I.

Executive Summary

This annual report outlined the performances and progresses made by the New Citizen Health Care Project between January and December 2012. Glaxo Smith Kline (GSK)’s “New Citizen Health Care Project” was developed based on “Migrant Population’s Reproductive Health Improvement Project” in 2009, and it has now become China’s first comprehensive community health promotion project that provides health education and services to the migrant population and their families in urban areas. Between 2009 and 2012, the “NCLC” of Xintu Centre for Community Health Promotion, Shanghai rooted in operating and developing communities, discovered community charity leaders, established community mutual aid network, and accomplished the project’s initial strategy of “Community Development Oriented Health Promotion”. The centre’s operation facilitated the migrant population’s positive behaviour change, enhanced the target communities’ reproductive health levels, and improved the Social Inclusions of the target communities. 2012 was the 4th year of the project. With the continuity of the Corporate-Charity cooperation model and the influence of “NCLC” brand, as well as the establishment of New citizen Beijing centre, we have further enhanced the concept of “New Citizen Health Care Project” and focused the work of 2012 on the general health concept theme of “Health, Participation and Learning For Social Inclusion”. “Social Inclusion” is a process of mutual cooperation and adaptation between individuals, groups or cultures to target the building of a positive harmonious society. The Social Inclusion of the migrant population includes economic, social, and psychological/cultural inclusion. Out of those aspects, Economic (employment) is the basis of living in a society, whereas inclusions at social and metal cultural levels are the factors which determine the span and depth of migrant population’s level of Social Inclusion. The “New Citizen” brand adopts the Reproductive Health and Family Support as the basis to commit to the improvement of migrant population’s local employment and habitation, while obtaining more information, skills and opportunities to participate in more aspects of social life. In 2012, we have further strengthened the cooperative relationships with the government, international organizations and various social partners. The brand power and social influence of “NCLC” has been further enhanced which attracted more attention from social media, corporations and volunteers to make more contributions with us for the “Health, Participation and Learning For Social Inclusion”. 2012 is also a critical year when we encountered many new challenges. The migrant communities were facing some uncertainties as a result of the policies, the previous Minhang Centre was removed, and the Huangpu communication station did not further execute our project due to cooperative concept issues. We have been, however, striving to advancing the capabilities of the new Minhang Centre, Hongkou Centre and Beijing Centre, developing the autonomous leader’s leaderships and Health Ambassadors’ knowledge reserves, and improving the project’s execution levels. With the unremitting efforts from all participants, “NCLC”—New Minhang and Beijing Centre were all formally launched.

II. Health “Improves Social Inclusion with Health” is the core objective of GSK’s New Citizen Health Care project. The migrant population’s access to medical resources has been limited by issues such as registered residence and economic power in comparison to the urban population. NCLC applies the community development concept which believes that a migrant community has the ability to resolve its own problems through mutual aid. It worked by encouraging communities to recruit migrant population as Health Ambassadors and adopted series of Health Intervention strategies and methods to emphasis on the core sub-projects below as well as improving the migrant population’s abilities on managing their own health.

1. Good Luck Mums 1) Project Introduction “Good Luck Mums” is a project that serves the women, children and their families of the migrant population to provide health care and medical consultation for women in pregnant and perinatal period and their babies. Adopt general intervention measures, including recruiting migrant women as “Health Ambassadors”, to develop the necessary tools for mutual-aid Mother-Baby Care services and to improve the migrant women’s abilities on self and mutual aid. 2) Project Concept We believe that, the migrant population is capable of improving their personal health care and management capabilities as well as their baby nurturance and care skills during the pregnancy and perinatal period. Utilize Health Ambassador trainings to improve Health Ambassador’s professional skills and techniques; Utilize Health Ambassador Field Workshops to help more mothers and children from migrant families. Utilize expert forums to provide the migrant population with opportunities to communicate with experts face to face for obtaining scientific knowledge on personal and child care. Utilize Health Ambassador household visits to understand migrant pregnant mothers’ practical needs and to offer the necessary support. 3) Accomplishment •

In 2012, the Good Luck Mums project organized trainings and field workshops 82 times throughout the year;

A total of 3,189 people received face to face health education promotions and health improvement services;

Over 200 households received visits;

Organized children’s charity health checkup once, which was attended by 42 5-year-olds from migrant communities;

The health checkup discovered abnormal health issues on 14 of the participants, out of which 2 had tonsil hypertrophy, 2 had nasitis, 1 had conjunctivitis, 1 had heterotropia, 1 had caries, 1 had pectus carinatum, 4 had oversized phimosis, 1 had cryptorchidism, and 1 had growth retardation.

4) Work Method •

Health Ambassador Capability Building Training:

On June 13th, NCLC—Minhang invited Dr. Xuejuan WU of Pujiang Community Medical Centre to provide training and consultation on newborn and postpartum cares. ✓

During the training, Dr. WU explained the knowledge of newborn’s umbilical cord care, skin care, oral care and how to diagnose abnormal conditions of a baby from his excrements;

Puerpera care focused on 5 aspects, which included nutrition care, better uterus restoration, breast care (breast feeding), wound care and contraception;

At the end of the training, Dr. WU answered questions and offered advice relating to the training contents.

Health Ambassador Field Workshop

In the morning of August 13th, NCLC organized a Prevention and Control of Child Hand-Food-Mouth Disease promotional activity as a sub-project of Good Luck Mums project. Financial Times reporter Patti interviewed the event and visited Guijing Village community with the New Citizen Centre’s Health Ambassadors to observe the migrant population’s daily lives in detail. ✓

During the event, Curator Juxin YUAN of the Life Centre patiently explained the symptoms, route of transmission, causes and preventive measures of the Hand-Foot-Mouth disease to the parents;

The Health Ambassadors printed out health education materials for all parents in the event to remind them the significance of preventive measures in protecting their babies’ health;

During the workshop, Xintu project staff used IPAD to play the animated hand wash exercise for kids while playing animation music to teach them the steps for washing hands properly. The children enjoyed it very much;

Reminded the children that taking care of their hand hygiene is a very convenient and effective way to prevent the diseases.

