You do not have to be good in playing, just running after the ball alone will help get you into shape. This is a great way to strengthen your cardiovascular system and lose weight. DANCING: - Dancing is a fun and it really does not matter what type of dance or music. The whole idea is to move your body. Dancing has long been recommended as an avenue to fitness. Belly Dancing is an excellent workout for your stomach, waist and hips ladies. VCR :- If you have a VCR or DVD, rather than just using it for your favorite comedy or action-packed movie, try sticking in some good workout tapes. Even taking 15 minutes every day to work out will get you started. Try that for two weeks and you will be surprised at the results. Once you see that 15 minutes a day makes a difference, you will be encouraged to increase the time spent. YARDWORK: - Whether you’re pulling weeds, planting flowers, mowing the grass or chopping wood: All of these activities can help you get into better shape. Don’t cheat yourself though. Using a riding mower to cut the grass won’t help you use the muscles or get your blood pumping.

Mastalgia or Breast pain

Breast pain (mastalgia) is the most common breast related complaint among women. Almost 80% of women experience pain in the breast/axilla, during there lifetime. But only 10 to 20% need treatment. Though 90% of breast pain is not due to breast cancer the woman should consult in the event of breast pain. There are two main types of breast pain: ÂÂ Cyclical ÂÂ Non-cyclical ¾¾ Cyclical breast pain : This type of pain has characteristic features, which varies from one woman to other, and is individualised. Typically there is feeling of heaviness in breast mostly bilateral (though patient may feel more pain in one breast than opposite), usually diffuse starts just before menstrual cycle or exaggerated if already continuous pain is present. Increase in pain is present during the period for 2-5 days and then subsides with end of cycle. The cause of this premenstrual pain is due to hormonal (estrogen and progesterone), Mediated water retention in the breast tissue and distension of the ducts and glands within breast. This pain may be associated with feeling of swelling or lumpiness, then it is termed as fibrocystic disease. This pain subsides after menopause. Though the pain is due to hormonal changes, many factors like stress, weight gain, diet and life style determine the pattern of pain particular person, which can only be clarified by monitoring pain chart monthly and treating the individual accordingly after ruling out cancer. ¾¾ Non-cyclical breast pain : This is a less common type of pain which may occur due to causes within breast or outside. Usually seen in middle or elderly women. It may also occur in younger women with history o f injury, operation on breast. This is typically localised to one part of breast and patient can often pinpoint the site. It may be tender to touch. It is not due to hormonal changes except in few postmenopausal women tking hormonal replacement therapy. It is not influenced by pattern of menstrual cycles and may persistent. A special and common type of Non-cyclical breast pain is Tietz syndrome (Costochondritis), due to inflammation (Arthritis), of Costochondral junction (junction of rib with cartilage), occuring >40 years. This pain is actually outside breast but is felt through breast. The other causes are increasing age, osteoporosis, poor posture, ill-fitting bra, biopsy and old trauma to chest. ¾¾ Approach : Once a woman is suffering from persistent breast pain she should consult a physician, who will evaluate the pain by detailed clinical history and examination, followed by mammography&/or ultrasonography supplemented by biopsy if needed, all these methods used according to the findings and

need. After diagnosing and once breast cancer is excluded, the appropriate treatment is instituted after discussing with the patient. ¾¾ Treatment : Most of the women don’t need any specific treatment the form of drugs or surgery. Nearly 70% of women can controlled with reassurance and life style modification. Usually a step by step approach with increasing complexity is used depending on motivation and response of the women. Re assurance --> Pain chart and life style modification -> Mild analgesics -> Primrose oil, Vitamin E The reassurance includes explaining the woman that the pain is not due to cancer or any surgical disease and is a benign condition without harm to life. Then motivating her to mark the severity and timing of pain on pain chart daily 3 times. This is red by the physician and advises the appropriate measures. Minor life style modifications include: ÂÂ Avoiding excess weight gain ÂÂ Avoiding excess fat in diet ÂÂ Avoiding smoking and alcohol intake ÂÂ More of fresh fruits and vegetables with high vitamin content ÂÂ Avoiding excess coffee and chocolates ÂÂ Reducing high salt intake in diet ÂÂ Occasional use of analgesics. All these measures help in red uction of pain score in most of the patients. If it is still persistent then primerose oil is advised. Some physicians advie vitamin E and its role is doubtful. With reassurance, pain charting, adjustments in diet and habits most women respond. If still pain is persistent then, prescription of medication is thought off. The breast pain is mostly influenced by psychology of the person. There is low threshold for prescription of the drugs, as the benefit with many drugs available for breast pain are marginal or temporary and are associated with many other serious side effects and are not cost effective for many women.

Spiced Corn & Rice Pilaf

Ingredients •2 teaspoons Cardia Olive Oil •1/4 cup finely chopped onion •1 3-inch cinnamon sticks •3/4 teaspoon cumin seeds •1/4 teaspoon ground cardamom •1/4 teaspoon salt •1 cup brown basmati or long-grain brown rice •2 3/4 cups reduced-sodium chicken broth, or vegetable broth •2 tablespoons hulled pumpkin seeds •1 cup fresh corn kernels, (from 2 ears) or frozen Preparation Heat oil in a large saucepan over medium-high heat. Add onion and cook, stirring often, until lightly browned, about 3 minutes. Add cinnamon stick, cumin seeds, cardamom, salt and rice; cook, stirring often, until spices are fragrant, about 1 minute. Stir in broth and bring to a boil. Reduce heat to low; cover and simmer until the liquid is absorbed and the rice is tender, 35 to 40 minutes. Meanwhile, toast pumpkin seeds in a small dry skillet over medium-low heat, stirring constantly, until fragrant, 1 to 2 minutes. Transfer to a bowl to cool. When the rice is ready, stir in corn, cover and cook until heated through, about 5 minutes. Remove the cinnamon stick. Fluff the pilaf with a fork and fold in the toasted pumpkin seeds. Nutrition Per serving: 129 calories; 3 g fat ( 1 g sat , 2 g mono ); 2 mg cholesterol; 22 g carbohydrates; 0 g added sugars; 4 g protein; 2 g fiber; 126 mg sodium; 80 mg potassium. IGRT for Breast Cancer

Traditional Radiation techniques treating breast cancers, incorporated large volumes of the heart and had negative impact on overall survival and outcome. A relative increase of 30% in cardiac deaths was found for women treated before the 1980s. Modern radiation modalities are aimed at minimising dose to the heart, the latest being image guided radiation therapy.

