EAT RIGHT, LIVE WELL BY REBECCA CUTHBERT

T

astes change, dietary needs evolve, and food trends come and go. It’s hard to know what you need more of, what you need less of, and how to keep food fun. But just because you’re only making meals for one or two doesn’t mean dinner must be an endless rotation of frozen food and canned soup. Instead, try new things, explore your options, and always check with your medical professionals about specific dietary guidelines. Vitamins and nutrients maintain health, fight infection, and keep you strong enough to enjoy activities, says Karen Krnjaich, registered dietician and certified diabetes instructor with Independent Health. “As we get older, our bodies have different needs, so certain nutrients become especially important for good health,” she adds. “One common deficiency, often in people over the age of fifty, is Vitamin B12. You can get B12 through any food that comes from an animal: meat, fish, dairy products, and eggs, [but] as people get older they can develop a reduced ability to absorb vitamin B12. “Balance problems, difficulty thinking, numbness or tingling in the hands or feet can all be symptoms of a B12 deficiency,” Krnjaich continues. “Often the condition is overlooked or confused with something else, so it’s important to have a conversation with your doctor, if you think you are at risk.”

“At Fox Run, our menus are designed for variety,” says Nutrition Care Manager Katie Ketchum of Fox Run, Orchard Park. “A lot of seniors rely on food memories—favorite meals, comfort foods. We offer those, because excluding what you enjoy can negatively affect your quality of life. But, wellness and nutrition are so important, so we balance that with ‘fit meals,’ like low-sodium and calorieconscious foods, so everyone can find what they want and what they need.” Ketchum adds that it’s a lot easier to eat well when you have fun with food and make meals social events. “There’s a social aspect to eating that we all enjoy,” she notes. “Have a meal with friends. Take a cooking class. We offer those at Fox Run so that people can try new recipes or experiment with new ingredients. We also offer nutrition classes, and, recently, we had a food and wine pairing event!”

Ketchum reminds those cooking at home to include the three staples of dinner: protein, veggies, and grains. And to stick to budget and leave time for other activities, use the freezer. “Remember that you can still cook or bake for just one or two people,” she says. “Freeze portions to save for later. It’s cost effective and saves time.” There are so many ways to have fun with food. Shop with friends, and have everyone buy one never-beforetried item. Form a “diners club,” and alternate cooking and hosting. Plan a birthday dinner at a new restaurant. Invite family members over and teach them one of your “secret” recipes. Or, go exploring at a farmers’ market. The farmers’ market at Fox Run will be back this spring, with old favorites and not-so-familiar finds. Plus, there are on-site cooking demonstrations to teach you how to achieve culinary success in your own kitchen.

FOREVER YOUNG But, before you embark on a dietary adventure, check with your doctor. Certain combinations of medication and food can produce harmful effects—and if you have a chronic condition, such as diabetes or high blood pressure, there may be food no-nos on your list. On the other hand, as Krnjaich noted, your doc may tell you that you have a deficiency; in that case, you can seek out great new menu items to fill that gap. If you have an active lifestyle or seem to always be on the go, bring

along healthy snacks. Something as simple as a small bag of almonds, an apple, or a protein-rich smoothie can carry you from one meal to the next without your energy flagging. Seniors today have more going on than ever before. Between visiting family, shopping with friends, volunteering, working, playing, and traveling, it’s easy to forget to eat well. But without key nutrients,

balanced diets, and important vitamins, energy levels and even moods can suffer. However, with a little forethought and an open mind to try new things, you might just discover your inner foodie! KEEPING ROMANCE ALIVE BY JUDITH A. RUCKI

H

ow do we keep romance alive, especially if we have been married or in a relationship for many years? We’re all familiar with the small gestures like flowers for no reason or a love note tucked inside a brown bag lunch.

