Medical Home What is Medical Home? Medical Home is a multidisciplinary home visit service comprising nurses, doctors and therapists that delivers care for patients with selected acute medical conditions in their own home. The average length of care for patients in the Medical Home programme is about one week. The Medical Home team provides 24 hours a day, 7 days a week phone support and conducts daily reviews, including home visits as medically needed.
Benefits of Medical Home We recognise the importance of family, friends and familiar surroundings in our patients’ comfort and well-being. As such, Medical Home provides patients with the choice of receiving clinical care at home, instead of being admitted to the hospital. At home, patients can recover with fewer hospital- associated problems such as hospital-acquired infections. At the same time, their care needs can be better assessed and addressed in their usual living environment, thus maximising safety and independence.

Medical Home Charges Patients under the Medical Home programme will be charged a heavily subsidised daily rate. This covers home visits by Doctors and Nurses of the Medical Home team. Therapist visits, nursing procedures, consumables, prescriptions, blood tests and other investigations may be charged separately, if carried out during the programme.
