aricose veins are gnarled, enlarged veins. For many people, varicose veins and spider veins — a common, mild and medically insignificant variation of varicose veins — are simply a cosmetic concern. For other people, varicose veins can cause aching pain and discomfort. Sometimes the condition leads to more serious problems. Varicose veins may also signal a higher risk of other disorders of the circulatory system. Varicose veins are a common condition in the India, affecting up to 15 percent of men and up to 25 percent of women. Treatment may involve selfhelp measures or procedures by your doctor to close or remove veins. ªª Signs and symptoms: Some people with varicose veins don’t experience any discomfort from the condition. When painful signs and symptoms do occur, they may include: ªª An achy or heavy feeling in your legs, and burning, throbbing, muscle cramping and swelling in your lower legs. Prolonged sitting or standing tends to make your legs feel worse. ªª Itching around one or more of your veins. ªª Skin ulcers near your ankle, which represent a severe form of vascular disease and require immediate attention. Varicose veins are dark purple or blue in color and may appear twisted and bulging — like cords. They commonly appear on the backs of the calves or on the inside of the leg. However, they can form anywhere on your legs, from your groin to your ankle. ¾¾ Causes: Arteries carry blood from your heart to the rest of your tissues. Veins return blood from the rest of your body to your heart, so the blood can be recirculated. To return blood to your heart, the veins in your legs must work against gravity. Muscle contractions in your lower legs act as pumps, while toned, elastic vein walls help blood return to your heart. Tiny one-way valves in your veins open as blood flows toward your heart then close to stop blood from flowing backward. As you get older your veins can lose elasticity, causing them to stretch. The valves in your veins

Any vein may become varicose, but the veins most commonly affected are those in your legs and feet. That’s because standing and walking upright increases the pressure in the veins in your lower body.

Blood pools in your veins, and your veins enlarge and become varicose. The veins appear blue because they contain deoxygenated blood, which is in the process of being recirculated. Some pregnant women develop varicose veins. Pregnancy increases the volume of blood in your body, but decreases the flow of blood from your legs to your pelvis. This circulatory change is designed to support the growing fetus, but it can produce an unfortunate side effect — enlarged veins in your legs. Varicose veins may surface for the first time or may worsen during late pregnancy, when your uterus exerts greater pressure on the veins in your legs. Hemorrhoids are varicose veins located in and around the anus. ¾¾ Risk factors: These factors increase your risk of developing varicose veins: ¾¾ Age: Aging causes wear and tear on the valves in your veins that help regulate blood flow. Eventually, that wear causes the valves to malfunction. ¾¾ Sex: Women are more likely than men to develop the condition. Hormonal changes during pregnancy, premenstruation or menopause may be a factor. Female hormones tend to relax vein walls. Taking hormone replacement therapy or birth control pills may increase your risk of varicose veins. ¾¾ Genetics: If other family members had varicose veins, there’s a greater chance you will too. ¾¾ Obesity: Being overweight puts added pressure on your veins. ¾¾ Standing for long periods of time: Your blood doesn’t flow as well if you’re in the same position for long periods. ¾¾ Screening and diagnosis:

RESPIRATION: If it is failing, give artificial respirationMOUTH to MOUTH or MOUTH to NOSE BLEEDING: Arrest the bleeding and protect the wound. Apply direct or indirect pressure. Cover with a dressing; apply a pad and firm bondage. Keep at rest. FRACTURES: IMMOBILISE it with a well padded stiff support reaching the joints on either side. Apply bandages on either side of the site and at the joints on either side support. BURNS AND SCALDS: A burn is caused by dry heat and a scald by moist heat like steam, very hot water or oil. IMMEDIATELY cool the area with cold water - for 15 mins till pain subsides. DO NOT break blisters, or apply anything on the burns, cover with a sterile or clean cloth, pad and bandage, give fluids. NOSE BLEEDING: Sit him up facing the breeze and the head slightly forward. Ask him to breathe through the mouth and NOT to blow the nose. Apply a cold compress over the nose. The soft part of the nose may be pinched close with the fingers 10 mins. Cold application on the back of the neck and forehead may help. ANIMAL BITES: Wash with soap and plenty of water, loose bandage may be applied- Get quick medical aid. FAINTING: Lay him dawn and loosen the clothing around chest and vaist. Turn head to one side. The legs may be raised a little. Do not attempt to give any solids or liquids. On recovery a small quantity of drink maybe given and allowed to sit up and move after rest. GENERAL: If you are in doubt about the treatment DO NOT DO anything. You may do more harm. CUTS & ABRASIONS: Cleanse area thoroughly with soap and warm water, carefully washing away any dirt. Apply direct pressure to wound until bleeding stops. Put sterile bandoge on wound. Look out for signs of infection like swelling, redness, pain, fever and pus. Seek medical attention if the wound is very deep, if it is on a sensitive part of the body and if it shows any signs of infection. ELECTRIC SHOCK: Try to remove the victim from the source of electricity as soon as possible with the help of a dry wooden stick, rubber gloves, thick bulk of dry cotton, dry cloth. Avoid direct contact. Turn off the master switch to disconnect the power If he is not breathing, begin rescue breathing immediately. If there are any signs of respiratory distress start mouth to mouth respiration at once. Take him to the doctor as soon as possible.

Skin cancer

Skin cancer develops primarily on areas of sun-exposed skin, including the scalp, face, lips, ears, neck, chest, arms and hands, and on the legs in women. But it can also form on areas that rarely see the light of day — your palms, beneath your fingernails, the spaces between your toes or under your toenails, and your genital area. Skin cancer affects people of all skin tones, including those with darker complexions. When melanoma occurs in those with dark skin tones, it’s more likely to occur in areas not normally considered to be sun-exposed. A cancerous skin lesion can appear suddenly or develop slowly. Its appearance depends on the type of cancer. ¾¾ There are three major types of skin cancer Basal cell carcinoma: It’s most common, easily treated and the least likely to spread. Basal cell carcinoma usually appears as one of the following: A pearly or waxy bump on your face, ears or neck A flat, flesh-colored or brown scar-like lesion on your chest or back Squamous cell carcinoma: is easily treated if detected early, but it’s slightly more apt to spread than is basal cell carcinoma. Most often, squamous cell carcinoma appears as one of the following: A firm, red nodule on your face, lips, ears, neck, hands or arms A flat lesion with a scaly, crusted surface on your face, ears, neck, hands or arms

Melanoma: It is the most serious and responsible for most skin cancer deaths. Melanoma can develop anywhere on your body especially appears on the trunk, head or neck for men and In women, most often develops on the arms or legs. Warning signs of melanoma include: A large brownish spot with darker speckles located anywhere on your body A simple mole located anywhere on your body that changes in color, size or feel or that bleeds A small lesion with an irregular border and red, white, blue or blue-black spots on your trunk or limbs Shiny, firm, dome-shaped bumps located anywhere

possible that varicose veins can recur. Prevention: There’s no way to completely prevent varicose veins. But improving your circulation and muscle tone can reduce your risk of developing varicose veins or getting additional ones. Traditional, common-sense approaches include: ªª Exercise. Get your legs moving. Walking is a great way to encourage blood circulation in your legs. Your doctor can recommend an appropriate activity level for you. ªª Watch your weight, and your diet. Shedding excess pounds takes unnecessary pressure off your veins. What you eat makes a difference, too. Follow a low-salt, high-fiber diet to prevent the swelling that may result from water retention and constipation. ªª Watch what you wear. Avoid high heels. Lowheeled shoes work calf muscles more, which is better for your veins. Don’t wear tight clothes around your waist, legs or groin. Tight panty-leg girdles, for instance, can restrict circulation. ªª Elevate your legs. To improve venous circulation, take several short breaks daily to elevate your legs above the level of your heart. For example, lie down

with your legs resting on three or four pillows. Avoid long periods of sitting or standing. Make a point of changing your position frequently to encourage blood flow. Try to move around at least every 30 minutes. ªª Don’t sit with your legs crossed. This position can aggravate circulation problems. ¾¾ Self-care: Wearing compression stockings is often the first approach to try before moving on to other treatments. Compression stockings are worn all day. They steadily squeeze your legs, helping veins and leg muscles move blood more efficiently. The amount of compression varies by type and brand. Prominent varicose veins on the back of the right leg caused by a leaking valve at the junction between superficial and deep veins behind the knee (saphenopopliteal junction). Varicose veins on the inner aspect of the right thigh and calf caused by a leaking valve in the groin (sapheno femoral incompetence). High pressure in the superficial veins has caused typical skin changes in the “gaiter” area above the ankle with eczema, pigmentation and superficial skin ulceration.

