A GOOD TEST FOR THE ELDERLY MALE-PSA Dr. Sanjay Consultant Urologist Assoc. Professor, Vydehi Hospital, Whitefield, Bangalore-560066 Ph: 080 – 28413381/82/83 Despite its relatively small size, the prostate accounts for substantial and disproportionate health problems in older men, the most significant of which is cancer. Prostate cancer has become the most common cancer of males in several developed countries. Prostate specific antigen (PSA) is a protein produced by the cells of the prostate gland. PSA is present in small quantities in the serum of normal men, and is often elevated in the presence of prostate cancer and in other prostate disorders. A blood test to measure PSA is the most effective test currently available for the early detection of prostate cancer. Higher than normal levels of PSA are associated with both localized and metastatic prostate cancer (CaP). ¾¾ Biochemistry Prostate specific antigen (PSA) is a glycoprotein manufactured almost exclusively by the prostate gland ; PSA is produced for the ejaculate where it liquifies the semen and allows sperm to swim freely. It is also believed to be instrumental in dissolving the cervical mucous cap, allowing the entry of sperm ¾¾ Clinical significance PSA is normally present in the blood at very low levels; normal PSA levels are defined as between 0-4.0 ng per milliliter. Increased levels of PSA may suggest the presence of prostate cancer. However, prostate cancer can also be present in the complete absence of an elevated PSA level, in which case the test result would be a false negative. PSA levels can be also elevated due to prostate infection, irritation, benign prostatic hypertrophy (enlargement) or hyperplasia (BPH) or recent ejaculation, in which case it may give a false positive. It is a myth that digital rectal exam raises PSA. The rate of increase of PSA (the PSA velocity) may have value in prostate cancer prognosis. Men with prostate cancer whose PSA level increased by more than 2.0 ng per milliliter during the year before the diagnosis of prostate cancer have a higher risk of death from prostate cancer despite undergoing radical

prostatectomy. ¾¾ Prostate cancer screening The U.S. Food and Drug Administration (FDA) has approved the PSA test for annual screening of prostate cancer in men of age 50 and older. PSA levels between 4 and 10 ng/mL (nanograms per milliliter) are considered to be suspicious and should be followed by rectal ultrasound imaging and, if indicated, prostate biopsy. PSA is false positive-prone (7 out of 10 men in this category will still not have prostate cancer) and false negative-prone (2.5 out of 10 men with prostate cancer have no elevation in PSA). The current guidelines of the American Cancer Society(ACS) recommend that men over age 50 should be offered the digital rectal exam and PSA tests yearly. Screening should be offered annually to AfricanAmerican men and those with a family history of prostate cancer upon reaching 45 years. Other racial and ethnic groups, such as Asian- and Hispanic-Americans have a lower risk of prostate cancer, and may not benefit from screening. Screening is likely not useful for men over age 70 or with other significant medical problems and a life expectancy of fewer than 10 years. These guidelines may be changing. A new European study has shown that a thorough screening for prostate cancer every 4 years is adequate. The screening comprises a PSA blood test, a digital rectal exam, and a transrectal ultrasound ¾¾ The Prostate-Specific Antigen (PSA) Test: Questions and Answers ¾¾ What is the prostate-specific antigen (PSA) test? Prostate-specific antigen (PSA) is a protein produced by the cells of the prostate gland. The PSA test measures the level of PSA in the blood. The doctor takes a blood sample, and the amount of PSA is measured in a laboratory. Because PSA is produced by the body and can be used to detect disease, it is sometimes called a

biological marker or tumor marker. It is normal for men to have low levels of PSA in their blood; however, prostate cancer or benign (not cancerous) conditions can increase PSA levels. As men age, both benign prostate conditions and prostate cancer become more frequent. The most common benign prostate conditions are prostatitis (inflammation of the prostate) and benign prostatic hyperplasia (BPH (enlargement of the prostate). There is no evidence that prostatitis or BPH cause cancer, but it is possible for a man to have one or both of these conditions and to develop prostate cancer as well. PSA levels alone do not give doctors enough information to distinguish between benign prostate conditions and cancer. However, the doctor will take the result of the PSA test into account when deciding whether to check further for signs of prostate cancer. ¾¾ Why is the PSA test performed? The U.S. Food and Drug Administration (FDA) has approved the PSA test along with a digital rectal exam (DRE) to help detect prostate cancer in men age 50 and older. During a DRE, a doctor inserts a gloved finger into the rectum and feels the prostate gland through the rectal wall to check for bumps or abnormal areas. Doctors often use the PSA test and DRE as prostate cancer screening tests; together, these tests can help doctors detect prostate cancer in men who have no symptoms of the disease. The FDA has also approved the PSA test to monitor patients with a history of prostate cancer to see if the cancer has come back (recurred). An elevated PSA level in a patient with a history of prostate cancer does not al ¾¾ For whom might a PSA screening test be recommended? Doctors’ recommendations for screening vary. Some encourage yearly screening for men over age 50, and some advise men who are at a higher risk for prostate cancer to begin screening at age 40 or 45. Others caution against routine screening, while still others counsel men about the risks and benefits on an individual basis and encourage men to make personal decisions about screening.. Several risk factors increase a man’s chances of developing prostate cancer. Age is the most common risk factor, with nearly 70 percent of prostate cancer cases occurring in men age 65 and older. Men who have a father or brother with prostate cancer have a greater chance of developing prostate cancer. African American men have the highest rate of prostate cancer, while Asian and Native American men have the lowest rates. In addition, there is some evidence that a diet higher in fat, especially animal fat, may increase the risk of prostate cancer. ¾¾ How are PSA test results reported? PSA test results report the level of PSA detected in the blood. The test results are usually reported as nanograms of PSA per milliliter (ng/ml) of blood. Most doctors consider PSA values below 4 ng/ml as normal Higher a man’s PSA level, the more likely it is that cancer is present. But because various factors can cause PSA levels to fluctuate, one abnormal PSA test does not necessarily indicate a need for other diagnostic tests. When PSA levels continue to rise over time, other tests

