotropes (Pimobendan, Dobutamine and Digoxin) The use of this important drug class is controversial. The speaker will sort through the controversies and give firm recommendations for their use (or lack of) in asymptomatic and symptomatic (including emergent) canine mitral valvular disease and dilated cardiomyopathy, and hypertrophic cardiomyopathy in cats. We will also discuss its use in transitioning to employment of beta-blockers in dogs with heart failure, its use to support renal function in heart failure, and when (if) it should be used with digoxin. Pathophysiology and Diagnosis of Feline Cardiac Disease The clinically relevant pathophysiology and new methods of diagnosis of feline hypertrophic cardiomyopathy, the most important heart disease of cats, are detailed. Echocardiographic pitfalls, auscultatory findings and biomarkers and genetic testing are covered. How these tests and findings are used to differentiate other common heart disease in cats provides a practical overview to this presentation. Treatment and Prognosis of Feline Cardiac Disease Nuances of managing hypertrophic cardiomyopathy, recent breakthroughs, and controversies are provided. In addition, new information regarding the prevention of systemic emboli is presented. The expected outcome and prognosis based on signs at presentation provide the practitioner with useful information with which to communicate with owners of affected cats. Feline Hypertension: Risks, Diagnosis, Management and Prognosis The incidence, target organ risk, diagnosis, management and prognosis of feline hypertension will be discussed in an evidence-based manner. Attendees will leave with an overview of this syndrome and a logical, straightforward way to make decisions regarding the diagnosis

and management, while avoiding attendant pitfalls. Small Animal Track 2

Alice Villalobos, DVM, FNAP Dr. Alice Villalobos is 2005-06 President of the American Association of Human Animal Bond Veterinarians (AAHABV). She is a wellknown pioneer in the field of cancer care for companion animals and a founding member of the Veterinary Cancer Society. A 1972 graduate of UC Davis, she completed Dr. Gordon Theilen’s first mock residency program in oncology and has served the profession by consulting, writing and lecturing in the rapidly growing field of veterinary oncology and end of life care. Quality of Life for Cancer Patients at the End of Life There is a clear need for our profession to organize and improve quality of life evaluations. The public is demanding more home care for their aging, ailing and terminally ill pets. The pet owning public is requesting that our profession do more to help provide end of life care services in the form of pet hospice or Pawspice. Since most geriatric animals have concurrent conditions, when the pet is burdened with cancer and its related treatment issues, quality of life is threatened. How can our profession respond to this need? The HHHHHMM Quality of Life Scale provides a guideline for pet owners and the veterinary team to maintain the patient’s quality of life to sustain an end of life relationship that nurtures the human animal bond. Are Veterinarians Kinder to their Cancer Patients at the End of Life than Physicians? It might be that until recently, veterinary education focused on only three stages of life: the puppy and kitten stage, the adult stage and the senior stage. This author proposes that there is a true “Fourth Stage of Life” that may occur at any age yet it might last for quite a while but has been bypassed,. That fourth stage of life, which the love and tenacity within the human-animal bond will no longer bypass, is “The End of Life Stage.” Veterinarians are regarded by society as the most trusted and truthful professionals next to the nursing profession, which started the world wide hospice movement to spare end of life patients further futile medicine which often caused adverse events that lead to complications and unnecessary suffering. What can society learn from veterinary medicine? We are qualified to help share our experience in helping animals transition peacefully and painlessly when they are dying. >>> Continued on Page 9

Decision Making for Cancer Patients at the End of Life When pets are at the end of life (EoL), there is a need to reach out and enhance relational capacities and remove communication barriers between doctors and clients to improve decision making. Clients may experience insensitivity from reception staff on the telephone, the brisk attitude of a receptionist or an officious office manager. If they are rushed, it is impossible to think of everything they need to say. Clients often leave with unsaid details that might impact patient care. Clients often fail to receive compassion and valuable practical wisdom and personalized advice that they so desperately need to make informed decisions for their EoL pets. They often feel alone with little guidance for their problem so they ask Dr. Google, Twitter and chat rooms to help them make decisions instead of relying on their veterinarian. Preventing Over Treatment at Veterinary Cancer Referral Centers and E Clinics Pet owners feel committed to care for their pets with loyalty at the end of life. The diagnosis of cancer, its treatment and its recurrences and relapses will become a more common clinical dilemma. We will interact with specialists and advanced technology more often in every day practice. What can generalists do to prevent the over-treatment of their patients at emergency clinics and veterinary cancer referral center? The family veterinarian is obligated to refer cancer patients for overnight care at the local emergency facility and for specialty oncology consultations. Clients will find their own way to a specialist or expert even if they are not given a direct referral by their family veterinarian. So what can be done to prevent over-treatment at a VCRC? How can the family practice veterinarian communicate concern without appearing selfish or over protective or backward? There is no perfect path and no perfect choice. This paper offers suggestions for decision making and weighing probabilities to prevent overtreatment when the odds are contradictory. End of Life Support with Metronomic Chemotherapy & Immunonutrition There is more interest in metronomic chemotherapy in the past several years. What is it? Metronomic chemotherapy is the use of low dose, higher frequency anticancer drugs, which may be effective in the inhibition of common malignant mechanisms and pathways such as angiogenesis, mitosis and metastasis. Metronomic chemotherapy is a viable palliative option for end of life cancer patients who have resistant, inoperable or advanced cancer. This presentation will introduce useable metronomic chemotherapy protocols that offer a kinder and gentler approach to treating cancer patients in “Pawspice” and end of life care management programs. Aging is associated with changes inside cells such as a reduction in the numbers and function of mitochondria and the accumulation of

