• Estudios recomiendan tener entre 0.5 mm a 2 mm de dentina sana remanente. A mayor efecto férula mayor resistencia a la fractura. • En esta dentina remanente se realiza el bisel y el contra bisel. • Ferrule aumenta la resistencia a la carga dinámica oclusal, mantiene la integridad del cemento, y disminuye el stress en la unión poste y muñón.

Cohen 2007. • Al dejar pernos con terminacion junta tope, es decir sin ningún tipo de efecto férula, la acción de palanca se incrementa y se aumento el riesgo de tener fractura radicular. • Al comparar dientes con efecto férula y sin el. La probabilidad de fractura radicular se incrementa.

Configuración coronal y efecto ferrule • La preparación de las paredes axiales del muñón deben ser paralelas para aumentar la resistencia • Un milímetro de dentina remanente coronal sobre el hombro presento una resistencia aceptable mínima. • La línea de terminación no influye en la resistencia. • El bisel y contra bisel no influyen en la resistencia, pero si mejora la adaptación y selle del perno.

• Cementación aumenta tensión. • Debe existir un canal o vía de escape, para evitar fracturas o estallidos. • El uso de Cementos con viscosidad adecuada para este procedimiento es recomendado. Y llevarlo en consistencia fluida preferiblemente. • Realizar aislamiento para proteger el cemento de la humedad y canales secos y adecuadamente manejados.

• Evaluar el estado de la endodoncia antes de realizar el perno colado. • Tener en cuenta la cantidad de diente o estructura dental remanente para poder realizar un adecuado perno colado. Paredes paralelas del muñón. • Es muy importante lograr un efecto ferrule de 2mm. En caso de ser requerido puede obtenerlo realizando extrucción con ortodoncia del diente o cirugía periodontal para lograr un mejorar pronostico. Rodear toda la periferia y estar en dentina sana. • Trate de no ensanchar demasiado el conducto con las fresas de remoción. Para evitar el riesgo de fracturas a futuro o perforaciones .
