Magnesium (Mg++)
A Critical Piece of the Electrolyte Puzzle
Ionized Magnesium (Mg++), not Total Magnesium (tMg), is the only physiologically active form of magnesium. Magnesium bound to protein, or chelated to phosphate, citrate, sulfate, or carbonate is inactive.
tMg is an unreliable substitute for Mg++ Mg++ may be abnormal while tMg is normal, and vice versa.1,2
Mg++, and Ca++ can now be measured in the lab or at the point of care to provide a complete electrolyte analysis: Na+, K+, Ca++, Mg++, Cl-, HCO 3 -
Mg++, Ca++, and K+ ion abnormalities are common in critical care medicine.
Mg++, Ca++, and K+ ions are interdependent and play a role in numerous disease processes, including diabetes, hypertension, kidney disease, cardiovascular disease, cardiac arrhythmia, and sepsis.
Mg++ is a vasodilator, Ca++ is a vasoconstrictor. Both are synergistic in maintaining vascular and bronchial smooth muscle tone.
Mg++ ion is an antagonist to Ca++ ion entry into cardiomyocytes.3
Serial monitoring of Mg++, Ca++, and K+ ions are all important in correcting or avoiding cardiac arrhythmias and cardiomyocyte necrosis.4,5,6
Hypokalemia may be unresponsive to potassium repletion unless hypomagnesemia is first corrected.7
Electrolytes (Na+ , K+ , Ca++ , Cl -) are all measured as ions because that is their only clinically active form. Now Mg++ can be measured the same way.
If you are measuring K+ and Ca++ , you should also be measuring Mg++

lizes two novel circulating proteins to accurately diagnose non-alcoholic steatohepatitis (NASH) and liver fibrosis, and can score the stages of both diseases, without the need for invasive liver biopsy. The new test uses artificial intelligence (AI) to accurately predict the stages of the whole spectrum of liver diseases, from NAFLD to NASH and liver fibrosis.
Kit Detects Immunity to COVID-19 and Variants
Scientists from the Singapore-MIT Alliance for Research and Technology (SMART, Singapore; smart.mit.edu) and Nanyang Technological University (NTU, Singapore; www.ntu.edu.sg) have developed a quick test kit that can tell if a person has immunity against COVID-19 and its variants, based on the antibodies detected in a blood sample. Different from ART test kits – which look for the presence of viral proteins produced during a COVID-19 infection to determine if a person is infected – this rapid POC test kit is a serology test that measures antibodies made by the patient.
Study Urges Setting Up Reserve Lab Capacity for Future Pandemics
Researchers at University of Cambridge (Cambridge, UK; www.cam.ac.uk) have published a report stating that the lack of early COVID-19 PCR testing capacity had a knock-on effect on other health services in 2020. The researchers suggest that a system of reservist lab scientists should to be set up now to provide surge capacity that will help respond faster – and more effectively – to future outbreaks of infectious disease.
First Test to Diagnose Acute Rheumatic Fever
Acollaborative network of researchers led by the Cincinnati Children’s Hospital Medical Center (CCHMC, Cincinnati, OH, USA; www.cincinnatichildrens.org) has undertaken a project to identify and validate biomarkers for acute rheumatic fever (ARF), which initiates a slow but progressive process of heart valve damage dubbed rheumatic heart disease. These biomarkers will serve as a basis for the world’s first sensitive and specific diagnostic test for ARF.
MCED Tests Mark New Era in Early Cancer Detection
Researchers have presented new data supporting the accuracy of multi-cancer early detection (MCED) blood testing that could have major implications for future
cancer care provision. The PATHFINDER study presented at the ESMO Congress 2022, organized by the European Society for Medical Oncology (ESMO, Lugano, Switzerland; www.esmo.org) is the first prospective investigation to show that an MCED test can detect cancer in patients with undiagnosed cancer, as previous studies used tests only in patients already known to have cancer.
Breakthrough Method Diagnoses
Parkinson’s from Skin Swabs in Three Minutes
Scientists at The University of Manchester (Manchester, UK; www.manchester. ac.uk) have found a new method to detect Parkinson’s disease by analyzing sebum with mass spectrometry. The scientists have
found that sebum can be used as a diagnostic biofluid, which is rich in hydrophobic endogenous metabolites. The research team sees this as a major step forward towards a clinical method for confirmatory diagnosis of Parkinson’s, for which to date there is no diagnostic test based on biomarkers.
