Pregnancy Tests In Singapore: Which Trimester And Costs Congratulations, you’ve gotten a positive on your pregnancy test kit, and you’re now a mum- and dad-tobe! In 9 months (give or take a few days), your baby boy or girl will be unleashed unto the world, and there’s definitely a lot to do and prepare in advance! But, for the moment, now what?

Schedule an appointment with an Obstetrician and Gynaecologist, and listen to their advice on what you two should do during your pregnancy. One part of pregnancy that we’ll be looking at today will be the prenatal screenings and tests that you’ll have to do. There’ll be quite a bit of them, but you and your ob-gyn will be able to come up with a doable schedule.

Why Do Prenatal Screenings and Tests? These tests are performed during pregnancy to ensure the health of both mummy and her baby, and to find out if baby has any conditions, or the risk of them happening. If there are any conditions present and aren’t taken care of, mum and baby could be at risk. Each test allows your doctor to keep track of your pregnancy and ensure that you’re both healthy and doing well.

FIRST TRIMESTER TESTS 1. Blood Tests During your first pregnancy visit, your doctor should ask you to do a Full Blood Count test. It’s mainly done to check if you have anaemia or thalassaemia. Other blood tests that can be done are: Hepatitis B, Rubella immunity, syphilis, and HIV. Procedure: Blood sample will be drawn at the clinic Costs: Depending on your clinic charges

SECOND TRIMESTER TESTS

2. Dating Scan This ultrasound scan can be done from 6 to 12 weeks, and lets you know if your baby is in the right position, if you’re expecting more than one baby, which stage of pregnancy you’re in, and your expected due date. It also checks for any potential abnormalities in the structure of your baby’s bones and organs. Procedure: Ultrasound scan Costs: Depending on your doctor’s charges

1. Foetal Anomaly Screening Scan (FA Scan) This scan can be done from 18 to 21 weeks, and is a much more detailed ultrasound scan with the goal of examining your baby’s anatomy for major anomalies. They’ll focus on the brain, face, spine, heart, lungs, stomach, bowel, kidneys, limbs, genitalia, and umbilical cord. The growth of your baby, amount of amniotic fluid, placental position, and uterine blood flow are also assessed during this scan. Procedure: Blood sample will be drawn at the clinic Costs: Depending on your clinic charges 2. Glucose Screening This is usually done from 24 to 28 weeks, and checks for gestational diabetes – a condition that could develop during your pregnancy, and is usually temporary. As with all glucose tests, you’ll be required to fast for at least 8 hours. This test has been made compulsory at some hospitals (KKH and SGH). Your gynaecologist should advice you accordingly. Procedure: You’ll be asked to drink a glucose solution and blood sample will be drawn Cost: $20 - $40

3. One Stop Clinic for the Assessment of Risk (OSCAR) Usually done from 11 to 13 weeks, it’s also known as OSCAR. This is a first trimester option offered to all pregnant women as an earlier and more accurate method of screening for risk of fetal anomalies, in particular, Down’s Syndrome. Accuracy: 90% with 5% false positive Procedure: Ultrasound measurement of fetal NT (thickness of fluid-filled area behind fetal’s neck) and a blood sample will be drawn Costs: $340 - $380

4. Non-Invasive Prenatal Tests (NIPT) There are 2 types of NIPT in Singapore – Harmony Test (can be done as early as 10 weeks) and Panorama Test (as early as 9 weeks). The tests analyse the DNA in a sample of your blood to predict the risk of Down Syndrome (trisomy 21) and two other genetic conditions, trisomy 18 (Edwards Syndrome) and trisomy 13 (Patau Syndrome). The 2 tests can also determine the gender of the fetus. Accuracy: 99% with 0.1% false positive Procedure: Blood sample will be drawn Costs: $1,200 - $1,400

THIRD TRIMESTER TESTS 1. 3D or 4D Scans These scans are more for parents to have as a keepsake of their unborn baby rather than for medical reasons. The 3D scan is a three dimensional photograph of the baby, whilst a 4D scan is a video of the baby. The best time to obtain a good picture of the baby’s face is between 26 – 32 weeks. Procedure: Ultrasound Scan

Plan a shopping trip with your mum, your MIL (to deepen your bonds and relationship with her, throw her some compliments here and there), or your besties. Focus on yourself, get some fancy maternity wear, sexy new maternity lingerie, cosmetics that are safe for pregnant use, comfy shoes or footwear, and whatever else you need!

