aricose veins are gnarled, enlarged veins. The word “varicose” comes from the Latin root “varix,” which means “twisted.” Any vein may become varicose, but the veins most commonly affected are those in your legs and feet. That’s because standing and walking upright increases the pressure in the veins in your lower body. For many people, varicose veins and spider veins — a common, mild and medically insignificant variation of varicose veins — are simply a cosmetic concern. For other people, varicose veins can cause aching pain and discomfort. Sometimes the condition leads to more serious problems. Varicose veins may also signal a higher risk of other disorders of the circulatory system. Varicose veins are a common condition in the India, affecting up to 15 percent of men and up to 25 percent of women. Treatment may involve self-help measures or procedures by your doctor to close or remove veins.
Varicose veins can cause aching pain and discomfort. Sometimes the condition leads to more serious problems. Varicose veins may also signal a higher risk of other disorders of the circulatory system.
Signs and symptoms
Some people with varicose veins don’t experience any discomfort from the condition. When painful signs and symptoms do occur, they may include: ªª An achy or heavy feeling in your legs, and burning, throbbing, muscle cramping and swelling in your lower legs. Prolonged sitting or standing tends to make your legs feel worse. ªª Itching around one or more of your veins. ªª Skin ulcers near your ankle, which represent a severe form of vascular disease and require immediate attention. Varicose veins are dark purple or blue in color and may appear twisted and bulging — like cords. They commonly appear on the backs of the calves or on the

inside of the leg. However, they can form anywhere on your legs, from your groin to your ankle. Spider veins are similar to varicose veins, but they’re smaller. Spider veins are found closer to the skin’s surface and are often red or blue. They occur on the legs, but can also be found on the face. Spider veins vary in size and often look like a spider’s web or a tree branch. Causes: Arteries carry blood from your heart to the rest of your tissues. Veins return blood from the rest of your body to your heart, so the blood can be recirculated. To return blood to your heart, the veins in your legs must work against gravity. Muscle contractions in your lower legs act as pumps, while toned, elastic vein walls help blood return to your heart. Tiny one-way valves in your veins open as blood flows toward your heart then close to stop blood from flowing backward. As you get older your veins can lose elasticity, causing them to stretch. The valves in your veins may become weak, allowing blood that should be moving toward your heart to flow backward. Blood pools in your veins, and your veins enlarge and become varicose. The veins appear blue because they contain deoxygenated blood, which is in the process of being Pg 6.... recirculated.

If one were to avoid cholesterol all through one’s life and eat a diet very low in fat, he/she could hope to live for an extra three days to three months only! he experimental study revealed that the “lecithin” in egg reduces the fat. But a minor chemical in the egg, lecithin, has the capacity to lower cholesterol in the laboratory. There is no denying the fact that egg is an excellent food when wholesome; as it is the beginning of life and naturally has all the ingredients needed to create life. Eating an egg a day might not harm anyone, but to say that it helps human health in this novel way is a bit far fetched. We do not get wholesome eggs at all these days in the market. With our greed we want the birds to come to the table from the egg stage in less than three weeks. To that end lots of hormones are used in growing the chicks. To cap it we give them lots of antibiotics to keep them away from many infections that kill them in their artificial atmosphere. So both the bird on your plate and its egg would be having a good dose of the hormone (growth hormone) and antibiotics. Each egg contains nearly 210 mg of cholesterol in its yellow. Only ten per cent of the body’s total cholesterol pool comes from our diet. Ninety per cent of the cholesterol is made in our liver; (endogenous) based on the genes that we have inherited from our ancestors. We could only change that part of our cholesterol for our good by changing our parents, if that is scientifically possible and, not by any other means. Diet, egg or no egg, only contributes to ten per cent of our blood cholesterol. Eating even grass would only lower that cholesterol by ten per cent. It really does not matter what you eat as long as you keep your fat within 25% of your total calorie intake. Eat an egg every day, if that is part of the total fats that you consume, do it with relish, and enjoy good health provided the egg is wholesome. Do not, however, add an egg daily to your diet, if you are already taking your share of fat from other sources, in addition. The best way to lower your body’s cholesterol, other than diet restrictions, is to have daily physical exercise. Please do not forget that cholesterol is very important for total health. Every single cell of the human body, of which there are around one hundred thousand billion in all, has its wall made up of cholesterol. This wall has to remain intact to prevent ageing, cancer, and all other dangerous diseases, brought on by oxidants-contents of the cell’s interior, which should not spill into the surrounding area of the cell. It is not what you eat that kills you, but it is what eats you (negative thoughts) that kill you. Enjoy eating an egg happily, if you must, but do not eat other fats in addition to keep your fat intake within limits. But do not worry about what you eat, on the dining table. Your mind must be absolutely happy while eating. “Prasanna aathma, indriya, manaha, swastha ithyabhideeyathe.” Happiness, universal love and altruism keep one healthy. Some pregnant women develop varicose veins. Pregnancy increases the volume of blood in your body, but decreases the flow of blood from your legs to your pelvis. This circulatory change is designed to support the growing fetus, but it can produce an unfortunate side effect — enlarged veins in your legs. Varicose veins may surface for the first time or may worsen during late pregnancy, when your uterus exerts greater pressure on the veins in your legs. Hemorrhoids are varicose veins located in and around the anus.
ªª Watch your weight, and your diet. Shedding excess pounds takes unnecessary pressure off your veins. What you eat makes a difference, too. Follow a low-salt, high-fiber diet to prevent the swelling that may result from water retention and constipation. ªª Watch what you wear. Avoid high heels. Lowheeled shoes work calf muscles more, which is better for your veins. Don’t wear tight clothes around your waist, legs or groin. Tight panty-leg girdles, for instance, can restrict circulation. ªª Elevate your legs. To improve venous circulation, take several short breaks daily to elevate your legs above the level of your heart. For example, lie down with your legs resting on three or four pillows. ªª Avoid long periods of sitting or standing. Make a point of changing your position frequently to encourage blood flow. Try to move around at least every 30 minutes. ªª Don’t sit with your legs crossed. This position can aggravate circulation problems.
Risk factors These factors increase your risk of developing varicose veins: ªª Age. Aging causes wear and tear on the valves in your veins that help regulate blood flow. Eventually, that wear causes the valves to malfunction. ªª Sex. Women are more likely than men are to develop the condition. Hormonal changes during pregnancy, premenstruation or menopause may be a factor. Female hormones tend to relax vein walls. Taking hormone replacement therapy or birth control pills may increase your risk of varicose veins. ªª Genetics. If other family members had varicose veins, there’s a greater chance you will too. ªª Obesity. Being overweight puts added pressure on your veins. ªª Standing for long periods of time. Your blood doesn’t flow as well if you’re in the same position for long periods.
Treatment
Fortunately, treatment usually doesn’t mean a hospital stay or a long, uncomfortable recovery. Selfhelp measures — such as exercising, losing weight, not wearing tight clothes, elevating your legs and avoiding long periods of standing or sitting — can ease pain and prevent varicose veins from getting worse. Varicose veins that develop during pregnancy generally improve without medical treatment within three months after delivery. If you don’t respond to self-help or if your condition is more severe, your doctor may advise one of these varicose vein treatments: Sclerotherapy, Laser surgeries, Catheter-assisted procedures, Vein stripping, Ambulatory phlebectomy and Endoscopic vein surgery Prevention ªª There’s no way to completely prevent varicose veins. But improving your circulation and muscle tone can reduce your risk of developing varicose veins or getting additional ones. ªª Exercise. Get your legs moving. Walking is a great way to encourage blood circulation in your

