La mayor seguridad para la mejor oftalmología De manera progresiva avanzamos hacia la recuperación de lo que hasta hace muy pocos meses considerábamos normal y lo hacemos tanto con el dolor por lo sufrido, como con la esperanza de que los sólidos pasos que damos afiancen el marco de seguridad deseable.

En el Instituto nuestra prioridad ha sido, y es, preservar en todo momento la salud de nuestros pacientes y asistirles en el ámbito oftalmológico en todo cuanto estimen oportuno.

Dos principios que han guiado también nuestra actividad durante lo más duro de la pandemia, periodo durante el que no hemos dejado de atender aquellos casos que no podían

esperar, así como las urgencias sobrevenidas.

El Instituto es, por su propia naturaleza, un espacio sanitariamente seguro, pero aún así hemos reforzado los protocolos de actuación en este campo y realizado inversiones en equipamientos con el mismo objetivo, pues nuestra intención es siempre disponer de un grado adicional de seguridad al que las nomas sanitarias establecen en cada momento.

Y creemos haberlo hecho sin menoscabo de la comodidad del paciente y , por supuesto, de la eficiencia en los diagnósticos y tratamientos necesarios, aspectos en los que todos nuestros profesionales juegan un papel de especial relevancia.

De ello damos cuenta en este nuevo numero de Vega junto a las secciones habituales, que reflejan el trabajo para contribuir a la normalidad que tanto deseamos en todos los ordenes.

Quisiera, para finalizar, desde el emocionado recuerdo a quienes han sido victimas de esta cruel pandemia, trasladar la mayor gratitud a todos nuestros pacientes por su renovada confianza, pues este hecho nos estimula para continuar mejorando las prestaciones médicas que les ofrecemos.

The best safety for the best ophthalmology We are progressively advancing to recover what, until a few months ago, we considered normal. We do so both with the pain of all we have suffered and with the hope that the sound steps that we are taking reinforce the desirable safety framework. At the Institute our priority has always been, and continues to be, to preserve our patients’ health and help them in all they see fit within the field of ophthalmology. These two principles have also guided our activity during the worst of the pandemic, a time in which we have not stopped attending those cases that could not wait, as well as any sudden emergencies.

The Institute is, by its very nature, a safe healthy environment, but we have still reinforced our standard operating procedures in this area as well as invested in equipment with the same objective, as our aim is to always have an additional level of safety to that established by health regulations at all times.

We believe we have achieved this without prejudice to the patients’ comfort and, of course, to the efficiency of necessary diagnosis and treatments, aspects in which all our professionals play a especially significant role.

Quiénes somos About us El Instituto Oftalmológico Fernández‑Vega (IOFV) es un Hospital Monográfico de Oftalmología El apellido Fernández-Vega está ligado a la práctica de esta especialidad médica desde hace más de 130 años. Hoy por hoy, en el Instituto se dedican todos los esfuerzos a la prevención, diagnóstico y tratamiento médico y quirúrgico de las enfermedades que comprometen la visión. Las instalaciones del Instituto en Oviedo ocupan una superficie de unos 12.000 metros cuadrados, donde trabajan alrededor de 200 personas. Cada año se atiende a más de 110.000 pacientes y se realizan 9.000 cirugías.

Para mantenerse a la cabeza en innovación y devolver así a la sociedad parte de lo recibido de sus pacientes, el IOFV ha impulsado la Fundación de Investigación Oftalmológica (FIO), donde se realiza investigación biomédica traslacional en oftalmología y ciencias de la visión y que cuenta con la generosa colaboración de otras entidades y fundaciones comprometidas en la lucha contra la enfermedad.

Toda esta actividad genera una gran capacidad docente para

profesionales de la salud dedicados a la visión (oftalmólogos, ópticos-optometristas, enfermeros y estudiantes de Medicina) que se materializa a través de diversos programas de pre y posgrado en colaboración con la Universidad de Oviedo, práctica que ha desembocado en la calificación de la entidad como Instituto Universitario.

Además, el Instituto atiende a personas desfavorecidas a través de la Fundación Fernández-Vega, presente tanto en España como en misiones internacionales, de las que son buena muestra las llevadas a cabo en los últimos meses a Perú, Liberia y Camboya.

The Instituto Oftalmológico Fernández‑Vega (IOFV) is an Eye Hospital The surname Fernández-Vega has been linked to the practice of this speciality for over 130 years.

At present, the Institute devotes all its efforts to the prevention, diagnosis and medical and surgical treatment of those diseases that jeopardise vision. Currently, the facilities of the Institute in Oviedo occupy a surface area of approximately 12 000 square

metres, in which close 200 people work. Every year over 100 000 patients are seen and 8 000 surgeries are performed.

In order to stay ahead in innovation and to return to society part of what its patients have given to the Institute, the IOFV has launched the Fundación de Investigación Oftalmológica (FIO) (Eye Research Foundation). The Foundation carries out biomedical translational research in ophthalmology and vision sciences in conjunction with the generous contribution of other entities and foundations committed to the fight against this disease.

All this activity generates a great teaching capacity for health professionals devoted to vision (ophthalmologists, opticians-optometrists, nurses, students of Medicine) that materialises itself through different pre and post graduate programmes in collaboration with Universidad de Oviedo, which has led the entity to be classified as a University Institute.

Furthermore, the Institute cares for disadvantaged people through the Fundación Fernández-Vega, present in Spain as well as in international missions, such as the ones carried out in the past few months in Peru, Liberia and Cambodia.

El Instituto Oftalmológico Fernández-Vega (IOFV), en su esfuerzo constante por ofrecer una atención excelente, ha introducido importantes mejoras para garantizar la seguridad de pacientes y profesionales en la actual situación, aunque estos principios han sido prioritarios desde su origen.

Las medidas adicionales están centradas, sobre todo, en el paciente. Por un lado, se han establecido nuevos protocolos para las visitas clínicas y quirúrgicas que minimizan los contactos. También se han incorporado varios puntos de higiene de manos, desinfección y protección personal y, además, tanto el equipo médico como de administración centran sus esfuerzos en favorecer la seguridad en todo momento.

Antes de acceder a la clínica, los pacientes se someten a un breve triaje para descartar que presenten síntomas de coronavirus.

El doctor Luis Fernandez-Vega Cueto-Felgueroso, oftalmólogo especialista en córnea y cristalino, ha participado de forma muy activa en el diseño de estas medidas adicionales, y sintetiza algunas de las más relevantes, al tiempo que afirma: “añadidas a las que ya se encontraban en vigor, definen un espacio de altísima seguridad para pacientes y profesionales, quienes pueden tener la total certeza de que no se va a regatear ningún esfuerzo en este sentido, ni mucho menos bajar la guardia en su aplicación”.

Pasos previos al acceso a la clínica Se ha establecido un nuevo protocolo de acceso para conocer el estado general de salud de quienes acuden a consulta o a una cirugía. Se trata de un breve chequeo, en el que el equipo médico también toma la temperatura, de manera que se descarta la posibilidad de que algún paciente acuda a su cita con síntomas de coronavirus.

Dos puntos de acceso, una misma salida Una de las principales novedades es la disposición de boxes individuales para los pacientes que acuden con motivo de una cirugía. Tras acceder por la puerta específica para este servicio, el personal asigna a cada paciente un box en el que se prepara para la operación. Una vez finalizada esta, accede a la zona de vestuarios y sale de la clínica por la puerta de recepción de cirugías.

El recorrido es diferente para los pacientes que acuden a consulta. El acceso se realiza por la puerta de esta área pero, tras su encuentro con el doctor, la salida está prevista también por los accesos de cirugía.

De esta manera, los pacientes que entran no coinciden en ningún momento con los que salen de la clínica y se minimiza así el riesgo ya de por si muy pequeño, dados los controles previos, de contagio.

The Best Ophthalmology, With The Utmost Safety The Instituto Fernández-Vega Introduces New Hygiene And Prevention Measures The Institute in its constant commitment to offer an excellent care, it has introduced signifi- cant improvements to guarantee the safety of both patients and professionals in the current situation, even though these principles have been a priority from the outset.

Additional measures are mainly focused on patients.

On the one hand, new protocols have been established for clinical and surgical visits minimising contact. The Institute has incorporated several hand hygiene, disinfection and personal protection locations and both the medical and administrative staff are focused on fostering safety at all times. Dr Luis Fernandez-Vega Cueto Felgueroso, ophthalmologist specialist in cornea and crystalline has participated very actively in the design of these additional measures. He summarises for Vega some of the most relevant ones while he asserts that “added to those we already had, these establish a highly safe location for patients and staff, who can rest assured that no effort will be spared to this avail, nor much less will we let down our guard”.

Steps before coming into the clinical practice A new access protocol has been established to know the overall health of those who come for a consultation or surgery. This consists of a brief check-up in which the medical team checks the temperature of the patient to rule out the possibility of anyone patient coming to their appointment with symptoms of coronavirus.

tes y también de los profesionales médicos. La limpieza es pormenorizada, con una solución desinfectante, tanto de los aparatos y el material empleado por cada doctor como de los sillones y espacios utilizados por el paciente.

Se ha dispuesto también una alfombra desinfectante para facilitar la higienización del calzado y puesto a disposición de los pacientes mascarillas quirúrgicas y gel hidroalcoholico de manos.

Además de todas las medidas de seguridad adicionales y los protocolos específicos para garantizar la seguridad de pacientes y profesionales, al finalizar cada jornada las zonas comunes son sometidas a un proceso de limpieza y desinfección en profundidad.

Una de las principales novedades es la instalación de barreras físicas en distintos puntos de la clínica. En recepción, una pantalla de metacrilato cubre todo el mostrador. Para las salas de espera se ha diseñado un sistema específico que evita el contacto directo entre pacientes. Para garantizar la higiene de estas salas, el equipo de desinfección procede a limpiar estos espacios tras cada uso.

Tras aplicar una solución viricida a suelos y superficies, el equipo de limpieza efectúa la desinfección de todos los elementos de contacto, como son los pomos, pasamanos, apoyabrazos, teléfonos o interruptores. Como medida adicional, el IOFV ha incorporado también la limpieza y desinfección diaria de aceras y accesos exteriores.

The consulting room, a safe environment The rooms in which the physicians receive and examine the patients are cleaned and disinfected after every consultation to guarantee the patients’ and the medical professionals’’ safety. Cleaning is exhaustive: both the apparatus and the material used by every doctor, such as armchairs, and the spaces used by the patients are cleaned with a disinfectant solution.

Reinforcement in the cleaning and disinfection of the Institute at the end of the day As well as all the additional safety measures and specific protocols to guarantee the safety of patients and professionals, at the end of every working day the common areas are subjected to an exhaustive cleaning and disinfecting process.

After applying a virucidal solution to floors and surfaces, the cleaning team disinfects all contact elements such as doorknobs, banisters, armrests, telephones or switches. As an additional measure, the Institute has also taken upon the daily disinfection of pavements and external access points.

All these safety and hygiene measures supplement the regular measures taken by the Institute, which continues to seek to offer the best quality to its patients.

“El trasplante endotelial es complejo y laborioso pero permite una rehabilitación precoz y eficaz del paciente con distrofia de Fuchs” El Dr. Luis Fernández-Vega Cueto-Felgueroso, oftalmólogo de la Unidad de Córnea y Cristalino del IOFV, profundiza en la Distrofia de Fuchs, una de las enfermedades más frecuentes del endotelio, la capa más interna de la córnea. El trasplante endotelial es el tratamiento más avanzado e implica la recuperación del paciente en apenas tres meses. ¿En qué consiste esta patología?

La córnea está dividida en tres capas: endotelio, capa más profunda; epitelio, la más superficial y externa del ojo y estroma, situada entre ambas. La función del endotelio es bombear el líquido interno del ojo que trasuda hacia el estroma, manteniéndolo trasparente para el paso de la luz.

Nuestros trabajos en investigación han permitido realizar avances muy alentadores en este campo.

Cuando existe una distrofia endotelial, dicha bomba funciona con dificultad y el líquido empieza a pasar a la capa de colágeno compacto, convirtiéndolo así en esponjoso. Esto provoca visión borrosa, especialmente por la mañana, ya que durante la noche el ojo está cerrado y no evapora por sí solo.

En los estadios más avanzados de la enfermedad, la visión borrosa es continua y aparecen ampollas de líquido bajo la capa más externa del ojo, el epitelio. Éstas son dolorosas y pueden llegar a infectarse y provocar cicatrices. ¿Qué tratamientos existen?

El más moderno y eficaz, con el cual en el IOFV tenemos muy buenos resultados y mucha experiencia, es el trasplante endotelial, que consiste en la obtención de una fina capa de endotelio de la córnea donante que se cultiva en el laboratorio para preservar su vi-

talidad y que se usa para sustituir el endotelio dañado.

La técnica quirúrgica para conseguirlo es compleja y laboriosa, pero permite una rehabilitación precoz y eficaz. Si las células están correctamente adheridas a su lugar natural y el injerto prende, el paciente puede rehabilitarse visualmente en los primeros tres meses tras la intervención. El riesgo de rechazo es menor que con los trasplantes completos, y tiene además la ventaja de que, si las células donantes no llegan a funcionar o se agotan con el tiempo, se puede realizar un nuevo injerto de manera poco agresiva. El trasplante endotelial también es bueno para otras patologías endoteliales.

Comenta que existe el trasplante endotelial, mediante el cual solo se sustituye la capa dañada más interna de la córnea. Entonces, ¿se puede sustituir también las otras capas, estroma y epitelio, si están deterioradas?

Efectivamente. Tradicionalmente se trasplantaba la córnea entera; actualmente esto solo se hace en casos en los que todas las capas de la córnea del paciente están dañadas. A esta técnica quirúrgica le denominamos queratoplastia penetrante. Hoy en

“Endothelial Transplant is Complex and Arduous But Enables Early and Effective Rehabilitation of Patients with Fuchs’ Dystrophy” Dr Luis Fernández-Vega Cueto-Felgueroso elaborates on Fuchs’ Dystrophy, one of the most common diseases of the endothelium, the innermost layer of the cornea. Endothelial transplant is the most state-of-the-art treatment and entails the recovery of the patient in just three months.

What does this pathology entail?

The cornea is divided into three layers: endothelium, the deepest layer; epithelium, the most superficial of the eye and the stroma, located between the other two. The function of the endothelium is to pump the internal liquid of the eye that leak to the stroma, keeping it clear to allow light to go through.