IGRT C-T Simulation

Better sparing of lung tissue using IMRT IGRT is a major technical innovation for high precision radiation therapy. IGRT requires new high end linear accelerators which also incorporate a CT scanner. It takes into account all possible technical, positional and physiological changes in the tumor and the normal tissues by taking CT scan images during the dose (radiation) delivery on a daily basis. We share our experience of respiratory gated IGRT technique in reducing dose to normal heart in a patieA patient of carcinoma left breast underwent breast conservation surgery by way of lumpectomy and axillary lymph node dissection. After completion of chemotherapy, she was planned for radiation therapy to whole breast. Figure 1 shows radiotherapy treatment plan with gated IGRT on axial slice, while figure 2 shows conventional treatment at the same slice. Using gated IGRT technique, it is possible to keep heart outside

Fruit cakes are an all time favourite snack and dessert for people of all ages. Easy to make and good to eat, let’s learn here to make healthy fruit cake. Ingredients •1 1/2 cups fresh orange juice •1 1/2 cups chopped candied orange peel, (about 8 ounces) •1 cup chopped dried cherries, (about 6 ounces) •1 cup chopped dried apricots, (about 6 ounces) •1 cup chopped dried pineapple, (about 6 ounces) •3/4 cup currants, (about 4 ounces) •1 cup all-purpose flour, divided •3/4 cup whole-wheat pastry flour •1 teaspoon ground cinnamon •1/2 teaspoon ground allspice •1/2 teaspoon baking powder •1/2 teaspoon salt •1/2 cup sugar •1/4 cup Cardia Olive Oil, or Gold Winner Sunflower Oil •2 tablespoons unsalted butter, at room temperature •3 tablespoons molasses •1 teaspoon vanilla extract •2 large egg whites, divided •3 glacéed red cherries cut in half, for garnish •8 pecan halves, for garnish ¾¾ Preparation ªª Position rack in center of oven; preheat to 275°F. Coat two 8 1/2-by-4 1/2-inch loaf pans or 3 mini loaf pans (6-by-3-inch) with cooking spray and lightly dust with flour. ªª Bring orange juice to a simmer in a small pan over medium-high heat. Mix orange peel, dried cherries, apricots, pineapple and currants in a large bowl; pour the warm juice over the fruit and let sit until most of the liquid is absorbed, about 10 minutes. Strain out excess juice; toss the fruit with 1/2 cup all-purpose flour until well coated. Set aside. ªª Whisk the remaining 1/2 cup all-purpose flour, whole-wheat flour, cinnamon, allspice, baking powder and salt in a medium bowl until blended. ªª Place sugar, oil and butter in a large bowl. Beat with an electric mixer on medium speed until smooth and creamy, about 2 minutes. Beat in molasses and vanilla, and then add egg whites one at a time, scraping the sides of the bowl as necessary. ªª Stir in the dry ingredients. Pour the batter over the fruit and mix well. Divide the mixture among the prepared pans. Decorate the tops with glacéed cherries and pecans. ªª Bake the cakes until lightly browned and firm to the touch, about 1 hour and 40 minutes. A toothpick inserted in the center should come out clean. Let cool in the pans on wire racks for 10 minutes. Unmold and cool completely. ¾¾ Tips & Notes To “ripen” fruitcakes, wrap the cooled loaves in a piece of cheesecloth drizzled with 2 tablespoons bourbon or brandy, then seal tightly in plastic wrap and foil; refrigerate for up to 2 months, wetting the cloth every 2 weeks. Thyroid Nodules

Thyroid nodules are lumps which commonly arise within an otherwise normal thyroid gland. They may indicate a thyroid neoplasm, but only a small percentage of these are thyroid cancers. However, the most important issue to be addressed during evaluation of a thyroid nodule is to rule out malignancy/ malignant potential in the nodule ¾¾ Causes 1. Adenoma 2. Dominant Nodule of Multinodular goitre 3. Thyroid cyst 4. Malignancy ¾¾ Presentation: Often these abnormal growths of thyroid tissue are located at the edge of the thyroid gland so they can be felt as a lump in the throat???. When they are large or when they occur in very thin individuals, they can even sometimes be seen as a lump in the front of the neck. Sometimes a thyroid nodule presents as a fluidfilled cavity (called a thyroid cyst). Often, solid components are mixed with the fluid. Thyroid cysts most commonly result from degenerating thyroid adenomas, which are benign, but they occasionally contain malignant solid components. ¾¾ Diagnosis: After a nodule is found during a physical examination, a referral to an endocrine specialist may occur. Most commonly an ultrasound is performed to confirm the presence of a nodule, and assess the status of the whole gland. Measurement of thyroid stimulating hormone and anti-thyroid antibodies will help decide if there is a functional thyroid disease such as Hashimoto’s thyroiditis present, a known cause of a benign nodular goiter. Fine needle biopsy for histopathology is also used. ¾¾ Fine needle biopsy One approach used to determine whether the nodule is malignant is the fine needle biopsy (FNB), which some have described as the most cost-effective,

