Un centro multidisciplinar Una de las señas de identidad de nuestro Instituto, y que más he‑ mos perseguido desde sus inicios, es el amplio abanico de subespe‑ cialidades oftalmológicas que es capaz de prestar en un mismo espacio y hacerlo, además, con las máximas garantías científicas y tecnológicas actuales.

sobre Traumatología Ocular en‑ cargado, y editado, por la Socie‑ dad Española de Oftalmología al Dr. Álvaro Fernández‑Vega, subdi‑ rector médico de nuestra entidad y uno de los mejores especialistas de retina de España, con un me‑ recido reconocimiento y prestigio internacional.

Este hecho supone una induda‑ ble ventaja comparativa cuando un accidente requiere la inter‑ vención, a menudo muy rápida, de especialistas en distintas dis‑ ciplinas, dado que ello favorece y facilita tanto la unidad de diag‑ nóstico como la de las posteriores intervenciones, lo que aumenta así las posibilidades de alcanzar el objetivo fijado y también la como‑ didad del propio paciente.

La publicación, de extrema utili‑ dad para el oftalmólogo genera‑ lista o profesional menos formado en traumatismos oculares, pone de manifiesto, entre otras mu‑ chas circunstancias, la ventaja de poder acceder en procesos com‑ plejos a clínicas multidisciplinares en las que puedan actuar a la vez cirujanos especializados en las di‑ ferentes estructuras del ojo.

Viene esta breve reflexión al hilo de la presentación del libro/guía

constante y exigente, y a la que no regateamos ningún esfuerzo, pues somos conocedores de los resultados que la misma ofrece a nuestros pacientes. De ahí la satis‑ facción con que seguimos los im‑ portantes progresos que en ese camino han obtenido ya miem‑ bros de la quinta generación of‑ talmológica familiar y de los que damos cuenta, en parte, en este nuevo número de Vega junto a un interesante artículo que aborda un apartado de creciente interés, como es la cirugía refractiva, y en el que oftalmólogos de nuestro Instituto que se han sometido a ella, reflexionan sobre la misma.

A multidisciplinary centre One of the distinguishing marks of our Institute, and one which we have sought since the beginning, is the wide range of ophthalmo‑ logical subspecialties delivered in one same space with the highest current scientific and technologi‑ cal guarantees.

This fact represents an undoub‑ ted comparative advantage when an accident requires the interven‑ tion, and oftentimes a fast one, of specialists from different disci‑ plines. It promotes and facilitates the diagnosis and the subsequent interventions, thus increasing the chances of meeting the objective and as well as increasing the pa‑ tient’s comfort.

This brief reflection follows the presentation of the book/guide on Eye Traumatology requested,

and edited, by the Spanish Socie‑ ty of Ophthalmology to Dr Álvaro Fernández‑Vega, medical deputy director of the entity and one of the best Spanish specialists in re‑ tina, duly renowned and of inter‑ national prestige.

This publication, of extreme im‑ portance for a general ophthal‑ mologist or a professional less trained in eye trauma, brings to light, amongst many other ele‑ ments, the advantage of being able to access, during complex processes, multidisciplinary prac‑ tices where the patient can be seen by surgeons specialised in the different structures of the eye.

This undoubtedly requires an effort, an important allocation of resources to a constant and de‑ manding training, one in which

we do not spare any effort, as we are aware of the results that this offers our patients. Thus the satisfaction with which we follow the important developments that the members of the family’s fifth generation of ophthalmologists have achieved, some of which are reflected in this issue of Vega to‑ gether with an interesting article addressing an issue of increasing relevance, i.e. refractive surgery; in this article, ophthalmologists of the Institute who have undergo‑ ne this intervention reflect on the experience.

Quiénes somos About us

El Instituto Oftalmológico Fernández‑Vega (IOFV) es un Hospital Monográfico de Oftalmología El apellido Fernández‑Vega está ligado a la práctica de esta espe‑ cialidad médica desde hace más de 130 años. Hoy por hoy, en el Instituto se dedican todos los esfuerzos a la prevención, diag‑ nóstico y tratamiento médico y quirúrgico de las enfermedades que comprometen la visión.

Las instalaciones del Instituto en Oviedo ocupan una superficie de unos 12.000 m2, donde traba‑ jan alrededor de 200 personas.

Cada año se atiende a más de 110.000 pacientes y se realizan 9.000 cirugías.

Para mantenerse a la cabeza en innovación y devolver así a la so‑ ciedad parte de lo recibido de sus pacientes, el IOFV ha impul‑ sado la Fundación de Investiga‑ ción Oftalmológica (FIO), donde se realiza investigación biomédi‑ ca traslacional en oftalmología y ciencias de la visión y que cuen‑ ta con la generosa colaboración de otras entidades y fundaciones comprometidas en la lucha con‑ tra la enfermedad.

Toda esta actividad genera una gran capacidad docente para

Además, el Instituto atiende a personas desfavorecidas a tra‑ vés de la Fundación Fernán‑ dez‑Vega, presente tanto en España como en misiones inter‑ nacionales, de las que son bue‑ na muestra las llevadas a cabo en los últimos meses a Perú, Li‑ beria y Camboya.

The Instituto Oftalmológico Fernández‑Vega (IOFV) is an Eye Hospital The surname Fernández‑Vega has been linked to the practice of this speciality for over 130 years.

At present, the Institute de‑ votes all its efforts to the pre‑ vention, diagnosis and medical and surgical treatment of those diseases that jeopardise vision.

Currently, the facilities of the Institute in Oviedo occupy a surface area of approximately 12,000 square metres, in which close 200 people work. Every year over 110,000 patients are seen and 9,000 surgeries are performed.

In order to stay ahead in innova‑ tion and to return to society part

of what its patients have given to the Institute, the IOFV has launched the Fundación de In‑ vestigación Oftalmológica (FIO) (Eye Research Foundation). The Foundation carries out biomedi‑ cal translational research in oph‑ thalmology and vision sciences in conjunction with the generous contribution of other entities and foundations committed to the fight against this disease.

El Dr. Álvaro Fernández‑Vega presenta el libro oficial de la SEO sobre Traumatología ocular El subdirector médico del Instituto, junto al también oftalmólogo José García Arumí, recibió el encargo de una publicación-guía de la SEO para todos aquellos doctores que tratan a pacientes con trauma severo y en el que han colaborado más de 130 expertos Cada cuatro años, la Sociedad Española de Oftal‑ mología (SEO) encarga un libro sobre un tema es‑ pecífico que pueda ser de interés para sus socios, a un grupo de oftalmólogos expertos en esa mate‑ ria. Tanto este, como a qué expertos se les encarga, se vota por los socios en la Asamblea Nacional y la mencionada publicación es la más importante de las que edita la Sociedad. Asimismo, su presentación es considerada la ponencia más destacada del Congre‑ so de la SEO. En 2014, el encargó recayó en los doctores Álva‑ ro Fernández‑Vega Sanz y José García Arumí. El li‑ bro-guía sobre “Traumatología Ocular” se presentó el pasado septiembre en el congreso de la SEO ce‑ lebrado en Granada. En su redacción colaboraron 134 expertos, tanto nacionales como extranjeros.

Como dice el Dr Ferenç Kuhn, presidente de la In‑ ternational Society of Ocular Trauma, en su prólo‑ go a esta ponencia, las lesiones traumáticas siguen siendo una carga terrible para la persona lesionada, para su familia y para la sociedad. Además, suponen un desafío importante para el sistema de atención médica que debe procurar identificar al equipo de cirujanos que tiene las mayores posibilidades de res‑ taurar la visión de un ojo traumatizado o, al menos, preservar el globo ocular y sus anejos con una apa‑ riencia normal, y decidir cómo trasladar al paciente lesionado a la entidad de tratamiento más adecuada en el menor tiempo posible.

A pesar de vivir en la era “on line”, estos libros im‑ presos no pasan de moda pues siempre están dis‑ ponibles como referencia en esos momentos proble‑ máticos en los que la información relevante - “¿qué tengo que hacer ahora?”- se necesita con urgencia.

Este libro es también un recurso importante para quienes tratan a pacientes con trauma severo y de‑ sean obtener una imagen completa y actualizada de la subespecialidad.

La obra está dividida en seis secciones y más de 50 capítulos, que abarcan todos los aspectos de la trau‑ matología del globo ocular y sus anejos, desde la historia, clasificación, epidemiología y prevención o las características psicológicas y legales del trauma ocular, hasta todas las peculiaridades relacionadas con el diagnóstico de las lesiones causadas por un traumatismo y su posible tratamiento. Dr Álvaro Fernández‑Vega Presents the Official Book of the SEO on Ocular Traumatology The medical deputy director of the Institute, together with ophthalmologist José García Arumí, received the commission for a publication-guide from SEO for all those physicians that treat patients with severe trauma; a book in which over 130 experts have collaborated Every four years, the Sociedad Española de Oftal‑ mología (SEO – Spanish Society of Ophthalmology) commissions a book on a specific topic that can be of interest to its members to a team of ophthalmo‑ logists expert in the matter. The members of the So‑ ciety, during the National Assembly, vote both the topic and the experts who will be responsible for the commission; the aforementioned publication is the most important amongst those published by the Society. Likewise, its presentation is considered the most important lecture of the SEO Meeting.

In 2014, the commission went to physicians Álvaro Fernández-Vega Sanz and José García Arumí. The guidebook on “Ocular Traumatology” was presen‑ ted last September in the meeting of the SEO that took place in Granada. 134 experts, both national and international, collaborated in its writing.

According to Dr Ferenç Kuhn, president of the In‑ ternational Society of Ocular Trauma, in his prolo‑ gue to this lecture, trauma injuries are still a terrible burden for the injured party, his family and society.