by your doctor or dermatologist. Causes Skin cancer begins in your skin’s top layer — the epidermis. Healthy new cells push older cells toward the skin’s surface, where they die and eventually are sloughed off. This process is controlled by DNA — the genetic material that contains the instructions for every chemical and biological process in your body. Skin cancer occurs when this process malfunctions. When DNA is damaged, changes occur in the instructions, which can cause new cells to grow out of control and form a mass of cancer cells. The role of UV light: Much of the damage to DNA in skin cells results from ultraviolet (UV) radiation found in sunlight and in commercial tanning lamps and tanning beds. Tanning beds deliver high doses of ultraviolet, which is dangerous. Other factors that may contribute to skin cancer: Sun exposure doesn’t explain melanomas or other skin cancers that develop on skin not ordinarily exposed to sunlight. Heredity may play a role. Skin

Less common skin cancers Other, less common types of skin cancer include: Kaposi sarcoma. This rare form of skin cancer develops and causes red or purple patches on the skin or mucous membranes. Like melanoma, it’s a serious form of skin cancer. It’s mainly seen in people with weakened immune systems, such as people with AIDS, and in people taking medications that suppress their natural immunity, such as people who’ve undergone organ transplants. Merkel cell carcinoma. In this rare cancer, firm, shiny nodules occur on or just beneath the skin and in hair follicles. The nodules may be red, pink or blue and can vary in size from a quarter of an inch (about 6 millimeters) to more than 2 inches (about 50 millimeters). Merkel cell carcinoma is usually found on sun-exposed areas on the head, neck, arms and legs. Unlike basal and squamous cell carcinomas, Merkel cell carcinoma grows rapidly and often spreads to other parts of the body. Sebaceous gland carcinoma. This uncommon and aggressive cancer originates in the oil glands in the skin. Sebaceous gland carcinomas — which usually appear as hard, painless nodules — can develop anywhere, but most occur on the eyelid, where they’re frequently mistaken for benign conditions. Actinic keratoses appear as rough, scaly, brown or dark-pink patches. They’re most commonly found on the face, ears, lower arms and hands of fair-skinned people whose skin has been damaged by the sun. Not all skin changes are cancerous. The only way to know for sure is to have your skin examined

cancer can also develop from exposure to toxic chemicals or as a result of radiation treatments. Risk factors These factors may increase your risk of skin cancer: Fair skin. Having less pigment (melanin) in your skin provides less protection from damaging UV radiation. If you have blond or red hair, light-colored eyes, and you freckle or sunburn easily, you’re much more likely to develop skin cancer than is a person with darker features. A history of sunburns. Every time you get sunburned, you damage your skin cells and you have a risk of developing skin cancer. Excessive sun exposure. Anyone who spends considerable time in the sun may develop skin cancer, especially if your skin isn’t protected by sunscreen or clothing. Tanning also puts you at risk. A tan is your skin’s injury response to excessive UV radiation. Sunny or high-altitude climates. People who live in sunny, warm climates are exposed to more sunlight than are people who live in colder climates. Living at higher elevations, where the sunlight is strongest, also exposes you to more radiation. Moles. People who have many moles or abnormal moles which look irregular and are generally larger than normal moles — are more likely to become cancerous. If you have a history of abnormal moles, watch them regularly for changes. Precancerous skin lesions. Having skin lesions known as actinic keratoses can increase your risk of developing skin cancer. These precancerous skin growths typically appear as rough, scaly patches that range in color from brown to dark pink. They’re most common on the face, lower arms and hands of fair-skinned people whose skin has been sun damaged. A family history of skin cancer. If one of your parents or a sibling has had skin cancer, you may be at increased risk of the disease. A personal history of skin cancer. If you developed skin cancer once, you’re at risk of developing it again. Even though it is been successfully removed can recur in the same spot, often within two to three years. A weakened immune system. People with weakened immune systems are at greater risk of developing skin cancer. This includes people living with HIV/AIDS or leukemia and those taking immunosuppressant drugs after an organ transplant. Fragile skin. Skin that has been burned, injured or weakened by treatments for other skin conditions is more susceptible to sun damage and skin cancer. Exposure to environmental hazards. Exposure to environmental chemicals, including some herbicides, increases your risk of skin cancer. Age. The risk of developing skin cancer increases with age, primarily because many skin cancers develop slowly. The damage that occurs during childhood or adolescence may not become apparent until middle age. Still, skin cancer isn’t limited to older people. Basal cell and squamous cell carcinomas are increasing fastest among women younger than 40. When to seek medical advice: See your doctor if you notice a new skin growth, a bothersome change in

your skin, a change in the appearance or texture of a mole, or a sore that doesn’t heal in two weeks. As with most cancers, early detection increases the chances of successful treatment. Don’t wait for the area to start hurting — skin cancer seldom causes pain. Tests and diagnosis: Your doctor may suspect cancer by simply looking at your skin. But to properly diagnose skin cancer, your doctor or dermatologist will need to take a small sample of your skin (biopsy) for analysis in a lab. A biopsy can usually be done in a doctor’s office using a local anesthetic. Skin cancer is generally divided into two stages: Local. In this stage, cancer affects only the skin. Metastatic. At this point, cancer has spread beyond the skin. Treatments and drugs Treatment for skin cancer and the precancerous skin lesions known as actinic keratoses varies, depending on the size, type, depth and location of the lesions. Often the abnormal cells are surgically removed or destroyed with topical medications. Most skin cancer treatments require only a local anesthetic and can be done in an outpatient setting. Sometimes no treatment is necessary beyond an initial biopsy that removes the entire growth. If additional treatment is needed, options may include: Freezing, Excisional surgery, Laser therapy, Mohs surgery, Curettage and electrodesiccation ,

Radiation therapy, Chemotherapy. Prevention: Most skin cancers are preventable. To protect yourself, follow these skin cancer prevention tips: Avoid the sun between 10 a.m. and 4 p.m. Because the sun’s rays are strongest during this period, try to schedule outdoor activities, even in winter or when the sky is cloudy. Remember, sunburns and suntans cause skin damage that can increase your risk of developing skin cancer. Sun exposure accumulated over time also may cause skin cancer. Wear sunscreen year-round. Sunscreens don’t filter out all harmful UV radiation, but still play a major role in an overall sun protection program by blocking UVA rays. Choose a broad-spectrum sunscreen on all exposed skin, including your lips,

ancer is a scary word. We all know someone in family or friend got cancer and died of it. So we all think that cancer means end of life or death. But wait-----, this not always true. Many people are

living with a diagnosis of cancer. Cancer is actually a group of diseases. Different types of cancer can behave very differently. For example, lung cancer and breast cancer are very different diseases. They grow at different rates and respond to different treatments. That is why people with cancer need treatment that is aimed at their particular kind of cancer. Chances of cancer cure depend on the type of cancer and what stage it is detected and how best it is treated. ¾¾ What is a cancer? We all know that human body is made up of cells. There are billions of cells in each person’s body. Normally, cells grow and divide to form new cells, as the body needs them. When cells grow old, they die, and new cells take their place. Sometimes, this orderly process goes wrong. New cells form when the body does not need them, and old cells do not die when they should. These extra cells can form a mass of tissue

