Crack Open a Six Pack
I
would love to tell you that the four exercises on page 24 will give you the washboard abs that you have always wanted; but unfortunately it takes a lot more than that. What I can tell you is that it will help strengthen your core. Couple that with the right exercise and diet plan and you can shed excess belly fat.

A strong core is very important in everything you do, whether it be standing up or moving a heavy box. But if you’re looking to see those abs, then you will have to shed that extra weight that is covering them. This becomes the true obstacle in our busy lives of fast-food dining, hour-long commutes to and from work, eight-hour desk jobs, a plop on the couch in front of the tube in the evenings and then hours of sleep. If I just described your life, an hour of exercise a day just isn’t enough to combat belly fat. Nope, you need to find other ways to move throughout your day and monitor what you are loading your body with for fuel. Start asking yourself the following questions throughout your day: 1. Can I complete this task by myself manually? For example— • Push mow your lawn and clean your own house. Not only will you burn extra calories but you will save money, too. • Instead of emailing or calling your coworker, walk to their desk to relay information. • Walk, jog or bike to work. • Take the stairs.

• Instead of sitting in your car when you drop your kids off at practice, head out on a walk, jog or bike ride. You expect your kids to be active after school, so why not you? • Try a walking meeting with your co-workers. • Invest in a treadmill desk or standing desk. • Stand or walk during calls. • Do a quick exercise break during TV commercial breaks.

Here are some additional tips: • Start a food journal or try an app like Lose It! to help you keep track of calories in and your breakdown of protein, fat and carbs. Spend a couple of weeks until you have that “Ah-ha!” moment. • Everything in moderation. Don’t deprive yourself, but make sure that your portions are reasonable. • Set your alarm to remind yourself to move. • Take a 15-minute walk during lunch or take a class at a nearby gym. • Invest in a pedometer and/or heart rate monitor. • Focus on whole foods and less processed foods. Throw your bars away and reach for veggies, fruit or nuts. You will feel more satisfied and fuller longer. • Cook everything up on Sunday for your week so you can grab and go or just heat up a meal after a long day. • Stop rewarding yourself with food or alcohol. Instead, try new workout clothes, a massage or vacation. I know that this sounds like a lot, but don’t try it all today, this week or this month. Take one or two things and change them. Once it has become second nature, add another change. You can do it all—get moving, live healthy and be happy!

The Science of Platelet Rich Plasma (PRP) for Rejuvenating Skin
A
s we age, most of us experience a gradual weakening of our facial muscles, facial volume loss and loss of skin quality/elasticity. Natural aging can be accelerated by genetics, the shape of our face, facial expressions and other factors like a poor diet, smoking and sun exposure. Research demonstrates that we have the power to heal and regenerate tissue in all areas of our body. Cellular regeneration has tremendous benefits for our skin. For faces and hands, aging treatments like Platelet Rich Plasma (PRP) therapy uses your own platelets, rich in your own adult stem cells and growth factors, to generate new collagen and blood supply. The treatment helps to repair and regenerate damaged skin and reverse the signs of aging.
The power to heal lies within.
How does PRP treatment rejuvenate skin? Platelets are the cells in the blood that help tissue to heal and grow new cells. When Platelet Rich Plasma (PRP) is properly injected into specific areas of the skin, it promotes your own collagen to grow, regenerates tissue and naturally smooths and tightens the skin. Studies document that PRP grows new collagen when injected into the skin, and new studies show that PRP can soften sun damage and other aging skin issues. Is PRP safe for cosmetic use? Yes, PRP has been safely and effectively used for cosmetic treatments. PRP has been used in orthopedic medicine, dentistry and reconstructive surgery for over 30 years. Now PRP’s uses are expanded for use in speeding up wound healing as well as for cosmetic face and hand rejuvenation.

Can PRP injections be performed with other treatments and dermal fillers? Yes, PRP can be combined with almost every treatment or procedure to enhance results. Most of my patients combine PRP injections with surgical procedures (facelifts), injectables, laser treatments and facials. Some patients choose PRP as a stand-alone treatment to improve the overall quality, tightness and texture of their skin. How much improvement can I expect from PRP, and how long will it last? Since PRP comes from your own body, the results vary for each person. My patients have noticeable improvement in the tone, texture and overall quality of their skin. They have been impressed with the results. Some of my patients have experienced improved volume in their hands and faces, as well as a shorter recovery from facial surgery procedures.