Furthermore, they entail a significant challenge to the healthcare system that must identify the team of surgeons with a better chance of restoring vision to a traumatised eye or, at least, preserve the eye‑ ball and its annexes with a normal appearance, and decide how to transport the injured patient to the most appropriate healthcare facility in the shortest amount of time possible.

Despite living in the “on line” era, this printed books are not out-dated as they are always handy as a re‑ ference in those problematic cases in which relevant information – what do I have to do now? – is urgently needed. This book is also an important asset to tho‑ se who treat patients with severe trauma and wish to have a full and updated picture of the subspecialty.

The book is divided in six sections and over 50 chap‑ ters, which comprise all aspects of ocular traumato‑ logy (the eyeball and its annexes), from its history, classification, epidemiology and prevention or the

Complex trauma is usually referred to centres such as the Institute, which have specialists in all the different areas of the eye

psychological and legal charac‑ teristics of ocular trauma to all the peculiarities related to the diagnosis of the injuries caused by a trauma an their potential treatment. One must bear in mind that it is a general ophthalmologist in an ER who first attends an ocular trauma, and it is possi‑ ble that neither the physician nor the institution are prepared to treat a complex ocular trauma. There are important chapters devoted to the protocol that this ER ophthalmologist must follow in these cases, giving priority to the injuries that can be life-threa‑ tening to the patient, and subsequently classifying the degree of urgency of the ocular trauma and whether it requires a treatment limited to initial sur‑ gery, such as a simple closure of the wound, befo‑ re being transferred to another centre where more complex surgeries can take place, or if the patient needs to undergo the whole treatment – closure of the wound and complex treatments – all at once in a centre specialised in trauma.

The complexity of a piece like this stems from the fact that an ocular trauma can affect all the structu‑ res of the eye – eyelids, tear ducts and orbit – and, at the same time, cause injuries to the eyeball and within, damage varied structures – cornea, anterior chamber and iris, crystalline, retina or optical nerve.

All this enables the participation of a large number of surgeons, expert in the different affected areas and in very different age ranges, as it varies widely to treat ocular trauma in a child than in an elderly patient. Thus, over 130 specialists in ophthalmology and in other specialties that can be involved in the treatment of ocular trauma have collaborated in the book.

For his part, Dr Jesús Merayo Rovés was responsible for the chapters of Epidemiology and Ocular Trau‑ ma Prevention together with Jesús Moreno-Arrones Quesada, and the chapter on Ocular Chemical Inju‑ ries, with Jesús Moreno Quesada, Tomás Parra, An‑ drés Fernández-Vega Cueto‑Felgueroso and Carlos Fernández-Vega Gonzalez. Furthermore, the latter, wrote the chapter on Permanent Keratoprosthesis, together with Drs María de la Paz Fideliz, Victor Charoenrook and Juan Álvarez de Toledo. Drs José Alfonso Sanchez, Belén Alfonso Bartolozzi, Carlos Lisa and Jose Ignacio Blazquez are the authors of one of the most detailed chapters of the book, de‑ voted to the surgery of the traumatic pathology of the crystalline, and Dr Mónica Fernández-Vega Sanz wrote a comprehensive chapter on the anaesthesia of ocular trauma and the drugs that must be used at different moments in the procedure.

Con una miopía de 1,5 dioptrías, el Dr. Luis FernándezVega Cueto-Felgueroso se decidió, hace ahora ocho años, a operarse mediante la técnica láser PRK. Lo mismo hizo su compañera la Dra. Belén Alfonso hace tres. Por su parte, el Dr. Miguel Naveiras, con miopía alta y astigmatismo, se sometió a la colocación de un implante con lente ICL, mientras que los Dres.

Mónica Fernández-Vega y Andrés Fernández-Vega Cueto-Felgueroso recurrieron a la técnica de femto -LASIK para corregir sus defectos de miopía.

Todos ellos son oftalmólogos, con la excepción de la doctora Mónica Fernández-Vega, anestesista, y el mejor ejemplo de que los profesionales de la vista también se operan. “Ahora me resulta más cómodo trabajar con nuestros grandes microscopios quirúrgicos, ya que las gafas no me interfieren con ellos. Además, poder hablar desde la experiencia propia me permite entender mejor y ponerme más en la piel de mis pacientes”, apunta el Dr. Naveiras.

“Siempre se me olvidaban cuando más las necesita‑ ba: para ir al cine, a clase, conducir… por no hablar de la incomodidad que suponen a la hora de hacer deporte. Para mí ha sido una liberación reducir mi dependencia de ellas”, señala.

La Dra. Belén Alfonso advierte que pasar por una operación constituye “una decisión muy personal”, si bien a veces son ellos mismos quienes recomiendan estas técnicas por cuestiones terapéuticas. “Las indicamos, por ejemplo, en casos de anisometría -diferencias de graduación entre ambos ojos que no se pueden corregir con una gafa-, cuando no se toleran las lentes de contacto o en los casos contrarios: cuando se realiza un uso abusivo de las mismas”, aclara.

El Dr. Luis Fernández-Vega Cueto-Felgueroso va en la misma línea: “Debe operarse quien lo desee. En el Instituto no sólo tenemos médicos operados, hay otros muchos profesionales de otras áreas que han decidido dar el paso, como también los hay que siguen llevando sus gafas”, señala. El especialista no duda en afirmar que, en todos los casos, se trata de buscar aquello que beneficie más a cada paciente.

“En el Instituto tenemos a diferentes oftalmólogos y profesionales de otros ámbitos operados con diferentes técnicas. Cada una es distinta, no las hay ni mejores ni peores, simplemente se trata de personalizar cada caso para adaptarnos al tipo de ojo, número de dioptrías, grosor corneal…”, explica. “La mejor prueba es que de cuatro primos oftalmólogos, mi hermano Andrés está operado con una técnica y yo con otra, mientras que mi primo Carlos no puede operarse, ya que no cumple nuestros parámetros de seguridad, y mi primo Álvaro no quiere operarse porque le gustan las gafas y no usa lentes de contacto”.

El doctor también asegura que es trascendental poseer toda la tecnología prediagnóstica para poder indicar la técnica que más le conviene al paciente y reducir al máximo las complicaciones. Y, por supuesto, disponer de las armas terapéuticas más actuales para realizar las cirugías.

El Instituto Fernández-Vega y la compañía aérea Iberia han renovado, un año más, su acuerdo de colaboración, por el que tanto los pacientes como el personal del centro se benefician de unas tarifas y condiciones especiales y que está vigente desde marzo de 2010. La firma del acuerdo se efectuó por parte del profesor Luis Fernández-Vega, director médico del IOFV, y del director de Ventas para Latinoamérica y Acuerdos Institucionales, Víctor Moneo.

Este convenio ofrece descuentos en casi todo el rango de tarifas de Iberia, tanto en vuelos nacionales como internacionales.

Desde su entrada en vigor han sido numerosos los pacientes de todos los puntos de España que se han beneficiado de estas ventajas, que pueden conseguirse contactando con la agencia Boreal Viajes de Oviedo.

National Meeting of the Sociedad Española de Farmacia Hospitalaria

The Sociedad Española de Of‑ talmología Veterinaria (SEOVET – Spanish Society of Veterinary Ophthalmology) organised, to‑ gether with the IOFV, Escaner Ve‑ terinario Asturias, Pia Technolo‑ gies and EDC Veterinary, a course on iontophoresis and crosslinking devoted to veterinary ophthal‑ mologists, conducted by the ex‑ pert in veterinary ophthalmology, Dr Frank Famose. The attendees had the opportunity of visiting the Institute’s facilities.

Professor Luis Fernández‑Ve‑ ga Sanz and Dr José F. Alfonso were present in the 29th Mee‑ ting of the Sociedad Aragone‑ sa de Oftalmología that took place in Zaragoza gathering the main Spanish ophthalmologists to address issues related to intraocular lenses. The specia‑ lists of the Institute participa‑ ted in the round table entitled “Multifocality”.

Silvia Berisa, responsable de Farmacia Hospitalaria del IOFV, asistió como ponente a la 63 edición del Congreso Nacio‑ nal de la Sociedad Española de Farmacia Hospitalaria (SEFH).

En su primera charla, revisó las toxicidades oculares producidas por fórmulas magistrales, mien‑ tras que en la segunda presentó los resultados del proyecto BIO TEAR en el que se desarrolló un tejido artificial para realizar prue‑ bas de toxicidad.

Salamanca acogió en noviem‑ bre las XXVI jornadas de la So‑ ciedad de Oftalmología Cas‑ tellano Leonesa ‘La mácula: presente y futuro’, una cita en la que participó el Dr. Álvaro Fer‑ nández-Vega, director de la Uni‑ dad de Retina y Vítreo del Ins‑ tituto. Su ponencia versó sobre casos muy complicados en los que se asocian problemas de retina con una muy mala visuali‑ zación del fondo del ojo debido a alteraciones corneales.

El Dr. José F. Alfonso, también como director, y la Dra. Lucía Ibares han sido ponentes en el curso ‘Queratocono y seg‑ mentos corneales’ organizado en noviembre por OftalTech en Madrid. Este curso forma parte de un programa de formación para oftalmólogos y residentes denominado BrainTrust y tie‑ ne como objetivo entender el diagnóstico del queratocono y su tratamiento con segmentos intraestromales.

El Dr. Álvaro Fernández‑Vega participó en noviembre en el 49º Congreso de la Sociedad Cata‑ lana de Oftalmología con una in‑ tervención sobre cirugía combi‑ nada de córnea y vitrectomía en traumatismo severo incluida en la mesa redonda “Traumatología de la retina”, tema en el que el doctor del Instituto es experto.

Salamanca hosted in November the 26th Meeting of the Socie‑ dad de Oftalmología Caste‑ llano Leonesa ‘La mácula: pre‑ sente y futuro’ (Macula: Present and Future), an appointment in which Dr Álvaro Fernández‑Ve‑ ga, head of the Unit of Retina and Vitreous of the Institute, took part. His lecture revolved around very complicated cases in which retinal issues are asso‑ ciated to a very poor visualisa‑ tion of the eye fundus due to corneal alterations.