In cases of endothelial dystrophy, the function of this pump is hindered and the liquid starts to flow to the layer of compact collagen, turning it into cancellous. This causes blurry vision, especially in the mornings, as during the night the eye is closed and does not evaporate on its own.

In the most advances stages of the disease, blurry vision is ongoing and fluid blebs appear in the outermost layer of the eye, the epithelium.

These are painful and can get infected and cause scarring.

Our research work has enabled us to carry out very encouraging progress in this field

What treatments are available?

The most state-of-the-art and efficient one, with which we have obtained very good results and with which we have vast experience at the IOFV, is endothelial transplant. This treatment consists on obtaining a thin layer of endothelium from a donor’s cornea that is cultured in the lab to safeguard its vitality and which is subsequently used to substitute the damaged endothelium.

Los bancos de tejidos han sido un elemento clave en la mejora de las queratoplastias. día, sin embargo, se pueden realizar los llamados trasplantes o queratoplastias lamelares o laminares de la córnea, que se realizan para quitar y sustituir únicamente las partes que están concretamente localizadas, pudiendo así realizar trasplantes selectivos. Es decir, si se daña la capa más superficial (epitelio), realizamos la técnica denominada SALK (Superficial Anterior Lamellar Keratoplasty). Sin embargo, si este es más extenso y profundiza a través del estroma, se realiza un DALK (Deep Anterior Lamellar Keratoplasty). Por último, si el problema está únicamente en la parte más profunda, como en el caso por ejemplo de distrofia de Fuchs, en el endotelio, realizamos un DMEK (Descemet Membrane Endothelial Keratoplasty).

Esta técnica quirúrgica es muy novedosa, ¿en qué beneficia al paciente?

Este tipo de trasplantes lamelares han ido ganando terreno en los últimos años, en detrimento de los trasplantes penetrantes, hasta el punto de que el 90% de los que se realizan en la actualidad en el Instituto Oftalmológico Fernández-Vega son del primer tipo de esta modalidad que comentamos, por capas. La principal ventaja de los trasplantes lamelares es el menor índice de rechazo, que hoy se sitúa por debajo del 5%, mientras que en el caso de los trasplantes penetrantes el riesgo a los diez años es del 40%. Además, la rehabilitación visual del paciente es mucho más rápida. La dificultad para el cirujano es que con los trasplantes lamelares trabajamos con micras de espesor, por eso la curva de aprendizaje es más larga. En el Instituto ya llevamos tiempo realizándolos con muy buenos resultados. ¿Qué importancia tienen los bancos de tejidos de donantes para poder realizar este tipo de cirugías?

Han sido un elemento clave en la mejora de las queratoplastias, en particular de los trasplantes lamelares, dado que nos permiten preparar en las mejores condiciones las córneas que vamos a implantar. Es fundamental realizar un estudio completo de la córnea del donante, utilizando microscopios adecuados para su valoración y son evaluados para seleccionar qué capas tienen sanas y cuáles cumplen los requisitos para convertirlas en tejido donante. Así, en el supuesto

de que un donante tuviera una cicatriz en su estroma, todavía se podría aprovechar su endotelio para que otra persona pueda ver. En Asturias, el Centro Comunitario de Sangre y Tejidos de Cruz Roja realiza una labor puntera y fundamental en este campo.

En cuanto a investigación, ¿qué avances están presentando en este campo?

En el Instituto apostamos por la investigación de patologías oculares a través de la Fundación de Investigación Oftalmológica que cuenta con su soporte en todos los aspectos. En la unidad de medicina regenerativa y terapias, tenemos estudios recientes en cuanto a la utilización de córneas artificiales y de cultivos celulares. En cuanto a células madre, estamos investigando la utilidad de injertar que están presentes en el epitelio que se extraen del ojo que no está dañado o bien de un donante, para mejorar los resultados en patología de la superficie de la córnea.

En el futuro, podría ser posible la inyección de células en la cámara anterior.

También estamos trabajando con suero autólogo enriquecido con plaquetas, conocido por las siglas PRGF, que ha demostrado tener propiedades regenerativas muy importantes en la patología de la superficie ocular, tanto de la conjuntiva como de la córnea. Nosotros lo utilizamos en tres presentaciones como son colirio, membrana o coágulo para disponer de diferentes opciones según la patología de que se trate; con una fórmula que elimina los factores inmunológicos perjudiciales que pueden favorecer la inflamación. Tissue banks have been paramount in the improvement of keratoplasty surgery The surgical procedure to achieve this is complex and arduous but enables an early and effective rehabilitation. If the cells are correctly adhered to its natural place and the graft takes, the patient can recover its vision within the first three months following the intervention.

The risk of rejection is lower than in full transplants, and has the advantage that, if the donor’s cells do not work or eventually run out, a new graft can be performed in an almost non-aggressive way. Endothelial transplant is good for other endothelial pathologies You have mentioned endothelial transplant, where only the damaged, innermost layer of the cornea is substituted. Does this mean that the other layers, stroma and epithelium can also be substituted if they are damaged?

Indeed. Traditionally the full cornea was transplanted; currently this is only done in cases where the patient’s whole cornea is damaged. This surgical technique is known as Deep Keratoplasty. However, nowadays surgeons can perform what we know as lamellar transplant or keratoplasty, this is carried out to remove and substitute just localised areas, thus enabling selective transplants. This means that if the outermost layer (epithelium) is damaged we carry out a technique named SALK (Superficial Anterior Lamellar Keratoplasty). However, if it is more widespread and involves the stroma, a DALK (Deep Anterior Lamellar Keratoplasty) is carried out. Lastly, if the problem only involves the innermost layer, like in the case of Fuchs’ Dystrophy in the endothelium, we carry out a DMEK (Descemet Membrane Endothelial Keratoplasty).

This is a very novel technique, how is it beneficial for the patient?

This type of lamellar transplants have been gaining ground in the last few years at the expense of penetrating transplants, to a point where currently 90% of those carried out at Instituto Oftalmológico Fernández-Vega belong to the first type we commented, by layers. The main advantage of lamellar transplants is a lower rejection rate, currently fewer than 5%, while in the case of penetrating transplants the risk at ten years in 40%. Furthermore, visual recovery for the patient is much quicker. The complication for the surgeon is that with lamellar transplants we work with microns in thickness and therefore the learning curve is longer. We have been performing them out for a while at the Institute with a good success rate.

How important are donors’ tissue banks in order to perform this type of surgeries?

Tissue banks have been a key element in the improvement of keratoplasties, specifically in lamellar transplants, as they enable us to prepare in the best possible conditions the corneas we are going to implant.

It is paramount to carry out a full study of the donor’s cornea using the appropriate microscopes for their assessment and subsequent selection of those healthy layers that meet the criteria to become donor’s tissue. Thus, in the event that the donor had a scar on the stroma, the endothelium could still be used to help another patient regain vision. In Asturias, the Centro Comunitario de Sangre y Tejidos (Community Centre of Blood and Tissues) of the Red Cross carries out innovative and crucial work in this field. In the future, injection of cells into the anterior chamber might be possible.

In regard to research, what are the latest advances in this field?

At the Institute we engage in the research of eye pathologies through the Fundación de Investigación Oftalmológica (Eye Research Foundation), which receives full support. In the Unit of Regenerative Medicine and Therapies, we have recent studies on the use of artificial corneas and cell cultures. As for stem cells, we are researching the use of grafting the cells that are obtained from an epithelium that is not damaged or from a donor to improve the results in pathologies of the surface of the cornea.

We are also working with autologous platelet-enriched plasma (PRGF) that has proven very important regenerative qualities in the pathology of ocular surface, both in the conjunctiva and the cornea. We use three product presentations: eye drops, membrane or clots to have different options depending on the pathology, with a formula that removes harmful immunological factors that can stimulate swelling. We are also analysing the diagnostic and therapeutic improvements available for tumours of the ocular surface. We are researching to improve the eyesight of our patients. We are fully committed and we will strive for it to remain that way.

Alternativas de tratamiento en la enfermedad del ojo seco Durante los últimos años se ha producido un aumento de la conciencia en todo el mundo sobre la enfermedad de ojo seco (EOS), tanto por su gran incidencia como por su repercusión en la calidad de vida de los que la padecen, por ello constituye una de las consultas más frecuentes en atención primaria y en oftalmología, segun explica la Dra. Lucía Ibares, especialista en Superficie Ocular y Córnea del IOFV.

El ojo seco se estableció como enfermedad hace tan solo 30 años y, desde entonces, la definición ha evolucionado a medida que han avanzado los conocimientos del tema. La más reciente y completa es la presentada en julio del 2017 por la Sociedad de Superficie Ocular y Película lagrimal, que promovió la segunda edición del informe internacional Dry Eye Workshop (DEWS II), elaborado por un comité formado por 156 expertos de 23 países.

Concepto de la enfermedad

Se describe en dicho informe como “una enfermedad multifactorial de la superficie ocular que se caracteriza por una pérdida de la homeostasis de la película lagrimal se acompaña de síntomas oculares y en cuya etiología intervienen la inestabilidad de la película lagrimal, la inflamación y daño de la superficie ocular y alteraciones neurosensoriales”. Con ello se especifica que el ojo seco es algo más que un problema de la lágrima ya que se ven afectadas todas las estructuras implicadas en la producción y mantenimiento de la película lagrimal como la córnea, la conjuntiva, los párpados, las pestañas, las glándulas lagrimales principales y accesorias que producen la parte acuosa de la lágrima, las glándulas de meibomio (glándulas situadas en el espesor del párpado) que producen la parte lipídica de la lágrima y los nervios que Inervan dichas estructuras.

Los síntomas Los más frecuentes son la sensación de sequedad, arenillas y quemazón. También se presentan, aunque con menos frecuencia, la secreción filamentosa, la borrosidad visual transitoria y el enrojecimiento.

Como la película lagrimal constituye la última capa del sistema dióptrico ocular, también se puede dar disminución de visión en diferente grado en función de la gravedad.

Clasificación del ojo seco

Según el DEWS I (la primera edición del informe internacional del ojo seco) se clasifica en dos tipos: el ojo seco por falta de secreción acuosa lagrimal y el ojo seco de tipo evaporativo, donde la lágrima “es de mala calidad” y se mantiene en el ojo menos tiempo del que debería. Dentro de la primera clase, se encuentra el Síndrome de Sjögren que es una enfermedad autoinmunitaria caracterizada por inflamación linfocitaria y destrucción de las glándulas lagrimales y salivares, así como de otros órganos exocrinos. En la nueva clasificación DEWS II ambas se interrelacionan y se incluye un nuevo tipo de ojo seco relacionado con el componente neuronal (los nervios), con lo que se incluyen como pacientes de ojo seco aquellos con síntomas, pero sin signos (el médico en la exploración no ve nada). Es lo que se define como dolor neuropático. En segundo lugar,

Treatment Options in the Dry Eye Disease During the last few years the awareness of Dry Eye Disease has raised in the world, both due to its high incidence and its repercussion in the quality of life of those who suffer it, thus becoming one of the most common consultations in primary health care and ophthalmology according to Dr. Lucía Ibares, specialist in Ocular Surface and Cornea of ​​ the IOFV. Dry Eye was defined as a disease just 30 years ago and, ever since, the definition has evolved as knowledge about its origin has advanced. The most recent and complete one is the one presented in July 2017 by the Ocular Surface and Tear Film Society, which promoted the second edition of the international report Dry Eye Workshop (DEWS II), elaborated by a committee constituted by 156 experts from 23 different countries.

Concept of the disease Described in the aforementioned report as: “Dry eye is a multifactorial disease of the ocular surface characterized by a loss of homeostasis of the tear film, and accompanied by ocular symptoms, in which tear film instability and hyperosmolarity, ocular surface inflammation and damage, and neurosensory abnormalities play etiological roles.” This clarifies that dry eye is more than a problem of the tear as all the structures involved in the production and maintenance of the tear film, such as the cornea, the conjunctiva, the eyelids, the eyelashes, the main and accessory tear glands that produce the aqueous part of the tear, the Meibomian glands (located in the thickness of the eyelid) that produce the lipid part of the tear and the nerves that innerve these structures are affected.

The symptoms

The most common symptoms are the feeling of dryness, a itchy sensation and burning. Less frequently, one can present filamentous secretion, transient blurry vision and redness. As the tear film constitutes the outermost layer of the ocular dioptric system, there can also be a loss of vision to a greater or lesser extent depending on the severity of the disease.

Diagnóstico En primer lugar, es importante conocer que no existe ninguna prueba patrón de oro para el diagnóstico de ojo seco, por lo que se recomienda realizar una detallada revisión de signos y síntomas por un especialista. Las pruebas que se emplean con más frecuencia son una adecuada historia clínica con cuestionarios de síntomas suministrados al paciente, determinación de la agudeza visual y una exhaustiva exploración de la superficie ocular con la lámpara de hendidura con especial detenimiento en los párpados, su borde libre, la película lagrimal, la córnea y las glándulas de meibomio. Se deberá analizar también el tiempo de ruptura lagrimal (que mide tiempo de permanencia de la lágrima en el ojo) y el test de Schirmer (test que mide la producción de lágrima acuosa) que consiste en colocar en ambos párpados inferiores unas tiras de papel milimetrado durante cinco minutos. Con el análisis de todo lo anterior el oftalmólogo analiza los síntomas, la cantidad y calidad de la película lagrimal junto con el grado de inflamación de la superficie ocular.

Tratamiento

Los procesos etiopatogénicos descritos suelen ser irreversibles, por lo que es importante saber que el tratamiento de la EOS es de larga duración y normalmente para toda la vida debido a la naturaleza crónica, progresiva y lenta de esta patología. Existirán momentos de empeoramiento o mejora, pero siempre es necesario mantener un tratamiento basal. El DEWS ha publicado unas recomendaciones que sugieren diferentes opciones de tratamiento en función del nivel de gravedad de la enfermedad valorados del uno al cuatro.

Estas recomendaciones también pueden aplicarse de forma gradual, ampliando el tratamiento al siguiente nivel si las medidas anteriores han sido insuficientes o si mejoran los síntomas al bajar al nivel previo.