Extra Virgin Olive Oil:
i
Avoiding the Hype and Improving Your Health
If you are like most people who have spent more than five minutes on the Food Network, you are probably aware that extra virgin olive oil (EVOO) has the ability to impact a person’s diet and boost health. However, there is also a conundrum. Because of the enormous amount of product available at grocery and specialty stores, the average consumer can quickly feel overwhelmed and wonder if they are getting the EVOO that packs the most punch. Follow these tips when searching for the best EVOO:
oil will come in many shades.
With all the options, how do you choose?
• First, make sure you are shopping in the right place and pick a specialty store over a supermarket. The reason? What you pick up at the grocery store could be rancid. After six months, olive oil begins breaking down chemically, losing flavor, its aroma and, ultimately, the antioxidants inside the polyphenols that make it healthy for you. Keep in mind that the average olive oil on the shelf at any grocery store is probably one to two years old—far from fresh—and while the majority of EVOOs can allow up to two years of quality use, by the time you pick a bottle off the shelf, it is likely past its prime.
• Consider the harvest season of the country of origin. During the winter, EVOO from Northern Hemisphere countries such as Italy, Spain or Greece will be fresh. In the summer, identify oils originating in the Southern Hemisphere, like Chile, Australia and Argentina. Focusing on the current harvest season will provide you with the freshest product.
• As you search for a specialty store, look for a retailer who stores the oil in clean, stainless steel containers in a temperature-controlled environment. The retailer should also bottle the EVOO as they sell it, and you should be allowed to taste the product that you are purchasing. If it tastes greasy, meaty, metallic, moldy or is tasteless, move on. • Consider the container the EVOO is sold to you in. Look for a bottle or container that protects against light, and make sure that you are also considering what you are
10

• In order to avoid purchasing an EVOO that has undergone chemical refinement, look for labels that only state “extra virgin.” If the label includes “light,” “extra extra virgin,” “olive pomace oil” or simply “olive oil,” keep looking.
going to be using the oil for. You should purchase a quantity that can be consumed in three to six months. If you buy too much, you will risk throwing it out once the oil begins the chemical breakdown process. • Throw any myth you have heard about the oil’s color out the window. There are no rules on what color equals quality. Good

Extra virgin olive oil is one product where you can really taste the difference. Realize that high quality oil can be used for everything: dipping bread, drizzling on pizza, in grilling, sautéing and more. Just remember the health benefits in this oil come when you do your homework prior to throwing a bottle in your shopping cart. For more information on how to select the best EVOO that will complement your health, diet and cooking needs, visit Artisano’s Oils & Spices at 1101 East 86th St, Suite B or www.ArtisanosOils.com.