Dr José F. Alfonso, also as di‑ rector, and Dr Lucía Ibares have been lecturers at the course ‘Queratocono y segmentos cor‑ neales’ (Keratoconus and cor‑ neal Segments) organised last November by OftalTech in Ma‑ drid. This course forms part of a training programme for oph‑ thalmologists and residents en‑ titled BrainTrust; its objective is to understand the diagnosis of keratoconus and its treatment with intrastromal segments.

Dr Álvaro Fernández‑Vega participated last November in the 49th Meeting of the Sociedad Catalana de Oftalmología with a speech on combined cornea and vitrectomy surgery in severe trauma, included in the round table “Trauma of the Retina”, an issue in which the Institute’s physician is an expert.

Curso de cirugía en segmento anterior y ocular de Alcon

11ª Jornada Catarata Más

El Aula Magna del Hospital Uni‑ versitario Fundación Jiménez Díaz, en Madrid, acogió un cur‑ so monográfico de queratocono organizado por STAAR Surgical, OCULUS Iberia y Ferrara Ophtal‑ mics en el que participó el Dr. Al‑ fonso. Durante su intervención, dio las claves para resolver, me‑ diante el empleo de lentes ICL, los problemas de refracción en pacientes con queratocono que no pueden ser tratados con láser excímero.

El Dr. Carlos Lisa expuso la experiencia del IOFV con lentes fáquicas ICL y sus indicaciones, cirugía y seguimiento en el curso de cirugía en segmento anterior y cirugía ocular organizado por el Instituto Alcon en el Hospital de León. Durante el curso se ofrecieron 32 charlas a cargo de 28 ponentes procedentes de 14 centros hospitalarios nacionales y se abordaron técnicas como cirugía corneal laminar, cirugía del limbo o el implante de segmentos intraestromales.

Las jornadas Catarata Más es‑ tán dirigidas a oftalmólogos y residentes y tienen por objetivo abordar todos los temas rela‑ cionados con la cirugía de cata‑ ratas. El Dr. José F. Alfonso y la Dra. Belén Alfonso participaron como ponentes en su undéci‑ ma edición; el primero, con tres lecciones magistrales sobre el manejo de las incisiones en la cirugía de catarata y la segun‑ da, con una charla sobre la lente epicristaliniana de colamero en pseudofaquia.

The main hall of the Hospital Universitario Fundación Jimé‑ nez Díaz, in Madrid, hosted a monographic course on kera‑ toconus organised by STAAR Surgical, OCULUS Iberia and Ferrara Ophtalmics involving the participation of Dr Alfon‑ so. During his intervention, he gave the key to solve, using ICL lenses, refraction problems in patients with keratoconus that cannot be treated with Excimer laser.

Dr Carlos Lisa presented the IOFV’s experience with ICL pha‑ kic lenses and their indications, surgery and follow-up during the course on anterior segment surgery and ocular surgery or‑ ganised by the Instituto Alcon at Hospital de León. The cour‑ se offered 32 lectures given by 28 speakers from 14 Spanish hospital centres and addressed issues such as laminar corneal surgery, limbal surgery or the implantation of intrastromal segments.

The Meeting Catarata Más is devoted to ophthalmologists and residents and aims to ad‑ dress issues related to cataract surgery. Dr José F. Alfonso and Dr Belén Alfonso participated as lecturers in its eleventh edi‑ tion; the former one with three master lectures on the manage‑ ment of incisions in cataract sur‑ gery and the latter with a lec‑ ture on collamer epi-crystalline lenses in pseudophakia.

III curso de actualización para MIR y postMIR

La 61 edición del Congreso Por‑ tugués de Oftalmología tuvo como tema principal el enve‑ jecimiento del sistema visual y como secundarios, la presbicia, el glaucoma, la degeneración macular asociada a la edad y el ojo seco. El Dr. José F. Alfonso fue uno de sus ponentes, con una intervención centrada en la queratoplastia penetrante protegida.

La reunión anual de la AOP, que en esta ocasión se celebró en París, congregó a relevantes pro‑ fesionales de todas las especiali‑ dades médicas, entre los que se encontraban los profesionales del Instituto Luis Fernández-Ve‑ ga Cueto‑Felgueroso y Michel Tazartes. El primero expuso su experiencia en los dos Fellows‑ hip que ha realizado en el ex‑ tranjero (Londres y Miami). Por su parte, el Dr. Michel Tazartes trató las inyecciones estéticas fa‑ ciales y cómo evitar problemas.

Un año más, la Sociedad Espa‑ ñola de Cirugía Ocular Implan‑ to-Refractiva (SECOIR) contó con diferentes especialistas del IOFV para impartir formación a profesionales en su III curso de actualización sobre córnea y ci‑ rugía faco-implanto-refractiva.

Así, se dieron cita en Toledo los doctores Andrés y Luis Fernán‑ dez-Vega Cueto-Felgueroso, el profesor Luis Fernández-Vega y los doctores José F. Alfonso, Be‑ lén Alfonso, Ignacio Rodríguez Uña y José Luis Urcelay.

The 61st edition of the Portuguese Ophthalmology Meeting was mainly devoted to the ageing of the visual system and subsequently to presbyopia, glaucoma, agerelated macular degeneration and dry eye. Dr José F. Alfonso was one of its lecturers, with an intervention focused on protected deep keratoplasty.

The annual meeting of the AOP, which on this occasion took pla‑ ce in Paris, gathered relevant porfessionals from all medi‑ cal specialties, amongst which were the professionals from the Instituto: Luis Fernández-Vega Cueto‑Felgueroso and Michel Tazartes. The former presen‑ ted his experience in the two fellowships he has undergone abroad (London and Miami).

On his part, Dr Michel Tazar‑ tes addressed facial aesthetic injections and how to avoid problems.

As every year, the Sociedad Española de Cirugía Ocular Implanto-Refractiva (SECOIR – Spanish Society of Implant and Refractive Ocular Surgery) included the participation of different specialists of the IOFV to train professionals in its 3rd updating course on cornea and phaco-implant and refrac‑ tive surgery. Thus, Drs Andrés and Luis Fernández-Vega Cue‑ to-Felgueroso, professor Luis Fernández-Vega and Drs José F. Alfonso, Belén Alfonso, Ig‑ nacio Rodríguez Uña and José Luis Urcelay gathered in Toledo.

“Llegué al Instituto por sugerencia de un oftalmólogo venezolano y otro francés” Valentín Vigil, asturiano de origen, viajó desde el país latinoamericano para operarse de unas pronunciadas cataratas Los últimos meses antes de operarse no podía leer el periódico. “Ahora puedo ver hasta la letra pequeña de las cajas de medicamentos… ¡sin gafas!”, cuenta satisfecho.

Valentín Vigil, asturiano afincado en Venezuela, donde vive desde hace 70 años, viajó desde el país latinoamericano hasta Oviedo, donde conserva familia, para ser intervenido de cataratas en ambos ojos.

“No sé si fue un descuido del oftalmólogo que me atendía allí o que yo lo fui dejando mucho, el caso es que cuando me hicieron los exámenes previos a la operación de cataratas me dijeron que tenía las córneas muy deterioradas y que no se atrevían a operarme porque no tienen banco de córneas y la cirugía de cataratas en mi caso podía estropearlas aún más”, explica. Por eso, decidió volver a Asturias, tierra que llevaba 16 años sin visitar, y aprovechar para acudir a consulta en el Instituto.

Así, el verano pasado le operaron de la primera catarata, en el ojo izquierdo, porque “era el que menos afectado estaba”, según detalla su mujer Nancy García.

Tras la intervención esperaron mes y medio meses para ver si el ojo recuperaba sólo, pero como no fue así, se le realizó también un trasplante de córnea endotelial tipo DMEK, en los que el IOFV es pionero. Una vez que el ojo izquierdo estuvo completamente recuperado, se hizo lo mismo con el derecho: en este caso y en vistas de lo que había sucedido con el otro ojo, cataratas y trasplante en el mismo acto quirúrgico. En una semana, empezó a recuperar la visión.

“Valentín ha sido un paciente espectacular, nos hemos cogido cariño mutuo ya que hemos tenido que trabajar mucho en su caso durante los últimos tres meses. Además, como caducaba el visado de su mujer, tuvimos que actuar contrarreloj”, señala

el Dr. Luis Fernández‑Vega Cueto‑Felgueroso. El diagnóstico de Valentín fue una Distrofia de Fuchs, que se trata de una patología del endotelio corneal (la capa más profunda de la córnea) que puede dar visión borrosa e, incluso, hacer que la córnea se descompense tras la operación de cataratas. “Hoy en día se soluciona haciendo un trasplante lamelar de la parte posterior de la córnea; es decir, solo reponemos la capa más interna por lo que los índices de rechazo son menores y la recuperación visual es especialmente rápida”, apunta el especialista.

“Estoy muy satisfecho y profundamente agradecido al Dr. Luis”, asegura Vigil. “Me pareció joven, pero quedé encantado. Daba por hecho que al estar trabajando aquí tendría una formación impecable. Tuvo una atención excelente en todo momento y el resultado es estupendo”, afirma. Este paciente se volvió a Venezuela habiendo recuperado su visión al 100% en los dos ojos y con la mente puesta en volver a revisión el verano que viene. Ahora recomendará el Instituto a todo el que quiera escucharle.