the tips of your ears, and the backs of your hands and neck. Apply sunscreen 20 to 30 minutes before sun exposure and reapply it every two hours throughout the day, as well as after swimming or exercising. Wear protective clothing. Sunscreens don’t provide complete protection from UV rays. That’s why it’s a good idea to also wear dark, tightly woven clothing that covers your arms and legs, and a broad-brimmed hat. Don’t forget sunglasses. Look for those that block both UVA and UVB rays. Avoid tanning beds and tan-accelerating agents. Tanning beds emit UVA rays, which may be as dangerous as UVB rays — especially since UVA light penetrates deeper into your skin and causes precancerous skin lesions. Be aware of sun-sensitizing medications. Some common prescription and over-the-counter drugs — including antibiotics; certain cholesterol, high blood pressure and diabetes medications; birth control pills; nonsteroidal anti-inflammatory drugs; and the acne medicine— can make your skin more sensitive to sunlight. If you are sensitivity to sunlight, be sure to take extra precautions. Check your skin regularly and report changes to your doctor. Examine your skin often for new skin growths or changes in existing moles, freckles, bumps and birthmarks. With the help of mirrors, check

your face, neck, ears and scalp. Examine your chest and trunk, and the tops and undersides of your arms and hands. Examine both the front and back of your legs, and your feet, including the soles and the spaces between your toes. Also check your genital area, and between your buttocks. Have regular skin exams. Consult your doctor for a complete skin exam every year if you’re older than 40 or more often if you’re at high risk of developing skin cancer. A guide for Self-examination: To detect melanomas or other skin cancers, use the A-B-C-D-E skin selfexamination guide, A is for asymmetrical shape. Look for moles with irregular shapes, such as two very different-looking halves. B is for irregular border. Look for moles with irregular, notched or scalloped borders — the characteristics of melanomas. C is for changes in color. Look for growths that have many colors or an uneven distribution of color. D is for diameter. Look for growths that are larger than about 1/4 inch (about 6 millimeters). E is for evolving. Look for changes over time, such as a mole that grows in size or that changes color or shape. Moles may also evolve to develop new signs and symptoms, such as new itchiness or bleeding.

called a growth or tumor. This tumor can be benign or malignant. Malignant tumors are cancer. In what ways benign tumors are different?

Benign tumors Can be removed and usually do not grow back Do not invade the tissues around them

Malignant tumors Often can be removed but sometimes grow back Can invade and damage nearby tissues and organs

Do not spread to other parts of the body

Can spread to other parts of the body (metastasis). Tumor cells can directly invade the adjacent organs or enter into blood stream or lymphatic system.

Rarely life threatening

May be life threatening.

Most cancers are named for where they origin. For example, lung cancer starts in the lung, Lymphoma is cancer that starts in the lymphatic system. And Leukemia is cancer that starts in white blood cells. When cancer spreads and forms a new tumor in another part of the body, the new tumor has the same kind of abnormal cells and the same name as the primary tumor. For instance, breast cancer that spreads to the liver is still called breast cancer, not liver cancer. For that reason, it is treated as breast cancer, not liver cancer. Lots of researches are happening to know what causes cancer, and how it grows and progresses, new and better ways to prevent, detect, and treat it. Researchers also are looking for ways to improve the quality of life for people with cancer during and after their treatment. ¾¾ Risk Factors for Cancer A risk factor is anything that increases a person’s chance of getting a disease. When a person who has

a risk factor is diagnosed with cancer, there is no way to prove that the risk factor actually caused the cancer. Different kinds of cancer have different risk factors. Some of these factors can be modified like food habits and life style changes. Where as certain risk factors like age and sex cannot be modified. The important risk factors, which are more likely to affect personal cancer risk, include tobacco use, unhealthy diet, and physical activity. Tobacco and cancer risk: Cancers of the lung, mouth, larynx, bladder, kidney, cervix, esophagus, and pancreas are related to tobacco use, including cigarettes, bidies, cigars, chewing tobacco, snuff, smokeless tobacco in the form of pan masala and Gutkha. Smoking alone causes one-third of all cancer deaths. The risk of having lung cancer and other smoking-related cancers is related to total lifetime exposure to cigarette smoke. This is measured by, number of cigarettes smoked each day, the age at which smoking began, the number of years a person has smoked. There is no way to accurately calculate a person’s risk of getting cancer, but the more you smoke and the longer you do it, the greater your risk. ctopic means “out of place.” In an ectopic pregnancy, a fertilized egg has implanted outside the uterus. The egg settles in the fallopian tubes in more than 95% of ectopic pregnancies. This is why ectopic pregnancies are commonly called “tubal pregnancies.” The egg can also implant in the ovary, abdomen or cervix, so you may see these referred to as cervical or abdominal pregnancies. None of these areas has as much space or nurturing tissue as a uterus for a pregnancy to develop. As the fetus grows, it will eventually burst the organ that contains it. This can cause severe bleeding and endanger the mother’s life. A classical ectopic pregnancy does not develop into a live birth. ¾¾ When is it likely to happen? An ectopic is most commonly found between the fourth and tenth week of pregnancy - usually from weeks five to seven. ¾¾ Who is at risk? An ectopic pregnancy can happen to any woman, but there are circumstances, which make it more likely. These might include: ªª Women who have had pelvic inflammatory disease (which is most often cause by the sexually transmitted infection chlamydia or gonorrhoea) as this can cause damage and scarring the fallopian tubes. Some experts believe that up to half of all ectopic pregnancies are related to the chlamydia infection. and come and go or vary in intensity. ¾¾ Any of the following additional symptoms can also suggest an ectopic pregnancy: When you come to the hospital, you will be asked to have a urine pregnancy test. If the urine test comes back positive, you’ll probably be given a quantitative hCG test. This blood test measures levels of the harmone Beta human chorionic gonadotropin (hCG), which is produced by the placenta and appears in the blood and urine as early as 8 to 10 days after conception. Its levels double every 2 days for the first several weeks of pregnancy, so if HCG levels are lower than expected for your stage of pregnancy, one possible explanation might be an ectopic pregnancy. You’ll also get an ultrasound examination, which can show whether the uterus contains a developing fetus or if masses are present elsewhere in the abdominal area. But t he ultrasound might not be able to detect every ectopic pregnancy. You may also have a pelvic examination to locate the areas causing pain, to check for an enlarged, pregnant uterus or to find any masses. Even with the best equipment, it’s hard to see a

in cases of emergency or extensive internal injury. However, majority of the times in our hospital it is removed using laparoscopy, a less invasive surgical procedure. The surgeon makes small incision in the lower abdomen and then inserts a tiny video camera and instruments through these incisions. The image from the camera is shown on a screen in the operating room, allowing the surgeon to see what’s going on inside your body without making large incisions. The ectopic pregnancy is then surgically removed with or without removal of the fallopian tube depending upon how damaged it is. ¾¾ What about Future Pregnancies? Some women who have had ectopic pregnancies will have difficulty becoming pregnant again. This difficulty is more common in women who also had fertility problems before the ectopic pregnancy. Your prognosis depends on your fertility before the ectopic pregnancy, as well as the extent of the damage that was done. The likelihood of a repeat ectopic pregnancy increases with each subsequent ectopic pregnancy. Once you have had one ectopic pregnancy, you face an approximate 15% chance of having another.

RETINAL DETACHMENT

T

he retina is the neurosensory tissue that lines the back inside wall of the eye. Just like the film in a camera, the retina transfers the light coming into our eye into vision. The center of the retina is called the macula and is the part responsible for fine detailed vision, i.e. reading, recognizing faces, etc. The remainder of the retina, the peripheral retina, is for side vision. When the retina detaches, it separates from the back wall of the eye and looses its blood supply and source of nutrition. The retina will degenerate and lose its ability to function if it remains detached. Central vision will be lost if the macula remains detached. ¾¾ The causes of retinal detachment can be divided into: ªª Rhegmatogenous Retinal Detachment: This detachment is due to a retinal break tear which allows the liquid vitreous (the fluid that fills the center of the eye ball) to pass through the break and lift off the retina. ªª Exudative Retinal Detachments: This type of detachment is due to pull on the retina usually from fibro-vascular tissue within the vitreous cavity. Proliferative diabetic retinopathy is the most common cause of traction retinal detachments ªª Tractional Retinal Detachments: This type

