Exercise in Nature: Riding my bike makes me happy, especially when I can ride on beautiful less populated, trails around town. For me, this activity used to be much harder because transporting my bike was a pain. The previous bike rack that was on my car had complicated straps and required me to lift my bike up high. Thanks to the expert advice of Mike Wagaman, owner of Peachtree Bikes, I have a new Küat NV bike rack and now, I can load my bike on my car in less than one minute!
Music: I find it amazing how the right kind of music can instantly put a smile on my face, whether I’m doing chores around the house or having a spontaneous night out dancing to The Mike Veal Band. Pet a Pet: I am a pet grandmother to my daughter’s dogs, Gus (pictured) and Harley. Pets bring true joy to so many of us. If you’re looking for a furry friend, Angels Among Us Pet Rescue and the Atlanta Humane Society are great places to start. Relax: You can breathe, meditate, stretch—just slow down and take a minute to be grateful for health, love, and life. I enjoy Qi Gong for meditation, and going to Motion Stretch Studio and Concourse Athletic Club (I love the Sivananda Yoga class) for a good stretch. All the Best!

from the editor A Fresh Start My favorite time of year has arrived once again. I love how the holidays bring out the best in people and remind us to cherish friends and family. And let’s be honest, I’m sure we all love the festive feasting that takes place during this season. Once the celebratory indulging slows and the new year creeps closer, I begin to shift my attention away from my favorite dessert (my mom’s homemade apple pie gets me every time!) and toward more nutritious choices. As I do this, I’ll definitely be incorporating some of the advice health and fitness expert Tammy Stokes provides in our “Keeping It Clean” article (p. 25). Can You Dig It? By Katherine Sauceda
If you’re looking to enhance your skills in cultivation, you’ll want to dig into one of the many classes held at Pike Nurseries, like the one our Best Self Atlanta/Atlanta Best Media team recently did during a Team Activity Day outing. In the roughly 45-minute Make and Take Houseplant Succulent class (at the Buckhead location) led by Matt Mehlberg, we got the dirt on the basics of planting and caring for succulents. This beginner-friendly workshop started with a brief explanation of the plants we would be working with—echeverias, hens and chicks, and sedums. As each team member selected two plants (our art director, Dona Halliday, courageously selected four plants and two pots), massaged the roots to loosen them, and planted the pair in a clay pot with fresh soil, Mehlberg described simple tips to assure our succulents’ survival. His advice included how much sunlight the flora needed, soil enhancements like Dynamite Cactus & Succulent Food, and more. Luckily for those with busy schedules, our little buddies only need water once or twice a month.

It can be difficult to instill and maintain healthy habits in our children. Even if you pack your fridge with lots of fresh veggies and other healthy organic options, through school events, birthday parties, or sleepovers, they will eventually be tempted by sugary foods and drinks that offer no nutritional value. As a mom, I fondly remember the days my children, Kubby and Rania, would eat avocado, any vegetable I made, and happily drank my smoothies. Now, since they are both tweens, it’s a struggle. But as parents, we can’t give up or check out—the health of our kids is the foundation for their brains, hormones, energy, mood, and so much more. Here are five ways you can help your children establish healthy eating habits.

Be the example. Your own diet is being watched every second by these little ones. What you choose is registering in their brains and they will subconsciously mimic your choices as they get older.
Provide healthy options and alternatives to their favorite snacks or meals. Do they love ice cream? Try a lower sugar or coconut milk version. Lemonade? Dilute it to lower the amount of sugar in each serving. I know I am making progress when my children push back on items that are just too sweet or too salty.

Help them identify what foods make them feel good and what makes them feel bad. Bad foods are “never” or “sometimes” foods, the others are “always” foods. Limit junk options in the home, but don’t eliminate. If you totally get rid of the unhealthy snacks, they will feel deprived. But by limiting the amount that is available, the message is clear: these are “sometimes” foods, not “always” foods and this is all we can keep at home.

Teach them how fun cooking healthy can be. Stock your pantry and fridge full of “always” foods. Bring your children in the kitchen with you to help learn how to prep food. This creates an interest in food. They’ll also learn that fresh always tastes better than packaged.