Nivel 1 • Educación y modificaciones medioambientales/ dietéticas como, por ejemplo, señalar la importancia de aumentar la frecuencia de parpadeo y evitar factores que incrementen la evaporación, como la exposición al viento sin gafas de sol o a ambientes con humedad baja; así como evitar estar rodeado de tóxicos como el humo de tabaco. Además, es importante prevenir y tratar la alergia ocular.

• Revisión de las medicaciones sistémicas que puedan agravar la sequedad como varios ansiolíticos y diuréticos.

• Uso de lágrimas artificiales sin conservantes, incluidos geles y pomadas. A pesar de la gran cantidad de formulaciones existentes en el mercado, no existe una “lágrima ideal” debido a la gran complejidad de la lágrima normal • Terapia palpebral: con el objetivo de tratar la blefaritis o disfunción de glándulas de meibomio Nivel 2 • Fármacos - Antinflamatorios: como corticoides tópicos y fármacos inmunosupresores, como la ciclosporina.

According to DEWS I (the first edition of the international report on dry eye) it is classified in two types: aqueous-deficient dry eye and evaporative dry eye, where the tear “is of poor quality” and stays in the eye less time than it should. Within the first type, we can find the Sjögren Syndrome, an autoimmune disease characterised by lymphocytic inflammation and destruction on the tear and salivary, as well as other endrocrine organs. In the new DEWS II classification both interrelate and a new type of dry eye related to the neuronal component (the nerves) is included, thus including as dry eye patients those with symptoms, but no signs (the doctor does not see anything in the exploration). This is defined as neuropathic pain. Secondly, there would be the opposite case, patients with many signs in the exploration, but no symptomatology, which would be triggered by a reduced corneal sensitivity.

Diagnosis In the first place, it is important to know that there is no gold standard test to diagnose dry eye, so a thorough examination of the signs and symptoms by a specialist in ophthalmology is recommended. The most commonly used tests are a good clinical history record with symptoms’ questionnaires given to the patient, determination of the visual acuity and a thorough examination of the eye surface with the slit lamp, paying special attention to the eyelids, their free edge, the tear film, the cornea and the Meibomian glands. Other tests to perform are the tear breakup time (measuring the time the tear film stay without breaks in the eye) and the Schirmer test (and the Schirmer test (test that measures the production of aqueous tear) consisting of placing both inferior eyelids in special paper strips for 5 minutes. With the analysis of all the aforementioned as a whole, the ophthalmologists analyses the symptoms, the quantity and quality of the tear film together with the level of inflammation of the ocular surface.

This etiopathogenic processes described are usually irreversible, it is therefore important to know that the treatment of the DED (Dry Eye Disease) is a long-term one and usually lifelong due to the chronic, progressive and slow nature of this pathology.

There will be times when the disease will worsen or improve, but a baseline treatment is always necessary. The DEWS has published some recommendations that suggest different treatment options depending on the severity of the disease valued from 1-4. These recommendations can also be applied gradually, taking the treatment to the next level if the previous measures are insufficient or bring the treatment down if the symptoms improve.

Conclusiones Es importante recordar la dificultad del diagnostico y clasificación del síndrome de ojo seco por la gran variabilidad de signos, síntomas y factores relacionados. Además, en su tratamiento es de vital importancia tener en cuenta los factores ambientales y personalizar el tratamiento según las necesidades concretas del ojo. No existe una lágrima artificial ideal, por lo cual cada paciente deberá encontrar con su oftalmólogo la que mejor se adapte a sus necesidades. Por último, tanto el diagnóstico como el tratamiento de la enfermedad de ojo seco están en continuo desarrollo por la cantidad de estudios básicos que se están llevando a cabo en la actualidad.

Conclusions In conclusion, I will say that it is important to highlight the difficulty that exist when diagnosing and classifying dry eye disease due to the great variability of the signs, symptoms and related factors.

Furthermore, it is of vital importance to take into account environmental factors in its treatment and customize the treatment according to the specific needs of the eye. There is no ideal artificial tear, so each patient will have to find together with his ophthalmologist the one that best meets his needs.

Lastly, both the diagnosis and the treatment of the dry eye are constantly evolving due to the amount of basic studies that are currently being carried out.

El ojo del surfista En la playa, las radiaciones solares y los reflejos del mar no solo suponen un riesgo para la piel sino también para los ojos. Por ello, proteger la visión resulta crucial, especialmente para quienes practican con frecuencia deportes náuticos. La patología más habitual en personas que se exponen de forma constante al agua de mar, el salitre, la arena y las radiaciones solares es el pterigium, comúnmente conocido como ‘ojo del surfista’

¿Qué es el ‘ojo del surfista’?

Se trata de un pequeño bulto que aparece en el globo ocular, habitualmente en la parte más cercana al lacrimal, aunque puede llegar a invadir la córnea y alterar su transparencia. Este crecimiento anormal de tejido es generalmente benigno pero resulta molesto y puede llegar a afectar a la visión. Es más frecuente y grave en los pacientes que viven en zonas de climas tropicales.

Además del pterigium existe otra dolencia asociada a una exposición habitual a las radiaciones del sol. Es la pinguécula, una mancha o pequeño bulto de color amarillo que aparece también en el globo ocular, en la zona más cercana a la nariz, y suele presentar una forma triangular. Se trata de otro crecimiento anormal del tejido, solo que este se acaba transformando en un depósito de grasa, proteína o calcio. Sería algo parecido a la formación de un callo en la piel.

¿Cuáles son los factores de riesgo?

Una exposición habitual al ambiente de la playa es el principal factor de riesgo para contraer estas patologías, aunque cualquiera que pase mucho tiempo al aire libre puede sufrirlas. Especialmente si se trata de zonas con mucho polvo y arena y en las que la exposición a la luz solar y el viento sea constante.

De ahí que se las denomine comúnmente ‘ojo del surfista’, aunque también suelen padecerlo agricultores, pescadores, jardineros, esquiadores, alpinistas, montañeros y marineros, si al estar al sol no usan gafas adecuadas, con el filtro correspondiente.

El pterigium es más frecuente en la población adulta, especialmente entre quienes tienen entre 30 y 50 años. La pinguécula, en cambio, se presenta con mayor asiduidad en personas que superan esa franja de edad. Además, quienes sufren a menudo sequedad en los ojos son más propensos a padecerlas.

Los síntomas

Esta patología suele provocar enrojecimiento y picor aunque suele ser indolora.

Los síntomas suelen ser leves y transitorios y, aunque pueden presentarse sin dolor, sí que producen una sensación molesta, enrojecimiento y picor, como si tuviera algo dentro del ojo. Además, suelen inflamarse y pueden llegar a causar una visión borrosa.

Surfer’s Eye

At the beach, sun radiation and the glare of the sea are a risk both for the skin and for the eyes. It is therefore paramount to protect one’s vision, especially for those who frequently practice water sports. Pterigium, commonly known as ‘Surfer’s Eye’, is the most common condition in individuals who are regularly and constantly exposed to seawater, saltpetre, sand and sun radiation.

What is the ‘Surfer’s Eye’?

Surfer’s Eye is a small swelling that appears in the eye, usually in the area closest to the lachrymal, although it can invade the cornea altering its transparency. This abnormal tissue growth is generally benign but is unpleasant and may affect the patient’s eyesight. It is most frequent and severe in patients who live in tropical regions.

On top of Pterigium, there is another condition associated to a regular exposure to sun radiation. This condition is known as Pinguecula, a yellowish spot or growth in the eye, in the area closest to the nose, usually presenting a triangle shape. This is another unusual tissue growth, but this one ends up creating fat, proteins or calcium deposits. It would be something similar to the formation of a corn on the skin.

What are the risk factors?

A regular exposure to a beach environment is the main risk factor to suffering these pathologies, although anyone who spends a lot of time outdoors can develop them. This occurs especially in areas with a lot of dust and sand and where the exposure to sunlight and wind is constant. This is why they are commonly known as ‘Surfer’s Eye’, although farmers, fishermen, gardeners, skiers, mountain climbers, alpinists and seamen also suffer them if they do not use the appropriate sunglasses with the corresponding filter.

This pathology usually causes redness and itchiness despite being painless.

Pterigium is the most common among adults, especially among those between 30 and 50 years of age.

However, Pinguecula is more common in older individuals. Furthermore, those who often suffer from dry eye are more likely to suffer these conditions.

¿Cómo se trata?

Los tratamientos de estas dolencias dependen de la fase en la que se encuentren. Si el pterigium o la pinguécula se detectan en una fase inicial, el tratamiento puede consistir en gotas oftalmológicas o lágrimas artificiales que reducirán los síntomas, como la hinchazón, el picor y el enrojecimiento, y frenarán la evolución.

Symptoms are usually mild and transient and, even though they can present themselves painlessly, they do cause irritation, red eyes and itchiness, as if one had something in one’s eye. Furthermore, they tend to swell and can even cause blurry vision.

Cuando ya tienen un tamaño mayor, la solución pasa por la extirpación quirúrgica. Se trata, no obstante, de un procedimiento sencillo, de unos treinta minutos, que se lleva a cabo con anestesia local y cuya recuperación es rápida, pues se realiza de forma ambulatoria. La mayoría de los pacientes hacen vida normal al día siguiente de la intervención.

What is their treatment?

La mejor prevención para esta dolencia es el uso de gafas con filtros que impidan el paso de al menos un 95% de los rayos ultravioletas.

Prevenir el ‘ojo del surfista’ La mejor manera de prevenir el pterigium o la pinguécuela es proteger los ojos de la radiación solar con unas gafas con filtros que impidan el paso de un 95-100% de los rayos ultravioletas. Ademas, las lentes protegerán el ojo de la arena y el viento.

Para quienes tienen tendencia a sufrir sequedad en los ojos, es aconsejable emplear también gotas oftalmológicas o lágrimas artificiales para mantener el globo ocular hidratado. Lo mejor es aplicarlas antes y después de la exposición al ambiente de la playa o al aire libre.

Ante los primeros síntomas es conveniente acudir a la consulta del oftalmólogo. Cuanto antes se detecte la dolencia, menos invasivo será el tratamiento.

For those who often suffer from dry eye, the use of eye drops or artificial tears is also advisable to keep the eye hydrated. It is best to apply them before and after the exposure to the beach or outdoors environment.

It is convenient to visit the ophthalmologist at the first signs of symptoms. The sooner the condition is detected, the less invasive the treatment will be.

El lupus eritematoso es una enfermedad crónica y autoinmune en la que el propio sistema de defensa o sistema inmnunitario del paciente ataca sus órganos y tejidos.

Los síntomas oculares más frecuentes son la sensación de arenillas y ojos pesados, provocados por la inflamación de la superficie ocular. Cuando la inflamación afecta a la parte trasparente del ojo (queratitis) o a la esclerótica (escleritis) a los pacientes les molesta mucho la luz (fotofobia) y tienen dolor ocular.

Otra manifestación ocular de esta dolencia es la inflamación de tejidos internos del ojo, lo que ocasiona uveítis. Esta complicación puede provocar, a parte del dolor, visión borrosa e incluso pérdida de visión. Otra de las posibles afecciones tiene que ver con la inflamación de los vasos de la retina (vasculitis) que fundamentalmente da visión borrosa. que también estén inflamados las arterias y las venas en otras localizaciones.

En muchos pacientes las manifestaciones oculares son las únicas que tienen los pacientes de Lupus y preceden incluso años a la afectación de otras partes del cuerpo, por lo que el diagnostico precoz y el tratamiento de la enfermedad pueden minimizar las complicaciones del Lupus.

Asimismo, esta dolencia puede llegar a tener impacto en el sistema nervioso central y provocar neuritis óptica y oclusiones vasculares retinianas, sobre todo en pacientes con los marcadores en sangre de anticuerpos antifosfolípidos. En estas ocasiones será necesario tratar las obstrucciones vasculares y las trombosis venosas tanto a nivel de oftalmólogo como añadir al régimen de tratamiento de la enfermedad la terapia inmunomoduladora y la anticoagulante.

¿Cómo se detecta el lupus eritematoso?

Si los primeros síntomas del lupus eritematoso se dan en los ojos, el oftalmólogo realizará una exploración completa investigado también si hay manifestaciones sistémicas por ejemplo de reuma. Tras esta primera parte, se procederá a solicitar un análisis de sangre, en el que se incluyan los marcadores de enfermedades autoinmunes. En cuento al tratamiento de las manifestaciones oculares leves se realizan con medicación local pero en la gran mayoría de las ocasiones es necesario el tratamiento sistémico con corticoides y / o inmunomoduladores. El manejo de esta enfermedad se suele realizar de forma multidisciplinar, coordinado con el reumatólogo, medico de familia o internista.

Es importante descartar que existan enfermedades autoinmunes como el lupus eritematoso cuando el paciente (y con más frecuencia la paciente) tiene un ojo seco grave que no cede a tratamientos clásicos de la lubricación ocular, presente dolor ocular o inflamación de la esclera, la córnea o intraocular. Manifestaciones como la escleritis o vasculitis son indicadores de que pueden estar afectadas otras partes del cuerpo y por norma general es necesario el tratamiento sistémico con inmunomoduladores. Cuando hay riesgo de oclusiones vasculares, se debe añadir también un tratamiento anticoagulante.

Hay que tener en cuenta, por tanto, que las revisiones oftalmológicas pueden ayudar al diagnóstico precoz de este tipo de enfermedades y también a prevenir complicaciones graves.

Lupus erythematosus is a chronic and autoimmune disease in which the patient’s defence system or immune system attacks its organs and tissues.

The most common ocular symptoms are the feeling of grittiness and heavy eyes, caused by the swelling of the ocular surface. When the inflammation affects the white part of the eye (keratitis) or the sclera (scleritis) patients suffer discomfort caused by the light (photophobia) and ocular pain.

Another symptom of the disease is the swelling of internal ocular tissues, causing uveitis. This complication can cause, as well as pain, blurry vision and even vision loss. Another potential complaint can be the inflammation of the vessels in the retina (vasculitis), which mainly causes blurry vision. As you can well imagine, if the vessels in the retina are affected, it is very likely that the arteries and veins in other places are also swollen.

For many patients, ocular symptoms are they only symptoms of Lupus they feel. These can precede by many years the complaints of other parts of the body, therefore an early diagnosis and the treatment of the disease can minimise Lupus-related complications.

