Anesthesia duration was not associated with specific medical complications, overall medical complication rate, or mortality. When accounting for demographics and significant preoperative factors, including free flap type, increased anesthesia duration remained significantly associated with overall complications, surgical complications, and postoperative transfusion. The study was published on October 5, 2017, in JAMA Facial Plastic Surgery.

Advanced Technology Maps Complex Cardiac Arrhythmias he Acutus Medical (Carlsbad, CA, USA; www.acutusmedical.com) AcQMap High Resolution Imaging and Mapping System is an ultrasoundbased electrophysiology solution designed to visualize cardiac anatomy using both standard voltage-based and higher resolution dipole density (charge-source) maps of the human heart. Electrophysiologists can acquire more than 115,000 points every minute in order to generate CT-quality cardiac chamber reconstructions, with electrical activity displayed as waveform traces. The system can be used with all existing commercially available cardiac ablation platforms, and allows remapping of the entire chamber at any time during the ablation procedure in just a few seconds, so as to continually visualize any changes from the prior mapping.

Emergency Intubation Bests BMV for CPR new study held at Avicenne Hospital (Bobigny, France; www.chu93. A aphp.fr/lhopital-de-bobigny/) suggests that bag-mask ventilation (BMV) fails to improve on endotracheal intubation (ETI) in out-of-hospital cardiac arrest patients. The study, which included 2,043 out-of-hospital cardiac arrest patients from 20 centers in France and Belgium who underwent CPR, revealed the same rate of survival with good neurological outcome at day 28 in both groups. But analysis also showed that the BMV technique failed in 6.7% of patients, compared to a lower failure rate of just 2.1% with ETI. In addition, there was a much greater incidence of regurgitation and/or aspiration with BMV. Despite these drawbacks, the researchers declared that they recommend BMV as the standard method to ventilate out-of-hospital cardiac arrest patients.

Infection Control Mandated Emergency Care For Sepsis Proves Effective

he HeartMate 3 left ventricular assist device (LVAD), developed by Abstudy by researchers at the University of Pittsburgh School of Medicine T bott (Abbott Park, IL, USA; www.abbott.com) provides a new option for A (Pitt; PA, USA; www.medschool.pitt.edu) and other institutions reveals physicians managing advanced heart failure (HF) patients in need of shortthat rapid completion of a mandated three-hour bundle of sepsis care prototerm hemodynamic support. The system consists of a magnetically levitated centrifugal-flow pump designed to reduce trauma to blood cells passing through it and to improve outcomes. The pump is implanted above the diaphragm, next to the native heart, and is attached to the aorta, thus leaving natural circulation in place. HeartMate 3 can pump up to 10 liters of blood per minute, using magnets keep the rotor in place, at the same time generating an artificial pulse that combats the formation of thrombus in the device. The patient wears an external, wearable controller, driveline, and battery system that powers the pump.

Novel Implant Combats Diastolic Heart Failure new elastic implant developed by CorAssist Cardiovascular (Haifa, IsA rael; www.corassist.com) can improve cardiac diastolic function in heart failure patients with preserved ejection fraction (HFpEF) by helping to expand the ventricle wall, thus assisting the heart to fill with blood. Placed inside the left ventricle in a minimally invasive surgical (MIS) procedure, the spring-like CORolla device works by storing energy during the heart’s systole (contraction) phase and releasing it to the left ventricle during the diastole (relaxation) phase. Internal expansion forces are thus distributed directly to the left ventricle wall and septum, enhancing their elastic characteristics. CORolla is implanted through a transapical approach (TAA) or percutaneous approach (PA) in an off-pump procedure.

Emergency Medicine Medical Disaster Drone Demonstrates Lifesaving Capabilities eveloped at the William Carey University College of Osteopathic MediD cine (WCUCOM; Hattiesburg, MS, USA; www.wmcarey.edu), the new ambulance drones can carry telemedical packages in order to support victims and rescue personnel during medical disasters such as natural catastrophes, mass shootings, or terrorist attacks. As part of the ongoing Health Integrated Rescue Operations (HiRO) project, the drones also provide immediate and secure access to a physician on the other end of the screen, bringing much-needed guidance amidst the chaos and confusion of a disaster situation. To do so, the critical care kit includes Google Glass, which allows the wearer to be hands free and to move away from the drone while still maintaining audio and visual contact with the physician.