sensitive and accurate test. FNB or ultrasound-guided FNA usually yields sufficient thyroid cells to assess the risk of malignancy, although in some cases, the suspected nodule may need to be removed surgically for pathological examination. ¾¾ Blood tests Blood or imaging tests may be done prior to or in lieu of a biopsy. The possibility of a nodule which secretes thyroid hormone (which is less likely to be cancer) or hypothyroidism is investigated by measuring thyroid stimulating hormone (TSH), and the thyroid hormones thyroxine (T4) and triiodothyronine (T3). Tests for serum thyroid autoantibodies are sometimes done .as these may indicate autoimmune thyroid disease (which can mimic nodular disease). ¾¾ Imaging The blood assays may be accompanied by ultrasound imaging of the nodule to determine the position, size and texture, and to assess whether the nodule may be cystic (fluid filled). Also suspicious findings in a nodule are hypoechoic, irregular borders, microcalcifications, or very high levels of blood flow within the nodule. Less suspicious findings in benign nodules include, hyperechoic, comet tail artifacts from colloid, no blood flow in the nodule and a halo, or smooth border. Nuclear medicine can be used to image the thyroid with radioactive technetium (Tc)

or iodine (I) imaging of the thyroid. An 123I scan showing a hot nodule, accompanied by a lower than normal TSH, is strong evidence that the nodule is not cancerous. ¾¾ Treatment of benign nodules If a thyroid nodule isn’t cancerous, there are several treatment options: Watchful waiting. If a biopsy shows that you have a benign thyroid nodule, your doctor may suggest simply watching your condition, which usually means having a physical exam, ultrasound scan and thyroid function tests at regular intervals. Thyroid hormone suppression therapy. This involves treating a benign nodule with levothyroxine , a synthetic form of thyroxine that you take in pill form. The idea is that supplying additional thyroid hormone will signal the pituitary to produce less TSH, the hormone that stimulates the growth of thyroid tissue. Although this sounds good in theory, levothyroxine therapy is a matter of some debate. There’s no clear evidence that the treatment consistently shrinks nodules or even that shrinking small, benign nodules is necessary. Surgery. All malignant thyroid nodules require surgical excision. Occasionally, a nodule that’s clearly benign may require surgery, especially if it’s so large that it makes it hard to breathe or swallow. Surgery is also considered the best option for people with large multinodular goiters, particularly when the goiters constrict airways, the esophagus or blood vessels. Nodules diagnosed as indeterminate or suspicious by a biopsy also need surgical removal, so they can be examined more thoroughly for signs of cancer.

There are some specially designed thicker soft lenses retain more of a rigid shape and may contribute to the liquid lens effect to some extent. There are designs being used for keratoconus, and they are helpful in mild to moderate cases. ¾¾ RGP CONTACT LENSES Rigid Gas Permeable (RGP or GP) contact lenses are primary option for correcting KC vision. The rigid lens masks the underlying irregular cornea and functions as the new refractive surface of the eye, with the tear film filling in the space between the back of the contact lens and the front of the eye. “Rigid” defines the type of lens. “Gas Permeable” describes the lens material. There are many different RGP lens designs. ¾¾ Without a Contact Lens With a Rigid Contact Lens “Piggy-backs”: This is a two system: an RGP lens worn on top of a soft lens. The RGP lens provides crisp vision and the soft lens acts as a cushion providing comfort. Hybrid lenses: This is a lens combination that has an RGP center surrounded by a soft skirt. Scleral lenses: These are large diameter lenses that rest on the white part of the eye, called the sclera, and vaults over the cornea.

The size can be a scary prospect for some, but scleral lenses have many advantages. Because of their size, they do not fall out, dust or dirt particles cannot get behind them during wear. They are surprisingly comfortable to wear because the edges of the lens rests above and below the eye lid margins so there is no lens awareness. The introduction of rigid gas permeable (RGP) materials has made this design more readily available. ¾¾ CONTACT LENSES Eye infections, while infrequent, can be devastating, preventing patients from wearing their contact lenses for days or weeks and sometimes resulting in permanent corneal scarring and loss of vision. A thorough understanding of the role played by each of the rigid lens care products will help to keep lenses clean and eyes healthy. ¾¾ CARE SYSTEMS FOR RIGID LENSES They consist of: A daily cleaner, to remove tear film oils, mucus, cosmetics, and other debris from lens surfaces. Cleaning solution containers have a red tip to warn patients that the solution should not be put in the eyes. If cleaner is accidentally instilled in the eyes, it should be flushed out immediately, preferably by dunking the entire face into a sink filled with water. A combination wetting/soaking solution for lens storage and reinsertion in the eye, which kills bacteria and other microorganisms, keeps lenses from warping, and acts as a “cushioning agent” when the lenses are placed in the eyes . An optional enzymatic cleaner (in either liquid or tablet form), for patients who build up a lot of protein on their lenses. Not all rigid lens wearers need an enzymatic cleaner. A lubricating and rewetting drop, to instill in the eye while you are wearing lenses. This will help to flush debris from under the lenses and help the lenses to glide more smoothly and comfortably in the eyes. Rub the lens between your fingers! It may warp or crack! If the lens curvature is too steep for your index finger to clean the inner lens surface, try using your pinkie or a cotton swab. Rinse the lens with cool or lukewarm water. Hot water will warp the lens. Do not rinse your lenses over an open drain! Replace the right lens in the case and repeat the steps for your left lens. Soak lenses overnight or for at least 4 hours. In the morning, wash your hands and insert your lenses directly from the wetting/soaking or