En cuanto al régimen de vida, en las enfermedades autoinmunes cada vez se da más importancia a las medidas preventivas, pues mantienen ‘tranquilo’ al sistema inmunitario de defensa. Entre ellas están las dietas antiinflamatorias pautadas por el profesional experto en el tema, el ejercicio físico equivalente a caminar una hora al día, las medidas que evitan el estrés y conservar los ritmos de sueño adecuados.

How is lupus erythematosus detected?

It is important to rule out autoimmune diseases such as lupus erythematosus when the patient (most frequently female patients) has a severe dry eye that does not respond to classic treatments of ocular lubrication, presents ocular pain or inflammation of the sclera, cornea or intraocular swelling. Symptoms such as scleritis or vasculitis indicate that other parts of the body might be involved and, as a general rule, systemic treatment with immunomodulators is needed. An anticoagulant treatment should be added when there is risk of vascular occlusions.

Therefore, one has to bear in mind that eye checkups can help an early diagnosis of this type of diseases as well as to prevent severe complications.

About the treatment Some professionals use drugs that may have ocular toxicity, such as the by-products of quinine (hydroxychloroquine). If the patient were going to need a long-term use of this drug, the doctor would need to assess its substitution for other drugs with less eye toxicity and carry out regular revisions, including a colour vision test, tests involving the retina or ocular electrophysiology to those patients who are taking these drugs.

As for the lifestyle regime, emphasis is increasingly put on prevention measures, as these maintain the defence immune system ‘calm’. Among these are anti-inflammatory diets prescribed by an expert professional, physical exercise equivalent to walking for one hour per day, measures to prevent stress and preserve adequate sleeping patterns.

Por tercer año consecutivo, la Fundación Fernández-Vega (FFV) se ha trasladado hasta Honduras para intervenir de cataratas, primera causa de ceguera reversible en el mundo, a pacientes locales. Una operación que se realiza sin apenas ninguna dificultad en España pero que, en países con recursos limitados se convierte en una tarea verdaderamente complicada.

Durante los cinco días que ha durado la expedición, la Fundación ha conseguido realizar exitosamente más de 100 cirugías. “El 80% de las operaciones que se han realizado han sido para eliminar cataratas. En al‑ gunos casos los pacientes llegaron siendo personas to‑ talmente dependientes, con un porcentaje de ceguera de casi el 100%”, ha destacado el doctor Héctor, oftalmólogo hondureño con el que colabora la Fundación.

Habitualmente las cataratas se diagnostican y eliminan de manera precoz en cuanto la enfermedad empieza a dar los primeros síntomas, sin embargo en países subdesarrollados los pacientes pueden acabar perdiendo la vista ya que la cirugía de cataratas requiere de una tecnología que en países como Honduras escasea. “Todo el material del que disponemos y todos los aparatos que recibimos son gracias a asociaciones”, ha explicado el Dr. Robles, quien ha destacado que este año se ha facilitado

una gran cantidad del equipamiento necesario para realizar este tipo de cirugías.

Sin embargo, la escasez de recursos en esta especialidad médica no es solamente palpable a nivel material y tecnológico. Honduras es un país con una gran escasez de oftalmólogos y, por ello, la labor de fundaciones como la de la Fundación Fernández-Vega adquiere una gran importancia. “A pesar de la gran formación de sus oftalmólogos, la escasez de profesionales en algunas áreas del país y la falta de recursos materiales y tecnológicos hacen que la ayuda de asociaciones como la nuestra y como otras como la Fundación CAMO y la de Central American Eye Clinics sean muy importantes” ha señalado el Dr. Alvaro Fernández-Vega.

Los doctores han realizado con éxito numerosas cirugías de cataratas, operación complicada en países con recursos limitados.

The Fundación Fernández-Vega Performs Over 100 Surgeries In Its Last Solidarity Trip To Honduras Fundación del Instituto Oftalmológico Fernández- Vega, Fundación CAMO and Central American Eye Clinics collaborates with Doctors Héctor and Marco Robles, the only ophthalmolog y specialists for an area populated with a million inhabitants.

For third year in a row, the Fundación Fernández-Vega (FFV) has travelled to Honduras to perform cataract surgeries, first cause of reversible blindness in the world, to local patients. Performing this surgery entails little complications in Spain but becomes really difficult in countries with limited resources.

During the five-day expedition, the Foundation has been able to successfully perform over 100 surgeries. “80% of the surgeries performed have been to remove cataracts. In some cases patients arrived completely dependent, with a blindness percentage of nearly 100%,” highlighted Dr Héctor, ophthalmologist from Honduras who collaborated with the Foundation.

Usually cataracts are diagnosed and removed as soon as the disease presents its first symptoms, however in developing countries

patients can loose their eyesight as in countries such as Honduras the technology required to perform cataract surgery is scarce. “All the material we have and all the equipment we receive are thanks to associations,” explained Dr Robles, who has highlighted that this year he has received a lot of the equipment necessary to perform this type of surgery.

However, the lack of resources in this medical field is not restricted to materials and technology. Honduras is a country with a big lack of ophthalmologists and that is why the work of foundations like the Fundación Fernández-Vega is so important. “Despite the great training of its ophthalmologists, the lack of professionals in some areas of the country and the lack of material and technological resources make the help of associations such as ours and others like Fundación CAMO and Central American Eye Clinics very important” stated Dr Alvaro Fernández-Vega.

Como cada año la Fundación Fernández-Vega junto con especialistas de todo el mundo han contribuido de manera solidaria con su talento y medios técnicos para que personas sin recursos puedan recibir el tratamiento que necesitan. “Todos tenemos que poner nuestro granito de arena para intentar mejorar la vida de personas que realmente lo necesitan”, ha concluido Álvaro Fernández-Vega.

As every year the Fundación Fernández-Vega together with specialists from all over the work has contributed in solidarity with its talent and technical resources so that destitute individuals can receive the treatment they need. “We all have to do our bit to improve the life of those who really need it,” concluded Álvaro Fernández-Vega.

En cuanto a la posibilidad de volver a Honduras, los doctores Fernández-Vega han expresado su deseo de regresar el próximo año y así subsanar otras enfermedades que no pudieron tratar. Durante su estancia tuvieron una paciente con cataratas y hemovítreo. El tratamiento del hemovítreo requiere un aparato de vitrectomía del que por desgracia no disponían en el Hospital, por lo que no se pudo corregir, cuando con el material necesario, es una intervención que apenas conlleva 10 o 15 minutos. Los Doctores esperan poder volver y colaborar con lo necesario para que en el futuro se puedan llevar a cabo vitrectomías.

Nuevas soluciones para corregir la catarata y la presbicia La cirugía de catarata es uno de los procedimientos quirúrgicos más comunes, siendo la primera opción de tratamiento para el 85% de los pacientes1,2. El Profesor Luis Fernández Vega, director médico del Instituto Oftalmológico Fernández Vega, habla de esta intervención, que consiste en la sustitución del cristalino opaco por una lente intraocular y permite corregir otros defectos refractivos como la presbicia, implantando lentes que reestablecen las propiedades ópticas del ojo además de devolver la transparencia.

Los últimos avances en lentes intraoculares ofrecen la posibilidad de recuperar la visión a todas las distancias y reducir al máximo el perfil de alteraciones visuales.

¿Qué es la presbicia y qué síntomas presenta? ¿Qué la diferencia de la catarata?

La catarata se produce porque el envejecimiento natural del ojo provoca que el cristalino se vuelva opaco, de modo que el paciente no es capaz de ver bien, ni siquiera con sus gafas perfectamente graduadas.

Por su parte, la presbicia es un defecto refractivo, consecuencia inevitable asociada a la edad. Con el tiempo, el cristalino pierde su flexibilidad y capacidad para enfocar la luz, sobre todo en distancias cortas.

Esto provoca visión borrosa y la necesidad de usar gafas de cerca. Paulatinamente, el paciente dejará de enfocar correctamente objetos a distancias cercanas de uso cotidiano como pueden ser los libros, los ordenadores o su teléfono móvil. Debido a que la presbicia es un proceso degenerativo, existen algunos tratamientos que corrigen parcialmente la pérdida visual de cerca. En este grupo se encuentran, por ejemplo, las gafas, las lentillas, la cirugía refractiva con lente intraocular o cirugía de cristalino transparente.

Ambas condiciones afectan directamente al cristalino y hoy día es posible ponerles solución con un mismo procedimiento. Un avance del que podría beneficiarse toda la población ya que, al ser un defecto asociado al envejecimiento, todas las personas presentarán algún grado de presbicia a lo largo de su vida, y se estima que la gran mayoría desarrollará cataratas. ¿Cuál es el impacto de la pérdida de visión en la vida de los pacientes?

Según los datos que se pueden extraer de la encuesta europea ‘Mucho que ver’, realizada por Alcon, el 76% de los pacientes con catarata experimenta visión borrosa pero también identifican como sínto-

mas la dificultad de ver claramente por la noche, los halos de luz, el desvanecimiento de los colores, o la visualización de puntos negros.

En cuanto a la presbicia, sus síntomas más frecuentes son la dificultad para enfocar los objetos en visión cercana, la necesidad de alejar el libro o el teléfono móvil para poder ver mejor, la sensación de que las letras de un texto “bailan” o están borrosas, los dolores de cabeza al fijar la vista en la lectura durante mucho tiempo y la fatiga ocular (enrojecimiento, ojo seco, escozor, sensación de arenilla, lagrimeo).

A 9 de cada 10 personas les preocupa el impacto que la pérdida de visión tendría en realizar las tareas que más disfrutan, además del riesgo de caídas o accidentes y la pérdida de independencia que implicaría.

“Los pacientes podrán recuperar la visión para la mayor parte de tareas de la vida diaria con las lentes intraoculares” ¿Qué tipo de lentes intraoculares existen para corregir la presbicia? ¿Cuáles son los últimos avances?

Existen distintas opciones de lentes intraoculares, que podemos clasificar en dos categorías en base a su funcionalidad óptica: lentes monofocales y lentes para corrección de la presbicia. Esta segunda categoría se puede subdividir en lentes multifocales (o trifocales) y lentes de rango de visión extendido.

Las lentes intraoculares monofocales tienen la misma potencia en toda la superficie, corrigiendo la visión a una única distancia, y se utilizan para tratar determi-

Professor Luis Fernández Vega, medical director of Instituto Oftalmológico Fernández Vega, speaks about this intervention, consisting in the substitution of the opaque crystalline for an intraocular lens, enabling the correction of refractive errors such as presbyopia by implanting lenses that re-establish the optic properties of the eye as well as returning its transparency.

The latest advances in intraocular lenses offer the possibility of recovering vision at all distances as well as keeping to a minimum the profile of visual alterations.

What is presbyopia and what are its symptoms?

How is it different to cataracts?

Cataracts are caused because the natural ageing of the eye causes the crystalline to turn opaque, so that the patient is not able to see properly, not even with perfectly correct prescription glasses.

For its part, presbyopia is a refractive defect, age-related inevitable consequence. Over time, the crystalline looses its flexibility and ability to focus light, mainly at short distances. This causes blurry vision and the need to use glasses for near vision. Progressively, the patient will not be able to focus daily objects, such as books, computes or mobile phones, at close distances. Given that presbyopia is a degenerative process, there are some treatments that partially correct near vision loss. Among these are, for example, eyeglasses, contact lenses, refractive surgery with intraocular lens or surgery of the clear crystalline.

Both conditions directly affect the crystalline and it is currently possible to solve them in one same procedure. The whole population would be able to benefit from this advance as, given it is an age-related defect, everyone will present some degree of presbyopia at some point in their lives; it is estimated that most individuals will develop cataracts.

What is the impact of vision loss in the patients’ lives?

According to the data of the European survey ‘More to See’ carried out by Alcon, 76% of patients with cataracts suffers blurry vision and also identify as a symptom the difficulty of seeing clearly at night, halos around lights, fading of colours or seeing black dots.

As for presbyopia, the most common symptoms are the difficulty to focus objects in near vision, the need to move the book or the mobile phone away to see better, the feeling that the letters in a text ‘dance’ or are blurry, headaches when focusing their eyesight for a long time or ocular fatigue (reddening, dry eye, itchiness, a feeling of grittiness, tearing).

9 out of 10 patients are worried about the impact the loss of vision would have on the performance of their activities of choice, as well as the risk of falling or accidents and the loss of independence this would imply.

What type of intraocular lenses are there to correct presbyopia? What are the latest advances?

There are different intraocular lenses options, we can classify these in two categories depending on their optical performance: monofocal lenses and lenses for the correction of presbyopia. This second category can be divided into multifocal lenses (or trifocal) and lenses with an extended range of vision.

Monofocal intraocular lenses have the same power in the whole surface, correcting vision at a single distance; it is used to treat certain refractive errors such as myopia, hyperopia or astigmatism. Meanwhile, intraocular lenses that correct presbyopia have advanced technology and materials that enable a high-resolution image at all distances – far, intermediate and near.

Con los implantes multifocales (o difractivos) actuales, la mayoría de los pacientes consigue independencia de gafas para la vida diaria con un rendimiento alto de la visión y una corrección óptica estable a lo largo del tiempo. Estos implantes corrigen la mayoría de los defectos refractivos: miopía, hipermetropía, el astigmatismo o la vista cansada, pero pueden surgir determinadas alteraciones visuales, como halos y deslumbramiento.

Hoy día existe una nueva clase de lente intraocular no difractiva que cuenta con una tecnología que ofrece la posibilidad de prescindir del uso de gafas para prácticamente todas las actividades diarias, proporcionando un rango de visión completo y un nivel de molestias visuales similar a una lente monofocal, aunando lo mejor de cada tipo de lente.

“La OMS calcula que este año se realizarán en el mundo más de 32 millones de cirugías de catarata” ¿Qué diferencias presentan las lentes de visión extendida sobre otras LIOs? Ofrecen muy buena calidad de visión a larga y media distancia, en condiciones fotópicas y mesópicas. Por otra parte, la independencia de gafas en visión cercana es inferior respecto a la que ofrecen las lentes trifocales, pero la visión del contraste y las alteraciones visuales son similares a las de las lentes monofocales.

En definitiva, aportan una gran calidad visual desde la visión lejana hasta la visión funcional de cerca sin la presencia de alteraciones visuales ni penalización de la sensibilidad al contraste asociadas con otro tipo de lentes. ¿En qué medida pueden contribuir a mejorar la funcionalidad visual de estos pacientes?