BE A QUEEN

They know what you’re going through, because they’ve been through the exact same thing – and it’s great for them, and especially your mum, to share the experience that they’ve went through. It’ll definitely bring you closer, you’ll get some much needed insight as to what comes after baby is born, and it’ll allow them to reminisce about their memories when you were young.

BE MORE FORGIVING INVOLVE YOUR (OF OTHERS & YOURSELF) PARTNER I’m pretty sure you’ve noticed, but there’s a lot going in your body. There’s a new life that’s growing in you, your hormones are all over the place, you’re hungry but your appetite’s being messed with by nausea, you’re getting rashes in places you never knew you could get rashes in, your growing belly needs constant moisturising and scratching, and that’s just the tip of the pregnancy iceberg. So take it easy (I know it’s easier said than done), because stress is bad for both you and baby! Look on the brighter side of things, focus on the positives that you’re going through – forgive yourself for the things you say, do, or feel, and forgive others for the things that they do to annoy you because it’s usually never on purpose.

Plan a date night with your significant other once every two weeks – get out of the house and watch a movie, have dinner, or stay home, get some microwaveable popcorn plus ice-cream, and watch some movies that you two watched way back when you started out dating (nostalgia power!).

Sometimes, your partner wants to help out, but they don’t really know how to – so it’s up to you to help guide them along. Ask them along when you’re heading for your ob-gyn appointments, let them know what you need (regardless of whether it’s a foot or shoulder rub or some time alone), get them to attend prenatal classes with you – it’s all for helping them to get a better understanding of what you’re going through.

Being pregnant is somewhat of a free pass on things that you can do and favours that you can ask from others, but remember: “with great power, there must also come great responsibility”. Exercise your pregnancy powers in moderation, lest you annoy your loved ones to no end. With that said, you can take all the guilt-free naps and eat all the ice-cream (unless you’re lactose intolerant) that you want! Visit a spa, get a pre-natal massage, give your hands and feet a treat with a manipedi, get the full royal treatment but remember to be a just and kind queen at the end of the day!

Pregnancy Bloating And Indigestion: Causes And How To Prevent You can blame these not-so-fun pregnancy symptoms largely on the hormones your body is producing (what else is new?), specifically all the estrogen and progesterone. Your body’s making more of both now to relax the muscle tissue. Unfortunately, this causes your gastrointestinal tract to relax a little too much, leading to side effects like bloating, indigestion, constipation, stomachaches and, yes, all that horrific gas you’ve been having lately. But sadly, you can’t blame it all on the hormones. Believe it or not you could actually be adding to your own discomfort just by what you’re eating or what you wear.

8 EXERCISES To Naturally Induce Labour

Pelvic Rocks

LEANING

SQUATTING

WALKING

Lie down on your back, keeping your knees bent & foot on the floor. Flatten your back against the floor, simultaneously bending your pelvis upwards and hold for 10 seconds, then slowly release.

Make sure you have a professional instructor for your guidance at all times. Leaning against an exercise ball can exert pressure on your lower back and pelvic muscles, helping you get ready for childbirth.

Squatting helps the gravity act on your baby and tends to push it downwards, helps in inducing labor naturally. If you’re concerned about all the funny looks that you may get for squatting in a public park, you can also try this at home.

Walking is belived to be particularly helpful in inducing uterine contractions in women who may have been advised bed rest during pregnancy/ who didn’t vcarry out physical activity whatsoever.

This exercise, when done twice a day for 10 minutes, can help your pelvic muscles get ready for labor.

KEGAL EXERCISE This exercise may be wonderfully effective in inducing labor among women who are overdue. Releasing and contracting the pelvic muscles (the muscles that are involved in stopping the flow of urine) for around 10-15 minutes a day is a good way to get the pelvic muscles ready for delivery.

The ball also helps you ease pains during labour and ensure a natural delivery that is quick and effortless.

BUTTERFLIES

Strengthens and stretches the muscles of the back, thighs and pelvis. It keeps your pelvic joints flexible, eases blood flow to your lower body and eases labor. All you need to do is to sit on the floor with the bottom of your feet together. Then using your elbows, press both the knees gently towards the floor to feel the stretch in your inner thighs. You can use a wall for supporting your back. Remain in the position for 10 to 15 seconds and repeat the stretch 5 to 10 times.