In the morning of 27th February 2012, Professor SHEN of International Maternity &Child Health Hospital visited NCLC—Sanlin to teach maternity health and nutrition knowledge to the mother-to-bes from the migrant population of the community, and the lecture was warmly welcomed by the audience. Through the lecture, the audience realized that: ✓

During the pregnancy, it is not appropriate to eat too much and too fast, neither is it good to eat too many fruits;

It is recommended to eat much fish and roughage and to eat animal liver least once a week; 6


It is also advised to eat frequently with a small amount at each meal while taking proper amount of exercises and drinking 2 glasses of milk powder for pregnant women every day;

Seaweed soup with dry shrimps is a good source for calcium supplementation;

Prenatal training can be started from 5 months before due date;

Constipation during pregnancy can be eased by eating sweet potatoes.

The mother-to-bes in the community all thanked Prof. SHEN for recommending safe and nutritious diet plans which are also economic at the same time. The food list is very suitable for the needs of these mother-to-bes from the migrant population.

Expert Forum on Maternity Health and Nutrition Mothers raised questions of concern to the experts

At 1pm July 8th, 2012, doctors from Shanghai Children’s Medical Centre’s Pharmacy Department visited Sanlin Life Centre in the heat to provide medication knowledge to the community members, such as the characteristics of children’s medication, how to choose the suitable medicine, details of children’s medication that need to be taken care of, the differences between dietary supplement and medicine. They also brought knowledge on household remedies and their storage. The correct ways to feed infants with medicines were specially welcomed by young mothers. And the mothers were also taught how to store the medicines to prevent them from deteriorating.

Between 20th–25th April, the Health Ambassadors of Minhang District entered the families of community members to visit those mothers who just recently gave birth to share the health knowledge they learnt from the NCLC with those migrant mothers in the community.

5) Case Study The 35 year old Hua ZHANG (alias, same as below) is from Huaibei of Anhui province and has been living in Shanghai for 6 years. She now lives in Guijing Village of Sanlin Town, Pudong, and is running a small grocery shop for living while her husband works as a builder. The couple has a 13 year old daughter. In August, 4-month-pregnant Hua Zhang was introduced by her fellow villager to a mother-baby health forum— Maternity Health Care organized by NCLC. The Health Consultant realized that Hua has already undergone 4 abortions after the first child and this is her 6th pregnancy. Furthermore, she has never taken any prenatal checkup since becoming pregnant. Hua told the consultant that she did not take any prenatal checkup during the first pregnancy either, and she even did not go to the hospital but delivered the baby in her own home with help from a midwife. Therefore she thought it was not necessary to do so this time. Based on her situation, the Health Consultant advised Hua to take a prenatal check in a good quality hospital. Hua took the consultant’s advice carefully by visiting Siyang Hospital of Pudong regularly for checkups. She eventually gave birth to a boy at the end of December 2012 in Siyang Hospital. During the delivery, Hua had adherent placenta which was, fortunately, treated successfully by professional doctors; and hence both the mother and her boy were fine.

2. Childhood Nutrition 1) Project Introduction In 2006, evaluation index research data from Chinese Children Development Outline (2001-2010) and Chinese Women Development Outline (2001-2010) (a.k.a. “the 2 Outlines” hereafter) conducted by the Ministry of Health across the country indicated that, among all under-5-year-olds who suffered from malnourishment, 9.9%

of them had growth retardation, 5.9% had low weight, and 2.2% was too skinny. Infant malnourishment is closely related to unscientific diet. In terms of birth weight and weight increase in the first 6 months, research showed no significant difference between children in our country and those from the advanced ones. The differences appear gradually, however, after the 6th month. One key reason to that is that, many parents failed to add supplementary diet timely and appropriately for infants after they are 6 months old, which significantly affected the infants’ growth rate. Especially in rural areas and migrant communities in urban areas, it is very difficult for supplementary diet to meet the Timing, Reasonability, Safety and Nutrition requirements from various aspects, including the time to add such supplementary diet and the nutrition compositions of it. In 2012, Xintu Project team used the NCLC (Shanghai Sanlin Centre, Minhang Centre and Beijing Centre) as the base to launch its Childhood Nutrition Project. The project aimed at providing its target audience with guidance on infant nutrition and scientific feeding, 0-3 year old migrant baby health information management service and so on. Its objective was to enhance the awareness of scientific feeding for infants among migrant maternity women and their families. In areas directly serviced by this project, the breast feeding rate for 0-6 year old babies increased by 20%. 2) Project Strategies and Key Work Methods Strategy 1: Health education and behavioral intervention on individual’s lack of scientific feeding awareness, knowledge and skill ✓

Intervention Lesson Development –Your baby’s first “kiss”.

Apply trend behavioral intervention directly at the target.

Considering the intervention targets are migrant women, this is a single method, which can be applied easily without involving extra costs.

Behaviour Generalization 1. Make Breast Massage Pads into “kiss” shape lips to help with breast feeding; 2. Make lip-shape round Infant Supplementary Diet Recipe poster to be put on the walls in the homes.