may be needed. ¾¾ What if the test results show an elevated PSA level? A man should discuss elevated PSA test results with his doctor. There are many possible reasons for an elevated PSA level, including prostate cancer, benign prostate enlargement, inflammation, infection, age, and race. If no other symptoms suggest cancer, the doctor may recommend repeating DRE and PSA tests regularly to watch for any changes. If a man’s PSA levels have been increasing or if a suspicious lump is detected during the DRE, the doctor may recommend other tests to determine if there is cancer or another problem in the prostate. A urine test may be used to detect a urinary tract infection or blood in the urine. The doctor may recommend imaging tests, such as ultrasound or cystoscopy. Medicine or surgery may be recommended if the problem is BPH or an infection. If cancer is suspected, a biopsy is needed to determine if cancer is present in the prostate. During a biopsy, samples of prostate tissue are removed, usually with 5a needle, and viewed under a microscope. ¾¾ What are some of the limitations of the PSA test? Detection does not always mean saving lives: Even though the PSA test can detect small tumors, finding a small tumor does not necessarily reduce a man’s chance of dying from prostate cancer. PSA testing may identify very slow-growing tumors that are unlikely to threaten a man’s life. Also, PSA testing may not help a man with a fast-growing or aggressive cancer that has already spread to other parts of his body before being detected. False pos0147itive tests: False positive test results occur..3041when the PSA level is elevated but no cancer is actually present. False positives may lead to additional medical procedures that have potential risks and significant financial costs and can create anxiety for the patient and his family. Most men with an elevated PSA test turn out not to have cancer; only 25 to 30 percent of men who have a biopsy due to elevated PSA levels actually have prostate cancer (3). False negative tests: False negative test results occur when the PSA level is in the normal range even though prostate cancer is actually present. Most prostate cancers are slow-growing and may exist for decades before they are large enough to cause symptoms. Subsequent PSA tests may indicate a problem before the disease progresses significantly. ¾¾ Why is the PSA test controversial? It is not clear if the benefits of PSA screening outweigh the risks of follow-up diagnostic tests and cancer treatments. For example, the PSA test may detect small cancers that would never become life threatening. This situation, called over diagnosis, puts men at risk for complications from unnecessary treatment such as surgery or radiation. The procedure used to diagnose prostate cancer (prostate biopsy) may cause side effects, including bleeding and infection. Prostate cancer treatment may cause incontinence and erectile dysfunction. For these reasons, it is important that the benefits and risks of diagnostic procedures and treatment be taken into account when considering whether to undertake prostate cancer screening. www.bangalorehealthtv.com

HYPERPARATHYROIDISM Parathyroid glands control the amount of calcium in our blood and bones. About 1 in 250 people (1 in 100 women over 50 years ) develop a parathyroid gland tumor causing parathyroid disease called ‘Hyperparathyroidism”. Hyperparathyroidism is a destructive disease that causes high blood calcium, which leads to health problems and even early death. It is an easily curable condition in most patients and can be done in 20 minutes with the removal of the parathyroid gland tumor. Parathyroid glands (four in number) are small glands of the endocrine system located in the neck generally behind the thyroid gland. They are normally the size of a grain of rice. Occasionally they can be as large as a pea and still be normal. Normal parathyroid glands are the color of spicy yellow mustard (see figure) behind the pink thyroid gland. The light blur colored tube in the center is the trachea (windpipe). The corotid arteries are shown on both sides of the thyroid gland running from the heart into the brain. Note that the figure shows three normal parathyroid glands and one diseased and enlarged parathyroid tumor. The function of the parathyroid glands is to regulate the calcium level in our bodies within a very narrow range so that the nervous and muscular systems can function properly. The parathyroid glands recognise low calcium levels in the blood and produce more parathyroid hormone. The hormone acts on the bones, extracting calcium from them and putting it in the blood. When the blood calcium becomes normal again, the glands stop producing the hormone. Surgery is the most common treatment for primary hyperparathyroidism and provides a cure in at least 98 percent of all cases. A surgeon will remove only those glands that are enlarged or have a tumor. If all four glands are affected, a surgeon will likely remove only three glands and perhaps a portion of the fourth — leaving some functioning parathyroid tissue.

those with full mutations, but also the maternal carriers as well as the maternal grandfathers. Awareness of this disorder and its myriad variations is essential for those that treat adults as well as children. Fragile X Syndrome needs to be considered for children with severe developmental delays, autism, learning disabilities and ADHD as well as women with primary ovarian failure, anxiety, and certain learning disabilities. Adults who are carriers may present with tremor and ataxia as they age. The highly complex genetics of Fragile X will be explored in this talk along with its various manifestations. ÂÂ Fragile X Syndrome: specifics of developmental variation and identification of children with the disorder will be discussed in detail in order to assist the clinician in identification. Identification of those affected with Fragile X Syndrome is critical as there are new pharmacologic treatments on the horizon which may be able to address the underlying abnormalities at the cellular level and which may be capable of reversing the symptoms of the disorder. Essential therapeutic interventions as well as pharmacologic management of behavioral symptoms will be reviewed. ÂÂ Autism Spectrum Disorder (ASD) is a range of complex neurodevelopmental disorders, characterized by social impairments, communication difficulties, and restricted, repetitive, and stereotyped patterns of behavior. ASD varies significantly in character and severity, occurs in all ethnic and socioeconomic groups, and affects every age group. In this talk, we will explore the etiology and clinical manifestations of ASD to help the clinician with the early identification and diagnosis of ASD. ÂÂ Autism Spectrum Disorder (ASD) is highly heterogeneous and since there is no unifying pathogenesis, there is no unified treatment approach. In order to better understand and characterize the complex ASD disease syndrome, many clinicians are subdividing ASD individuals on the basis of their personal strengths and weaknesses. This allows the clinician to offer customized therapeutic interventions. In this talk we will discuss the multi-disciplinary approach to developing an individualized treatment plan for the ASD individual. There were 5 parallel workshops conducted by these speakers. There were 3 live surgeries (retinal, spine and cochlear implant) parallel telecasted in three halls.. Dr Ryan Kotton performed Botox procedure on three children with spasticity. The sessions were further enriched by the scientific workshops, where the medical and rehabilitation professionals saw these specialists in action and gained tremendously through these sessions.