When this happens it is called endometriosis. Endometriosis can cause pain before and during the menstrual period. For some women, the pain is mild. For others, it can be severe. Severe endometriosis also may lead to infertility.

This article will explain: The symptoms of endometriosis How it is diagnosed. How it may be treated
that grows in the ovaries may cause a cyst {also known as an endometrioma) to form. Endometrial tissue outside the uterus responds to changes in hormones. It also breaks down and bleeds like the lining of the uterus during the menstrual cycle. This bleeding can cause pain, especially before and during your period. The breakdown and bleeding of this tissue each month also can cause scar tissue, called adhesions. Sometimes adhesions bind organs together. Adhesions also can cause pain. The symptoms of endometriosis often worsen over time. In many cases, treatment may help keep the condition from getting worse.
The Endotnetrium
The lining of the uterus responds to changes that take place during a woman’s monthly menstrual cycle. The cycle often is about 28 days. First, the endometrium grows and thickens to prepare for a possible pregnancy. If pregnancy does not occur, the endometrium then thins and is shed by bleeding. These changes are triggered by hormones (estrogen and progesterone) made by the ovaries.

What Is Endometriosis?

With endometriosis, tissue like the endometrium is found in other areas in the body. It looks and acts like tissue in the uterus. It most often appears in places within the pelvis: Ovaries, Fallopian tubes, Surface of the uterus, Culde-sac (the space behind the uterus). Bowel, Bladder and ureters, Rectum Endometrial tissue may attach to organs in the pelvis or to the peritoneum. It also may be found in other parts of the body. This is very rare, though. Who Is at Risk?
Endometriosis is most common in women in their 30s and 40s, but it can occur any time in women who menstruate. Endometriosis occurs more often in women who have never had children. Women with a mother, sister, or daughter who have had endometriosis also are more likely to have it. Endometriosis is found in about three quarters of the women who have chronic pelvic pain.

You will be given pain relief for these procedures, Endometriosis also can be treated during a laparoscopy. If endometrial tissue is found during the laparoscopy, your doctor may decide to remove it right away.

The main symptom of endometriosis is pelvic pain. Such pain may occur with sex, during bowel movements or urination, or just before or during your menstrual cycle. Menstrual bleeding may occur more than once a month. Severe endometriosis also may cause infertility. Although these symptoms may be a sign of endometriosis, they could also be signs of other problems. If you have any of these symptoms, see your doctor. No one is certain of the cause of endometriosis. For most women, a small amount of blood and cells flow through the fallopian tubes into the abdomen during their period. For women with endometriosis, the cells in the blood that flows through the tubes attach to other places and grow. Endometrial cells also may be carried through blood and lymph vessels. The amount of pain does not always tell you how severe your condition is. Some women with slight pain may have a severe case. Others who have a lot of pain may have a mild case. Many women with endometriosis have no symptoms. In fact, they may first find out that they have endometriosis if they are not able to get pregnant. Endometriosis is found in about one third of infertile women. Women often find that symptoms are relieved while they are pregnant In fact, many of the drugs used to relieve symptoms of endometriosis are based on the effects of hormones produced during pregnancy.