“I am not sure if it was an over‑ sight of the ophthalmologist who was seen me there or if it was I who neglected it for too long, but the fact is that when they run the tests previous to the cataract sur‑ gery I was told that my corneas were too deteriorated and that they didn’t dare operate on me because they do not have a bank of corneas and cataract surgery in my case could make them even worse,” he explains. Thus, last summer he underwent surgery for the first cataract, on the left eye, because “it was the least affected one” according to his wife Nancy García. After the intervention they waited a month and a half to see if the eye would heal on its own, but as this wasn’t the case, he also underwent a DMEK corneal en‑ dothelial transplant, in which the IOFV is pioneer. “Valentín has been a great pa‑ tient, we have become very fond of each other as due to his case we have had to talk a lot in the last three months. Furthermore, as his wife’s visa was about to expire, we had to work against the clock,” says Dr Luis Fernán‑ dez‑Vega Cueto-Felgueroso. Va‑ lentín’s diagnosis was Fuchs’ Dys‑ trophy, pathology of the corneal endothelium (the deepest layer of the cornea) that may entail blurry vision and even corneal de‑ compensation following cataract surgery. “This is currently solved with a lamellar transplant of the posterior part of the cornea; i.e. we only replace the inner layer and therefore rejection cases are fewer and visual recovery is espe‑ cially fast,” says the specialist.

“I am very happy and deeply grateful to Dr Luis,” says Vigil. “I thought he was young, but I was really delighted. I assumed that if he was working here he must be impeccably trained. He showed excellent care at all times and the result couldn’t be better,” he sta‑ tes. The patient returned to Ve‑ nezuela after recovering 100% of his sight in both eyes and thinking about his return next summer for his check-up. He will now recom‑ mend the Institute to anyone who will listen.

La retinosis pigmentaria, una silenciosa enfermedad hereditaria La retinosis pigmentaria es la causa más frecuente de degeneración hereditaria de la retina. Fue diagnosticada por primera vez a finales del siglo XIX y, si bien todavía queda mucho camino por recorrer, en las últimas décadas se ha avanzado de manera notable en el conocimiento de los diversos factores que intervienen en su aparición y desarrollo Se trata de una enfermedad re‑ lativamente habitual, presente en uno de cada 2.000 nacimien‑ tos en España, lo cual implica que existen en nuestro país más de 15.000 afectados y más de 60.000 portadores de mutacio‑ nes con potencial para causar la enfermedad.

La retinosis pigmentaria engloba a un grupo heterogéneo de en‑ fermedades genéticas que afec‑ tan, en primer lugar, a las células encargadas de la visión noctur‑ na, los bastones, por lo que los síntomas iniciales de la enferme‑ dad son la ceguera nocturna, la mala adaptación luz-oscuridad y la pérdida progresiva de campo visual que, en fases finales de la enfermedad, termina siendo tu‑ bular o en cañón de escopeta.

A medida que avanza la enfer‑ medad, también se pueden ver

afectados los conos, que son las células que se utilizan para la vi‑ sión diurna, cuando la intensidad de luz es más alta y con las que vemos en color, ya que contienen pigmentos visuales. La Fundación Fernández‑Vega atendió a más de 350 personas en el Día Mundial de la Visión La iniciativa, que se llevó a cabo en la plaza de la Escandalera de Oviedo, tiene por objetivo concienciar a la población de la importancia de realizar revisiones periódicas de la vista para detectar y prevenir enfermedades oculares graves Una consulta especial en la plaza de la Escandale‑ ra de Oviedo. En total, unas 350 personas que fueron atendidas por los doctores Ignacio Rodríguez-Uña y Belén Alfonso, ambos profesionales del Instituto.

La fundación realiza de forma periódica este tipo de actividades destinadas a concienciar a la po‑ blación de la importancia de realizarse revisiones periódicas de la vista para prevenir y detectar en‑ fermedades oculares graves. Lo ideal es acudir una vez al año. “Hay patologías asintomáticas que sólo se detectan con la exploración”, explicó la Dra. Belén Alfonso. Sin embargo, hasta un 40% de los ciudadanos que pasaron por la carpa o bien no se habían hecho nunca una revisión ocu‑ lar o, si se la hicieron, fue hace mucho tiempo.

Asimismo, durante la jornada los doctores tam‑ bién recordaron que los pequeños que abusan de las pantallas de los dispositivos digitales tie‑ nen una predisposición mayor a padecer mio‑ pía, ya que sólo desarrollan la visión cercana y no la lejana. “Si se quiere prevenir o reducir, lo que se tiene que hacer es estar al aire libre para favorecer el enfoque de lejos”, subrayó el Dr. Rodríguez-Uña.

The foundation performs this type of activities on a regular basis to raise awareness amongst the population on the importance of checking one’s eyesight to avoid and detect severe eye diseases.

Check-ups should take place, ideally, once a year.

“There are asymptomatic pathologies that can only be detected upon examination,” explained Dr Belén Alfonso. However, 40% of the citizens who gathered in the tent had either never had an eye check-up before or if they had, it was years ago.

Likewise, during the day the physicians reminded that young children who overuse digital devices have a higher predisposition to myopia, as they only develop near vision and not far vision. “If one wants to prevent or decrease this, one must be outdoors to promote far vi‑ sion focus,” stressed asymptomatic Dr Rodríguez-Uña.

En el Día Mundial de las Perso‑ nas con Discapacidad, celebra‑ do el 3 de diciembre, la Dra. Eva Villota y las optometristas Beatriz Valcárcel y Silvia García se desplazaron hasta la Asocia‑ ción Arbolar para llevar a cabo revisiones oftalmológicas a miembros de esta asociación.

“Que no tengan que despla‑ zarse a hospitales o centros es‑ pecializados ayuda a la hora de tener un trato más cercano con los pacientes”, señaló Villota.

Un equipo de la FFV integrado por la Dra. Belén Alfonso, los optometristas Juan Queiruga y Adriana Berastegui y Abraham Álvarez, de mantenimiento, se desplazó hasta la sede de Cári‑ tas Asturias en Oviedo para rea‑ lizar revisiones oftalmológicas a niños en riesgo de exclusión social. Cáritas lleva más de 70 años de lucha contra la pobreza y la FFV colabora con ella con el objetivo de mejorar la vida de los más vulnerables.

On the International Day of Peo‑ ple with Disabilities, observed on December 3, Dr Eva Villota and optometrists Beatriz Valcár‑ cel and Silvia García travelled to Asociación Arbolar to perform eye check-ups to members of this association. “The fact that they do not need to travel to hospitals or specialised centres helps to have a more personal treatment with patients,” sta‑ ted Villota.

A team of the FFV consisting of Dr Belén Alfonso, optometrists Juan Queiruga and Adriana Be‑ rastegui and Abraham Álvarez, from maintenance, travelled to the headquarters of Cáritas Asturias en Oviedo to perform eye check-ups to children at risk of social exclusion. Cáritas has been working against poverty for over 70 years and the FFV collaborates aiming to improve the lives of the most vulnerable.

Un equipo formado por los doctores Javier y Lucía Fernández‑Vega y César Sierra, así como el optometrista Javier Lozano, acompañados por la esposa del Dr. Javier, Mar Rodríguez y su hija, María, que desarrollaron una intensa labor asistencial, se trasladó por noveno año consecutivo hasta el país asiático gracias a la colaboración que mantiene la fundación con Kike Figaredo, el Prefecto Apostólico de Battambang El pasado noviembre la Funda‑ ción Fernández‑Vega se despla‑ zó hasta Camboya para realizar revisiones de la vista a diferentes colectivos de la zona. Se trata de una expedición ya habitual en la agenda de la Fundación, ya que durante los últimos nueve años ha viajado hasta el país asiático gracias al acuerdo que mantie‑ ne con el Prefecto Apostólico de Battambang, el asturiano Kike Figaredo.

Las revisiones se llevaron a cabo por el equipo formado por los doctores Javier y Lucía Fernán‑ dez‑Vega Sanz y César Sierra, co‑ laborador del IOFV, que se tras‑ ladó con este fin desde EEUU, el optometrista Javier Lozano, y acompañados por Mar Rodrí‑ guez, esposa del doctor Javier

Fernández‑Vega y su hija, María.

En total, vieron a más de 250 per‑ sonas en la propia prefectura, a las que hay que sumar el cente‑ nar de niños que atendieron en Tahen y a los trabajadores de una fábrica textil de la zona, to‑ das personas con discapacidad.

“Tienen metralla en los ojos. A algunos ya los habíamos visto en años anteriores”, comenta la Dra. Lucía Fernández‑Vega.

Además de la revisión oftalmo‑ lógica, los doctores también rea‑ lizaron pequeñas cirugías en los casos que así lo requirieron. “Por ejemplo, se operaron varios pte‑ rigium, patología muy frecuen‑ te en ese país, y se eliminaron pequeñas tumoraciones palpe‑ brales”, cuenta el Dr. Javier Fer‑ nández‑Vega. “La pena es que

a veces algunos pacientes no se presentan a sus cirugías: tienen miedo de no poder trabajar al día siguiente, porque eso les supone quedarse sin comer”, añade Mar Rodríguez.

Entre las patologías oculares de‑ tectadas en estas revisiones, se encuentran los defectos de re‑ fracción –miopía, hipermetropía, astigmatismo y presbicia–, pro‑ blemas alérgicos y de sequedad ocular y enfermedades relaciona‑ das con la abundante radiación UV del sol, como pterigium, ca‑ taratas y degeneración macular.

Por ello, una de las medidas más importantes que llevó a cabo este equipo de profesionales del Insti‑ tuto en Camboya fue la entrega de unas 300 gafas –la mayoría de ellas donadas por pacientes del Instituto–, y muy especialmente para los niños, ya que los proble‑ mas visuales no corregidos son una de las causas más importan‑ tes del fracaso escolar. Asimismo, se dispensaron todos los medica‑ mentes necesarios para las enfer‑ medades oculares detectadas y hasta 100 gafas protectoras para evitar la radiación solar.

This has become a regular expedition in the agenda of the Foundation as it has travelled to the Asian country for the last nine years thanks to the agreement it maintains with the Apostolic Pre‑ fect of Battambang, the Asturian Kike Figaredo.