waste products inside the cells and the loss of immunosurveillance. All these aging changes lead to an increase in the development of cancer in older people and animals. This presentation discusses specific agents for immunonutrition, which in combination have anti cancer, anti aging and longevity potential that often benefit senior and geriatric patients in end of life decline. Creating an End of Life Care “Pawspice” Practice End of Life can be defined as living in the last 25 percent of life span. Pet lovers are demanding that veterinarians step up and provide more comprehensive end of life services for their end of life pets as long as the pet is able to maintain a good quality of life (QoL). Many veterinarians don’t know about or avoid the challenges that come with end of life care. Why is that? Many veterinarians say that they are obligated to the prevention and relief of animal suffering and they feel that end of life care drags out the inevitable. They may also be uncomfortable addressing difficult discussions about of life and death. This presentation helps set up the philosophy and the basic considerations that are necessary for creating your own end of life care hospice or Pawspice practice in your clinic or house call practice. Sandra Godden, DVM, BSc, DVSc Dr. Sandra Godden is a 1993 graduate of the University of Guelph. After two years in mixed practice, she completed a DVSc degree in dairy production medicine. From 1998 to present she has worked at the College of Veterinary Medicine, University of Minnesota, student teaching, completing applied research and outreach in dairy production medicine. Update on Colostrum Management We will spend time on prevention of dystocia and expose some myths of prevention. The Utrecht technique of handling bovine dystocias will be discussed with videos to demonstrate the technique. Pasteurized Milk Feeding Systems – Capturing the Benefits and Avoiding the Pitfalls There can be significant benefits to adopting a pasteurized milk feeding system including improved calf health, growth and economics. However, more intensive management is required in order to avoid common mistakes. This presentation will discuss these benefits as well as discuss how a veterinarian can get involved in advising producers on how to set up, manage, and monitor these systems to capture the maximum benefit.

Pros, Cons and Best Management Practices for Group Housing and Computer Feeding of Preweaned Dairy Calves Some dairy farmers are asking their veterinarians whether they should consider adopting a computerized feeding system and/or are looking for advice on how best to manage a group housing system already in place. These systems may offer potential advantages including ease of delivering milk to the calves and reallocation of labor within the calf program. Dr. McGuirk became board certified in the American College of Veterinary Internal Medicine in 1982. With degrees from the College of Veterinary Medicine at Cornell University and the Ontario Veterinary College, Dr. Ollivett has taught courses in ruminant health management and animal health and disease. Arthur Mueller, DVM Dr. Arthur Mueller is known across the state as the hog veterinarian of Wisconsin. He has served as a great resource to a number of pork producers in providing consultations on health related questions and diagnosis’. In addition, he has served as the manager of the Heritage Swine group, providing management and veterinary services to the 7,500 sow wean-to-finish system. Practical Swine Medicine Parts 1- 3 These sessions will review of principal diseases and procedures used in swine medicine. The emphasis will be on diagnostics, disease control, treatments, reproduction and commonly seen swine medical problems. Jammie Verstoppen, DVM Dr. Jammie Verstoppen completed a Bachelor of Science in Animal Science from UW-Platteville in 2004, and Doctor of Veterinary Medicine from the University of Wisconsin-Madison in 2008. After practicing as an associate veterinarian in mixed animal practice for six years, he opened Seymour Veterinary Hospital, LLC in August 2014. Common Poultry Diseases and Diagnostic Considerations This session will focus on identifying clinical signs, selecting appropriate diagnostic tests, and understanding the pathology of many poultry diseases that may be presented to a general practitioner willing to examine poultry. There will be four major categories; bacterial, viral, parasitic, and deficiency diseases that will be presented. Discussion will include information on disease identification and preventive management recommendations. Diagnostic options including potential laboratories for submission of samples will also be presented Common Poultry Therapeutics and Surgical Procedures for the General Practitioner This session will focus on developing a therapeutic plan for some of the most common poultry diseases presented to a general practitioner. Information regarding on-label and extra-label use of specific drugs as well as some herbal therapies will be presented. Determining appropriate meat and egg withhold times through FARAD will also be presented. Specific surgical procedures both for obtaining diagnostic samples as well as surgical management of common poultry ailments will be included in the session.