Medications
In some cases of endometriosis, medications or NSAIDs (nonsteroidal antiinflammatory drugs) may be used to relieve pain. These drugs will not treat any other symptoms of endometriosis. Hormones also may be used to relieve pain. The hormones also may help slow the growth of the endometrial tissue and may prevent the growth of new adhesions. It will not make them go away, though. Hormone treatment is designed to stop the ovaries from releasing hormones. The hormones most often prescribed include: ªª Oral contraceptives ªª Gonadotropin-releasing hormone (GnRH) medicines ªª Progestin ªª Danazol These medications are not for all women. As with most medications, there are some side effects linked to hormone treatment. Some women may find the relief of pain is worth the side effects. These medications do not relieve pain in all women. Oral contraceptives: Birth control pills often are prescribed to treat symptoms of endometriosis. The hormone in them helps keep the menstrual period regular, lighter, and shorter and can relieve pain. Your doctor may prescribe the pill in a way that prevents you from having periods. Gonadotropin-releasing hormone. GnRH is a hormone that helps control the menstrual cycle. GnRH agonists are drugs that are much like human GnRH but many times stronger than the natural substance. They lower estrogen levels by turning off the ovaries. This causes a short-term condition that is much like menopause. GnRH can be given as a shot, an implant, or nasal
Diagnosis
If you have symptoms of endometriosis, your doctor may do a physical exam, including a pelvic exam. If other causes of pelvic pain can be ruled out, your doctor may treat endometriosis without doing any further exams or surgery. Endometriosis can be mild, moderate, or severe. The extent of the disease can be confirmed by looking directly inside the body. This can be done by

Treatment
Treatment for endometriosis depends on the extent of the disease, your symptoms, and whether you want to have children. It may be treated with medication, surgery, or both. Although treatments may relieve pain and infertility for a time, symptoms may come back after treatment.

Surgery most often is done by laparoscopy. During laparoscopy, endometriosis can be removed or burned away. Not all cases can be handled with laparoscopy. Sometimes a procedure called laparotomy may be needed. Discuss with your doctor which method may be best for you. After surgery, you may have relief from pain. Symptoms may return, though. Many patients are treated with both surgery and medications to help extend the symptom-free period. Symptoms return within 1 year in about half of women who have had surgery. The more severe the disease, the more likely it is to return. If pain is severe and doesn’t go away after treatment, a hysterectomy (surgery to remove your uterus) may be an option. Endometriosis is less likely to come back if your ovaries also are removed. After this procedure, a woman will no longer have periods or be able to get pregnant. There is a small chance that your symptoms will come back even if your uterus and ovaries are removed.

Side effects in women taking this medication may include: ªª Hot flushes ªª Headaches ªª Vaginal dryness ªª Thin bones Treatment with GnRH most often lasts at least 3 months. To help reduce the amount of bone loss from long-term use, your doctor may prescribe certain hormones or medications to take along with GnRH agonists. In many cases, this therapy also may reduce other side effects. After stopping GnRH treatment, you will have periods again in about 6-10 weeks. Progestin: The hormone progestin also can be used to shrink endometriosis. Progestin works against the effects of estrogen on the tissue. Although you will no longer have a monthly menstrual period when taking progestin, you may have irregular vaginal bleeding. Progestin is taken as a pill or injection. Side effects in women taking this medication may include: ªª Mood changes, Weight changes, Bloating, Sexual problems Danazol. Danazol is another type of hormone that shrinks endometrial tissue. It lowers levels of estrogen and progesterone. It is taken as a pill for at least 6 months. You will no longer have a menstrual period while taking danazol. The side effects of danazol may include: ªªWeightgain, Acne, Deepening of the voice, Hirsutism

Coping
Endometriosis is a long-term condition. Many women have symptoms that occur off and on until menopause. Keep in mind that there are treatment options. A woman can work with her doctor in making the right decision for her. It also may help to talk with other women who are coping with endometriosis. Ask your doctor or nurse to suggest a support group in your area.

Finally
Endometriosis can cause pain and infertility. It often can be treated with success. You may need more than one kind of treatment. If you have any symptoms of endometriosis, see your doctor.
Surgery
Surgery may be done to remove endometriosis and the scarred tissue around it. In most severe cases of endometriosis, surgery often is the best choice for treatment. Healthy ovaries and normal fallopian tubes are left alone as often as possible.

Loss of nutrients is low when the leaves are used in the form of salad. This is used in coloring of jelly and other products and the red dye is used in postal seal in China. Being a climber, Indian spinach needs support. Green and fleshy leaves are borne alternatively on fleshy light green climbing stem. Leaf margin is even. Medicinal properties are found to be similar in the red and green spinach types. Botanically known as Basella rubra and Basella alba are red and white varieties respectively. Mix with lemon juice. ªª Vegetable Soup: Grated coconut 1 cup , Ginger a small piece, Garlic 4 flakes, Cinnamon a small piece, Cumin seeds Vi spoon, Chilly powder 1 spoon, Coriander 2 spoons -- Grind all the above ingredients. Cut spinach, potatoes, beans, carrot, onion and tomatoes to small pieces. Cook them with a little salt. Add to this the above ground paste. Boil the same for a few minutes ªª Spinach Dosa: Soak rice in water for 1 hour. Grind the wet rice along with cut spinach. Add salt. Make dosa on hot tawa.