conditioning solution. Do not rinse first! The solution will help to cushion the lenses as you insert them. In addition, if the lenses are rinsed with water after they have been disinfected, there is a chance that they may become contaminated from impurities in the water supply. ¾¾ ABNORMAL SYMPTOMS: ªªPain: when placing lenses on the eyes, while wearing the lenses, or after removing them ªªBurning, a sensation of heat, redness, excessive tearing, or discharge ªªInability to keep the eyes open ªª Extreme sensitivity to light ªª Severe or persistent haze, fog, or rainbows around lights ªªSevere irritation ªªWhite spots on the cornea ¾¾ DO’S AND DON’TS FOR RIGID LENS WEARERS Clean your lenses at night, immediately after you remove them from your eyes. Leaving them in the soaking solution without cleaning until morning reduces the effectiveness of the disinfecting chemicals. In addition, rinsing the cleaner off with tap water just prior to replacing the lenses in your eyes might introduce bacteria or other microorganisms that would ordinarily have been killed while the lenses soaked overnight. ªª If your lenses ever dry out, soak them for at least 4 hours before you wear them again, since they may have warped or flattened in curvature when the liquid evaporated. ªª Do not mix and match solutions made by different manufacturers. Doing so may cloud your lenses, cause them to become gummy, or irritate your eyes. Make sure that the cleaning, soaking, disinfecting, and wetting/rewetting products you buy are all part of the same care system. ªª Always store your lenses in a case with ridges on the bottom. Lenses tend to suction onto smoothbottomed cases and may chip if you try to pry them up at the edges. ªª Replace your case each time you buy a new bottle of wetting/soaking or cleaning/soaking solution. Cases tend to become contaminated. You can clean case with your contact lens cleaner using a brand new inexpensive toothbrush. Be sure to rinse the case well before using it again. ªª If the skin on your hands is rough, ask your contact lens fitter if you can use a manually-agitated device such as the Allergan Hydramat or a mechanical device such as the Clensatron. These devices work like miniature washing machines and may protect the lenses from becoming scratched from your rough skin. ªª If you remove your lenses with a suction cup make sure you know exactly where the lens is before placing the suction cup on the eye. Suction cups should be cleaned with contact lens cleaner and rinsed after each use. They can also be soaked in wetting/soaking or cleaning/soaking solution. If you drop a lens, do not drag it along a surface. Wet your finger with wetting/soaking or rewetting solution and touch it gently to the lens to lift it.

It consists of nerve tissue that senses the tight entering the eye. Its function is to send impulses through the optic nerve back to the brain, where it gets translated into the images that we see. The sclera is the whitish, opaque part of the eye, which is connected to the cornea. Its role is to provide protection and meet the purpose of attachment for the extraocular muscles that enables eye to move. ¾¾ How do our eyes work? Every day we see things with our eyes. We see the sky, trees, cats, birds and the faces of our friends, but how do we see all that? Light enters our eye through the pupil. The cornea, at the front of our eye, bends the light. The light passes through the lens to the retina. It then focuses on the retina, like a little picture of whatever we are looking at, but it is upside down! Nerve cells in the retina send that “Image” to our brain. The brain has the ability to interpret that the image is upside down and we see it right side up. ¾¾ Eye nutrition and Eye Health Like any other part of your body, your eyes also need looking after. Blocked tear ducts are a condition seen in 2 to 4 percent of newborns and of these cases, 90 percent resolve naturally. Amblyopia, or lazy eye as it is more commonly known, is a common condition that affects one out of every 40 children. To correct this condition the child will probably need to wear an eye patch on the normal eye. Although cataracts are much more common in adults, children can be born with this condition or develop it later. Cataracts must be removed as soon as possible irrespective of the age. Another common eye problem often seen in infants and children is strabismus, or crossed eyes. Surgery to correct strabismus is generally quick and painless and the child can usually resume normal activities within a day or two. More common in school-age children, infectious conjunctivitis, or pink eye, is one of the most common eye conditions, due to its highly contagious nature. Treatment of pink eye can depend on which type it is. Bacterial conjunctivitis is treated with antibiotics in a drop or ointment. Viral conjunctivitis cannot be treated with antibiotics and will usually clear up on its own in just a few days.

Gynecologic problems

Many women don’t suspect they’re pregnant until they’ve missed a period, but there may be other indications ªªSickness and/or feelings of nausea ªªBreast tenderness, tingling and enlargement ªª The small bumps - ‘Montgomery’s tubercules’ on your nipples becoming more obvious ªªStomach pains ªªTiredness ªªNeeding to urinate more often ªª Going off certain foods, such as coffee and fatty foods ªªIncreased vaginal discharge The length of your pregnancy is dated from the first day of your last period, even though you actually conceive two weeks or so after this date (depending on the length of your cycle). Your baby is likely to be born two weeks either side of the due date. An accurate assessment of the duration of pregnancy is of great importance, especially if the woman develops complications later in her pregnancy ¾¾ GIVE A PROPER MEDICAL HISTORY Some medical conditions may become worse during pregnancy, e.g. women with heart valve disease may go into cardiac failure while a hypertensive woman is at high risk of developing pregnancy induced hypertension. ¾¾ Tell the doctor if u have any of the following: (1) Hypertension. (2) Diabetes mellitus. (3) Rheumatic or other heart disease. (4) Epilepsy. (5) Asthma. (6) Tuberculosis. (7) Psychiatric illness. (8) Any other major illness. ¾¾ Blood tests Normally, a small sample of your blood is taken at your first antenatal appointment. You may also be asked to give a sample in later pregnancy. The first test can: (1) Identify your blood group. (2) See whether your blood is rhesus positive or negative (3) Check for

conditions that could affect your health or your baby’s (this may include HIV). (4) Check for anaemia The blood test shouldn’t be painful and should only take a minute or so Blood tests can also be used to estimate the risk of Down’s syndrome. A blood sample is taken at about 16 weeks to measure three substances: alpha-fetoprotein (AFP), unconjugated oestriol and human chorionic gonadotrophin. Together with the mother’s age, these give an estimate of risk. The level of AFP can also be used to assess the risk of a neural tube defect, such as spina bifida. Routine checks at appointments are likely to include: ªª Blood pressure ªª Weight ªª Palpation - feeling your tummy ªª Listening to your baby’s heart - this can be done with a stethoscope, a Pinard (looks like an ear trumpet) or a Doppler, which uses ultrasound ªª Questions about your baby’s movements ªª Urine tests ªª Checking for any swelling in your legs, arms or face ªª Questions about how you feel ¾¾ WHAT SYMPTOMS OR SIGNS, WHICH MAY INDICATE THE PRESENCE OF SERIOUS COMPLICATIONS, MUST BE DISCUSSED WITH THE WOMEN? 1. Gout is one of the forms of arthritis or joint inflammation. Usually one or two joints are affected at a time. Commonly joints of feet and ankles are inflamed in gout. The commonest site is the ball of the big toe. The first attack of gout subsides in a week or so without treatment. There may be a long gap of many months or years between attacks. As time passes the attacks become more frequent and more severe involving many joints. When the frequency of attacks increases progressive joint damage sets in. This is known as chronic gout. What is osteoporosis? ‘Osteo’ means bone. Osteoporosis literally means ‘porous bones’. Osteoporosis is thinning of the bones that occur with age. Bones become weak and fragile, until even a slight impact may cause them to break. ¾¾ What happens to bones during osteoporosis? By about 20, the average woman has acquired 98% of her skeletal mass. New Skills to Develop self-confidence I had an opportunity to interact with the readers of a famous Tamil Magazine. It was a two-day programme. At the end of the programme, a young man asked me. ‘Swamiji I am a post graduate. It is more than ten years since I completed my studies. So far, I have not received even a single interview card from the employment exchange. So, I am working as a cashier in a restaurant. This job has no connection whatsoever with my educational qualifications. Last month, there was a shortage of cash at the counter. I do not know how it happened. The shame of being accused loomed large in my mind. I thought could pawn one of my wife’s jewellery and replace the missing amount. But my wife

