ATDC ANOTHER SUCCESSFUL CONFERENCE
Our 32nd annual Practical Ways to Achieve Targets in Diabetes Care (ATDC) conference occurred July 11-15, 2023, and was once again a great success. This annual course is designed to help healthcare providers caring for patients with diabetes that include endocrinologists, physicians, physician assistants, nurse practitioners, and certified diabetes educators. About 42% of the participants were providers. We hosted 427 attendees representing 40 US states and 6 countries this year. The program highlighted the importance of pre-pre-diabetes, especially for subjects with type 2 diabetes (T2D), such that early detection will lead to interventions to prevent the development of diabetes; we also discussed national, international, and pediatric guidelines for diabetes management. Since more people are using continuous glucose monitors (CGMs), a session was dedicated to discussing the international consensus on the use of CGMs for diabetes pharmaceutical clinical trials. As the percentage of people diagnosed as overweight or obese is increasing globally, a session focused on the use of newer GLP analogs vs bariatric surgery. The FDA representatives informed the audience of regulatory challenges in device approvals and inpatient diabetes device approvals.
Another plenary was focused on diabetes management in the elderly, very young (toddlers), and the role of virtual care. A new drug (teplizumab) was recently approved by the FDA for delaying the onset of type 1 diabetes (T1D) by 1-2 years. A plenary was dedicated to detecting T1D early and the subsequent management and reimbursement challenges. There were lectures focusing on newer glucagon preparations for treating hypoglycemia and adjunctive therapies’ role (like SGLT-2 inhibitors and GLP-1 analogs) for managing T1D. It appears that the prevalence of T1D during the COVID-19 pandemic increased and this, along with its immunology, was discussed in a session focused on the evolving spectrum of T1D. A Hands-On Device workshop, that included all the devices (CGMs, pumps, hybrid-closed loop systems, inhaled insulin, etc.) was very well attended and we plan to bring this session back for 2024. This is a four-day academic, CME-accredited conference and next year’s dates are July 10-13. On Saturday, August 5th, the 9th Annual Run for the Ring 5K Walk/Run & Kids Fun Run, proudly presented by Diamond Homes, took place at the Barbara Davis Center for Diabetes, nestled on the picturesque University of Colorado Anschutz Medical Campus. This year's event was masterfully hosted by the renowned Robbyn Hart from 97.3 KBCO, with Gaven Wilson debuting his rendition of the National Anthem. A remarkable 28 teams and a total of 434 runners, walkers, and strollers joined forces to support the quest for the Brass Ring - finding a cure for type 1 diabetes. The Kids Fun Run was launched with great enthusiasm by Rocky, the SuperMascot of the Denver Nuggets, who cheered on the young participants as they raced by. Featuring Otter Pops, two foam parties, hamster ball, and an exhilarating obstacle course, this year provided endless fun and excitement for the entire family. A special acknowledgment goes out to Dr. Lori Sussel and The Barbara Davis Center Research Division Team, who formed the largest team with an impressive 33 members. The awards ceremony, presented by Dexcom, celebrated first-place finishers in seven different age divisions, awarding medals and Runners Roost gift certificates. We applaud Eric Monda for being the top male finisher in the 5K and Gail Vaikutis for achieving the top female finisher title. Nathan Quilling was the top individual fundraiser, raising $1,300 single-handedly. Fundraisers like Nathan play a pivotal role in supporting The Guild of the Children's Diabetes Foundation, enabling vital contributions to research, diabetes awareness and education, assistance for families in need, continuing education scholarships, and sponsored social activities for children and their families. HOW DID YOU GET INTO DIABETES CARE? I started off going into pediatrics because I really enjoy working with children and their families. Also, my mother is a pediatrician, so I was inspired by her work. During my training, I spent a week at diabetes camp, and it was a real eye opener at the time. I remember seeing an insulin pump for the first time in my career - this was a LONG time ago! The kids and counselors I met were amazing and I discovered that diabetes and endocrine care was the right path for me to make a difference in the lives of others. DO YOU HAVE PERSONAL A TYPE 1 CONNECTION? I have some family members with type 2 diabetes, but I do not have any family members with type 1 diabetes. I would say that my involvement with diabetes community, camp, and families I have worked with at the BDC have been my connections to type 1 diabetes. Over the years, I have also developed friendships with many people living with type 1 diabetes in Colorado and from all over the U.S. HOW LONG HAVE YOU BEEN WITH THE BDC? I joined the faculty of the Barbara Davis Center in the summer 2003, so I reached the 20-year milestone this past summer. WHAT MAKES THE PEDS CLINIC AND THE PEDS TEAM AT THE BDC SO UNIQUE? When I started my career at the BDC, I learned that the community here is strong and tight knit much like a family. The clinical and research teams are committed to improving the lives of people living with diabetes and their families. It is rewarding to work alongside people who are so passionate about what they do and work so hard to make a difference in people's lives. It’s great to be able to work with such an impressive and enthusiastic team. WHAT HAS CHANGED WITH PEDIATRIC DIABETES CARE IN THE TIME YOU HAVE BEEN PRACTICING? So many things have changed. I have been most impressed with the advances in technology. We have seen continuous glucose monitors (CGM) go from bulky, wired devices storing data for review several days later to much smaller CGM systems providing real time data, alerts, and alarms. That means we can now use that CGM information to feed into the pumps so the software and algorithms can help adjust the insulin delivery 24/7. So, for instance in the middle of the night if someone is getting too much or too little insulin, the system can adjust in real time. That's a really big change that leads to the ability to get better blood sugar control, which over time will translates into lower rates of complications. I also see that CGM is much more common now and makes a big difference in how people are managing their diabetes. These are amazing advances and I think they have made a big difference in how diabetes is managed.