Just keep doing gentle squats till you feel a burning sensation coming up.

Walking is believed to help your baby descend into the lower part of the uterus and helps the cervix dilate, getting it ready for labor.

LUNGES

Climbing a few flight of stairs slowly will help you have a safe labor. It will elevate your heart beat, and the motion makes your baby’s head drop down into the birth canal.

I never knew the value of sleep, until I had a baby. Sleep seemed to be the most elusive thing with my newborn around. What was once an easy thing seemed to be next to impossible. I had sleepless nights for the first few months, while my baby was happily awake. It’s normal for some newborns to sleep up to 20 hours per day according to Scott Cohen, pediatrician and author of Eat Sleep Poop: A Common Sense Guide to Your Baby’s First Year. A newborn sleeps for 16-18 hours on an average. They sleep for a longer time during the day and stay awake through the night. Getting the baby to sleep is challenging, but being aware of sleep-related dynamics, might ease you and prepare you for those sleepless nights to come. Here’s how the newborn’s sleep is going to be like in the first month:

The newborn will sleep for 30 minutes to three hours at random times while feeding in between. You have to wake him up frequently for feeds. When newborns go to sleep, they slip into ‘active sleep’. This is similar to REM (rapid eye movement) sleep in adults, where they wake up quickly at the slightest of sounds. Babies are likely to sleep in this state for 25 minutes, and then transition to ‘quiet sleep’, which is a deep state of sleep. Unlike active sleep, newborns do not wake up quickly during quiet sleep.

The newborn is likely to sleep anywhere between 16-18 hours a day. Now that WEEK you are fairly acquainted with his feeding time and sleep pattern, you can let him sleep and wake him up for regular feeds.

Babies learn to fall asleep on their own by the third week. He will be more awake, and wakes up or cries whenever he is hungry or his clothes are soiled. You can keep track of his feed time, and that will make him sleep when he feels full.

By the fourth week newborns will sleep for 12 – 16 hours. But some baby’s sleep for nine hours while a few other babies might sleep for 19 hours in a day. You can try introducing a sleeping routine at this time.

Every baby has an individual rhythm, some babies sleep for hours, while others are up in an hour. Newborns are not ready for a strict sleep routine or schedule until they are six months old. Motherhood comes naturally to a woman. You will instinctively know when the baby is hungry or sleepy, or when he needs your warm touch. So, don’t lose your patience, instead keep trying at least for two months. We hope our tips will help you set a sleep routine for your baby. Symptoms, Causes And Solutions

A newborn’s tummy is extremely delicate. His system is still developing, and occasional tummy troubles are bound to happen. This in turn will cause you to go into panic mode as he’s crying, and you don’t know what’s the cause! Learn how to recognise the common issues and tackle it like a pro.

SPITTING UP/ REFLUX

GAS PAIN What it is: Air can get into baby’s belly and, as she’s digesting, get stuck. Think of it like a balloon of air in the intestines! It causes pressure, which can be painful. Causes: Babies are prone to gas. But it’s totally natural— it gets produced by the normal bacteria that live in baby’s gut. Crying, fussing and bottle-feeding can all put extra air bubbles there too. How to spot it: If baby has gas pain, his belly might look inflated, or he might be arching his back or squirming a lot because it’s uncomfortable. Baby’s probably fussy too, which can make the gas even worse, since he could swallow a bunch of air while crying. Usually, gas pain is at its worst when baby is about 6 to 8 weeks old. But know that gas pains aren’t usually cause for medical concern, and soon baby will outgrow getting them so badly. How to help: Try to get the gas out - Lay baby down and gently bicycle his legs forward and backward, and try pushing his knees to the chest, repeating several times. If you’ve seen a doctor for this, they will give you gas drops for baby, that could help too.