Some couples might indulge in a romantic cruise or opt for an exotic vacation. But what really keeps the fires burning? Interestingly enough, several couples willing to answer the question said it’s the “little things” that can make all the difference. Here are a few examples. Expecting leftovers for dinner and discovering instead a nicely set table, dimmed lights, soft music, and your favorite meal waiting enhances the mood. One wife loves it that her husband stands up when she is leaving or returning to the table at a restaurant. He holds her chair and, basically, behaves in an old-fashioned, gentlemanly way. Garnering similar appreciation are manly gestures like holding the car door or running to bring the car around when the weather is frightful. One wife who had been working late several days in a row came home to find her husband had done the laundry, down to folding all the towels. She said that meant more to her than receiving an expensive gift. Husbands who travel regularly for business say they are thrilled to return

home with nothing planned for them other than comfort food and a chance to unwind. Taking over responsibilities that your partner dislikes cannot be overrated. He hates doing taxes, so she gets them done. She isn’t interested in cars, so he makes sure her tank is full and the inspection sticker is current. Not complicated tasks, but a relief for whoever dislikes doing them. Most couples have at least some common interests. But not every woman loves sports and the majority of men would probably just as soon stay home than be hauled off to an origami workshop. Having time to enjoy their own interests is important to many couples. If he wants to spend hours watching the playoffs, she is just as happy to round up a group of girlfriends and go see a foreign film replete with subtitles. Time spent together is great, but now and then time needs to be set aside for individual interests and hobbies. It boils down to giving each other some space rather than being constantly joined at the hip. Speaking of space, creating places

in the home where each partner can retreat, uninterrupted, can prove blissful. It could be anything from a man cave for him to a quiet place where she can read or do yoga. Everyone has pet peeves. And partners always know which buttons to push. So what if he leaves the seat up? Put it down. She drops her briefcase by the door? Pick it up. No nagging! Sometimes learning the fine points of romance means looking no further than your own family. One of my uncles was a big formidable man and a former boxer. Stopping by to visit one day, I saw him standing outside the door, puffing on his cigar and waving around a handful of doilies. When asked what he was doing, he said his wife worked just as hard as he did all week. If he could help with the cleaning, he was happy to do so. I never heard my aunt complain that her fine linens smelled of cigar smoke. But she did smile a lot. FY Judith A. Rucki is a frequent contributor to Forever Young.

SEXUAL HEALTH AND AGING: Keep the Passion Alive

Sexual feelings don’t disappear as you age. Here’s how to keep the flame burning.

Communication is key Talk with your partner. Help your partner understand what you want from him or her. Be honest about what you’re experiencing physically and emotionally.

Aging and men’s sexual health Testosterone levels gradually decline throughout adulthood and erectile dysfunction becomes more common. Several medications are available to help men achieve or sustain an adequate erection.

Aging and women’s sexual health As women approach menopause, their estrogen levels decrease,

which may lead to vaginal dryness and slower sexual arousal. Emotional changes are somewhat more common in women.

Medications and sexual health If you think you are experiencing sexual side effects from a medication, consult your doctor. It may be possible to switch to a different medication with fewer sexual side effects.

When one partner becomes ill If you’re ill, your sexuality may temporarily take a back seat to other needs. If you’re the caregiver, find a way to set aside the caregiver role from time to time and be a partner instead.

Dealing with differences in desire If you mainly avoid sex, consider taking charge of some engagement. If you usually initiate sex, try talking with your partner about what you need.