ªª Spinach Sambar: This can be prepared with cut pieces of stem and leaves of spinach. All the ingredients used in the preparation of soup can be used in making dosa.. ªª Spinach Kootu: Coriander seeds 25 gms, Blackgram 1 spoon, Fenugreek 14 spoon, Cumin seeds Yi spoon, Red chillies 4-5, Pepper Yi spoon. ---Roast all the ingredients separately and pound them into fine powder. Cook turdhal. Add cut spinach, tamarind juice, jaggery and salt. Boil. Mix the above prepared powder. Garlic, asafoetida and mustard seasoning is optional. ªª Idli: Instead of usual blackgram dhal used in the preparation of Idli, the spinach along with rice can be used. You can make soft, green idlis in this method. ªª Spinach & Blackgram Curry: Soak blackgram dhal for 2 hours. Grind it coarsely. Cut spinach. Season with mustard, green chillies, curry leaves, turmeric and then add cut spinach and ground blackgram. Roast till it is golden brown. Add cumin seeds, coriander, salt and chilli powder.

Well, the patient approaches the doctor with hope of complete cure, so when a progressive N-M disease is diagnosed it does not have complete cure. The treating team has to face the problem of explaining the condition to the patient.

DUCHENNE MUSCULAR DYSTROPHY
Duchenne muscular dystrophy is the most common muscular dystrophy found in the group. A specific treatment for the disease is not available, which makes it much complicated in aspects of physical, emotional, social and educational means. The studies on DMD patients suggest that, 62% of the parent’s problem were ‘practical’ related to lifting, bathing, toileting, dressing and finding suitable accommodation .23% were having problems related to ‘service ‘ i.e , medical and rehabilitation care. 15% were having ‘emotional’ problem, i.e parents were helplessly seeing the deterioration of the condition. Once the condition is being explained, the rehabilitation plan starts I. PASSIVE STRETCHING EXERCISES The clinical features begin at the age of 6-8 yrs, with the child falling off quiet often and difficulty in climbing steps. By the age of 9-11 yrs, climbing of stairs is completely seized and the child can no longer be able to stand from the floor. This happens because of ªª Muscular weakness ªª Contractures in hip, knee and ankle Passive stretching of hip flexors and lying on stomach are best exercises to maintain muscles functional strength. Stretching of the muscle groups for 5 secs, repeat it 10 times / twice /day. Night splint for foot are more effective in contracture cases. II. ACTIVE EXERCISES

Mascular dystrophy rehabilitation is a very useful for the medical teams as well as family members of the patient. It helps to create awareness regarding the disease.
NEURO MUSCULAR DISORDER REHABILITATION (DUCHENNE MUSCULAR DYSTROPHY) Maximum resistance exercises gives improvement for beginning 4 months, then a plateau occurs and later there won’t be much improvement found. Active exercises aiming functional capacity improvement are much useful than only resistance exercises. Active movements should be started at the earliest. Swimming and intensive training in Yoga are much more beneficial. III. RESPIRATORY MUSCLE TRAINING 75% of deaths in DMD are due to respiratory failure, uncontrolled studies showed improvement in vital capacity after breathing exercises. 6 weeks of the respiratory exercises training is required to find the improvement. Early starting of respiratory training is more beneficial. However over training may deteriorate the condition. Long standing respiratory muscle weakness may lead to scoliosis, thoracic mechanical abnormalities and reduced lung compliance, weak cough and recurrent sleep related hypoxia. IV. VENTILATION SUPPORT Chronic respiratory failure with sleep hypoxia may lead to anorexia, weight loss, apathy, depression, morning headache, insomnia, hypoxia can lead to muscle weakness. Positive pressure ventilation is useful in sleep hypoxia. Accepting or rejecting assisted ventilation is a decision to be made by patient and family. V. ORTHOSIS Functional weakness is that of quadriceps muscles. In early days children falls because of hurrying and careless about balance. Early use of braces may slow down the patient

llergic Rhinitis is one of the commonest problems encountered in day to day practice in ENT. Though the disease itself is not life threatening, it leads to lot of troublesome symptoms for the patient, that the disease can’t be lured, makes the situation worse and after many a time it is associated with Bronchial Asthma. Allergic Rhinitis is a disease characterized by episodic watery discharge from the nose Sneezing, Itching & obstruction in the nose. It is also associated with itching in the eyes, ears, throat & watering of eyes. Patient will be absolutely symptomless in between the episodic attacks.
¾¾ Allergic Rhinitis is an IgE mediated response of nasal cavity to air borne allergens. There are two types:
1. Seasonal: Present only during particular season. 2. Perennial: Symptoms will be present throughout the year. Allergens (substances which trigger the symptoms) which precipitate the episodic attacks vary from house dust to pollen from the trees & flowers & include wide variety of substances. Each individual is unique, in the sense, he will be sensitive to certain specific allergens which others may not be sensitive to. And also there is some familial predisposition also, meaning if the parents or siblings are suffering from allergic Rhinitis, the chances of patient suffering from Allergic Rhinitis would be more. Allergic Rhinitis can be present in any age or sex. The symptoms include episodic sneezing, watery, nasal discharge, itching in the nose & nasal Obstruction. On examination, patient will have swollen mucosa & bluish tinge of inferior turbinates. It could be associated with redness of eyes, swollen eyes & black circles around the eyes. Diagnosis is based on symptoms, examination & Investigations. Investigations include Absolute eosinophillic Count, Nasal smear, Skin test & provocation test. Management of the patient is of paramount importance than management of the disease itself as the disease is not completely curable & is present throughout, with remissions in between. Patient often is depressed as the symptoms are quite troublesome & shows only temporary cessation on drug intake only to recur after sometime. Patients keep changing the drugs, the doctor & many a time the system of medicine itself in pursuit

of complete cure of symptoms. Hence management includes treating the patient along with, impressing the patient of the nature of disease he is suffering from. Once the patient is convinced of the episodic nature of disease, the compliance of patient would improve.