There’s an old cliché, “An apple a day keeps the doctor away.” But I have a new saying for you, “Eat clean. Live lean. Get rest. Decrease stress. Live your best.” It’s basic and true. The reality is that good health is based on simple principles. As a longtime health and fitness industry professional, my best advice is not to complicate it. I remember meeting Jack LaLanne (nicknamed “The Godfather of Modern Fitness”) when I was living in Los Angeles. LaLanne was an American fitness icon who lived healthily until he died at the age of 96. He had a very simple philosophy on food. He ate two meals a day and avoided snacking. His diet was mainly vegetarian but included fish. LaLanne was known for his wise and quirky sense of humor that criticized the consumption of processed foods and he was often quoted for giving this advice, “If man made it, don’t eat it.”

Understanding the essentials of clean eating is important. Clean eating is based on the foundation of eating foods that enhance health, rather than deteriorate it. Food can be a good medicine or a bad drug. Eating a clean food diet is your first step toward better health. To eat clean means to eat real. When you eat whole foods and products made from ingredients that are free from unnatural additives, you are eating clean. Adopting this food philosophy creates better health and often results in fewer doctor visits.
Keep It Fresh Produce is the king of clean. Spend most of your time and money in this section of the grocery store. Beans, legumes, and sources of healthy fats like nuts, seeds, and high-quality olive oil are all good.
Ditch Protein and Dairy Reduce or eliminate your consumption of meats. When you do choose to eat meat, make sure it is high quality (without hormones, antibiotics, humanely raised) and grass-fed. If you eat fish, choose smaller, cold-water fish like wild-caught salmon, Atka or Atlantic mackerel, anchovies and sardines, all of which often contain low levels of mercury. It’s best to buy organic eggs because they contain lower amounts of inflammatory omega-6 fatty acids and have more nutritious omega-3 fatty acids. Clean food diets can include small portions of dairy, but make sure it doesn’t negatively impact your body. Dairy is a common allergen, and in some people can cause inflammation, bloating, and constipation.
Choose Great Grains Did you know that some of the longest living populations in the world include moderate portions of whole grains in their diet? When chosen correctly, they are slow-burning complex carbohydrates that are rich in vitamins, minerals, and fiber. The best whole grains are naturally gluten free, like whole oats, quinoa, and whole-grain rice. If you don’t have a gluten intolerance, then sprouted grains, whole rye, and barley are tasty

options. Finally, clean choices for sweeteners are honey, maple syrup, coconut nectar, molasses, and stevia, but if you can avoid sweetness altogether, even better. Eating clean is pretty straightforward. The tricky part is hidden ingredients. When you order grilled chicken nuggets at a popular fast-food restaurant, you might be surprised to know that there are over 20 ingredients in those nuggets and two of them are different forms of sugar. Mastering clean eating involves knowing what’s in your food. If you can avoid chemicals, food that is overly processed, and unrecognizable ingredients, you’ll do great.

My “no” list Beware of these ingredients found in common foods and avoid eating products made with them: • Monosodium glutamate (MSG): Asian-influenced food, chips, soups, broths and lunch meats • Trans fats: fried foods, fast foods, margarine, coffee creamers, crackers and baked goods • Artificial sweeteners: gum, toothpaste, breath mints, chewable vitamins, liquid vitamins, beverages • Sulfur dioxide: beer, dried fruits, vinegar, potato products, pickled foods, canned coconut milk, cider, soft drinks, and bottled lemon and lime juices • Nitrates: lunch meats, bacon, ham, cured meats, canned meat, jerky, and smoked fish • High-fructose corn syrup: breads, candy, salad dressing, soda, sweetened yogurt, canned fruit, juice, cereals, and ketchup • Chlorine and aluminum: bleached table salt, rice milk, firming agents in pickled foods, and baking powders • Food dyes: breakfast cereals, soft drinks, baked goods, desserts, gum, medicines and beverages Since so much of my work as the founder of West Coast Workout and Cafe West Express centers around food, fitness and lifestyle, I am frequently asked, “So, what do you eat?” I eat a diet of real foods that are minimally processed and contain an

Yes, I eat one every day because apples are rich in flavonoids and pectin fiber. • Oats. Oh, how I love the fiber-rich flax and energy a bowl of Life Porridge from Cafe West Express gives me in the wintertime. • Berries, because they are packed with antiaging, beautifying, and antioxidant nutrients • “Pond water.” It’s awful tasting, but the mix of spirulina, grasses, chlorella and brewer’s yeast give me undeniable energy to do what I do every day. I’m sharing one of my favorite quotes about food from social scientist Claude Fischler, which says, “If you are what you eat and you don’t know what you’re eating, do you know who you are?” Think about it.