Strategy 2: Organize the community “Health Ambassador” team to arrange club events in targeting migrant women’s lack of social support. ✓

Develop a community “Health Ambassador” team to collect the health information of members, maternity women and infants while undertaking appropriate member record management;

Organize club event in the Life Centre once every month for members to help each other and share feeding experiences;

“Confession of Kiss”—Invite fathers or other relatives to attend. If the mother performed “breast feeding today”, and/or “adding the correct supplementary diet today”, may the father please kiss the mother; and it is also encouraged to record the moment and a portrait photo or video clip will be produced for the participant for free;

Develop a Feed 0~3 year Old Babies Scientifically—Health Ambassador Handbook.

Strategy 3: Use household visits and guidance in targeting migrant mother’s insufficient use of medical services ✓

Divide the service targets into different groups via household visits to discover their actual medical needs;

Health Ambassadors worked with professional volunteers to select local medical resources address book and handed out Local Medical Treatment Maps along with Popular Doctors Index door to door;

Help Health Ambassadors with generating resources—doctors, such as making cards like “a greeting card to my doctor—stamp of a baby’s kiss”, and “my doctor wrote to me”.

4) Accomplishment •

In 2012, Childhood Nutrition Project launched a migrant community feeding status research, which collected 239 pieces of questionnaire (109 from Shanghai Sanlin Centre, 70 from Minhang Centre, and 60 from Beijing Centre);

When the completed questionnaires were collected, the invalid questionnaires were removed and OpenMRS software was used for data entry and analysis. A total of 226 pieces of effective data was entered, and every piece of questionnaire information contained 22 variables.

With assistance from Curator of the Life Centre and the Health Ambassadors, the New Citizen Infant Health Handbook was produced, as well as a Community Work Package—Illustrated Infant Health Handbook Training Tools.

3. Green Harbor 1) Project Introduction Family is a harbor for happiness and harmony. It gives couples or partners a sweet home; and it could give children or senior people a place with love and care. “The Green Harbor” project is a special project of New Citizen Health Care program. The initial objective of the project is to develop a lesson for migrant women, particularly those who used to be a victim of or is currently suffering from domestic violence, to help themselves as well as each other. We firmly believed that women striving in a safe, friendly and non-judgmental environment will make their own lives better! 2) Project Concept In 2012, we summarized the project experiences generated in the past 3 years and further discovered the community needs through community visits. We found that, instead of the traditional idea of “couples holding on to each other to expect the mercy of god”, modern couples who moved into urban areas believe more in “couples working together for the family”. They, on one hand, are keeping the deeply rooted rural life style; while on the other hand, the roles and relationships of the family members are all changing under the influence of city lives. Many working couples are busy with work so they have hardly any chance to sit down and communicate. The lack of communication between couples would often cause conflicts. Therefore, the key objective of our Green Harbor project in 2012 is to “Build a harmonious relationship; Stay away from domestic violence”. This project operated in communities mainly through “Health Ambassador Capability Building Training” and “Health Ambassador Field Workshop”. 3) Accomplishment •

In 2012, the Green Harbor project organized a total of 28 events with 765 direct attendees.

Developed and executed the Harmonious Marriage and Family training course of the Green Harbor project;

Organized the Health Ambassador Capability Building Training and Harmonious Marriage and Family training course 11 times throughout the year.

Health Ambassador Capability Building Training—United Nation’s Convention on the Rights of the Child.

On April 19th and May 30th, the NCLC –Minhang &Sanlin invited Ms. Minjuan YE of “Save the Children” Shanghai Health Project to make a United Nation’s Convention on the Rights of the Child workshop for migrant women in the community, community doctors and Xintu project staff. By engaging the audience in discussions, the workshop helped them to recognize the 4 basic rights of children: the Right to Life, the Right to Protection, the Right to Development and the Right to Participation, which enhanced the community’s awareness on protecting children’s rights.

Health Ambassador Capability Building Training Harmonious Marriage and Family Training Course

The NCLC—Sanlin invited senior marriage consultant Ms. Xingrong ZHOU on May 23rd to provide the Green Harbor training course to the Health Ambassadors. This course contained 11 sessions and the session on that day was the first one: Modern Marriage and Family. The course mainly explained the major causes to issues often occur in modern marriages and families, such as the lack of communication, and their correct solutions. Case studies were used to help the Health Ambassadors gain in-depth knowledge on the key causes to marriage disputes in families so they could better manage their own families.

On the evening of March 9th, Kailin ZHOU, who just became a NCLC Health Ambassador visited the Green Harbor workshop in Guijing Village’s Lijiazhai excitedly. Kailin introduced the principles of happy families and couples, for example that both parties in the relationship must treat each other equally, respect each other, communicate properly, and learn to tolerate.

Community Event Day—International Women’s Day Special Event: Green Harbor’s Art of Marital Relationship

On March 8th 2012, Curator Juxin YUAN of NCLC took Health Ambassadors to the Liangyou Food Wholesale Market on Lingzhao Road for the March 8th International Women’s Day community event—Green Harbor’s Art of Marital Relationship. ✓

Over 200 audiences received the Art of Marital Relationship community promotion on this community activity day;

One of the migrant women was particularly interested in our health project since she felt the subjects were of great help;

By talking to the woman, the Health Ambassador realised that the she quarreled very often with her husband in their everyday life but she did not know where did she do wrong. As a result, the frequency of the quarrel kept increasing;

Curator Juxin YUAN of the Life Centre listened to the situation carefully and provided thoughtful advice. The migrant woman appreciated it very much: “Thank you so much for talking with me for so long and listened to my story.” That was an extraordinary conversation and Xinju was therefore nicknamed “Aunt of the community’s migrant population”.