What it is: We’re sure you know what this is, because you’ve probably been slimed many times already! What can cause it: Chalk it up to an immature digestive system. The valve that closes the stomach off from the oesophagus and prevents food and drink from going back up isn’t as strong in babies as it is in adults. The result? Baby’s food can easily make a reappearance —especially when she eats too much or too fast. How to spot it: Spitting up is just spitting up, as long as baby seems perfectly content afterward. It will only be a medical problem if baby is coughing, choking, gagging, has poor weight gain or projectile vomiting. How to help: Work on prevention. Stop halfway through a feeding (when you switch breasts or baby’s had half the bottle) for a burp so baby has extra time to digest and you get excess air out before it gets stuck in there. Then burp baby at the end of the feeding too. If you’re having trouble getting the burp out, try some different positions. Also try keeping baby upright for about 20 minutes after the feeding, while she’s digesting. Other than that, there isn’t much more you can do. If you’ve got a frequent spitter-upper, accept that you’re going to have to be super-prepared with extra clothes, bibs and burp cloths when you take baby out. You’ll probably do a ton of laundry too. It’s a pain, but temporary—it usually gets better when baby is around 6 months to one year old.

DIARRHOEA CONSTIPATION What it is: It’s not what you probably think—babies can poop anywhere from 8 to 10 times a day to once every 7 to 10 days and still be considered just fine, as long as they’re not uncomfortable. Rather, constipation is about them having trouble pooping when they actually have to go. What can cause it: Baby can get constipated when you switch to a new food, like from breast milk to formula, formula to regular milk, or when you introduce new solids. How to spot it: Keep an eye on consistency. If baby is constipated, the poop will come out as hard little balls or he’ll be farting a lot and will be straining to try to poop. Also, beware of black, red or maroon baby poop, which could mean there’s blood in it. Blood could be a sign of something more serious, so call baby’s doctor if you see it. Yellow, green and brown poops are fine! How to help: First, ask baby’s doctor to recommend a treatment. Depending on baby’s age, the doctor may say it’s okay to give baby some apple or pear juice.

What it is: Diarrhoea in babies refers to frequent, loose, really watery stools. Diarrhoea is scary in babies because it can cause them to become dangerously dehydrated. What can cause it: Diarrhoea usually only happens if baby’s exposed to a virus (maybe through a big sibling) or if she’s eaten something that was contaminated or spoiled, though certain foods can also cause a problem. How to spot it: Normal poop in babies, especially if breastfed, tend to be runnier than the normal poop in adults—so don’t mistake that for the big D. What you’re looking for is an “excessive watery stool”. How to help: Since baby is losing the fluid she needs, diarrhoea requires a visit to the doctor to make sure baby is not losing too much weight and to detect the root of the problem. Make sure baby’s drinking plenty of liquids (breast milk or formula) and is wetting at least four to five diapers a day if he or she’s an infant (as opposed to the three to four for toddlers). Avoid giving baby juice as juice can make it worse because all its sugar content causes the gut to excrete more water.

Night Feeding and Sleep While sleeping through the night is considered a developmental milestone, the age your child learns to master it can vary from 6 months to toddlerhood. At some point, your child will naturally wean off night feeds and just sleep—even if you do nothing to encourage it. Pediatricians recommend avoiding any unnecessary night feedings. Night feeding can be a cause of sleep problems. If a child wakes up to eat several times during the night, a very wet diaper or digestive problems may cause him to wake up and ask for more feeds, creating a vicious cycle. Night weaning may not appeal to all. The benefit of night feeding, such as bonding between parent and child, depends on the individual. In the end, as with all parenting decisions, you should do what’s best for you and your family.

Reduce the amount of milk Give your child a smaller feed. Remove 30ml of milk the first night for two to three nights. Once your child adjusts to the amount, reduce it again and so on until you can stop altogether. A breastfed baby can adjust to shorter durations at the breast.

Offer other forms of liquid Instead of giving your baby a bottle of milk, offer water. Water is less interesting and filling compared to milk and they are less likely to wake up for it.

Self-soothing Night Feeding and Sleep Try to prolong the intervals between feedings. You can try patting and singing your baby back to sleep, rather than sucking on a bottle.

Dream feeding

As your child grows older, they develop a vivid imagination that may cause them to wake up at night. Allow your child to soothe himself back to sleep instead of running to his side the minute he stirs.

Dream feed is commonly known as sleepy feed. This happens at around 11pm, or just before you go to bed. Start by gently rousing your sleeping baby and encourage him to drink milk. Your baby will usually take the feed with his eyes closed, before you gently place him back to sleep.

Moms, please don’t be fanatical about cleanliness at this time. However, make sure to maintain the basic standards of hygiene as babies develop the habit of putting everything in her mouth now.