Congratulations on the arrival of your baby! Are you prepared for the arrival of your baby’s first tooth? Follow these guidelines and your baby will be on his or her way to a lifetime of healthy smiles. Caring for Gums Even before your baby’s first tooth appears, his or her gums can benefit from your careful attention. After breast- or bottle-feeding, wrap one finger with a clean, damp washcloth or piece of gauze and gently rub it across your baby’s gum tissue. Baby’s First Tooth When that first tooth makes an entrance, it’s time to upgrade to a baby toothbrush. If your little one doesn’t react well to the introduction of a toothbrush, don’t give up. Switch back to a damp washcloth for a few weeks and try the toothbrush again. Brushing with Toothpaste When a few more teeth appear, you can start using toothpaste with your child’s brush. The American Academy of Pediatric Dentistry (AAPD) recommends that you start brushing your baby’s teeth with fluoride toothpaste. If your child is 2 or younger, use a smear of toothpaste (about the size of a grain of rice). For children 2 or older, a pea-sized amount of fluoride toothpaste will be enough. First Visit to the Dentist It’s important that you bring your baby in for a visit within six months of the first tooth’s eruption—usually around his or her first birthday. Since decay can occur in even the smallest of teeth, the earlier your baby visits us, the more likely he or she can avoid problems. We’ll look for any signs of early problems with your baby’s oral health, and check in with you about the best way to care for his or her teeth.