Anatomy of the Uterus The reproductive system of a woman is made up
of the following parts: (Fig 1) ÂÂ Vagina ÂÂ Womb or uterus ÂÂ The neck of the womb-cervix ÂÂ Fallopian tubes ÂÂ Ovaries The uterus is part of a woman’s reproductive system. It is the pear-shaped muscular bag that protects a growing baby during pregnancy. ¾¾ Anatomy of the Uterine Fibroid Uterine fibroid are tumors or lumps made of muscle cells and other tissue that grow within the wall of the uterus. Fibroids may grow as a single tumor or in clusters. A single fibroid can be less than one inch in size or can grow to eight inches across or more. Some times they appear as bunch or clusters like grapes. ¾¾ Types of Fibroid Fibroids are classified according to their location within the uterus. There are three primary types of fibroids: (Fig 2)

Uterine Fibroids is the one of the most common disease women face in their childbearing age. About 77% of the women of their age group have this problem. Most of the women will not know that they have fibroids because most of the them are harmless and produce no symptoms. a.Subserosal Fibroids: Fibroids that develop in the outer portion of the uterus and continue to grow outward. b.Intramural Fibroids: Fibroids that develop within the uterine wall and expand, which makes the uterus feel larger than normal. c.Submucosal fibroids: A type of fibroid that develops just under the lining of the uterine cavity. ¾¾ Symptoms of Fibroid Many women don’t feel any symptoms with uterine fibroids. children. Hysterectomy – Removal of the uterus. This surgery is done for women who already have children. ªª Endometrial ablation–The endometrial lining of the uterus is destroyed. This surgery controls very heavy bleeding. This is done in women who already have children or who don’t want children. ªª Myolysis: This uses electric shock to shrink the fibroids. ªª Embolization: Cuts off the blood supply to the uterus and the fibroid. This is a common procedure when symptoms are not severe. ¾¾ Complication of the Surgery Following complications may arise after surgery: Risk of needing a blood transfusion, Infection, Difficulty in intercourse, Pain, Urination problem. ¾¾ Precaution to be taken before surgery: There is growing tendency of removing the uterus even for younger women although the fibroids are small and symptomless.There are incidences of removing the uterus even in young women who have not completed their child bearing duty. This should be strongly avoided both by doctors as well as patients. It is always ideal for the patient to seek second opinion whenever surgery is advised by one doctor. Medical line of treatment should always be tried before surgery. Sometimes they are used before surgery, to shrink the fibroids, making them easier to remove. MRI Compatible Pacemakers- a proven boon for cardiac patients
It is well known that for patients with implanted cardiac devices, MRI scans are unsafe and contraindicated. Research studies state that 50%–75% of patients with an implantable cardiac device need a MRI scan over the rest of their lifetime with implanted device. While a traditional pacemaker helps to treat a form of cardiac arrhythmias known as bradycardia (slow heart beats) and normalize the heart rhythm, it may pose challenges for cardiac patients with other health issues, who need to be exposed to a MRI scan. These include the new age young adults as well as the high-risk elderly patients, with diseases of the brain, spinal cord, or joints – anything which involves tissue degeneration or trauma! Furthermore it may also include individuals over the age of 65 who are twice as likely to be prescribed a scan for the liver, gall bladder, pancreas and kidney. She was implanted with dual chamber pacemaker due to complete heart block (CHB). Three years back, she had been through a contrast CT scan of the abdomen which was performed for suspected acute pancreatitis. Hirandodi is a perennial climber and is important both as a medicinal crop and as a fodder crop. Leaves and roots are useful in skin infection and wounds and occasionally used in nasal and ear problems. However, their major use is to prevent prolapse of uterus and vulva in controlling habitual abortion in women. The roots are used in cardiac diseases, haemorrhage, as diuretic and to cure fever and eye disorders. Alcoholic (50%) extracts of roots and leaves show anti-bacterial activity against gram-positive and gram-negative bacteria including Micrococcus pyogens and also active against Trycophyton rubrum. It is a tonic given for general debility and such similar conditions to strengthen the body. The leaves are also used to cure

asthatna, cough and against ring worms. They are wellknown for their galactogouge properties and given to lactating mothers. It is a rich source of vitamin ‘A and is cooling, eye tonic, nutritive and aphrodisiac drug. A much branched twining shrub; bark is yellowish brown, deeply cracked, leaves are ovate, cordate coriaceous, glabrous above, more or less finely pubescent beneath. The flowers are borne in many flowered cymes, greenish yellow, lateral or sub axially. Flowering is observed from May to June months, Fruiting begins from October and continues up to November. ¾¾ PARTS USED Leaf, Root, Stem. ¾¾ MEDICINAL PROPERTIES ÂÂ As it is galactogenic, advised for the post-natal feeding mothers. ÂÂ The root paste if applied controls Urticaria. ÂÂ The leaf juice or the decoction relieves constipation. ÂÂ The root decoction is best for cough. ÂÂ The leaf juice with honey improves immunity in children. ÂÂ This is also best fodder for cattle. Hirandodi can be used as other plant leaves in preparing the food like curry, Thambuli and so on. It increases the resistance power against many diseases. ¾¾ RECIPE Curry : It can be prepared using the leaves and stems. The cut leaves are cooked along with turdhal and salt. Specially recommended to lactating mothers. Thambuli : Roast the leaves along with cumin seeds. Grind to a paste along with grated coconut and salt. Mix this paste with curds or butter milk.