Permite a los que padecen de cataratas recuperar la visión de lejos, intermedia y cerca funcional. Es decir, podrán recuperar la visión para la mayor parte de tareas de la vida diaria: leer, conducir o ver una película. Y en aquellos pacientes en que prima sobre todo recuperar la independencia de las gafas para el desarrollo de la mayoría de estas actividades, les permite recuperar dicha independencia a pesar de poder necesitar la corrección para algunas tareas de visión cercana más exigentes. ¿Por qué es importante incorporar nuevas opciones de LIO para la cirugía de catarata?

La catarata es la patología ocular relacionada con la edad más frecuente y la principal causa de ceguera evitable. La Organización Mundial de la Salud calcula que en el año 2020 se realizarán en el mundo más de 32 millones de cirugías de catarata al año.

Escuchar las necesidades del paciente es fundamental, porque su satisfacción es alcanzable cuando se han interpretado muy bien estas. Por este motivo, es importante poder ofrecer a los pacientes que se operan de catarata la posibilidad de corregir la presbicia, sin perder la buena calidad visual que ofrecen las lentes monofocales, aunque puedan requerir de una corrección óptica para tareas muy concretas.

With current multifocal implants (or diffractive), most patients are able to do without eyeglasses for their daily life with a high visual performance and a stable optic correction throughout time. These implants correct most refractive errors: myopia, hyperopia, astigmatism or tired eyes, but may cause certain visual alterations such as halos and glare.

There is currently a new type of non-diffractive intraocular lens with a technology that offers the possibility of getting rid of eyeglasses for most of the daily activities, giving a full-range vision and a level of visual discomfort similar to a monofocal lens, combining the best of every type of lens.

What are the differences between an extended vision lens and the other IOLs? These offer a very good quality of vision at long and intermediate distance both in photopic and mesopic conditions. Furthermore, the independence from eyeglasses in near vision is less than that offered by trifocal lenses, but contrast vision and visual alterations are similar to monofocal lenses.

In short, they offer a great visual quality from far vision to near functional vision without visual alterations or detriment to the sensitivity to contrast associated to other type of lenses.

To what extent can this contribute to improve the visual functionality of these patients?

It enables patients with cataracts to recover far, intermediate and near functional vision. This means they will be able to recover their eyesight for most of their

daily life activities: read, drive or watch a film. And for those patients who mainly want to get rid of their eyeglasses to carry out these activities, it enables them to recover this independence despite the potential need for some correction for some of the most demanding near vision activities.

Why is it important to add new IOL options for cataract surgery?

Cataracts are the most common age-related ocular pathology and the main cause of avoidable blindness. The World Health Organisation estimates that in 2020 over 32 million cataract surgeries will take place all over the world.

The more the techniques and processes advance in quality and efficiency, the higher the demands of the patients, who are increasingly more demanding with the results they seek. It is paramount to listen to the needs of the patient, as it is possible to reach the patient’s satisfaction when one has correctly interpreted these needs. It is therefore important to be able to offer patients who are undergoing cataract surgery the possibility of correcting presbyopia without loosing the good quality of vision offered by monofocal lenses, even if this means the need for an optical correction for very specific tasks.

El coronavirus también repercute en la salud ocular.

Algunos informes recogidos por la Academia Americana de Oftalmología revelan que puede causar una conjuntivitis vírica aunque no suele revestir mucha gravedad. La enfermedad afecta principalmente a las vías respiratorias pero ya se han detectado casos de conjuntivitis con hallazgo positivo del virus en secreciones oculares. En otros estudios se han comunicado casos de pacientes con inflamación conjuntival importantes que afectaría más o menos al 1% de los pacientes infectados con COVID-19.

La forma de contagio es por gotas de saliva que expulsamos por la boca al hablar, al toser o al estornudar. También es reseñable el contagio por medio de las manos, cuando están contaminadas por el virus y nos tocamos la boca, la nariz y los ojos o bien cuando tocamos a otras personas. De tal forma que, además de mascarillas para evitar el contagio por efecto de gotas en aerosol, también es muy importante el lavado y desinfección constante y frecuente de las manos y mantener la distancia social siempre que sea posible.

Cómo identificar la conjuntivitis vírica Los síntomas más frecuentes de conjuntivitis vírica son picor, quemazón, lagrimeo, sensación de cuerpo extraño, inflamación de párpados y enrojecimiento.

Además, si el paciente presenta dificultad respiratoria, fatiga o tos acompañada de fiebre, el cuadro clínico puede corresponderse con COVID-19. Aún así, al ser la conjuntivitis por coronavirus poco frecuente, no debemos alarmarnos ya que podría ser causada por otros virus, bacterias o simplemente ser una alergia.

Prevención y tratamiento Además de las recomendaciones de prevención anteriormente descritas, se pueden aplicar otras más concretas para evitar una conjuntivitis vírica. Como hemos indicado, esta enfermedad tiene una gran facilidad de contagio. Para evitarla, es recomendable el uso de gafas de protección, y no compartir toallas, sábanas ni productos cosméticos.

Los especialistas aconsejan acudir al oftalmólogo ante los primeros síntomas. El tratamiento de la conjuntivitis vírica dependerá del tipo y gravedad, aunque en la mayoría de los casos puede resolverse con colirios y en pocos días.

Es preferible sustituir las lentillas por gafas durante la pandemia.

Extremar las medidas de higiene, también con las lentillas En estos momentos es preferible manipular lo menos posible los ojos y su entorno, así como la cara en general. Por eso, es mejor limitar el uso de lentillas y los pacientes que las usen, deben extremar la higiene. Si las lentillas son reutilizables, deben seguir un protocolo minucioso para evitar contagios.

Antes y después de manipularlas, es necesario lavarse bien las manos. Las lentillas deben limpiarse con frote mecánico y un limpiador adecuado, en una solución salina estéril. Además, tienen que ser desinfectadas con peróxido de hidrógeno al 3% y retrasar la neutralización hasta, al menos, seis horas después de que el peróxido haya actuado sobre la lente en el frasco indicado para ello. How Does Coronavirus Affect The Eyes?

The disease caused by COVID-19 has a myriad of clinical manifestations. Some studies prove that it can cause viral conjunctivitis. Dr Luis Fernández-Vega Cueto-Felgueroso explains its symptoms and how to prevent them.

Coronavirus also affects ocular health. Some reports by the American Academy of Ophthalmology state that the disease can cause viral conjunctivitis, although usually it is not very severe. This disease mainly affects the respiratory tract but there have been cases of conjunctivitis with positive findings of the virus in ocular secretions. Other studies have reported cases of patients with significant swelling of the conjunctiva affecting more or less 1% of patients infected with COVID-19.

The mode of transmission is through droplets of saliva that we expel while talking, coughing or sneezing.

It is also worth noting that it can be spread through the hands, when these are contaminated with the virus and we touch our mouth, nose or eyes, as well as when we touch someone else. Therefore, on top of using masks to avoid infection due to aerosol droplets, it is also very important to constantly and frequently wash and disinfect our hands and maintain social distancing whenever possible.

How to identify viral conjunctivitis The most common symptoms of viral conjunctivitis are itchiness, burning sensation, tearing, foreign body sensation, swelling of the eyelids and reddening. Furthermore, if the patient presents respiratory distress, fatigue or cough with high temperature, the clinical presentation can correspond to COVID-19.

However, as conjunctivitis is not frequently caused by coronavirus, we should not be alarmed as other viruses, bacteria or even just an allergy can cause it.

Mejor gafas que lentes de contacto durante la pandemia Con todo ello, sería aconsejable que no se utilizasen las lentes de contacto y las sustituyesen, siempre que fuera posible y de manera excepcional mientras dure la pandemia de coronavirus, por las gafas correctoras, ya que si usamos las lentes de contacto, al manipularlas podemos contagiarnos y contagiar a los demás, si después tocamos puertas, materiales o a través de las manos. Así lo ha manifestado la Sociedad Española de Oftalmología. Por su parte, la OMS también reconoce que el uso de gafas puede ser importante para reducir la posibilidad de infectarse y de transmitir el virus. Así que las gafas de vista y sol pueden también ser una buena barrera de protección frente al virus.

Prevention and treatment As well as the aforementioned prevention advice, there are other more specific recommendations to avoid a viral conjunctivitis. As we have mentioned, this disease is easily spread. To avoid it, it is advisable to use safety goggles, and avoid sharing towels, sheets and cosmetic products.

Specialists recommend visiting the ophthalmologist at the fist symptoms. The treatment of the viral conjunctivitis will depend on the type and severity, although in most cases it will be solved with eye drops in just a few days.

It is advisable to replace contact lenses with eyeglasses during the pandemic.

Hygiene measures must be stepped up with contact lenses too At the present time, it is best to manipulate the eyes and its surroundings, as well as the face in general, as little as possible. Thus, it is best to limit the use of contact lenses and those patients who do use them should maximise hygiene standards. If the contact lenses are reusable, patients should follow a strict protocol to avoid infection. It is necessary to wash one’s hands before and after touching the contact lenses. These should be cleaned using a mechanical rub and an appropriate cleanser, in a sterile saline solution. Furthermore, they have to be disinfected with hydrogen peroxide 3% and postpone neutralisation up to, at least, six hours after the peroxide has acted on the lens in the bottle suitable for this purpose. Subsequently, a neutralisation tablet will be introduced in this same container. An hour later, the contact lenses will be ready to be used.

Better eye glasses than contact lenses during the pandemic Thus, it would be advisable to avoid using contact lenses and to replace these, whenever possible and exceptionally for the duration of the coronavirus pandemic, with eyeglasses. According to the Spanish Society of Ophthalmology (Sociedad Española de Oftalmología), if we use contact lenses, when manipulating them we can infect others as well as ourselves when we touch doors, different materials or through our hands. The WHO has also acknowledged that the use of eyeglasses can be an important factor to decrease the possibility of getting infected and spreading the virus. Therefore, prescription eyeglasses and sunglasses can be a good protection barrier against the virus.

Itchiness, burning sensation, tearing, and foreign body sensation, are some of the symptoms of viral conjunctivitis associated to COVID-19.

Son la mejor opción cuando las condiciones del ojo no permiten realizar una cirugía láser, dándoles una oportunidad de conseguir su rehabilitación visual. ¿Qué diferencias hay entre la cirugía con lente EVO ICL y una cirugía Láser convencional?

Son dos técnicas completamente diferentes, no comparables entre sí y tampoco condicionantes.

Dependiendo de las características personales del paciente (edad, profesión o aficiones), y de las características anatómicas del ojo como su tamaño, el espesor de la córnea y el tamaño de la cámara anterior, elegiremos un procedimiento u otro. De hecho, en ocasiones, podemos combinar ambos tratamientos.

Por esta razón es muy importante realizar estudios personalizados. Habitualmente empleamos el láser en graduaciones bajas, y la lente ICL en graduaciones altas, pero como hemos comentado anteriormente, existen lentes de baja potencia que utilizaremos en casos concretos. La diferencia fundamental entre ambas técnicas es la mejor calidad visual que se obtiene con la ICL. Como paciente, ¿Cómo es el postoperatorio? ¿Cuánto tiempo necesita para reincorporarse a su rutina diaria?

El postoperatorio después del implante de una lente ICL es, en general, cómodo y sencillo. La primera semana el paciente deberá tener cuidado de no darse un golpe ni frotarse los ojos. Es importante cumplir correctamente con el tratamiento postoperatorio para evitar infecciones y conseguir una adecuada rehabilitación visual. Además, los primeros días será necesario dormir con protectores oculares para evi-

tar frotamientos o golpes nocturnos. Por lo demás, la rehabilitación visual se logra en la primera semana, con una calidad visual similar a la que se obtiene con lentes de contacto. ¿Qué cuidados especiales debe llevar un paciente operado con lentes EVO ICL? Las recomendaciones generales durante la primera semana del postoperatorio son evitar cualquier tipo de golpe o deporte de contacto. Se podrán realizar deportes aeróbicos como bicicleta estática o caminar desde la primera semana. Será recomendable que eviten lugares sucios, playas o piscinas los primeros 15 días. A partir del mes de la cirugía podrán hacer vida completamente normal.

Finalizando esta entrevista, ¿podría resumir las principales ventajas de la cirugía refractiva con lente fáquica EVO ICL? Las lentes epicristalinianas están diseñadas para la corrección de la miopía, hipermetropía y astigmatismo. El paciente debe saber que se sitúan entre el cristalino y la cara posterior del iris, siendo una técnica aditiva, y no sustitutiva del cristalino. La técnica quirúrgica consiste en introducir la lente plegada dentro del ojo a través de una micro-incisión, y colocarla en su posición correcta. Se trata de una lente compuesta por un material biocompatible y resistente, que no se degrada con el paso de los años.

Existen diferentes tamaños disponibles para adaptarse a las diferencias anatómicas de cada ojo. Escoger el tamaño adecuado es fundamental para que la adaptación de la lente sea la correcta. What is the difference between a surgery with an EVO ICL lens and a conventional laser surgery?

These are two completely different techniques, which cannot be compared to one another or their constraints. Depending on the personal characteristics of the patient (age, profession and hobbies), and the anatomical characteristics of the eye such as size, thickness of the cornea and size of the anterior chamber, we will choose one procedure or another.

In fact, sometimes we can combine both treatments.

It is therefore paramount to carry out customised studies. We usually use laser in cases of low refractive power, and ICL lenses in cases of high refractive power, but as we have mentioned before, there are lenses with low prescription that we will use in certain cases. The fundamental difference between both techniques is the best visual quality obtained with the ICL.

What special care does a patient who has undergone surgery with an EVO ICL lens has to take?

Overall recommendations for the first week following surgery are to avoid any kind of blow or contact sport. Aerobic sports such as static cycling or walking can be performed from the first week. It is advisable to avoid dirty places, beaches or swimming pools for the first 15 days. Patients will be able to resume completely normal life 1 month following surgery.

To end the interview, would you kindly summarise the advantages of refractive surgery with an EVO ICL phakic lens?

Epicrystalline lenses are designed for the correction of myopia, hyperopia and astigmatism. The patient must be aware that this are placed between the crystalline and the posterior face of the iris; this an additive technique and therefore the crystalline is not substituted. The surgical technique consists of introducing the lens folded into the eye through a micro-incision and placing it in the correct position.

The lens is made of a biocompatible and resistant material that does not degrade over time. There are different sizes available to adapt to the anatomical differences of each eye. It is key to choose the appropriate size for a correct adaptation of the lens.