Manual Therapy to Restore Function and Relieve Pain Physical therapy conjures images of exercising on stair-climbers or parallel bars, but the power to heal—and heal quickly—also rests in the hands of skilled therapists delivering manual therapy. Through manual therapy, trained therapists utilize hands-on pressure and manipulation to restore flexibility and relieve pain. When combined with the traditional exercises of physical therapy, manual therapy can hasten recovery and minimize the need for medication. What is manual therapy? The American Physical Therapy Association defines manual therapy techniques as “skilled hand movements and skilled passive movements of joints and soft tissue.” A trained physical therapist uses hands-on manual therapy to improve flexibility, increase range of motion, induce relaxation, and reduce pain and swelling—in short, minimize the consequences of surgery and injury, and restore lost functions. While almost any physical therapy (PT) patient can benefit from manual therapy, it’s especially beneficial for: • Orthopedic surgery patients: Orthopedic procedures promise new lives to those suffering from deteriorating joints, but the aftermath includes pain, swelling, and stiffness. Manual therapy offers a speedier recovery from any orthopedic procedure, including knee replacement, knee or shoulder arthroscopy, ACL reconstruction, and rotator cuff repair. • Low back pain: At some point in their lives, 80% of American adults will experience low back pain. Whether the cause is a physical condition, heavy lifting, or long hours at a desk, manual therapy realigns and strengthens the spine’s complex system of bones and tissue to reduce pain and improve movement. • Injuries: Mishaps can happen at any time. The pain and stiffness of sprains and strains can make it hard to report for work or care for family. As long as the injury doesn’t require surgery, research shows that the sooner manual therapy can be delivered, the faster the recovery. How manual therapy helps: The first visit to a physical therapist starts with an assessment to determine pain levels and movement limitations. In many cases, the primary goals of the resulting treatment plan will be to reduce pain and swelling in order to increase range of motion. In delivering manual therapy, physical therapists might move joints through their range of motion without any help from the patient, or conduct “joint mobilizations.” That’s when physical therapists use their hands to zero in on micromovements at key bone linkages, conducting manipulations that patients can’t do themselves, and encouraging flexibility by degrees. The small advances in movement add up to improved mobility for the entire structure, known as the joint capsule.
Manual therapy techniques may also include: • Soft tissue mobilization using rhythmic stretching and pressure to loosen tense muscles. • Strain and counterstrain to reset muscles to the point of greatest comfort. • Muscle energy techniques, which is when the patient contracts a muscle for a few seconds while the practitioner applies a precise counterforce, with the goal of mobilizing restricted joints and lengthening shortened muscles. A key benefit of manual therapy is efficient use of the time spent in PT. Trained practitioners direct their efforts to the areas of greatest need. A “test-treat-retest” approach continually assesses the effectiveness of each technique, finding those that work and discarding those that don’t deliver progress. What is piriformis syndrome?
What causes piriformis syndrome?
Piriformis syndrome is what we, in medicine, call a “zebra.” You might ask, “What does that mean?” I will get to that later in this article.
• Microtrauma/repetitive trauma due to prolonged sitting on hard surfaces
• Overuse of the piriformis muscle due to long-distance walking or running
• Direct compression or “wallet neuritis” compression of the piriformis and, hence, the sciatic nerve due to sitting on a fat wallet
To fully understand what piriformis syndrome is requires breaking down the words. The piriformis is a muscle in the inferolateral (lower outer part) of the buttock and is mainly responsible for rotating the hip and leg externally (outwardly).
• Anatomical variations
A syndrome is defined as a collection of signs and symptoms associated with a specific health-related issue.
• Typically, both divisions (peroneal and tibial) of the sciatic nerve pass underneath the piriformis muscle (89%)
So, piriformis syndrome is a collection of signs and symptoms attributable to the piriformis muscle. More specifically, it’s sciatica that is not attributable to the low back. In other words, it is not attributable to a herniated disc, stenosis (tight nerve canal(s) due to arthritis) or some other nerve compressive condition originating in the back. Rather, it is attributable to an issue in or abnormality of the piriformis muscle. It is extremely rare for the lumbar spine to not be responsible for sciatica, which leads to piriformis syndrome a being termed a “zebra.”
• One of the divisions runs superior to (above) or through the piriformis muscle (11%)—and is more susceptible to compression
During many arduous years of medical education, medical students and residents are trained to think in terms of differential diagnoses. When a patient presents with a collection of signs and symptoms, we are taught to go through a mental checklist of all of the possible diseases or conditions that could be responsible, even unusual or rare ones. While we were encouraged to (and often rewarded for) come up with unusual or exotic “differentials,” our professors would often redirect our tangential thinking by saying, “When you hear hoofbeats, think horses, not zebras!” Meaning, common causes are far more likely to be responsible—common is common, unusual is rare.
How common is piriformis syndrome? It depends on the source, but prevalence is estimated to range from 0.3 to 6%, or anywhere from three cases per 1,000 to six cases per 100. Regardless of what the actual prevalence is, it is not very common. Nevertheless, someone with pain radiating down the leg will have piriformis syndrome. Signs and symptoms:
• Tenderness and pain in the buttock area
• Sciatic-like pain with tingling and numbness and/or weakness radiating down the back of the thigh, calf, and foot • Worsening of symptoms after prolonged sitting, climbing stairs, walking or running
Diagnosing piriformis syndrome:
• While signs and symptoms are often identical to sciatica attributable to the spine (aka lumbar radiculopathy, or nerve- root irritation or impinging in the spine) there are a couple of defining features seen only in piriformis syndrome
• Sciatica-type symptoms in the absence of a herniated disc, stenosis or other condition of the lumbar spine on magnetic resonance imaging (MRI)
• Electrodiagnostic (EMG/Nerve Conduction Study) revealing sparring of muscles proximal to (above) the piriformis muscle, namely the lumbar paraspinals and some of the gluteal (buttock) muscles