In general for my growth the excellence of the following factors in the pelvis of my mother make it happen. ªª There should be excellence of the factors responsible for my creation i.e. mother’s ovum, father’s sperm, satmya wholesomeness of energy for conception, nourishing digestive product of the mother’s food and a balanced satwa mind. ªª Adoption of proper regimen excellence of the properties of food by my mother during pregnancy. ªª Sufficient availability of heat and nourishment is very important for my growth and development. Heat and nourishment reach me through the process of upaneha sweda – (transudation) impulses moving along the same neuron and upasweda –( conduction) impulses moving from one cell to another respectively. ªª 4. Proper time is essential for my full growth --The developmental time of an organism is the number of days required for the organism to mature .The development of living being requires precise coordination of all basic cellular processes in the terms of space and time. ªª 5. I have Instinctive or natural tendency which helps me grow through successive stages. If there is defect in any of the above factors responsible for the production of the foetus there will not be delivery of the foetus because it will get destroyed in the pelvis of the mother and will be thrown out. Mother desirous of an offspring endowed with excellent qualities should refrain from unwholesome diet and regimens. She should perform virtuous acts and resort to diet and regimens which are beneficial. I still remember when my mom experienced a bout of vomiting, My God! I felt weak .When my mom took a sweet dish latter it was wholesome to my body. It gave me the flow of electrolytes into my body I felt there was rejuvenation in my blood , muscles , bones , bone marrow , ojas , semen and longevity . It was so soothing to my six sense organs .It not only promoted strength and complexion but also alleviated my pitta bioenergy and kapha bioenergy and removed the effects of toxins. My thirst was relieved and so was my burning sensation and it promoted healthy skin, hair voice and strength at the subtle level. , cold , intestinal discomfort , fever with chills , bloated feeling in the stomach , constipation , sweetness in the mouth ,vomiting , loss of sensation and voice , cervical lymphadenitis , elephantiasis , pharyngitis , adhesions in the bladder vessels , throat, eye diseases etc. Sweet taste is present not only in sweet preparations but also in fruits, vegetables, grains like rice wheat etc. Addiction in the intake of sweet food for a pregnant woman makes the offspring suffer from obstinate urinary disorders including diabetes, dumbness and excessive obesity. My mom conceived me in the month of winter. There was lot of depression in the atmosphere outside. My moods were also very low. It was during this period I first experienced sour taste through my mom. Sour taste increases secretions and is sharp; my taste bud got activated and so was the food, my appetite got stimulated and I experienced the wave of nourishment and energy flowing through my body. It enlightened my mind, strengthened my sense organs alleviated wind bioenergy. My heart woke up as if it got special nourishment, caused salivation helped me in swallowing moistening and digestion of food .In Toto sour taste was refreshing light hot and unctuous. Sour taste, which increases secretions, is sharp but not pungent. Sour taste has all the above mentioned benefits experienced by the baby when used in moderation but if used in excess you will experience excess thirst , sensitivity of teeth , a bristling of the hair on the skin, as from cold or fear; horripilation also called goose flesh, goose pimples,liquification of kapha bioenergy , aggravation of pitta bioenergy , vitiates blood , decomposes muscles and makes them flabby , bring about swelling in the body especially in people who suffer from weakness , cachexia -weakening condition of body and mind due to a chronic illness .Sour taste has a heating property and they don’t prove beneficial to people who have been injured or wounded. Ulceration , trauma , contagious bites , burns, fractures swelling, dislocation , inflammation of the skin caused by contact with poisonous urine or poisonous animals all refuse to heal when given a diet consisting of sour taste. Sour food causes Bruises, excision, incision, separation, perforation and crushing, type of injuries highly inflammatory and make one have burning sensation in that area and also cause burning sensation in the throat chest and cardiac region. When taken in excess Sour food makes the offspring suffer from bleeding disorders and diseases of the skin and the eye. One evening my mom ate a little more than usual and that night she had severe discomfort in the tummy. I too shared her experiences. That day I tasted salt taste for the first time. My mom took a pinch of salt with warm water .There was complete relief of her discomfort after this .Saline taste either prevents formation of gas in the gastrointestinal tract or facilitates the expulsion of said gas, thereby combatting flatulence. It cause deliciousness of food, they are neither heavy unctuous nor hot, do the work of excision and incision, are sharp, mobile. If used in excess saline taste vitiates pitta bioenergy and blood. This makes one experience excessive thirst, fainting, heating sensation, erosion, and depletion of muscle tissue, obstruction of the functioning of senses, premature wrinkling greying baldness and problem with fertility. People suffering from skin disorders have to take extra precautions because excess salt in their food triggers the sloughing of the bodily parts afflicted with skin diseases , aggravate the symptoms of toxins , bursts inflamed parts dislodge shaky teeth and aggravate bleeding disorders , acidity , skin diseases , gout , etc. A pregnant woman taking an excessive salty diet can make her offspring suffer from early onset of wrinkles in the skin, greying of the hair and baldness. My mouth feels clean , I feel there are secretions through nose , watering through the eyes ,in total I feel there is proper action of the sense organs says my Mom when she eats food like black pepper, ginger and chillies . My experience was also unique. The immune system you all know exists mainly in the blood. Blood flow is movement of prana which hypes courage and valour. Whenever my mom tastes pungent in the form of herbs or medicines it improves my circulation, liquefies, softens, secretes, flushes and does the breaking up and dissolving thick or hardened masses such as mucus. It warmed my liver (which is responsible for blood thickness), and expelled excess Vata, Pitta, and Kapha. I felt the surge of vibrating energy flowing through me. Made of fire element, pungent taste is sharp and concentrated, fast acting and intense, spreading quickly to all tissues. Any penetrating substances like digestive enzymes, aggressive substances like the immune system, anything that “penetrates� all tissues like the nervous system and the blood, intelligent tissues such as the analytical centres of the brain, etc. have pungency or sharpness. A pregnant lady using too much pungent food has an offspring which is weak deficient in energy and infertile. It was New Year day and it is a custom to eat neem leaves with jaggery. I tasted bitter taste for the first time on ugadi day when my mom ate it .Though neem leaves which are bitter to taste are by themselves not delicious but when taken with jaggery or any other taste they promote deliciousness. They are antitoxic and germicidal say Ayurvedic physicians and employ them in treating fainting disorders, burning sensation, itching, obstinate skin diseases, including leprosy, thirst and fever. They promote firmness of the skin, and muscles, promote carmination and digestion, purify milk, cause drying and help in depletion of moisture, fat muscle fat, bone marrow lymph pus, sweat urine stool pitta and kapha. They are unctuous cold and light. If used in excess because of their qualities like unctuousness, roughness and no sliminess they reduce the plasma, blood muscle, fat bone marrow, and reduce strength, cause emaciation weariness unconsciousness giddiness dryness of the mouth and produce other diseases due to the vitiation of vata bioenergy. If a pregnant lady uses bitter food in excess it will make her baby weak, emaciated and produce weakening condition of the body and mind due to illness. I tasted astringent taste when my mom had diarrhoea and took astringent tea. My mom had complete relief and doctor said astringent taste is sedative and constipative. They produce pressure on the affected part and cause granulation, absorption, and stiffness; they are capable of correcting bleeding disorders because they absorb the body fluids and are dry cold and heavy. But it should not be used in excess because they produce dryness of the mouth, affliction of the heart, distension of the abdomen, obstruction of speech, constriction of the circulating channels, black complexion , destruction of manhood .As they get digested slowly they obstruct the passage of flatus,