Green Harbor Expert Forum

On May 27th and October 29th of 2012, NCLC-Beijing organized 2 expert forums on the themes: Parent-Child Relationship and Parent-Child Communication. The forums invited National Level 2 senior psychological consultant Jinjin YU and Dr. Xiaonian YU of Aviation General Hospital respectively to make lectures on parentchild education and communication topics for the community’s migrant families. The forums were warmly welcomed and greatly appreciated by mothers in the community.

4. Beautiful Women 1) Project Introduction The “Beautiful Women” project aims to provide support on reproductive health by particularly emphasizing on the promotion of women’s contraception knowledge and solutions to unexpected pregnancies. 2) Project Concept We believe that, the migrant population is capable of improving the management of their own reproductive health through learning. At the same time, effective learning and proactive actions could be used to prevent and delay such diseases. We also believed that, women’s beauty comes from the perfect combination of internal and external beauty; therefore an attractive new citizen lady shall have both the beauty of health and manner. In 2012, the Beautiful Women project did not only continue with providing mutual help for migrant women on reproductive health, but also attempted to introduce training courses that helped migrant women to increase their external beauty.

3) Accomplishment •

In 2012, the Beautiful Women project launched 82 sessions of events in which a total of 2017 audiences received education and promotion on women’s reproductive health and the significance of gynecological checkups;

Organized community migrant women’s gynecological checkups where a total of 170 women received the free gynecology health checkup service;

Organized 3 sessions of Breast Cancer Prevention Month jogging event, where a total of 260 migrant members participated in the jogging.

4) Work Method •

Health Ambassador Capability Building Training

On March 29th, Curator Juxin YUAN of NCLC—Sanlin led a team of Sanlin Centre’s key Health Ambassadors along with Dr. Qingmin WANG--Director of Sanlin Health Care Centre’s Gynecological Department, to NCLC —Sanlin. They arranged a great training course there on the symptoms and prevention of female colpitis for the Health Ambassadors who newly joined the NCLC and the migrant women in the community. The course was warmly welcomed and greatly appreciated by all of the community Health Ambassadors and audiences. ✓

In the training, the Health Ambassadors learnt the 4 causes to colpitis: 1) Weakened immune system; 2) Infection caused by inappropriate ways to clean the anus and vagina; 3) Unclean sex lives and lack of personal hygiene; and 4) Wearing tight underwear or polyester for too long;

Via the training, migrant women in the community realized the importance of hygiene details that could easily be overlooked in their daily life with regards to causing colpitis;

Dr. WANG emphasized on the causes and treatments on trichomonas vaginitis. It helped the Health Ambassadors to better understand their own health issues and attracted great interests.

Since March to May 2012, Health Ambassadors of NCLC—Sanlin engaged inside the migrant communities to undertake field workshops on health improvement projects on a very broad scale. The workshops covered the subject in a comprehensive way to pass the scientific health information to more people--from individuals to families, were greatly welcomed with sincere gratitude by all brothers and sisters in the migrant community.

Beautiful Women Expert Forum

On December 10th 2012, NCLC—Beijing invited gynecology specialist Ms. Nanfang YANG to the centre to explain gynecology health knowledge to the community’s women audience. ✓

A total of 15 migrant women from the community attended the forum;

The migrant women audiences said that, they actually did care very much about their own health but they were too shy to point many of the gynecological questions out;

Ms. YANG gave patient and detailed answers to the questions raised, including irregular monthly cycles, how did cervical erosion occur, what is a cystis, and how to prevent breast cancer;

Ms. YANG also offered different solutions to various illness conditions.

On March 9th 2012, NCLC—Sanlin invited beauty consultants from Mary Kay to provide an excellent beauty class for the migrant women in the community. ✓

The beauty consultants particularly introduced knowledge on skin structure, skin types and how to wash face correctly;

The beauty consultants taught group by group and offered makeup lessons to the migrant women attending in the event;

5. Healthy You & Me 1) Project Introduction At the moment, the AIDS epidemic of our country has spread from the high risk group to ordinary people. Along with the economic development of the country, the size of migrant population kept growing day by day, and the migration of population increased the speed of spreading between rural and urban areas. The promotion to protect the legal rights of the migrant population and to improve their health environment, as well as improving their knowledge on AIDS, is one of the prioritized directions for preventing AIDS. “Healthy You &Me” is a project that aims to increase the awareness of migrant population’s autonomy organization on sexual transmitted diseases/AIDS and the protective and reacting abilities on the prevention and control of AIDS by migrant workers and their families. 2) Key Work Methods The project utilized Expert Training, Health Ambassador Capability Building and Health Intervention activities to improve the community’s awareness on preventing and controlling sexually transmitted disease and AIDS. Meanwhile, the project also organized migrant workers to attend voluntary AIDS checkups to help discover the disease and treat it as early as possible. 22

In 2012, the “Healthy You &Me” project organized 62 events with a total of 3480 participants;

Throughout the whole year, the project mobilized and organized voluntary migrant worker health checkups 8 times with a total attendees of over 800 people;

On the afternoon of July 25th 2012, Xintu Brand Manager Liying YI and Project Manager Yu TIAN gathered in Yaohua Road training base to accept the knowledge training on Understand Hepatitis B and prevent it scientifically. Also came to the event was the Curators of Sanlin Centre, Minhang Centre and Hongkou Centre as well as the key Health Ambassadors. This training was held by Station Director Seng TANG of Hongkou Communication Station to help everyone better understand the relevant knowledge on Hepatitis B. Main contents of the training come as follows: ✓

The transmission route of Hepatitis B, its characteristics, and notice items for pregnant women with Hepatitis B;

How to gain good hygiene habits in everyday life and via regular health checkups;

National regulations relating to Hepatitis B patients.

In the forum, participants actively engaged in the discussions to tell stories around themselves. They also probed together into the possible ways of spreading Hepatitis B knowledge in their work in the future and how to lead their community members to better understand and prevent Hepatitis B.

