The sexual abuse of children is a horrible reality, and many are unaware of its extent. This episode is to breaks the silence around the issue. Child sexual abuse (CSA) can be physically violent or non-violent in nature. It can be very difficult to talk about sexual abuse and even more difficult to acknowledge that sexual abuse of children of all ages including infants happens every day. The rape of a 6-year-old girl at a school in Bangalore has refocused attention on sexual

abuse of children in India. Sexual abuse robs children of their childhood and creates a loss of trust, feelings of guilt and self-abusive behaviour. It can lead to antisocial behaviour, depression, identity confusion, loss of self-esteem and other serious emotional problems. It can also lead to difficulty with intimate relationships later in life. The sexual victimization of children is ethically and morally wrong. Under the law, child sexual abuse is an umbrella term describing criminal and civil offenses in which an adult engages in sexual activity with a minor or exploits a minor for the purpose of sexual gratification. How to overcome zz Primary school children need ageappropriate information about bodies zz Children need to be provided with important skills and knowledge to help protect their personal safety and wellbeing. zz Teach your child about secret touch – this is most effective and easily understood by children using language such as: ‘It’s not OK for an adult or older person to touch a private part of your body for no reason and ask you to keep it a secret.’ zz Children need to know that they can speak openly to a trusted adult and that they will be believed if they are troubled An appreciable move by Karnataka government by passing Goonda Act(covers sexual offenders, acid attackers) Sexual offenders will henceforth be termed “sexual predators”. The new nomenclature has been given an by amendment to the Karnataka Prevention of Dangerous Activities of Bootleggers, Drug Offenders, Gamblers, Goondas, Immoral Traffic Offenders, Slum-Grabbers and Video or Audio Pirates (Amendment) Bill, 2014, passed by the legislature. Child sexual abuse is the involvement of a child in sexual activity that he or she does not fully comprehend, is unable to give informed consent to, or that violates the laws or social taboos of society.
Child abuse or maltreatment constitutes all forms of physical and/ or emotional ill-treatment, sexual abuse, neglect or negligent treatment or commercial or other exploitation, resulting in actual or potential harm to the child’s health, survival, development or dignity in the context of a relationship of responsibility, trust or power meted out to a person below the age of eighteen Child sexual abuse is a form of child abuse in which an adult or older adolescent uses a child for sexual stimulation. Indecent exposure (of the genitals, female nipples, etc.) to a child with intent to gratify their own sexual desires, physical sexual contact with a child, or using a child to produce child pornography is of the few forms. Severe forms of sexual abuse include: zz Assault, including rape and sodomy zz Touching or Fondling a child zz Exhibitionism- Forcing a child to exhibit his/her private body parts zz Photographing a child in nude Other forms of sexual abuse include: zz Forcible kissing zz Sexual advances towards a child during travel zz Sexual advances towards a child during marriage situations zz Exposing a child to pornographic materials Who commits the crime? Child Sexual abuse can be committed by person responsible for the care of a child or related to the child. It can be: Family members, relatives, friends, doctors and other medical staffs Teachers etc. The act is done to gratify or satisfy the needs of the other person. These may include but is not limited to: zz Coercion of a child to engage in any unlawful activity. zz Exploitative use of a child in prostitution or other unlawful sexual practices. zz Exploitative use of children in pornography. Effects zz Psychological effects: Child sexual abuse can result in both short-term and long-term harm, including psychopathology in later life. Indicators and effects include depression, anxiety, eating disorders, poor self-esteem, somatization, sleep disturbances, and dissociative and anxiety disorders including post-traumatic stress disorder. While children may exhibit regressive behaviours such as a return to thumb-sucking or bed-wetting, the strongest indicator of sexual abuse is sexual acting out and inappropriate sexual knowledge

and interest. Victims may withdraw from school and social activities and exhibit various learning and behavioural problems including cruelty to animals, attention deficit/hyperactivity disorder (ADHD), conduct disorder, and oppositional defiant disorder (ODD). Teenage pregnancy and risky sexual behaviours may appear in adolescence. Child sexual abuse victims report almost four times as many incidences of self-inflicted harm Physical effects: zz Injury: Depending on the age and size of the child, and the degree of force used, child sexual abuse may cause internal lacerations and bleeding. zz Infections: Child sexual abuse may cause infections and sexually transmitted diseases. zz Neurological damage: Traumatic stress, including stress caused by sexual abuse, causes notable changes in brain functioning and development. How to talk to your kids about abuse Talking about abuse can be a difficult subject to breach with children. However, like many important discussions, it’s not something to wait to talk about until it comes up on its own. In fact, by then it may be too late. Getting Started… Children as young as four years old can understand the basic concepts of good touches, bad touches and confusing touches. So, please mark on your calendar to sit down with each of your children to talk about these three things: Explain the three different types of touch zz Good touches are those touches that make us feel happy, safe and loved. Most of the touch we get is good touch. zz Bad touches are those touches that hurt us; they feel like an “ouch.” Some examples are kicking, hitting and biting. zz Sexual abuse touch is defined as “forced or tricked touch of private body parts.” The key words are forced and tricked. zz a force is when someone makes you do something you don’t want to do or don’t understand. zz a trick is when someone lies to you, fools you, pretends or calls something a game, that isn’t a game, so they can touch your private body parts or have you touch theirs. zz Tell your children that sexual abuse is confusing because it doesn’t necessarily hurt; the touch can feel good, but it is not okay.

Teach your child to say NO: Children need to know that they can say NO to any touch that they do not want—even if the person touching them is an adult or someone they know. Teach your child that they have the right to trust their own feelings and to ask questions when they feel uncomfortable or confused by someone’s behavior. Tell them it’s okay to speak up: Teach your child that when abuse happens, it’s never the child’s fault and that it is okay to break promises they make about abuse. Keep the conversation going: As you talk with your children about abuse, keep in mind that you may need to revisit the discussion as they get older. Teach your child The Underwear Rule: About one in five children falls victim to sexual violence, including sexual abuse. You can help prevent this happening to your child. A child should not be touched by others on parts of the body usually covered by their underwear. And they should not touch others in those areas. The Underwear Rule has 5 important aspects. zz Your body is your own: Children should be taught that their body belongs to them and no one can touch it without their permission. Children have the right to refuse a kiss or a touch, even from a person they love. Children should be taught to say “No”, immediately and firmly, to inappropriate physical contact, to get away from unsafe situations and to tell a trusted adult. zz Good touch – bad touch: Children do not always recognise appropriate and inappropriate touching. Tell children it is not okay if someone looks at or touches their private parts or asks them to look at or touch someone else’s private parts. The Underwear Rule helps them to recognise an obvious, easy-toremember border: the underwear. zz Good secrets – bad secrets: Secrecy is a main tactic of sexual abusers. That’s why it’s important to teach the difference between good and bad secrets and to create a climate of confidence. Every secret that makes them anxious, uncomfortable, fearful or sad is not good and should not be kept; it should be told to a trustworthy adult (parent, teacher, police officer, doctor). Other helpful hints to accompany The Underwear Rule zz Prevention and protection are the responsibility of an adult: When children are abused they feel shame, guilt and fear. Adults should avoid creating taboos around sexuality, and make sure children know whom to turn to if they are worried, anxious or sad. zz Reporting and disclosure : Children need to be instructed about adults who can be part of their safety network. They should be encouraged to select adults whom they can trust, are available and ready to listen and help. zz Known perpetrators: In most cases the perpetrator is someone known to the child. Informing parents regularly about someone who gives gifts, asks to keep secrets or tries to spend time alone with a child must be a set rule in the house.