Treating piriformis syndrome:
• Anti-inflammatory medications: nonsteroidal anti-inflammatory drugs (NSAIDs) and/or oral steroids
• Stretching program, mainly focused on the piriformis muscle, often physical therapist directed
• Therapeutic ultrasound and/or iontophoresis (the use of electric current to transport solubilized medication across the skin barrier)
• Massage
• Injection(s), typically of an anti inflammatory medication such as methylprednisolone (cortisone), preferably with guidance (ultrasound or fluoroscopy) in an attempt to relax the piriformis can be utilized if other, non-interventional treatments fail
• Alternative treatments such as acupuncture and chiropractic care may be appropriate in some cases
• Botulinum toxin (BOTOX) injections have been used in rare, extremely refractory cases to relax the piriformis muscle for an extended amount of time when other injection treatments have provided relief, albeit temporary relief
Preventing piriformis syndrome:
• Avoid the causes mentioned above, including prolonged sitting on a hard surface or thick wallet, long-distance running or walking
• Maintain piriformis muscle flexibility
• Stretch frequently! It’s better to stretch 20 times for one minute than one time for 20 minutes
Though horses are far more common, zebras do exist!

Being active is a big part of who you are. How do you set an example for your family and friends to live this kind of lifestyle? I joined Windy Hill for the nursery and during this time, my family has seen that working out is a priority and is primary to my well-being.

If you were helping a person start their fitness journey, what advice would you give to them?
I like to think that I spend just as much time using the amenities at the club as I do participating in the classes. We love the pool when it is warm outside. We are regulars at the café. My husband and I
I would suggest they always keep in mind the goal of progress, not the finish line. Fitness is always evolving and a work in progress. I hope to keep on keeping on till I cannot anymore.

As winter approaches, shorter days can shortchange your health. Cold weather can hamper our activity and less sunlight can bring us down. However, minor adjustments to our routines and attitudes will yield dividends to our health. When we spend more time indoors, we tend to move less and eat more. An easy way to reverse this trend is to take a brisk walk prior to any between-meal snack. Better yet, remove unhealthy munchies from reach. Those summer activities such as swimming and yardwork must be replaced with scheduled exercise. We often only think about drinking extra water when we sweat or when we are thirsty. In reality, heating the indoors dries the air we breathe and hastens dehydration. In addition, when the lining of our airway becomes dry, we weaken the barrier against viruses that cause the common cold and the flu. Start your day with a big glass of water to make up for the dehydration that happens when we sleep. More time inside means more time close to others. When it comes to preventing the spread of winter ailments, the answer is clear—frequent handwashing and annual flu shots. Last year, more than 80,000 people died from the flu. Don’t fall for excuses such as, “it causes the flu” or “it’s only 30% effective this year.” Of course, washing your hands between every activity makes sense. Interestingly, most of these respiratory viruses can spread by direct contact with infected surfaces, in addition to breathing in the small droplets. Bonus tip: always wash your hands before touching your face.
When the sun goes down, so can our mood. Men, in particular, tend to mask the symptoms of depression. Here are some suggestions for sidestepping gloominess: • Make a point of taking short breaks to go outside several times each day • Plan extra social activities with family, friends, and other loved ones • Take a vacation • Incorporate daily stress reduction techniques such as time management, meditation, and exercise • Bolster your sleep routine with light exposure shortly after awakening

In Memory of Dr. Tamara Chachashvili, a physical medicine and rehabilitation physician with Resurgens Orthpaedics, passed away on October 27, 2019 following a brief illness.
Dr. Chachashvili joined Resurgens Orthopaedics in 2002 and was beloved by her Resurgens Orthopaedics family as well as her patients who came from all over Atlanta to her practice in Lawrenceville, GA. Dr. Chachashvili received a medical degree in Tbilisi, Georgia, formerly Soviet Republic, with a specialty in anesthesiology. Upon relocating to the United States, Dr. Chachashvili completed a Physical Medicine and Rehabilitation residency at Wayne State University in Detroit, Michigan and a Fellowship at the Non-Surgical Orthopaedic and Spine Center in Marietta, Georgia. After joining the Resurgens Orthopaedics location in Lawrenceville, GA, she developed a very successful physiatry practice specializing in non-surgical spine care, spinal injections and electromyography and nerve conductive studies. She quickly became the preeminent PM&R practitioner in Gwinnett County.

She was adored by all who knew her and will be sorely missed.” Outside of the office, Dr. Chachashvili, or Dr. C, as she was known, enjoyed spending time with her husband and son, gardening and cooking. She was an avid traveler and particularly enjoyed meeting her sisters at far off locations around the world.
She was a member of the American Academy of Physical Medicine and Rehabilitation, the Physiatric Association of Spine, Sports and Occupational Rehabilitation, the American Society of Interventional Pain Physicians, The North American Spine Society, the Georgia Orthopaedic Society and the AOSpine International.