acute attack along with decongestants, which can be used as tablets or as local drops. The disadvantages of Antihistamines are that they cause sedation. Newer generation Antihistamines are supposed to be less sedative but again, patients feeling of sedation is highly subjective. Decongestants when used for long duration can cause rebound nasal obstruction & hence patients should be discouraged from using them on long term basis, as there will be rebound congestion & lead on to Rhinitis Medicamentosa. Hence they should not be used for more than 2 weeks at a stretch. Topical steroidal nasal spray are recent drugs which are devoid of any serious systemic side effects & are the hope as for as long term management of Allergic Rhinitis is concerned. But a word of caution is appropriate, here, sprays should not be used for more than 6 weeks at a stretch. Surgical treatment of Allergic Rhinitis will include chemical Cautery, Electro Cautery, CryoCautery, partial inferior turbinectomy & will provide some relief of symptoms at least temporarily, as the basic pathology is not treated. Instead only the effects of Allergic Rhinitis like swollen turbinates are trimmed only to recur at later stage. Management of Allergic Rhinitis is an ongoing process & the patient should be improved upon the nature of the disease, the limitations of the treatment modalities available test the patient be disappointed with the management of Allergic Rhinitis.

What Paper is used to make Tea Bags?

Side effects of Polypropylene polybenzyl isocyanate, or PPI, is a polyurea-based polymer derived from isocyanate monomers that is used in some glues, aerosol foams, plastics, paint and household products. PPI is listed as a “hazardous agent” by the National Institutes of Health because it irritates skin and breathing passages at high concentrations. It releases carbon dioxide and carbon mono oxide. Some painters exposed to PPI and related isocyanate chemicals have developed occupational asthma. In chronic toxicity tests, mice exposed to low levels of PPI developed no side effects. Only those mice exposed to high concentrations developed lung symptoms. The high doses used in this study were 1.0 and 6.0 mg per cubic meter for 6 hours each day, 5 days each week for 2 years. The duration-adjusted exposure levels for these high concentrations are equivalent to 11 ppb and 70 ppb, 24 hours/day for two years. Traditional Tea: In India, traditionally, tea is prepared by simmering with water and straining the mixture. There are no hazardous effects when tea is prepared by this traditional method as compared to the modern teabags.
Description: A tea bag is a small porous bag or envelope that holds tea or herbs. This bag is dipped into hot water and allowed to steep. It can then be removed to keep the tea free of loose leaves. Most tea bags are made out of a paper fiber. History: Tea has been used as a beverage for more than 4,000 years. Loose leaf tea was not replaced by tea bags until the 20th century. In 1908, a New York City tea merchant named Thomas Sullivan sent samples of hie tea to customers packaged in small silk bags. Many of his customers dunked the bags into water to steep, and thus was born the tea bag. The silk bags were replaced by gauze and then, eventually, by treated paper bags. Process: Tea bag paper is made primarily from abaca hemp, a product of a Philippine banana tree that is also known as Manila hemp. This hemp is bleached and processed. The tea bags are then treated with a heat-sealable thermoplastic such as PVC or polypropylene on the inside. Types: While the majority of tea bags are made out of paper, there are still some bags made from silk, nylon or muslin. Shapes: A majority of tea bags are shaped as rectangular or square envelopes. Head injury is any trauma that leads to injury of the Scalp, Skull, or Brain. These injuries can range from a minor bump on the head to a fatal bra in tear. Head injury can be classified as either Closedor Penetrating. ªª Closed Head Injury: The head sustains a blunt force by striking against an object. A concussion is a type of closed head injury that involves the brain. ªª Penetrating Head Injury: High velocity object breaks through the skull and enters the brain. ªª Any blow to the head can result in a head injury. Head injuries can cause damage to the: ªª Scalp (skin covering the top of the head)- such as a minor bump on the head or scalp wound that

Bleeding in the brain often starts within the first 24 hours after ahead injury and can last for three days or longer. It is imperative to watch for signs of a serious head injury during the first 24 hours.
HOW TO RECOGNIZE HEAD INJURY? SIGNS OF HEAD INJURIES - that alert the need for immediate medical care include: ªª Loss of consciousness, confusion, drowsiness or sleepiness ªª Inability to move any part of the body or weakness in an arm or limb. ªª Dent, bruise, cut or blood on the scalp ªª Severe headache with vomiting ªª Stiff neck ªª Blood or fluid that comes from the mouth, nose or ear ªª Blindness, blurred or double vision ªª Convulsions or fits
GUIDELINES ¾¾ IF THE VICTIM HAS SEVERE BLEEDING ªª The site should be covered with a cloth and keep the pressure applied till he is taken to a hospital.
ªª Shifting has to be swift and smooth so that the
patient has the ‘golden hour’ with his side for his fast recovery. ªª If you suspect injury to the neck, please make sure to immobilize the neck with whatever is available, be it even two bricks covered with some cloth! This is because if the victim has spine injury, further moving the neck will cause