Looking forward not back Many couple’s sexual arousal and activity levels change with age. Ask yourselves what’s satisfying and mutually acceptable. Excerpted from mayoclinic.org

BUFFALO AND BEYOND BY REBECCA CUTHBERT

M

any retirees have “travel” on their to-do lists, but when the time comes to make nebulous plans into actual ones, folks often don’t know where to start. Where you should you go? How can you make sure you’re getting a good deal? How can you keep yourself—and your finances—safe? How can you optimize chances of having so much fun that you’ll be eager for more? The best plan? Consult professionals. From local day trips to international adventures and everything in between, touring and travel agencies can customize packages to both budget and activity level. “One of the most important factors when booking a big trip is to use a reputable travel agency,” says Marilynn Militello, The Travel Team Inc. in Buffalo. “An agency ensures that you are getting exactly what you purchased, plus travel agents are happy to help. We can assist in all aspects of planning, right down to coordination of tours and restaurants.” “At Way To Go Tours, our clients have their daily itinerary set,” says Jerry Waldkoetter, of Way To Go Tours Inc. in Tonawanda. “They know that they’re going to have full days sightseeing and eating at the best restaurants; we do that research for them. We usually leave the hotel at around 8:00 a.m., and get back around 8:00 p.m. No one wants to pay for a vacation where they’re sitting around in their hotel room. Our packages are an excellent value.” Travel clubs offer seniors the opportunity to meet new friends, try new things, and feel secure in having company on vacations. Traveling with a group is a great way to have fun and stay safe, and more experienced adventurers will always look out for the newbies, assures Laurie Russo, president of the Cheektowaga Seniors’ Travel Club. “Stay with your group for safety,” Russo advises. “Follow the leader, because you don’t want to get lost or be taken advantage of. And while you’re having fun, be aware of your surroundings.”

Russo offers a few triedand-true tips for protecting property, as well. “Don’t keep your wallet in your back pocket,” she advises, noting that pickpockets check there first. “We say to get those pouches from AAA, the ones that tie around your neck and hang down your shirt. And purse straps can be easily cut; better to leave [purses] behind.” To manage expenses, Russo suggests a credit card and some cash; use small bills, and don’t keep it all in one place. Credit cards have an advantage over travelers checks, which aren’t accepted everywhere, and personal checks, which are probably accepted even less. Plus, if a credit card is lost or stolen, you can call the company right away to freeze it, and likely won’t even be responsible for any theft charges. Alert your credit card company before your trip, so they don’t flag your charges as unusual, which could possibly result in a hold. While you’re on the phone, ask if there are transaction fees (particularly for overseas trips): “You don’t want to use a card that will charge you for using it,” cautions Russo. Russo also recommends that travelers wash hands often, take an immunesystem supplement (like Airborne), and wear comfortable clothes. And Militello advises that seniors always bring more medication than they think they’ll need, in case there are travel delays. “And always carry medications in a carry-on bag,” she says. “That way, if luggage is delayed, you won’t be without your medicine.”

And speaking of trip trouble, “Travel insurance is invaluable,” Militello says. “If something happens and you can’t make the trip, you can get a refund. And if you happen to become ill or hurt while away, travel insurance can provide the needed assistance to get you home.” To ensure expedition should such a situation arise, have health insurance information with you, as well as emergency contacts and medical ID bracelets. If traveling with a group or agency, share that information with tour leaders before you leave home. AAA has a wealth of information available for would-be travelers wanting to plan special trips or sightseeing tours, and its agents can also point out deals, maps, itineraries, and valuable coupons. Visit a AAA physical location in Clarence, Ken-Ton, Orchard Park, or Amherst; or go online to westerncentralny.aaa.com for more on trips and membership. Don’t let “travel” linger on your wish list. With so many agents, packages, tour groups, and amazing destinations, you can discover the beautiful world in Buffalo and beyond! ASSISTED LIVING OR AGING IN PLACE? That is the question BY TOBY F. LAPING, PH.D.