Rhegmatogenous Retinal detachment This is the most common type of retinal detachment and usually occurs when the vitreous separates from the back wall of the eye. The vitreous is the clear gel that fills the central cavity of the eye. The formed vitreous gel liquefies with age and eventually falls away or separates from the retina. This is called a posterior vitreous detachment (PVD) and is a normal event occurring in most people sometime between 40-70 years of age. A Retinal Detachment can occur once there is a retinal break. The liquid vitreous passes through the break and goes under the retina. The retina will then start to detach from the underlying tissue. Since most tears occur in the peripheral retina, the detachment will first cause loss of a portion of the side vision. This can be seen as a curtain or dark shadow involving the peripheral vision. As the detachment extends towards the macula, the shadow will also enlarge. Central vision will be lost if the macula detaches. Without surgical repair, most Retinal rhegmatogenous detachments will eventually involve the entire retina and all vision will be lost ¾¾ Repair of Rhegmatogenous Retinal detachment Fortunately, over 90% of retinal detachments can be repaired with a single procedure. Currently, there are 3 different surgical approaches to repairing a detachment: scleral buckle procedure, vitrectomy and pneumatic retinopexy. Scleral buckle: This surgical procedure has been in use for more than 30 years, and, until approximately 20 years ago, was the only procedure available. It is still commonly used for rhegmatogenous retinal detachments, especially when there are no complicating factors. The procedure involves localizing the position of all the retinal breaks, treating all retinal breaks with cryo / laser and supporting all the retinal breaks with a scleral buckle. The buckle is usually a piece of silicone sponge or solid silicone. The type and shape of the buckle varies depending on the location and number of retinal breaks. The buckle is sewn onto the outer wall of the eyeball (sclera) to create an indentation or buckle effect inside the eye. The buckle is positioned so that it pushes in on the retinal break and effectively closes the break. Once the break is closed, the fluid under the retina (subretinal fluid) will usually spontaneously resolve over 1-2 days. Sometimes the

Computer Vision Syndrome (CVS) For details contact

surgeon elects to drain the subretinal fluid at the time of surgery. Most often, a scleral buckle procedure can be done with local anesthesia and as same day surgery (in and out of the hospital on the same day). Vitrectomy: Usually referred to as a Pars Plana Vitrectomy (PPV), this procedure was first used - 20 years ago and has been continuously refined and improved since then. It is most commonly used for traction retinal detachments from diabetes but is also used for rhegmatogenous detachments especially if they are associated with vitreous traction or Proliferative changes / vitreous hemorrhage. The procedure involves making small incisions into the wall of the eye to allow the introduction of instruments into the vitreous cavity (the middle of the eyeball). The first part of the procedure usually is the removal of the vitreous using a vitreous cutter. Then, depending on the type and cause of the detachment, a variety of instruments (scissors, forceps, pics, lasers, etc...) and techniques (excision of tractional bands, air-fluid exchange, silicone oil fill, etc...) are used to reattach the retina. Recently suturless 23 / 25 guage Vitrectomy has become popular as it is more patient friendly and the visual rehabilitation is quicker. In this procedure as there are no sutures the post operative irritation and watering is less. A PPV can also usually be done as same day surgery and with local anesthesia. It is sometimes important to maintain a specific head position after surgery to keep the retina attached. Pneumatic Retinopexy: Over the past 10 years, this has become a popular way to repair a straightforward rhegmatogenous retinal detachment, especially if there is a single break located in the superior portion of the retina. This procedure involves injecting a gas bubble into the middle part of the eye (vitreous cavity). It is then critical to position oneself so that the gas bubble covers the retinal break. If the break can be covered by the bubble, the subretinal fluid will usually resolve within 1-2 days. The retinal tear is treated with cryo before the bubble is injected or with laser after the retina has flattened. The main advantages of this approach are that it can be done in the office, thus avoiding hospitalization and that it avoids seme of the complications of scleral buckling surgery, although it has its own set of complications.

omputer Vision Syndrome (CVS) is a very real problem for many people who spend hours daily in front of a computer screen. CVS is a complex of eye and vision problems. CVS is characterized by visual symptoms which result from interaction with a computer display or its environment. Eye strain and other symptoms of CVS occur in adults as well as children. ¾¾ Causes of Computer Vision Syndrome CVS is caused by our eyes and brain reacting differently to characters on a computer screen than they do to printed characters. Our eyes have little problem focusing on printed material that has dense black characters with well-defined edges. But characters on a computer screen don’t have the same degree of contrast and definition. Words on a computer screen are created by combinations of tiny points of light (pixels), which are brightest at the center and diminish in intensity toward their edges. This makes it more difficult for our eyes to maintain focus on these images. Instead, our eyes want to drift to a reduced level of focusing called the “resting point of accommodation” or RPA. Our eyes unwillingly

This non-stop flexing of the eyes and focusing of muscles creates the fatigue and eye strain that commonly occur during and after computer use. Another cause is reduced blinking. When we look at the computer screen we tend to blink less than what we should. This leads to dryness of eyes and burning, itching etc. ¾¾ Symptoms of Computer Vision Syndrome If one spends more than two hours/day in front of a computer screen, it’s likely that he will experience some degree of CVS. Symptoms of CVS include headaches, loss of focus, burning eyes, tired eyes, double vision, blurred vision, neck and shoulder pain. ¾¾ How does your vision affect your body? As you age, your eyes will develop a condition called presbyopia. This means that for people in their 40s or older, the computer screen gets a little fuzzy, even with glasses. Trying to read a fuzzy screen can give you a headache. ¾¾ Where do the other aches and pains come in? These are often caused by trying to read the screen through the bottom portion of bifocals or though half-eye reading glasses. There are innumerable substances in the environment which can cause mild to violent reactions in many people. These reactions range from true allergies due to Intolerance of certain foods and substances, to those resulting from pollution

llergic reactions may occur within a few minutes of the patient coming in contact with the allergen, or they may be delayed for several hours or even several days. Almost any part of the body can be affected by allergies. The portion of the body which is affected is called a shock organ. Common sites are the nose and eyes, the skin, chest, intestines and ears. Allergic reactions are caused by a wide range of substances and conditions. These include pollen, dust, cosmetics and animal hair, poisonous plants, serums, vaccines and drugs; physical agents such as heat, cold and sunlight; as well as a variety of foods. Among the numerous allergens in the food department, the more common ones are oranges, milk, eggs, wheat, fish, chocolates, cabbage, potatoes, tomatoes and strawberries. Symptoms: The symptoms of allergy are as varied as the substances causing the reaction.

These include recurring headache, migraine, dizziness, irritability, nervousness, depression, neuralgia, sneezing, conjunctivitis, diabetes, eczema, heart-burn, hay fever, indigestion, constipation, diarrhoea, gastric ulcer, asthma, overweight, high blood pressure, chest pain, heart attacks, a stuffy or runny nose, shortness of breath, swelling of the face and eyes, etc. The same food can cause different symptoms in different people. Many allergies are multiple and may be caused by multiple allergens. Causes: Allergy is an indication of lowered resistance and internal disharmony caused by dietetic errors and faulty style of living. It is believed that the major cause of allergy is feeding babies such foods as cereals, meat, corns, whole milk, etc. Before they reach the age of 10 to 12 months. These foods cause allergic reactions as babies lack the proper enzymes needed for their digestion before that age. Babies should be breast-fed for at least eight months as this is nature’s way of providing all the required nutrients during this period. Another important cause of allergy is today’s processed foods loaded with numerous chemical additives, many of which cause powerful reactions. An allergic condition can result from diet imbalances. There can be a breakdown in the body’s ability to handle sugar due to excessive intake of refined sugar and consequent blood sugar irregularities, or mineral and vitamin imbalances due to defective dietary patterns. Emotional and psychological stress can also lead to allergies. According to research on stress, allergic symptoms are often nothing more than body’s reaction to stress. A person can go through chronic stress,

