P

roper management of healthcare services in India is a much debated issue today. There is a significant missing piece in the complex puzzle of managing and delivering healthcare services to us as consumers. Although, we have a diverse and fragmented healthcare system of players such as hospitals, diagnostic centers, clinics, insurance providers who are rather loosely held together, there is no integrated nationwide healthcare system that helps in streamlining the delivery process. This lack of unified healthcare system has led to setting up of wrong objectives, wrong forms of competition, wrong strategies and structures. We are stuck between a situation of excessive use of healthcare services on the one end and an inability to access or afford on the other. The number of hospitals and doctors has increased significantly. However, significant number of patients does not seem to get enough medical care. The demand for these services is going to accelerate further, growing in line with the population growth also major health care services are concentrating on cities focusing on urban rich people. Therefore, there is a drastic need for a consumer centric healthcare system, where we individual consumers are empowered to take informed decisions. More and more patients are getting empowered with credible medical information available on the internet, in turn are demanding to be better informed about their specific conditions and seek the best treatment options available. A major premise of the consumer- driven healthcare is to place more responsibility on the end user to ensure both healthcare decision-making and expenditure control. In theory, this makes a lot of sense. In practice, at least for some of those consumers who are well educated and fairly informed, this is already yielding better financial and healthcare outcomes. But unfortunately, most consumers are hardly equipped or for that matter, don’t even want to navigate the healthcare system on their own. Another critical aspect is when we have ill-prepared consumers representing themselves without adequate support; the result is bound to be greater disaster than what it is today. The healthcare system is complex and if consumers are supposed to play a more active role, they will need a good deal of help and support. A people friendly health care system is need of the hour.

INGUINAL HERNIA

n inguinal hernia occurs when soft tissue — usually part of the intestine — protrudes through a weak point or tear in your lower abdominal wall. The resulting bulge can be painful — especially when you cough, bend over or lift a heavy object. Not necessarily dangerous by itself, an inguinal hernia doesn’t get better or go away on its own. An inguinal hernia can lead to life-threatening complications. For this reason, your doctor is likely to recommend surgical repair of an inguinal hernia that’s painful or becoming larger. Inguinal hernia repair is a common surgical procedure.

If you still aren’t able to push the hernia in, the herniated intestine may have become trapped (incarcerated) in the abdominal wall — a serious condition that may require immediate medical attention. This condition may be accompanied by nausea, vomiting, fever, rapid heart rate, sudden pain that quickly intensifies, and a hernia bulge that turns red, purple or dark. If any of these signs or symptoms occur, call your doctor right away.

Symptoms

Causes

Some inguinal hernias don’t cause any symptoms, and you may not know you have one until your doctor discovers it during a routine medical exam. Often, however, you can see and feel the bulge created by the protruding intestine. The bulge is usually more obvious when you stand upright, especially if you cough or strain. ¾¾ Inguinal hernia signs and symptoms include: ªª A bulge in the area on either side of your pubic bone ªª A burning, gurgling or aching sensation at the bulge ªª Pain or discomfort in your groin, especially when bending over, coughing or lifting ªª A heavy or dragging sensation in your groin ªª Weakness or pressure in your groin ªª Occasionally, in men, pain and swelling in the scrotum around the testicles when the protruding intestine descends into the scrotum ¾¾ Signs and symptoms in children Inguinal hernias in newborns and children result from a weakness in the abdominal wall that’s present at birth. Sometimes the hernia may be visible only when an infant is crying, coughing or straining during a bowel movement. In an older child, a hernia is likely to be more apparent when the child coughs, strains during a bowel movement or stands for a long period of time. ¾¾ When to see a doctor See your doctor if you have a painful or noticeable bulge in the area on either side of your pubic bone. The bulge is likely to be more noticeable when you’re standing upright, and you usually can feel it if you put your hand directly over the affected area. You should be able to gently and easily push the hernia back into your abdomen when you’re lying

Inguinal hernia Some inguinal hernias have no apparent cause. But many occur as a result of: ªª Increased pressure within the abdomen ªª A pre-existing weak spot in the abdominal wall ªª A combination of increased pressure within the abdomen and a pre-existing weak spot in the abdominal wall ªª Straining during bowel movements or urination ªª Heavy lifting ªª Fluid in the abdomen (ascites) ªª Pregnancy ªª Excess weight ªª Chronic coughing or sneezing