urine, stool and semen and cause emaciation, weariness , thirst and stiffness by virtue of their roughness ,dryness and sliminess .They produce diseases like hemiplegia , spasms , convulsions facial paralysis etc. because of vitiation of vata bioenergy .If astringent taste is taken in excess during pregnancy it make the offspring grey in complexion , constipated and having reverse peristalsis Excess wine consumption can deteriorate the memory of the baby make it fickle minded and keeps the thirst button always on .Eating a lot of iguana flesh can make a baby prone to diabetes , stone in the bladder and dribbling of urine . Addiction to pork when pregnant can produces redness in the eyes, sudden obstruction of respiration and excessive roughness of the hair of the offspring .Excess fish consumption is also bad because it can cause delayed closure or nonclosure of eyes . Resorting to vocal abuses and physical assaults make the progeny epileptic. Habitually resorting to sexual intercourse makes the progeny physically ill formed, shameless and subjugated to women .Constantly given to grief makes the progeny fearful thin and short lived. Thinking ill of others makes the offspring antisocial, envious and subjugated to women. Stealing makes the offspring exceedingly lazy, malicious and of inactive disposition and resorting to anger makes the offspring fiercely, deceitful and jealous. Keeping mind calm and composed is prescription for a pregnant woman. My mom should not Resort to seats which are uncomfortable irregular or high .Sitting anywhere for long periods can make her feet and ankles swell and her legs cramp. To prevent cramps she can walk around and do simple stretches. It’s always good for my mom to empty her bladder completely to help her prevent leaks as it does when the bladder is too full. She should not try to hold urine when she has the urge to urinate .It is good to lean a little forward while urinating to completely empty her bladder because when she is housing me it gives less space to store urine and it becomes more difficult to stop the flow ‘when the bladder is full . Early in pregnancy muscles in the body are relaxed including those in the gastrointestinal tract. This makes food move more slowly but can lead to gassiness. In addition expanding uterus places increased pressure on the rectum which can impair her muscles to control and lead to passing wind .There are no concerns for me about my mom passing gases .Most gas is controlled by the bacteria in large intestine working to break down the food which was not digested through enzymes in the intestines. If she tries to withhold the flatus she may experience intestinal cramps. To maintain the healthy functioning of the bowel the urge to move the bowel should not be ignored because the defecation reflex if ignored will not activate until it get stimulated again. Moving dry stools latter can involve straining and trauma which can lead to development of small tears in her rectum called anal fissures. Resorting to difficult and unsuitable exercises is not recommended when one is pregnant and is best