Unknown perpetrators : In some cases the perpetrator is a stranger. Teach your child simple rules about contact with strangers: never get into a car with a stranger, never accept gifts or invitations from a stranger. Help : Children should know that there are professionals that can be particularly helpful (teachers, social workers, ombudspersons, physicians, the school psychologist, the police) and that there are help lines that children can call to seek advice.
What to do if you suspect abuse When you suspect your child has been abused, it is very important not to be angry with your child. zz Do not make your child feel as if they have done something wrong. zz Do not interrogate the child. zz You could ask what may have happened, when and with whom, but do not ask why it happened. zz Try not to be upset in front of your child. Children can easily feel guilty and may hold back information. zz Try not to jump to conclusions based on little or unclear information. zz Reassure your child that you will do something about it, and contact someone who could help, like a psychologist, child care specialist, doctor, social worker or the police. zz special help lines and centres responsible for helping child victims of sexual violence have been set up. Treatment The initial approach to treating a person who has been a victim of sexual abuse is dependant upon several important factors: zz Age at the time of presentation zz Circumstances of presentation for treatment zz Co-morbid conditions The goal of treatment is not only to treat current mental health issues, but to prevent future ones. Children often present for treatment in one of several circumstances, including criminal investigations, custody battles, problematic behaviours, and referrals from child welfare agencies. The three major modalities for therapy with children and teenagers are family therapy, group therapy, and Individual therapy. Which course is used depends on a variety of factors that must be assessed on a case by case basis. For instance, treatment of young children generally requires strong parental involvement, and can benefit from family therapy. Adolescents tend to be more independent, and can benefit from individual or group therapy. The modality also shifts during the course of treatment, for example group therapy is rarely used in the initial stages, as the subject matter is very personal and/or embarrassing. Major factors that affect both the pathology and response to treatment include the type and severity of the sexual act, its frequency, the age at which it occurred, and the child’s family of origin. zz

World Health Organization preserved one day in a year to discuss and to disseminate the knowledge, to the general population. “WORLD AIDS DAY is observed on 1st DECEMBER” every year since 1988. All scientific information is given to the common man on the road. ART is no cure for HIV / AIDS. zz ART reduces HIV replication. Body viral load is reduced. Opportunistic infections are reduced. zz Cost of management is reduced. Patients can lead a productive life. zz ART has changed HIV/AIDS from a virtual death sentence to a chronic manageable disease. Terminologies used in ART – zz ARV – Anti Retroviral agents zz ART – Anti Retroviral therapy zz HEART – Highly effective Anti Retroviral therapy zz HAART – Highly Active Anti Retroviral therapy zz MEGA HEART – More than 6 drugs are used. zz Salvage Therapy – Where miscellaneous drugs like Hydroxyurea are used zz Recycling Therapy - Where drugs previously used are restarted zz Induction & Maintenance - Start at a lower dose, then, maintain at higher doses zz Subtractive Therapy – Start 4 drugs and reduce to 3 drugs Life Cycle of HIV – 1) HIV virus attaches and penetrates CD4+T Lymphocytes. Viral gp120 attaches to CD4 receptor and also Chemokine receptors CXCR4 and CCR5. Viral gp 41 drills a hole in the cell wall, viral nucleus seeps into the cytoplasm of a CD4 cell. Empty viral cell wall seals the hole of CD4 cell. 2) Once HIV nucleus enters the cytoplasm, reverse transcriptase enzyme gets activated and converts HIV RNA to linear HIV DNA. Linear HIV DNA gets converted into circular DNA. Circular DNA enters the CD4 cell nucleus. 3) HIV Integrase gets activated in nucleoplasm and helps viral DNA to get integrated with CD4 cell chromosome. After few years (4-7 years in India, and 10-12 years in America). HIV gets activated and sends genomic RNA and messenger RNA from nucleus to cytoplasm. The mRNA brings necessary glycoprotein’s from smooth and rough endoplasm and puts it over genomic RNA. Viral protease enzyme helps to form full virus either by adding or removing glycoprotein’s from genomic RNA.

4) This virus buds out from CD4 cell and pinches a little cell wall and takes it as envelope and matures. It is ready to infect new CD4 cell. This life cycle takes around 2
days. Every time a daughter HIV buds out, that CD4 cell dies. Normally CD4 cell range from 600-1200 cells/ mm3. If their number declines to <250, patients develops AIDS. Viral set point – Within 6 months, the host’s immune response is able to control the infection to a point where the number of virus particles produced per day equals the number of particles destroyed per day. This steady state is called viral set point. The higher a patient’s viral set point, the greater the risk for disease progression. 4. Integrase Inhibitors. 5. Protease Inhibitors. 6. Fusion Inhibitors. 7. Maturation Inhibitors. Nucleoside Reverse Transcriptase Inhibitors – Drug Analogue of Dose Zidovudine Thymidine 300 mg bid. (Azidothymidine) Stavudine Thymidine 20-30 mg bid. Zalcitabine Cytidine 0.75 mg tid. Lamivudine Cytidine 150 mg bid. Emtricitabine Cytidine 200 mg od. Didanosine Adenine 150-200 mg bid. Abacavir Guanosine 300 mg bid. Nucleotide Reverse Transcriptase Inhibitors – Tenofovir Adenosine 300 mg. disoproxil fumarate Mechanism of action – NRTIs enter the CD4 cells and are converted to their corresponding triphosphate derivatives, which have a high affinity for HIV reverse transcriptase enzyme, which is required for DNA synthesis. The NRTIs are nucleoside analogues. They competitively inhibit reverse transcriptase, are incorporated into viral DNA chain and terminate DNA chain elongation. Thymidine kinase converts zidovudine to zidovudine monophosphate, and the same enzyme converts zidovudine monophosphate to zidovudine diphosphate and zidovudine triphosphate. Zidovudine triphosphate is a false nucleoside and gets incorporated into viral DNA and will not form a complete DNA, and DNA is broken into pieces. Hence NRTIs are also called chain terminators. Tenofovir is a nucleotide analogue and competitively inhibits HIV reverse transcriptase similar to nucleoside analogues. Non nucleoside Reverse Transcriptase inhibitors – Nevirapine 200 mg od X 14 days. Induction dose. 200 mg bid, maintenance dose. Delavirdine 400 mg tid. Efavirenz 600 mg at bedtime. Etravirine 200 mg bid. Rilpivirine 25 mg od. Mechanism of action – The NNRTIs bind to reverse transcriptase enzyme and inactivate the enzyme. Hence viral RNA will not get converted into viral DNA. NNRTIs are effective only on HIV-1. Protease Inhibitors – Saquinavir / Ritonavir. Saq/r 1000/100mg bd. 1600/200mg bd. Indinavir / Ritonavir. Idv/r 800/100mg bd. Lopinavir / Ritonavir lpv/r 400/100mg bd. 553/133mg bid when combined with EFV or NVP Nelfinavir nfv 1250mg bd. Atazanavir atv 400mg bd. Atazanavir / Ritonavir. 300/100mg bd. Amprenavir 1200mg bd. Mechanism of action – The protease inhibitors bind competitively to HIV protease and block viral maturation. This makes the daughter viral particles immature and noninfectious. HIV protease activity is essential for the activation of viral enzymes and HIV replication. It is needed for the production of mature virion and for viral infectivity.