By this time she may learn to sit on her own. However, don’t worry if she is yet to develop this skill. She is quickly learning the fact that she can control her hands. This shows development of her fine motor skills. The size of her head is however bigger than her body, but she will look more proportionate than before. Her head has flattened at the back by this age. You may find her teeth appearing at this age and you can notice changes in her gums. Make sure to keep changing her position while placing in her cot, so that she does not lie on her same position for long.

Your baby at this age will love animal voices and will begin to understand that different creatures make different sounds. You can show her pictures of animals and tell her about the different noises they make. You will find her waving her toys madly, without knowing how to use them. You need to assist her while playing and help her discover her different vomiting, rash, etc. To help her develop her eye-hand coordination, you can give her cups and blocks to stack. It will be a big challenge for her at this age. Give her different textures to touch. A velvety napkin or a wooden toy and a silky scarf will encourage her to touch them again and again.

Iron fortified rice cereals are a good option that you can introduce at this age. Since breast milk does not contain much iron, it is important to start with foods that are rich in iron. In case your baby does not appreciate taste of the rice cereal much, you can add boiled fruits in pureed forms. Apple or pear can be a good choice. Addition of fruits will also mean addition of Vitamin C that will actually help in iron absorption. However, continue feeding her milk, as it is still her primary source of food.

Sleeping Patterns Your little one will still enjoy sleeping thrice a day. You can expect her to sleep for 10 hours at night. Again, there is nothing to worry if she doesn’t seem very keen on sleeping. You can use different techniques and strategies to lull her to sleep. Self soothing techniques are the best that will ensure she gets back to sleep even if awake at the middle of the night.

No Room for Error On average, most healthy tots will start crawling from about 7-10 months. Crawling is a strong indicator that baby will attempt to balance on two legs to start walking soon, and they do so from approximately 9-16 months. And, according to renowned paediatrician William Sears, over 50% of babies will be walking – albeit to the beat of their own drum – by the time they hit 12 months. Between crawling and making more concerted efforts to walk, most babies will typically begin to “cruise” – that is, learning to hoist themselves up to a standing position before literally taking baby steps to move forward. To steady themselves, babies will reach out for anything within immediate reach for balance and support. The most likely objects babies will have access to are pieces of furniture and gradually, as their coordinated motor skills develop, so too, will their selfconfidence as they pick up courage, stability, speed and accessibility in their learning-to-walk journey. In what will seem like a blink of an eye, parents soon learn that even turning their backs for a second can be somewhat risky business. An unsupervised, cruising baby can go places very quickly and his curiosity and natural clumsiness can render him accident-prone: knocking over and/or pulling down items; touching things he shouldn’t; tripping, falling over or slipping on something left on the floor… the list goes on. Some areas in the home have more lurking “dangers” than other parts of the house, and that’s why it is so imperative to safety proof all rooms in your home and not just those where baby spends most of his time, like the nursery or living room. We are certainly not advocating restricting baby to just his playpen or treating your home like a potential minefield. As much as possible, parents should try to give their baby more opportunities and room to roam about as freely as possible. That’s why it is advised to baby-proof your home well in advance (recommended when baby is aged 3-6 months) rather than be caught unawares when the time finally does come when baby starts becoming mobile. Babyproofing needn’t require a major overhaul to your home or lifestyle, even though the idea may appear intimidating to new parents. Responsibility means taking action, and even if some adjustments seem minor and involve just simple common sense (e.g. keeping sharp objects like scissors and knives out of sight; securing your medicine cabinet with a small padlock), nothing is too insignificant when it comes to enhancing baby’s safety and your peace of mind! We’ve done a sweep of the entire house to compile a handy list of babyproofing tips that can be implemented throughout your home. As mentioned, no extensive renovation works needed! Stay safe!

Electrical appliances

Wherever these are located in the home, make it a habit to unplug and switch these off when they are not in use.

Electrical outlets

Replace all sockets with childproof ones. If this is not feasible, use outlet covers (for sockets constantly in use), outlet plates (for sockets occasionally in use; these usually have a sliding cover that can be moved when the socket is required); and outlet plugs (for sockets rarely or seldom used).