During 2 consecutive nights between 17th and 18th August, staff of Hongkou Communication Station visited Kunming Road green space for AIDS knowledge outdoor promotion and on site consultation activities. Kunming Road green space is a popular summer hideout for migrant workers, and that was why the outdoor promotion took place there. ✓

Health Ambassadors of Hongkou station were split into 2 teams to conduct on site Q&A for over 20 attendees.

In the 2-day event, a total of 200 promotional materials on the prevention of AIDS and condoms were handed out;

Via the on-site promotion of Health Ambassadors, most participants claimed they have a better understanding on preventing AIDS and how to use condoms correctly.

Mobilization and Organization of Voluntary AIDS Checkups

On July 8th, staff of NCLC—Hongkou station, with great support from Hongkou District Disease Control Centre,

mobilized migrant workers in the centre to attend voluntary AIDS checkups. The event included

introductions on the prevention of AIDS, promoting China’s free treatment principles and encouraged them to attend voluntary checkups. ✓

The event mobilized 100 participants while 65 of them attended the checkup;

The checkup confirmed 1 positive and 3 syphilis patients;

Due to the sensitive identify of the participants, no image recording was taken during the event on July 8th.

Expert Forum

Around the noon of October 12th, Hongkou station organized a health forum on Women’s Health and the prevention of sexually transmitted disease &AIDS. ✓

Most of the migrant women attending the forum worked as baby-sitters or confinement nurses. Due to the limited education levels received, they could barely or hardly receive any knowledge on sexually transmitted disease and AIDS;

This type of knowledge is critical to every adult;

Hongkou Station invited Dr. ZHOU to give a detailed lesson on the origin, cause and symptoms of AIDS and sexually transmitted diseases.

These migrant women were surprised by the knowledge and found the lesson very useful. They asked NCLC to hold more events like this to provide more varieties for their spare time as well as increasing their health knowledge.

On July 19th, the Director of Hongkou station led a team of key Health Ambassadors to the foot message rooms near Tilanqiao area to promote the prevention of sexually transmitted disease and AIDS towards the women working in those foot message rooms and provided them with condoms. “To Ride; Not to Discriminate” riding event in the promotion of World Hepitis Day received supports from www.ganbaobao.com.cn (“Liver Baby”), www.519liver.com (China Liver), and Chinese Foundation of Hepitis Prevention and Control’s “Love Your Liver for the Life” project, which provided books and documentation materials for activity participants. They also promoted and reported the event on the websites.

“Run, Camel Run” World Breast Cancer Prevention and Control Month long-distance running event received online promotional support by “Base No. 0” and “SHElovestravelling”. Photos of amazing moments captured during the event were reported by China News Net and forwarded around by many internet social media including Sina, 163 and Phoenix. “On October 20th, Shanghai, World Breast Cancer Prevention and Control Month’s Shanghai Version “Run, Camel Run” city 7.8km running event was held along Suzhou creek of Shanghai. The event was initiated by a Shanghai charity organization which, via running, drew women’s attention to the significance of breast cancer and increased public awareness on the prevention and control of breast cancer.”

III. Participation “Participation facilitates Social Inclusion” aims at promoting the migrant population’s ability on urban adaptation and strengthening viscosity of their Social Inclusion. 1. Community Venture “Community Venture” is tailor made for the migrant population in accordance with their current life situation. Many of the migrant population were industrial worker families which came from other regions. After moving to Shanghai, some of families had children so the women had to stay at home to look after the children. As a result, the family lost one share of its income. Under this circumstance, we combined the wishes of migrant women: on the one side they wished to stay home to look after children while on the other hand they hoped to contribute to family income. A project was designed to provid sewing lessons and trainings to open Taobao online stores for the migrant population. We hope the promotion on migrant population’s self-employment can develop a group of outstanding migrant population with the spirit of self-improving and entrepreneurship before gradually bring more and more migrant families into this group.

As of the end of 2012, this project has already took in 10 migrant women and preliminarily achieved profitability with several thousand RMB.

2. Urban Adaptation In order to facilitate the better urban adaptation of migrant population in the aspects of urban lifestyle, culture, skills, etc., NCLC mobilized social resources to hold a variety of activities for the migrant population to help them integrate into the city.

3. All of the community residents who participated in NCLC activities were changed from little doubts to full trust on the life centre. The brand of NCLC were been rooted into the communities. During 2012, NCLC received many visitors and they all highly recognized the services and modes of our life centre.They also learnt and used the mode of our life centre to be their reference of conducting their own community services.

