FIGHTING BACTERIA

The quest to develop new antibiotics is challenging, but one new discovery might be on the near horizon. A research team at the University of Zurich is working with the natural antibiotic thanatin, derived from spined soldier bugs, which appears to prevent Gram-negative bacteria from forming an outer membrane. Bacteria cannot survive without an outer membrane, so thanatin effectively shields healthy cells from bacterial attack. Researchers using the study results, as reported in the journal "Science Advances," are developing substances that specifically inhibit protein-protein interactions in bacterial cells. The Swiss drug maker Polyphor is currently using the newly-discovered mechanism in its search for new antibiotics.

A Road Map to Person and Family Engagement The following are recommended person and family engagement (PFE) policies and practices for hospitals; they are fundamental to improving quality of care for all patients and in building authentic partnerships among patients and care providers.

Support PFE with policies and dedicated resources

Implement and inform patients and families of family-initiated rapid response teams (RRT) educating them on how and when they are used

Share and teach principles of person- and family-centered care with staff

Develop and execute systems to encourage partnerships among patients, families, and care providers to improve patientprovider decision making

Conduct change of shift reporting at the bedside and invite patients and family members to be active participants

Conduct and inform patients and families of physician/ multidisciplinary rounds and encourage their participation

EXPANDING ELECTRONIC PATIENT ENGAGEMENT As information technologies expand, many hospitals and health systems are using new tools to increase patient engagment, according to survey results reported in the American Hospital Association publication “Trendwatch.� To better serve patients, some health care organizations are offering new options that enable patients to electronically manage select areas of their health care. Today, 50 percent of hospitals and health systems allow patients to request refills for prescriptions online, up from just 22 percent in 2012. Sixty-eight percent give patients the ability to schedule appointments online, up from 37 percent in 2012. Eighty-seven percent of hospitals and health systems have a system that allows patients to pay their medical bills online, compared to 70 percent in 2012. Finally, 68 percent allow patients to send messages to their providers securely through the EHR, up from 55 percent in 2014 (the first year that question was included in the survey). Electronically enabling these activities enhances access to care as well as patient engagement. Survey results show that large hospitals and health systems are more likely to give patients the ability to request prescription refills online, schedule appointments online, pay bills online, and securely message with providers. Smaller hospitals and systems are adding these services at a slower pace.

TARGETING TELEHEALTH ADMINISTRATION The rapid rise of telehealth services has spawned a new academic fellowship at Jefferson Health, which includes Philadelphia University and Thomas Jefferson University. Working in partnership with the telehealth company Teladoc Health, Jefferson Health's telehealth academic fellowship includes a specialized curriculum for physicians interested in a career providing care remotely. The new fellowship, reportedly only the second such fellowship in the country, focuses on the administrative side of virtual care. (Jefferson launched a fellowship focusing on the clinical aspects of telehealth several years ago.) The new initiative is funded by Teladoc, which will help design the curriculum. The program has two separate pathways: one led and held on-site at Jefferson Health and the other designed and delivered by Teladoc. The latter program will provide training on leadership, including driving consumer behavior; telehealth policy leadership and advocacy; software innovation; clinical-quality oversight; and management. Judd Hollander, M.D., senior vice president of health care delivery innovation at Thomas Jefferson University, said in a statement that the school has established a multidisciplinary program for telehealth, and the next step is to educate future leaders so that they can master telehealth and keep providing the care patients need, when and how they need it. -AHA Center for Health Innovation

CONSUMER ALERT: SHORT-TERM HEALTH PLANS Following recent changes to federal rules, Arkansas Insurance Commissioner Allen Kerr issued an alert to Arkansas consumers about short-term, limited-duration insurance (STLDI) health plans.

“I encourage all people purchasing insurance to carefully review their coverage no matter what type of coverage they are looking to purchase,” he says. “Short-term health plans have been available to consumers for decades, but with recent changes to federal rules, consumers should be aware of high-pressure sales pitches claiming short-term plans are ACA-compliant – which they are not.” The key difference between short-term, limited-duration insurance plans and health plans compliant with the Affordable Care Act (ACA) are: • Coverage is for less than 12 months; • Not required to offer same level of benefits (maternity, prescription drugs, dependents, etc.); • Can refuse coverage or charge higher premiums based on health history; and • Not required to cover pre-existing conditions. Those selling STLDI plans are required to warn consumers that these plans do not meet the minimum essential coverage requirements of the ACA. Arkansas consumers who have questions or concerns on any insurance plan, including any high-pressure or confusing sales tactics, are urged to call the Arkansas Insurance Department at 1-800-852-5494.

CLIMATE CHANGE AND HEALTH

FINANCING IS THE FIRST STEP.

In the “Fourth National Climate Assessment” released in late November, one area of risk addressed is the impact of climate change on the health of U.S. citizens. According to the report, “Impacts from climate change on extreme weather and climate-related events, air quality, and the transmission of disease through insects and pests, food, and water increasingly threaten the health and well-being of the American people, particularly populations that are already vulnerable.” The report correlates deterioration of health with advancing climate change in several areas: • Air quality is negatively affected through rising levels of ground-level ozone pollution and an increasing spread of particulates from wildfire-generated smoke. • Rising air and water temperatures have negative effects on food and water quality. Increased heat brings higher levels of waterborne and foodborne diseases, and raises the rate of heat-related deaths. • Climate change alters the geographic range and proliferation of disease-carrying insects and pests, such as Lyme disease-bearing ticks and virus-carrying mosquitoes. The report indicates that “adaptation and mitigation policies and programs that help individuals, communities and states prepare for the risks of a changing climate [are needed to] reduce the number of injuries, illnesses and deaths from climate-related health outcomes.” To read the report, go to globalchange.gov.