OCTOBER
Rapid POC Test for Methicillin Resistance
Anew test for the quick diagnosis of Methicillin-resistant Staphylococcus aureus (MRSA) not only provides rapid RT-PCR results but differentiates whether the bacterial strain is MRSA or methicillin-sensitive (MSSA) which promotes
targeted therapy. The MRSA/SA rapid test on Vivalytic by Bosch Healthcare Solutions GmbH (Waiblingen, Germany; www. bosch-healthcare.com), a POC platform brought to the market by Randox Laboratories (Crumlin, UK; www.randox.com), delivers reliable results in under an hour directly at the POC.

New data has validated the benefits of Daxor Corporation’s (Oak Ridge, TN, USA; www.daxor.com) BVA-100 (Blood Volume Analyzer), a diagnostic blood test, in reducing hospital length of stay (LOS) for heart failure patients. The BVA-100 is the only diagnostic blood test cleared by the FDA to provide safe, accurate, objective quantification of blood volume status and composition compared to patient-specific norms.
Swab Test Identifies Dangerous Cervical Cell Changes Years Before They Occur
Researchers at University College London (London, UK; www.ucl.ac.uk) and University of Innsbruck (Innsbruck, Austria; www.uibk.ac.at) have developed a new swab test that identifies potentially dangerous cervical cell changes up to four years before they happen. The swab test works by examining the DNA methylation, a type of modification “on top” of the DNA, of cervical cells. The research found that the new cervical screening method was more sensitive than other currently available tests and could reliably identify advanced cell changes and cervical cancer.
Liquid Biopsy Testing Platform Offers Unprecedented Sensitivity
in MRD Detection
Anovel liquid biopsy testing platform allows detection of minimal residual disease (MRD) with up to a 100-fold improvement in sensitivity over existing assays. Foresight Diagnostics, Inc.’s (Aurora, CO, USA; www.foresight-dx.com) MRD platform is based on the Phased variant Enrichment and Detection by Sequencing (PhasED-Seq) technology. By detecting more than one mutation, PhasED-Seq can more accurately distinguish tumor-derived cell free DNA (i.e., ctDNA) from healthy cell free DNA – enabling detection of ctDNA at levels below one part-per-million (<0.0001%).
Simple Blood Draw Could Revolutionize Early Breast Cancer Detection
Scientists at the University of Southern California (USC, Los Angeles, CA, USA; www.usc.edu) have found indications that a special blood test called a liquid biopsy could determine whether a patient has breast can-
cer at its early stage and if that cancer is unlikely to return. The results of the study demonstrating the liquid biopsy results for early breast cancer detection raise hopes that one day doctors could detect breast cancer in patients with a simple blood draw.
Non-Invasive Process Determines
Blood Glucose Levels Without Sample
Researchers at Kennesaw State University (Kennesaw, GA, USA; www.kennesaw.edu) have created a noninvasive process that can identify the exact value of blood glucose with 90% accuracy without taking a blood sample. Their GlucoCheck process uses light shone through human tissue, in either the ear or finger, and a small camera to capture images on the other side. The team then uses a model to study the amount of light absorption in those images to determine blood glucose concentration.
AI Diagnostic Tool Could Deliver Results in Two Minutes Using Fingertip Sample
Biofluids such as synovial fluid, blood plasma, and saliva contain proteins that are an important biomarker for the diagnosis of several health conditions. Scientists at Swansea University (Swansea, UK; www.swansea.ac.uk) have programmed a specially designed platform to detect the concentration of these proteins to assist in diagnosis and monitoring disease progression. The platform would use AI to speed up the process of detecting biomarkers in biofluids, resulting in faster test results for health conditions such as cardiovascular disorders and Alzheimer’s disease.
NOVEMBER
Advanced Mass Spectrometry Could Provide Clues to Beating Brain Tumors
Glioblastoma is one of the most difficult cancers, let alone diseases, to treat. Now, scientists at the Hackensack Meridian Center for Discovery and Innovation (CDI, Nutley, NJ, USA; www.hmh-cdi.org) have found that a critical new pathway to treating glioblastoma might be found in the complex diversity within the tumor tissue. Using advanced mass spectrometry the team assessed five human samples of brain tumor tissue and looked at a series of different lipids, in different sections of the tumor and the surrounding environment, and found a series of possible treatment candidates.