D

eciding between assisted living or remaining in one’s home isn’t easy. Most of us want to stay where we are, but know it’s not necessarily wise, so we feel pressured to move; it’s a common dilemma without a perfect solution. In weighing options, here are some of the many issues you’ll need to think about. First, how safe is your home and what should be done for you to remain there? At minimum, consider: Must you climb stairs on a regular basis? If so, can your bedroom and bathroom can be relocated to the main floor? If not, how safe are the stairs and banisters? Are the steps clear of clutter? Is your balance good enough to manage going up and down? Can you put a chair on the landing in case you get tired? Would a chairlift be helpful and affordable, and would you use it? Is the bathroom as safe as can reasonably be expected? For example, have you a raised toilet seat, and are there grab bars (not the towel rack, please) to use when standing? If you have a stall shower, is there a bench in case you’re suddenly tired or weak? If you must step into a bathtub, are there grab bars? Is there an abrasive surface on the floor of the tub? Would a transfer bench make entry to the bathtub safer? How about the kitchen? Lifting hot pots can be dangerous; arrange things so that’s not necessary. Also, stretching to reach often used items is hazardous. Can you work at the counter while sitting down? Be objective; is the kitchen safe for you as it is? How much do you need to cook? How is your nutrition? Are you cooking healthy meals? If you’re eating “tea and toast” for many meals, that’s a danger sign that calls for a different meal strategy. Share meals with neighbors, get Meals on Wheels, or eat at a hot meals site. How consistently do you take your medications as ordered? Can

you maintain your health without the presence of a nurse? If you’re alone or your partner has health problems, get a medical alert system. Newer models have GPS and fall awareness systems that summon help if you’re unconscious. How about costs? If your home is a difficult financial stretch, perhaps a reverse mortgage is suitable. There may be entitlements including veterans benefits you can access. Or, consider senior housing now while you have resources to pick a place that appeals to you. It’s critical to interact with others regularly, so if days pass when you talk with no one, that’s a red flag for mental health. Call a friend for lunch. Become active in a group or organization; go as volunteer or participant, but go! Or, seriously, consider moving. Which brings us to the other side of this equation: what should you consider about assisted living? First, your budget. Some assisted living complexes are full rate as long as you live there, while other complexes will accept a reduced rate after a period of time. Be aware that most enhanced senior living facilities only accept full rates. (If money is an issue and if you’re a veteran or spouse of a veteran, you may be eligible for Aid and Attendance to subsidize your income.) Have a conversation with admissions staff. Don’t hesitate to ask challenging questions about anything that leaves you in doubt. For example, some assisted living complexes have relationships with higher levels of care

so residents can move within the same system if health deteriorates, with less attention to income and resources; ask what happens if you need more care. If hands-on help would be useful, is there staff to give as much as help as you’d like? How much additional cost does that incur? Remember they are marketing; you are the buyer. If you find a place that appeals to you, spend time in the lobby, watching people and observing if they’re friendly and/or responsive to you. Ask residents if they’re happy and recommend the place. Eat several meals at different times of day; judge the food, and whether the residents/tenants welcome you at their tables, and if conversation feels comfortable. If you prefer eating alone, does that seem possible? Are there facilities to match your interests, e.g., exercise or crafts rooms? Whether children or friends support or challenge the idea of your moving, bring them along when evaluating a facility. The extra pairs of eyes and ears help when assessing your potential new home. HEALTHY LIVING FOR A SHARP MIND BY MARIANNE POTRATZ

Y

ou’ve probably experienced the frustration of walking into a room and forgetting why you were headed there. Or the panic of having left the house and suddenly wondering if you turned the iron off. If these moments are becoming more frequent, is it simply aging, or something more? And is there anything you can do to keep your mind sharp?

Lifestyle is key to brain health The better you take care of your body, the better it will work, and that includes your brain. “The human brain is an organ [that] needs care, feeding and exercise,” says Dr. Lee R. Guterman, PhD MD, medical director of neuroscience at Catholic Health System in Buffalo. A healthy lifestyle can delay memory issues, or even prevent certain memory issues completely, adds Dr. Kinga Szigeti, MD PhD, a board-certified neurologist with specialty training in genetics and cognitive disorders. In addition to practicing at UB|MD Neurology, Szigeti is also director of University at Buffalo’s Alzheimer’s

Disease and Memory Disorders Center, where researchers are working to improve methods of disease prevention, diagnosis, and treatment. “Clinical studies have shown physical and mental exercise are extremely beneficial for keeping your mind functioning at its best,” Szigeti says. Aim for at least thirty minutes of moderately intense aerobic activity, at least five days per week, and always check with your doctor before beginning an exercise regimen. In addition to physical activity, mental exercise—anything to stimulate and engage the brain—is also important. And this doesn’t just mean online memory games or crossword puzzles.

