So (sorry Mom) I talk. Hiding feelings, suppressing opinions and burying emotions really doesn’t help anyone. In fact, keeping it all pent inside may actually become toxic, subjecting the body to the undue stress that causes or aggravates a variety of health concerns. The same can be true for fear and anxiety, the ultimate inhibitors of change. Letting go of the familiar is uncomfortable, maybe even painful. To me, this quote by Socrates addresses the conflict felt at a personal crossroads—knowing that the status quo is no longer productive, but lacking courage to move forward. Often, instead of facing that fear of the unknown, we busy ourselves with “fixing” things or make excuses for inaction. When all we really need to do is allow excitement for future possibilities to provide motivation. Each day, we are given choices to be constructive or destructive. To build up or tear down. To boost or deflate. To speak out or remain silent. To embrace change or stagnate.

bestfitness 2016 Best Self Atlanta Magazine Courage Award Winners Readers were invited to share their stories of transformation while overcoming difficulty for the chance to be awarded the Best Self Atlanta Magazine Courage Award at the 2016 Lee Haney Games.. Eric Montgomery’s and Miranda Karasch’s continued dedication to becoming their best selves is why they are the winners of this year’s Courage Award.

Eric Montgomery’s journey to health began in 2012. He had just turned 53 and found himself on a myriad of prescriptions. “I weighed 234 pounds and wasn’t involved in any type of physical activity. I ate what I wanted, when I wanted and as much as I wanted,” says Montgomery. Realizing the severity of his condition, he decided to make a change. “I AFTER started slowly with light cardio and weight routines,” BEFORE says Montgomery. Eventually Eric ended up losing two inches off his waist and dropping down to 212 pounds.

Miranda Karasch In December 2014, Miranda Karasch was the heaviest she’d ever been. “I was tired, stressed out and severely unhappy,” says Karasch. She decided to dedicate a year of her life to focusing on improving her fitness and health. “I wish I could say I fell in love with the process immediately, but I didn’t. It took four months of working with a personal trainer for something to start shifting,” says Karasch. She began to gain momentum. “I wanted to get better, get stronger, lift more—and I got sick of listening to my own excuses. My mental transformation began and, soon after, so did my physical one.” Even the heart-breaking end of her 13-year marriage couldn’t slow down her progress. “The gym became my safe place. Training was my one constant, my therapy,” says Karasch. In May of 2016, she was offered the opportunity to expand her business to the state of Georgia. Her biggest concern? Finding a new gym. “Deciding to invest in me to become the best version of myself strengthened not only my body but also my mind and my spirit. The best part is that I’m not done and I never will be,” says Karasch.

He then went on to enter a FitTrip Challenge—a 12-week fitness and nutrition contest conducted by Corporate Sports Unlimited. “During the contest, my body fat percentage dropped from 17.8 to 13.1 percent. I lost 11 pounds of fat and gained three pounds of muscle,” explains Montgomery. To his surprise, he emerged as the men’s winner for his gym, 191 Resolution Fitness, and winner of the 2016 Achievement Award. “I am no longer a high-blood pressure or high-cholesterol patient. I am down to one pill for diabetes, taking insulin only as needed, which is not that often. That’s my story of inspiration and success,” says Montgomery with pride.

“We know that surgery and anesthesia cause stress, which puts the body in a negative protein balance,” says Stuart Phillips, professor of kinesiology at McMaster University and fellow of the American College of Nutrition. “So ideally patients should make sure to consume adequate protein before surgery, too.” Chicken, beef, eggs and fish contain the high-quality protein that the body craves to repair cells. Phillips suggests consuming at least 25 grams of protein per meal. One egg, for example, contains six grams of highquality protein. Three ounces of beef contains 22 grams of protein.

I remember my mom with a pin in her mouth pinning together the fabric on me and feeling like I was a celebrity. I was in love from the first dress.”

“The night I got incorporated, I went out to celebrate and saw this man and was sure he was my husband,” she remembers. “I walked across the room and told him, ‘I know this is going to sound crazy, but I’m going to marry you.’ Pretty productive day: Started my dream career and met my husband!”

“Productive” is a bit of an understatement when describing Spinola’s no-holdsbarred lifestyle. Since that fateful day one of both her courtship of her future husband and founding of her company, the wildly popular fab’rik series of affordable upscale clothing boutiques, her business empire has grown exponentially and her family has expanded to include four children, ranging in age from two to nine. For good measure, she’s even added to the mix a nonprofit that comes to the aid of victimized women and girls. She attributes her success to the focus, drive and organization she says is essential to getting it all done.