refused asked my mother. She only fired me, but did nothing to help me! Kidney problems: Blood vessels in the kidney get closed or become narrow and hence kidneys are damaged. 25% of the diabetics die of kidney failure. Heart problems: If blood sugar* is not controlled for years, the blood vessels become hard or thick leading to Atherosclerosis. Such persons may get heart disease, stroke, high B.P. etc. If there is the history of Diabetes in your family, get the Glucose Tolerance Test done once in a year. If you are a diabetic, get your blood sugar examined once in 2-3 months and change the treatment accordingly. Stop drinking and smoking. Get Lipid Profile Test of your blood (for cholesterol, HDL, LDL, Triglycerides). After 40, go for Master Health Check-up every year. Foot problems; There is every chance of feet getting damaged in diabetes. Therefore blood circulation will not be proper there, as a result they lose the sense of touch in their feet and legs. There is a chance of the thin skin on the feet getting damaged. It takes longer time for the wounds to heal. If you do not notice the wounds, they gather pus and lead to Gangrene. When the leg or any part of it is completely damaged, thay may have to be amputated. Cares to be taken Take the following cares regarding the feet and the fingers. Don’t go out without proper protection to your feet. Invariably wear chappals when you walk. They should check-up their feet everyday if there are any wounds, scratches, inflammation, pus formation in their feet. Should show the feet, to the doctor whenever they go for consultation. Cut the nails of the fingers of the legs. See that it does not wound your fingers while cutting nails. Change your socks daily. See that they are clean. Use comfortable foot wear. It is better if they have cushion. Better use leather wear than synthetic ones. When you are using new shoes, practise with them for some time before you use. Care in the use of Antibiotics Patients who are using anti-diabetic tablets or taking Insulin should be very careful if they have to’\use antibiotics. They should not use Sulpha group antibiotics like Septran, Bactrim, Ciplin etc. Their use will bring down the level of blood sugar-the disease called Hypoglycemia. It is the same situation, if they consume alcohol after taking antidiabetic tablets. Sex problems: If people have been suffering from Diabetes for a very long time, the blood veins in the male organ may become narrow or weak, thereby obstructing blood flow. Consequently erection problems arise— delayed erection or unsteady erection. In women, many of the problems are not there. In some, vaginal lubrication may come down. Their sex organ may be subjected to Fungal infection and bacterial attack. Exercises and Yogasanas In order to keep the disease under control, a diabetic must, apart from forming certain good habits, exercise regularly. Exercise will help reduce the blood sugar by letting the muscles use the sugar in the blood. Heart diseases and other diseases of veins are kept in check if exercise is done regularly. Walking, jogging, running, swimming, skipping etc., will do good to a diabetic.

Exercises like “Pranayama” also does good to diabetics. Care during the exercise Don’t exercise in empty stomach. Increase the exercising time slowly and steadily. Don’t exercise any time you please. Do them at a fixed time and regularly. Brisk walking is the best exercise to the diabetics. Don’t go into weight-lifting exercises. Always do your exercise with the advice of your doctor. Dissolve half a teaspoon of salt in one cup of warm water. Use this as mouth wash to cure mouth ulcers/ boils. Apply a few drops of salt water in the ear which is paining due to the entry of ants in it. The ants die and pain disappears. Give fomentation with salt on the swollen joints to bring down pain and swelling. You can cure sore throat and toothache by gargling with salt water and washing the mouth with it. Salt water for this purpose can be prepared by mixing one tea-spoon of salt in one cup of water. Wash your hair with water in which a little salt has been dissolved to cure dandruff and stop hair loss. Make it a habit to massage your body with salt before bath, at regular intervals, to cure frequent attacks of common cold. This is good for improving blood circulation, immunity to diseases and low blood pressure. Chew a salt crystal with one clove to remedy the discharge of sputum with cough. Before dressing a wound, clean it with a piece of clean cotton dipped in salt solution which acts like weak carbolic acid. After washing, dry the wound with another piece of cotton, which has already been dipped in salt solution and dried. Mix Tulasi juice with salt and apply it on the part of the body affected by scorpion bite to bring down pain. Excess of salt intake causes accumulation of excess water in the body, increases body weight and affects health. Patients suffering from kidney problems, blood pressure and skin diseases should reduce salt consumption as much as possible. Pieces of cloth are allowed to absorb the juice and dried in shade. These pieces are burnt in castor oil lamp. Soot is collected by covering the flame with a vessel. When milk or curd is to be added to tomato masala of gravies, bhuno tomatoes well till dry and oil separates before adding the curd or milk. Bhunoing well lessens the sourness of the tomatoes and the added milk or curd does not curdle. If the tomatoes are not well bhunoed, there are chances of getting a curdled gravy. When adding curd to curries, always remember to beat the curd. Remove the masala from fire when adding the curd or milk to the masala. Stir well to mix. Return to low heat. If your curry turns out a bit oily and pungent, take two bread slices and crumble them coarsely. Add this to the curry and mix well. Bread absorbs the excess oil and spice. If your gravy becomes too salty, make a few small balls of atta (chappati dough) and put them in the gravy. Give 2-3 boils. Let them remain for some time in the curry. Before serving remove these balls which have absorbed the extra salt. To thicken gravies and also add flavour to the gravy, grind 1-2 tbsp of soaked cashew nuts or almonds or magaz (watermelon seeds) or chironji to a fine paste. Add to the gravy and relish the difference it makes. 1 tbsp of kasoori methi (dried fenugreek leaves) added to gravies enhances the taste but a little extra may turn the gravy bitter. Onions and Garlic Place onions in the fridge for ½-1 hour before chopping them. Your eyes will not water. Heating the knife before cutting up onions can prevent you from shedding tears. Soak garlic in water for sometime. It peels easily. The best way to keep garlic handy is to peel all the flakes and store in a jar of oil. This flavoured oil is great for salads and seasoning. When using pressure cooker place the days requirement of garlic pods on its lid for 10 minutes. They will be easier to peel. Add salt to onion when frying

