An Introduction to Our Lab
“Communication and knowing the patients needs are the most important tools to achieve outstanding results! The way we work at BVDL allows us to be in constant contact with the clinicians and, if possible, the patient.”
Request Team
The innovative design and organization of our lab combines the best of both worlds. Doctors choose and work with the same technicians all the time, developing the personal one-on-one relationship necessary for consistent results. As experienced technicians from around the world, we have each other’s knowledge and expertise as a resource, along with the materials, equipment, and benefits of a mid-size, full service lab. For over two decades, our foundation and key to success has been a solid background with the Pankey/Dawson concepts in occlusion and function. This, combined with the AACD standards for materials and techniques with cosmetics has led to a level of predictable success in functional esthetics; which is usually reserved for private in-house doctor/technicians teams. We have the real cosmetic/occlusal connection! Our services address the needs of today’s contemporary fee-for-service practice. Along with most general dentistry procedures, our specialties are with cosmetic work, implants, and combination fixed/removable cases. You will also benefit from our 30 years of experience with pretreatment case planning. We view cases with the “end in mind” and what it will take to achieve those results. From soft tissue changes to case design and material selection, we combine our collective knowledge and experience to your case planning. This helps to avoid problems and can make, even the most complex cases predictably successful.

Pretreatment Planning
“Envision the end before you begin!” These are Dr. Peter Dawson’s profound words of advice. With over 500,000 units of experience from which to draw, we welcome the opportunity to participate in your pretreatment case work-up, especially with implant, combination fixed/removable, and complex multi-disciplinary restorations. We are glad to provide suggestions for preliminary mouth preparations to improve esthetics, the choice of materials, case design, and diagnostic wax-ups to facilitate fabrication of provisional restorations.

Pretreatment/Case Planning Photo Checklist To assist you in treatment planning, we need your insights through photographs. We have compiled a number of views to give you our best.
1. Full face smiling • have patient sit upright or stand • pictures should be taken at a right angle, with patient’s head straight • smile should be natural not forced
6. Maximum Intercuspation with retractors • shows front-to-back progression
2. Smile - without retractors • frame the image just inside nose-to chin; captures relationship of upper smile line to lower lip line
7. Rest position • to show display relationship of incisal plane of lower teeth to lower lip • very important image to evaluate wear cases
3. Profile - close-up • shows the upper incisal edge position to inner vermillion border of lower lip
8. Smile - teeth apart • displays the silhouette of upper and lower incisal plane
4. High smile line • have patient laugh or smile as high as possible to determine maximum tooth display • provides information on embrasures, symmetry of gingival heights, and information for margin placement
9. Smile - head tipped down slightly (with patient sitting, shoot down from above) • shows the relationship of arch form to lower lip • especially good for reviewing anterior pontics
5. “E” position • shows relationship of upper incisors to upper lip • age appropriate tooth wear may be evaluated
In cases with severe wear, request an old photo from high school, college, or a wedding.
Margin placement is critical, especially if gingival embrasures will show. Here, supragingival margins have resulted in “black holes between teeth.
Golden proportion is one of several tools to consider in the case wax-up.
The relationship of the upper smileline to the lower lip line is one of the most important images to capture in case planning.
The every other tooth technique, as described by Dr. Dawson, is one of the most precise ways to communicate incisal edge position.

Diagnostic Wax-up There is no question the diagnostic wax-up can be an invaluable aid, especially on comprehensive multiple unit cases. These cases are handled with high priority care. We can offer suggestions on materials, case design, preliminary mouth preparation, sequencing, equilibration, and estimates. The actual wax-up can be from a simple white wax-up with changes noted on the cast (top left), a full contour wax-up of all teeth involved (middle), to a multi color natural wax-up to share in case presentations with your patient (lower left).

Smile Imaging As an aid to help patients envision recommended treatment, Bay View offers a personalized computer imaging service. We do not use a “packaged imaging program to stamp teeth into place. Images are individually created according to your special instructions by technicians who understand your special needs. Personalized imagery allows the patient to compare the possible outcome of your treatment plan positioned next to their current dental situation. We are able to offer results as ink jet hard copies or digit images for your computer consultation/presentations.

Fixed/Detachable These are also sometimes referred to as “hybrid cases, since the design is somewhat between a fixed and removable. These cases are implant supported, screw retained, and thus removable, but only by the doctor, not the patient. They are typically used on the lower arch, ideally with 5 implants, placed in the anterior and premolar region, with the restoration cantilevered distally depending on the Anterior/ Posterior (AP) spread of the implants.
Removable Also referred to as fixed/removable since fixed work is involved with the implants, and the restoration superstructure with the teeth is removable by the patient for cleaning. These designs will include the largest variety, with more or fewer implants involved, and an almost endless number of bars, magnets, and attachments, available. These cases may be implant or tissue supported.

Case Criteria What we need to give you our best work! Quality costs less! Our experience shows that thorough treatment planning, investing more time in preliminary mouth preparation, and taking accurate records will consistently save time and improve the end results with even the most complex cases. We submit the following suggestions.
• Study cast of originals - these should include full extension of the ridges, flanges, and palate. • Facebow mounted study cast of provisionals - not the preparations; this should be mounted in the office to avoid distortion in shipment. • Pre-op images - with and without retractors; see our photo guidelines and checklists on pages 15 and 32. • Diagnostic images of temporaries and full face - without retractors, that show the relationship of teeth-to-lips. • Shading images of natural teeth - with the shade guide in the picture. • Well-prepared teeth - two plane reduction; be sure to avoid undercuts with deep chamfer shoulder preps. • Full arch impressions of casts - including full extension of flanges and palate. • Accurate centric relation/maximum intercuspation bite record - we recommend Blu-Mousse or Futar D. • Posterior neutral zone registration - if edentulous. • Written detailed instructions. • Enough time to do a great job - usually 2-3 weeks (large cases, please call to preschedule).