As a patient, what does post-operatory look like?

How long does it take for the patient to resume his daily routine?

The post-op following the implantation of an ICL lens is usually comfortable and simple. During the first week the patient will have to be careful to avoid bumping or rubbing the eye. It is important to follow the post-op treatment appropriately to avoid infections and achieve an adequate visual recovery. Furthermore, during the first days it will be necessary to sleep with eyewear protection to avoid bumps or rubbing at nigh time. Otherwise, visual recovery is achieved during the first week, with a visual quality similar to that obtained with contact lenses.

Tengo 50 años y hace 5 años me realizaron un trasplante de córnea, pero solo de las capas más superficiales. Cuando voy a las revisiones me dicen que estoy muy bien, pero en el día a día no veo por ese ojo porque la graduación que me ha quedado es muy alta y no la puedo llevar en mi gafa. ¿Hay alguna solución?

En primer lugar, la situación en la que se encuentra es relativamente frecuente. Después de un trasplante de córnea, es habitual presentar graduaciones residuales que, en ocasiones, pueden

ser altas e incompatibles con una gafa, debido a la diferencia de graduación de un ojo con el otro. Existen métodos para realizar una corrección de la refracción residual después de un trasplante laminar anterior, igual que sucede con el resto de trasplantes de córnea. Dependiendo del tipo de graduación (astigmatismo, miopía y/o hipermetropía) podremos elegir una técnica que consista en lentes intraoculares aditivas o sustitutivas del cristalino; o técnicas de fotoablación con láser o moldeado corneal con segmentos intracorneales. La re-

habilitación visual de un paciente trasplantado termina cuando obtenemos una graduación compatible con el ojo contralateral.

Aunque las técnicas láser son las más conocidas y las más empleadas en los últimos años, existen otras posibilidades para la corrección de la miopía, como son las lentes intraoculares que se implantan delante del cristalino y que reciben el nombre de ICL (Implantable Collamer Lens). El hecho de tener una córnea fina nos puede limitar la fotoablación con láser de Excímero, pero podemos optar a otros procedimientos alternativos que nos posibilitan la independencia de la gafa y una buena calidad visual.

Tengo 65 años. Cuando era pequeña me diagnosticaron de miopía y queratocono. Siempre he estado bien con mis lentes de contacto duras, pero en estos últimos años he perdido mucha visión. En la ultima revisión que me hice me dijeron que mi queratocono estaba bastante avanzado y que, además, tenía cataratas, y que me tenia que operar. ¿Qué debería hacer?

En su caso, antes de operar la catarata, lo primero que deberíamos hacer es regularizar la córnea. Un queratocono avanzado requiere tratamiento específico según el grado y el estado de la córnea. Actualmente disponemos de nuevos métodos diagnósticos y terapéuticos para queratoconos avanzados y de largo tiempo de evolución. En su caso lo más probable es que haya que implantar unos segmentos de anillo intracorneales con la idea de corregir el queratocono antes de operar la catarata.

Cuando se haya conseguido rehabilitar la córnea, se puede proceder a la cirugía de catarata,

Tengo 55 años y 4 dioptrías de hipermetropía. Me gustaría operarme, pero me han dicho que tiene que ser con una lente intraocular que sustituya el cristalino. Las lentes que me tienen que poner, ¿podrían corregirme también la presbicia? Teniendo en cuenta mi edad, ¿tendría que cambiármelas más adelante? ¿tendría que operarme de catarata en un futuro?

Efectivamente. En su caso la mejor opción quirúrgica sería el implante de una lente intraocular. Actualmente disponemos de lentes monofocales, bifocales y trifocales. En su caso, lo más habitual es que podamos emplear lentes trifocales, que corrigen la hipermetropía y que otorgan una mayor independencia de la gafa para la visión lejana, intermedia y cercana. De todas maneras, para poder determinar que tipo de lente es la más adecuada y personalizar el tratamiento deberíamos realizar un estudio completo y detallado.

En cuanto a el tiempo de duración, estas lentes que empleamos actualmente tienen unos materiales óptimos y biocompatibles, que no necesitarían ser reemplazados con el paso de los años.

La sustitución del cristalino natural por un cristalino artificial implica que no va a tener catarata en un futuro y, por lo tanto, ese problema estaría a la vez resuelto.

I am 50 years old and I underwent a corenal transplant 5 years ago, but just of the most superficial layers. When I go for my check-ups I am told that I am doing very well, but on a day-to-day basis I do not see through that eye because the prescription left is too high and my eyeglasses do not support it. Is there any solution?

First of all, your situation is fairly common. It is normal to present residual prescription following a corneal transplant; these can sometimes be high and incompatible with eyeglasses due to the difference in prescription between both eyes.

There are methods to correct residual refraction following an anterior lamellar transplant, as is the case with the rest of corneal transplants.

Depending on the type of prescription (astigmatism, myopia and/or hyperopia) we can decide on a technique involving additive intraocular lenses or those that substitute the crystalline; or laser photoablation techniques or corneal moulding with segments intracorneals. The visual recovery of a patient who has undergone a transplant ends when we obtain a prescription that is compatible with the contralateral eye.

I am 34 years old and I have 4 dioptres of myopia. When I went to an eye check-up intending to undergo surgery I was told that my cornea was too thin and that therefore it was not advisable. Is there any other option for my case?

In your case, to be able to answer your question, we would need to carry out a full and customised study of the anatomical and functional status of your eye.

Even though laser techniques are the most well-known techniques and the most commonly used in the last few years, there are other options to correct myopia, such as additional lenses that are implanted in front of the crystalline and which receive the name of ICL (Implantable Collamer Lens).

Having a thin cornea might restrict the use of Excimer laser photoablation, but we can carry out alternative procedures that provide independence from eyeglasses and a good visual quality.

I am 65 years old. When I was young I was diagnosed with myopia and keratoconus. I have always managed really well with my hard contact lenses, but these last few years I have

lost a lot of vision. During my last check-up I was told that my keratoconus was fairly advanced and that I also had cataracts and therefore needed to undergo surgery. What should I do?

In your case, before undergoing cataract surgery, the first step is to rehabilitate the cornea. An advanced keratoconus requires specific treatment depending on the extent and status of the cornea. We currently have new diagnostic and therapeutic methods for advanced keratocouns with a long evolution time. In your case, it is very likely that we need to implant intracorneal ring segments to correct the keratoconus before operating the cataract.

Indeed. In your case the best surgical option is the implantation of an intraocular lens. There are different options currently available: monofocal, bifocal and trifocal. In your case, we would most likely be able to use trifocal lenses, which correct hyperopia and provide a higher independence from eyeglasses for far, intermediate and near vision. However, to determine which lens is more appropriate and customise the treatment we would need to carry out a complete and exhaustive study.

Once the cornea is rehabilitated the cataract surgery can take place; the procedure will have to be carried out with a specific and customised technique where choosing the lens and calculating its power are two fundamental premises.

La higiene y la desinfección son cruciales para el Instituto, más aún en tiempos de coronavirus.

El departamento de limpieza está formado por once personas que, además, se encargan de lavar y planchar cada día la ropa de cama y los uniformes de todos sus compañeros.

Medidas extraordinarias ante el COVID-19 La organización de la jornada en el departamento de limpieza se ha modificado para hacer frente a la crisis de Covid-19. En las últimas semanas se ha intensificado la desinfección de superficies tanto en las zonas de recepción como en los despachos y consultas. También se han reforzado las medidas de protección para todos los empleados, incluidos el equipo de limpieza, que ahora utiliza mascarilla y bata desechable.

Acuden en cuestión de minutos y desinfectan la sala antes de que acceda el siguiente paciente. “La coordinación de todo el equipo resulta indispensable para cumplir con las tareas cotidianas y tener capacidad de

reacción ante los imprevistos”, señala Rosi Marrón.

El servicio de lavandería y plancha está a pleno rendimiento durante las horas que la clínica permanece abierta, con dos personas encargadas de estas tareas. Allí llega la ropa de cama de pacientes hospitalizados y sus acompañantes y también los uniformes de los empleados.

Lo que más valora el equipo de limpieza es el buen ambiente y la profesionalidad del equipo.

Raquel Reguera, encargada del departamento, explica: “El día a día, a pesar de ser exigente, resulta muy llevadero”. El buen ambiente se palpa en cualquier conversación que tienen entre ellas.

Un motivo más para reconocer el talento y la entrega de todo el departamento de limpieza.

Cleaning, Paramount in Times of Coronavirus Almudena, Raquel, Estela, Beatriz, Verónica, Rosi, Carmen, Aníbal, Margarita and María José are responsible for keeping all the areas and surfaces of the Instituto Oftalmológico Fernández-Vega clean and hygienic, a task that has been intensified in the last few months.

At the end of the working day, when the medical team of the Instituto Fernández-Vega finishes with consultations and performing surgeries is the turn of the cleaning department. Rosi Marrón, Carmen Suárez, Margarita Alonso, María José López and Aníbal Villar constitute the night team.

Aníbal, who started working at the IOFV when its headquarters were at calle Uría, is the most senior member: He has seen the Institute grow and values above all the human team that makes it possible: “the Institute is like a big family that has grown over the years and of which we all feel an essential part.” Years later came Carmen Suárez: “When hospitalised patients are the only ones left at the hospital, we keep working to ensure all the facilities are ready first thing in the morning,” she states.

Hygiene and disinfection are paramount for the Institute, even more so in times of coronavirus.

Extraordinary measures to face COVID-19 The schedule of the cleaning department has been modified to address the Covid-19 crisis. During the last weeks the disinfection of surfaces in the reception areas and in the consultation rooms and offices has been intensified. Protection measures have also been reinforced for all staff members, including those in the cleaning department who now use disposable masks and overalls.

Coordination is key to an exhaustive cleaning The cleaning department of the Institute organises the day in two morning shifts, two afternoon shifts and one night shift.

Raquel Reguera is the head of the department and together with Almudena Rodríguez, Estela Sánchez, Beatriz Álvarez y Verónica Fernández constitute the morning team. The cleaning and disinfection of the different areas takes place while the clinical practice runs as usual.

The department has two telephone lines that enable coordination.

Sometimes, while they are hoovering the carpet in the assembly hall one of these two telephone lines rings. One of the physicians is calling because they have had a patient with conjunctivitis. They go within minutes and disinfect the room before the next patient comes in. “The coordination of the whole team is paramount to keep up with the daily tasks and to be able to face any unexpected events,” says Rosi Marrón.

ment and professionalism of the team. Raquel Reguera, head of the department explains: “Everyday work, despite being hard, is more manageable.”

The good environment can be seen in any given conversation among them. This is one more reason to acknowledge the talent and devotion of the whole cleaning department.

With COVID-19 the use of operating room disinfectant has become widespread to the full facilities of the Institute.

La Fundación de Investigación Oftalmológica del Instituto Fernández-Vega crea un nuevo sistema para predecir el glaucoma

Los Dres. Héctor González, Lydia Álvarez y Montserrat García forman el grupo de Genética Ocular de Glaucoma de la Fundación de Investigación del Instituto Fernández-Vega.

El equipo ha desarrollado un panel de biomarcadores que ayuda a predecir la enfermedad en un 80% de los casos estudiados. El avance abre la vía a realizar un diagnóstico en base a una prueba in vitro como un análisis de sangre.

El grupo de Genética Ocular de la Fundación de Investigación del Instituto Fernández-Vega, formado por los doctores Héctor González, Lydia Álvarez y Montserrat García, ha dado un gran paso en el diagnóstico y tratamiento del glaucoma, una enfermedad que aun no tiene cura. Los investigadores han logrado desarrollar un panel de biomarcardores que facilita la predicción de la enfermedad hasta en un 80% de los casos estudiados. Es el resultado de nueve años de trabajo.

El avance abre la vía a realizar un diagnóstico en base a una prueba in vitro como un análisis de sangre.

De momento, este panel se utiliza en pruebas de laboratorio y está centrado en pacientes con glaucoma primario de ángulo abierto o glaucoma pseudoexfoliativo, así como en estudios con modelos animales.

El desarrollo del mismo podría suponer en el futuro la posibilidad de realizar un diagnóstico de glaucoma en base a una prueba in vitro como un análisis de sangre.

“Desde la Fundación de Investigación Oftalmológica estamos consiguiendo prometedores avances en el diagnóstico y tratamiento del glaucoma”, señala Héctor González Iglesias, investigador principal del Grupo de Genética Ocular de la FIO. “Nuestras investigaciones están centradas en tres áreas clave: genética del glaucoma, mecanismos de neuroprotección y búsqueda de biomarcadores”, añade el investigador.

Además del desarrollo de este panel de biomarcadores, la FIO ha conseguido identificar algunas mutaciones en el gen LOX1 (lysyl-oxidasa like protein 1) en la población española, que suponen el principal factor de riesgo genético para desarrollar glaucoma pseudoexfoliativo. Asimismo, en un estudio multicéntrico internacional en el que ha participado la FIO, también ha identificado una variante en este mismo gen que protege hasta 25 veces más a la persona que la porta de desarrollar este tipo de glaucoma.

Más allá del diagnóstico y el campo de la genética, la Fundación de Investigación Oftalmológica también

está trabajando en nuevos métodos de tratamiento del glaucoma.

Una de las líneas de investigación más novedosa presentada en la Tesis doctoral del oftalmólogo Dr. Andrés Fernández-Vega Cueto-Felgueroso es el estudio de nuevos mecanismos de neuroprotección basados en el empleo de filtros y conversores espectrales en lentes intraoculares. Esta nueva estrategia propone la creación de un conversor capaz de transformar la luz azul, nociva para los ojos, en luz roja, beneficiosa para la retina, y de esta manera emplearla como un sistema de neuroprotección durante el desarrollo del glaucoma.

“Investigar el glaucoma supone un gran reto, ya que nos enfrentamos a una enfermedad ocular neurodegenerativa con una etiología diversa y que afecta en última instancia a las células ganglionares de la retina, las neuronas que conectan con nuestro cerebro”, señala el Dr. Andrés Fernández-Vega Cueto-Felgueroso. “Estamos estudiando el glaucoma desde una perspectiva multidisciplinar, contemplando tanto los aspectos genéticos, oftalmológicos y neurológicos, apunta.

La FIO también trabaja en la creación de nuevos mecanismos de neuroprotección con lentes intraoculares.