become sensitive to common foods or common place substances like petrol fumes. Treatment: There are various ways to tackle many of the allergic disturbances. First, the sources must be identified. This is a difficult but not impossible task. Second, once the sources are discovered, they should be avoided. Third, and most important, general health and resistance should be built up to establish immunity to them. There are two methods to detect disturbing foods. The first method is the trial and error elimination diet. This automatically eliminates many hazards and foods. Keep to organic, untreated, unprocessed foods as far as possible and you will eliminate another set of hazards such as pesticides, various sprays and other poisons. After having eliminated as many disturbing factors as possible, a self-search should be carried out to ascertain any suspicious symptoms from foods. It is advisable to try an elementary diet, excluding suspected foods for two weeks until the cause is detected. Occasionally, by changing the brand or the type, you can find a food substitute that does not upset you. Another way to detect the cause of allergy is “pulse test�. The method is as follows: Check your pulse before a meal. Then limiting that meal to one food only, with for half an hour after eating and take your pulse again. A slight increase is considered normal, even up to 16 extra beats. If your pulse does not rise above 84, you may be allergy free. But if your pulse rises beyond that point, and remains high an hour after the meal, you have found your food allergy. The best way, however, to prevent or overcome allergies is to strengthen the overall physical resistance so as not to fall an easy prey to every allergen that comes along. To start with, the patient should fast on fresh fruit juices for four or five days. Repeated short juice fasts are likely to result in better tolerance to previous allergies. After the fruit juice fast, the patient can take a mono diet of vegetables or fruits such as carrots, grapes or apples, for one week. After that one more food is added to the mono diet. A week later the third food is added and so on. After four weeks, the protein foods can be introduced, one at a time. In case an allergic reaction to a newly introduced food is noticed, it should be discontinued and a new food tried. In this way all real allergens can be eventually eliminated from the diet. The body requires a large alkaline reserve for its daily activity. The many emergencies of acid formation through the day from wrong foods, fatigue, mental stress and lack of sleep can be met by the competency of the alkaline reserves. Boosting the normal body reserve of alkaline by liberal use of alkaline forming foods is essential for those suffering from allergies. The foods which should be excluded from the diet are tea, coffee, chocolate, cola drinks, alcohol, sugar, sweets and foods containing sugar, refined cereals, meats, fish, chicken, tobacco, milk, cheese, butter, smoked, salted, pickled foods and foods

containing any chemical additives, preservatives and flavouring. These foods cause either toxic accumulations or over stimulation of adrenal glands or strain on pancreatic enzymes production or disturb the blood sugar balance. For preventive purposes, the entire C complex vitamins known as the bioflavonoid, are recommended. They gradually strengthen cell permeability to help immunise the body from various allergies, especially hay fever. Often the addition B5 , or pantothenic acid brings great relief to allergy sufferers. Multiple allergies may result from poor adrenal gland functioning. In such cases liberal amounts of pantothenic acids help cure them, although the recovery will take several weeks. An adequate intake of vitamin E is also beneficial as this vitamin possesses effective anti-allergic properties, as some studies have shown. An exciting remedy for allergy has been discovered by an Indian physician, Dr. Hement Pathak. He found that the use of five drops of castor oil in a little juice or water taken on an empty stomach in the morning, is highly beneficial for allergies in the intestinal tract, skin and nasal passages. Dr. Pathak, has reported numerous cases of allergic protection by this method. For allergic conditions in which an element of stress is present, it is essential to employ such methods as relaxation, exercise, meditation and mind control. These methods will reduce or remove stress and thereby contribute towards the treatment of allergies. Yogic asanas like yogamudra, ardhmatsyendrasana, sarvangasana, shavasana and anuloma-viloma pranayama are also beneficial.

The human body has an Immune system that protects against attack against infection. Once HIV enters the body it attacks the immune system and lowers the effectiveness of the immune system. This increases vulnerability of the human body to infections. This leads to repeated attacks from various pathogens leading to frequent illnesses and finally death. In this way a healthy person after infection with HIV becomes prone to debilitating attacks from various infections resulting in death. Once a person is afflicted with HIV and as the disease progresses it leads to wasting away of the body and the person becomes a shadow of his or her healthy self and finally dies. ¾¾ What is HIV? The expanded version of the acronym is; H - Stands for human I = stands for immuno-deficiency V= stands for virus, which is a small germ The Human Immuno-deficiency Virus enters human body and attacks the immune system. This weakens the body to fight against any type of sickness. The infection occurs at different stages depending on how the person feels and the type of symptoms the prevail. ¾¾ Following are the four stages of HIV infection; ªª Acute HIV syndrome - This occurs when the person is first infected with the virus, which multiplies and spreads through the body. The person may not have any symptoms at all (about half the time), or may have flu-like symptoms such as fever, muscle or joint pains, swollen lymph glands and a rash. If these symptoms 24

occur, they usually last for a few weeks and then disappear. Window period: it takes between 6 weeks to 6 months (average 3 months) for the person with HIV to test positive through standard HIV diagnostic tests. During this time, infected persons have the virus in their body and can spread the infection but do not test positive. Asymptomatic stage: virus replicates in deep tissues such as testes and brain where it may remain without dividing for many months or years. It is those deep-seated reservoirs that appear to be responsible for the continued proliferation of the virus over many years. This is the stage of clinical latency, which might last for 3 months to 17 years depending on the immune status of individual patients. ªª Asymptomatic HIV infection (latent infection) – a few weeks after the initial infection, a person infected with HIV will feel perfectly well. If they have any flu like symptoms then these will go away and the person will enter the latent stage of HIV infection. This stage usually lasts for several years (from 2-10 years in most people). During this time nobody can tell that theicts about HIV and AIDS person is infected, not even a medical specialist, unless an HIV antibody test is performed. ªª Symptomatic HIV infection - During the symptomatic stage of HIV infection, even though the person looks and feels well, the virus is slowly destroying the immune system. Eventually the immune system cannot work properly and the person will start to get some mild infections or other symptoms. However, at this stage the symptoms due to HIV infection are still quite mild and definitely not life threatening. ªª AIDS - the end stage of infection due to HIV. ¾¾What is AIDS? AIDS describes the final result of HIV infection, or a damaged immune system that results in an impaired ability to fight off most infections. The term AIDS is a description of a syndrome associated with many opportunistic infections, which may occur in different parts of the body. ¾¾ How HIV transmits? There are four main ways an uninfected person can get HIV: ªª Through unprotected vaginal, anal or oral sexual intercourse with an infected person. ªª Through transfusion of untested (infected) blood like a blood or blood products transfusion ªª From an HIV infected mother to her baby before or during birth, or through breast milk. ªª Through sharing and using un-sterilized needles. ¾¾ For the virus to be transmitted two things must be present: ªª There must be an adequate quantity of the virus present to cause infection. This can occur only with blood, semen, vaginal and cervical fluids and breast milk. ªª There must be a suitable route like mucous membrane (thin skin) or broken skin for the virus to reach the particular cells for which it has an affinity. An HIV positive person can remain healthy for a long time, i.e., may have HIV in the body, but may not have any opportunistic infections. This person’s status would be HIV positive, not AIDS positive. It is important to understand the difference between HIV and AIDS as a person with HIV can stay without any serious sickness for a long time, but can transmit the virus to an uninfected person. ¾¾ Causes of spread of AIDS ªª Multiple Partners ªª Infected mother to her baby ªª Infected needles ªª Infected blood ªª Injectable Drug Use In India, sexual transmission is the most commc mode of transmission of HIV. More than seventy five percent of all transmission is sexual in India. Unprotected an and vaginal sex between men and men or men and wome (with one partner infected) can transmit the virus from tf infected person to the uninfected. This is especially likely 1 happen if either partner has another sexually transmitte infection such as syphilis or gonorrhoea. Anal sex is moi risky because it can result in anal tears. This makes it easit for HIV to enter. Women are socially vulnerable to HIV infection. The vulnerability of young women is the highest especially through penetrative sex, because their sexual organs are still not fully developed and are more prone to wounding and tearing. This does not mean that young women behave any differently from others. It just means that their young bodies are physically most at risk for the virus. Oral sex can also transmit the virus. (Oral sex is contact between the genitals of one partner and the mouth of the other partner. Deep kissing is not oral sex.) Unprotected oral sex should be avoided if there are sores present in the mouth or on the genitals. Oral sex on the penis is safer when a condom or dental dam is used. A dental dam is a small square