6. Case Study—“The Transformation of a Migrant Woman in Shanghai” Ms. Hongjian CHENG from Anhui Province lives in Guijing Village of Sanlin Town, Pudong District, with her husband. When they first arrived at the city, her husband asked her to stay at home to take care of their children. And she was asked to be better just stay in the room since they live in an area with intensive migrant population, bad security and mixed flow of people. At the beginning, she agreed with her husband and stayed at home to take care of children without going out. One day by accident, she took her daughter to the garden of senior centre to use the fitness equipment and found NCLC to be a charity organization. She then tried to find out what the NCLC was doing out of curiosity. At that time, Curator Ms. Juxin YUAN was on duty, and she told Hongjian that NCLC is a charity organization which provides help and service to the community’s migrant population, think for them, and work for them…… After listening to the introduction, Hongjian felt very excited. As a member of the migrant population, she deeply understood the difficulties of working in a strange place. She made a prompt decision by telling YUAN she wanted to join NCLC and become to a member because she wanted to contribute her own efforts to the community’s migrant population. After joining the Life Centre, Hongjian enthusiastically presented her efforts to the community’s migrant population, and she applied to be a Health Ambassador one month later. By now, Hongjian has already been with the Life Centre for more than 1 year and she always tell everyone the following story in their conversations: “After joining the Life Centre, I am growing and learning new knowledge every day. If knowledge is water, then I am a piece of sponge which can continuously acquire knowledge. I started from learning how to start a computer, how to apply for QQ, and how to typewriting and chat. By now, I can already write microblogs, use skype to hold meetings, and use computer to write monthly brief about our Life Centre. Moreover. I also learnt to do online shopping, which is very convenience and cheap. I also learnt dancing with the dancing class…… After all, I learnt many skills here. My friends all looked at me with new eyes and they think I know more than them. They didn’t believe I could do so many things with computer. So I like our Life Centre, and I cannot live without it. The Life Centre is my second home.” With all the projects she participated in, the Beautiful Women project benefited Hongjian the most. From the ignorant on female body health to now, she nearly became a “community expert” on female health issues. Since joining NCLC, she visits the community’s migrant population families every day; communicates with women in the community; and introducing knowledge on women’s health. She could even speak about venereal disease which used to be a topic too shy for her to talk. Now she has become a “community celebrity” from the previous “never go out, never make friends” house woman. While walking on the way from Shangpu West Road to NCLC, Hongjian would always meet friends who greet and chat with her. In her heart, NCLC has made her a spiritually rich person.

At the same time, what pulled her heartstrings,touched her, and let her selflessly contributed to the NCLC is that, NCLC provides a study platform for the community’s migrant children. NCLC carries out various interest classes for the migrant children, such as oil painting, foreign language, taekwondo and night care classes and so on. The teachers are volunteers from charity organizations. They are so caring and professional that from the eyes of children, they are the dearest person. Hongjian CHENG is a representative of our Health Ambassador’s team, but she is also only an ordinary member.

She is the epitome of many Health Ambassadors. During the days in NCLC, these Health

Ambassadors received a lot but also contributed so much. Within thousands of migrant people, many of them think similarly as Hongjian CHENG from defining themselves as a “stranger” to considered themselves as the “new citizen”of the city. We also believe that, our NCLC will have a great tomorrow in the promotion of community inclusion! Use our love to build the prettiest community!

IV. Learning Use learning to facilitate Social Inclusion is an efficient way for migrant population to be included into the society. During the process of migrant population’s promotion of abilities on personal health management and community affair involvement, their self-discipline and innermost driving force were also greatly enhanced. Learning is the proper way to continuously help themselves managing their own health, attending broader community affairs. And the most important point is that it can promote the quality of their lives and work. This chapter will focus on presenting the operating situation and current progress of our vocational training and Technology Station training provided to the migrant population.

1. Vocational Training 1) Brief Introduction Migrant population is an important labor resource for the city; and employment and entrepreneurship is the essential condition for them to integrate into the city life. Along with abundant migrant population moved to the city to live and work, on one hand, they devoted significant contribution to the city’s economic growth and modernization construction. On the other hand, however, due to their limited educational levels and lack of advanced work skills overall, most of the migrant population did not received any professional and technical training before entering the city, and that probably caused their difficulties in finding stable employment. In 2012, Xintu continuously mobilized the community to help migrant population to obtain the abilities for becoming a new citizen. Such abilities included not only positive self-identity, sufficient information (health information, living information and social communication, etc.), but also appropriate skills (vocational skills, urban life skills, and learning skill, etc.) 2) Training Program In 2012, in accordance with the community’s requirement, NCLC took Sanlin Centre as the pilot platform to offer two professional vocational training courses, they were: •

2. Training in Technology Station 1) Brief Introduction Along with the development and popularization of internet science and technology, the technology is heavily influencing human life and remodeling the ways that human obtain information, communicate and practice social life. There is no exception for migrant population. Modern technology is able to accelerate the Social Inclusion of migrant population. On the contrary, in the technology age, the more serious social isolation will appear if the migrant population cannot access to the technological services and integrate into the technological society because of any subjective and objective barriers. Technology station devotes itself to facilitate the Social Inclusion of migrant population through technology. It repairs/maintains and reconfigures the collected electronics products from public to extend the service life, and then popularize the usage of these products to the migrant population, which can bring the electronics surplusvalue into their entire usage and promote the migrant population to share the splendid information world with others. 2) Training Program Up to December 2012, the Technology Station has already launched a series tailor-made computer training courses for the migrant population in NCLC--Sanlin, Minhang and Hongkou, and they were all were wellreceived. The contents of training consisted of instant messaging tools, online shopping, online ticket bookin, basic application of office suite, video editing, and so on. There were 29 class hours in total which were carried out circularly in the year. 3) Training Method This project adopted the interactive teaching method which invited professional computer training team and university student volunteers to open a special teaching area for the migrant population who wanted to learn computer skills. It also guided migrant population to master the knowledge and techniques of surfing on internet, helped them getting into online communities, helped them build skills to adapt to urban life and work.

5) Cases Study •

Beneficiaries of “Technology Station” in 2012: NCLC community work team

Background of beneficiaries: in 2012, all the NCLC curators and key Health Ambassadors used skype to conduct net meetings and online trainings at least 1-2 times per month, which saved travel time and expenditures as well as improving the efficiency of communications.