In many people, the abdominal wall weakness that leads to an inguinal hernia occurs at birth when the abdominal lining (peritoneum) doesn’t close properly. Other inguinal hernias develop later in life when muscles weaken or deteriorate due to factors such as aging, strenuous physical activity or coughing that accompanies smoking. In men, the weak spot usually occurs along the inguinal canal. This is the area where the spermatic cord, which contains the vas deferens, the tube that carries sperm, enters the scrotum. In women, the inguinal canal carries a ligament that helps hold the uterus in place, and hernias sometimes occur where connective tissue from the uterus attaches to tissue surrounding the pubic bone. ¾¾ More common in men Men are more likely to have an inherent weakness along the inguinal canal because of the way males develop in the womb. In the male fetus, the testicles form within the abdomen and then move down the inguinal canal into the scrotum. Shortly after birth, the inguinal canal closes almost completely, leaving just enough room for the spermatic cord to pass through, but not large enough to allow the testicles to move back into the abdomen. Sometimes, however, the canal doesn’t close properly, leaving a weakened area. There’s less chance that the inguinal canal won’t close after birth in female babies. In fact, women are more likely to develop hernias in the femoral canal, an opening near the inguinal canal where the femoral artery, vein and nerve pass through. Weaknesses can also occur in the abdominal wall later in life, especially after an injury or certain operations in the abdominal cavity. Whether or not you have a pre-existing weakness, extra pressure in your abdomen from straining,

heavy lifting, pregnancy or excess weight can cause a hernia.

Risk factors ªª Being male. You’re far more likely to develop ªª ªª

an inguinal hernia if you’re male. Also, the vast majority of newborns and children with inguinal hernias are boys. Family history. Your risk of inguinal hernia increases if you have a close relative, such as a parent or sibling, with the condition. Certain medical conditions. Having cystic fibrosis, a life-threatening disorder that causes severe lung damage and often a chronic cough, makes it more likely you’ll develop an inguinal hernia. Chronic cough. A chronic cough, such as occurs from smoking, increases your risk of inguinal hernia. Chronic constipation. This leads to straining during bowel movements — a common cause of inguinal hernias. Excess weight. Being moderately to severely overweight can put extra pressure on your abdomen. Pregnancy. This can both weaken the abdominal muscles and cause increased pressure inside your abdomen. Certain occupations. Having a job that requires standing for long periods or doing heavy physical labor increases your risk of developing an inguinal hernia. Premature birth. Infants who are born sooner than normal are more likely to have inguinal hernias. History of hernias. If you’ve had one inguinal hernia, it’s much more likely that you’ll eventually develop another — usually on the

your doctor include: What is likely causing my symptoms? Are there any other possible causes for my symptoms? ªª What kinds of tests do I need? ªª Do I need treatment? ªª If you recommend a watch-and-wait approach, how will you monitor my condition? ªª If you recommend surgery now, what type or procedure is right for me? ªª What should I expect during my recovery from surgery? ªª What is the risk of complications from surgery? ªª What is the risk of my symptoms recurring? ªª Is there anything I can do to prevent a recurrence of this problem? ªª I have these other health conditions. How can I best manage them together? ªª Are there any restrictions that I need to follow? ªª Should I see a specialist? What will that cost, and will my insurance cover it? ªª Are there any brochures or other printed material that I can take with me? What websites do you recommend? In addition to the questions that you’ve prepared to ask your doctor, don’t hesitate to ask questions during your appointment at any time that you don’t understand something. What to expect from your doctor Your doctor is likely to ask you a number of questions. Being ready to answer them may reserve time to go over points you want to talk about in-depth.

Incarcerated hernia. This complication of an inguinal hernia occurs when a loop of intestine becomes trapped in the weak point in the abdominal wall. This may obstruct the bowel, leading to severe pain, nausea, vomiting and the inability to have a bowel movement or pass gas. Strangulation. When part of the intestine is trapped in the abdominal wall (incarcerated hernia), blood flow to this portion of the intestine may be diminished. This condition is called strangulation, and it may lead to the death of the affected bowel tissues. A strangulated hernia is life-threatening and requires immediate surgery.

Preparing for your appointment

You’re likely to start by seeing your family doctor or a general practitioner if you have an inguinal hernia. However, in some cases when you call to set up an appointment, you may be referred immediately to a digestive disorders specialist called a gastroenterologist or to a general surgeon. Because appointments can be brief, and because there’s often a lot of ground to cover, it’s a good idea to be well prepared. Here’s some information to help you get ready and learn what to expect from your doctor.