Immunoassay, Mini Mass Spec Combined for Malaria Detection
Biochemists at Ohio State University (Columbus, OH, USA; www.osu.edu) have used ionic probe technology for malaria detection that provides a more stable
alternative to reagents used in conventional immunoassays, making them potentially useful for work in areas where cold storage is not widely available. The stability and sensitivity of the developed paper-based immunoassay platform will allow miniature mass spectrometers to be used for POC malaria detection as well as in large-scale surveillance screening to aid eradication programs.
First-Ever Saliva-Based Concussion Test to Be Available In 2023

(Nijmegen, The Netherlands; www.enzyre.com) is developing a breakthrough ambulant diagnostic technology for blood coagulation testing that will enable hemophilia patients to determine and monitor their coagulation status from home in real-time. The company’s unique diagnostic technology platform, EnzyPad, will enable hemophilia patients to self-test in a home setting, and eventually transfer their coagulation status to their treating physician through a mobile phone app and a cloud-based application.
New Analyzer Monitors Blood Ammonium Levels at POC
Researchers at the Autonomous University of Barcelona (UAB, Barcelona, Spain; www.uab.cat) have developed an analyzer that monitors the level of ammonium in the patient’s blood using a decentralized method at the POC, instead of sending blood samples to be analyzed in hospital laboratories, where traditional equipment is used. The new device aims to decentralize the blood ammonium determination, making the analysis possible at smaller healthcare centers with direct blood measurements and no need for previous treatments.
Revolutionary, Non-Invasive Technique Measures Glucose Levels without Drawing Blood
In a recent study affiliated with UNIST (Ulsan, Korea; www.unist.ac.kr), researchers have demonstrated a revolution-
ary, non-invasive technique for testing blood glucose levels, using electromagnetic (EM)-wave-based glucose sensor inserted under the skin. Their findings have attracted much attention, as it eliminates the need for patients with diabetes to constantly prick their fingers with a glucose meter.
New Molecular Microscopy Tool Uncovers Breast Cancer Spread
Researchers from the Wellcome Sanger Institute (Cambridgeshire, UK; www.sanger.ac.uk) have developed a new technology that can trace which populations of breast cancer cells are responsible for the spread of the disease, and for the first time highlights how the location of cancer cells could be as important as mutations in tumor growth. The results suggest that sometimes it is not only the genetics that influence how cancers grow, but also the location of the tumors.
First-Ever Blood Test Could Detect Deadly Heart Inflammation within Hours
Researchers at Queen Mary University of London (London, UK; www.qmul.ac.uk ) have discovered a molecular signal in the blood that could pave the way for the first blood test to diagnose myocarditis, or inflammation of the heart muscle, in as little as a year. The researchers hope that this finding will improve diagnosis of myocarditis and help people to get the treatment they need earlier, reducing the risk that they will develop life-threatening complications such as abnormal heart rhythms or heart failure.
Blood Test Could Screen for Changes in Proteins Prior to Breast Cancer Diagnosis
Researchers from Leiden University Medical Center (Leiden, The Netherlands; www. universiteitleiden.nl) used mass spectrometry to detect changes in the levels of particular proteins in people’s blood up to two years before they were diagnosed with breast cancer. The findings could form the basis of blood testing for people with a genetic predisposition or a family history of breast cancer to ensure the disease is diagnosed early when the chances of survival are greatest.
DECEMBER
Low Cost, Sensitive Blood Test for Cancer Makes Regular Monitoring Affordable
Scientists from the National University of Singapore (NUS, Singapore; www. nus.edu.sg) have discovered a novel lowcost method of testing for cancers. Called the Heatrich-BS assay, the new test sequences clinical samples that have been heated in order to isolate cancer-specific signatures found in a patient’s blood. By comparing the results of their blood analysis with CT scans that imaged the size of patients’ tumors, the team found that there was a high correlation between how much cancer-specific DNA was detected
in a patient’s blood sample and the size of their tumors over time.
New Blood Test Detects ‘Toxic’ Protein Years Before Alzheimer’s Symptoms
Previous research has shown that the seeds of Alzheimer’s are planted years – even decades – earlier, long before the cognitive impairments surface that make a diagnosis possible. Those seeds are amyloid beta proteins that misfold and clump together, forming small aggregates called oligomers. Now, a team led by researchers at the University of Washington (Seattle, WA, USA; www.washington.edu) has developed a laboratory test that can measure levels of amyloid beta oligomers in blood samples.