Entry Inhibitors – a) Fusion Inhibitor – Enfuvirtide is a recent introduction for use in AIDS. It binds to a glycoprotein of the virus and inhibits the binding of the virus to the host cell membrane, and thereby blocks the entry of the virus into the cell, hence called fusion inhibitor, thus prevents transmission of HIV. Enfuvirtide 90 mg is given subcutaneously twice daily. Enfuvirtide is expensive and requires parenteral administration. Used cautiously in patients who are not responding to other ART drugs. b) CCR5 Receptor Antagonists – CCR5 is a coreceptor involved in fusion and entry of the virus into the CD4 cells. Maraviroc and Vicriviroc selectively bind to CCR5 receptors and block the entry of HIV into the cells. Maraviroc 300 mg bid and Viriviroc 30 mg OD. Integrase Inhibitors – Integrase is a viral enzyme necessary for viral replication in both HIV-1 and HIV2. Raltegravir and Elvitegravir bind to integrase and prevent integration of HIV-DNA into the chromosome of the host cell. Treatments of HIV infection – Currently several drugs are available for the treatment of HIV infection and with appropriate medication it is possible to control the disease and prolong the life expectancy. Combination of drugs like HAART regimen is used to improve efficacy and delay the development of resistance. WHO recommends fixed dose combination to improve compliance. Zidovudine or tenofovir combined with lamivudine or emtricitabine form the first line drugs. If first line therapy fails, second line drugs are given. WHO 2010 Guidelines – When to start: All adolescents and adults including pregnant women with HIV infection and CD4 counts of < 350 cells/mm3, should start ART, regardless of the presence or absence of clinical symptoms. Those with advanced clinical disease should start ART irrespective of their CD4 cell count. What to use in first line therapy: First line therapy should consist of an NNRTI + two NRTIs, one of which should be zidovudine or tenofovir. Three drugs are given. 1. Zidovudine or Tenofovir + 2. Lamivudine. + 3. Nevirapine or Efavirenz. What to use in second line therapy: Second line ART should consist of ritonavir boosted protease inhibitor plus two NRTIs, one of which should be zidovudine or tenofovir. Tenofovir or zidovudine. plus Didanosine. plus Lopinavir/r or Atazanavir/r. Occupational Post Exposure Prophylaxis – In health care workers exposed accidentally, ART should be initiated as soon as possible within first 4 hours. Not later than 72 hours. Basic Regimen – AZT 300 mg + Lamivudine 150 mg twice daily for four weeks. Expanded Regimen – AZT 300 mg + Lamivudine 150 mg + Lopinavir or Ritonavir 200 mg twice daily for four weeks. www.bangalorehealthtv.com

What is a prostate? Where is it? Only males have a prostate. It is a gland in the male reproductive system, located just below the urinary bladder. As shown in the diagram, it surrounds a part of the urethra, (the tube that carries urine from the bladder, out through the penis]. What does the prostate do? The prostate contributes to the milky fluid that carries the sperm produced by the testis. The prostatic fluid is also thought to supply nourishment for the sperm. Since the prostate contributes a portion of the semen volume throughout the better part of a man’s life, many men are able to father children well into their 70’s and 8o’s. What is Benign Prostatic Hyperplasia (BPH)? BPH is the non-cancerous enlargement of the prostate gland in males. Throughout childhood the prostate is very small. It begins to enlarge during puberty and approximately doubles in size; then the growth slows. At about age 45, the prostate often starts growing again and continues to enlarge for the rest of man’s life. This is considered to be due to hormonal changes. The degree of this enlargement varies in each person.
The enlargement itself is not the problem. As it continues to enlarge, it can frequently start to squeeze the urethra (like pinching a straw]. This can interfere with the normal flow of urine and cause uncomfortable symptoms.
An enlarged prostate generally does not interfere with sexual functioning. But severe urinary symptoms however can be embarrassing and inhibit a man’s sexual activity. Do all males suffer from symptoms of an enlarged prostate? Not so. 4 out of every 10 men over age 55, 7 out of 10 men in their 70’s, and by age 80, an even larger number, suffer from symptoms of an enlarged prostate. What are the urinary symptoms of an enlarged; prostate? One or more of the following: zz Frequent urination, especially at night zz Sudden, uncontrollable urge to urinate zz A poor urine stream zz A sense of incomplete bladder emptying zz Difficulty in starting urination zz Incontinence of urine zz Total blockage of urine zz Bleeding in the urine zz Urine infection How is BPH diagnosed? zz Your doctor will take a detailed medical history, including past and present medical problems He will examine the prostate gland by inserting a

Just because the prostate is enlarged, it does not need to be treated. If your symptoms are mild you may not need any treatment at all. If your symptoms are severe, or if the result of the tests shows that the urinary tract is significantly affected, you must undergo early therapy. What are the management options? Depending upon the severity of your symptoms, several management options are available. Your Urologist will discuss these options with you before deciding on anyone method. MEDICAL MANAGEMENT These are suitable only for mild symptoms and when the degree of obstruction is not severe. These medications have to be taken on a long-term basis, and have a proven benefit in well selected patients. Depending upon the drug used, you may experience minor side effects. SURGICAL MANAGEMENT zz Currently the best method of treatment. The gold standard among various surgical options is the transurethral resection of the prostate (TURP). This is done by passing an endoscope through the urinary passage (urethra) and the inner core of the gland is removed, widening the passage. zz About 25 to 95% of patients have failure to ejaculate after intercourse, as the semen is pushed backwards into the bladder instead of the outside. Very rarely, some of people have leaking of urine. Decreased erectile function is known to occur with aging. zz All tissue removed at the time of surgery will be sent for examination to confirm the benign nature of the gland. Very occasionally the presence of malignancy may be a surprise report. zz Treatment for BPH does not eliminate future incidence of prostate cancer, as this originates in an entirely different portion of the gland. Therefore annual evaluation may still be considered important for men who have undergone surgery for benign prostate gland enlargement.

A global health threat The outbreak of Ebola virus in West Africa is frightening for many reasons. The virus can live on surfaces that are soiled with blood or other body fluids, but sterilizing hospital equipment with bleach kills Ebola. As the virus spreads through the body, it damages the immune system and organs. Ultimately, it causes levels of blood-clotting cells to drop. This leads to severe, uncontrollable bleeding. POLLUTION AND SEXUALITY An environment is a complex of surrounding circumstances, conditions, or influences in which a thing is situated or is developed, or in which a person or organism lives, modifying and determining its life or character. The natural environment is such an environment that is relatively unaffected by human activity. Soil, Sky, Heat (Sun), Air and Water (Panchaboothas); they together made an environment that is fit for our existence. Environment motivates to react: We are miserable when are in desert and delighted when we are in lush garden. In a hospital we are in different mood and in disco different. Environment motivates us to change our mood and reaction is in accordance. Pollution Pollution is the release of environmental contaminants. The act or process of polluting or the state of being polluted, especially the contamination of soil, water, or the atmosphere by the discharge of harmful substances. Soil pollution Soil pollution is defined as the build-up in soils of persistent toxic compounds, chemicals, salts, radioactive materials, or disease causing agents, which have adverse effects on plant growth and animal health.
Water pollution Water pollution occurs when undesirable foreign substances are introduced into natural water. The substances may be chemical or biological in nature. Common pollutants include human or animal waste; disease-producing organisms; radioactive materials; toxic metals such as lead or mercury; agricultural chemicals such as pesticides, herbicides, or fertilizers; acid rain, Industrial discharge of chemical wastes and byproducts, Discharge of poorly-treated or untreated sewage, petroleum products, and high-temperature water discharged from power plants, often called “thermal pollution.” Pollutants in water are dangerous for human or animal consumption and harm crops.