At work, they learnt to use mailbox to send/receive emails and transmit documents. The daily operation and project activities held by life centres were all published on the internet for the first time via microblog and photo galleries. They logged on to microblogs to publish activity information and tasks like forward and comment posts became increasingly easy. So far, the Technology Station project has already trained around 10 community training lecturers. Most of those training lecturers are the key Health Ambassadors of NCLC. They mastered the Internet application expertise through the Technology Station first, and they would then pass on those skills to community’s fellow migrant population when they became proficient. The community training lecturers conducted face-to-face training with the community trainees in small groups of 1 to 3. Meanwhile, these lecturers still kept on improving themselves with trainings from “Technology Station”. “My friends described my lifestyle as being more fashion and modern. I also felt my thought is pulled ahead of my friends!” --- migrant women who received the training from Technology Station confidently said so.

V. Special Chapter for Beijing Centre 1. Introduction of NCLC--Beijing 1) Location: NCLC--Beijing is located in Huo Village of Huangcun Town, Daxing District. The registered population in this town is 57,000 and it contained a migrant population of 131,000. The total migrant population registered on the district’s information platform has reached a total of 581,100. 2) Community Environment: There are 5 administrative villages around this centre, which are Huo Village, Liu Village, Li Village, Sun Village, Gui Village and Xingge Village. The total migrant population exceeds 50,000 while the local population is about 10 thousand. Majority of the migrant population came from Sichuan, Northeast region, Hebei, Henan, Heilongjiang, and Shanxi, etc., while biggest share of them are from Henan, Heilongjiang and Hebei. They are mainly engaged in catering, construction, installation, manufacturing, apparel, general merchandise industries and so on. 3) Community Map: Under the guide of curator Mr. Liu from NCLC—Beijing, the community’s Health Ambassadors visited each village, studied the community environment and drew a community map together. “Even having stayed here for 4 or 5 years already, this is the first time I looked into the community and understood it so thoroughly!” the Health Ambassadors said.

4) Introduction of the centre The construction area of Beijing Centre is 240 m2, and it is divided into 7 functional zones: Technology Station, Free-Heart Station, Curator Office, Book Bar, Multifunctional Activity Room, Friendship Superstore and Children’s Playground. Different training methods and contents are carried out in different zones. For example, trainings relate to psychological counseling will be carried out mainly in the “Free-Heart Station”; trainings for Health Ambassadors and curators will be carried out in the Curator Office; activities for children’s urban adaptability will normally be held in the Book Bar and Children’s Playground; and the Multifunctional Activity Room will be opened to activities for the joint participation between women, children and male workers.

Project Development Status Because of the intensive demand from community’s new citizen, although NCLC--Beijing is still in the period of early establishment, we have already organized a series of activities in the exploring process of project capabilities. Such development has been done in line with the core principles of being “based on responding to community demand, copy the new citizen brand experience, develop potential star projects”. 1) Good Luck Mums On 1st July 2012, the life centre Maternity and Child Health Care Service was established. Equipment like baby milk warmer, electronic scale, electronic blood pressure meter are available here. The curator organized community residents to study the knowledge about maternity and infant health.

2) Childhood Nutrition On 16th November 2012, Life Centre invited Ms. Lu CHEN, the Childhood Nutrition Project Manager of Shanghai Xintu to Beijing to provide professional trainings on Childhood Nutrition to the curator and Health Ambassadors.

29th November 2012, the centre organized a maternity and child health care knowledge tea-party. The participated mothers shared their knowledge on methods to keep baby healthier and suggestions on infant feeding. The curator guided participants to study health materials designed by our Children’s Feeding Project Group.

On 6th December 2012, a door-to-door survey about infant feeding methods of the migrant population was carried out. A total of 60 valid feedbacks were received.

3) Beautiful Women On 28th October 2012, we interacted with NCLC—Shanghai to carry out the Breast Cancer Awareness Month Jogging event. After the jogging, all the participated community residents shared their knowledge about preventing breast cancer. “With this activity, I enjoyed the movement and felt happy. At the same time, I realized the importance of breast health to me.”--one participator said.

On 10th December 2012, the centre invited gynecological expert Mr. Nanfang YANG to present issues like a etiological agent of breast cancer, breast self-examination methods, the causes of irregular menstrual/cervical erosion/cyst, and how to prevent breast cancer, etc. to women members of the community.

4) Healthy You & Me On 29th October 2012, the life centre’s curator led Health Ambassadors to promote men’s health knowledge in the nearby communities.

5) Green Harbor On 27th May 2012, the life centre invited Ms. Jinjin CUI, a National Level 2 Senior Psychological Counselor to the centre to provide training and consulting on child-parent education to the community residents.

On 29th October 2012, the life centre invited Mr. Xiaonian YU, Director of Aviation General Hospital’s Psychosomatic Medicine Department to the centre to provide 1-to-1 consultation to community members on methods of communicating with children and the proper ways to deal with the relationships with children.

6) Urban Adaptability After the community mobilization, the life centre is currently carrying out a two-hour children’s activity on each Saturday morning. The activity is mainly about child drawing. It brought paper cutting, games, dancing, singing, improvement of living habit, telling stories into the process of drawing to help the children to integrate into the surrounding environment in their pleased way.

In order to let the community residents to better understand the community and be aware of the service targets of our life centre, the curator organized their own “community survey� with Health Ambassadors during September 2012. They approached several villages such as Li Village, Sun Village and Liu Village to collect information on the life situation and issues from those residents and drew a community map.

3. Progress of Star Projects On the basis of our new citizen brand projects, the Beijing Centre arranged two new star projects – Free-Heart Station and Technology Station. By the end of 2012, the basic hardware and necessary training programs for these two projects have been completed. 51


1) Free-Heart Station Cooperatively developed with the Psychosomatic Medicine Department of Aviation General Hospital and Green Rili (Beijing) Psychosomatic Scientific Development Research Centre, this project mainly focuses on providing services on mental health to the communities. The required biofeedback apparatus are in place already, and curators and the selected Health Ambassadors have been systematically and professionally trained by the Aviation General Hospital. The main courses include: General Knowledge on Psychological Health; Introduction of Modern Biological Health Intervention Techniques; Training on Psychological Intervention of Special Groups (Children &Women); Training On Psychological Intervention of Special Groups (Migrant Workers &The Elderly); Learning Of Biofeedback Apparatus Application in Clinical Psychology and Practice in Clinical Psychology.