Molecular Test Rapidly Diagnoses Sepsis molecular test developed at Tongde Hospital of Zhejiang Province (Hangzhou Shi, China; www.zj.gov.cn) rapidly and reliably diagnoses sepsis, a potentially life-threatening complication of bacterial infections. The TaqMan-based multiplex real-time polymerase chain reaction (PCR) probes can detect bacteria samples with positive rate of 96% at a concentration of 100 colony-forming units (CFU)/mL, and 100% at a concentration greater than 1,000 CFU/mL. In a clinical study of 30 patients with clinical symptoms of sepsis, 18 patients were confirmed positive by Multiplex real-time PCR and seven patients were confirmed positive by blood culture. In addition, no PCR products are detected when DNAs from oth-

col is associated with lower in-hospital mortality. Hospitals can tailor how they implement the protocols, but must always include a blood culture to test for infection, measurement of blood lactate, and administration of antibiotics within three hours of diagnosis, collectively known as the threehour bundle. Study results revealed that in patients who did not complete the 3-hour bundle within 12 hours, higher risk-adjusted in-hospital mortality was associated with longer time to completion, longer time to the administration of antibiotics, but not with the completion of a bolus of intravenous fluids.

Updated Guidelines Released for Preventing Surgical Site Infections he American College of Surgeons (Chicago, IL, USA; www.facs.org) has T updated the guidelines detailing the measures that patients, surgeons, and hospitals can do to prevent surgical site infection (SSI). Issued dealt with by the updated recommendations include counseling patients to completely refrain from smoking for a minimum of four to six weeks before elective surgery; maintaining short-term blood glucose control before, during, and after surgery; use of topical and local antibiotics to reduce infections; using clippers used instead of razors to remove hair in the surgical site; showering before surgery; and routine double gloving to protect the surgeon. The updated guidelines were published in the January 2017 issue of the Journal of the American College of Surgeons (JACS).

Wound-Irrigating Device Combats Surgical Infections he Prescient Surgical (San Carlos, CA, USA; www.prescientsurgical. T com) CleanCision irrigating wound retractor device combines wound irrigation and protection to thwart surgical site infections (SSIs) in abdominal surgery for incisions 7-16 cm in diameter. A double-walled sheath delivers a sterile irrigating solution to the wound edges by gravitational feed from an external fluid bag that is connected with a Luer connector into the device, which is delivered to the wound edges through a permeable outer layer in the integrated sheath. The inner layer of the sheath, on the other hand, is impermeable to the fluids present in the surgical field. After coming into contact with the wound edge, the excess fluid is removed through a separate chamber within the sheath via connection to wall suction.

Salted Surgical Mask Deactivates Aerosol Viruses eveloped at the University of Alberta (Edmonton, Canada; www. D ualberta.ca) and Kyung Hee University (Seoul, South Korea; www. khu.ac.kr), the universal, reusable virus deactivation system is based on common sodium chloride salt that coats the fibrous filtration unit of the surgical mask. When an aerosol droplet carrying influenza virus contacts the treated filter, the droplet absorbs the salt on the filter. The virus is exposed to increasing concentrations of salt as the droplet evaporates, suffering fatal physical damage when the salt returns to its crystalized state. The salt-coated filters proved especially effective in deactivating influenza viruses, regardless of subtypes, and even following storage in harsh environmental conditions. The feeding tubes are equipped with sensors that relate the tube tip to the lower esophageal sphincter (LES), reducing the incidence of misplacement during first positioning and monitoring position continuously during the course of the feeding. The smARTrack console is also equipped with a motorized mechanism that automatically stops working if the feeding tube moves out of position during ongoing use, and can guide an operator to correctly re-position the tube.

Catheter System Drains Recurrent Pleural Effusions he BD (Franklin Lakes, NJ, USA; www.bd.com) PleurX catheter system enables patients to drain pleural fluid or ascites at home, instead of requiring them to return to the hospital for thoracentesis. The system consists of a catheter inserted in the chest wall for draining pleural effusions, or into the abdomen for malignant ascites, and proprietary drainage bottles. To drain fluids, the free end of the catheter is connected to the drainage line on the external vacuum bottle, which then automatically draws out the fluid by pressure differential; a roller clamp controls flow on the drainage line. The PleurX catheter can also be used in place of a chest tube for the delivery of talc or bleomycin for chemical pleurodesis, offering an additional treatment option.

Hospital-Acquired Anemia Affects One-Third of Patients study at the University of Texas Southwestern Medical Center (UT A Southwestern; Dallas, TX, USA; www.utsouthwestern.edu) shows that one in three patients hospitalized for medical problems experience hospitalacquired anemia (HAA), a drop in hematocrit due to surgical blood loss and blood tests during hospitalization. The most severe form of HAA, defined as a hematocrit of 27% or less at the time of discharge, was associated with a 39% increase in odds of being readmitted or dying within 30 days of hospital discharge. The two strongest potentially modifiable predictors of developing moderate or severe HAA were length of stay and a major procedure. The study was published in the May 2017 issue of the Journal of Hospital Medicine.