In general noise pollution refers to any noise irritating to one’s ear which comes from an external source. Noise pollution can be caused by many sources including highways, vehicles, factories, concerts, airconditioners, engines, machine, aircraft, helicopters, alarms, public address systems, industrial development and construction work. Air pollution Air pollution is the presence of any chemical, physical (e.g. particulate matter), or biological agent that modifies the natural characteristics of the atmosphere. The atmosphere is a complex, dynamic natural gaseous system that is essential to support life on planet earth. Air pollution is composed of many environmental factors. They include carbon monoxide, nitrates,

Water pollution causes approximately 14,000 deaths per day, mostly due to contamination of drinking water by untreated sewage in developing countries. Pollution and Fertility Environmental factors contribute to the severity of infertility, and that this may worsen the effects of pre-existing genetic or medical risk factors. Stress and environmental pollution are two of the main causes of growing infertility among urban couples. Worldwide, one in every six couples has difficulty in conceiving. About 50 percent of the infertility problems are contributed by the male factor. But stress and environmental pollution are considered two key factors for childlessness among couples now. An increasing number of reports suggest that chemical and physical agents in the environment, introduced and spread by human activity may affect fertility in humans. Stress and environmental pollution reduce the sperm count in males. While a couple is considered infertile if after one year of adequate unprotected intercourse the woman fails to conceive, zz

Syringes and needles tips should be cut down before disposed them into garbage so that one cannot reuse those materials. zz Emission test to each vehicles should be done frequently. Environmental Education Environmental education is a learning process that increases people’s knowledge and awareness about the environment and associated challenges, develops the necessary skills and expertise to address the challenges, and fosters attitudes, motivations, and commitments to make informed decisions and take responsible action. Conclusion Since the industrial revolution many countries have experienced centuries of deforestation, and governments and Non-governmental organisations (NGO’s) should take up the programs of reafforestation (the restocking of existing forests and woodlands which have been depleted, with native tree stock) and also in supporting private and semi-private enterprise in programs of afforestation. It is the duty of every one of us to preserve our environment in the best interest of our own health. It is also our duty to hand over a better environment to a next generation.

A Complicated case of muliple fibroid uterus 70 Nos Fibroids removed from young girl has been successfully operated at Dr. RAMESH HOSPITAL A 26 years old unmarried girl, born to the couple with agricultural background. She being the younger daughter, presented at the outpatient department at Dr Ramesh Hospital on 17/6/14 with complaints of severe congestive dysmenorrhoea , regular cycles with heavy flow of 1 yr duration. She had previously undergone laparoscopic assisted myomectomy at Dr Ramesh hosiptal in 2009, uterus size of 16-18 weeks with multiple fibroids. Now on evaluation, uterus size was 28 weeks, with ultrasound showing multiple fibroids. MRI confirmed multiple fibroids and all fibroids mapped. Patient was planned for laparoscopic assisted myomectomy- and was posted on 25/6/14, after correction of anaemia. Uterus was enlarged with multiple sized fibroids with posterior uterine wall adhesions. Proceeded with myoma enucleation and adhesiolysis. Total of 70 in number enucleated , size ranging from 1cm-8cm weighing 1 kg were removed sucessfully. Since patient had significant blood loss, Intraoperatively 5 packed cells and 2 blood components given. Perfect hemostasis achieved. Patients vitals closely monitored throughout the surgery and postoperatively. Thus myomectomy was successfully accomplished , retaining the uterus of young girl and hysterectomy avoided. It is a very rare case and this is the first time in Karnataka that 70 fibroids are removed from a single patient retaining the uterus.

LASER ASSISTED HATCHING IN IVF Invitro Fertilisation (IVF) is a boon to infertile patients. IVF is a process by which an egg is fertilized by sperm outside the body. IVF is a major treatment for infertility. The process involves monitoring woman’s ovulatory process, removing egg from ovaries & getting them fertilized by husband’s sperm. The unfertilized oocyte is surrounded by a membrane called zona pellucida. The zona pellucida ensures that only one spermatozoa enters and fertilizes the egg. After fertilization, the embryobegins to cleave into a two-cell, then a four-cell and so on. The process of incubating the embryo from the zona pellucida (zona hatching — ZH) is one of the most important factors which determinate the implantation. Under

natural conditions, the expansion of the embryo together with the produced enzymes, causes the rupture of the zona pellucida. The final effect of this phenomenon is the release of the embryo (hatching). The cells of the embryo in contact with the endometrium of the uterus start the process of implantation. It is at early cleavage stage that assisted hatching can be performed on embryos. It has been shown that women older than 37 years of age have a tendency to produce oocytes with a harder and/or thicker zona pellucida than younger women. The same goes for women with a high level of follicle stimulating hormone (FSH). LASER ASSISTED HATCHING The problem of a harder zona pellucida is that the embryo may not hatch and thus not attach to the woman’s womb. Hatching of the embryo is necessary to achieve pregnancy. Zona

pellucida thickness influences sperm penetration, even when the spermatozoa are considered normal. From a clinical point of view, a thick zona pellucida (> or = 22 microns) could be an indicator for the use of microinjection procedures. Laboratory procedures involved in IVF may add to the hardening of the zona, as the embryo is not continually exposed to the enzymes present in the natural environment of the Fallopian tubes. There is also evidence to suggest that the process of freezing and thawing embryos causes the zona to harden and using assisted hatching can be beneficial. Several studies have shown that using a laser is superior to chemical and manual hatching. Laser assisted hatching has several advantages including; minimal handling of the embryo and delivering fast and exact control over the drilling of the hole. Laser assisted hatching is performed by the scientists, prior to the embryos being transferred back to the uterus. Today’s technology has helped us largely in manipulating with the gametes and embryos in IVF laboratory. Since sperms and embryos are microscopic, the Successful introduction of lasers in the IVF lab, allows the embryologist to deliver a controlled amount of energy at a specified spot with exquisite precision. The advantages of a laser are multiple - it is precise; atraumatic, and non-contact, which means there is no need to touch the egg or embryo when using the laser. Assisted hatching involves using the laser to create a precise opening in the shell (zona) of the embryo prior to transferring it. Such an opening helps the embryo to hatch more easily, thus increasing the chances of it implanting successfully in the uterus. biopsies for preimplantation genetic diagnosis (PGD). The laser allows to create a well-defined opening through the zona, through which it is possible to safely and atraumatically remove a single blastomere from the embryo. Other uses of Laser in IVF: Laser in ICSI Researchers have also used the laser for ICSI (intracytoplasmic sperm injection). The trickiest part of ICSI is the technical difficulty involved in capturing a single motile sperm and crushing its tail in order to immobilize it prior to injecting it into the egg. Using a laser beam, this can be performed with much greater ease and speed, thus dramatically reducing the time taken to perform ICSI. ICSI can be especially difficult to perform when there are very few eggs, or the eggs are fragile, because these are easily damaged when the injection pipette is introduced through the zona into the cytoplasm of the embryo. Laser assisted ICSI allows us to create a very small opening in the zona, through which the injection pipette carrying the sperm can be easily carried into the egg. This technique has been shown to reduce damage to fragile eggs during ICSI, and increase fertilization rates as well.

