A systematic review published by Barton et al. [4] has shown that patients with PFPS may have several disorders of the foot mechanics. These disorders include delayed timing of peak rear-foot eversion, increased rear-foot eversion at heel strike and reduced rear-foot eversion range [4].

In a recent study, these authors could show [5] a relationship between earlier rear-foot eversion and the emergence of PFPS. In another study, Barton et al. [3] could also show that increased peak rear-foot eversion was associated with increased peak internal rotation of the tibia in PFPS patients (Fig. 5).

Tratamento Estudos relatados nesta revisão suportam que o uso da técnica Tape e Brace contribui para o alinhamento da patela e redução da dor em SDPF, embora sejam estudos de curto prazo.

O uso de órtese também foi descrito como um tratamento efetivo para a diminuição de dor e melhora funcional do pé pronado na SDPF. Porém, segundo o estudo mais pesquisas deveriam ser realizadas com o intuito de entender melhor os benefícios ainda incertos.

A fisioterapia é a forma mais frequentemente estudada na SDPF. Heintjes et al. publicou uma revisão no Cochrane sobre exercícios para SDPF e registraram o efeito positivo na redução de dor. Como parte do resultado positivo foram descritos: exercícios de alongamento, agachamento, ergométricos, estáticos de quadriceps, elevação da perna, leg press, subir e descer escadas. Outros programas de exercícios também incluíram fortalecimento de abdutores de quadril. A frequência mais utilizada foram programas com duração de 6 semanas.

Os artigos relacionam Tape, Brace, Órtese para os pés como tratamentos efetivos para a diminuição de dor e melhora funcional das alterações causadas pela síndrome. Os programas de reabilitação que se utilizam de exercícios como alongamento para os isquiostibiais, fortalecimento dos flexores de quadril e abdutores também foram relatados como efetivos para o tratamento da dor.

Em suma, a SDPF é uma entidade clínica que causa dor na região anterior do joelho em pacientes com ou sem mudança patológica. O mal alinhamento da patela devido ao mal alinhamento funcional ou valgo dinâmico pode ser a principal causa. Também foi associado ao mal alinhamento o desequilíbrio de quadríceps, rigidez dos ísquios tibiais e trato iliotibial. O artigo sugere que sejam realizados estudos contendo programas de exercícios incluindo membros inferiores, quadril e tronco e mais estudos sobre os efeitos do braces e órtese de pé.

Embora descritos diversos fatores biomecânicos envolvidos na SDPF e os tratamentos existentes, fica uma questão para refletirmos. Será que o foco no tratamento deve ser o alinhamento da patela? Tratamentos segmentados podem ser eficientes nos diversos tipos de alterações funcionais que encontramos comumente nos consultórios?

Seria fundamental que futuras pesquisas pudessem correlacionar o estudo morfológico da coluna vertebral, coxofemorais, joelhos e pés com a qualidade do padrão gestual e o histórico de atividades esportivas de indivíduos com SDPF. Com o objetivo de obter uma visão mais interligada das causalidades.