TELL ME ABOUT A PATIENT EXPERIENCE THAT HAS REALLY IMPACTED YOU AS A PROVIDER. I have had my own family members who have had to deal with serious medical conditions, and we relied on the clinical team to take good care of them. These experiences remind me what it is like to sit across from the provider. I try to keep this in mind and show the kind of compassion I want my own family to get when they go to the doctor. WHAT DO YOU DO FOR FUN WHEN YOU ARE NOT AT THE BDC? HOBBIES? I enjoy getting outside for hikes and running. I am also a big sports fan. I grew up watching college football, so I enjoy watching football. When I have time, I enjoy golf.
TELL ME ONE FUN FACT ABOUT YOURSELF. I was an Avalanche fan before I moved to Colorado. It is so great to be in Denver where there are so many great sports teams and venues. I also enjoy dad jokes! WHAT IS YOUR FAVORITE COLOR? Purple. WHAT’S YOUR FAVORITE FOOD OR DISH TO MAKE? I like trying many different foods, but my favorite is probably Italian. My favorite dish is Chicken Parmesan. WHAT IS YOUR PET’S NAME? Our dog’s name is Stella. She is four years old and weighs about 40 pounds. We got her from a rescue organization about a year ago and she has brought a lot of fun to our home. She loves to play fetch and go on long walks.
WHAT MOTIVATIONAL OR INSPIRATIONAL ADVICE DO YOU HAVE FOR TYPE 1S? Don’t let diabetes get in the way of what you want to accomplish in your lives. Sometimes that’s not easy, but remember you have a team supporting you and helping you use the tools available to live your life and do great things.

TELL ME ABOUT THE ADVANCEMENT OF TELEMEDICINE AT THE BDC. The BDC started our first telemedicine clinics in 2012 with two centers in Wyoming. Prior to 2012, providers were spending time traveling to different sites to get to see patients. We replaced some outreach clinics with telemedicine that allowed us to reach out to patients more efficiently. Over time, we expanded that network of partners in Colorado and Wyoming so that we could continue to reach people all over Colorado, and parts of Wyoming, where they really didn't have pediatric diabetes coverage. In 2020, the COVID pandemic made in person visits very challenging – they even stopped for a little while. We knew that we could use telemedicine as a tool to continue to connect with our patients, answer questions, and provide support and care. At that point, our team went from a subgroup of clinicians that had used telemedicine to just about everybody in clinic learning about telemedicine. We pivoted very quickly within a couple of weeks to using telemedicine throughout the clinic to do at home telemedicine, which we really hadn't done before the start of the pandemic. This took us from doing a couple hundred visits a year to thousands of visits in 2020 by telemedicine.

The Barbara Davis Center for Diabetes (BDC) has been active PREVENTION AND in designing and conducting type 1 diabetes (T1D) intervention and prevention studies for over 25 years. Our current T1D EARLY DETECTION intervention and prevention research team is spearheaded by a of experienced Investigators who oversee all aspects of our OF T YPE 1 DIABETES team clinical investigations. Drs. Andrea Steck, Peter Gottlieb, Kimber Written by Andrea Steck, MD
Simmons, Aaron Michels and Taylor Triolo serve as the Principal Investigators on our Prevention Team, and we work directly with each other, our clinical research nurses, our regulatory coordinator, our team manager, our study coordinators, and research participants to be a strong clinical research trial site. Our entire research team has clinical research experience with immunomodulatory drugs as we actively participate in both TrialNet and industry sponsored T1D intervention and prevention studies. Led by Dr. Steck, the TrialNet team at the BDC is one of the top recruiting Type 1 Diabetes TrialNet Clinical Centers.