If the fresh drug is not available dry drug can be used. Bhuamlika is a wonder herb found growing as a weed of rainy season in India. It is commonly known as Hazardana or Bhooyi aamla in Hindi and Nelanelligida in Kannada. Week 15 and 16 A great experience of being a fetus, I am surrounded by the amniotic fluid, splashing about and moving freely like an astronaut. I am practicing facial expressions too. Mom is nourishing herself with right food to make my muscles and bones stronger. 18

A great experience of being a foetus: The most enjoyable period of my foetus hood has just begun when I am 15 wks and going into 16wk. My playtime experience has started. I am able to grasp the umbilical cord and put the thumb into my mouth and am able to kick at the amniotic sac and I love those splashes “WOW! At present there is almost a cup of amniotic fluid in my mom’s amniotic sac in the uterus – presently my cabin. I have also started practising all sorts of facial expressions to pose for the camera like squinting, yawning, showing surprise, grimacing; I know it will come handy when I am introduced to relatives and friends by my Mom. Amniotic fluid increases around the baby so that it can move freely floating like an astronaut in space and the umbilical cord is now mature. The cord contains 2 arteries and one vein enclosed and protected by a thick gristle like substance called whastons jelly. This makes the

cord slippery and allows moving freely around the baby and avoiding compression. Baby’s skin starts to become water proof. The amniotic fluid is increasing, made by baby’s kidneys and bladder this urine does not contain waste products, since these are transported across the placenta. Baby is bigger than the placenta but you could still cradle it in one hand. I am 15 weeks going into 16 week I am now 15cms long and 6inches under, from head to toe probably weighing about 120 Gms; more in proportion physically but head still accounts for about 1/3 of the body. When my mom lies down the top of the uterus can now be felt nearly half way between the pubic bone and belly button. My vocal cords are now formed and now I am proud I can make different facial expressions. I was startled when my aunt pressed my mom’s belly from outside, and the quivering I reacted with I discovered was totally new reflex I had started presenting. At the age of 16 wks.’ there is a growth spurt and have doubled and added inches to my length. Right now I am of the size of the avocado 4 &1/2 inches long and 3&1/2 ounces. My legs are much more developed, head is more erect and eyes have moved closer to the front of the head. Ears are close to the final position, patterning of scalp has begun and my toe nails have started growing. Lots of things are happening my heart is now pumping about 25 quarts of blood each day and this continue to increase as my body develops and demands increase. Women who have had a baby before may start to feel small sensations of their baby’s movements this week but is more common between 18to 22 wks. . Few women start producing colostrum from their breasts in 16 to 20 wks.’ of pregnancy. Colostrum is the first fluid a woman’s breasts produces. It is a clear creamy yellowish substance that is syrup in consistency. It may be possible for you to hand express drops of colostrum from your nipples during pregnancy or you may notice colostrum leaking. Mom’s energy has returned

The continued suffering of a painful or disfigured eye, the social stigma, all lead to psychological setbacks for the patient. Ocular Prosthesis Custom made ocular prosthesis: Any patient who has an anophthalmic socket may benefit from custom made prosthesis- this may be post-surgical, after enucleation, evisceration, or orbital exenteration. zz The surgery may be performed for a condition of intra-ocular tumor, infection, or trauma: none of these conditions is a contraindication for fitting of a custom prosthesis. zz A disfigured blind eye with volume loss such as with phthisis bulbi can receive a custom-prosthesis. zz A painful blind eye or an anterior staphyloma can receive a prosthesis after preliminary surgery. zz Congenital anomalies such as severe microphthalmos or anophthalmos can also be fitted with a custom prosthesis. zz A patient who has been previously using stock eyes, experiencing irritation, pain or discharge would also benefit from superior cosmesis, motility and wearer comfort of the custom prosthesis. zz Patients on stock eyes who show enophthalmic appearance, squint, or defective closure of the lids will have improved appearance with a custom made eye. zz Pre-Fitting Assessment zz Before fitting of the custom prosthesis, the socket is assessed to ensure it is healthy. Neem has been used as an insecticide, insect repellent, and oral dentifrice, and in traditional medicine to treat malaria, diabetes, worms, and cardiovascular and skin diseases. It reportedly has contraceptive, antiulcer, and fungicidal properties, as well as applications relevant to cancer. Aegle marmelos Botanical name :Aegle marmelos Malayalam name : Koovalam Origin, Distribution and Composition of Bael Fruit The bael is a large tree, 8 to 10 meters in height. It has a big stout trunk, unusual branches with long, straight outgrowth, aromatic leaves, sweet scented and greenishwhite flowers. The fruit is woody and smooth, 5 to 15 cm in diameter. It has numerous seeds which are densely covered with fibrous hair and are embedded in a thick aromatic pulp. The flesh is eaten fresh or dried. Bael tree is held sacred by the Hindus. The history of this tree has been traced to Vedic period (2000 B.C. 800 B.C.). The mention of bael fruit has been made in Yajurveda. The bael tree has great mythological significance and abounds in the vicinity of temples. The leaves of the tree are~ traditionally used as sacred offering to Lord Shiva, the God of health. Lord Shiva is believed to live under the bael tree. The bael tree is indigenous to India and is grown throughout the subcontinent a;’ well as most countries of South-east Asia . An analysis of the bael fruit shows that it consists of moisture 61.5 per cent, protein 1.8 per cent, fat 0.3 per cent, minerals 1.7 per cent, fibre 2.9 per cent and carbohydrates 31.8 per cent per 100 grams of edible portion. Its mineral and vitamin contents include calcium, phosphorus, iron, carotene, thiamin, riboflavin, niacin and vitamin C. Its calorific value is 137. Several chemical constituents have been isolated and from various parts of the bael tree. These include alkaloids, coumarins and steroids. The leaves contain skimianinc, sterol and aegelin. The active constituent of the fruit is marmorosin, which is identical to imperatorin. Odler coumarins contained in the fruits are altoimperatorin and B sitosterol. An Analysis of Popular Health Science Literature in Kannada Communication plays an essential role in disease prevention and health promotion aspects.
Health forms an important part of one’s life and success of an individual in one’s activities depends on his positive health. A healthy population is the strength of a nation. Each individual has to take care of his health to be able to lead a normal life in the society. Today, ample facilities are available to people to take care of their health. Health information, education and communication are a component of good health to the people. Communication plays an essential role in disease prevention and health promotion aspects. Programmes designed to promote changes in health behavior and to encourage early detection and prompt treatment of illness have demonstrated that mass media and other communication strategies can be effective in reducing the risk of illness (US Dept. of Health Publication, 1995). Channels of health education available in India include inter-personal communication, electronic media (audio-visuals), traditional methods (traditional plays and songs) and print media (pamphlets, leaflets, posters, brochures, manuals health care documents, periodicals like magazines, newspapers and books). Print media is very effective in giving health information and promoting health. Health science books are most important communication strategies in providing health information through the print media. The publication of books is increasing in spite of the fear that electronic media will depress in (Report of Task Force – Kannada, 2000). Based on proliferation of these types of books, one can assume that their role in the management of illnesses is significant. An analysis of books provides some insight into the literature. It seems logical that most books emphasize treatments and that alternative treatments, currently undergoing a wave of popularity, are emphasized. Medical educators might wish that patients received more information on the epidemiology, risk factors and natural history of illnesses (Aparna Padiyar, et al. 2001). Effective communication of health information through books is a difficult task. Use of appropriate words in regional language understandable to the reader, authenticity of information, availability of information form part of effective communication. The objective of the present study is undertaking an in-depth study of only the published Kannada books on popular health sciences. Dr. R. Shivaram, a renowned physician and editor of ‘Koravanji’ a humour magazine (akin to the British ‘Punch’ magazine) of yester years can be considered as the father of popular medical and health science, who disseminated these subjects to the

