NHG Community Health Team

The NHG Community Health Team provides personalised holistic care through comprehensive clinical treatment for you in your home.

This includes geriatric assessment, home assessment, medication management, caregiver training and home therapy. This is a non-emergency home care service.

Operating Hours and Charges

Monday - Friday: 8.30am - 5pm Sat, Sun and PH: Closed

This service is available only through hospital referrals.

• Visit is charged based on patient’s means-test* results

• Charges exclude cost of medications, lab tests and procedures**

• Medisave and Medishield cannot be used for payment

*Means-test is a method to calculate the subsidies you will receive from the government when you use care services.

** Procedures such as wound dressing, blood test, blood glucose monitoring (hypocount), urinary catheter insertion / wearing, nasogastric tube insertion, etc. will incur additional charges.