The check-ups were carried out by the team comprised of doctors Javier and Lucía Fernández‑Vega Sanz and César Sierra, collabora‑ tor of the IOFV, who travelled to this avail from the USA, optome‑ trist Javier Lozano, and accompa‑ nied by Mar Rodríguez, doctor Javier Fernández‑Vega’s wife and their daughter, María. All in all, they attended over 250 indivi‑ duals at the prefecture, to which we have to add the hundred chil‑ dren they saw in Tahen and the workers of a textile factory of the area, all of them disabled. “They have shrapnel in their eyes. We had already seen some of them

their own surgery: they are afraid of not being able to work the fo‑ llowing day, as that means not ea‑ ting,” adds Mar Rodríguez.

Amongst the ocular pathologies seen in these check-ups are: re‑ fraction defects –myopia, hype‑ ropia, astigmatism and presbyo‑ pia, - allergic problems and ocular dryness issues as well as high UV sun radiation-related diseases,

such as pterygium, cataracts and macular degeneration.

Thus, one of the most important measures carried out by this team of professionals from the Institute in Cambodia was to provide them with approximately 300 eyeglas‑ ses – most of them donated by patients of the Institute. These were mainly for children, as unco‑ rrected visual problems are one of the most common causes for school failure. Likewise, all the ne‑ cessary drugs for all the eye-rela‑ ted diseases identified were given out as well as up to 100 protective eyewear to avoid sun radiation.

El Dr. Álvaro Fernández-Vega González, especialista de la Unidad de Retina del Instituto Oftalmológico Fernández-Vega (IOFV), nos da cuenta de su paso por uno de los mejores servicios de retina de Estados Unidos, el Hospital “Illinois Eye and Ear infirmary of Chicago”. Allí realizó, el pasado año, un fellowship en retina médico-quirúrgica, gracias a la beca que le concedió la Sociedad Española de Retina y Vítreo (SERV). Este programa le ha permitido, no solo adquirir el máximo nivel de superespecialización, sino también tener la ocasión de ver la forma de trabajar en otros hospitales de prestigio internacional.

Experience in Chicago “A fellowship like the one I’ve done enables the physician to work and learn from different experts in retina with many years of experience, and to incorporate the best of each to one’s own trajectory” Dr Álvaro Fernández-Vega González, specialist of the Department of Retina of the Instituto Oftal‑ mológico Fernández-Vega (IOFV), tells us about his stay in one of the best retina services of the United States, the Illinois Eye and Ear Infirmary Hospital in Chicago. Last year he undertook at the aforementioned hospital a fellowship in medical and surgical retina thanks to the grant awarded to him by the Sociedad Española de Retina y Vítreo (SERV – Spanish Society of Retina and Vitreous).

This programme has enabled him both to achie‑ ve the maximum level of super-specialisation, and the possibility of seeing how things work at other internationally renowned hospitals.

How would you assess the experience in general?

First of all, I would like to thank the Spanish Society of Retina and Vitreous for the grant they gave me last year to be able to undertake this programme in the United States.

As for my fellowship in Chicago, I believe it has been very positive. I have been able to work closely with some of the best retina specialists in the United Sta‑ ted, from whom I have learnt a lot. I have attended numerous patients with very different pathologies; I have improved my surgical abilities learning new techniques, and I have participated in scientific stu‑ dies. Furthermore, I have had the opportunity to in‑ teract with many fellow professionals, some of which have become good friends, so I believe that I have created a good professional network for potential future collaborations.

A fellowship… how does it contribute to your training?

I believe it to be a great added value to the specia‑ lised training that an ophthalmologist can receive.

During the residency one learns general ophthalmo‑ logy, as well as rotating through different subspecial‑ ties. However, a fellowship enables one to acquire a

greater level of specialisation in one part of the eye, in my case the retina. During this period one has the opportunity of working with different experts with many years of experience, seeing different surgical techniques and diverse decision-making strategies, so one can incorporate the best of each to one’s own trajectory.

I believe it is certainly a great idea that the Instituto Oftalmológico Fernández-Vega (IOFV), as a bench‑ mark centre in teaching, research and medical and surgical care, opts and gives the opportunity to young ophthalmologists to undertake this type of international programmes; these enable us to acqui‑ re a higher level of super-specialisation, as not many specialists have this experience.

Why did you decide to go to the United States?

Why not remain in Spain?

Despite the very high level in ophthalmology in Spain, I believe we are lacking in a specialisation system similar to the American one or the one present on some European countries. The State in Spain does no offer a fellowship programme to con‑ tinue one’s training in a specific area, so if you de‑ cide to complete your training, you often need to go abroad. That is why I applied to this programme in Chicago, which has some of the most prestigious American specialists in retina, such as Dr Chan, Dr Mieler or Dr Lim, to whom I thank their trust and dedication during all my stay. During my time at University of Illinois in Chicago, I had the opportu‑ nity to see and treat multiple pathologies that here can only be seen in some big national reference centres, such as ocular oncology or paediatric oph‑ thalmology. Furthermore, working in a country like the United States, and specially in Chicago, such a multicultural city, enables you to work with all kinds of patients with different diseases that are very rare in Spain and therefore difficult to see, which widens one’s knowledge.

¿Cómo valoraría la experiencia de manera global?

En primer lugar, me gustaría agradecer a la Sociedad Española de Retina y Vítreo (SERV) la beca que me otorgó el año pasado para poder cursar este programa en Estados Unidos.

En cuanto a mi estancia en Chicago, creo que ha sido muy positiva. He podido trabajar codo con codo con algunos de los mejores retinólogos de Estados Unidos, de quienes he aprendido mucho. He atendido a multitud de pacientes con muy diversas patologías; he mejorado mi habilidad quirúrgica aprendiendo nuevas técnicas, y he participado en diferentes estudios científicos. Además, he tenido la oportunidad de relacionarme con muchos compañeros de profesión, algunos de los cuales han llegado a ser buenos amigos, por lo que creo que he establecido una buena red de contactos para posibles colaboraciones en el futuro.

Realizar un fellowship, ¿qué aporta a su formación?

Creo que es un gran valor añadido a la formación especializada que puede recibir un oftalmólogo.

Durante la residencia aprendes oftalmología general, además de rotar por las diferentes subespecialidades. Sin embargo, un fellowship te sirve para adquirir un mayor nivel de especialización en una parte del ojo, en mi caso la retina. Durante este período tienes la oportunidad de trabajar con varios expertos con muchos años de experiencia, viendo diferentes técnicas quirúrgicas y distintas estrategias en la toma de decisiones, por lo que puedes incorporar lo mejor de cada uno a tu propia experiencia.

Creo que es un acierto que el Instituto Oftalmológico Fernández-Vega (IOFV), como centro referente en docencia, investigación y asistencia médicaquirúrgica, apueste y brinde la oportunidad a los oftalmólogos más jóvenes de realizar este tipo de programas internacionales que nos permiten adquirir el mayor grado de superespecialización, pues no muchos especialistas cuentan con esta experiencia.

¿Por qué decidió irse a Estados Unidos? ¿Por qué no quedarse en España?

A pesar del altísimo nivel del que goza la oftalmología en nuestro país, creo que se echa en falta un sistema de especialización como el americano o el de algunos otros países europeos, ya que, en España, el Estado no ofrece un programa de fellowship para poder seguir formándote en un área concreta, por lo que, si quieres completar tu formación, muchas veces tienes que irte al extranjero. Por eso opté a este programa que ofrecían en Chicago y que cuenta con algunos de los retinólogos más prestigiosos de EE. UU., como son el Dr. Chan, el Dr. Mieler o la Dra. Lim, a quienes agradezco su confianza y dedicación durante todo el tiempo que estuve allí. Durante mi estancia en la University of Illinos at Chicago, tuve la oportunidad de ver y tratar multitud de patología que aquí sólo se ven en algunos grandes centros de referencia nacionales, como son la oncología ocular u la oftalmología pediátrica. Además, el trabajar en un país como Estados Unidos, y en especial en Chicago, que es una ciudad tan multicultural, te permite tratar con todo tipo de pacientes con diferentes enfermedades que aquí son muy raras y difíciles de ver, lo que amplía tu conocimiento.

Sin duda el IOFV es el mejor lugar para culminar mi formación, pero haber estado en otro centro del máximo nivel me aporta nuevas ideas y formas de trabajar que pueden resultar muy enriquecedoras. ¿Por qué eligió la UIC y cómo fue el programa de admisión?

La mayoría de fellowships en Estados Unidos para extranjeros, que no han hecho la residencia en EEUU, son fellowships de investigación. Yo quería uno que además de investigación pudiera hacer clínica y cirugía. Tras informarme bien me enteré de que la UIC, donde tenían un servicio muy bueno, habían sacado una plaza de retina clínica para extranjeros, por lo que decidí contactar con el hospital y enviar mi currículum. El Dr. Chan -quien sería mi jefe- me seleccionó para una entrevista durante el Congreso de la Sociedad Americana de Oftalmología en Chicago, por lo que, aprovechando que iba a ir al congreso, me reuní con él, y finalmente me admitió para empezar allí al año siguiente. Después de infinitos trámites y papeleo, que me llevaron meses para obtener las licencias adecuadas, por fin aterricé en Chicago.

Undoubtedly, the IOFV is the best place to finish my training, but having been in another centre of the hi‑ ghest level gives me new ideas and ways of working that can turn out to be very enriching.

Why did you choose the UIC and what was the ad‑ mission programme like?

In the United States, most fellowships for foreig‑ ners who haven’t done their residency in USA are research fellowships. I wanted one that, on top of re‑ search, included clinical and surgical practice. After some thorough research, I heard that the UIC, which has a very good department, had an opening for cli‑ nical retina for foreigners, so I decided to contact the hospital and send my CV. Dr Chan, who would then be my boss, selected me for an interview during the Meeting of the American Society of Ophthalmology in Chicago, so, taking advantage of the fact that I was attending the meeting, I met him and was sub‑ sequently admitted to work there the following year.