common man. His writings on mental health aspects in the titles ‘Manonandana’ and ‘Manamanthana’ are considered classical even today. Other writers of yester years were the late Dr. Gopalakrishna Rao and Dodderi Venkatagiri Rao who published on an equally important topic of medical science, i.e., sex and sexuality. They have dealt with this subject seriously and scholarly. In the recent days, the late Dr. Anupama Niranjana, the late Dr. Narayanappa and more recently Dr. D.S. Shivappa, Dr. C.R. Chandrashekar, Dr. P.S. Shankar, the late Dr. S.J. Nagalothimath among others have written on various health topics. There are also examples where some medical writers have shown their creativity by writing on stories, novels, biographies in the field of medical sciences. A few such novels have also been successfully brought out as movies and have been widely appreciated by the people. A large number of boos in Kannada on various topics of health are being published since several decades. There has not been a study to look into the content of these books in respect of how the health information is being communicated to the readers, viz., content analysis which includes: (a) the depth of the subject; (b) the utility of the subject to the reader; (c) the adequacy of information; (d) the presentation of the subject including language whether it is understandable; (e) the format of presentation including paragraph construction, easily understandable words, use of illustrations, the use of good type font for better readability, the quality of printing; (f) the cost per book whether the reader can afford to buy or not; (g) provision of bibliography for further references and (h) index. The availability of books is not organized. Because of this lacuna, the areas in which health literature needs to be created is not properly known.

The present study is an attempt at analysing and consolidating the available printed Kannada language books on popular health science literature. The product analysis covers the subject matter of health science areas, the place of publication and the trend of publication with respect to time. The other variables of study include authors, publishers, physical size of the books and price. The objective of the present study is to evaluate health science Kannada books on sample basis. The evaluation is made according to the information provided in the books on five aspects. They are (i) social aspects of illness, (ii) nature of illness (biological aspects of illness) (iii) typography, (iv) explanation of technical terms, provision of subject index and finally the provision of references. Another major objective of the present research was selection of few books for detailed evaluations on (i) content (ii) relevance (iii) clarity (iv) method of presentation (v) utility value (vi) readability (vii) illustrations and (viii) format of presentation of subject matter. The Present Study In the present study, efforts were made to find the contribution of the popular health science books to the health of people by way of the kinds of books published in Kannada language and their contents. First and foremost effort of the study was collecting information on the books published. Books available with individuals, libraries and many places had to be browsed. 951 books were available from different sources. These books had to be listed in chronological order for detailed analysis. The objectives of the analysis were: 1. Compile the available printed popular Kannada health science books 2. Development of a tool to analyse the published books 3. Evaluation of the books using the tool developed. 4. Draw conclusions in the light of the above detailed analysis. The classification of 951 books was made along with the help of a qualified statistician. Detailed evaluation: The detailed analysis of selected sample books was made. Sixty (60) popular health science books in Kannada language were selected for the purpose. The evaluation was done using the tool developed by the researcher for the purpose. The study was conducted under – 1. Readability: (i) content aspect, (ii) relevance aspect, (iii) clarity aspect. 2. Method of presentation: (iv) utility value and (v) Information aspect. The evaluation of information provided in randomly selected 60 health science books in Kannada was made in respect of four aspects, viz.,

(i) social aspects of illness, (ii) nature of illness, (iii) causes of illness, (iv) prevention of illness and promotion of health. Tentative Conclusions 1. Popular Kannada health science is a discipline by itself. Books under this discipline have been written by eminent medical practitioners, academicians, writers in science, non-medical writers, journalist and those interested in disseminating health messages. 2. Information, Education and Communication (IEC) being the pre-requisites in maintaining one’s health, the popular Kannada health science books effectively reach the common man with health science messages through IEC. 3. Most of the books published are in the area of allopathic medicine. Books in ayurvedic medicine find second place. Many authors have written books and also published books by themselves. 4. There are standard publishing houses in Karnataka who have published high quality popular Kannada popular health science books, highly useful to the common man. These publishers are mainly concentrated in cities of Bangalore, Mysore and Dharwad. Often, books published in cities do not reach the interior places. 5. Health science publishing is a flourishing business. Small and medium size publishers can develop their publishing business and grow to become major publishers. Health science publishing is also a service activity and publishers can promote this activity with more conviction as they are accountable to the society. 6. Many writers, doctors and others involved in creation of health science literature need to improve their skills of writing and communication abilities. Introduction of health communication courses need to be considered in Universities and other educational institutions. 7. The need for health information to the public in many districts is the need of the hour. People in economically backward districts do not pay attention t taking care of their health. They think about their health only when it becomes inevitable for them to go to the doctor. Often people suffer mainly because of their ignorance. 8. There is need to publish more and more popular health books to bring awareness among people about health. 9. Patient education is a vital component of an age in which a greater emphasis is placed on patient autonomy, health promotion and self care. Patients no longer take appositive role in their care, and studies show in informed, involved patient is a healthier one. 10. Low literacy has been linked to poor health outcomes such as higher rates if hospitalization and less frequent use of preventive services. Both these outcomes are associated with higher healthcare costs. The physical effects of besity affecting your sex life Before anything else, there is a myth pertaining to sex and obesity that needs to be dispelled. New research has brought into question several claims related to obesity that were previously accepted as truths. One of them is the fact that sex is a great workout that burns anywhere from 100 to 300 calories for short sessions. Latest studies show this to be false. One problem with obese people is that they lack stamina, therefore being unable to sustain high energy activities for long periods of time. The research revealed that the average amount of calories burned during a session of sex was less than 50. This means that it cannot be considered an effective weight loss tool by itself. It can, however, still be a powerful motivator to keep going when combined with a healthy diet and a workout regimen. Obesity does not, by itself, lead to a bad sex life. However, there are physical conditions that go handin-hand with obesity that can cause such problems. One study revealed that as much as a third of all obese people have issues with their sex drive, performance, desire or a combination of the three. One problem that can cause this is high cholesterol. Most people that are overweight have elevated cholesterol levels as an unhealthy diet is oftentimes a main cause of obesity. Such a problem can also have an impact on sexual performance and desire. This issue is much more common in men than in women. Furthermore, the same symptoms can be caused by insulin resistance which is, again, something related to a poor diet. This can also be an early indicator of type II diabetes. However, from a medical standpoint, this condition can also be a direct cause for erectile dysfunction. Surgery is available as a treatment, but it is never the recommended option and, in some cases, it is not even possible. Weight loss through diet and exercise is always the best solution to treat this problem. There is another medical issue that further complicates matters and has a negative impact on the sex life of people struggling with weight problems. Excessive amounts of body fat will increase the levels of SHBG (sex hormone binding globulin) in the system. This is a natural chemical that binds to sex hormone testosterone, which is also used to provide a healthy sexual libido and increase sex drive. Higher amounts of SHBG means that there is less testosterone left to handle the duties associated with a normal sex life and therefore there is a decrease in desire. Obesity also interferes with a person’s sex life in other ways. One study found that it is much harder for obese women to find a sexual partner than it is for women of normal weight. They were 30% less likely to have had a sexual partner within the last year. The differences between obese men and regular men were not significant. Not only that, but the same study revealed that obese women were also more likely to engage in risky sexual behavior. One common issue was the lack of contraceptive measures as overweight women were not as likely to anticipate a sexual encounter and plan ahead. Therefore unplanned