IMPORTANT POINTS ªª Many times seriously injured are taken to smaller hospitals where a neurosurgeon is not available to attend the needy. ªª By the time available doctors assess him and advise you to ‘SHIFT TO A BIGGER HOSPITAL’, most precious minutes are already lost ªª Don’t forget! Many of these victims can still be saved if attended immediately. ªª Sometimes, a few hospitals agree to admit the victim in, and advise to get a CT scan of the head elsewhere. ªª If the facility is not available in that hospital, then again you will be ‘ROAMING’ endlessly from one place to another, and often with grave consequences. ªª It is stressed again, that if you feel there is brain injury, ‘RULE OF THE THUMB’ is to shift to a bigger center with CT scan facilities and of course, a neurosurgeon.
MANAGEMENT ªª First & Foremost - shift him or her out of the road
and loosen all the clothing especially over the neck and chest to allow sufficient breathing. ªª Check for the Pulse - over the wrist where we wear our watches, and over the side of the neck or groin. If the pulse is feeble or not felt, bend over his chest and listen to the heart beats. If you cannot hear any, then CARDIAC MASSAGE should be tried without wasting anytime. ªª Check for the breathing - you can keep the back of your palm against the nostrils and feel for airflow. If it is inadequate, mouth-to-mouth breathing should be done. ªª Check for the bleeding - Any bleeding site should be compressed with the help of some clothing.

This is the critical stage for the patient who is on the border of ‘LIFE AND DEATH’ and he should be given the best possible attention, as soon as possible. ªª This holds good even if such a hospital is little far away from the accident spot as it is ‘BETTER

Thus a person with pitta constitution is prescribe a stem of Katunimba( –Neem or the Margosa tree )or Arjuna (Arjuna myrobalan), A person of Vata constitution the stem of Banyan tree and a person of Kapha constitution the stem of Arka((Calotropis procera) or Karanja (Indian beech –Pongamia glabra) . These stems though well known for their effect on teeth and gums the fresh juice in the various stems when digested and absorbed also exerts its beneficial effects on different tissues too. To quote some examples the stem of Arka- calastrophes procera is prescribed for brushing in infertile persons whose semen count is less to increase the count in semen. The banyan, pomegranate, Arjuna and kutaja (Kurchi-Holarrhena Antidysenterica) stems as brushes increase the complexion of the skin and helps in candidates who suffer from skin disorders. , Badara stem improves the quality of voice in singers, khadira (Acacia catechu) stem exerts cleansing action on the mouth, and mango imparts health. Each tooth should be brushed individually by koorchaka i.e. a soft brush. The tooth brush should be rotated over the gums and the teeth in a vertical manner from below upwards in the lower jaw and from above upwards in the upper jaw. Side to side brushing is avoided as it may damage the neck of the teeth and also the gums. One should vigorously rinse and gargle the mouth after brushing the teeth as well as after eating any food .Place the brush at a 45 degree angle and use short downward strokes and round circular movements about 10 strokes per section and keep brushing for at least 4 to 5 minutes. Noted Tooth powder and tooth paste: Use powders of Triphala (Amalaki , Haritaki and Bibheetaki ) or Trijataka(Cinnamom,cardamom, and tamala patra )with honey and saindava salt for people with vata and pitta constitution.For those with kapha constitution Trikatu (ginger , pepper and pippali) with honey and saindava salt can be used as a tooth paste. Massaging the gums daily with Triphala powder mixed with sesame oil maintains their health and luster. General Tooth paste: As a general measure one can use tooth powder consisting of ginger , pepper, pippali, cinnamon, cardamom, Tamalpatra , tejovathi, Triphala , catechu and saindava salt which should be used as a paste after mixing it with til oil .Before use powder must be filtered through a fine cloth. Exercise to have tighter and better complexion and better and stronger gums. Like rest of our body mouth changes with age Understanding these changes makes it easier to maintain a healthy mouth and teeth. Facial exercises starts with chewing .It stimulate the nerves which release the harmones activating the gut. Individuals who chew have their first bowel movement sooner than those who do not chew.

Gargles and rinses
As you become old or in some conditions the mouth is partly dry because we produce less saliva as we age .Sometimes it could be the effect of medications. regime effective enough to slash away those gum problems too. Mouth washes can keep the mouth moist. Mouth rinse (usually preparedo out of decoction of herbs) control the pathogen (bad bacteria) in the mouth without disturbing the good bacteria in the mouth. A major focus in the development of mouth rinse that work as synthetic super salvers generally envisioned as a home based addition to a regular brushing regimen are being aimed not only at fighting decay but also strengthen teeth and repair the developing cavities Gargling the mouth after meals, eating any other food and after brushing the teeth keeps the mouth total protection from the lurking bacteria -the effect of reminants hiding in the mouth. With hot water it exerts a cleansing action on the mouth, teeth, gums and tongue and imparts a light and fresh feeling to the mouth. Gargling with cold milk has a cooling and cleansing and healing action on the ulcers in the mouth. Rinsing with oil exerts a cleansing action and strengthens action on the tongue, gums, teeth, mouth and voice. It prevents cavities and it is extremely useful for curing hypersensitivity of gums to sour articles, also improves the sense of taste. Gargling with decoction of khadira , lodra , triphala, arjuna, and catechu gives the teeth stronger longer life.