Children and Diabetes
f you’ve ever met anyone with diabetes, then you already know how life-consuming this disease can be for so many. A mother can’t let her son take a cookie from the cookie jar without checking his blood sugar first. A father can’t bring home a cupcake from work for his daughter without first making sure her insulin levels are acceptable. And that is just the surface of how life-altering a life with diabetes can be. I recently watched a documentary about a girl diagnosed with Type 1 diabetes at the age of two. She’s nine now, and has a solid routine of testing her blood sugar eight times a day. When this little girl and her siblings hear the ice cream truck outside, they jump out of the house just like every other kid would; but sometimes this young girl has to leave her frozen delight in the freezer as she waits for her blood sugar to drop. Meanwhile, she watches her brothers and sisters devour their sweet treats. It is heartbreaking and unfair. Here are just a few of the recent statistics on diabetes: • Nearly 30 million children and adults in the United States have diabetes. • Another 86 million Americans have prediabetes and are at risk for developing Type 2 diabetes. • The American Diabetes Association estimates that the total national cost of diagnosed diabetes in the United States is $245 billion. So what are the differences between the two types? Type 1 Diabetes (T1D) is an autoimmune disease, in which a person’s pancreas stops producing insulin, a hormone that enables people to get energy from food. It occurs when the body’s immune system attacks and destroys the insulin-producing cells, called beta cells, located in the pancreas. While its causes are not yet entirely understood, scientists believe that both genetic factors and environmental triggers are involved. Its onset has nothing to do with diet or lifestyle. There is nothing you can do to prevent T1D, and at present, nothing you can do to get rid of it. Type 2 Diabetes, also known as insulin resistance, is when the body’s blood glucose (sugar) levels rise higher than the normal range. Initially your pancreas makes
14
ACTIVELI FEGUI DE . COM
extra insulin to make up for it, but over time it isn’t able to keep up and cannot make enough insulin to keep your blood glucose at normal levels. Type 2 diabetes is the most common form of diabetes and affects all ages. Signs to look out for in your child and others: • Frequent Urination – This will happen because the kidneys want to flush out the extra glucose in the bloodstream. The child will urinate more frequently and in larger amounts. Bed-wetting may be one of the first signs of Type 1 diabetes in a child, especially if they haven’t exhibited that behavior before. • Drinking a lot – The child will be thirsty, wanting to replenish the fluid lost from urinating so much. • Losing weight, even though the child has a healthy appetite – The body, no longer able to process glucose for energy, begins to break down muscle and stored fat to provide fuel for hungry cells. • Feeling tired all the time. This is because the body cannot properly process glucose for energy. Steps to take if you suspect diabetes: 1. If your child has any of these symptoms, please take them seriously. Diabetes can
/November 2014
be a fatal disease. Set an appointment with your child’s pediatrician as soon as possible. 2. If your child has been diagnosed, create a treatment for them. A treatment plan is a mapped out schedule consisting of blood sugar testing, meals, portioned out/ measured carbohydrates and exercise. 3. If you do not know anyone that has diabetes but want to support the research, then sign up to do a walk. Check out stepout.diabetes.org to find an event near you. My sincere desire is that this article has inspired you to look into the harsh effects of diabetes and how it can affect anyone, no matter how healthy. This November, let’s get out there on social media and create some buzz about Diabetes Awareness Month. I’d love to hear your feedback and/or answer your questions! The Use of Mesenchymal Stem Cells and Platelet Rich Plasma in the Treatment of Sports Injuries Life is a risky game.
U
sing platelet rich plasma in tendon and ligament injuries has several potential advantages, including faster recovery and possibly even a reduction in recurrence, with no adverse reactions.
M
any famous athletes have used platelet rich plasma (PRP) and/or mesenchymal stem cells to treat injuries such as knee sprains and chronic tendon injuries. Examples include Tiger Woods, Peyton Manning, Jeff Saturday, Rafael Nadal, Hines Ward and many others. These types of conditions have typically been treated with medications, physical therapy or even surgery. PRP—and more recently, mesenchymal stem cells—have allowed these athletes to return more quickly to competition. Pittsburgh Steelers’ Hines Ward and Troy Polamalu used it before winning Super Bowl XL. Presently, PRP use in tendon and ligament injuries has several potential advantages, including faster recovery and possibly even a reduction in recurrence, with no adverse reactions described. Sports injuries include injuries to the tendons, ligaments, joints and muscles. Rotator cuff injuries of the shoulder and tendon injuries of the elbow are very
16
ACTIVELI FEGUI DE . COM
common, but any tendon or ligament can be involved. Sprains are ligament injuries, and involvement of the knee and ankle are common, as are hamstring injuries in runners. All of these injuries usually require the athlete to be out of action for a prolonged time. In older, and sometimes not-soold athletes, joint problems due to osteoarthritis can limit participation. Steroid injections can help temporarily but are detrimental in the long run; so they are limited to a few a year. PRP injections have improved pain and function, and in up to 73% of patients, appeared to delay the progression of osteoarthritis, which is a progressive disease. Individuals treated with mesenchymal stem cells to the knee have had a 75% decrease in pain scores and a 75% improvement of lifestyle scores at six months after treatment. The injection of mesenchymal stem cells is the only treatment short of joint replacement for avascular necrosis, which is bone death. Mesenchymal stem cell injection is the only

treatment, medical or surgical, that has shown enough increase in growth of both articular and meniscal cartilage of the knee for the increase to be seen on MRI. Pain from the discs of the neck and back can also limit athletic performance and can be treated by stem cell therapy. There are ongoing studies that are very promising. As explained in previous issues, platelets initiate healing and are found in the blood. PRP is made by spinning the blood to concentrate the platelets and then injecting the concentrated platelets into the injured tissue. Stem cells can be derived from either the body fat or the bone marrow. Fat or adipose tissue has five hundred times the number of the type of stem cells needed to repair sports injuries, so it is preferred. The FDA will allow the use of stem cells for treatment if they are used on the same day in the same patient from which they are taken. Embryonic stem cells are not legal and are not used for sports injuries in the United States.

My gratification comes when a clients says to me, ‘Wow! I never thought I could do that!’”
A Matter of Life
If Mindy ever has any doubt as to the importance of strength training and maintaining good health, she looks to her father. At 65 years old, Mindy’s father is an Indiana golf super senior state champion and stays active with golf and weight-lifting three to four times a week. He’s also a three-time pancreatic cancer survivor. Mindy’s father believes it’s his active lifestyle and exercise routine that has helped him to overcome this brutal cancer time and time again. You may be thinking what science has proven: Now is the time to get active. If you’re new to active living, Mindy suggests starting out by setting realistic goals. “I would recommend setting a goal to do something every day. Even if you can only set aside 20 to 30 minutes one day, a small bout of exercise is better than nothing.” Mindy also reminds us that nourishing our body properly is at least as important as staying active. “Start respecting your body!” she says. “Don’t eat bad foods, drink more water and start moving! Read labels on your foods, and steer clear of long chemical-sounding words. It’s that simple—there’s not really a secret formula.” Additionally, Mindy believes that it’s important to surround yourself with like-minded people. She suggests joining a local group or club that can help you gain knowledge and set and achieve goals. “I’m involved in a community group called Fit Livin’, which also has a multisport team,” she says. “My best friends are a part of this group, and it’s open to anyone who wants to set goals and live a healthy life!” Mindy looks forward to many more years of triathlon competition. “I’d love to qualify to race in Kona again some day,” she says. And while she realizes it may not be possible to sustain a triathlon career forever, Mindy is excited about the new challenges that surely lie ahead. “I will always look for something to challenge my body and mind, and allow me to conquer new adventures.” By Matthew Hume