(browning), they will brown fast and turn soft sooner. For garnishing pulaos, etc., fry onions with a pinch of sugar. They will brown faster. Rice and Dal To prevent water from overflowing while cooking rice coat the rim of the vessel with butter or ghee. Add 1-2 slices of lemon or a little lemon juice to rice while cooking. It keeps it white and grainy. Add 1 tsp of oil to rice while boiling, it will not boil over. Add a few drops of oil while cooking dals to reduce the cooking time and frothing. To 1 cup of dal, 3-4 cups of water are generally added depending on the type of dal. Chappati and Poori The dough should always be kept away covered for at least ½ hour after kneading. If chappatis are made immediately, they are not soft. While making chappatis, use hot water to mix the flour. The chappatis will turn out very soft and tasty. Add a little milk to the dough for fluffier puris. Before washing the milk utensil, knead the dough in it. You get a softer dough and a cleaner utensil. For a protein rich diet add ½ kg of soya bean flour to 3 kg wheat flour for making paranthas. Do not throw away the water after making paneer. Use it for making dough for chappatis, which will give softer, tastier and more nutritive chappatis. Spread newspaper under the rolling board (chakla) while making chappatis. All the dry flour will fall on the paper and it becomes easy to empty it into the dustbin. Snacks Add a handful of puffed/crushed rice (chirwa or poha) to the pakora batter for extra crisp pakoras. ªª Add a handful of puffed/crushed rice (chirwa or poha) to the rice while soaking for idlis. You will get fluffier idlis. ªª If dosas stick to the tawa, cut an onion into half. Clean tawa with the cut side of the onion and a little oil. make dosas. Dosas will not stick. To add more flavour to dosas, add a few methi daana (fenugreek seeds) to the batter. And taste the difference! To make neat and crisp dosas, everytime you apply oil on the tawa, sprinkle salt water over it. Salt should be added to the dosa batter before it is kept for fermentation. Often the first dosa sticks to the tawa and refuses to come out. This can be solved by greasing the tawa with oil and a pinch of salt. Add a little dahi (yogurt) to dahi vada batter to get softer vadas. They will also absorb less oil while frying. For getting a thick, crisp coating on cutlets or rolls, dip the prepared snack in eggwhite beaten with a few tbsp of water and then roll in fine bread crumbs. If you do not take eggs, dip them in a thin batter of maida and water and then roll in bread crumbs. Fry till well browned. In the absence of bread crumbs, suji may be used to get a crisp coating. If your cutlets fall apart, dip some bread slices in water for a second, squeeze it and add to the cutlet mixture. You may also add a raw egg to the cutlet mixture for binding instead of the bread, if you take eggs. Wrap sandwiches without cutting the sides, in foil or cling wrap to keep them soft. Cut the crust only at the time of serving to prevent the edges from drying. To make crisp potato chips, soak them in cold water for 1 hour. Drain. Wipe dry and deep fry. To get extra crisp chips, sprinkle the soaked, dried chips with some maida (plain flour) before frying. Maida absorbs any excess water present. For deep frying any snack, add small quantities to the oil at one time. This maintains the oil’s temperature. If too many pieces are added together, the oil turns cold and a lot of oil is then absorbed by the snack. After deep frying, let the oil cool down. Add a little quantity of fresh oil to used oil before reusing. This prevents oil from discolouring. Important Tips for Baking Perfect Cakes Measure the ingredients with great care. Using correct amounts of ingredients is more important than the beating of the cakes. Invest in a good electric hand mixer. It simplifies