“As a mom of four kids, I can get more done in an hour than I used to get done in a day—my to-do list is what some call a work of art,” Spinola says. “But it really just means I like to be prepared. There is no way I could run a company, a nonprofit and have four kids if I wasn’t always thinking ahead and anticipating what needs may come up. Growing up, Spinola’s parents taught her to reach beyond their means to achieve everything she wanted. Her mother made custom outfits for her, introducing her to fashion as a way to elevate everyday life. “My mom made my clothes, so we would spend weekends at fabric stores picking

That first love quickly grew into a full-blown passion that continued throughout Spinola’s youth and into adulthood. “Fashion has always been second nature to me. While I have never been a girly girl, I have always had this deep-rooted love for high fashion,” she says. “My entire room was wallpapered head to toe with fashion magazines growing up.”

Right out of college, Spinola entered the corporate world as a business consultant who helped companies streamline for maximum productivity and efficiency. But after a couple of years, she realized it was time to adjust her focus and redirect her path. She wanted a partner and a family, and something was going to have to give for her to get there. “I wanted a husband, lots of kids, to stop traveling every week and, somehow, an incredible career doing what I loved— which was the fashion world. sure how I was going to have it all, but I was going to try,” she says. Spinola’s dreams were big, but at that time she had no idea just how big they would get. She opened the first fab’rik boutique on August 1, 2002, in Midtown Atlanta on West Peachtree (that store has since moved to Atlantic Station). That was monumental to her at the time, but since then she has watched many other women realize big dreams with their own fab’rik stores. The 45th fab’rik boutique just opened in Chicago. “I never set out to franchise, but as women asked me to open fab’rik in their cities, it became clear that owning a boutique was not only my dream, but

nonprofit, free fab’rik, facilitates weekly visits to safe houses and spends time with girls and women who have been the victims of sex trafficking, homelessness and abuse. It’s a beautiful, safe way to be there for these women.” At the center of Spinola’s extremely full life is focusing on helping others do what she considers herself fortunate to be able to do each day: live her passion and her purpose. “I hope I inspire women to dig deep to figure out what makes your heart beat, because that is your passion and what breaks your heart, because that is your purpose—then follow it relentlessly.”

Basically Bent A joint is a meeting place of bone and muscle, tendons and ligaments. Cartilage is also present and, depending on the joint, there may also be synovial fluid for lubrication. There are six different types of synovial joints in the body that move in different ways. One type can hinge, for example, like the elbow, while another fits together like a ball and socket, like in the shoulder. The knee is your largest joint and the most complex, with three bones, two types of cartilage and ligaments, and tendons. According to Spero G. Karas, MD, director of Emory’s Orthopaedic Sports Medicine Fellowship Program, the joints that are most problematic are the weight-bearing joints such as the knee, hip and ankle. When walking up and down stairs, for example, the knee bears three times one’s body weight—so, a 150-pound person exerts 450 pounds of weight on the patella. A squat, like a lunge on the tennis court, can exert five to seven times your body weight on the same joint. The hip joint withstands similar forces, according to Stanley H. Dysart, MD, of Pinnacle Orthopaedics. Rising from a seated position to standing burdens the hip with three to five times your body’s weight. Joints are work horses, made for wear and tear and constant usage. But they weren’t made to last forever, to endure severe or recurring trauma or to withstand certain diseases.

Inflamed All musculoskeletal pain is inflammatory in nature; joint pain falls into this category. When the body “diagnoses” itself as damaged or ill, it activates blood vessels, blood cells and hormones to initiate its own “treatment.” That’s what causes swelling and subsequent pain. This inflammation can be short-term, related to, say, a sprained ankle, or it could be chronic, as in conditions like osteoarthritis or lupus. FITNESS GOALS AND OBJECTIVES FOR THOSE WITH JOINT PAIN: • Increase or maintain functional strength, endurance, functional capacity and joint stability (stability is more important than strength) and balance and coordination • Increase cardiovascular capacity • Increase or maintain joint range of motion

WHAT TO AVOID: • High-impact exercises and extreme ranges of motion (such as with some stretching) • Exercise with painful/ swollen joints

WHAT IS RECOMMENDED: • Low-intensity and low-impact workouts • Pool workouts (especially in the deep end) would be great for decreasing any joint swelling due to the water pressure. • Postural exercises, joint stabilization and spinal stability (utilizing neutral spine) exercises are recommended. Pilates would be a fantastic way to work all of these. • For cardio—swimming, biking, upper-body ergometers (or arm bikes) and walking as tolerated would be great options. • Participants may wish to ice before and/or after workouts or take anti-inflammatory meds (usually at least an hour before) prior to exercise. the exact cause of the inflammation must be assessed. Julia Kao, MD, a hip- and knee-replacement surgeon with Resurgens Orthopaedics, emphasizes the importance of getting a correct diagnosis and when to intervene. “If joint pain is affecting your life, figure out what it is. Don’t self-treat for too long.” She often sees patients who have waited until their problems are quite serious—a man who came in after his knee buckled to find out that he had severe arthritis, an older woman who had been seeing a chiropractor who actually had a broken hip. Pain that doesn’t resolve itself is a clear indicator that it’s time to make an appointment with a physician.