of fine condom rubber that can be placed in front of the mouth/tongue during oral sex to protect from direct contact with the genital area. Open sores on the vagina/penis or sexually transmitted infections (STI) also increase the possibility of HIV transmission. Infected blood can also spread the HIV virus. This can be from untested blood transfusion, or from an un-sterilized needle or any skin piercing instrument that is stained with blood. Therefore, care needs to be taken with needles for injections (by doctors or intravenous drug users), instruments used for tattooing and with razors. Make sure that these instruments are sterilized again before each new person uses them. This also applies to all surgical instruments where blood is involved. Open wounds should be kept covered. Mothers can transmit the virus to babies, before, during or after birth. In resource poor countries HIV positive woman can transmit HIV to their babies in one out of three cases. This transmission can happen in the womb, at birth, or from breast milk. HIV can be transmitted through the breast milk of an infected mother to her child, but to a limited extent. However, UNAIDS still recommends that all mothers is developing countries should breastfeed their babies. This is because the advantages of breast milk (increased imnmunity against diseases such as measles, polio and diarrhoea) outweigh the risk of HIV infection and exposure to infection from substitutes given by bottle. The current recommendations are exclusive breast-feeding for 3 to 6 months and then complete stoppage. Mixed feeding ( breast milk with artificial feeds and/or water) is not recommended. ¾¾ How is HIV not transmitted? ªª Vomit, faeces, sweat, tears and urine are all quite harmless, provided that they do not contain any blood. Thus cleaning or bathing a patient is qui safe, provided all open wounds are covered. Sharing toilets is also safe. ªª Kissing does not spread the virus, unless both pc actually have bleeding gums, than there may be small risk. ªª Mosquitoes, fleas and bedbugs cannot spread the virus either. The body system of the insects does not allow HIV to survive or to be transferred. ªª Sharing food does not spread the virus. ªª Shaking hands does not spread the virus. ªª Hugging carries no risk at all. Nor does sharing sheets, towels, clothes or eating utensils. ªª Swimming or bathing together carries no risk ¾¾ How can we prevent the transmission of HIV? If we are involved in sexual activities, the safest way of limiting the transmission of the virus is for all of us to adopt safer sexual practices, independent of whether we are negative or positive. This allows us both to protect and care for others, as well to protect and care for ourselves. ªª Total Abstinence. By not at all having sex with anyone, one can prevent acquiring STI or HIV infection. However one could acquire the infection through other modes of spread, more so, human beings are sexual beings and adoption of celibate life is not at all practical, as most people,

including those who want to have children or to get married, find this option inappropriate. However it has been proved extremely effective in slowing down the transmission of HIV among young men and women, especially if they choose to delay their sexual debut. Temporary abstinence. This can be a good solution for some people, especially for those who travel alone on work away from their usual partners for some time However; many people also find this a difficult option. Only having sex with one person who only ever has sex with you. Because of the length of the HIV Iincubation period, sexual history is always relevant. This does not give guaranteed protection, because in some cases, one partner in a couple may have been infected before they started being mutually faith without showing any symptoms. The virus can last 10 years or more in the body before making someone ill. Both partners may wish to get tested before beginning their monogamous relationship. Using condoms. This method is reliable only if condoms are used correctly and with every act sexual intercourse. Antibodies for HIV in a human body take approximately 6 to 12 weeks to develop after infection. That is why the test results would only show positive 6 weeks or longer after infection. This is called the window period. During these 6 weeks, while the test results show HIV negative, the

They are: ªª No person may be tested without his or her consent and it should be an informed consent. ªª Test results are confidential. ªª Pre and post-test counselling is an inclusive part of testing. An informed consent means that through pre-test counselling a person has been made aware, in a language s/he understands, of the possible consequences of the test. Informed consent would mean that the person has given consent to undergo the test after understanding the details of the test without any coercion. Confidential testing means that only the client and the health workers involved in the person’s direct care know that a test was performed and have access to the results. This information should not be given to other health providers, health authorities, employers, insurer, schools, family members or any other third party without the person’s very clear permission under any circumstances. Pre-test counselling is a confidential dialogue between a client and a trained health worker aimed at explaining the HIV test and the possible implications of knowing one’s HIV status. It is intended to lead to an informed decision to undergo or refuse to test. It is also intended to provide accurate information about HIV and AIDS with the aim of encouraging risk reduction behaviour and coping. Post-test counselling is a confidential dialogue between a client and a trained health worker aimed at explaining the HIV test result (HIV positive, HIV negative or HIV Indeterminate), providing appropriate education, support, referral, coping skills and encouraging risk reduction behaviour. A person can go for a blood test either voluntarily or on referral by a doctor/counsellor or through any other health care provider. An individual could voluntarily go for a HIV test when s/he believes himself/herself at risk, they participate in risk behaviours like unprotected sex, sharing needles, blood transfusion or they have a STL An individual could go to a voluntary counselling and testing centre also to get information on HIV. The earlier people go for a test the earlier they can access preventive and / or care services, thus prolonging their health life and most importantly

improve their quality of life. KSAPS is committed to contain the spread of HIV in India by building an all-encompassing response reaching out to diverse populations. It endeavours to provide people with accurate, complete and consistent information about HIV, promote use of condoms for protection, and emphasise treatment of sexually transmitted diseases. KSAPS works to motivate men and women for a responsible sexual behaviour. Karnataka State AIDS Prevention Society, established in 1999 to implement National AIDS Control Programs in the state. Since Karnataka state is one of the six high prevalence states in India, adult HIV prevalence in several districts exceeding 1 % prevalence rate since past 9 years. As a part of decentralization of different components of National AIDS Control Program (NACP) and KSAPS1 activities, District AIDS Control and Prevention Unit established. Currently 26 districts of Karnataka have functioning D APCUs. ¾¾ Introduction to Red Ribbon Club Background: In the context HIV prevention program in the country, mobilizing and organizing youth focused to prevent them from HIV infection is the need for the hour. While looking at the statistics pertaining to HIV infection among youth in the country, it estimated to be 35% of reported total HIV infected population in the country. However, limited data regarding female adolescent and HIV infection is available, it is reported in several studies that female adolescent are more vulnerable to HIV then their counter part. Thus it is imperative to organize this group including their counter part male adolescent to increase their knowledge on HIV prevention so that the HIV burden on the state could be reduced. Here are few statistics pertaining to HIV and youth. ªª Of the over 1 billion youth in the age group of 1524 years, worldwide, it is estimated that about 10 million are living with HIV. ªª It is reported that every day, about 6,000 youth estimated to be infected with the virus. ªª In India over 35% of AIDS cases reported are below 25 years of age ªª 50% of new infections occur among youth in the age group of 15 to 24 years. ªª According to the BSS report HIV/AIDS prevention, care & support programs in the country reach only 15% of young people.

Correct your sleeping posture so that you are in a comfortable position to sleep. Keep your bedding and pillows clean and dry so that you enjoy going to bed. Dust and mites may cause allergies which may make it difficult for you to sleep. Your mattress should be comfortable not too hard nor too soft so that your body gets the correct support. Avoid watching TV or use of computers before going to bed. Have a light dinner before going to sleep as a heavy dinner may cause indigestion and can hamper your sleep. Take a short leisurely walk after dinner but do not go for a vigorous exercise just before sleeping. Do not go to sleep in empty stomach. Too little food or no food may make you feel hungry and you may toss and turn throughout the night. If you are habituated to sleep in the dark turn all lights off before going to sleep. Draw all curtains and if required switch on a dim night lamp. Try to avoid tea and coffee just before going to sleep as these are stimulants and disallow sleep. Starchy foods such as rice, pasta and potatoes induce sleep and act as a sedative. Sweetened milk is a very good bedtime drink and

