Imaging/Contrast Agents Adaptable Contrast Media Improves MRI Diagnostics
Interventional Radiology Angiographic Algorithm Facilitates Coronary Artery Assessment
n adaptable protein structure developed by Caltech and Leibnizhe Pie Medical Imaging (Maastricht, The Netherlands; www.piemedical A Forschungsinstitut für Molekulare Pharmakologie (FMP; Berlin, GerT imaging.com) Cardiovascular Angiographic Analysis Systems for vessel many; www.leibniz-fmp.de) absorbs dissolved xenon in a self-regulating Fractional Flow Reserve (CAAS vFFR) software module can calculate presway that allows for higher quality MRI scans. The new contrast media is based on nanometric hollow protein structures produced by bacteria in order to regulate flotation depth, similar to a miniaturized swim bladder in fish. Unlike conventional contrast media, these “gas vesicles” always absorb a fixed portion of the xenon present in the environment. As much more xenon is absorbed into the vesicles than with conventional contrast media, the image contrast is many times higher than the background noise, and image quality is significantly improved, allowing identification of disease markers that occur in relatively low concentrations.
sure drop in the coronary artery non-invasively, obviating the need for a pressure wire and hyperemic agent. The software is designed to reconstruct a 3D model of the vasculature using just two angiograms taken during a standard catheterization procedure in order to assess pressure drop, expressed as a vFFR value. The 3D reconstruction also enables assessment of the severity and percentage of coronary artery stenosis. For percutaneous coronary interventions (PCI), CAAS vFFR can offer a unique combination of functional and anatomical lesion assessment to support the interventional cardiologist in the clinical decision making process.
PET Tracer Identifies and Tracks Bacterial Infections new positron emission tomograA phy/computed tomography (PET/ CT) radiotracer, developed at the University of Louisville (UL; KY, USA; www. louisville.edu), can identify and track the degree of bacterial infection in lungs with high selectivity and sensitivity. In a study designed to validate the new tracer, 2-18F-fluorodeoxysorbitol (18F-FDS), albino C57 female mice were inoculated with either live Klebsiella pneumoniae to induce lung infection, or the dead form of the bacteria to induce inflammation. Half of the mice inoculated with the live bacteria were imaged with PET/CT using 18F-FDS, and the other half were screened by bioluminescent imaging (BLI). The results revealed that while both showed good correlation with colony forming units (CFU), only 18F-FDS was able to differentiate lung infection from lung inflammation. The study was published in the January 2018 issue of The Journal of Nuclear Medicine.
Augmented Reality Fluorescence Facilitates Vascular Neurosurgery surgical fluorescence solution uses A indocyanine green (ICG) dye to display cerebral anatomy in natural colors, augmented by real-time vascular flow, all in a single image. Integrated Breast Ultrasound Make Exams Easier and Faster
Annual Review 2018 The greater detection rate for combined DM and DBT was notable for small and medium invasive cancers, but not large ones, and was stronger for ductal carcinoma in situ (DCIS). The study was published on June 5, 2018, in Radiology.
Magnetic Resonance Imaging (MRI) Multiparametric MRI Technique Could Replace Tumor Biopsies novel multiparametric MRI (mpMRI) algorithm can recognize clear A cell carcinoma (ccRCC) and provide information about the nature and aggressiveness of the tumor, without having to perform a biopsy. Developed by the University of Texas (UT) Southwestern Medical Centre (Dallas, TX, USA; www.utsouthwestern.edu), mpMRI requires multiple scans to make a diagnosis, including anatomic T1-weighted and T2-weighted MRI sequences; functional sequences, such as diffusion-weighted imaging MRI and dynamic contrast-enhanced MRI; and other images in order to indicate if fat cells are present in the tumor. By combining different types of scans, a clearer picture of the scanned area becomes available. Injecting a contrast agent also helps to enhance the image.
Multiparametric MRI Effective For Fatty Liver Assessment new study concludes that the Perspectum Diagnostics (Oxford, UK; A www.perspectum-diagnostics.com) LiverMultiScan multiparametric MRI device, which is designed to enable non-invasive and quantitative liver fat characterization, especially in non-alcoholic fatty liver disease (NAFLD), could halve the number of potential liver biopsies required. The system uses MRI data to calculate images of proton density fat fraction, T2, and T1 in the liver, which have been shown to correlate with histological measures of steatosis, hemosiderosis, and fibrosis. As T2 MRI is influenced by iron deposits, it can be used to assess hepatic iron overload, while T1 is influenced by the type and structural organization of a tissue.