Types of gargle
There are two techniques involved in gargling. One technique involves using of a large quantity of fluid either oil, milk or medicated decoctions in a large quantity into the mouth( that one is unable to move it inside the mouth).. One should hold the fluid in the mouth, till the nose and eyes start watering. Here the fluid penetrates the oral mucosa and gums by pressure and exerts its special action. However this is not prescribed for children under the age of five. In the other technique one holds and moves the solution in the mouth. Warm oil (generally sesame oil)

is used for diseases of the mouth where wind bioenergy is involved. These are the diseases which make your mouth dry and rough Milk and sugar water with sweet and cold medicines is used for soothing action on heat bioenergy of the mouth which causes disorders like ulcers in the mouth and gums. Medicated decoction with astringent, sour or salty taste is used for its cleansing action in disorders of water bioenergy where you have excessive salivation and stickiness in the mouth . Swish the rinse through out the entire mouth making sure to cleanse in between the teeth rinse for 5 to 10 minutes is usually sufficient to cleanse the germs and freshen breath .After rinsing spit out the rinse .Blow your nose to clean any excess mucus from your nasal passage. Scientist’s evidence reports that in addition to tooth brushes and oral hygiene with gargles , tongue cleaning should be an additional part of oral health care... Use a special tongue scraper as a part of your mouth care hygiene appointment. You can do this by making sure you reach the back of your tongue and then with the light pressure action pull the scraper forward, cover your tongue until the debris is removed. Do this twice a day and follow it with mouth wash. One should use a thin plate of gold, silver or copper for cleaning the tongue. One may alternatively use a leaf or a thin wooden plate. The tongue cleaner should be soft and smooth with rounded edges . Its length should be ten fingers. Cleaning the tongue helps to get rid of waste products and bad odor of the mouth improves taste sensation and removes bad odour of the mouth.It improves taste sensation and exerts tonic effects on the tongue. It has been proved that the bacterial flora in the mouth is considerably reduced by cleaning the tongue. Its value in preventing the disease of the mouth was realized by modern medicine only very recently. If stems can have such an enormous impact on oral hygiene and a profound and almost immediate impact on our mood and mental energy why not exploit the power and keep healthy the natural way.

They also help in keeping blood pressure under control and keeping bones healthy. They make certain hormones that our body needs to stay healthy.
There are no symptoms in the early stages of the disease. Symptoms occur only in the late stages of the disease. Early detection of CKD is possible only by testing the blood and urine. There are two major types of kidney disease, namely acute and chronic kidney disease. The acute variety is due to various causes like drugs, infections, dehydration, radio contrast injections etc. The acute kidney injury or failure is curable. Some may need dialysis support for a short period before recovery. Chronic kidney Disease (CKD) is one in which the kidneys are damaged slowly and irreversibly. etecting Childhood Blindness using Innovative, Low-Cost Screening Methods: Find the
Blind Visual screening for school children is effective in detecting and treating visual defects at an early stage. Unfortunately, many Indian children with visual disabilities do not attend schools, leaving them to be raised at home. There are many hurdles in detecting, diagnosing and treating such children. We intend on bridging the gap by ensuring that all children with visual disabilities are detected, and provided access to proper ophthalmology support to treat and rehabilitate them. Narayana Nethralaya has developed many creative new strategies to detect children with vision and life threatening eye conditions. The group of strategies has been called ‘Find the Blind’, which was conceived by Dr. Ashwin Mallipatna, a pediatric

ophthalmologist who leads this group of pediatric community ophthalmology projects.
Photo Red Eye Diagnosis (Photo-RED)
Red reflex screening is a standard prescribed method to detect very young (pre-verbal) children with visually threatening eye defects. A policy from the American Academy of Pediatrics suggests that all infants and children be screened by a pediatrician or personnel specifically trained in screening for these defects. When the red-reflex is altered (either absent, or looking white or looks unequal in the two eyes) the child would warrant a full eye examination. In resource-poor countries like India, this procedure is not carried out as a routine. The test involves the use of an instrument called an ophthalmoscope to check the eyes for the red reflection which is not unlike the “annoying” red reflexes in flash photographs. At the Hospital for Sick Children in Toronto, Dr. Ashwin Mallipatna developed a scientifically sound strategy to consistently induce the appearance of a red-eye in digital flash photographs of children. The strategy uses an inexpensive digital camera set with the right parameters and using the proper subject conditions to ensure that a red-reflex be obtained in all normal children. We pilot tested this strategy and found that it was effective in detection of vision and life threatening eye conditions in very young children, including a pediatric eye cancer called retinoblastoma. An appropriately trained red-reflex screener would take digital photographs in the prescribed way and send suspicious photographs to a pediatric ophthalmologist. The obtained photographs are read by a pediatric

ophthalmologist for possible abnormalities and the results sent back to the red-reflex screener. The utilisation of this technology has won the Humanitarian Award from Technology Review (India), which is a publication affiliated to Massachusetts Institute of Technology in the United States.
Child to Child Strategy
A key-informant is a person who is responsible for bringing a medical condition to the notice of the medical authorities. In this project our objective was to effectively train village citizens to be keyinformants and identify obvious eye abnormalities and visually-impairment in the children of their community. We trained community health workers, non-health workers and school children with the knowledge required to find young children who may be facing a lifetime of blindness within their community. Figure 2: Home screening of a visually disabled child detected using the child-to-child strategy. The development and implementation of a pilot project was carried out by Narayana Nethralaya. After an hour of a lecture, the key-informants would go out into the community and report cases of childhood blindness from their neighbourhoods. Through this pilot project, we were about to find children with blindness up to 16 kilometers away from where the teaching was conducted within a week of our teaching. Our key informants were able to identify about 50 children with possible eye problems. On a follow-up screening of the children identified, we were able to find about 25 children with blindness. The children with untreatable blindness were put on rehabilitation programs. This program is unique and novel in its ability to rapidly screen rural communities for children with treatable and untreatable blindness, and bring them to proper care. This strategy has proven to be more efficient in the detection of severe blindness when compared to house-to-house screening, vision screening in schools or eye camp related activities. In the coming years, we plan to expand our area of coverage. This strategy has the potential to find children with physical and intellectual deficits as well.