your work besides giving good results. Always beat in one direction. To beat the egg whites light as air, they should be at room temperature while beating. Always use a clean, dry bowl and beater for beating. While using raisins or dry fruits or peels in a cake, first coat them with a little flour (maida) and then add to the batter. They will then not sink to the bottom of the cake. Remember to always sift the baking powder with the maida at the time of making the cake. Never use baking powder which is more than a year old. Check the manufacturing date on the container. Always preheat the oven at the temperature at which you have to bake your cake for at least 10 minutes. Always grease your baking tin and dust (sprinkle) with flour (maida). Baking should always be done in aluminium containers. Do not open the oven door again and again to check the cake as this causes variation in the temperature and hence affects the baking. Test your cake with a clean knife at the place where the cake has risen the most, i.e. at the highest point, before removing from the oven. Permit the cake to cool for sometime before you remove it from the tin. Never cool a cake under the fan as this will make the cake hard. What Went Wrong With Your Cake A Heavy Cake: Too little baking powder; too much flour; mixture not creamed enough; flour mixed too vigorously; oven too slow. A Dry Cake: Too much baking powder or flour; not enough fat or liquid; too long in the oven. A Sunken Cake: Too much liquid, baking powder or sugar; too little flour; oven door slammed or cake moved during baking; taken out from oven too soon. A Peaked Cake: Insufficient fat or baking powder; too much flour; oven temperature too high. A Badly Cracked Top: Oven too hot; cake tin too small; too much flour; not enough liquid. Never over-cook food in a microwave as it becomes tough and leathery. Give the dish a little standing time before you test it, to avoid over cooking. ªª Never pile food on top of each other. It cooks better, evenly and quickly when spaced apart. ªª Food cooks better in a round container than in a square one. In square or rectangular bowls, food gets overcooked at the corners. ªª Do not add salt at the time of starting the cooking as it leads to increase in the cooking time. ªª Do not add more water than required, however a little water must be added to prevent dehydration of the vegetable, which results in the loss of natural juices. Addition of extra water increase the cooking time. ªª Do not deep fry in a microwave (the temperature of oil cannot be controlled). ªª Do not cook eggs in their shells (pressure will cause them to explode). ªª Do not cook and reheat puddings having alcohol (they can easily catch fire). ªª Do not use containers with restricted openings, such as bottles. ªª Use deep dishes to prepare gravies, filling the dish only 3/4 to avoid spillage. ªª Do not defrost canned foodstuff. ªª Do not use aluminium foil for covering dishes. Do not reheat foods (sweets like ladoos, burfi etc.) with silver sheet, as it leads to sparking. ªª Fill the cake tin 3/4 with cake batter, allowing space for cake to rise. ¾¾ THANDAI This frothy milk shake is delicately perfumed and exotically flavoured. Serves 15-20 ½ cup dry rose petals 1 cup melon seeds (magaz) 6-8 green cardamoms (eliachi) 2 tbsp peppercorns (kali mirch) ½ cup poppy seeds (khus khus) ½ cup almonds (badam) 2 tbsp fennel seeds (saunf) 1 cup milk ªª Put all the ingredients except milk in a grinder and grind to a powder. ªª To make 1 serving, put 1 cup milk in a mixer. tbsp sugar, 2 tbsp of the prepared thandai powder and 3-4 ice cubes. Give 4-5 churns till frothy. Pour in glasses and serve. ¾¾ BHANG KE PAKORE Onion and potato pakoras have a happy ingredient in the batter - bhang - which is used during the festival of Holi. Serves 8-10 2 cups gram flour (besan) 2 tbsp semolina (sooji) 1 tsp garlic paste 10 gm bhang seed powder 2 potatoes 2 onions 15-20 spinach leaves - dipped whole in cold water ¼ tsp baking soda (mitha soda) 1 tsp carom seeds (ajwain) 1 tsp pomegranate seed powder (anardana) salt to taste, ½ tsp red chilli powder oil for frying ªª Peel and slice potatoes and onions. Sprinkle ½ tsp salt and ½ tsp red chilli powder and keep aside for 15-20 minutes. Pat dry spinach leaves. ªª Sieve gram flour, semolina, baking soda and salt together. Add garlic, bhang seed powder, carom seeds red chilli powder, salt and pomegranate seed powder. Add water and make a thick batter. Keep batter aside for 10 minutes. Dip vegetables in it. Due to advancement of the present civilization, urbanization, physical inactivity, over nutrition, chronic under nutrition, obesity, stress, drug abuses etc. the Diabetes is becoming commoner day by day. It has a world -wide distribution. So far about 98.9 million sufferers of Diabetes are in the world of which Asian countries contribute 46.9 million, almost 50% Diabetes of the world. About 20 million are in India. ¾¾ HOW IS DIABETES CAUSED? The Ayurvedic concept on the manifestation of Diabetes is essentially associated with “Mithyaharavihara” (improper dietary habits and physical exertion). This principle has to be variously interpreted in the causation of different diseases. The dietary and physical factors which induce ‘Diabetes’ as described by the ancient physicians of India are: ªª The food stuff which has properties of Brmhana (containing more carbohydrate and fat than proteins). ªª Sheetha veerya (cold in nature and action). ªª Amla rasas (consisting more acidic properties). ªª Snigdha guna (which increases Kapha dosha, Medo dhatu and Mutra mala).

Habit of over eating and smoking. With regard to the physical factors, the ancient physicians of India give emphasis on the following factors which predispose Diabetes. ªª Habit of consuming food stuff with Madhura rasa (sweet taste). ªª Habit of indulgence in excessive sleep, especially in day time. ªª Excessive indulgence in sexual and other sedentary habits, these can vitiate the Kapha dosha which leads to Diabetes. ªª Climate and geographical nature. They may not have symptoms for many years. Symptoms may develop rapidly (weeks or months) or sometimes much more slowly and may be

If you happen to have any one or multiple symptoms of Diabetes, then it’s the right time to check with a physician. The treatment for Diabetes starts only after consulting with a physician for proper diagnosis. The physician first will understand your presenting complaints and may advice a couple of tests to confirm Diabetes. If the complaints or the test analysis reports or both confirm Diabetes, then treatment for Diabetes has to be considered. ¾¾ WHAT HAPPENS IF DIABETES IS NOT TREATED? Untreated Diabetes increases the risk of long-term complications. Neuropathy (impact on the nervous system — most commonly causing numbness, tingling and pain in the feet, and also increasing the risk of skin damage due to altered sensation. Neuropathy contributes to the risk of Diabetes - related foot problems, such as foot ulcers, that can be difficult to treat and occasionally require amputation. As well, proximal Neuropathy causes painful muscle wasting and weakness. WHY IS ‘DIABETES’ A SILENT KILLER? Heart disease noted in 68% of Diabetes related deaths among people aged 65 years or older. Adults with Diabetes have heart disease death rates about 2 to 4 times higher than adults without Diabetes and the risk for stroke is 2 to 4 times higher among people with Diabetes. Diabetes is the leading cause of new cases of blindness among adults aged 20–74 years. In 2005–2008, 4.2 million (28.5%) people with Diabetes aged 40 years or older had Retinopathy. Diabetes is the leading cause of kidney failure, accounting for 44% of all new cases of kidney failure. A total of 202,290 people with end- stage kidney disease due to Diabetes are living on chronic dialysis or with a kidney transplant. About 60% - 70% of people with Diabetes have mild to severe forms of Neuropathy. Hearing loss is about twice as common in adults