Sealed Magnet Delivers Helium-Free MRI Scanning new magnetic resonance imaging (MRI) scanner includes a fully A sealed magnet that enables helium-free operations. The Royal Philips (Philips; Amsterdam, The Netherlands; www.philips.com) Ingenia Ambition X 1.5T MRI with BlueSeal magnet technology permanently seals helium inside the device, reducing the total amount needed to operate a conventional MRI system (1,500 liters) to less than 0.5% (a mere seven liters). As a result, the fully sealed system is around 900 kg lighter than its predecessor and does not require a vent pipe, significantly reducing installation challenges and construction costs resulting from the use of conventional magnets. An EasySwitch solution allows the BlueSeal magnetic field to be easily turned off if an item is stuck in the bore; once resolved, an automated ramp-up sequence brings the magnet back to operation, minimizing downtime.
Carotid Artery MRI Assesses CVD Risk Better Than Ultrasound
he Royal Philips (Philips; Amsterdam, The Netherlands; www. study by researchers at the U.S. National Institutes of Health (NIH, T philips.com) Ultimate Breast Solution is an innovative patient-centric A Bethesda, MD, USA; www.nih.gov) and other institutions suggests ultrasound solution for breast assessment that combines advanced imaging, full solution elastography, screening, and precision biopsy. The Ultimate Breast Solution uses the PureWave eL18-4 ultra-broadband linear array transducer and Philips PureWave crystal technology, as well as ElastQ Imaging shear wave and strain elastography to help assess a wide array of breast lesions.
Combined Digital Screening Detects More Breast Cancers new study by researchers at AUSL Reggio Emilia (Italy; www.ausl.re.it) A suggests that a combination of digital mammography (DM) and digital breast tomosynthesis (DBT) detects 90% more breast cancers than digital mammography alone. The study, which involved 19,560 women, revealed that the combination of DM and DBT detected 8.6 cancers per 1,000 cases, a rate almost twice that of the 4.5 per 1,000 detected by mammography alone. DBT alone detected 72 of 80 cancers found in the DBT+DM group.
ic resonance imaging (MRI) improves cardiovascular disease (CVD) risk assessment, when compared to ultrasound. The study, which enrolled 698 participants without prior history of clinical CVD, measured wall thickness with ultrasound and both non-contrast proton-density–weighted MRI and intravenous gadolinium-enhanced MRI. During subsequent follow-up, the correlations between CCA wall thickness measurements and coronary heart disease and stroke were quantified. The results revealed that the association between wall thickness and CVD was stronger with both non-contrast and contrast-enhanced MRI than it was for ultrasound. The study was published on October 9, 2018, in Radiology.

The targeted muscle reinnervation (TMR) procedure involves rerouting severed motor nerves by surgically transferring them to the motor points of the denervated target muscles, which, after reinnervation, can contract in response to neural control signals intended for the missing limb, allowing the body to re-establish its neural circuitry and alleviating phantom and residual limb pain. In addition, advanced prosthesis movement and control occurs in response to attempts to move the missing limb, making control easier and more intuitive, resulting in more refined functional movements and more degrees of motion.
Relaxation App Helps Children Cope With Procedural Pain HCC biofeedback-assisted relaxation app developed by artist George Khut, A PhD, of the University of Sydney (Australia; sydney.edu.au), helps children control procedural pain, such as during peripheral blood collection and cannulation. The BrightHearts biofeedback mediation relaxation app, designed for mobile phones and tablet computers, combines standard distraction methods with the principals of biofeedback relaxation training in order to focus children’s attention away from their object of their anxiety and fear. The app uses heart rate, measured through a pulse oximeter worn on the ear or thumb, to help children observe and regulate their response during painful procedures. The children are rewarded with sounds and visuals that respond to decreases in heart rate over different periods of time, which they learn to influence with their breathing and relaxation techniques.

Air Pollution Accounts for Millions of Asthma ER Episodes Annually mbient air pollution is major contributor to the global asthma burden, A responsible for millions of emergency room (ER) visits, according to a new study by the Stockholm Environment Institute (York, UK; www. sei.org/centres/york/), and other institutions. The results showed that nine to 23 million annual asthma ER visits globally (8-20% of total asthma ER visits) were triggered by ozone, a pollutant generated when emissions from cars, power plants, and other combustion sources interact with sunlight, and five to 10 million asthma ER visits (4-9% percent of total global asthma ER visits) were linked to PM2.5, small particles of pollutants that can lodge deep in the lung’s airways. Human-derived emissions were responsible for about 37% and 73% of ozone and PM2.5 asthma ER impacts, respectively, with the remaining impacts attributable to naturally occurring ozone precursor emissions, such as those from vegetation and lightning, and PM2.5 factors such as dust and sea salt.
Antibacterial Clays Can Help Combat Bacterial Infections new study by Arizona State University (ASU; Tempe, USA; www. A asu.edu) and other institutions reveals that clays with antibacterial activity contain Iron (Fe) in one or more mineral phases. The antibacterial mechanism appears to involve reactions with soluble redox active trace metals (e.g., Fe2+), with a significant role for Al3+ implicated in the binding to phospholipids in cell membranes, causing protein misfolding and poration, which may enhance the intracellular influx of reduced metals. The clays also manifest intracellular nanosecond Fenton reactions that produce hydroxyl radicals that reduce nearby biomolecules – such as DNA and proteins – during metal oxidation. This antibacterial mechanism affects both planktonic and biofilm states of bacteria. Clays have been used both internally and topically for medicinal purposes by indigenous cultures worldwide.