Pregnant women are at risk of preterm birth when the cervix begins to efface (shorten) and dilate earlier than necessary; in such cases, a cervical pessary (a small silicone ring-shaped device placed around the incompetent cervix) placed early in the pregnancy is used to close it; it is removed later on when the risk of a preterm birth has passed.

New Research Blood Group May Affect Heart Attack Risk ccording to a study at the University Medical Centre Groningen (The A Netherlands; www.umcg.nl), healthcare professionals should consider a person’s blood group when assessing cardiovascular risk, as people with A, B, and AB blood types may be at greater risk, particularly of heart attacks, than individuals with O blood types. The study, which included data of more than 1.3 million adults, found that 2.5% of those with a non-O blood group experienced a cardiovascular event, compared with 2.3% in those who had an O blood group. The researchers noted that people with non-O blood types had higher concentrations of the von Willebrand factor, and also tended to have higher cholesterol levels, a known risk factor for poor cardiovascular health.

Pathogen Transmission Rife in Hospital Settings ospital rooms, and not just the patients in them, can spread germs H through contact with health care personnel, according to a new study by researchers at Duke University (Durham NC, USA; www.duke.edu). Cultures were collected from the sleeves, pockets, and midriffs of a new set of surgical scrubs of 40 ICU nurses, the bodies of all patients the nurse cared for during each shift, and patients’ room contents (bed, bedrail, and supply cart). The researchers confirmed 12 instances of pathogen transmission from the patient or the room to the scrubs, 6 incidents of transmission from patient to nurse and room to nurse, and 10 transmissions from the patient to the room. The pockets and sleeves of the scrubs were the most likely to be contaminated, as were the bed rails in the rooms.

Nobel Prize Awarded for Circadian Clock Discoveries he 2017 Nobel Prize in Medicine was jointly awarded to scientists JefT frey C. Hall, Michael Rosbash, and Michael W. Young for their fundamental discoveries on the molecular mechanisms controlling the circadian rhythm. After isolating the “period” gene, they discovered PER, the protein encoded by the period gene, accumulated during the night and degraded during the day. Thus, PER protein levels oscillate over a 24-hour cycle, in synchrony with the circadian rhythm. Another gene, called doubletime, delays accumulation of PER protein. Additional proteins they discovered showed how the period gene was activated, as well as the mechanism by which light can synchronize the clock. The paradigm-shifting discoveries of the Nobel laureates established the key mechanistic principles for the circadian biological clock.

Imaging/Contrast Agents Fluorescent Dye Detects Benign Brain Tumors TL38, an intraoperative molecular imaging (IMI) dye developed at the University of Pennsylvania (Penn; Philadelphia, USA; www. upenn.edu), is a new folate analog that could help identify pituitary adenomas, which are known to overexpress folate receptor alpha (FR ). In a study of 15 adult patients who presented with pituitary adenomas who were infused with OTL38, intraoperative near infra-red (NIR) imaging delineated the main tumors in all patients. OTL38 consists of two parts; vitamin B9 (a necessary ingredient for cell growth), and a NIR glowing dye. As pituitary adenomas grow and proliferate, they overexpress folate receptors more than 20 times above the level of the normal pituitary gland. The dye binds to these receptors and thus permits tumor identification.

Palette of Fluorescent Molecules Advance Biological Imaging esearchers at the Howard Hughes Medical Institute (HHMI; Ashburn, R VA, USA; www.hhmi.org) have created a colorful spectrum of fluorescent dyes by utilizing specific chemical molecules called rhodamines. By incorporating 3-substituted azetidine rings into classic Janelia Fluor (JF) fluorophore structures, researchers can now tune the spectral and chemical properties of rhodamine dyes. The new strategy provides new fluorescent and fluorogenic labels with excitation ranging from blue to the far-red. For a study published on September 4, 2017, in Nature Methods, the researchers lit up cell nuclei, made larval fruit fly brains shine, and highlighted visual cortex neurons in mice that had tiny glass windows fitted into their skulls. The extent of gadolinium deposition varies; although some linear contrast agents appear to cause greater MRI signal changes than some macrocyclic agents, deposition of gadolinium has also been observed with macrocyclic agents. While the deposition itself has been confirmed, the clinical significance of the retained gadolinium in the brain is unclear. The review was published on August 10, 2017, in The Lancet Oncology.

The binding affinity of the tracer was not affected by heparin, aspirin, or other anticoagulants, and was cleared rapidly from the blood stream. A uio.no) suggests that one in three women with breast cancer detected by a mammogram may be treated unnecessarily. To estimate the amount of over-diagnosis, the researchers evaluated associations between mammography screening and the size of detected tumors in women aged 35 to 84 years. The researchers assumed that if screenings work as intended, the number of small, curable breast tumors should increase, reducing the number of large cancers by about the same amount. The results revealed, however, that screening and removal of small, early-stage tumors was not associated with a reduction in the incidence of advanced cancer, suggesting that one in every three invasive tumors and cases of DCIS represent over-diagnosis.