steroid use, let’s change it.” Nearly one in two people have knee arthritis by the time they’re 85, and one in four have hip arthritis. Obesity can make arthritis worse, much earlier. “The weight conversation is a tough one,” says Dr. Kao. “Sometimes I see a patient in their thirties with terrible arthritis. The good news is that one pound lost [equals] four pounds of stress taken off your knee, for example. Losing even a little bit of weight helps with all joint pain.” She recommends

Painful Truth So when something goes wrong, what’s the culprit? Conditions like “tennis elbow,” which is a type of tendinitis, muscle strains and bursitis are easily treated. Many autoimmune diseases that are tougher to treat cause joint pain, from lupus to fibromyalgia to rheumatoid arthritis (RA). Very often, the cause of joint pain is osteoarthritis (OA), a degenerative disease of the joints that breaks down the cartilage that keeps them moving smoothly and pain-free. When people talk about arthritis, they are usually talking about OA. “Arthritis is caused by a mix of factors. If everyone in your family has terrible knees, you’ll probably have terrible knees,” says Dr. Kao. “If you were a college athlete and had terrible injuries, the effects will show up when you’re older. If it’s a factor we can’t change, like genetics, we’ll just deal with it. But if it’s a modifiable risk factor, like weight or smoking or drinking or 40

putting together a plan with your general practitioner or investigating medical weight-loss programs. An important thing to remember about OA is that it’s “persistent and progressive,” says Dr. Dysart. It can, and will, get worse if you don’t protect your joints.

The RICE Way The first step of joint-pain treatment is always RICE: rest, ice, compression, elevation. “Rest means relative rest, not the complete cessation of activities,” says Dr. Karas. “The body needs movement. If you’re completely sedentary, you’re doing yourself more harm than good. An injured joint needs a gentle range of motion. Synovial fluid, the fluid that

there are other options: steroid injections in the joint, or viscosupplementation, an injection of lubricating fluid in the knee. Physical therapy may be needed to strengthen the muscles around a joint. “The biggest misconception people have is thinking that because they do aerobic activity, they’re strengthening all of their muscles,” says Dr. Dysart. “Aerobic activities are good for the heart, but they don’t necessarily strengthen the shock absorbers around the hip, for example.” The exercises you learn in physical therapy can help with both strength and balance, warding against falls and instability. “A singles tennis player who’s feeling stiff and finding it strenuous to cover the entire court can switch to doubles tennis,” says Dr. Karas. “It’s about finding reasonable modification. If running five miles a day has become uncomfortable, switch to doing it three times a week and take a good long walk, bike or go to the pool. One of the best exercises for cardiovascular health that’s low impact on joints is a good long walk. The difference between calories burned running and walking a mile is only about 20 percent.”

How does LASIK work? LASIK is a procedure that uses a specialized laser to treat errors in the eye, improve your vision, and reduce, or eliminate, the need to wear glasses or contact lenses. Essentially, LASIK changes the shape of the cornea, the transparent layer forming the front of the eye, which controls and focuses the entry of light into the eye.

Am I a good candidate for LASIK? Until you come in for a consultation, we won’t know for sure. The truth is that the majority of people who meet the age and general health requirements and have a stable vision prescription may qualify as a good candidate. So, even if you’ve been told you were not a good candidate in the past, you should consider a second opinion from an Ophthalmologist who specializes in LASIK and other refractive procedures. For example, previously, you may have heard “You can’t because you have astigmatism.” However, these days, with advanced iLASIK technology, people with astigmatism, or other vision imperfections, may qualify too.

Am I ready for LASIK? Generally, you need to be at least 18 years of age to qualify. You also need to have a stable vision prescription to be a good LASIK candidate. I can track your vision and let you know if your eyes have stabilized.

Who shouldn’t get LASIK? There are certain vision prescriptions LASIK simply doesn’t address and certain eye and medical conditions that prevent potential patients from getting LASIK. In most cases, I can recommend other alternative vision correction procedures to help correct your vision. Learning to Give: GivingPoint Empowers Atlanta Youth to Give Back

During their research, they found that a number of young people expressed interest in getting involved with social causes, but lacked the tools to do so. The GivingPoint Institute offers a 10-month-long workshop that ultimately equips 15 to 25 local high-school students

Seniors Who Selfie Even though their mother had barely so much as turned on a computer, Jane Ratliff and her brother were convinced a tablet would make a great gift for her 86th birthday. The realization that there was a whole generation of adults cut off from popular technology simply because no one had shown them how to use it is what led Jane Jane Ratliff to create Bluehair Technology Group.