Vast Majority of Tonsillectomies are Unnecessary new study from the University of Birmingham (UB; United Kingdom; A www.birmingham.ac.uk) reveals that seven of every eight children who have their tonsils removed are unlikely to benefit from the procedure. The clips can be reloaded and repositioned intraoperatively, providing surgeons with the ability to address compression across multiple joints at the same time, without violating the joint surface, and delivering continuous compression immediately after implantation.
Robotics Autonomous Robot Reduces Hospital Labor Expenditure elay, a new autonomous service robot (ASR) for healthcare settings deR veloped by Swisslog Healthcare (Broomfield, Co, USA; www.swisslog. com) and robotic pioneer Savioke (San Jose, CA, USA; www.savioke.com), is a friendly, helpful, and sophisticated indoor ASR that helps free up nurses, pharmacists, lab techs, and other skilled workers from time-consuming drugs and specimen delivery tasks, allowing them to focus on more valuable work and patient care. Relay can operate elevators, open doors, and navigate in busy public corridors, delivering medicines, blood, lab specimens, snacks, and doc-
plasty (THA) procedures by establishing an accurate cup position, equalization of leg length, and restoration or maintenance of offset and joint center of rotation. The imageless guidance system, which is based on a miniature camera and tracker that do not interfere with surgical workflow, ensure correct alignments, thus helping to prevent dislocations, revision surgery, leg length discrepancies, and readmissions. Intellijoint HIP is suitable for anterior, lateral, and posterior surgical approaches, and can be used with all major implant vendors.
Expansion System Stabilizes And Restores Vertebral Body novel spinal implant provides a fulA ly controlled and comprehensive solution for the reduction of painful osteoporotic vertebral compression fractures (VCFs). The Stryker (Kalamazoo, MI, USA; www.stryker.com) SpineJack fracture reduction system is based on a modifiable titanium expander designed to realize the biomechnical restoration of a VCF by simultaneously restoring sagittal and coronal balance, coronal angulation, and endplate restoration. The procedure involves implant placement using a percutaneous transpedicular approach with fluoroscopic guidance, followed by a controlled anatomical craniocaudal expansion that preserves surrounding bone trabeculae. During expansion, a 500-Newton distraction force is applied along the cranio-caudal axis, similar to a car jack. A rack-and-pinion system blocks the expansion of the implant at the desired height, preventing loss of correction before low-pressure injection of polymethylmethacrylate (PMMA) bone cement, which envelops the implants, ensuring definitive stabilization of the fracture.

Transplantation/Implantation DNA Test Predicts Risk of Leukemia Relapse new study by Canadian and Korean scientists and physicians working A with the University of Toronto (Toronto, ON, Canada; https:// medicine.utoronto.ca) has identified DNA mutations that can indicate the presence of treatment-resistant leukemia cancer cells. As these same DNA mutations expand in frequency upon relapse, the researchers developed computational tools in order to parse the leukemia-driving mutations from the specimen data, thus allowing them to identify the DNA mutations with a low residual mutation frequency of 0.2%, which can be used as a surrogate marker to calculate the personal chance of relapse. Those patients who are identified at high risk of relapse can then undergo therapeutic intervention earlier, which can improve their outcome in the long run. The study was published on August 14, 2018, in the journal Blood.

Annual Review 2018 chain reaction (PCR) to perform gene expression analyses, the researchers compared biopsies from 16 kidneys that showed primary function upon transplantation with 16 exhibiting delayed graft function (a patient requiring dialysis within a week of transplantation). From the set of genes selected for comparison between the two renal biopsy groups, there were four genes that showed increased expression, including a gene for matrix metallopeptidase 3 (MMP3), and three other genes related to metabolism. The authors suggest the “gene fingerprint” might forecast likelihood of delayed graft function for donor renal tissue.
Urology Vapor System Obliterates Obstructive Prostate Tissue new minimally invasive system uses the natural thermal energy stored A in steam to treat benign prostatic hyperplasia (BPH). The NxThera (Maple Grove, MN, USA; www.nxthera.com) Rezum System is based on convective water vapor energy (WAVE) platform technology; when the water vapor contacts the tissue it condenses, releasing the stored thermal energy, causing the obstructive prostate tissue cells to die. In the weeks following a Rezum procedure, the body’s natural healing response eliminates the treated tissue, shrinking the prostate, allowing the urethra to open back up, and reducing BPH symptoms. The system also allows men to retain sexual function, unlike surgical BPH treatments, which typically have higher incidences of long-term sexual side effects.