“Social activities are great mental engagement,” says Szigeti. Playing cards with friends, for example, is stimulating because it involves both strategic thinking and socializing, and multitasking is a great mental workout. Taking classes, perhaps at your local senior center, exercises your brain, because “It is important to continuously challenge your brain with new facts and experiences,” says Guterman. “New input keeps your brain young.” Along with both kinds of exercise, a healthy diet is key to maintaining good brain health. “Nutrition is important as a means of providing adequate fuel for brain cells to generate signals,” says Guterman.

FOREVER YOUNG “Diseases that damage small blood vessels that feed the brain, like diabetes and high blood pressure, can cause small strokes that erode our ability to remember things.” Which means the same expert nutrition advice you’ve been hearing for years also applies to brain health. A vegetable-based diet low in sugar, salt, and processed foods is not only important for your waistline and your heart; managing cholesterol and blood pressure are also keys to staying mentally fit.

An aging mind, or something more Forgetting where you put your keys, or why you walked into a room, is usually a result of distraction, and not a concern. If you have a lot on your mind or simply aren’t paying attention, information may not register. But as you age, retaining information may require more focus, and it may take you a bit longer to learn new things. There are, however, some indications that it’s time to talk to a doctor. “If you or a loved one notice progressive loss of memory, or an inability to perform simple tasks, contact a health professional for evaluation,” says Guterman. You may not be able to recognize if your own memory is failing, but take note if people say you’re repeating yourself. “Learning slower as we age is normal, but once you have learned something, retention of that information is key,” says Szigeti, who posits, “Your significant other says you are going out with the Joneses for dinner tonight. You

know and like the Joneses, and you have a conversation about how you are looking forward to seeing them. If thirty minutes later, you are asking about plans for dinner and don’t recall the conversation, this is a red flag.” Memory loss and cognitive impairment have numerous causes, Guterman reminds us. “Sleeping disorders, depression, and anxiety can all lead to memory loss. Some medications cause memory loss. Cerebral atrophy, or loss of brain tissue, or shrinkage of the brain can result in both long- and short-term memory loss.” Your doctor can help determine the cause and scope of memory issues, and early intervention for diagnosed memory disorders is key to successful treatment. “New memory-stimulating medication can help mild to moderate memory disorders in many patients,” says Guterman. At the University at Buffalo’s Alzheimer’s Disease and Memory Disorders Center, new medications are being tested. “Getting involved in trials can get you access to these drugs ten years earlier than we can get them in the pharmacy,” says Szigeti. For more information on clinical trials at the UB’s Alzheimer’s Disease and Memory Disorders Center, call 716-859-3484. Part of the Center’s mission includes communicating with primary care physicians to be sure they have the proper tools to evaluate patients. To facilitate early treatment, brief cognitive screening is now mandatory at well visits for people over 65. If medical

treatment is deemed necessary, primary care physicians can treat patients directly, or patients can be referred to a specialist. “Knowing a diagnosis early can result in years of staying in your own home with a good quality of life,” notes Szigeti. There is misconception, says Szigeti, about treatments for diseases such as Alzheimer’s. “A big part of the conversation is knowing what to expect,” she maintains. “If a medication is designed to slow progression [of cognitive decline], the results might be difficult to measure in one person, because you don’t know how they would be progressing without it.” Evidence about the success of these drugs comes from clinical trials that have evaluated thousands of patients. So, treatment may be considered successful even while a patient may not feel any differently, or notice improvement. “It’s important for patients and their families to work with their doctors to achieve the maximum dosage a patient can tolerate while managing any side effects,” says Szigeti. If you have concerns about your cognitive health, or the health of someone you love, connect with your physician right away. As with many diseases and disorders, early intervention offers the best opportunity for successful treatment, and a healthy lifestyle is the best defense. Home Modification: A Solution That Makes Sense - by Robert Minicucci, President, Health System Services

