FIRST STEPS TO GROWING THE SPECIALTY It is hard to believe that ACFAS 2024 is only three months away, and with it my term ends and the next chapter begins. It is an honor to serve our profession and all of you within it, and to reflect on why we chose to make this our life’s work. No matter our specialization or style of practice, at one time we were all medical students earning the privilege of being called ‘Doctor.’ Many of us never thought of surgery as our calling until we took that first class, and the daunting decision to pursue one of the most demanding occupations in the medical community if not the world itself is a heavy choice for a weary student. But we made the choice to try, and every day we make a difference in our patients’ lives for it.

At podiatric medical schools across the nation, ACFAS Student Clubs serve as the ambassadors to the specialty for medical students who have taken an interest or declared their intention of becoming a foot and ankle surgeon. ACFAS provides these clubs with training and networking resources to prepare their members for their next steps while bringing them into the fold of our community. An ACFAS student membership is a compounding value that gives access to ondemand education, regional programming, and academic publication access that is critical for those considering if this the specialty they commit to. For those who have made the choice, ACFAS resources supplement their training and residency

with the latest perspectives and news across the industry to hit the ground running after graduation. As a foot and ankle surgeon, you will hopefully come across prospective future physicians exploring their path in medicine. Share your expertise and the passion for serving your patients as a testament to why we have chosen this path and support them on their first step along this journey. Soft Skills: It Takes More Than Good Hands From an Interview with Ellianne Nasser, DPM, FACFAS, Member of the Practice Management Committee Throughout medical school and residency, surgeons are fully immersed in critical knowledge of the human body and the distinct skills needed to perform the life-changing procedures our patients need. But as new surgeons graduate residency and begin their first years of practice, many realize that some of the most important skills were not taught in the classroom or the skills lab. ‘Soft skills’ are used on a daily basis both professionally and personally, but surgeons rarely received direct training in these key abilities that directly impact their peers, institution, and quality of patient care. Three critical soft skills stand out as priorities that every surgeon should initially learn, and then continue to self-evaluate themselves on throughout their career.

Communication Whether it is determining what a patient is experiencing during initial diagnosis or managing tasks in a practice, the ability to collect and transmit important information can save a medical professional time, effort, and stress that can pile on when details or intentions are missed. For diagnosis and situational evaluation, a surgeon must find the ‘right questions’ that do not confuse or intimidate the listener. Learning to listen carefully and visualize smaller details is a matter of personal discipline that takes practice. When it comes to presenting information, translating the key message into language that the patient or peer can understand will prevent their having to fill in misunderstood details with assumptions.

But once a surgeon can collect details and express their necessary information, framing both sides of the conversation with a demeanor and tone that expresses necessary competence with care and consideration. Focusing on brisk efficiency and facts can intimidate listeners and make the surgeon seem less approachable, while presently too passively can make an urgent inquiry or task seem less important than it is. Most importantly, every person will need to find their own voice and style, emulating a mentor’s communication style exactly may not account for differences in culture or circumstances. A good communicator knows their audience, but also knows themselves as the actor.

One lesson often learned too late is the need to prioritize personal requirements and a life outside your practice to prevent burnout and maintain the surgeon’s long-term priorities. Empathy A surgeon’s ability to objectively view a body’s inner workings without being emotionally compromised is a career requirement, but this ability has gained the profession a wider reputation for being jaded and socially disconnected from their patients. Every surgeon gradually finds their balance between stepping back as a clinical observer who must make the right decision, and as a human being supporting their patient at a painful moment in their life. Difficult patients be a test of patience, but they are a bigger test of a surgeon’s ability to understand why the person is making these choices and what they are seeking from your interactions. In the context of peer interactions and staff management, this same dynamic of stopping to question the intentions and contexts faced by others can deescalate a conflict into a signal for what a person needs. Understanding the other side of the story can mean the difference between having ‘enemies’ in the workplace and a cohesive team with diverse perspectives.

Time Management Time management is the skill that can hit a new surgeon the hardest in their first year of practice as they navigate their new responsibilities while resetting their personal life after graduation. Juggling the demands and priorities of multiple patients, leadership figures, and their own needs has often been said to be the worst part of becoming a surgeon. Task efficiency comes with practice, and making mistakes is essential to seeing how they occurred and how they can be prevented. One lesson often learned too late is the need to prioritize personal requirements and a life outside your practice to prevent burnout and maintain the surgeon’s long-term priorities. Skipped meals and lack of sleep can have compounding consequences that can gradually impact performance in the operating room. Being able to ‘unplug’ from patient cases and workplace dynamics regularly can improve focus and overall engagement.

Understanding the other side of the story can mean the difference between having ‘enemies’ in the workplace and a cohesive team with diverse perspectives.

Whether a surgeon is opening their first practice or a seasoned educator, taking time to evaluate soft skills and how they impact the life of a surgeon is an ongoing opportunity to improve oneself as a professional as well as a person.