Electrical cables, cords, adapters

This cluster of cords and wires is usually found in the living room, bedroom or study and is normally traced to: TV set, media players, gaming console, computer, phone charger. Their accompanying cables and cords are not only unsightly, but pose as an electrocution risk too. Minimise access by using cable organisers, cord shorteners, reel winders as well as adapter covers.

Kitchen/Bathroom/ Staircases/Other “restricted” Rooms Prevent baby from gaining access to these areas by using safety gates, or doorknob covers and locks. For the bathroom or toilet, ensure that all tubs or pails are emptied of water as babies have been known to drown in water with just a 1-inch depth. Use a toilet seat lock, and non-slip tiles and mats on the floor to prevent falls. Even though you are restricting access to the kitchen with a safety gate, take extra care by using stove knobs, lock away sharp objects and unplug appliances when not in use like toasters and blenders. When cooking, use the stove’s back burners, turn pot handles toward the back of the stove, and keep hot pots and pans out of reach.

Furniture

Attach corner and edge guards to tables, desks or counters with sharp edges. Heavy or unstable furniture such as bookcases, chests of drawers, cabinets, etc., should be securely attached to the wall with tip-over restraints/brackets, or at the bottom with floor anchors. Put heavy objects in the lowest drawers/ shelves and not on top. If you have previously placed breakable, decorative items like figurines, vases, etc. on furniture tops for display, these must be moved to alternative locations where baby is unable to reach them. Also refrain from using tablecloths and other fabric coverings as baby may tug at the edges, pulling down everything else that may be on top of these surfaces.

Household products/ medicines/Other toxic chemicals: Keep these away out of sight and reach in cabinets and drawers. Reinforce nonaccess by keeping these locked with their own keys, or use safety latches or mini padlocks/combination locks for added reassurance.

Plants Some plants may be poisonous; in any case, there is a risk your baby may accidentally consume the soil or gel the plant is grown in. Move the plants out of reach or out of the home if possible. Otherwise, seal the top of the pot with cling-wrap, plastic or fine mesh to minimise access to its contents. Last but not least, practise good housekeeping until it becomes a habit! Make sure all floors and surfaces are clean, and should you spot any potential choking hazards or other small items like coins lying around, promptly keep these in their proper place.

Notes On Safety Gates • Safety gates are available as wall-mounted (permanently affixed to the wall) or pressure-mounted types (portable and pressed on opposing walls). • Before buying, come prepared with the width measurements of the doorway. Gate extensions are also available for larger door openings. • A gate should be at least three-quarters of your child’s height, i.e., 22”/60cm high at least. • Any slat openings must be vertical and less than 3 inches apart (the narrower, the better), so that there is insufficient space for baby to get his head, hands or feet trapped in between.

COMMUNICATING WITH YOUR CHILD: How It Affects Them & Tips For Parents In parenthood, parents may find themselves struggling to find the right words to convey their thoughts and feelings. At times, the tone and choice of words may affect the intended communication, causing a negative development in a child’s growth or relationship with parents. Parenting can be an extremely challenging and daunting job. Communication with their child is further complicated by the fear of damaging the child’s neurobiological wellbeing and concomitant need to protect one’s offspring. As a result, many parents may find themselves being pre-occupied over the minute details of their parenting skills, while trying to care for a child’s basic needs. Parents ought to know that the way they interact with their child, even from when they are just a baby, is likely to have an impact on the way the child interacts with others in the world. Children do learn through modelling the behaviors of key role models in their lives.

Possible effects on children who hear mentally/ psychologically damaging comments regularly Verbal abuse, in which the parent speaks disrespectfully to the child or speaks damaging comments, takes on a personal toll since the child may rely on the parent for his very sense of self. This would impact their self-concept, self-esteem and view of the world. Over time, this can lead to emotional problems (i.e. depression), externalizing problems (i.e. anger and related outbursts) and even impact the way the child interacts with others as s/he matures and develops his or her own relationships.

Dr Sanveen Kang, Principal Clinical Psychologist, Thomson Paediatric Centre said, “Whilst it is possible to say things which may negatively impact your child’s opinion of themselves and the world around them, it is important to first understand what we mean by damaging. The second is to understand that “damage” often occurs over time, unless it is the case of abuse. One would need to reinforce the negative and unhelpful comments over a period of time in addition to the child experiencing the consequences of these statements if they were to become assumptions by which the child leads his or her life.