Moreover, we can never locate mind in our body but definitely its controlling part of system and can lead towards positivity or negativity, depending upon the moulding of the same. First basic technique:
Resonating with divine: A unique combination of Bhramari Pranayama with Lotus Posture which keeps away much physical disorder (respiratory, digestive, insomniac, blood circulatory) at the bay. For practicing this technique, choose a quite place in your house where interference of others is minimal. While facing east, sit in the posture of Padmasana, while keeping your spine erect. Close your eyes slowly and start humming. The sound should resonate in your system and your body will feel a slight vibration more to the throat, and diminishing towards lower part of body. The sound should be loud enough to make that desired vibration. For those who have not understood the term humming, a clue can be taken as- pronouncing the word “OM” while keeping your mouth shut. Take deep breath and do humming properly. This is required to be done for 30 Mins and than for 15 Mins in Shavasana with a thought of gratitude. Practice this process continuously without giving a single day break and let this meditation make you calm and be stress free in any situation posed to you and increase concentration power as well as intelligence. The movement of breath in right rhythm with sound vibration in nostril generates laughing properties and gives soothing mind frame. And hence with regular practice gives you immense joy to live. For reaping maximum benefit out of this meditation, make sure that you are doing this at the same place and on the same time. Its common in any society, with the occurrence of socially sensitive issues like rape, child sexual abuse, and, other forms of harassments, there are intense debates and tips for self care discussed in all media’s. But, gradually does this euphoria fade off ? In the recent reported incident’s, particularly the case of one little school child in our city it is over whelming to see support for this child victim and against the abuser in ways of protests, and, several other human chains and links. These activities, post, such horrendous acts, are sure signs of existing humanness and social bonding. This coming together is also indicators of prevailing social security, warmth and trust for all of us. While these demonstrations and rallies are absolutely justified, do they educate our children? No doubt, children will learn to stand up for a cause, but how do we ensure about protecting our children with the right education? We can create a world of difference by educating our children all about The Good Touch- Bad Touch and importantly what’s even beyond. Being a Consultant Psychologist in this city for more than two decades now, in the past three weeks I have received, numerous calls from parents who honestly feel they do not know how to communicate, how much to say or not say, what to communicate to their children about Good and Bad Touch. Survey of our city schools indicate lack of regular Student’s and Teacher’s Sensitization Programs in the different areas of Psycho- social competence. It is important that each and every child begin this learning very early in childhood and also receive regular booster dozes for building a Confident Self. Has this area, unlike health and hygiene programs, been neglected? Does our society at large and Educational Institutions in particular believe that this learning must and is conducted at home by parents? In that case, since each family set up is unique and different, how do we assess the presence and strength of these trainings and learning? Today it’s a universal agreement that every child requires this knowledge and training. It’s important however, to review the content of these trainings and recognize the methods used to impart these trainings. Parents, Teachers and Care takers who are involved in educating children about the Good & Bad Touch must be aware of what they include at every stage of a child’s growing age. Education about Good Touch – Bad Touch is age specific. It is to empower and create

the right attitude towards one’s life. During training, parents must be able to observe the process of change that is evident in their children. If the outcome of such home bound trainings is, Fear and Negativity that is being ingrained in children, then, it’s going to have a negative impact on the Child’s Psycho- social competence as they grow up. The process of The Good Touch- Bad Touch training, beginning from Childhood must bring about a Positive and Strong Self Concept in both girls and boys. With such trainings, when girls and boys enter Teenage they will have the competence to understand people within their social worlds and use this understanding to improve one’s well- being. Parents must remember not to make The Good Touch – Bad Touch training a list of Do’s and Don’ts which is generally a momentary emphasis for any child during that particular milestone of growing up. Have we given it a thought? When thousands of likeminded people can join hands and get- together amidst their hectic daily schedules to, support a victim and their family, and protest against abusers, they are also ready and open to learning and training. Healthy and Positive Attitude Building in any Community does not work by criticizing and Passing the Buck to any particular governance. Becoming sensitive to the requirements of these trainings and making them essential and mandatory through, all Educational Institutions for Parents (the stake holders) and their Children (our future citizens) is the first step we need to consider and take. Experienced and trained Professionals in the area of Mental Health can make this learning true and holistic. While some schools do approach for professional guidance, in some of my interactions with few school authorities, my efforts to showcase list of several Mental Hygiene Programs available for Parents, Teachers and students (Personal Skills training, Social Skills training, Inter-personal competence development) ) could not receive the due importance. I believe, today when we have agreed and welcomed various essential training for our children in areas of Physical Health, Food and Hygiene and Environment it’s time to include Psychologist’s and start the Mental Hygiene exercise in schools. These incident’s which has disturbed us, must be, lessons for all to, act now. Approaching a Psychologist should not be difficult anymore?

