Currently, human’s health is threatened by various factors. The worsening environmental pollution and food safety issues, mind and body pressure and unhealthy behavior habit brought by high-speed society, are all key factors for the frequently occurred chronic diseases and popularity of sub-health. Therefore, health services are slowly transformed from therapy to prevention to meet mass’s urgent health requirement; thus, communities and families become to playing a key role in keep healthy and fighting against diseases.

Xintu devoted itself to the development of comprehensive solution on preventing chronic diseases based on communities and families. The core of this scheme is establishing preventive service facilities in communities, thereafter, carry out healthy life style advocacy, sufferer peer education, diseases’ early detection and referral based on these facilities. Thus, “IVY Project” is the synonym of this solution.

Currently, IVY Project has already been popularized in over 10 communities in Shanghai, served over 100 thousands of residents, and developed over 4,000 membership of the healthy club. IVY is constantly cooperating with government, enterprises and patients associations, which advocates healthy life style and help people keep healthy.

Early Prevention for Dementia Project: Early detection, selfmaintenance and family support system based on community.
Population ageing has already become to a big challenge to the whole world. In China, the miniaturization of family and rapid population movement all cause the weakening of family supporting function. Thus, the influence brought by population ageing is particularly deep, and challenge is particularly severe. Meanwhile, since the institution for old-age care is still under-investment, which presented a huge gap, home-based care for aged population and community services are certainly become to the foundation and support for China’s elderly care service pattern.

In connection with individual and family, the issue of aging brings in a gradual function deprivation on living and social life, dramatic decline on quality of life, burden increase of family care. In order to response to the requirements from family, Xintu is cooperating with several community medical and care institutions, trying to establish a long-term care system, which set the prevention of disability as the core.

The first “Yongnianlou Living Center” of Xintu was established in 2010, Shanghai Wai Tan community. Within this possesses 100 thousands population community, Yongnianlou provided services and managed demands to over 600 disabled elders’ families, through establishing a peer supporting network, which covers the whole community. Currently, the service mode of Yongnianlou is popularized in many communities in Shanghai, Qingdao, and other cities.

The rapid development of China’s economy and accelerated transfer of rural labor force are the main reasons of the appearance of large amount floating population in city. According to the National Population and Family Planning Commission of P. R. China issued <2012 Development Report of China’s Floating Population>, currently, the total amount of China’s floating population is approaching to 230 million, which makes up 17% of the country population. Scale of floating population reached to a high record, meanwhile, the amount, flow direction, structure of migrants, as well as interest demand of those migrants are all coming up with significant changes. The most extrusive change is reflected in the migrants’ enhanced desire of permanent residence in their floated in area, also, the stability is increased. They are trying to integrate themselves into the city, even there are still many difficulties.

New Citizen Living Center is a non-profit entity owned by community, which provides public services to migrant, the beneficiaries are all with high-degree of autonomy, and they facilitate community health in the way of self-help and mutual aid. The innovation of “Living Center Mode” could be summarized as: ✤ Established in migrants inhabited region (village in town, industrial area, school for migrant children, etc.) ✤ Involved with different parties in the communities (local government, village committee, migrants representatives, etc.) to participate governance and decision-making affairs ✤ Cultivate community leader with social leadership, to achieve selfmanagement of migrants; ✤ Establish a mutual supported network covering the entire community, meanwhile, strengthen the function of community via the health ambassador team; ✤ Mobilize the community resources (space, human resources, technologies) to participate in providing services to migrants.

GlaxoSmithKline (China) Investment Co., Ltd. (GSK) is the strategic partner of Xintu’s “New Citizen Project”. GSK constantly supports New Citizen projects since 2009. It funded the living centers’ establishment, supported Xintu on promoting reproductive health of migrants. From 2011, the cooperation between GSK and Xintu expanded from Shanghai to nationwide, the new living center has been built up in Beijing. Government: Pudong District Family Planning Commission: Expressed high attention to “New Citizen” - the creative service mode for migrants, therefore, provided a full support on Xintu’s service expansion in Pudong District. Sanlin Government: Provided energetic support after establishment of the first living center located in Sanlin Town, it not only offered place for the living center, but also supported Xintu’s services to local migrants via purchasing the corresponding services package.

The degree of acceptance and consideration on the disabled is a mark of social civilization. Disabled person does not only need appropriate social security and welfare, but also the tolerant social environment, accessible social services and equal development opportunities. The works are always concentrated on “shortage” but not “capacity” for the disabled. They emphasized on the barrier of disabled person, hence, the potentiality of disabled person was not considered. Therefore, the concept and method of improve services to the disabled is as important and urgent as enhance the security level.
❖ “STICK” is an English word, which also has similar meaning in Shanghainese; ❖ “STICK” is able to express the meaning of help and care the disabled, support and coordination.
Xintu devoted itself to develop the self-help and mutual aid service mode for the disabled. It cooperates with government, takes community as the platform and family as the core. Meanwhile, Xintu mobilizes and integrates diversified social resources, carries out prevention, rehabilitation and support services for the disabled, helps them reconstruct their live, establish relationships, develop potentialities, and restore functions.

The World Health Organization (WHO) issued <Global Recommendations on Physical activity for Health> stated: Physical inactivity has been identified as the fourth leading risk factor for global mortality. This follows high blood pressure, tobacco use and high blood glucose. Levels of physical inactivity are rising in many countries with major implications for the general health of people worldwide and for the prevalence of non-communicable chronic diseases such as cardiovascular disease, diabetes and cancer and their risk factors such as raised blood pressure, raised blood sugar and overweight. In addition, non-communicable chronic diseases now account for nearly half of the overall global burden of disease. It is estimated currently that of every 10 deaths, 6 are attributable to noncommunicable chronic conditions.
