Smilow Cancer Hospital is dedicated to helping cancer survivors live healthy, cancer-free lives. New treatment options and innovative cancer therapies are helping cancer patients to live healthy lives long a er their cancer has been cured. e Survivorship Clinic is the rst dedicated, multidisciplinary resource for cancer survivors in the State of Connecticut to provide patients and their families with vital information on cancer prevention, wellness, supportive services, and the latest health research related to cancer survivorship.

Customized Services

e Survivorship Clinic provides dedicated support services for cancer survivors. ese services may include:

• Personalized wellness education and follow-up planning

• Psychosocial support for survivors and their families

• Nutritional counseling

• Exercise and tness recommendations

Specialized Team Care

e primary focus is on the prevention, detection, and treatment of complications resulting from cancer treatment. Additionally, the clinic sta provides guidance and direction to empower cancer survivors to take steps to maximize their health and quality of life following their cancer treatment. At the Survivorship Clinic, patients are evaluated by a team which includes an oncology practitioner, a registered dietician, a physical therapist, and a social worker. Cancer survivors are guided through the program by a program coordinator.

Individualized Treatment Plans

Prior to the clinic visit, the team reviews the patient’s past cancer diagnosis and treatment history. is information helps to design an individualized treatment summary and survivorship care plan for each patient.

Each patient will receive a written plan tailored to their speci c needs and concerns. A copy of this plan is also sent to their referring physician or primary caregiver. Each survivor is scheduled for a consultation and o ered a follow-up visit to further support adoption of healthy behaviors.