Blood Test Separating Bacterial and Viral Infections Reduces Antibiotic Overuse
Existing methods to diagnose whether a patient has a bacterial or viral infection include growing the pathogen in a petri dish, which takes several days, or polymerase chain reaction (PCR) testing, which requires knowing the specific pathogen to look for. Now, a new gene expression-based test developed by scientists at Stanford Medicine (Stanford, CA, USA; www.med.stanford.edu) could allow doctors around the world to quickly and accurately distinguish between bacterial and viral infections, thereby cutting down on antibiotic overuse.
AI Pathology Tool Helps Make Real-Time Diagnosis during Surgery
When surgeons send samples to a pathologist for examination, both
speed and accuracy are of the essence. The current gold-standard approach for examining tissues often takes too long and a faster approach, which involves freezing tissue, can introduce artifacts that can complicate diagnostics. Now, researchers at the Brigham and Women’s Hospital (Boston, MA, USA; www.brighamandwomens.org) have developed a new method that leverages AI to translate between frozen sections and the gold-standard approach, thereby improving the quality of images to increase the accuracy of rapid diagnostics.
Groundbreaking Method for Multi-Cancer Early Detection is More Practical and Cheaper
An international collaboration, researchers from Chalmers University of Technology (Gothenburg, Sweden; www.chalmers.se) have developed a new method for Multi-Cancer Early Detection (MCED) based on human metabolism. The method is based on an earlier discovery that so-called glycosaminoglycans – a type of sugar that is an important part of our metabolism – are excellent biomarkers to detect cancer noninvasively, paving the way to new opportunities for cheaper and more effective cancer screening.
World’s First Rapid Saliva-Based Pregnancy Test Eliminates Need for Blood and Urine Samples
Salistick from Salignostics (Jerusalem, Israel; www.salignostics.com) is the first rapid saliva-based pregnancy test for early and highly accurate pregnancy detection that does not require urine. The kit is based on the company’s revolutionary technology which detects the pregnancy hormone β-hCG in saliva from the first day of the missed period. SaliStick is expected to be available for purchase in several European countries, as well as South Africa, UAE, and Israel from the first quarter of 2023.
Simple Blood Test Can Detect Predictive Biomarker for Cognitive Impairment and Dementia
Ergothioneine (ET) is a unique diet-derived compound discovered more than 100 years ago by Charles Tanret. However, it was only in 2005 when scientists discovered a transporter specific for ET that facilitates the uptake and accumulation of ET in the body. Now, a new study by researchers from the National University of Singapore (NUS, Singapore; www.nus. edu.sg) has revealed that low levels of ET in blood plasma may predict an increased risk of cognitive impairment and dementia, suggesting possible therapeutic or early screening measures in the elderly.
Ultrafast, Ultra-Sensitive Protein Detection Method Enables Early Diagnosis
Methods to analyze trace amounts of proteins based on antigen–antibody reaction enable diagnosis at an early stage of many diseases, including cancer, dementia, and microbial infections. However, such methods either have limited sensitivity or require complex and time-consuming processing to allow antigen–antibody reactions to occur. Now, scientists at Osaka Metropolitan University (OMU, Osaka, Japan; www.omu.ac.jp) have discovered a new principle underlying light-induced acceleration of antigen–antibody reaction, allowing for simple, ultrafast, and highly sensitive detection of proteins.
PCR-Based CRC Early Detection Stool Test Fills Important Gap between FIT and Colonoscopy
Asimple-to-administer stool test detects colorectal cancer (CRC) with a sensitivity and specificity nearly as high as the invasive colonoscopy. Mainz Biomed N.V.’s (Mainz, Germany; www.mainzbiomed. com) ColoAlert is a highly efficacious and easy-to-use CRC test that detects bleeding and non-bleeding tumors through tumor DNA analysis and thus offers a better early detection than FOB tests. The ColoAlert stool test identifies changes in the bowel as accurately and early as possible by detecting tumor DNA and CRC cases in their earliest stages.
Blood Test for Protein Biomarker Could Detect Pancreatic Cancer at Very Early Stage
Astudy by researchers at Umeå University (Umeå, Sweden; www.umu.se) has found that in some people diagnosed with pancreatic cancer, levels of a particular substance in their blood can be seen to start rising slowly as early as two years before the cancer was diagnosed. This raises the possibility for future research to find ways to detect the dreaded cancer earlier.