After endless procedures and paperwork - it took me months to have all the appropriate licences - I finally landed in Chicago.

What was your daily life like?

The department had three American fellows, with whom I became very close, and I. The programme consisted of four rotations of three months each.

During this period we treated general medical and surgical retina, ocular oncology, paediatric ophthal‑ mology and retinal dystrophies, with one or two full days per week in the operating theatre. We had a very big volume of varied patients so the rotations gave me the opportunity to treat all types of patho‑ logies related with the retina. We saw from common diseases like diabetic retinopathy (much more severe and advanced cases than the ones we see in Spain, possibly due to the poor dietary and life habits), to very rare pathologies of the retina. Days were very intense because they don’t usually stop for lunch; thus we started between 7.30 or 8 am and finished at 6 pm, stopping for 5-10 minutes to eat something fast. To this we have to add the countless clinical sessions before starting the daily patient consulta‑ tions (2 or 3 times a week) and the grand rounds every Wednesday at the end of the activities, when the entire department would meet to present and discuss complex cases. Lastly, I also devoted time to research, so on top of continuing to study for my dis‑ sertation on myopia magna, I was involved in several clinical research projects alongside my colleagues. It was a very useful experience.

Would you recommend these training programmes to your colleagues?

Indeed. As I mentioned before, undertaking a fellow enables a level of specialisation that is not achieved

¿Cómo era su día a día?

En el servicio estábamos tres fellows americanos, con los que hice gran amistad, y yo. El programa constaba de cuatro rotaciones de tres meses cada una. En este período, tratamos retina médicoquirúrgica general, oncología ocular, oftalmología pediátrica y distrofias retiniana, con uno o dos días enteros de quirófano a la semana. Teníamos un gran volumen de pacientes y muy variados, por lo que durante estas rotaciones tuve la posibilidad de tratar todo tipo de patología relacionada con la retina.

Veíamos desde enfermedades comunes como la retinopatía diabética (en general casos mucho más graves y más avanzados que los que vemos en España, fruto, probablemente, de los malos hábitos de vida y alimenticios), hasta patologías muy raras de la retina. Las jornadas eran muy intensas ya que no hay costumbre de parar a comer, por lo que empezábamos entre las 7.30 u 8 de la mañana y terminábamos a las 6 de la tarde, con una parada de tan solo 5 o 10 minutos para comer algo rápido. A esto hay que añadir las innumerables sesiones clínicas antes de empezar la consulta (2o 3 a la semana) y los “grand rounds” de todos los miércoles al acabar las actividades, donde se reunía todo el servicio y presentábamos casos complejos Los Dres. para discutirlos entre todos. Por último, también pude dedicar tiempo a la investigación, por lo que además de continuar con el estudio de mi tesis sobre la miopía magna, me vi envuelto en varios trabajos de investigación clínica junto con mis compañeros.

Fue una experiencia muy provechosa. ¿Recomendaría estos programas de formación a otros compañeros?

Sin duda. Como he dicho antes, realizar un fellowship permite un grado de especialización que no se consigue con la formación estándar, además de aportar nuevas ideas y experiencias. Sin embargo, al trabajar en otros centros en el extranjero, también te das cuenta de que lo que tienes en casa es tan bueno o mejor. La Oftalmología en España, en general, y en IOFV, en particular, están al más alto nivel y cuentan con la máxima calidad. Manejamos las últimas novedades en tecnología, dominamos las técnicas médico-quirúrgicas más avanzadas y nos mantenemos actualizados acudiendo y presentando trabajos en los mejores congresos nacionales e internacionales. ¿Cuál es el siguiente paso?

De momento este año me han concedido el fellowship de la SERV (una de las pocas plazas de fellowship que existen en España y a la cual se accede por currículum), por lo que estaré trabajando en el Hospital de Bellvitge, en Barcelona, bajo la supervisión de los doctores Arias y Caminal.

Después, si todo sale bien, me gustaría volver a salir fuera, pero esta vez a Canadá. Mi idea es hacer otro fellowship quirúrgico de dos años, en el hospital de St Michael´s de Toronto, en uno de los centros más demandados por los especialistas norteamericanos para realizar cirugía de retina, antes de incorporarme definitivamente al IOFV y poder ofrecer a nuestros pacientes todo lo que he aprendido.

Para acabar, me gustaría agradecer al IOFV y a mi familia todas las facilidades que me han dado para poder llevar a cabo este fellowship, apostando siempre por la excelencia y la mejor formación.

También quería agradecerle a mi novia (pronto mujer) Sara, haberme acompañado en esta gran experiencia, mostrándome su apoyo en todo momento.

with the standard training, as well as being a sour‑ ce of new ideas and experiences. However, working in centres abroad one realises that what we have at home is as good or even better. Ophthalmology in Spain, in general, and at the IOFV specifically, are at the highest level and have the utmost quality. We have mastered state-of-the-art medical and surgical techniques and we keep ourselves updated by at‑ tending and presenting lectures at the best national and international meetings.

What is your next step?

For now I have been granted the SERV fellow (one of the few fellowship positions in Spain and one to which one can only access through CV), so I will be working at Hospital de Bellvitge, in Barcelona, under the supervision of Drs Arias and Caminal.

Afterwards, if all goes according to plan, I would like to go abroad again, but this time to Canada. My idea is to undertake a 2-year surgery fellowship at St Michael´s Hospital in Toronto, one of the centres with more requests from North American specialists to train on surgery of the retina, before definitively

going back to the IOFV and being able to offer our patients everything I have learnt.

Finally, I would like to thank the IOFV and my family for all the help they have given me so that I was able to undertake this fellowship, always betting on exce‑ llence and the best training. I also wanted to thank my girlfriend (soon-to-be wife) Sara for joining me in this great experience, showing me her support every step of the way.

Muchas gracias. He decidido hacerlo en glaucoma y cristalino. En glaucoma porque se trata de una enfermedad muy importante de diagnosticar y tratar a tiempo, pues al ser en muchos casos silenciosa,

puede llevar al paciente a la ceguera. Por eso es fundamental realizar revisiones oculares periódicas para detectarla cuanto antes. Esta patología tiene una parte clínico-quirúrgica que siempre me ha resultado muy interesante, y en la que disponemos de diversas estrategias terapéuticas, como pueden ser las gotas hipotensoras, el láser, la cirugía

Interview with Andrés Fernández‑Vega Cueto‑Felgueroso He is the second child of Professor Luis Fernández‑Vega and is part of the fifth generation of the family saga of ophthalmologists, together with his brother Dr Luis Fernández‑Vega Cueto‑Felgueroso, and his cousins Álvaro and Carlos Fernández‑Vega González and Lucía Castaño Fernández-Vega Congratulations, you already are an ophthalmolo‑ gist. Once you finish your internship, what would you like to specialise on?

Thank you. I have decided to specialise in glaucoma and crystalline. In glaucoma because an early diagnose and treatment of the disease are a very important, as it is often a silent disease that can cause blindness. It is therefore paramount to undergo regular eye check-ups to be able to detect the disease as soon as possible. This pathology has a clinical and surgical side to it that has always been very appealing to me; we have different therapeutic strategies such as hypotensive drops, laser, minimally invasive or filtration surgery, depending on the degree of the disease. Incidentally, I have always liked crystalline surgery, especially that of presbyopia and cataracts, as they solve, with surgeries that take more or less time, pathologies that limit significantly the lives of our patients.

Have you already began this process?

Indeed. I have started my super-specialisation in glaucoma and crystalline surgery through a Fellowship at Instituto Universitario Fernández‑Vega (IUFV), guided by Dr Pedro Pablo Rodríguez, Dr José Alfonso and Professor Luis Fernández‑Vega, my father. The IUFV offers training to ophthalmologists in the clinical, surgical and research fields thanks to the magnificent facilities of the Fundación de

Investigación Oftalmológica (FIO – Eye Research Foundation). Once I finish my Fellowship at IUFV and my PhD dissertation, I will continue my training abroad to be able to assess first hand how things are done outside Spain, the different diagnosis methods and surgical techniques.

I see you are already thinking of pursuing a PhD… Actually, I’ve been working on my dissertation for a few months already. It’s based in the two fields I mentioned before, glaucoma and crystalline, and we are analysing clinical and surgical data as well as basic research data. I have always taken an interest in research in the field of glaucoma, in which important advances are being made at FIO, both at the Department of Neurobiology of the Retina and the Department of Genetics of Glaucoma, on the therapeutic effects of red light and research on genetic biomarkers of the disease, respectively.

Speaking of international sojourns, what was your experience like in Colombia?

Very positive, indeed. As soon as I finished my internship in July, throughout the summer months, I rotated at Clínica del Caribe in Barranquilla, Colombia, with Dr Escaf and his team. This not only enabled me to improve my abilities in glaucoma surgery but I also learnt to perform such surgeries with the specific characteristics of the eyes of that country, where, for example, cataract surgeries are far more mature and advanced than the ones we do in Spain. It was a very fulfilling experience. I learnt different surgical techniques, acquired knowledge related to innovation in the field of ophthalmology and on how to manage a big ophthalmology centre in South America. I also made many friends and, therefore, a good professional network that will be very useful in the future. They have the latest advances for the diagnosis and treatment of different eye pathologies.

You have received the grant of the Sociedad Española de Cirugía Ocular Implanto Refractiva (SECOIR). What does it entail?

It was a privilege as it has enabled me to undergo a sojourn of two months at the Complejo Hospitalario Universitario in Orense with Dr Lorente as soon as I returned from Colombia. There, I furthered

mínimamente invasiva o la filtrante, según el grado de la enfermedad. Por otro lado, siempre me ha gustado la cirugía del cristalino, en concreto la de la presbicia y la catarata, puesto que permite solucionar, con cirugías más o menos rápidas, patologías que limitan mucho la calidad de vida de nuestros pacientes. ¿Ya ha comenzado ese proceso?