Make it happen Mindy says there’s not really a secret formula to getting in shape and eating right. “Try to be active everyday, read labels in the grocery store, steer clear of long chemical sounding words, and always carry a bottle of water,” she says. “If you’re looking for a jump start, join a gym, group class, or hire a fitness expert to help get things started.”

“The Youthful Triangle of Facial Beauty”
O
ver time, differing cultures and changing societal “norms” have changed the ideal of beauty. Some things, however, have remained constant. The attractive, youthful face is shaped as an inverted triangle. There is maximum volume in the upper face, with full temples and high, smooth rounded cheek bones. This is accompanied by some narrowing of the mid face, with a tapering to the chin at the bottom of the upside down triangle. Large, wide eyes, full lips and a well- defined jaw line complete the architecture of the attractive, youthful face. place for surgical procedures; however, by restoring volume the procedure can often be minimized or delayed. The fillers and volume restoring products now available allow us to restore the triangle of facial beauty with minimal downtime. The addition of two relatively new products, Sculptra and Voluma XC, allows surgeons the ability to rejuvenate the face with results that last over two years. The classes of fillers commonly used today include hyaluronic acids, L polylactic acid, calcium hydroxyapatite and fat. Described below are these various fillers. Hyaluronic acids are naturally found in the connective tissue of skin. Older skin has been shown to have significantly less hyaluronic acid than younger skin. Hyaluronic acid is effective because it binds itself to water, which makes it an ideal substance for adding volume to the skin.
What’s your definition of beauty?
T
he female face should have a soft double S curve, with the convex curve of the upper face softly blending into the concave curve of the lower face. As the aging process occurs, volume is lost in the tissue over the cheekbones, the temporal area, the nasolabial folds and chin. This loss results in a squaring of the face and a reversing of the triangle. Another result of this whole process is that without the supporting volume, the skin begins to wrinkle. There has been a shift in the way facial rejuvenation is approached by cosmetic surgeons. Treatment procedures have evolved, and it is no longer just tightening of loose skin or using fillers to treat fine lines. Those are two-dimensional solutions for a three-dimensional problem. We now know we must restore volume. There is still a

Juvederm and Restylane are two of the more common hyaluronic acids used for volume restoration in lips, nasolabial folds and marionette lines. Perlane and Juvederm Ultra Plus are slightly larger molecules and may be used in certain areas for more volume and slightly longer results. Voluma XC is the newest hyaluronic acid on the market. Because of a unique cross linking, it can be used deeper to restore volume of the upper midface with results that last up to two years. It is a product that yields significant lifting of face. Prevelle Silk and Belotero are also hyaluronic acids that are smaller molecules, well suited to fine lines around the mouth and under the eyes.

A major advantage of hyaluronic acids is that in the extremely rare event a patient isn’t happy with the results, they can be removed with an injection of hyluronidase.
L Polylactic Acid/Sculptra is another
wonderful volume restoring agent. This is a synthetic lactic acid. Lactic acid occurs naturally in our muscles after exercise. Sculptra is a product where lactic acid molecules are stuck together in a string (polymer). Sculptra is used to fill in hollows and depressions to give a volume restoration as well as lifting. Wrinkles will be improved like the re-expanding of a deflated balloon. As the Sculptra is absorbed, the body is stimulated to build its own collagen, making the results last for two years or longer.
Calcium hydroxyapatite is a substance
that contains tiny particles found naturally in human bones and teeth. Radiesse contains these particles suspended in a hydrogel; this creates a dermal filler. Radiesse is used in nasolabial folds and deep wrinkles around the nose and mouth.
Fat can also be used for volume restoration,
and we have been using fat with excellent results for over twenty years. With newer techniques, even better results using your own fat can be obtained. Long term results can be excellent due to stem cell activation. Lastly, no discussion of injectables is complete without recommending Botox or Dysport. These products are an absolute when it comes to reducing wrinkles in the forehead, glabella and crow’s feet around the eyes. In summary, with minimally invasive techniques using the injectables available, the “Triangle of Beauty” can be restored. To determine if you are a candidate and to learn which products will be best for you, contact our office for a complimentary facial evaluation.
