A First Visit to the Gynecologist
aking your daughter to the gynecologist for the first time can be scary for mother and daughter, but it doesn’t have to be. Whether she has heavy periods, painful periods, mood-altering PMS, wants to get on birth control, or her primary care doctor suggests it’s time to go, eventually your daughter will need to visit the gynecologist. Before you or she gets too worried, here’s what you need to know to get the most out of her first visit. The American College of Obstetricians and Gynecologists recommended that girls have their first visit between 13 and 15 years old. The main purpose is to begin a meaningful conversation that will continue for years to come. “If a young woman is planning on being sexually active it would be a good idea to have a gynecologic checkup and talk about prevention from sexually transmitted infections, contraception, and birth control,” says Joanne Hasman, certified nurse midwife at Special Beginnings Birth & Women’s Center. The sooner a provider gets to know your daughter, the more comfortable she will feel going forward. “For young girls, heavy periods and cramps are the 2 largest issues we see,” Hasman says. The American College of Obstetricians and Gynecologists reports that many girls share the same health concerns—like cramps and problems with menstrual periods; acne; weight; sex and sexuality; birth control; sexually transmitted infections; alcohol, drugs and smoking; and emotional ups and downs. They also tend to be interested in tips on making good lifestyle choices. As a parent it is essential that you take the time to ensure your daughter knows what to expect before her first visit to help foster this relationship with her provider. The first visit is basically a time to find out how to best take care of your body and learn what you can expect at visits down the road. First is the physical aspect. “We listen to the heart and lungs, check the thyroid, order bloodwork as needed and if someone has an issue we discuss it like long, heavy menstrual periods that need to be managed with medication, homeopathic remedies or herbal supplements,” says Hasman. Next up is the pelvic exam. You can explain this to your daughter beforehand, noting that the gynecologist will examine the vulva,
vagina and cervix, then perhaps perform a pap smear (doctors won’t often recommend this until age 21 unless the patient is sexually active), and check internal organs with a gloved hand. The pap smear checks for abnormal changes in the cervix that could lead to cancer. “We used to tell women to get a pap smear every year but the recommendations have changed based on age and whether they have a history of HPV infection,” says Hasman. This may be an uncomfortable conversation to have with your daughter, but the more you share with her, the better prepared she will be, and will help her form a good relationship with her doctor, and you. Here are some other tips to think about with your daughter. Get her Input • Does she want to see a male or female doctor? • Does she want to see your doctor or someone new? • Does she want you in the room with her? What will the Doctor ask? • Medical history • When did you start your last period? • Are you sexually active? • Could you be pregnant? • Your period particulars—Heavy flow? Pain? Bits of Wisdom Be honest! Your gynecologist needs to know as much about your body as you do to help you care for it properly. Don’t worry about what your vagina looks like. Gynecologists have seen it all, and don’t care what you look like down there. It may be embarrassing at first, but once you get over it, it’s great to have a good doctor in your corner to help with any problems that arise down the road. —Jamie Lober