avoided. Sleeping in excess makes the offspring drowsy, dull and deficient in digestive power and because of this my mother was engaged in activities to keep herself busy. If at all she feels tired she takes cat naps in day time but she takes her full dose of sleep in the night time. Constantly sleeping on back is not recommended for my mother. As I become bigger my mom’s uterus gets heavier. It’s best to choose another position than lying on her back because when she lies on her back the weight of the uterus presses on the major vein that returns the blood from her lower body to her heart. Lying on her back for an extended period of time could make her feel dizzy. If my mom can easily talk over the sound it should not harm my hearing .If she has to shout to be heard the sound is considered to be too loud for me and will affect my hearing and she has to take special care especially when she is exposed to loud noise. Hearing of deafening sounds in excess is not considered safe for a pregnant lady. A foetus prolonged and repeated exposure to very loud noise e.g. 8hrs a day shift in an industrial workplace raises the odds of baby suffering from hearing loss especially at higher frequencies .Such prolonged noise exposure can also increase the risk of premature delivery and low birth weight babies . Resorting to difficult and unsuitable exercise is to be totally avoided .Moderate exercise by my mother can help not only for boosting my metabolism but also for her to have a successful labour, safe delivery and to get back to her normal body weight and shape faster .She has to use this measure --- If she is out of breath and cannot carry on a conversation while she is exercising she is over doing it But activities in moderation like walking and swimming are considered safe in pregnancy. The above factors which are responsible for the impairment of pregnancy if resorted to by the father before conception can result in vitiation of the sperm which later results in a conception and can impair a pregnancy . Sukshma Vyayama Budhi tatha Dhrti Shakti Vikasaka (Developing the mind and will power):Posture: With the feet togetiier, the body erect and the mouth closed, tilt the head back as far as possible and keep the eyes wide open. Exercise: Concentrating on the crown of the head, inhale and exhale rapidly and vigorously through the nose with the bellows method. Start with 5 rounds in the beginning. ¾¾ Smarana Shakti Vikasaka (Developing the memory) Posture: The same as in previous exercise (Buddhi Tatha Dhrishti-Shakti-Vikasaka) but / instead of tilting the head backward it has to be held in the normal position and the eyes must focus on a spot 5 feet (150 cm.) in front of the toes. Exercise: Concentrate on the Brahma-Randhra, which is the Yogic name for the area just under the anterior fontanel. Inhale and exhale vigorously through the nose (Bellow effects), 5 times to being with. ¾¾ Medha Shakti Vikasaka (Developing the Intellect) Posture: Close the eyes, keeping the feet together and back erect. The chin should be lowered to rest on the sternal notch. Exercise: According to Yogic science, the center of intellectual vigour is in the depression at the back of the neck. Concentrate on this with effort. Then start the bellows breathing exercise by inhaling and exhaling with equal force, 5 times in the beginning. ¾¾ Poorna Anguli Shakti Vikasaka (Strengthening the fingers) Posture: Stand with the feet close together and the body erect. Throw out the arms in front, keeping them parallel to the ground at shoulder level. Exercise A: Let the fingers form the shape of the hood of a cobra, taking particular care to stiffen the entire length of the arms from the shoulder-joints to fingertips. The exercise will not be effective if enough force is not put into it to make the arms tremble. Maintain posture for five minutes to begin with. Exercise B: posture the same as for ‘A’. Repeat exercise ‘A’, with the arms bent at the elbows. The fingers should be spread in the shape of a cobra’s elbows. The fingers should be spread in the shape of a cobra’s hood. Repeat vice versa in anti clock wise direction. ¾¾ Shoulder joint Exercise In feet together standing pose, bend your both elbows & keep your hand on the shoulders. Now rotate your elbows in a large circle clock wise ten times & anti clock wise ten times. Remember that while your elbows come in front of chest they touch each other. ¾¾ Knee joint Exercise Up & Down movement: - In feet together standing pose, while bending slightly forward keep your both hands on respected knees. Now bend your knees down & get up. Rotation: - In feet together standing pose, slightly bend your knees down & keep your both hand on knees. Now rotate both knees in clockwise & anti clockwise direction. ¾¾ Benefits of Yogic Sukshma Vyayama ªª Above exercises reduce mental fatigue. ªª These exercises improve memory. ªª All these three exercises reduce and cure various types of mental strain and nervous exhaustion. ªª Exercises land 3 cause congestion of blood in the head and brain due to interference with the venous return, due to sharp bending of neck. Since the aeration of the blood is simultaneously improved this ensures improved nutrition to the tissues of the brain. ªª Exercises 1 and 3 cause acute stretching and bending effects on the nerves of the neck, upper part of the spinal cord and the peripheral nerves supplying the organs of the neck. When you meet someone, normally what you do instinctly is to shake hands using your right hand. Not with left hand. We do almost all our daily activities in our right hands. But have you come across people who does writing, eating, batting, bowling and doing all their daily works in left hands? Such people who predominantly use their left hands are called ‘lefties’. It is also said that such people are generally intelligent. Of course, the achievements of these lefties are noteworthy. Several achievers in different field are left handers. Well-known Hollywood star Marylin Munroe, Rex Harrison, the one-eyed British actor (who was awarded Best Actor Award for the film ‘My Fair Lady’), world-famous Leonardo da’ Vinci - all are left handers. Jimmy Conners, Martina Navratilova the well-known sportspersons use their left hands. Monica Seles of Yogoslavia, Arjentina’s Soccer player Maradona - are all left handers and have made name in their own fields. The unfavorable associations and connotations of the use of the left hand among cultures are varied. In some areas, in order to preserve cleanliness where sanitation was an issue, the right hand, as the dominant hand of most individuals, was used for eating, handling food, and social interactions. The left hand would then be used for personal hygiene, specifically after urination and defecation. These rules were imposed on all, no matter their dominant hand. Through these practices, the left hand became known as the “unclean” hand. Currently, amongst Muslims and in some societies including Nepal and India it is still customary to use the left hand for cleaning oneself with water after defecating. The right hand is commonly known in contradistinction from the left, as the hand used for eating (Wikipedia). Although many such left handers are comfortable,