Screening for islet autoimmunity in relatives of people with T1D has historically occurred though Type 1 Diabetes TrialNet in the United States, with the goal of enrolling islet autoantibody positive individuals into diabetes prevention trials. Screening for all four major islet autoantibodies (GAD, IA-2, insulin, ZnT8) has been available commercially since 2017 and is now available directly to consumers through programs such as T1Detect, making testing more accessible outside of a clinical research setting. With the start of our general population screening program Autoimmunity Screening for Kids led by Dr. Rewers (ASK) in Colorado, we have been at the forefront of encouraging changes to the enrollment criteria into TrialNet clinical trials, so that general population children found to have early T1D can now also enroll into TrialNet prevention trials. The paradigm for preventing and treating T1D is changing dramatically with the recent FDA approval of the first immunotherapy drug Tzield (ie the anti-CD3 monoclonal antibody teplizumab) to delay progression to Stage 3 T1D. It was the TrialNet TN10 prevention trial that showed a 2-3 year delay of progression to Stage 3 T1D in adults and children 8 years of age and older with Stage 2 T1D with 1 course of teplizumab. As of June 2023, the American Diabetes Association (ADA) guidelines report that screening for presymptomatic T1D can be done with measurement of islet autoantibodies and that referral to a specialized center for further evaluation and management should be considered when multiple islet autoantibodies are identified. The BDC prevention team conducts multiple clinical trials, including several drug infusions, in the Immunotherapeutic Infusion Center located on the 2nd floor of the BDC. Since the approval of Tzield for delaying onset of stage 3 T1D in November 2022, the BDC opened an Early Type 1 Diabetes clinic led by Dr. Simmons to identify individuals eligible for treatment. Individuals with islet autoimmunity and concern for dysglycemia are offered metabolic staging, and if confirmed at Stage 2 T1D, treatment with Tzield infusions. These 14-day infusions are also done in the BDC Immunotherapeutic Infusion Center under the supervision of experienced nurses, NPs and MDs. Our team has and will continue to work to advance our knowledge and understanding of the autoimmune pathways which lead to T1D in order to help design and implement trials to ultimately prevent T1D.

PRenatal and Obstetric Maternal Exposures and ISlet Autoantibodies in Early Life The purpose of this study is to find out more about how health and exposures during pregnancy, such as having an infection, diet and growth may impact later risk of islet autoimmunity in the child. We are also interested in finding out more about why having a father or sibling with type 1 diabetes increases risk of autoimmunity in the child more than having a mother with type 1 diabetes. You may be eligible to participate if you are a pregnant woman who: � Is between 8-24 weeks gestation with a singleton pregnancy. � Has type 1 diabetes for at least 12 months, or the father or a full sibling of the baby has type 1 diabetes. � Is 18 years of age or older. � Is willing to have blood, vaginal swabs and stool samples collected and willing to complete questionnaires about prenatal
exposures once per trimester during pregnancy and at 4-6 weeks after delivery. � Is willing to have your baby screened for genetic risk for type 1 diabetes, to complete online questionnaires and to have the child followed for measurement of islet autoimmunity according to the determined risk for type 1 diabetes, through 5-6 years of age. � The father is also asked to complete one visit during the pregnancy, but this is not mandatory

GOLF TOURNAMENT
Thank you to all who participated in and
supported our 2023 Empower Tied to a Cure Denver Golf Tournament presented by RE/MAX in honor of Ian McDonald! This year's tournament was held at Sanctuary Golf Club, the premier private course nestled in the foothills of the Rockies. The course serves as a refuge for wildlife and native plants, and provided a scenic and peaceful game of golf for the players. This amazing day of golf, food, and fun raised nearly $300,000 for the Children's Diabetes Foundation (CDF) and the Barbara Davis Center for Diabetes! We are so thankful to our event chairmen Joe Smolen, Michael McDonald, Dan Fuller, and Todd Schieck. At the Pay-It-Forward Hole, golfers heard from 10-year-old Drew Blackwelder, our CDF advocate who shared his personal story about living with type 1 for the past three years. Our celebrity long drive hole raised a recordbreaking amount of nearly $40,000 this year and featured Dan “Smackintosh” McIntosh, the world’s longest left-handed golfer who has made eight world championships. Players got to enjoy the opportunity to win a trip to Pebble Beach, two opportunities to win a car with a hole-in-one, and an incredible silent auction with unique sought-after items. I was heartbroken when I came back after COVID-19 and saw that all the toys had gone away. Scouts are willing helpers that are always looking for ways to benefit the community. Therefore, when I saw the playroom was empty, I knew I had found an opportunity to give back to the place that has provided my treatment for the last nine years. By working with the wonderful staff at Barbara Davis Center, we determined a few ways of sprucing up the playroom again. With the help of some of my fellow scouts of Troop 130 in Golden, we built a Lego table, a toy workbench and a dollhouse. I can’t wait to see how these new additions will be used by kids to make their visit a little less scary and a little more fun."