Buying Better Sunscreens
W
e all know how important sunscreen is to our skin. Without sun protection, we risk getting burned and, down the line, getting wrinkles, spots, and of course, skin cancer. But not all sunscreen is created equal. One common myth is that a super high Sun Protection Factor (SPF) is better. But studies have shown that any SPF rating above 60 is no more effective than its counterparts with SPF 50. Another quality factor when considering which sunscreen to buy is whether it is labeled “Broad Spectrum.” According to skincancer.org, “UVA rays cause skin damage that leads to skin aging and wrinkles. The shortest wavelengths of UVA rays also contribute to sunburn. It’s important to look for the words “broad spectrum” on the label, which means the product has ingredients that can protect you from UVA as well as UVB rays.” Sunscreens have been under scrutiny lately as studies show that sunscreens fortified with Vitamin A may actually speed the development of skin cancer. In May, a study in the Journal of the American Medical Association found that participants absorbed the chemicals avobenzone, oxybenzone, octocrylene and ecamsule through their skin very quickly, resulting in higher concentrations in their blood than recommended by the FDA. Those chemicals are also harming coral reefs in our oceans. Hawaii and the City of Key West, Florida recently passed laws banning sunscreens with oxybenzone and octinoxate. This information can be overwhelming when it comes to choosing the best sunscreen for your family, but fortunately there’s an excellent resource to turn to—the Environmental Working Group’s annual guide to sunscreens—to find a product you can rely on. This year EWG researchers rated the safety and efficacy of more than 1,300 SPF products, including sunscreens, daily moisturizers and lip balms with SPF values. This year’s guide (found at ewg.org) also offers tips to help shoppers find the right option for themselves and their families.
Protecting Skin Against Winter Weather
T
he combination of windy, cold weather outside and dry heat inside can be tough on anyone’s skin—especially kids’. Here’s how you can protect your child’s delicate skin from winter weather. Outdoors, remember to always keep your child’s skin covered, even if just making a quick trip from the house to the car, but especially for those who may be outside longer, such as bus-riders. Older babies and young children should be dressed in one more layer of clothing than an adult would wear in the same conditions. Inside, protecting your child’s skin begins with the bath routine. According to the American Academy of Dermatology, daily baths or showers are not necessary for children ages 6 to 11. While a nice warm bubble bath may seem like just the ticket for dry, itchy skin Annapolis Pediatric’s Nichole Tucker, CPNP, says less is more. “I do normally recommend spacing baths in the winter months,” says Tucker. “This will allow the body time to self lubricate! Keep baths short and sweet, you can even try switching to showers. Remember pat dry vs. rubbing and start applying lotion directly after bath.” Bath water should be tepid, not too hot, as this will only increase itchiness. At bath time, which should be limited to 10 minutes, avoid the bubbles and fragrant soaps. Opt instead for non-comedogenic soaps and lotions to prevent irritation and help with lubrication. Tucker suggests following your nose when choosing bath products, “If it smells like anything, don’t use it! Just because a label says nonfragrant does not always mean it is. I tend to avoid bubble baths as these are normally always scented.” For exacerbated skin, Tucker recommends a thin layer of 1-percent hydrocortisone cream, used sparingly, not more than twice a day for a week. The cream can be layered with Aquaphor or Vaseline. She says both of these products are excellent for chapped lips, too.

The Kidz Zone is available to kids ages 4 months to 12 years, and the Ballocity indoor playground gets kids moving. t’s more important than ever to keep to a budget while your family is growing and you’re trying to save money for everything from education to health care. But with kids’ demands that everything taste great and parents’ goal that it all be healthy, it’s a tall order to fill. Grocery budgets are blown every day, but having a few great, cost-effective recipes in your file is never a bad idea, and using pantry ingredients whenever possible will keep your budget in check.

I love a hearty breakfast, and a strata fits the bill every time. Plus, you make it ahead of time. A strata is like a type of bread pudding cooked with eggs, bread, and often a bit of cheese. Don’t be afraid to make this dish your own by using the veggies and cheeses you have available in your fridge. It’s completely adaptable. I like to amp mine up with just a little bit of cheese to keep the fat in check and load it with any veggies I’ve got on hand. I named this dish after incorporating a few of the ingredients my friend Erica likes, and truthfully, just couldn’t resist the name.
This is a recipe I’ve been making since I finished culinary school in 1995. It’s my teenage son’s absolute favorite, and he’ll slurp down the whole pot if I let him. Add in a salad or rolls to round it out a bit.
Forget ground turkey, we’re using turkey thighs in this recipe, which will make our chili richer and tastier while keeping the price down. Turkey thighs are readily available in the meat section at your grocery store.
• 1 tablespoon butter • 2 medium leeks (they look like green onions on steroids), white part only, washed & sliced • 4 medium russet potatoes, peeled and cut into 1-inch cubes • 4 to 6 cups low-sodium chicken broth (bonus points if you make it yourself) • salt to taste • pepper to taste • Optional: ½ cup whole milk
• 1 cup onion, chopped • 3 garlic cloves, minced • 1 4-ounce can green chilies • 1 tablespoon chili powder • 1 ½ teaspoons dried oregano • 1 ½ teaspoons ground cumin • 1 teaspoon salt • ½ teaspoon black pepper • ¼ teaspoon cayenne pepper • 1-15 oz can yellow hominy, drained and rinsed • 1-15 ounce can black beans, drained & rinsed • 1-28 ounce can crushed tomatoes • 2-3 turkey thighs, bone-in, skin removed, and discarded • 4 cups prepared rice, preferably brown
Serves 4
• 4 bread slices, cut or torn into cubes (leftover, slightly stale bread is great for this) • ½ cup cheddar cheese, shredded • ½ cup ham, diced • ½ cup asparagus, cut into 1-inch pieces, bottom ends trimmed • 1 cup sliced mushrooms (Optional: Cook briefly in a sauté pan until just browned and liquid has been released for more flavor) • 1 cup milk • 6 large eggs • ½ teaspoon mustard, dry (or 1 teaspoon prepared mustard) • ½ teaspoon salt • ¼ teaspoon pepper • ¼ cup Parmesan cheese, shredded 1. Spray an 8-inch square pan with nonstick cooking spray. Place the bread cubes, cheddar cheese, ham, asparagus, and mushrooms into the pan and toss well to combine. 2. In a separate bowl, whisk together the milk, eggs, mustard, salt, and pepper until well combined. Pour the egg mixture over the bread mixture and cover with foil. Place covered pan in the fridge, and allow the milk mixture to soak in at least one hour or up to overnight. 3. Preheat your oven to 375 degrees. Remove foil from strata and sprinkle with Parmesan cheese. Bake, uncovered, for 35-40 minutes. Allow to stand for 10 minutes (it will deflate a bit coming out of the oven), then serve.