WHAT IS HOME MODIFICATION? At its core, home modification is exactly what it says it is: modifying your home to make it safer, more comfortable, and more accessible for yourself or a loved one who might otherwise have to leave the familiarity of the family home for an institutional setting.

HEALTH SYSTEM SERVICES ABOUT • Over 20 years of home modification experience • Full range of home modification equipment • Expert installation by C.E.A.C. (Certified Environmental Access Consultants)

Essentially, it involves projects like widening doorways, creating safe mobility paths with handrails and grab bars, ensuring the accessibility of tubs and showers, and installing equipment like ramps, stairlifts, and other items that increase the safe livability of the home. But in another sense, home modification is freedom. It’s a way to provide the best possible care for yourself or a loved one without having to sacrifice the comfort and happiness of daily life. Furthermore, modifying the home, a process that involves fixed, one-time costs for the equipment and installation, is proven to be a more cost-effective long-term solution than institutionalization.

WHY SHOULD I WORK WITH HEALTH SYSTEM SERVICES FOR MY HOME MODIFICATION PROJECT? As with any home project, it makes sense to hire the right person for the job. You wouldn’t hire a plumber to build you a deck or an electrician to fix your toilet. So why would you want to work with someone who isn’t an expert in home modification solutions? For over 20 years, our team has helped countless people provide a safe, comfortable environment for themselves or their loved ones. Our experienced installation professionals are Certified Environmental Access Consultants (C.E.A.C.) - which means that unlike general contractors they’ve been specially trained in the process of adapting residences to safely overcome physical challenges.

HOW DO I MAKE A HOME MODIFICATION PLAN?

Ultimately, our number one goal is to ensure that every project lives up to the same standards of quality that we’ve insisted upon since our company was founded in 1996. From initial consultation to finished installation and training, our commitment to customer service is our driving force.

Facing the initial challenges of a home modification can seem daunting or overwhelming. That’s why it’s important to work with the experts at Health System Services.

Michael E. Merhige M.D. LLC:

Coronary Disease Reversal and Prevention has been serving the needs of cardiology patients in Western New York since 2000 with offices in Downtown Niagara Falls and on Sheridan Drive in Tonawanda. Dr. Merhige received his M.D. with honors @ University of Kentucky in 1980 and is Board Certified in Internal Medicine (Mayo Graduate School of Medicine, 1983) and Cardiovascular Disease (University of Texas – Houston, 1986). Dr. Merhige joined Buffalo Cardiology Associates in1989. We remain the only provider of ECP [external counter pulsation] clinical services in Western New York, offered at both locations. ECP is a non-invasive, painless procedure which promotes coronary artery collateral growth, relieving angina in patients with advanced coronary disease who prefer to avoid invasive procedures like coronary artery bypass grafting and percutaneous coronary intervention. The full suite of cardiology diagnostics is also offered including stress testing, echocardiography, nuclear cardiology and arrhythmia management services. MEM LLC emphasizes aggressive dietary, exercise and medical management to reverse and prevent coronary disease, minimizing the need for invasive coronary procedures.

Visiting Nursing Association of Western New York was awarded HomeCare Elite status for the eighth consecutive year. That ranks us as one of the top in the country. It wasn’t easy. To gain this recognition, we had to demonstrate excellence in quality outcomes, patient experience, quality consistency and more. But more than anything, it demonstrates that with VNA, you and your loved ones receive the finest care. To learn more, call us anytime at (716) 630-8000 or visit VNAwny.org.