it encourages the sleep inducing hormones. Munch a tea spoon full of saunf - it is a sleep promoting herb. It is advisable to keep your worries out of your bed room so that you can sleep peacefully. If they keep on coming back take a paper and pen and write the cause of worry. A shower before bedtime in the hot climate gives you a good night’s sleep. If not possible to bath take a wet towel and soak it in cool water and sponge your body. The cool soothing effect will help you to sleep. Sprinkle some lavender water on your bed sheets and pillows. The scent promotes relaxation which leads to better sleep. If you find a hard time to sleep try doing breathing exercises. This will clear your brain and you can sleep better. Avoid alcohol although you may feel that it may induce sleep but it leads to poor quality of sleep and you wake up tired and irritated. Do not take nap during the day or in the evening as the tiredness is reduced and makes it difficult for you to sleep. Getting a cold can derail your nutrition and activity plans. However, if you follow the strategies laid out for you, you can prevent this from happening. Treatment : To treat a cold by means of customary suppressive drugs like aspirin and codeline only paves the way for future trouble of a more serious nature. For such a treatment puts a sudden stop to the eliminative process then taking place and forces the toxic matter back into the tissues again. Moreover, drugs have no effect on the duration of the cold. It has been aptly said that a cold can be cured in a week by taking medicines, otherwise it will subside in seven days. The only real treatment for colds is a proper diet. The best way to begin the treatment is to put the patient on a fast for two days. Nothing should be taken during this period except warm water mixed with lemon juice and honey or fruit juice and hot water. A liquid diet of fruit juice in large amounts is necessary to neutralise the acid condition of the blood and hot drinks are needed to help clear the kidneys. Pineapple juice in particular is highly beneficial. A warm water enema should be used daily to cleanse the bowels during this period. The short juice fast may be followed by an exclusive fresh fruit diet for three days. In this regimen, the patient should have three meals a day of fresh juicy fruits such as apples, pears, grapes, grapefruit, oranges, pineapple, peaches, melon or any other juicy fruit in season. Bananas, dried or stewed or tinned fruits, should not be taken. No other foodstuff should be added to the diet as otherwise the whole value of the treatment is lost. After the exclusive fruit diet, the patient should gradually embark upon a well – balanced diet of three basic food groups, namely ÂÂ seeds, nuts and grains ÂÂ vegetables and ÂÂ fruits. It is advisable to avoid meat, fish, eggs, cheese and starchy foods for a few days. The patient should strengthen the system as a whole by taking a diet which supplies all the vitamins and minerals the body needs. Vitamin C, however, heads the list of these nutrients. It protects one against infection and acts as a harmless antibiotics. It is found in citrus fruits, green leafy vegetables, sprouted Bengal and green grams. The regular use of this vitamin in the optimum daily amount will prevent the common cold and if a cold has already appeared, large doses of this vitamin will relieve the symptoms and shorten its duration. Lime is the most important among the many home remedies for common cold. It is highly beneficial in all types of cold and fevers. It should be taken well diluted. Vitamin C – rich lime juice increases resistance, decreases toxicity and reduces the duration of the illness. Lime juice should be diluted in a glass of warm water, and a teaspoonful of honey should be added to it. It forms an ideal remedy for a cold and dry cough. Garlic soup is an ancient remedy to reduce

the severity of cold. Garlic contains antiseptic and antispasmodic properties besides several other medicinal virtues. The volatile oil in garlic flushes out the system of all toxins and thus helps bring down fever. Garlic oil combined with onion juice, diluted with water and drunk several times a day, has also been found in several studies to be extremely effective in the treatment of the common cold. Ginger is also an excellent food remedy for colds and coughs. Ginger should be cut into small pieces and boiled in a cup of water ; it should then be strained and half a teaspoon of sugar added to it. It should be drunk while it is still hot, in case of colds. Ginger tea, prepared by adding a few pieces of ginger into boiled water before adding tea leaves, is also an effective remedy for colds and for fevers resulting from cold. Turmeric, with its antiseptic properties, is an effective remedy for cold and throat irritations. Half a teaspoonful of fresh turmeric powder mixed in 30 grams of warm milk is a useful prescription for these conditions. Turmeric powder should be put into a hot ladle. Milk should then be poured in it and boiled over a slow fire. In case of a running cold, smoke from the burning turmeric should be inhaled. It will increase the discharge from the nose and will bring quicker relief. Water Treatment: A hot water bath, if it can be taken without undue exposure, is recommended as it helps relieve much of the congestion in the chest and nasal membranes. Hot packs or fomentations are excellent for treating chest and head colds. Steam bath, hot foot bath and hot hip bath are also beneficial as they stimulate perspiration. Steam inhalation will help relieve the congestion of the nasal tissues. Gargling with hot water mixed with salt is beneficial for a sore throat. Cold chest packs should be applied two or three times a day as they will relieve congestion of lungs and help in eliminating the accumulated mucus. Other useful measures in the treatment of common cold are mild sunbath, fresh air and deep breathing, brisk walks, sound sleep, adjustment of one’s clothes and habits to the requirements of the season, so as to nullify the effect of weather fluctuations. Yogasanas like bhujangasana, shalabhasana, dhanurasana, and yogamudra in vajrasana, yogic kriyas such as jalneti and vaman dhouti and pranayamas such as kapalbhati, anuloma – viloma and suryabhedana are beneficial in the treatment of the common cold.

HIV RESPONSE IN KARNATAKA

arnataka is one of four large states in South India with a relatively advanced HIV epidemic. As a part of the National AIDS Control Program (NACP)of the National AIDS Control Organization (NACO), Karnataka has been conducting the HIV Sentinel Surveillance since 1998 with the provisional prevalence for 2010 being 0.69% among general population. This is close to the NFHS-3 estimate of 0.69%. There is significant decline in adult HIV prevalence in the state - from 1.5% in 2003 and 2004 to less than 1% (0.69%) in 2010. Further, declining prevalence among women in the age group 15-24 probably indicates halting of new infections. More importantly, HIV positivity among the antenatal clinic (ANC) attendees has declined to 0.24% and among general clients to 4% as evidenced by data from in the Integrated Counseling and Testing Centers (ICTCs). However, HIV prevalence is 10 or more times higher among various high risk groups. Based on Surveillance data, given above, estimation of PLHIV in the state is 2.45 lakhs. Rapid scale-up of Integrated Counseling and Testing Centers (IUCs) has been undertaken during the past 3 years and Karnataka currently has a total of 1525 ICTC facilities spread across the State. The total general clients who have been tested in the above mentioned IUCs (from April to October 2012) has been 6, 54,464 and out of which 20,642 people have been found positive (3.20% positivity rate). Total pregnant women tested for HIV (April - October 2012) is 4, 95,305 and out of which 1092 were found positive (0.22 positivity rate). Every district of Karnataka has at least one Anti Retroviral Therapy (ART) centre. As of now there are 49 ART centers and 167 Link ART Centers functioning in the state. The cumulative number of HIV cases registered at ART Centers is 2, 32,761, out of which 85,605 are on Anti retroviral treatment. Out of this 39,950 are male, 39,888 are female and. 149 are transgender. Total children who are on ART are 5,618 (3214 male and 2404

female). By the end of 2012-13 the plan is to have 54,ART centers and 184 Link ART centers. There are ‘33 CD4 machines in the state at present with 8 more proposed. Community Care Centers (CCCs), which primarily focuses on ART adherence and psychosocial support of PLHIVs and their families, is implemented through funding from Global Fund Round six. Presently 27 CCCs are functioning. In addition, there are 30 Drop In Centers (DICs), funded by KSAPS, for the benefit of People Living with HIV (PLHIV).