1. Melt butter in a 4-quart saucepan over medium heat. Add leeks to pan and sauté gently, stirring occasionally, until wilted but not browned, 6-8 minutes. Add 4 cups chicken broth to the pan, and then add potatoes. Bring to a boil, and then immediately reduce to a simmer. Cook, uncovered, for 20-30 minutes, until potatoes are tender. Puree the mixture in a blender or food processor until smooth. Return soup to the pan and, if necessary, add more chicken broth to achieve the consistency you like. Add the milk, being careful NOT to bring to a boil (this will cause the milk to separate). Season to taste with salt and pepper. (If calories aren’t a major concern, substitute halfand-half or heavy cream for the milk for extra creaminess.)

1. Using a 4- or 5-quart slow cooker, add all ingredients except for the turkey thighs into the cooker and stir to combine. Place the skinned turkey thighs into the mixture and submerge. Place the lid on the slow cooker and set on low for 6-7 hours, or high for 3-4 hours, or until the meat pulls easily from the bone. Do not remove the lid during the cooking process, no matter how delicious it smells (each time the lid is removed during cooking, it adds 20-30 minutes to the cooking cycle).Remove the thighs to a cutting board and remove the bones. Using two forks, shred the meat and return to the cooker, along with any accumulated juices, stirring well to combine. Serve chili over rice.

Getting kids to the orthodontist early can set you up for successful smiles later.

arents often receive mixed messages about the best time to seek orthodontic treatment for their child. Should you see an orthodontist when your child still has his baby teeth or wait until the permanent teeth have erupted? Should you wait until it’s obvious that your child has an issue or consult an orthodontist early, before problems are evident? The truth is there is not a universal age or time to start treatment because every child has a unique mouth that warrants an individualized treatment plan. The American Association of Orthodontics suggests that all kids be evaluated by age 7, at which time they should have reached several dental and