Armed with a GPS enabled smart phone, an ophthalmic assistant will visit the home of each child with a visual deficit. These children will be briefly examined by the ophthalmic assistant and photos will be obtained and documented in a database. The parents will be advised about where and how they can seek help, and the location of the house will be recorded using the GPS. The progress of the child will be periodically monitored by health workers, using these GPS co-ordinates to find the house and child. Systematic registering of every child with a visual disability will help us keep track of their progress.

Diabetics and Ayurveda I. Some of the most useful Herbs for the Treatment of Diabetes are the following: Bitter Gourd (Momordica charantia), Bael (Aegle marmelos), Gurmar Leaves (Gymnema sylvestrae), Fenugreek (Trigonella foenum graecum), Turmeric (Curcuma longa),Onion (Allium cepa),Nayantatra (Vinca rosa), Neem (Azadirachtha indica), Garlic (Allium sativum), Sagar gota (Ceasalpinia crista) II. Simple remedies to avoid erectile dysfunction in diabetes: zzMix a pinch of turmeric with juice of emblica officinalis (Amla) and consume this in early morning. zzDrink triphala concoction daily in early morning. zzJuices of neem, guduchi (giloya) and Emblica officinalis prove beneficial if consumed in early morning. zzObese patients have to exercise regularly and aim at reducing weight. zzPractice yoga and meditation regularly to keep stress level low. zzMango tree leaves boiled in water and the juice is taken helps in curing diabetes. zzThe tender leaves of the mango tree are measured useful in diabetes. An infusion is prepared by soaking 15 gm of fresh leaves in 250 ml of water overnight, and squeezing them fine in the water in the morning. This filtrate should be taken every morning to manage early diabetes. zzAs an option, the leaves should be dried in the shade, powdered and preserved for use when necessary. zzDiabetes can be controlled by consuming fenugreek seeds. It is soaked in about a glass full of water and left overnight. Now these seeds are crushed and sieved from a fine cloth. Now this mixture is being drunk to see the wonderful result in 2 months. zzFenugreek seeds, when given in changeable doses of 25 gm to 100 gm daily, diminish reactive hyperglycaemia in diabetic patients. Levels of glucose, serum cholesterol, and triglycerides were also appreciably reduced in the diabetes patients when the seeds were consumed.

Drinking of water extract of Bengal gram by diabetes patients have shown significant improvement in their fasting blood sugar levels, glucose tolerance, urinary excretion of sugar, and general condition ªª Morning and evening walk is very important in diabetic condition. ªª Yogic asana like shashank asana is very effective in curing diabetes. ªª Diet control by in taking low calorie diet, low fat food and consuming high fiber diet is very effective means to prevent diabetes. ªª Garlic, ginger and onions are best options in curing diabetes. ªª Diabetes treatment with Jambul Fruit: The seeds must be dried and powdered. One teaspoon of this powder should be mixed in one cup of milk or water or half a cup of curd, and taken two times daily. ªª Diabetes treatment via Black Gram: For a milder sort of diabetes, two tablespoons of germinated black gram, taken with half a cup of fresh bitter gourd juice and a teaspoon of honey, is said to be valuable. It should be taken once each day for three to four months. A limit should be placed on the intake of carbohydrates. Still in severe cases, regular use of this combination, with other precautions, is helpful as a healthgiving food for the prevention of various complications that may arise due to malnutrition in diabetics. ªª Diabetes treatment via Other Foods: Besides
bitter gourd, certain other vegetables have been found helpful in diabetes. These comprise string beans, cucumber, onion, and garlic. Tea made of the pods of string beans is important in diabetes.

ªª A diabetic must pursue a low carbohydrates, low fat & moderate protein diet.
ªª Salads can be consumed in any quantities but high fat salad dressings should be restricted.
ªª Fruits can be taken as per suggested by your
dietician, fruit juices should be avoided and whenever possible fruit must be taken with the skin eg. Apple, Pear etc. ªª If possible avoid fruits along with your meals and you can take it 2 hours after or before a meal. ªª Each meal should enclose some source of protein as it doesn’t increase the blood sugar instantly For eg. Signs and symptoms Symptoms are individualistic in nature. Piles may be caused by more than one factor. Symptoms will resolve within a few days. The symptoms keep changing as the disease advances.
There are definite signs and symptoms that can highlight the presence of piles. However some symptoms are individualistic in nature. Several symptoms are mentioned here, all these symptoms including bleeding may not appear in all the individuals. In most cases, symptoms will resolve within a few days. Piles may be caused by more than one factor. The symptoms keep changing as the disease advances. Some of the symptoms are as follows: Bleeding while passing motion: The earliest symptom is often the fresh, red blood from the anal passage when the bowels are opened. Bleeding from the anus is more common with internal than external haemorrhoids. It can occur before, during or after defecation.
Red masses of tissue coming out during defecation: The most common symptom of internal hemorrhoids is bright red blood covering the stool, a condition known as hematochezia, on toilet paper.