June 14th is globally celebrated as “WORLD BLOOD DONOR’S DAY ”. The idea of celebrating blood donor’s day is to create more awareness and motivate more people for blood donating. A unit of blood will go a long way to save a patient while it will cost nothing but compassion for fellow beings. Why should we donate Blood? Medical technology has provided many life saving technologies and discoveries but there is still no substitute for blood. In a medical emergency the most important element is the availability of individuals’ surgery pahonts, trauma victims, cancer patients, thalassemia patient, individuals with anaemie, Dremature babies, pregnant woman during delivery and many more. Please remember tears of a mother cannot save her child but your blood can. Blood donation is the greatest of all donations, there is no bigger donation than blood donation. Who can donate blood? Any healthy individual, both male and female can donate blood. Man once in 3 months and women every 4 months. The universally accepted criteria for blood donor selection are : zz Age between 18 and 65 years. zz Hemoglobin, not less than 12.5g / dl zz Pulse, between 50 and 100 with no irregularities. zz Body weight not less than 45 kgs zz Temperature not exceeding 37.5°c zz Blood pressure should be normal, systotic between 100 to 140 mm and diastolic 70 to 100mm of Hg. Based on body weight, 350ml to 450ml of blood is taken at a time. The entire blood donation procedure is painless and there may be a little sting. When the needle is inserted, which can be minimized by using good quality needle but there should be no pain during the donation. The procedure will take hardly 10 minutes. The body will replace the blood volume placing within 48 hours and it takes 4 to 8 weeks to completely replace the red blood cells. Psoriasis A major skin disease and ayurveda has a good solution
What is Psoriasis? The name psoriasis is from the Greek language, meaning roughly “itching condition” (psora “itch” + -sis “action, condition”). Psoriasis is an autoimmune disease that causes raised, red, scaly patches to appear on the skin. It typically affects the outside of the elbows, knees or scalp, though it can appear on any location. In Ayurveda all skin diseases are named under ‘Kusta’. The literal meaning of kusta is that which changes the normal skin to abnormal state. What happens when it’s ignored? When skin diseases are not treated in time or if ignored it will spread all over the body quickly. Causes: The exact cause of psoriasis is unknown, but it is believed that a combination of several factors contributes to the development of this disease. In
ayurveda there are specific causative factors which is responsible for the initiation of skin disease.
Classification: Though the classics named different types of skin disease with different names its also said that all the skin diseases have the involvement of tridoshas and the names are given according to the predominance of dosha involved in it. Psoriasis appears in a variety of forms with distinct characteristics. Typically, an individual has only one type of psoriasis at a time. Generally, one type of psoriasis will clear and another form of psoriasis
will appear in response to a trigger. zz Plaque Psoriasis: Plaque psoriasis is the most common form of the disease and appears as raised, red patches covered with a silvery white buildup of dead skin cells or scale. These patches or plaques most often appear on the scalp, knees, elbows and lower back. They are often itchy and painful, and they can crack and bleed. zz Guttate Psoriasis: This form of psoriasis appears as small, red, separate spots on the skin. Guttate lesions usually appear on the trunk and limbs and can number in the hundreds. Sometimes lesions form on the scalp, face and ears. They usually are not as thick as the lesions that characterize plaque psoriasis. This form can precede or co-exist with other forms of psoriasis, such as plaque. zz Inverse Psoriasis: Inverse psoriasis shows up as very red lesions in body folds. It may appear smooth and shiny. Many people have another type of psoriasis elsewhere on the body at the same time. Inverse psoriasis is found in the armpits, groin, under the breasts and in other skin folds on the body. It is particularly subject to irritation from rubbing and sweating because of its location in skin folds and tender areas. It usually lacks the scale associated with plaque psoriasis due to the moist environment. characterized by white pustules (blisters of noninfectious pus) surrounded by red skin. The pus consists of white blood cells. It is not an infection, nor is it contagious. Pustular psoriasis is primarily seen in adults. It may be limited to certain areas of the body — for example, the hands and feet. Generalized pustular psoriasis also can cover most of the body. It tends to go in a cycle with reddening of the skin followed by pustules and scaling. zz Erythrodermic Psoriasis: Erythrodermic psoriasis is a particularly inflammatory form of psoriasis that often affects most of the body surface. It is a rare type of psoriasis, occurring once or more during the lifetime of 3 percent of people who have psoriasis. It generally appears on people who have unstable plaque psoriasis. This means the lesions are not clearly defined. Widespread, fiery redness and exfoliation of the skin characterize this form. Severe itching and pain often accompanies it. zz Psoriasis on Specific Locations: Psoriasis can show up anywhere—on the eyelids, ears, mouth and lips, skin folds, hands and feet, and nails. The skin at each of these sites is different and requires different treatments Health Conditions Associated with Psoriasis Individuals with psoriasis are at an elevated risk to develop other chronic and serious health conditions also known as “comorbidities.” It is estimated that up to 30 percent of people with psoriasis develop psoriatic arthritis. People with psoriatic diseases also are at greater risk of developing cardiovascular disease and depression. zz Psoriatic Arthritis: Up to 30 percent of people with psoriasis also develop psoriatic arthritis, which causes pain, stiffness and swelling in and around the joints and tendons. zz Cardiovascular Disease: Research continues to link psoriasis with an increased risk for cardiovascular disease, especially in people with severe psoriasis. People with severe psoriasis are 58 percent more likely to have a major cardiac event and 43 percent more likely to have a stroke. zz Diabetes: People with psoriasis are at an increased risk for type 2 diabetes, according to a 2012 study. People with severe psoriasis are 46 percent more likely to have type 2 diabetes. zz Depression: Psoriasis can cause considerable emotional distress for people, including decreased self-esteem, and an increased incidence of mood disorders, such as depression. One study estimates that approximately one-fourth of people with psoriasis have depression. zz Other conditions: Other comorbidities associated with psoriasis include cancer, obesity, metabolic syndrome and other immune-related conditions such as Crohn’s disease. How to Heal: Though conventional medical system is unable to pin point the specific cause for psoriasis, there are different triggering factors which helps in outbursting of psoriasis. Though there are lot of researches going on, there are some breakthrough attained about psoriasis in recent times, but still there is lot to be understand.

Considering the facts about the skin diseases and its reason, classics shown clear path to be free from skin disease. zz How a cleaned cloth gets coloured easily, in the same manner if one undergoes shodana chikitsas like vamana, virechana or raktasravana the elevated doshas gets eliminated from the body and then the timely administration of other medications will cure all skin diseases. zz When the doshas are aggravated more in the body, one should administer repeated vamana,virechana and raktamokshana keeping eye on the health condition of the person. If the shodana is done excessively ignoring the health condition can cause death. zz The skin diseases are considered difficult to treat because of its complex nature and given much importance to the repeated shodana. Emphasising the importance to shodana indicates that the to correct the cells abnormal behaviour is only possible when the body becomes completly free from toxins. zz One has to undergo repeated Vamana for every 15 days once, Virechana once in 30 days, shirovirechana has to be done once in 3days and Raktamokshana has to be done once in every six months. zz The repeated shodana will help in keeping the body free from accumulation of toxins which are responsible for the diseases. By correcting from the root and avoiding the causetive factors will help brings back normalcy in health. zz In the person who are unable to undergo Vamana or Virechana the doshas gets spread all over making disease difficult to treat. This definition is based upon the root words: spiritus, meaning breath indicating that the Spirit is the breath of life. Intelligentia, meaning “to choose between”. There are three major aspects of spiritual intelligence. They are: zz Identifying with one’s Higher Self or Spirit rather than with the ego. You are not necessarily the person which identifies with the mortal body, problems, jobs, relations, past, present or future. These are roles you play. You are a spiritual being having a human experience. zz Understanding Universal Law of Cause and Effect. Spiritual Intelligence means that you take complete responsibility for your life, your situation, and for yourself. Eating healthy and by including plenty of fruits and vegetable in your meals will speed up your recovery from labor and birth and also gives you energy that you will need to keep up with your child. Eat fresh & whole foods like vegetables and fruits as it contains lots of vitamins, minerals and fiber. Keep your protein intake between 62-65g/ day so that you don’t lose your muscle. Steer clear of packaged and processed food that often has no nutritional value. Eat small but frequently to increase your metabolic rate. Add healthy fat (omega 3 fatty acid) in your diet like fish, walnuts, egg, spinach, basil etc. Avoid Active and passive smoking as it blocks nutrition absorption. Its best to avoid alcohol as it enters breast milk and affects your baby causing drowsiness. Eat lot of iron rich food like spinach, green leafy vegetable, black beans etc. Eat complex carbohydrates like brown rice, wheat, sweet potato etc which has lots of fibers that will help you feel full as well as converts fat into energy.