Un millón de personas podría padecer glaucoma en España y casi la mitad no lo sabe Más de 80 millones de personas sufren glaucoma en todo el mundo, y se calcula que alrededor de un millón podría padecerlo en España.

Además, casi la mitad de las personas que viven con esta enfermedad no son conscientes de ello. El glaucoma, una enfermedad asintomática y sin cura, supone la principal causa de ceguera junto con las cataratas, la DMAE y la diabetes.

“Muchos ciudadanos solo visitan al oftalmólogo cuando muestran signos de enfermedad en su visión y en el caso de padecer glaucoma, la visión que se ha perdido ya no se puede recuperar pues actualmente no tiene cura.

Por ello, debemos acudir periódicamente al oftalmólogo a partir de los 40-50, ya que detectarlo a tiempo es fundamental para evitar su avance”, apunta el Dr. Pedro Pablo Rodríguez Calvo, director de la unidad de Glaucoma del Instituto Oftalmológico Fernández-Vega.

The Fundación de Investigación (Research Foundation) of the Instituto Fernández-Vega has taken a great step forward in the diagnosis and treatment of glaucoma, a disease that currently has no cure.

The specialists have been able to develop a panel of biomarkers that allows the prediction of the disease in up to 80% of the cases studied. This is the result of 9 years of work.

For the time being, the panel is used in lab tests and is devoted to patients with open-angle primary glaucoma or pseudoexfoliation glaucoma, as well as in studies with animal models. Its development may mean the possibility, in the future, of diagnosing glaucoma based on an in vitro test such as a blood test.

“From the Fundación de Investigación Oftalmológica we are making promising breakthroughs in the diagnosis and treatment of glaucoma,” points out señala Héctor González Iglesias, main researcher of the Team of Glaucoma Ocular Genetics of the FIO. “Our research in focused in three main areas; genetics of glaucoma, neuroprotection mechanisms and search of biomarkers,” adds the researcher.

As well as this panel of biomarkers, the FIO has been able to identify some gene LOX1 (lysyl-oxidasa like protein 1) mutations in the Spanish population, which are the main genetic risk factor to develop pseudoexfoliation glaucoma. Likewise, in an international multicentre study in which the FIO has participated, a gene variant has also been identified in this same gene protecting up to 25 times more the individual that carries it from developing this type of glaucoma.

Besides the diagnosis and the field of genetics, the Fundación

de Investigación Oftalmológica is also working in new treatment approaches for glaucoma. One of the most innovative research lines presented in the Doctoral Thesis of ophthalmologist Dr Andrés Fernández-Vega Cueto-Felgueroso is the creation of new neuroprotection mechanisms based in the use of filters and spectral conversion in intraocular lenses. This new strategy proposes the creation of a conversor able to transform blue light, harmful for the eyes, in red light, beneficial for the retina, thus using it as a neuroprotection system during the progression of glaucoma.

“Researching glaucoma is a great challenge as we face a neurodegenerative ocular disease with a different aetiology that ultimately affects the ganglion cells in the retina, the neurons that connect with our brain,” points out Dr Andrés Fernández-Vega Cueto-Felgueroso. He also adds, “We are studying glaucoma from a multidisciplinary perspective, taking into account genetic, ophthalmologic and neurologic aspects”.

The FIO also works in the creation of new neuroprotection mechanisms with intraocular lenses.

A million individuals may suffer from glaucoma in Spain and nearly half of them are unaware of it Over 80 million individuals suffer from glaucoma all over the world, and it is estimated that a million could have it in Spain. Furthermore, nearly half of the individuals who live with this disease are not aware of it.

Glaucoma, an asymptomatic cure-less disease is the main cause for blindness together with cataracts, AMD and diabetes.

“Many individuals only visit the ophthalmologist when they show signs of poor eyesight but if they have glaucoma the vision that has already been lost cannot be recovered as there is currently no cure for this disease. Therefore, we should visit regularly the ophthalmologist from the age of 40-50, as an early detection is paramount to stop the progression of the disease,” says Dr Pedro Pablo Rodríguez Calvo, head of the Unit of Glaucoma at the Instituto Oftalmológico Fernández-Vega.

El plasma rico en factores de crecimiento (PRGF®) podría proteger a la retina contra enfermedades neurodegenerativas como la degeneración macular asociada a la edad (DMAE). El hallazgo ha sido publicado en la prestigiosa revista International Journal of Molecular Sciences por el grupo de Neurobiología de la Retina de la Fundación de Investigación Oftalmológica (FIO) del Instituto Oftalmológico Fernández-Vega (IOFV) de Oviedo.

“Aunque se trata de un estudio en fase experimental, esta investigación abre una nueva forma de tratamiento para algunas enfermedades de la retina”, comenta la Dra. Susana del Olmo, investigadora principal en el grupo de Neurobiología de la Retina de la FIO. “En la investigación realizada se han desarrollado varios modelos biológicos que demuestran que el plasma rico en factores de crecimiento estimula los sistemas antioxidantes más importantes que se encuentran en nuestras células”, señala la investigadora.

El estrés oxidativo producido tanto por los factores endógenos como la edad o la predisposición genética, sumado a los factores exógenos como el estilo de vida, el tabaquismo o la obesidad están implicados en el desarrollo de enfermedades neurodegenerativas. Además, otros elementos como la luz visible de longitud de onda corta, muy presente en la sociedad actual, podrían suponer un riesgo añadido.

Estos factores, junto con la ausencia de tratamientos efectivos para algunas de las formas de estas degeneraciones, hacen que sea de máxima prioridad encontrar un tratamiento que ayude a mejorar el pronóstico de los pacientes.

La investigación desarrollada por la FIO ha sometido a los tejidos a una exposición de luz azul, tras la que han tratado a las células con plasma rico en factores de crecimiento. Mediante el análisis de diversas técnicas, y apoyado por los resultados, se ha llegado a la conclusión de que este plasma podría ser utilizado como tratamiento para reducir el efecto nocivo producido por agentes oxidantes, estimulando el propio sistema de defensa del cuerpo.

El estrés oxidativo producido por el envejecimiento, el tabaquismo o la obesidad está implicado en el desarrollo de enfermedades neurodegenerativas.

La FIO y el IOFV, referentes en el uso del plasma rico en factores de crecimiento La FIO y el IOFV han sido pioneros en la investigación sobre la aplicación del plasma rico en factores de crecimiento y su uso clínico en la superficie ocular está instaurado en el Instituto Oftalmológico Fernández-Vega (IOFV) desde hace tiempo. Ahora este grupo de investigación va un paso más allá para su aplicación también en la retina.

Este proyecto en cuestión se suma a los que está desarrollando actualmente la FIO con sus grupos de investigación especializados en superficie ocular, neurobiología de la retina y glaucoma, y cuenta con la financiación del Instituto de Salud Carlos III, Ministerio de Economía y Competitividad y la Unión Europea a través del Fondo Europeo de Desarrollo Regional (FEDER) (PI17/01549).

El plasma o suero rico en factores de crecimiento se obtiene a partir de la sangre del propio paciente, evitando posibles rechazos del sistema inmune.

La técnica consiste en una sencilla extracción de sangre para su posterior procesado y obtención de los llamados factores de crecimiento, que será el producto infiltrado para la regeneración de los tejidos dañados.

El plasma rico en factores de crecimiento se obtiene a partir de la sangre del propio paciente, evitando posibles rechazos del sistema inmune.

“Even though the study is in experimental stage, this research opens a new treatment path for some diseases of the retina,” says Dr Susana del Olmo, main researcher of the team of Neurobiology of the Retina at FIO. “In the research several biological models have been develop that prove that plasma rich in growth factors stimulates the most important antioxidant systems in our cells,” adds the researcher.

PRGF could be used as treatment to reduce the harmful effect produced by oxidising agents.

The oxidative stress caused by endogenous factors such as age or genetic predisposition, added to exogenous factors such as lifestyle, smoking or obesity are involved in the development of neurodegenerative diseases.

Furthermore, other elements such as short-wavelength visible light, very present in current society, could be an additional risk. These factors, together with the absence of efficient treatments for some of these degenerations, makes the need to find a treatment that helps to improve the prognosis of this patients a top priority.

The research carried out by the FIO has subjected the tissues to blue light and has subsequently treated the cells with plasma rich in growth factors.

Through the analysis of different techniques and supported by the results, the conclusion reached is that this plasma could be used as treatment to reduce the harmful effect caused by oxidising agents, stimulating the defence system of the body.

This project joins the ones that the FIO is currently carrying out with its research teams specialised in ocular surface, neurobiology of the retina and glaucoma, and is financially supported by (PI17/01549) of the Instituto de Salud Carlos III, Ministry of Economy and Competitiveness and the European Union through the European Regional Development Fund (ERDF).

Plasma or serum rich in growth factors is obtained from the patient’s own blood, thus avoiding a potential rejection from the immune system. The technique consists of a simple blood extraction that is subsequently processed to obtained the growth factors, which will be the product infiltrated for the regeneration of the oxidised tissues.

La diseñadora y empresaria Casilda Finat, vizcondesa de Rías, acaba de inaugurar tienda en Oviedo (González del Valle, 6). Tamara Falcó y Marta Hazas lucen con frecuencia las joyas y complementos de su firma, que han logrado conquistar a más de 178.000 seguidores en Instagram.

¿Cómo es la mujer que luce las joyas de Casilda Finat?

Es una mujer coqueta y presumida que sabe el poder de las joyas para hacerte sentir guapa y especial y a la que no le gusta gastar una millonada en ello. ¿Por qué decidiste hacer de las joyas y los complementos tu profesión?

Empecé a vender unos anillos a mis amigas un poco como diversión y para sacarme un dinero extra y acabó siendo mi profesión sin esperar nunca que lo fuese.

Me inspiro en todo lo que me rodea, objetos, luces, colores, naturaleza, lugares... ¿Cómo te defines? ¿Empresaria, diseñadora u orfebre?

Creo que empresaria y diseñadora, las dos cosas me divierten muchísimo.

Madrid, Sevilla y, ahora, Oviedo. ¿Por qué has elegido la capital asturiana para abrir tu nueva tienda?

Siempre he tenido un vínculo muy especial con Asturias, mi abuela es asturiana, de pequeña pasaba todos los veranos una semana en Gijón y cuando descubrí Oviedo me enamoré. Es una ciudad de la que cada rincón es bonito, señorial, donde no se puede comer mejor y la gente es acogedora, maravillosa y con mucha clase. Además siempre me encantó para ir de compras por lo que consideré que no podía ser mejor sitio para una de mis tiendas. ¿Qué colecciones has elegido para la nueva tienda de Oviedo?

Todas, quiero que no falte nada. ¿Qué importancia tienen Internet y las redes sociales para tu negocio?

Para mí, mucha. La gran acogida que he tenido ha sido gracias a mis más de 178.000 seguidores de Instagram, de los cuales muchos estaban en Oviedo y cada vez que venía me pedían que abriese una tienda aquí. ¿Por qué confías tu salud y la de tu familia en el Instituto Fernández-Vega?

Nunca he tenido una mala referencia del IOFV y cuando tuve que operarme no dudé en elegirlo como principal opción. Muchos amigos muy cercanos y miembros de mi familia ya lo habían hecho, como mi madre, y siempre han estado muy contentos. Es de las mejores decisiones que he tomado. El trato es maravilloso.

¿Cómo ha sido tu experiencia en la clínica? ¿En qué ha cambiado tu vida tras la cirugía de lente intraocular?

Soy otra desde que no utilizo gafas. Es una maravilla por fin poder ver sin tener que estar esclavizada a un objeto.

What is a woman who wears the jewellery of Casilda Finat like?

She is a coquettish woman who likes to get primped and knows the value of jewellery to make one feel pretty and special, and a woman who does not want to spend a fortune in achieving this.

Why did you decide to make a career of jewellery and accessories?

I started selling some rings to my friends for fun and to have some extra cash, and it ended up being my career without ever thinking about it.

Where do you get your inspiration from to create your jewels? How much of Casilda Finat is there in them?

I get inspiration from the world around me, objects, light, colours, nature, places…

How would you define yourself, as businesswoman, designer or goldsmith? I think that as businesswoman and designer, I really enjoy both.

Madrid, Sevilla and now Oviedo.

Why have you chosen the capital of Asturias to open your new shop? I have always had a very close bond with Asturias, my grandmother is from Asturias, when I was young I would spend a week in Gijon and when I discovered Oviedo I fell in love. In this city every corner is beautiful, majestic, and food couldn’t be any better, people are welcoming and great, with a lot of class. Furthermore, I always loved going shopping here so I thought there would be no better place for one of my shops.

What collections have you chosen for your new shop in Oviedo?

All of them, I don’t want anything to be missing.

What is the importance of the Internet and social networks for your business?

The importance for me is enormous. The great welcome I have received is thanks to the over 178 000 followers I have on Instagram, many of whom are in Oviedo and used to ask me to open a shop here every time I came.

Why do you entrust your health and that of your family to the Instituto Fernández-Vega?

I have never heard of a bad reference of the IOFV and when I had to undergo surgery I did not hesitate to choose it as my main option. Many of my close friends and relatives had already done so, such as my mother, and they have always been very happy.

It is one of the best decisions I have ever made. Everyone treats you marvellously.

What was your experience like at the clinical practice? How has your life changed following intraocular lens surgery?

I am a new woman since I don’t use eyeglasses. It is great to be able to see without being tied to an object.

AL PACINO Dentro de cada uno acecha su caricatura. Nos pasa a todos, pero especialmente a quienes se dedican al espectáculo. Será por la constante exposición al ojo ajeno encarnando personajes que nunca son el actor, pero que, como los invasores venidos del espacio, ocupan su cuerpo y comprometen su identidad. “Caricatura” viene del italiano “caricato”, exagerado y, según la RAE, designa a quien se especializa en imitar a personajes conocidos, en ocasiones improvisando. Es muy fina la frontera que separa la interpretación de la caricatura. Intentando marcar una línea divisoria, diríamos que el caricato no hace suyo el personaje, se queda en la superficie evitando la introspección. Lo que resulte de traspasar la línea dependerá del talento interpretativo, de si se trata de un mero imitador o de un verdadero artista.

Tomemos el ejemplo de Al Pacino en El irlandés, la última, larga y discutida película de Martin Scorsese.