2) Technology Station After a month of intense preparation, the decoration of Technology Station has now been completed and the first batch of collected computers has been delivered in place. And the curriculum design has been done. We have already got in touch with voluntary trainers from Lenovo and HP through Beijing Huizeren Volunteering Development Centre and the training of computer knowledge will be carried out periodically within 2013.

4. The Professional Resources of Beijing Centre With the launching of these professional projects and community activities in Beijing, a group of healthcare professionals were attracted. They are: Dr. Xiaonian YU: Director of Psychosomatic Medicine Department and Chief Physician in Aviation General Hospital, Doctor of Medicine/Psychology, Professor. Dr. YU possesses profound medicine and psychology literacy as well as more than 20 years of experience in clinical practice and education. He offered lots of professional suggestions to the Green Harbor Project and our star project- Fee-Heart Station. Meanwhile, he has also participated in volunteer activities in person; 52


Dr.. Nanfang YANG: Former gynecologic expert in Yun Nan Ping An Traditional Chinese and Western Medicine Integrative Hospital, director of teaching affair department in Yun Nan Province Health School with almost 30 years of experience in clinical practice and education. He is mainly dedicates on providing consultation, professional voluntary services to activities including Good Luck Mums and Beautiful Women project. Mr.Yuanhong XIAO: Executive Director of Marie Stopes International China (MSIC). He is mainly dedicated on offering consultation, promotional materials and volunteer services to activities such as Men’s Health and Beautiful Women. Meanwhile, NCLC also attracted the attention from a group of Beijing charity community insiders. VI. Cooperation and Development During the process of focusing on projects to facilitate the improvement of migrant population’s community health, NCLC has been actively builting a social network and structuring a platform for the migrant population to be included into the society. The government, enterprises, international organizations, charity partners, media and friends to new citizens who care about the migrant population came to NCLC and cooperated with us to explore the self-service mechanism of migrant population community and obtained domestic and international attention to spread the news around. The branding effect of NCLC has become increasingly prominent.

1. Conferences •

On 17th July 2012, Ms. Xiaomu GUO, Director-General went to Guangzhou Zhongshan University to attend the 2nd Seminar of “China Population’s Migration and Health”. Around 60 experts and scholars from U.S. Chinese Medical Association (UCMA), United Nations Research Institute For Social Development (UNRISD), Health Department of Guangdong Province, The Harvard University, The Johns Hopkins University, Chinese Academy of Social Sciences (CASS), The Peking University and The People’s University of China attended the meeting. As the representative of NGO, Ms.Xiaomu GUO made a keynote speech with the theme “The Current Situation and Difficulties of China’s NGO to Facilitate the Health of Migrant Population”.

During July 2012, 5 members from NCLC together with NCLC curator went to charity organizations like Qingdao Tongcheng and Xiao Chen Hotlines to share information and jointly discuss the models and experiences of self-services in the migrant population communities.

2. Society Visiting •

Around 50 officers from Changning District Family Planning Commission, who are in charge of migrant population events, visited our NCLC--Sanlin on 5th January 2012. Our Brand Manager Ms. Liying YI explained the NCLC works in details;

Professor Li LING, the director of Zhongshan University Migrant Population Health Policy Research Centre came to Shanghai on 6th August 2012 and specially visited our Xintu Centre for Community Health Promotion. Prof. Li came to NCLC to get a picture of Xintu’s specific practices and experiences on the issues from the health of migrant population.

4. Media Coverage •

Sweden Educational TV Service had interviewed the NCLC, and recorded the innovativeness of NCLC;

During June 2012, the People’s Daily reporter interviewed our Shanghai Xintu Centre For Community Health Promotion, called on all of our charity service brands and especially visited our Shanghai Life Centres to meet our curators. A report Brand, The New Challenge of Commonweal Organization was made after the visit.

Patti, a reporter from Financial Times interviewed our New Citizen Health Care Project, visited the migrant population communities with our NCLC Health Ambassadors, and recorded the launching of our health projects;

On 29th December 2012, NCLC held a New Citizen New Year Party with Daguu (www.daguu.com), and around 10 Chinese media reported this activity;

Shanghai TV Station News Perspective group reported our community autonomy idea as well as the selfhelping and mutual aid mechanisms.

5. Cooperation and Exchange •

Cooperated with Save The Children UK to implement Minhang Centre Maternal and Child Care Program in 2012;

Cooperated with Shanghai Pudong District Population and Family Planning Management Centre to conduct vocational training program;

During 2012, Helin Club and NCLC jointly conducted an one-year course of children art;

Foreign volunteers, such as Jason from USA and Eric from Canada carried out a three-months public practice in the NCLC, and designed series public activities, such as Community Basketball Match, Community Seminars Among Managers, Children Teeth-Loving Day, etc.;

From August 2012 up to now, the CSR department of Australia & New Zealand Bank (ANZ Bank) started to sponsor the young sportsmen from NCLC to carry out Children’s Football Training;

During 2012, the full time lawyers from Dawn Partners regularly came to the Hongkou Centre to conduct legal lectures for the migrant workers;

8th December 2012, ”Help On Portrait Painting” visited our NCLC Sanlin Centre and took pictures for the community residents;

Hilton Shanghai Hongqiao Hotel invited the migrant population families from Minhang Centre to attend the Love Outreach held in November 2012. 55