with Diabetes as those who do not have Diabetes. HIMALAYA LAUNCHES LIVER HEALTH INITIATIVE -‘LIVLINE’ Bangaluru based The Himalaya Drug Company has launched ‘Livline’, a national public health campaign for creating awareness on liver care and Hepatitis. Liver disease is among the top ten killer diseases in India, with lack of awareness being a strong reason for its high incidence. Diabetes has been one of the major killer diseases of the present day life. The disease is not only genetic but is linked to lifestyle and stress, both physical and mental. India continues to be the “diabetes capital” of the world, and by 2030, nearly 9 per cent of the country’s population is likely to be affected from the disease, warns the fourth edition of the World Diabetes Atlas launched by the IDF at the 20th World Diabetes Congress in Montreal, Canada. In the age of consumerism, fast food, processed food, every day, we are spoiled for variety and choices. Along with today’s fast moving consumption habits, it is no surprise to see growing rate of health problems such as diabetes and obesities. With increasing health concerns and rising unpopularity of the current range of synthetic alternative sweeteners, Stevia represents the next generation of alternative sweetener. Stevia rebaudiana Bertoni is a small perennial shrub that belongs to the family of aster or chrysanthemum family. It grows primarily in the Amambay mountain range of Paraguay. However nearly 400 species of stevia have been found around the world. Stevia rebaudiana is the only species at present, which possesses the ability to sweeten. The leaves of the stevia shrub contain specific substances (Glycosides), which produce a sweet taste but have no caloric value, apart from protein, fibres, carbohydrates, phosphorus, iron, calcium, potassium, sodium, magnesium, zinc, vitamin C, flavonoid and vitamin A. Stevia, which has earned the in-principle nod of the Food Safety and Standards Authority of India (FSSAI) – the country’s apex food regulator – could be the best bet for the nation’s agro-economy in future, owing to its medicinal and sweetening properties, according to Bala Prassad, chief executive officer, National Medicinal Plant Board (NMPB), and joint secretary, Union Ministry of Health and Family Welfare. Deliberating on the sweetener at the Fourth Stevia Global Summit India, which took place at New Delhi’s PHD Chamber of Commerce and Industry, Prassad said NMPB has decided to boost stevia cultivation in India. He added, “Under the current provisions, 20 per cent of the cost of production, which is set at Rs 3,12,500 per hectare, is subsidised by the board. But the minimum requirement would be five farmers and two hectare of land.” But there continued to be questions about the species of stevia that must be incorporated, because the plant has approximately 400 varieties. Dr Silvamurthy, former food scientist, Defence Research and Development Organisation (DRDO), said, “Stevia has several medicinal properties, which could be helpful in preventing several lifestyle diseases like obesity. Moreover, farmers could avail carbon credits by growing stevia.” Diabetes is a medical condition which should be monitored and treated by a qualified physician or health care practitioner. However, Stevia can be a part of a healthy diet for anyone with blood sugar problems since it is not believed to raise blood sugar levels. If in doubt, ask your doctor. However, if

Hepatitis Survey reveals one out of 50 Bengalureans have Hepatitis B ªª

BGS Global Hospitals in association with Integrated Liver Care Foundation launched the ‘Bengaluru Hepatitis Project’ ªª It is the first such study of its kind in India ªª Survey reveals the magnitude of Hepatitis B and Hepatitis C infections ªª This survey is of paramount importance as the disease is silent, showing no symptoms ªª Easily treatable if detected. Hepatitis B is preventable by vaccination BGS Global Hospitals in association with Integrated Liver Care Foundation launched the ‘Bengaluru Hepatitis Project’ an on-going survey to collect data on Hepatitis B and Hepatitis C infections among the urban population of Bengaluru. Doctors of the liver team at BGS Global Hospitals have founded the ILC Foundation; a not-for-profit organization under the aegis of which, this study the first of its kind in the country, has been initiated and conducted. This is in keeping with the theme of World Hepatitis Day this year, “This is Hepatitis. Know it. Confront It.” The survey reveals that one in 50 in the city have Hepatitis B and one in 200 have Hepatitis C. These findings were the outcome of Free Hepatitis screening camps organized in various areas in the city. The results are indicative of the magnitude of the disease burden and sound an alert to Bengalureans to take preventive measures for Hepatitis B and Hepatitis C infections. Awareness for these diseases is extremely low. To add to it, it is silent - majority of those infected are unaware of the infection since they do not have any symptoms. However they are easily treatable if detected early and can be completely prevented by getting vaccinated. Vaccinations are now available for Hepatitis B. If left untreated, both hepatitis B and C viruses can lead to liver cirrhosis which can cause complications such as bleeding, accumulation of fluid in the abdominal cavity, coma, liver cancer, liver failure and death. Hepatitis B and C are both ‘silent’ diseases and most infected individuals have no symptoms. If left untreated, both hepatitis B and C viruses can lead to liver cirrhosis. Liver cirrhosis, leads to complications such as bleeding, ascites (accumulation of fluid in the abdominal cavity), coma, liver cancer, liver failure and death. In the case of chronic hepatitis B, liver cancer may occur even before a person develops cirrhosis How is it transmitted? Adults get Hepatitis B or C through infected blood transfusion, contaminated needles and sexual transmission from a hepatitis B infected partner. Children usually get Hepatitis B from their mother during childbirth if the mother is infected. Demystifying the Myths: Hepatitis B and C is not spread by air, food, water or by casual personal contact. How is it diagnosed? Hepatitis B and C can be easily diagnosed by a simple blood test. Treatment There are effective medications to treat Hepatitis B and C. Prevention Vaccination is the best way to prevent Hepatitis B. However, there is no vaccine for Hepatitis C Hepatitis B can be prevented by avoiding direct contact with other people’s blood, practicing safe sex, not sharing combs, toothbrushes or other personal items and avoiding re-use of disposable needles.