Su creación del sindicalista Jimmy Hoffa abunda en exabruptos, gestos destemplados, explosiones de cólera. ¿Era así Jimmy Hoffa? Salvo los muy enterados casi todos ignoramos cómo hablaba, se movía y reaccionaba el personaje histórico. Pero intuimos que, si Hoffa hubiese tenido la oportunidad de parecerse a Al Pacino, se comportaría sin duda de esa manera. De hecho, el espectador no espera

que el personaje se parezca a Hoffa, sino a Pacino.

Es más, estamos dispuestos a dar por bueno el Hoffa que Pacino quiera servirnos, que será más creíble cuanto más se parezca, no al verdadero Hoffa, sino a Pacino. ¿Me siguen?

Uno tiene la impresión de que a estas alturas, el público va a ver (o se queda en casa viendo) películas en las que Pacino no interpreta a nadie más que a sí mismo. Sin llegar al extremo de un Jack Nicholson o un Johnny Depp, el Pacino de los últimos 20 años tiende al trazo grueso, adentrándose en el reverso oscuro del actor rozando la caricatura. Algo que, lejos de resultarles molesto, parece satisfacer a buena parte de la afición, como si actuar y exagerar fueran equivalentes. Lo que no sabemos es si el aludido es consciente y la caricatura una consecuencia lógica de su carrera como actor, los lodos de aquellos polvos. O, no hay que descartarlo, que el suyo sea un destino trágico y esté obligado a perpetuarse como el actor Al Pacino interpretando a un personaje llamado Al Pacino. El verdadero Pacino está en otros, en Michael Corleone, en Ricardo III, en Lefty, el mafioso tronado de Donnie Brasco...

Quién sabe si un día volverá a ser él mismo y no su caricatura. No hay que descartarlo. Tiene sólo 80 años. ¿Qué es eso para una leyenda?

One’s caricature lies in wait within each of us. It happens to all of us, but especially to those who work in the field of performance. It might be due to the constant exposure to alien eyes embodying characters that are never the actor but who, as the invaders from outer space, use their bodies and compromise their identity. “caricature” comes from the Italian word “caricato”, exaggerated and, according to the RAE (Spanish Royal Academy), designated those specialised in doing impressions of well-known individuals, sometimes improvising.

In an attempt to draw a line we would say that the caricato does not own the character, he stays on the surface avoiding introspection. What comes out of crossing the line will depend on the acting talent, if it is just an impressionist or a real artist.

As an example, let’s take Al Pacino in The Irishman, the last, long and controversial film of Martin Scorsese. His creation of trade unionist Jimmy Hoffa is full of harsh remarks, off-key gestures and outbursts of anger. Was Jimmy Hoffa like that? But for the well versed we ignore how the historical figure spoke, moved or reacted. But we sense that, if Hoffa had had the opportunity to resemble Al Pacino he would undoubtedly have behaved like that. In fact, the

public does not expect the character to resemble Hoffa, but to resemble Pacino. Moreover, we are ready to believe the Hoffa that Pacino gives us, who will be more believable the more he resembles not the real Hoffa, but Pacino. Do you follow me?

One is under the impression that at this point the public goes to see (or stays at home watching) films in which Pacino does not play anyone but himself.

Without reaching the extreme of Jack Nicholson or Johnny Depp, the Pacino of these last 20 years tends to be bold, far into the dark side of the actor grazing the caricature. Something that far from becoming annoying seems to satisfy most of his supporters, as if acting and exaggerating were equivalent. What we do not know is if the aforementioned is aware and the caricature a logical consequence of his career as an actor, you reap what you sow. Or, one cannot rule out, that his is a tragic destiny and is forced to perpetuate himself as the actor Al Pacino playing a character called Al Pacino. The real Pacino can be found in other, in Michael Corleone, in Richard III, in Lefty, the done for Mafioso in Donnie Brasco… Who knows if one day he will go back to being himself and not his caricature. We cannot rule it out. He is only 80 years old. What is that for a legend?

Donde viven los osos...

Para ser exactos, la tierra donde viven más osos en España y que, además, son los únicos descendientes de las primeras poblaciones oseras que habitaron en la Península Ibérica hace miles de años.

Nos referimos, claro está, al oso pardo cantábrico y a Asturias. A una especie que estuvo a punto de extinguirse y que hoy, pese a que el peligro no ha desaparecido, su futuro no está lleno de nubarrones, sino que experimenta una recuperación evidente que plantea nuevos retos de conservación, pero con un horizonte bastante más despejado que el de los años 80 en que se dieron los primeros pasos para evitar una catástrofe que no nos hubieran perdonado las próximas generaciones.

Ello ha sido posible por la confluencia de muchos esfuerzos de

no pocas entidades, públicas y privadas, y –de manera muy especial– de los vecinos de las llamadas comarcas oseras. Y es que quedan ya muy lejos los tiempos en que el oso era considerado una alimaña, un recurso o complemento alimenticio o, más recientemente, un trofeo cinegético.

Evitar esas situaciones exige buenas prácticas en el llamado turismo de naturaleza por parte de todos y también la regulación estricta por parte de las administraciones, que deben ser conscientes de los límites con que debe ser utilizado un bien tan sensible como el que nos ocupa.

Asturias alberga hoy, con diferencia, la mayor población osera de España, que se ha convertido en símbolo de un medio ambiente excepcional que es necesario preservar de interferencias que pongan en riesgo los avances habidos, pues nos encontramos ante una especie muy sensible a la actividad humana que, debido al creciente interés por la naturaleza, se aproxima a veces de forma inadecuada a espacios muy sensibles para la vida de estos animales, sobre todo en periodo de crianza de los pequeños oseznos.

Y es que el crecimiento turístico basado en la especie tiene unas servidumbres claras en lo que se refiere a número y pautas de comportamiento, lo que no quiere decir –ni mucho menos– que no sea un reclamo de primer orden cuando el foco se ponga sobre todo en la oportunidad y disfrute que comporta visitar la tierra donde viven los osos y no tanto en ver al animal en su hábitat, ya que esta última posibilidad será siempre, y por razones muy fáciles de entender, muy limitada.

Where the bears live...

that future generations wouldn’t have forgiven us were taken.

Actually, the land with the highest population of bears in Spain; these are the only descendants of the first populations of bears that inhabited the Iberian Peninsula thousands of years ago.

This has been possible thanks to the confluence of great efforts made by numerous public and private entities, and - especially - by the neighbours of the so-called bear counties. Long gone are the times where bears were considers varmint, a resource or food supplement or, more recently, a hunting trophy.

We are indeed referring to the Cantabrian brown bear and to Asturias. To a species that was on the brink of extinction and whose future, despite the danger still being present, is currently brighter, with an evident recovery that presents new challenges of conservation, and a much clear horizon than that of the 80s when the first steps to avoid a catastrophe

Asturias currently holds the highest bear population in Spain by far; it has become a symbol of an exceptional environment, which needs to be kept from interferences that might risk the advances made thus far. This species is very sensitive to human activity, and due to an increasing interest in nature sometimes individuals approach inappropriately areas

that are considered sensitive for the life of this animals, especially while they’re breeding their cubs.

This situations are avoided by following good practices in the so called nature tourism as well as by strict regulation from the government, which needs to be aware of the limits within which one can use an asset as sensitive as the one at hand.

The increase in tourism based in the species has clear easements regarding the number and behaviour pattern, which does not mean that it is not a first-rate lure when the focus is on the opportunity and enjoyment of visiting the land where bears live and not so much on seeing the animal in his habitat, as the latter will always be, for obvious reasons, very limited.

Somiedo es, sin duda, el epicentro de estos territorios, pero la visita en Proaza a la Casa del Oso y a los cercados donde viven en semilibertad ejemplares que no han podido reintroducirse en la naturaleza es también obligada, circunstancia que puede ser aprovechada para recorrer a pie o en bicicleta la llamada Senda del Oso, uno de los itinerarios

con mayor aceptación en Asturias entre quienes buscan un contacto con la naturaleza cómodo y de fácil acceso.

Y no se puede olvidar otros concejos del suroccidente asturiano, como el de Cangas del Narcea, donde la presencia de este plantígrado se encuentra muy extendida y un Parador Nacional en un antiguo monasterio supone una magnifica base para estimulantes recorridos en plena naturaleza.

La Fundación Oso de Asturias es una entidad de colaboración publico/privada que, además de gestionar los espacios en que viven los ejemplares que por distintas circunstancias no pueden hacerlo en total libertad, se encarga de la Casa del Oso, promueve diversas iniciativas tendentes a preservar el hábitat natural de la especie y lleva a cabo labores de educación y divulgación medioambiental.

Can this be a disappointment?

We really do not think so, at least not greatly, as the land where the bears live has other natural resources that are so exceptional that they can easily fulfil the pleasure sought, as well as an ethnographic heritage that complements the aforementioned with utmost satisfaction, not to mention other resources such as the gastronomic one or the catering industry which have notably

grown in quantity and quality in the last few years.

Somiedo is, undoubtedly, the epicentre of this land, but a visit to Proaza to the Casa del Oso (House of Bears) and the enclosure where those bears that couldn’t be reintroduced into nature live in partial freedom is mandatory. One could take advantage of this visit to walk or bike across the Senda del Oso (Path of the Bear), one of the

most widely liked itineraries in Asturias among those who seek a comfortable and accessible contact with nature.

One must not forget other municipalities of South-western Asturias such as Cangas del Narcea, with a wide presence of this plantigrade and a State-run Hotel (Parador Nacional) in a former monastery constitutes a great starting point for stimulating courses in nature.

The Fundación Oso de Asturias is a public/private collaborative organisation that, on top of managing the areas inhabited by those specimens that for some reason cannot live freely, is responsible for Casa del Oso, promotes different initiatives to preserve the natural habitat of the species and carries out teaching and environmental dissemination activities.

El Profesor Luis Fernández-Vega y el Dr. José F. Alfonso asistieron al Congreso FACOELCHE 20/20. El encuentro, dedicado a la cirugía facorrefractiva, se celebró del 6 al 8 de febrero en Elche.

El Prof. Jesús Merayo Lloves fue ponente en la Ocular Surface Masterclass, que se celebró en el Instituto de Microcirugía Ocular de Barcelona (IMO).

Este curso, que reunió a más de un centenar de expertos, tuvo como objetivo debatir sobre las mejores estrategias diagnósticas y los tratamientos más eficaces para las patologías relacionadas con el ojo seco.

Professor Luis Fernández-Vega and Dr José F. Alfonso participated in the FACOELCHE 20/20 Meeting. The gathering, devoted to phaco-refractive surgery took place from the 6th to the 8th of February in Elche.

Prof Jesús Merayo Lloves was a lecturer at the Ocular Surface Masterclass that took place at the Instituto de Microcirugía Ocular in Barcelona (IMO). This course, which gathered over 100 experts, aimed to debate the best diagnostic strategies and the most efficient treatments for the pathologies related to dry eye.

I Simposium Oftalmológico de Honduras

XXIII Curso Monográfico de Iniciación a la Oftalmología

24th ESCRS Winter Meeting (Marruecos)

El I Simposium Oftalmológico en Santa Rosa de Copán, en Honduras, contó con la participación del Dr. Álvaro Fernández-Vega González. El doctor estuvo acompañado por los Dres. Héctor Robles, Carlos Fernández-Vega González y Álvaro Fernández-Vega Sanz. Este evento científico reunió a expertos de todo el mundo que lideran la investigación e innovación en oftalmología, para abordar los retos presentes y futuros en salud visual.

Un año más, el Prof. Luis Fernández-Vega y el Dr. José F. Alfonso participaron en el curso monográfico de iniciación a la oftalmología organizado por el Instituto de Investigaciones Oftalmológicas “Ramón Castroviejo” de Madrid durante el mes de febrero y que alcanzó así su vigésimo tercera edición.

El Prof. Jesús Merayo participó en el 24th ESCRS Winter Meeting, celebrado del 21 al 23 de febrero en Marrakech (Marruecos).

24th ESCRS Winter Meeting (Morocco)

The 1st Ophthalmology Symposium took place on Friday 7 and Saturday 8 of February in Santa Rosa de Copán, Honduras, with the participation of Dr Álvaro Fernández-Vega González. This scientific event gathered experts from all over the world, leaders in research and innovation in ophthalmology to address the current and future challenges in visual health.

Once again this year Prof Luis Fernández-Vega and Dr José F. Alfonso have participated in the monographic course of initiation to ophthalmology organised by the Instituto de Investigaciones Oftalmológicas “Ramón Castroviejo” in Madrid in February thus achieving its 23rd edition.

Prof Jesús Merayo participated in the 24th ESCRS Winter Meeting that took place from the 21st to the 23rd of February in Marrakech (Morocco). The professor gave a lecture entitled ‘Immunological disorders of the cornea’ where he contributed presentations, lab tests and immunological pathologies of the cornea.

Webinar de actualización sobre el glaucoma

El encuentro contó con la participación del Dr. Álvaro Fernández-Vega Sanz y la Dra. Eva Villota Deleu La SERV concedió el diploma de la Beca SENIOR 2019 a la Dra. Eva Villota y el Dr. Álvaro Fernandez-Vega González recibió el diploma por haber finalizado el fellow en retina médico-quirúrgica en el Hospital Universitario de Bellvitge de Barcelona.

La Unidad del Glaucoma del IOFV organizó el webinar titulado “Actualización en Glaucoma”.

Ponentes nacionales de prestigio abordaron diversos aspectos de la cirugía del glaucoma. Destacó la conferencia del Dr. Andrew Scott (Moorfields Eye Hospital, Londres) sobre cómo mejorar la organización de las consultas de Glaucoma.

Virtual Meeting of the Sociedad Española de Retina y Vítreo (SERV)

Updating Webinar on Glaucoma

Dr Álvaro Fernández-Vega Sanz and Dr Eva Villota Deleu participated in this meeting of the Sociedad Española de Retina y Vítreo (SERV). The SERV gave the diploma of the SENIOR 2019 grant to Dr Eva Villota and Dr Álvaro Fernandez-Vega González received the diploma for having finished his fellow in medical-surgical retina at Hospital Universitario de Bellvitge.

Prestigious national speakers addressed different aspects of glaucoma surgery. Dr Andrew Scott (Moorfields Eye Hospital, London) talked about how to improve the organisation of Glaucoma consultations. It also included an interesting round table debating the main aspects of the future of this specialty.