Another concerning trend was the drop in positive predictive value (PPV), a direct result of the abnormal interpretation rate increasing much more than the cancer detection rate. Magnetic Resonance Imaging (MRI) Researchers Develop Faster Cardiovascular MR Protocol he results of an international study show that a new Cardiovascular T Magnetic Resonance (CMR) imaging protocol can reduce costs by 80%, and is three times as fast as current tests. A collaboration between researchers in Peru, the UK, the USA, and Brazil, the goal of the research was to develop a cost-effective and faster imaging protocol specifically for use in developing countries. The outcome of the collaboration was the new protocol, developed by researchers from University College London (UCL; London, UK; www.ucl.ac.uk), which uses gadolinium as the contrast agent. Costing just USD 150 and taking only 18 minutes on average to complete, its results are comparable to a traditional CMR, which can cost about USD 600-800 per scan.

MRIs Safe for Range of Cardiac Implantable Electronic Devices agnetic resonance imaging (MRI) appears to be safe for patients with M cardiac implantable electronic devices (CIEDs) such as pacemakers and defibrillators, according to a new study by researchers at the University of Utah (Salt Lake City, USA; www.utah.edu). The researchers examined 178 consecutive patients with CIEDs who underwent 212 MRI scans in the C-spine/head/neck (41%); T-spine/cardiac/shoulder and thoracic (33%); Lspine/abdomen/pelvis (33%); and lower extremity (13%). The results revealed no primary or secondary outcome events, and no disruption of pacing. Since the early 1990s CIEDs have been regarded as a near absolute contraindication to undergoing MRI scans, and have prevented many patients from receiving an often-optimal imaging test.

Nuclear Medicine Head and Neck Cancer Patients Can Benefit from Increased Radiation Therapy review by researchers at the Gustave Roussy Cancer Center (Villejuif A Cedex, France; www.gustaveroussy.fr/en) suggests that hyperfractionated radiotherapy (RT) twice daily, when combined with chemotherapy, enables patients to receive a higher dose of radiation and more effective RT, possibly without even increasing side effects. The review of studies included 28,804 patients worldwide with mouth, throat, or voice box tumors that had already spread to adjacent tissue. The results, from 117 different trials, found that cancer deaths were reduced by 20% compared with standard once-daily RT and chemotherapy. The treatment also reduced the risk of the cancer deteriorating by 23%.

Stereotactic Radiation Effective for Kidney Cancer tereotactic ablative radiation therapy (SAbR) demonstrates excellent S local control of metastatic renal cell carcinoma with a favorable safety profile, according to a new study conducted at the University of Texas Southwestern Medical Center (UT Southwestern; Dallas, TX, USA;

Annual Review 2017 www.utsouthwestern.edu). The results of the study showed that the overall one-year local control rate after SAbR was 91.2%. Local failures were associated with prior radiation therapy treatment, prior palliative-intent radiation therapy treatment, spinal metastasis location, previous systemic therapy status, and a biologically effective dose lower than 115 Gy. The dose received by 99% of the target volume was the strongest dosimetric predictor for local control. The study was published in the May 1, 2017, issue of the International Journal of Radiation Oncology, Biology, and Physics.

Proton Therapy Lowers Cancer Treatment Side Effects study by the Children’s Hospital of Philadelphia (CHOP, PA, USA; A www.chop.edu), and other institutions concludes that proton beam therapy (PBT) is as effective as photon radiation in pediatric patients with head and neck cancer, but with less impact on their quality of life (QOL). Clinical and dosimetric data were abstracted from the medical record in order to identify acute toxicities and early outcomes, which were then compared to historic photon radiation results. The researchers found that one year after treatment, 93% of patients were still alive, and 92% did not experience recurrence at their primary disease site. No patients suffered toxicities above grade 3, with the most severe toxicities being mucositis (4%), loss of appetite (22%) and difficulty swallowing (7%), which indicate significantly lower toxicity rates than those associated with photon radiation.

Machine Learning Tool Identifies Breast Lesions esearchers at the Massachusetts Institute of Technology (MIT; CamR bridge, USA) have trained a machine-learning tool to identify high-risk, biopsy-diagnosed breast cancer lesions that are unlikely to become cancerous, and do not require immediate surgery. The model takes into account patient age, lesion histology, and other standard risk factors, but also includes keywords from biopsy pathology reports. The researchers first trained the model using patients with biopsy-proven high-risk lesions. After training the model on two-thirds of the high-risk lesions, the researchers found that they were able to identify 97% of the lesions that were upgraded to cancer. The researchers also found that by using the model, they could help avoid almost one-third of the surgeries of benign tumors.