Scheduling
Because of the organization of our laboratory with each technician working separately, the schedules may vary. Smaller cases can often be absorbed into the schedule and returned within a 2-3 week turn around. Larger cases (six units or more) should be pre scheduled for best service. Time is also blocked off differently for certain types of restorations like veneers and porcelain inlays which routinely have a shorter turnaround.
Shipping and Receiving We provide prescription pads and inbound shipping via UPS for all cases. Just call our case coordinators to send you Rx forms and shipping labels. Before sending out a case, please, disinfect all impressions, models, and bites and carefully wrap them individually (we recommend bubble wrap). Seal prescriptions separately in plastic zip-lock bags.
OSHA Guidelines Our lab carefully follows OSHA’s new standards for shipping and receiving work. We do not recycle incoming packaging materials and we carefully redisinfect incoming impressions, models, and bites. We redisinfect outgoing work and heat-seal it for return shipment to you.

With over three decades of experience, we submit the following suggestions to make selecting and communicating critical shading information more predictable. • Have the patient sit upright or stand.

marketing. We have compiled the following list of suggestions for improving your photography.
• Remember, depth-of-field is generally 1/3 in front and 2/3 behind the focal point. For anterior clinical shots, manually focusing on the laterals produces better results than using the automatic focus. • Keep a ledger of each shot you take to help identify patients. If you have several patients on the same card, take a shot of each patient holding a nametag or take a shot of the name on the record. • If in need of a digital camera system, we highly recommend the Cannon 20D setup. General Dentistry Monolithic Posterior Restorations
Monolithic Posterior Restoration • $195.00 complete Includes model & die work and any of these monolithic shade restorations
Monolithic Posterior Restorations: Full contour high translucency IPS e.max Recent studies have shown full contour monolithic IPS e.max to be one of the strongest restorations in dentistry. It is also very esthetic. These restorations are ideal for premolars where esthetics are more visible. Since this material is very translucent, it is contra-indicated when the underlying prep is very dark and discolored. Best results are with a modified shoulder bur (Komet #6847KR) for margins and 3M RelyX Unicem2 resin material for cementation.
The technology, materials, and techniques may have changed, but the important concepts of contacts, occlusion, fit, and
Zirconia frame with GC Initial porcelain This is an exciting new ceramic material with beautiful fluorescence and opalescence, layered over our own CAD/CAM designed zirconia frames. Layered zirconia restorations have shown a lower failure and fracture rate than metal-ceramics or IPS e.max. The esthetics are exceptional and with the added strength of a zirconia frame, restorations can easily be cemented. These are excellent restorations for any posterior units, and particularly for small bridges and splinted units. The zirconia frame will also block out a dark or discolored prep. Best results are with a modified shoulder bur (Komet #6847KR) for margins and 3M RelyX Unicem2 resin material for cementation.
proper contours are still the same. The lost wax technique has given way to direct metal laser sintering for metal copings with perfect margins. It’s more accurate
Noble metal frame with GC Initial porcelain An exciting and high-tech upgrade to traditional metal-ceramics. These restorations feature an amazing new technology called direct metal laser sintering to create the metal copings. The frame is then opaqued and layered with the special Initial IQ porcelain just like the zirconia frames. These restorations are indicated anywhere traditional metal ceramics are preferred and may be conventionally cemented. Best results are with a modified shoulder bur (Komet #6847KR) for margins and 3M RelyX Unicem2 resin material for cementation.
and less costly. Lithium disilicate is now the new standard for all-ceramic restorations, and CAD/CAM designed zirconia full crowns have become the new “white gold”
Full contour zirconia restorations These restorations provide the strongest and most fracture resistant tooth colored material available in dentistry today. Because of the reduced translucency and esthetics, we recommend these restorations for molars only. They will appear more opaque than other options. However, when a full-cast gold crown is contraindicated due to cost or color, this is an excellent alternative you can prescribe with total confidence. These restorations can be conventionally cemented and are virtually indestructible. Best results are with a modified shoulder bur for margins (Komet #6847KR).
restoration. These exciting developments are revolutionizing laboratory technology and we are keeping up with the fast pace of change. The 75 year history of our family business was built on general restorative dentistry and we are very proud
Important Monolithic Crown Shading Tip It is important to remember that monolithic restorations use one solid layer of veneering material to achieve their strength and fabrication efficiency. Subtle color variations will come coming from the underlying tooth, frame, and fluorescence of the live stains. These restorations look very natural and blend nicely on most cases. However, we are somewhat limited and unable to duplicate very customized effects. For those cases, the option of custom layered restorations should be considered.
to continue that tradition by adding to our successful series of affordable restorations utilizing the newest in technology. These monolithic posterior restorations are guaranteed with a two week turn-around*.
Specific details for each type of restoration are provided to the right. • Two weeks from the day we receive the case.

Removable, Fixed/Removable, and Implant Removable Restorations
Removable cases will generally start with custom tray, bite rim and denture set-up to establish tooth position. This is a critical step in the process as everything will progress from that original determination. To help facilitate things, we will send small HD video cameras to record the set-up try-in and facilitate lab communication. For most implant/removable cases, after tooth position has been set-up, we will send a full contour composite frame to verify incisal edge position, vertical dimension, and centric relation. This extra step has proven to be a significant benefit as these critical functional determinates can be verified more effectively with a fixed appliance.