some people experience difficulties in some areas of activities. But, we who are right handers never bother about their problems. Wikipedia says: “Bias against left-handed people is discrimination, conscious or not, against people who are left hand dominant. Approximately 8–15% of the world’s population is left-handed. Yet many common tools are designed solely for use by right-handed people, making them difficult, painful, or unsafe for lefthanded people to use. These range from simple objects such as scissors to dangerous machinery such as power saws. Beyond such neglect, however, left-handed people have frequently been subjected to deliberate discrimination and disparagement. In many societies, they are considered unlucky or even malicious by the right-handed majority. Many languages use references to left-handedness to convey awkwardness, dishonesty, stupidity, or other undesirable qualities. Even in relatively “modern” societies, left-handed people historically have been—and in some places still are— forced from childhood to use their right hands for tasks which they would naturally perform with the left, such as eating and writing”. When such left handers use both their hands, fingers equally, they don’t face any problem. But not all can use both their hands very effectively. I have heard of a professor in a University ,who, long back used to write on the blackboard of the classs room in both his hands very legibly and equal speed. He would stand at the centre of the long blackboard and start writing in the left hand from the left hand side of the board and continue with his right hand. This he was doing with great ease. In our society, we use different kinds of equipments, vessels, musical instruments, sport items, etc. For example, screwing, fixing of a bulb, using scissors, computer key board usage, its mouse, some drilling, saw, drill press, band saw, textile machinery and many heavy machineries are made to operate by right

handers. According to some researchers, those using their left hands prominently face difficulties and get into trouble using such machineries made for right hand people. ¾¾ Do left handers live long? There is no authentic information about left handers living long life. There are several questions to be answered about left handers. ªª Is left handers genetic? In some families, yes, it is genetic. But in the long run, some people change over to right hands to be on par with others. ªª Some people start writing and doing their daily activities in left after they meet with any accident and become handicapped to use right hand. ¾¾ Writing by left handers ªª Imagine that there are 3-4 students sitting on a bench in a class room. A boy who uses his right hand is writing. Another boy sitting to his right is using his left hand to write. What happens here is – the left hand of the boy who is
writing in left hand is touching the right hand of the boy who is writing in his right hand. It becomes difficult for both of them to write freely. They have to sit quite apart. ªª Observe the way the left handers hold the paper or the book to write. They hold the paper slanted to 45 degrees and write. But they write quite neatly. ªª If all those in a bench are using their left hands for writing, they face no problems. When there are both left and right hand users in a single bench, there arises the problem. ªª In countries abroad, special scissors and pencil sharpeners are made to help left handers. Several student-friendly activities, equipments are available in foreign countries. Those using their left hands need not feel inferior. There are organizations abroad providing information about left handers. Gout or Commonly Known As Joints Inflammation Gout is one of the forms of arthritis or joint inflammation. Usually one or two joints are affected at a time. Commonly joints of feet and ankles are inflamed in gout. The commonest site is the ball of the big toe. The first attack of gout subsides in a week or so without treatment. There may be a long gap of many months or years between attacks. As time passes the attacks become more frequent and more severe involving many joints. When the frequency of attacks increases progressive joint damage sets in. This is known as chronic gout. This leads to disability and crippling. Gout is very rare in women but usually affects men. In women it is seen after menopause. ¾¾ Cause of Gout ªª Presence of uric acid crystals in joints causes inflammation of joints. ªª Uric acid is formed during metabolism of food and building up of body tissues. When the level of uric acid increases in blood than normal levels, the uric acid forms microscopic crystals in joints. These crystals lead to inflammation of joints. ªª In ayurveda Vaghbhata has said the cause of Gout is vitiated blood. The vitiated blood first affects the joints of feet. If you eat raw onion after meals, it can destroy the germs you might have ingested with drinking water. Roasted onion is a good remedy for dysentery and indigestion. Neuro-linguistic programming (or NLP) is an interpersonal communication model applied in psychotherapy and other contexts of communication and change.lt was cocreated by Richard Bandler and linguist John Grinder in the 1970s through observation and imitation of gestalt therapist Fritz Peris, family systems therapist Virginia Satir and psychiatrist Milton H. Erickson. What we say or do is not important but what we reaction we get back is to be analysed for gauging the method’s effectiveness. ‘Actions speak louder than words’. In NLP people are trained that when the two are in conflict, they should pay more attention to the actions. Choice is better than no choice. An idea from cybernetics that holds the most flexible element in a system will have the most influence or choice in that system. Present behaviour represents the very best choice available to a person. Therefore NLP creates new choices for the people so that they can act smarter. People already have all the internal resources they need to succeed.NLP just gives them that boost that they require to realise their own potential. Multiple descriptions in various perceptual contexts are better than only one. By doing so one can see a problem in new ways, or with less emotional attachment, and thus gather more information and develop new choices of response. This may also refer to other people and their view of a situation. Meet people in their own unique map of the world. This is based on the generalisation that an individual’s reality is not the same as any other’s and is linked to the principle that the map is not the territory. Language is a secondary representation of experience. The law of requisite variety or the first law of cybernetics states that in any cybernetic system the element or person in the system with the widest range of behaviours or variability of choice will control the system. Control in human systems refers to the ability to influence the quality of a person’s own and other people’s experience in the moment and through time. The person with the greatest flexibility of behaviour that is, the number of ways of interacting will control the system. All behaviour is geared towards adaptation. the context in which that behaviour originates. The behaviour a person exhibits is appropriate to their reality. All of a person’s behaviour, whether good or bad, is an adaptation. ªª ‘Possible in the world’ or ‘possible for me’ is only a matter of how. The process of determining ‘how’ you do it is called ‘modelling’, and it is the process by which NLP came into being in the first place. ªª The highest quality information about other people is behavioural. ªª It is necessary to make a distinction between behaviour and self.A person is not how he or she behaves. Behaviour is what a person says, does or feels at any moment in time. This is not a person’s self, however. A person’s self is greater than their behaviours. Healthcare in Karnataka is heading for a collapse Healthcare in Karnataka is heading for a collapse that could have disastrous consequences for the poor, especially in rural areas.
The healthcare fate of the poorest of the poor is being threatened for want of action by the Government of Karnataka.
Private and Charitable hospitals of Karnataka state are facing a major crisis due to rampant issue of BPL cards and in turn affecting the services under Vajpayee Arogyashree & Yeshasvini schemes.
Karnataka Private Hospitals’ Forum (KPHF) in association with Private Hospitals and Nursing Homes Association (PHANA) and Association of Healthcare Providers India (AHPI), Karnataka Private Medical Establishment Association (KPMEA) had a meeting of over 400 network Hospitals, including representatives of State IMA to resolve the current crisis. Hospitals have expressed their inability to continue treating beneficiaries of the Yeshasvini and Arogya Shree schemes. When these schemes were introduced 11 years ago, we embraced it, as it gave us an opportunity to serve the poor and working class people of Karnataka, offering them high quality healthcare, free of cost to them, yet sustainable for us through the schemes. Hospitals were willing to treat the scheme patients even at a loss, since it comprised a manageable component of our patient volume. By cross subsidising the schemes with paying patients, we managed to stay viable and sustainable, and continued offering quality healthcare to the poor. Over a period of time the issue of BPL Cards entitling these subsidized healthcare services has spiraled out of control. There are reportedly tens of lakhs of BPL cards issued to undeserving population in the state. It is these undeserving BPL Cardholders who avail most of the benefits of these schemes