Así es. Ya he empezado mi superespecialización en glaucoma y cirugía del cristalino mediante el Máster (Fellowship) en el Instituto Universitario Fernández‑Vega (IUFV), de la mano del Dr. Pedro Pablo Rodríguez, el Dr. José Alfonso y del Profesor Luis Fernández‑Vega, mi padre. El IUFV ofrece formación a oftalmólogos no solo en el ámbito clínico y quirúrgico, sino también investigador, gracias a las magníficas instalaciones que tiene la Fundación de Investigación Oftalmológica (FIO). Al finalizar mi Máster en el IUFV y mi tesis doctoral, continuaré con mi formación en el extranjero para poder valorar en primera persona cómo se trabaja fuera de España, los diferentes métodos diagnósticos y las técnicas quirúrgicas.

Veo que está pensando ya en realizar la tesis doctoral...

De hecho llevo ya unos meses con ella. Está basada en los dos campos de los que le hablaba, el glaucoma y el cristalino, y estamos analizando tanto datos clínico-quirúrgicos como de investigación básica. Me ha llamado siempre la atención la investigación en el campo del glaucoma, en la que se están logrando importantes avances en la FIO, tanto desde la Unidad Neurobiología de la Retina como la de Genética del Glaucoma, en cuanto a los efectos terapéuticos de la luz roja e investigaciones sobre biomarcadores genéticos de la enfermedad, respectivamente.

Respecto a estancias internacionales, ¿cómo ha sido su experiencia en Colombia?

Sin duda, muy positiva. Nada más finalizar mi residencia en julio, durante los meses de verano, realicé una rotación en la Clínica del Caribe en Barranquilla, Colombia, con el Dr. Escaf y su equipo, que me permitió no solo perfeccionar mis habilidades en la cirugía de glaucoma y cataratas,

sino que aprendí a realizar dichas cirugías con las características propias de los ojos en aquél país, en el que, por ejemplo, se realizan intervenciones de cataratas mucho más maduras y avanzadas que las que solemos hacer en España. Fue una experiencia muy enriquecedora. Aprendí diferentes técnicas quirúrgicas, adquirí muchos conocimientos en cuanto a la innovación en el campo de la oftalmología y cómo se gestiona un gran centro oftalmológico en Sudamérica. Además realicé multitud de amistades y, por lo tanto, una buena red de contactos profesionales que me serán útiles en el futuro. Allí cuentan con los últimos avances para el diagnóstico y tratamiento de las diferentes patologías oculares.

Ha recibido la beca de la Sociedad Española de Cirugía Ocular Implanto Refractiva (SECOIR). ¿En qué consiste?

Para mí fue un privilegio, ya que me ha permitido realizar una estancia de dos meses en el Complejo Hospitalario Universitario de Orense con el Dr. Lorente nada más llegar de Colombia. Allí pude ampliar mis conocimientos sobre el manejo quirúrgico de los pacientes con cataratas y pseudoexfoliación, así como el glaucoma asociado a este síndrome ocular y sus posibles complicaciones. También me llevo muchos amigos de esta experiencia que no olvidaré.

Para terminar, afronta su último año como presidente de la Asociación de Jóvenes Oftalmólogos Españoles (AJOE) dentro la Sociedad Española de Oftalmología. ¿Cómo ha sido la experiencia?

Efectivamente, este es mi tercer y último año tanto como presidente de la AJOE y también como representante español de los jóvenes oftalmólogos en la Sociedad Europea de Oftalmología (SOE).

Para mí ha sido un orgullo representarlos estos años, si bien es cierto que me ha requerido mucho trabajo y esfuerzo, sobre todo la organización anual de un pequeño congreso dentro de la Sociedad Española de Oftalmología. Desde la creación de la AJOE y en especial durante los últimos años, se ha trabajado muy duro para poder organizar reuniones interesantes y adecuadas al nivel de los residentes y adjuntos jóvenes. Intentamos cambiar cada año los formatos para hacer la reunión más dinámica y un poco diferente a lo que vemos habitualmente en los congresos.

Lastly, this will be your last year as president of the Spanish Association of Young Ophthalmologists (AJOE in Spanish) within the Spanish society of Ophthalmology. What was the experience like?

Yes, it is my third and last year both as president of the AJOE and as Spanish representative of young ophthalmologists at the Sociedad Europea de Oftalmología (SOE). It has been an honour to be their representative these last few years, but it has demanded lots of effort and work, especially the annual organisation of a small conference within the Spanish Society of Ophthalmology (SEO in Spanish). Since the AJOE was created and especially during the last few years, we have worked really hard to organise interesting meetings suitable to the level of the interns and young attending physicians. We try to change the format every year to make the meeting more dynamic and a little different to what we usually see during conferences.

“La presbicia o disminución de la capacidad de acomodación del cristalino para la visión de cerca se presenta a partir de los 40 años y empieza a ser clínicamente significativa a partir de los 45. La presbicia o también llamada vista cansada es la pérdida de acomodación del cristalino debido a la falta de elasticidad del mismo, lo que reduce la capacidad de enfocar en distancias cercanas. Es muy frecuente que las personas con presbicia acaben desarrollando cataratas en edad temprana, otra de las afectaciones más comunes ligadas al avance de la edad. Ambas condicionan el día a día de quien las padece, por lo que es importante corregirlas para mejorar la calidad visual del paciente.

La vista cansada no se puede prevenir, sino que va asociada a la edad. Hoy no existe un tratamiento farmacológico para poder solucionar la vista cansada. Solo se puede corregir con gafas o lentillas, o bien con cirugía.

Sí, de hecho, se trata de una intervención muy frecuente. Es más, gracias a la aparición de las lentes trifocales es posible que el paciente solvente diversos defectos de la visión en una misma intervención.

En la extracción del cristalino a partir de una mínima incisión corneal de 2mm. El cristalino se fragmenta en pequeños trozos, normalmente con ultrasonidos o láser, y posteriormente se succiona. Cuando la zona está limpia se procede a la implantación de la lente, que se despliega en el saco capsular, prácticamente de forma automática. ¿Qué avances tecnológicos han introducido las lentes trifocales?

En los últimos años, las lentes intraoculares han evolucionado sustancialmente gracias a los avances y tecnologías desarrolladas por expertos en salud ocular. Hoy, el material con el que están fabricadas es biocompatible, de modo que pueden permanecer muchos años en los ojos del paciente. Además, la ingeniería óptica ha sido capaz de diseñar diferentes tipos de lentes según los defectos que se quieren corregir. Las lentes trifocales por ejemplo, además de corregir la visión a corta y larga distancia como

ya hacían las lentes bifocales, permiten mejorar la visión a media distancia, algo muy relacionado con el estilo de vida actual, en el que se usan de forma habitual ordenadores y otras pantallas. ¿Tras la operación es posible lograr una visión óptima?

Sí, la intervención está basada en la búsqueda de unos resultados óptimos para el paciente, la tecnología nos ofrece hoy la opción de mejorar los resultados que obtiene planificando la intervención con un altísimo nivel de precisión y detalle. Además, el postoperatorio suele ser muy corto, de unas 48 horas aproximadamente. Durante los primeros quince días es recomendable no realizar grandes esfuerzos después de pasar por el quirófano. Pasado este tiempo, se puede recuperar el ritmo de vida normal.

Trifocal Lenses to Correct Presbyopia “Presbyopia or the decrease in the ability of the crystalline to accommodate for near vision usually appears at the age of 40 and starts being clinically significant at the age of 45. Studies estimate that in a few years the number of individuals between the ages of 50 and 70 affected by the disease will grow exponentially, so advances in this area are of vital importance” Professor Luis Fernández‑Vega Sanz, medical director of the Instituto Oftalmológico Fernández‑Vega shares this reflection. We talked with him to clarify some doubts and learn about a treatment option; trifocal intraocular lenses What is presbyopia?

Also known as eyestrain, it is the loss of accommodation of the crystalline due to a lack of elasticity, thus reducing the ability to focus in near distance. It is very common for individuals with presbyopia to develop cataracts at an early age, another of the most common age-related diseases.

Both diseases condition the daily life of people who suffer them, it is thus important to correct them in order to improve the visual quality of the patient.

What are the treatment options to fight the disease?

Presbyopia cannot be prevented, as it is agerelated. There is currently no drug treatment to cure tired eyes. It can only be corrected with the use of eyeglasses or contact lenses or through surgery.

Why is surgery advisable if one has presbyopia?

First, we need to carry out a customised study of the patient. This exhaustive analysis includes specific tests that determine, depending on the characteristics of the eye, whether it can undergo surgery, as there are times in which the intervention is not recommended and it is best to wait for the onset of cataracts. Surgery must be performed at the appropriate age for each patient and, barring exceptions, never before the age of 50.

The surgery generally has a fast and painless recovery. There is the possibility of correcting other refractive defects, such as astigmatism, hyperopia or myopia in the same surgery choosing the appropriate lens for each case. Furthermore, by removing the crystalline and substituting it for a lens, one avoids any potential onset of cataracts in the future.

Is it possible to remove presbyopia in a cataract surgery?

Yes, in fact it is a very common intervention.

Furthermore, thanks to the appearance of trifocal lenses it is possible for the patient to remedy different visual defects in a single intervention.

What are trifocal intraocular lenses?

They are responsible for substituting the crystalline when it is removed in a presbyopia or cataract intervention. The state-of-the-art trifocal typology enables vision to focus in three different distances: near, intermediate and far.

What does an intervention to introduce this type of lens entail?

It consists of the removal of the crystalline through a small corneal incision of 2 mm. The crystalline is fragmented in small pieces, normally via ultrasound or laser, and these are subsequently sucked out.