Meet the Mobile Movement for Autism Awareness By Nicole Letts The Cain family consists of five go-getters. There’s mom, Mañana, a commercial and residential realtor with Keller Williams and owner of Bridge Boutique. There’s dad, Khari, an esteemed record producer known throughout the music industry as “Needlz.” There’s big sister Amani, 15, an up-and-coming performer in her own right with a song on iTunes called “U Need Me Too,” and there’s little brother Nasir, 9, who, with a talented supportive family like his, is well on his way to discovering his own spot in the Cain dynasty. And then, there’s middle child Makayla, 11, an inspirational young lady whose story has laid the foundation for something much bigger than she can possibly imagine.

PMS is a rare genetic condition in which the 22q13 chromosome is deleted, causing weak muscles as well as other medical and developmental delays.

With a diagnosis in hand, Mañana and Khari dove headfirst into getting Makayla the necessary care she needed. That meant a lot of research, a lot of meetings, and a lot of cash. “We ended up paying $6,000 to $8,000 a month for therapy,” explains Khari. It was then that the family realized that their work with and for Makayla would be a marathon, not a sprint, and that unlike others who may find themselves in a similar situation, the Cain family was armed with the resources to give Makayla what she needed. “A lot of people don’t have a lawyer or advocate to help them. A lot of people don’t even know what an individualized education program [IEP] is, and those who do know, leave it up to the school to dictate their IEP. So for us, a lot of what we do is trying to bring awareness to this issue,” says Mañana. bestselfatlanta.com

The ABCs of Autism Spectrum Disorder According to the Centers for Disease Control and Prevention, autism, sometimes referred to as autism spectrum disorder, is defined as a developmental disability that can cause significant social, communication and behavioral challenges. Cheryl Klaiman, Ph.D., program director for diagnostic services at Marcus Autism Center emphasizes that every child develops differently, but some common early signs of autism might be:
That’s where the Cain family’s latest initiative comes in: Makayla Moving Autism, a traveling state-of-the-art bus equipped with seasoned, licensed professionals who will conduct screenings, provide parent education, implement therapies, and encourage advocacy. The plan is that in 2020 the bus will be seen around the city at various festivals and events but will primarily be an educational resource in communities of color. According to research conducted for Makayla Moving Autism, statistics are alarming for autism spectrum disorder diagnoses in children of color. For example, African-American children are over five times more
likely to be misdiagnosed with conduct disorders before being diagnosed with autism. Additionally, white children are roughly 19% more likely than African-American children and 65% more likely than Hispanic children to be diagnosed with autism. That means white children not only end up with more access to additional therapies, education and resources, but they’re also more likely to be given access at an earlier age.

Evidence of repetitive behaviors such as a tendency to line up toys, peer at objects through the corner of the eyes, hand flapping, rocking and other similar behaviors. Dr. Klaiman also stresses the importance of advocating for yourself and your child. “If you are concerned that your child might be showing symptoms of autism, there are resources to help you learn what, if anything, is going on with your child. Keep being an advocate and know that we are here to listen.”

The Cain family wants to make a dent in these statistics and change them for the better. While the Cains and their talented partners won’t be able to formally diagnose children on the bus, they can spread awareness and point people in the right direction for getting the answers they deserve, and the support they need. “We want people to return. We don’t want it to be a one-time experience. We’re all about developing relationships with these families,” Khari says. Makayla Moving Autism, the traveling autism education resource, will be Makayla’s legacy as well as a legacy for the entire Cain family. For now, the Cains have launched their fundraising efforts, which are live on a GoFundMe page, but even more information about the Cains and their plans can be found on their website, www.aslongasigotyou.com. Events and updates will be posted regularly, and soon enough, website visitors and donors will also be able to view a short documentary film of the same name about Khari and his family. “[The bus] is really about our journey to provide a better life for Makayla, and it’s turned into this big thing. She’s a big a part of it, and it’ll be great for people to meet the person who inspired it all,” says Khari.