skeletal growth milestones. “At age 7 the permanent teeth in the front and back start to erupt and we can get an idea if there will be a bite issue such as crowding, inadequate room for teeth to grow in, overbite issues or open bite issues where teeth do not come together in front,” says Dr. Steven Siegel at Orthodontists of Maryland. Other common issues include narrow jaws, oral habits like thumb or finger sucking, speech issues and sleep issues. Just because you bring your child to an orthodontist does not mean that he will require work. “Most parents just want to know that things are okay and we can give them peace of mind and self-assurance,” says Siegel. Some
kids come in every six months or once a year to have things monitored. “One of the most common orthodontist treatments done at an early age in children, perhaps 7 to 9 years of age, is palate expansion,” says Siegal, “which is when a child has too narrow a roof of the mouth or palate and it can result in significant problems such as cross-bites or dental crowding.” While it may seem like 7 is young to visit an orthodontist, most children don’t start treatment until they are several years older. But it’s good to have an idea of what issues your child might be facing, and monitor the situation over the next few years. There is no reason to feel anxious about orthodontic treatment. Malocclusions (imperfect positioning of the teeth when the jaws are closed) are treated in a faster and easier manner than ever before due to the advances in braces. “We have metal or ceramic (clear) which are permanently fixed to the teeth for the treatment duration,” says Annapolis orthodontist Dr. Mairead O’Reilly. Metal braces are still the most common treatment of misaligned teeth due to consistency of treatment success and cost-effectiveness. Lingual braces, which adhere to the backsides of teeth, are also an option, though they are significantly more expensive than traditional braces. For older teenagers, and adults as well, Invisalign aligners have become increasingly popular because they are invisible and more comfortable to wear. “You can think of them as very thin plastic slip covers,” says Seigel, “a whole series is made and each one is designed to move the teeth a little bit.” Parents and kids are always welcome to schedule a consult where the orthodontist can look at the teeth and probably take a panoramic X-ray to see what is going on under the gums. Today’s X-ray machines give off significantly less radiation than the machines of the past, and are quite safe. You will usually leave the appointment with options to correct the problem and customized treatment goals to think about. It is important to consider the length of time required for treatment, frequency of visits as well as the cost. Getting a second opinion is always a good idea as well; it’s perfectly acceptable to compare costs and treatment options from other orthodontists in your area. Now that you know when to start your children’s orthodontics visits, what can you expect next? Well, that depends on a lot of factors. You could visit once or twice a year after an initial consult, or your child could need procedures done right away. If your child needs braces, you can factor in an average cost of $5,000–$6,000, though that average sways widely depending on the severity of your child’s planned treatment. Also varying widely will be the average time in an orthodontist’s care. A successful braces treatment can last anywhere from 12 to 36 months, with 24 months being the average, according to the American Association of Orthodontics. If you have dental insurance, your child’s treatments could be covered, though often braces are considered cosmetic and aren’t fully covered. It’s best to check with your
health and dental insurance providers first before making assumptions on coverage. It’s all costly, but that doesn’t mean it’s not worth it! Most important, do not take matters into your own hands. A do-it-yourself orthodontics trend is on the rise. “Someone may go on YouTube and see or learn from others how to move their own teeth if they have a little space between their front teeth, and that can be very harmful,” says Siegel. There are companies offering home impression kits to make molds of your teeth and other accessories that people are using to treat themselves with little or no supervision, which is dangerous. “A number of things can go wrong when people try to treat themselves,” says Siegel. When you have a trustworthy orthodontist as a partner and advocate your child will be on the road to improved oral health, improved self-esteem and a picture perfect smile.

8 simple tips to keep sickness from spreading in your house By Betsy Stein

here are many ways to keep germs from spreading in your house when someone is sick, but nothing works better than everyone washing hands properly. “It’s the magic key,” says Charlotte Wallace, a community health nurse at Anne Arundel Medical Center. “Everyone can take a breath. Hand-washing is enough. You just have to do it, and do it properly.”

Wash your hands.
Invariably, when someone in the family is sick, parents want to know where to focus their attention to combat germs and stop them from spreading. Here are some tips from Charlotte Wallace; Barbara O’Connor, infection control manager at Howard County General Hospital; and Mary Wallace, director of infection prevention at University of Maryland Baltimore Washington Medical Center.

According to Charlotte Wallace, all you need is soap, water and friction to get germs off hands. Make sure to wash for at least 15 to 20 seconds, O’Connor adds, and don’t worry about antibacterial soap.

Cough in your elbow.
Don't touch your face. If you have germs on your hands, there is no way they can get into your system unless you put your finger in your nose, mouth or eyes, the nurses agree. “Touching your eyes is a good way to catch a respiratory illness,” O’Connor says. Teach kids to keep their hands away from their face.

“If Mom or Dad is sick, think about wearing a mask,” Mary Wallace adds.
Follow cleaning product instructions. “The most important thing is to use cleaners as directed,” O’Connor says. “It’s not so much what you use, but how you use it.” Some cleaners need to be left on a surface for a specific amount of time to be effective, she points out.

Focus on high-touch areas.

This means “anything that multiple hands come in contact with,” Mary Wallace says. Clean doorknobs, toilet and sink handles, light switches, counters, remotes, key boards, phones, toys, towels and counters.

Don't share.
No one should be sharing towels, washcloths, cups, dishes or even toys. Sick and well children should be separated.

Replace individual items.
Move toothbrushes away from the toilet, and replace them every six months and/ or boil them, the nurses say. Put toys in the dishwasher if it’s safe to do so.

Stay home.