by acquiring Yeshasvini and BPL cards, which they don’t deserve. Both schemes pay less than even the actual procedure cost, and the Government has not revised the charges since it’s introduction, while they have revised the insurance premium upwards three times in the same period under Yeshasvini scheme. In response to our representations, we have been given repeated assurances from the Yeshasvini and Arogya Shree Trusts that the price revision would be done. Yet no action has taken place for years on end. The fate of the poorest of the poor is at stake and disastrous consequences would follow, both to the people and the entire Healthcare industry, if the decision to revise tariff, lack of subject experts in TPA to decide upon per-auth’s and other issues are not taken urgently by the Government of Karnataka. Private Hospitals serving these schemes are currently becoming sick. Without active intervention they will soon die leaving the common man with no place to go. We are waiting for Chief Minister’s active intervention. Poor patients desperately needing these life saving operations need the Government Of Karnataka to act immediately and prevent the Hospitals from collapsing under the burden of unsustainable tariff, rules and procedures. Most of us don’t practice concrete planning methodologies for our works, and that is why we fail. To write success in our life or business, we must follow a definite course of actions - long terms or short terms. Long term plans can be easy to narrate, they are sensible and workable. For example, if we intend having a career, it is wise to work out a long term game plan for our respective industry or profession. And only then it is possible for us to foresee the progression, needed to make it to the position we aspire to hold. Work out what it takes to make those objectives. Work out what we seek in each step-gaining experience, handling responsibility, learning new skills, acquiring people management, understanding that sort of things. Work out how we can materialize each step, it doesn’t take much to work out what we need to acquire that how. Finally, we have to have an end game—the final goal. But it should be realistic and in tune with our capabilities, no more, no less. On the other hand, Short term plan vary according to our time span and urgency. For example we have short term plans of this month, this year, five years etc. Our one month short terms plan includes our current work projects, our one year plan schedules, projects which are being formulated, our five years plan lists, our dreams, ideas, wishes, goals etc.

School Aged children Vision Reading, writing, chalkboard work, and using computers are among the visual tasks students perform daily. A child’s eyes are constantly in use in the classroom and at play. When his or her vision is not functioning properly, education and participation in sports can suffer. As children progress in school, they face increasing demands on their visual abilities. The school years are a very important time in every child’s life. All parents want to see their children do well in school and parents do all they can to provide them with the best educational opportunities, But too often one important learning tool may be overlooked - a child’s vision. As children progress in school, they face increasing demands on their visual abilities. The size of print in schoolbooks becomes smaller and the amount of time spent reading and studying increases significantly. Increased class work and homework place significant demands on the child’s eyes. Unfortunately, the visual abilities of some students aren’t performing up to the task. When certain visual skills have not developed, or are poorly developed, learning is difficult and stressful, and children will typically: ªª Avoid reading and other near visual work as much as possible. ªª Attempt to do the work anyway, but with a lowered level of comprehension or efficiency. ªª Experience discomfort, fatigue and a short attention span. Because vision may change frequently during the school years, regular eye and vision care is important. The most common vision problem is nearsightedness or myopia. However, some children have other forms of refractive error like farsightedness (Hypermetropia) and astigmatism. In addition, the existence of eye focusing, eye tracking and eye coordination problems may affect school and sports performance. ªª Eyeglasses or contact lenses may provide the needed correction for many vision

problems. Vision Skills Needed For School Success: There are many visual skills beyond seeing clearly that team together to support academic success. Vision is more than just the ability to see clearly, or having 20/20 eyesight. It is also the ability to understand and respond to what is seen. Basic visual skills include the ability to focus the eyes, use both eyes together as a team, and move them effectively. Other visual perceptual skills include:  Recognition (the ability to tell the difference between letters)  Comprehension (to “picture” in our mind what is happening in a story we are reading), and  Retention (to be able to remember and recall details of what we read). ¾¾ Every child needs to have the following vision skills for effective reading and learning: ªª Visual acuity — the ability to see clearly in the distance for viewing the chalkboard, at an intermediate dis*^nce for the computer, and up close for reading a book. ªª Eye focusing — the ability to quickly and accurately maintain clear vision as the distance from objects change, such as when looking from the chalkboard to a paper on the desk and back. Eye focusing allows the child to easily maintain clear vision overtime like when reading a book or writing a report. ªª Eye tracking — the ability to keep the eyes on target when looking from one object to another, moving the eyes along a printed page, or following a moving object like a thrown ball.