Prevention response from Karnataka State AIDS Prevention Society (KSAPS) has been to saturate the coverage of populations who are most at risk of acquiring and transmitting HIV. This saturation currently covers around 79169 FSWs, 25212 MSM and 1292 Transgender population through Targeted Interventions. KSAPS also covers 1597 IDUS in the city of Bangalore and Kolar along with 190,000 migrants and 60,000 long distance male truckers through targeted Interventions. Karnataka has 12 districts under Link workers scheme which essentially focuses on HIV prevention and care and support in the rural areas. In Karnataka the availability of safe Blood is ensured through a network of 171 Blood Banks. Out of 171 Blood Banks, 40 are in Government Sectors, 23 are Voluntary/ Charitable sectors and 108 are in Private sector. In addition, there are 39 BCSUs (Blood Component Separation Units) in the state. The state also has 9 Blood Transportation vans, which provide the logistic support. The total collection of blood units during the year 2012 - 2013 (April-Oct) has been 3, 94,583 units with 65% as voluntary blood collection for the state. The State will continue to maintain a high

proportion of voluntary blood collection with a target to achieve voluntary blood collection as 100% by the end of the year 2012-13. The State expects to collect around 6, 00,000 units of blood and is committed to organize 4000 voluntary blood donation camps during the year 2012-13. KSAPS undertakes various Mass and Mid media campaigns through radio and television programs, by setting up of hoardings and bus and auto rickshaw panels, through folk troupe and door to door campaigning at high and vulnerable HIV prevalent taluks in the state. 732 Red Ribbon Clubs have been set up at colleges and there plans to set up around 1000 Red Ribbon Clubs more by the end of this year. During the year 2011-12, KSAPS has launched 41 legal AJDS clinics in collaboration with Karnataka State Legal Services Authority (KSLSA) in all Government supported ART centers across Karnataka. The Care Coordinators from the PLHIV DICs, Community Care Centers, and ART centers will form the contact points for the affected PLHIVs who will then forward these cases to the LSCs. KSAPS is planning to forge a partnership with CREDAI to ensure that the construction workers in construction sites, who are mostly migrants and at risk of getting and spreading HIV infection, are exposed to HIV messages and information on a regular basis. Yoga for Hypertension Hypertension is the term used to describe high blood pressure. Blood pressure is a measurement of the force against the walls of your arteries as your heart pumps blood through your body. Blood pressure readings are usually given as two numbers -- for example, 120 over 80 (written as 120/80 mmHg). One or both of these numbers can be too high. The top number is called the systolic blood pressure, and the bottom number is called the diastolic blood pressure. ªª Normal blood pressure is when your blood pressure is lower than 120/80 mmHg most of the time. ªª High blood pressure (hypertension) is when your blood pressure is 140/90 mmHg or above most of the time. ªª If your blood pressure numbers are 120/80 or higher, but below 140/90, it is called prehypertension. If you have pre-hypertension, you are more likely to develop high blood pressure. If you have heart or kidney problems, or if you had a stroke, your doctor may want your blood pressure to be even lower than that of people who do not have these conditions. ¾¾ Causes, incidence, and risk factors Many factors can affect blood pressure, including: ªª How much water and salt you have in your body ªª The condition of your kidneys, nervous system, or blood vessels ªª The levels of different body hormones You are more likely to be told your blood pressure is too high as you get older. Practice them daily for several weeks and notice the changes. Learn some basic stretches for the shoulders in this section. ¾¾ Stand Spread Leg Forward Fold Practicing the Standing Spread Leg Forward Fold can strengthen and stretch your inner and back legs and your spine. People with lower back problems should avoid doing the full forward bend. For beginners, you may use props like a folding chair to support your forearms. ¾¾ Cat Pose (Bidalasana) The Cat Yoga Pose teaches you to initiate movement from your center and to coordinate your movement and breath. These are two of the most important themes in Yoga practice. Keep in mind that the Cat Pose may not be advisable if you have any chronic or recent back pain or injury. ¾¾ Half Spinal Twist (Ardha Matsyendrasana) If done properly, the Half Spinal Twist lengthens and strengthens the spine. It is also beneficial for your liver, kidneys, as well as adrenal glands. Practice this Yoga Pose under the supervision of a Yoga instructor. In this section, learn how to perform the Half Spinal Twist. ¾¾ Wind Relieving Pose (Pavanamuktasana) The term Pavanamuktasana comes from the Sanskrit word ‘pavana’ which means air or wind and ‘mukta’ which means freedom or release. The Wind Relieving Pose works mainly on the digestive system. specifically, it helps in eliminating excess gas in the stomach. ¾¾ Double Leg Raises A Double Leg Raise is similar to a Single Leg Raise, only this time, you will raise both legs. In doing this Yoga Pose, make sure that the full length of your back is resting on the floor and your shoulders and neck are relaxed. On the Eve of World Prematurity Day, Bangalore hospital bags national award for premature infant blindness prevention Bangalore based Narayana Nethralaya Postgraduate Institute of Ophthalmology’s flagship Tele-Ophthalmology program,KIDROP (Karnataka Internet Assisted Diagnosis of Retinopathy of Prematurity) was awarded the prestigious “Innovation in Healthcare through PPP”Public Choice award at the e-India awards at Hyderabad on November 15th 2012. KIDROP was initiated in 2007 by Narayana Nethralaya and became the first PPPin the country to combat infant blindness in collaboration with the National Rural Health Mission, Govt. of Karnataka since 2009. With over 18 districts covered, and a state-wide expansion plan in progress, over 26,000 infant imaging sessions and over 800 vision saving treatments have been performed in premature infants, a few weeks old in rural outreach centres at no cost to the patient. KIDROP is now the worlds largest single centre managed TeleROP network. Dr. Bhujang Shetty, Chairman of Narayana Nethralaya remarked, ‘We feel privileged that our efforts in collaboration with the Government has got recognition”. Dr. Anand Vinekar, KIDROP Program Director, who received the award on behalf of the team said, “This PPP award has provided the right

impetus for us to expand this model to other states of our country to benefit thousands of babies born in our rural hospitals who risk blindness. The award incidentally coincides with the World Prematurity Day celebrated world over on November 17th. One in 10 babies born in the world are said to be premature and risk of ROP” ¾¾ What the award Citation read: “KIDROP is India’s first and now the world’s

largest tele-medicine network for tackling infant blindness. This indigenously developed customized tele-ophthalmology platform, is focused on preventing blindness from Retinopathy of Prematurity (ROP), which is the leading cause of infant blindness worldwide. The project seeks to create a network using tele-medicine to prevent, screen and treat infant blindness in rural infants who lack access to care with a focus on Retinopathy of Prematurity” This unique model has been studied by the IIM-A, and cited by Harvard Business Review and has won several service and innovation awards.

Bengalureans set Guinness record on the occasion of World Diabetes Day... Novo Nordisk, the global leader in Diabetes care achieves feat for record largest ‘Health Awareness’ program

t was a proud moment for Bengalureans! Commemorating World Diabetes Day, the citizens of Bengaluru participated with enthusiasm towards creating the largest ‘Health Awareness’ programme at the Palace Grounds on Sunday. Over 2000 people participated in this mammoth exercise organized by Novo Nordisk, the world leader in Diabetes care and thereby setting a new Guinness record for India. Mr. Melvin D’Souza, Managing Trustee, Novo Nordisk Education Foundation and MD, Novo Nordisk India received the certificate from the officials of Guinness Book of Records, who were also present at the occasion to monitor and evaluate the event. The event included a massive diabetes screening programme and other educational activities. Inaugurating the event, former Lokayukta Chief Santosh Hegde said, “I have been a Diabetic from the last 35 years managing it successfully nevertheless. A healthy lifestyle and regular check-ups are key to managing Diabetes. I am happy to be associated with such a cause and I believe events such as this are essential to not only educate people about diabetes but to also build large scale awareness among the public on basic and other critical aspects of Diabetes.” Speaking on the occasion, Mr D’souza said, “It is indeed a proud moment for Bengaluru and Indians at large. This achievement is further testimony to our efforts towards creating large scale awareness campaigns about Diabetes and related issues. It is however pertinent to note that this is only the tip of the iceberg and much more needs to be done in our journey towards improved healthcare and combating Diabetes. We believe initiatives such as these will help capture the imagination of the larger populace, thereby marching steadfastly towards a Diabetes free nation.” “In line with this thought we have also taken several initiatives at the State and National level. Be it impacting children with Diabetes through our ‘Changing Diabetes in Children’ (CDiC) programme or partnering with the State governments by launching and implementing the Changing Diabetes Barometer project, our endeavor has been to bring healthcare to the doorsteps of people and make healthcare accessible to all,” he added. However awareness about diabetes is poor. There is an urgent need for people to realize how diabetes can impact not just the life of the person with diabetes but also family members of the person, the community and the country at large. “Access to appropriate medication, quality of care and proper medical advice are critical to leading a healthy and active life and reduce the risk of developing complications in Diabetes. I would also like to advice people to go for regular check-ups and take necessary precautions to avoid any healthcare issues including Diabetes. Early detection of complications is an essential part of good diabetes care.