When the area is clean the lens is implanted, unfolding it in the capsular bag almost automatically.

What technological advances have the trifocal lenses introduced?

Intraocular lenses have evolved significantly in the last few years thanks to the advances and technologies developed by experts in ocular health. Currently, they are made with biocompatible material, so they can stay in the patient’s eye for many years.

Furthermore, optical engineering has been able to design different types of lenses depending on the defects that you want to correct. For example, trifocal lenses do not only correct near and far vision as bifocal lenses already do, they also improve intermediate distance vision, which is closely linked to our current lifestyle where we regularly use computers and other screens.

Is it possible to achieve optimal vision following surgery?

Yes, the intervention is based on finding optimal results for the patient; technology currently offers us the option of improving the results of the surgery by planning the intervention with a very high level of accuracy and detail. Furthermore, the post-op is usually very short, of approximately 48 hours. It is advisable not to make any great efforts in the 15 days following the intervention. After this period, one can resume a normal lifestyle.

El Dr. Luis Fernández‑Vega Cueto-Felgueroso responde a las dudas de los pacientes Estoy operada de cataratas con una lente que solo me da bue‑ na visión de lejos. Mis amigas que se han operado ven bien el ordenador y leen sin gafas. Me gustaría estar como ellas. ¿Ten‑ go solución?

Mi hijo de 17 años tiene que‑ ratocono y en la óptica le dije‑ ron que tiene cuatro dioptrías de astigmatismo. He leído en internet que la única opción es el trasplante de córnea. ¿Es cierto?

Muchas gracias por su pregun‑ ta. Si cumple ciertos paráme‑ tros de seguridad que habría que estudiar -superficie ocular, mácula y glaucoma- se podría trabajar en su caso. La primera opción es cambiar su lente mo‑ nofocal por una trifocal. En este supuesto habría que ver cuándo fue operada. Si fue intervenida hace poco tiempo tenemos más opciones de que sea viable. La segunda opción sería hacer un “piggy-back” con una lente bifo‑ cal; es decir, colocar la nueva len‑ te que le dé visión cercana por encima de la lente que ya lleva.

Tenemos muchos casos de este tipo con resultados satisfactorios.

Lo primero que le puedo de‑ cir es que no se alarme. Habría que hacerle estudios a su hijo y ver cómo de avanzado es el que‑ ratocono. Es cierto que antes la opción terapéutica única era el trasplante de córnea. Sin embar‑ go, hoy tenemos otras alternati‑ vas para parar la progresión de la enfermedad o/ y retrasar al máxi‑ mo el trasplante. En el Instituto tenemos una gran experiencia con anillos intracorneales que se colocan usando un láser de fe‑ mtosegundo. Estos anillos, ade‑ más, nos ayudarán a corregir el astigmatismo que menciona. Si nos pusiéramos en el peor caso, y necesitara un trasplante, haría‑ mos uno de las partes anterio‑ res de la córnea (técnica DALK), lo que nos permitiría preservar el endotelio corneal, es decir, la capa más profunda, y reducir así la incidencia de rechazos y de posibles complicaciones.

Hola doctores. Ya les he pre‑ guntado por Facebook, pero tal vez aquí puedan aclarar más mi duda. Tengo ojo seco y he probado múltiples alternativas sin ninguna mejoría: distintos tipos de lágrimas artificiales, pomadas, corticoides, ciclospo‑ rina… ¿Hay algo más que pue‑ da hacer?

Por supuesto hay más tratamien‑ tos que se podrían intentar para mejorar sus síntomas. Por lo que leo, no ha probado el PRGF (Plasma Rico en Factores de Cre‑ cimiento), que es un colirio que se realiza con su propia sangre: se procesa primero y después se calienta. Es una lágrima artificial que ha demostrado tener unas propiedades y composición muy similares a la natural, a diferencia de las comerciales que podemos comprar en las farmacias. Periodista y escritora, pero si tuviera que elegir… ¿con qué se queda?

Afortunadamente no tengo que elegir y puedo compatibilizar ambas pasiones. Ni la literatu‑ ra ni el periodismo son trabajos propiamente dichos; más bien son vocaciones que te llaman y te llevan a seguir un camino duro, laborioso, a menudo peligroso y no demasiado bien pagado, que compensa, no obstante, cual‑ quier sinsabor. Yo me considero una persona con suerte, además de una “curranta”. ¿Siempre hay algo de periodis‑ mo en la literatura?

Siempre. Y en la novela históri‑ ca, más, porque a la narrativa de ficción, a la emoción y los senti‑ mientos hay que añadir grandes dosis de investigación rigurosa que se aprende a llevar a cabo en la escuela del periodismo. Yo cuento historias. A veces reales, otras imaginarias, pero siempre honestas, escritas desde la ver‑ dad y que persiguen llegar al co‑ razón de quien las lea y conmo‑ verlo. En ese empeño convergen la periodista y la escritora. Se tra‑ ta de divulgar, de narrar hechos,

acaecidos o ficticios, de la mejor manera posible. ¿Está contenta con la acogi‑ da de su última novela, “La Peregrina”?

Muy contenta. La Peregrina ha estado desde su publicación en la lista de los más vendidos, va ya por la cuarta edición y va a ser traducida al italiano, de mo‑ mento… Una vez más, Asturias, el origen de España, el reinado de Alfonso II el Casto y Alana de Coaña se convierten en un talismán para mí. Las aventuras de mis personajes en esa época convulsa, el viaje que propongo en la máquina del tiempo, cauti‑ van al público. Después de tanto trabajo, es una enorme alegría.

De momento es todo un poco nebuloso, pero si Dios quiere acabará viendo la luz y definién‑ dose con contornos claros, como me sucedió a mí tras pasar por las sabias manos del doctor Alfonso, en la clínica Fernández Vega.

¡Ya lo creo! Mi padre era diplomá‑ tico y por eso yo, siendo españo‑ la, fui a nacer tan lejos. Cuando la violencia terrorista me obligó a marchar de la tierra vasca donde se hundían mis raíces, Asturias me devolvió el hogar que otros me habían robado. Un hogar pre‑ cioso, verde y acogedor, con un paisaje increíble y un paisanaje aún mejor. Fue amor a primera vista. En mi segundo viaje a esta tierra maravillosa me compré un terreno donde construí mi casa, cerca de Cudillero, y hasta allá me escapo siempre que puedo.

Para mí Asturias es sinónimo de felicidad.

What issues do you find more difficult to write about?

I do not have a preference, but it is true that I find Spanish his‑ tory, and in particular its origins in the Kingdom of Asturias, very inspiring. Any topic is good if can give way to a narrative that inclu‑ des excitement, action, values and principles, love, courage, confronted passions… the ingre‑ dients that, in my opinion, make a good literary stew.

Indeed! My father was a diplomat and that is why I, even though I’m Spanish, was born so far away.

When terrorist violence made me leave the Basque land where I was setting roots, Asturias gave me back a home that others had taken away from me. A beauti‑ ful home, green and welcoming,

with an amazing landscape and even better people. It was love at first sight. During my second trip to this wonderful land I bought a plot where I built my house, close to Cudillero, and I scape there as often as I can. For me, Asturias is synonym of happiness.

How did you learn about the Fernández‑Vega practice in Oviedo?

It was recommended to me at ABC, where I worked for many years, when I first though of un‑ dergoing surgery for my myopia in the early 90s. My co-workers who wrote about health agreed that its professionals were by far the best, as I could ascertain myself after visiting them. My colleague Cristina López Schli‑ chting and I underwent surgery at the same time, and we were both very happy with the results.

A long time afterwards, tired of not having a good near vision, I put myself again in their hands to change my crystalline and I was once again perfect. I currently read and write perfectly without eyeglasses thanks to their exper‑ tise. A debt of gratitude that I will never be able to repay.

Why do you entrust them with your eye healthcare?

Because they have proven that they are not only great professio‑ nals, but also great individuals.

I first heard of her when she was just a kid, but not any kid. She was Zoey Burtlett, the youngest daughter of the President of the USA in the first TV series that I was ever hooked to, the indispensable The West Wing, where Aaron Sorkin was able to make us believe that those who swapped those brilliant and inspired dialogues were ordinary human beings.

Zoey/Elizabeth was a free spirit, she would fall in love with whom (in principle) she shouldn’t and in the fourth season she had to… but I do not want to spoil the plot. I will only say that her role, when she was still a teenager, already had something of what was to come, a woman who doesn’t fit the mould that others have created for her, in the line of Peggy Olson in Mad Men or June in The Handmaid’s Tale, the characters that have turned her into one of the most currently coveted actresses.

Moss is a modern actress, starting with her unequivocal approaches to life: in the next to last red carpet she stepped on, that of the Emmy Awards, the sole of her shoes made very clear what she thinks of the corrupted environment of Hollywood, full of power abuse, starting with Harvey

Weinstein and his sex scandals. Moss belongs by right to the wave #MeToo that defends feminine dignity and condemns out loud male indignity. Her girl next-door appearance (next door in California, mind you) helps her in this endeavour, a real look of normality, even if her eternal smile leaves a trace of enigma, more relevant when we know that she follows the Church of Scientology. In any case, her modernity is also reflected in this accessible, so unsophisticated appearance. Don’t forget that we have seen Elisabeth grow fro our couches: we didn’t go out looking for her she came to our houses. She is not a big screen diva, those who by definition are unattainable, but a TV star. She is not a large format actress, but one of the small screens. Elisabeth Moss owes her current fame to successful TV series, and series, contrary to films, do not occupy two hours of our lives, they accompany us for days, weeks, months… becoming part of our daily lives. Times have changed for the industry and she who was Peggy and is now June represents this new time. The Olympus of actresses and actors is now domestic and portable. The eternity of a star is now measured in seasons. That od Elisabeth Moss keeps shining.