Clinical Poultry Case Presentations This session will present specific clinical cases managed by the presenter during the past few years of general mixed animal practice. Each case will be presented similar to the routine clinical presentation showing exam findings, diagnostic test results, and therapeutic or surgical considerations. Case outcomes will be provided and more time will be allotted for discussion of each case prior to starting the next case. Specific texts and other resources will be included in this presentation to help general practitioners become more familiar and comfortable managing poultry patients. Small Animal Lunch Bunch

Christopher Snyder, DVM, DAVDC Dr. Christopher Snyder is a Diplomat of the American Veterinary Dental College and Clinical Assistant Professor and Section Head of the Dentistry and Oral Surgery Service at the University of Wisconsin-Madison School of Veterinary Medicine. After graduating veterinary school at The Ohio State University and completing an internship in western New York, he joined the residency training program in veterinary dentistry at the University of WisconsinMadison. Dr. Snyder has authored journal articles, textbook chapters and has lectured nationally and internationally. Dr. Snyder’s academic interests include: maxillofacial trauma and reconstruction, oral surgery, and improving techniques for regional anesthesia. How to Deal with Challenging Dental Complications This lunch bunch session will discuss several common challenges with dental extractions in dogs and cats. Sheryl Shaw, DVM, MPH Dr. Sheryl Shaw is a 1989 graduate of the College of Veterinary Medicine at Washington State University. She spent 11.5 years in private practice in Wisconsin and then Montana. Dr. Shaw has been with the USDA since 2000, spending time in Food Safety (FSIS) and then APHIS Veterinary Services as an export veterinary medical officer and area epidemiology officer in Minnesota. Dr. Shaw completed a master’s of public health at the University of Minnesota in 2008 and the USDA Graduate School Executive Leadership Program in 2011. Dr. Shaw began her duties as the area veterinarian in charge in Wisconsin in November, 2012 and currently serves as Director of Service Center 3 within National Import Export Services in VS. Paul McGraw, DVM Dr. Paul McGraw completed his undergraduate studies at University of Wisconsin-River Falls and obtained his DVM degree at the UW-Madison School of Veterinary Medicine, Dr. McGraw worked in private veterinary practice for sixteen years before coming to DATCP as Assistant State Veterinarian in 2004. In April 2013, Dr. McGraw was elected as the State Veterinarian for Wisconsin. Wisconsin Veterinary Medical Association Announces First Food Armor™ Accredited Veterinarian and Certified Farm The Wisconsin Veterinary Medical Association (WVMA) has announced Dr. Glen Johnson, Reedsburg, as the first veterinarian to achieve Food Armor™ Accredited Veterinarian status. To achieve this status, Dr. Johnson went through over 15 hours of continuing education on food safety and proper medication use through the Food Armor™ program. Once complete, he needed to pass a quiz evaluation with a 90 percent or greater pass rate. “To maintain his accreditation status, Dr. Johnson will need to renew his Food Armor™ Accreditation every two years which includes a continuing education requirement focused on program updates and updates on food safety and proper medication use,” says Dr. Katie Mrdutt, Food Armor™ outreach specialist. Dr. Johnson worked with Cattail Dairy, Wonewoc, to help achieve his accreditation. Cattail Dairy is the first farm to become Food Armor™ Certified. “For a farm to earn Food Armor™ certification, they need to work with a Food Armor™ Accredited Veterinarian to implement and maintain a Hazards Analysis and Critical Control Points (HACCP) plan for proper medication use on their farm which meets all of the requirements of the Food Armor™ program,” says Dr. Mrdutt. To demonstrate the implementation of the program, the farm and their Accredited Veterinarian complete a Food Armor™ certification workbook that outlines their plan for monitoring medication use. This plan includes treatment protocols and standard operating procedures that describe how they will use medications on the farm. “In addition, there must be a recording plan for any treatments administered, along with a plan for veterinary oversight,” says Dr. Mrdutt.

To maintain Food Armor™ Certified status, farms needs to renew their certification annually, which will include a full review of the HACCP plan for proper medication use with their Food Armor™ Accredited Veterinarian. At each annual recertification, the Food Armor™ workbook is submitted to the WVMA for review and approval. A third party verification process will be used to annually ensure the integrity of the program, states Dr. Mrdutt. Food Armor™ is the first of program of its kind, requiring veterinarians and producer teams to work seamlessly to create a HACCP plan that is customized to the farm’s management and employee’s skills and needs. This program identifies potential risks (hazards) and provides the know how and action plan to implement safeguards and management practices (critical control points), creating an achievable HACCP plan. n Julia Simons