What you can do ªª Be aware of any pre-appointment

restrictions. At the time you make the appointment, be sure to ask if there’s anything you need to do in advance. ªª Write down any symptoms you’re experiencing, including any that may seem unrelated to the reason for which you scheduled the appointment. ªª Write down key personal information, including any major stresses and recent life changes. Also include your diet and exercise habits, including whether your job involves physical labor. ªª Make a list of your key medical information, including other conditions you’re being treated for and the names of the medications that you’re taking. Include on your list every prescription and over-the-counter drug you use, as well as any vitamins and supplements. ªª Take a family member or friend along. Sometimes it can be difficult to absorb all the information provided during an appointment. Someone who accompanies you may remember something that you missed or forgot. ªª Write down questions to ask your doctor. Your time with your doctor is limited, so preparing a list of questions can help you make the most of your time together. List your questions from most important to least important in case time runs out. For inguinal hernias, some basic questions to ask

Your doctor may ask: ªª When did you begin experiencing symptoms? ªª Have your symptoms stayed the same or gotten worse? ªª Do you have pain in your abdomen or groin? Does anything make the pain feel worse or better? ªª Do you notice a bulge in your groin when you stand up, cough, strain or lift heavy objects? ªª Have you been diagnosed or treated for any other medical conditions? When? ªª What physical activity do you perform on your job? ªª What other physical activities do you regularly engage in? ªª Do you have a history of constipation? ªª Have you had a previous inguinal hernia? ªª Have any of your close relatives had an inguinal hernia? ªª Do you or did you smoke? If so, how much? What you can do in the meantime While you’re waiting for your appointment, get emergency medical care if you develop nausea, vomiting or a fever, or if your hernia bulge turns red, purple or dark.

The procedure is a good choice for people whose hernias recur following traditional hernia surgery because laparoscopic methods allow surgeons to work around scar tissue from earlier repair. It’s also good for people who have hernias on both sides of the body (bilateral inguinal hernias). ªª Disadvantages of laparoscopic repair include an increased risk of complications and of recurrence following surgery. These risks are reduced if the procedure is performed by a surgeon with extensive experience in these kinds of repairs. You may not be a candidate for laparoscopic hernia repair if you have a very large hernia, if your intestine is pushed down into the scrotum or if you’ve had previous pelvic surgery such as a prostatectomy.

Prevention

You can’t prevent the congenital defect that may lead to an inguinal hernia, but the following steps can help reduce strain on your abdominal muscles and tissues: ªª Maintain a healthy weight. If you think you may be overweight, talk to your doctor about the best exercise and diet plan for you. ªª Emphasize high-fiber foods. Fresh fruits and vegetables and whole grains are good for your overall health. They’re also packed with fiber that can help prevent constipation and straining. ªª Lift heavy objects carefully or avoid heavy lifting altogether. If you have to lift something heavy, always bend from your knees, not from your waist. ªª Stop smoking. In addition to increasing your risk of serious diseases such as cancer, emphysema and heart disease, smoking often causes a chronic cough that can lead to or aggravate an inguinal hernia. ªª Don’t rely on a truss, a supportive garment designed to keep hernias in place. Contrary to what you may have heard, wearing a truss isn’t the best long-term solution for an inguinal hernia. A truss won’t protect against complications or correct the underlying problem, although your doctor may recommend wearing one for a short time before surgery.

Tests and diagnosis

A physical exam is usually all that’s needed to diagnose an inguinal hernia. Your doctor is likely to ask about your signs and symptoms and to check for a bulge in the groin area. Because standing and coughing can make a hernia more prominent, you may be asked to stand up and cough or strain as part of the exam.

Treatments and drugs

If your hernia is small and isn’t bothering you, your doctor may recommend a watch-and-wait approach. But enlarging or painful hernias usually require surgical repair to relieve discomfort and prevent serious complications. There are two general types of hernia operations: ªª Herniorrhaphy. In this procedure, also called “open” hernia repair, your surgeon makes an incision in your groin and pushes the protruding intestine back into your abdomen. Then your surgeon repairs the weakened or torn muscle by sewing it together. Often the weak area is also reinforced and supported with a synthetic mesh, a procedure called hernioplasty. After the operation, you’ll be encouraged to move about as soon as possible, but it may be as long as four to six weeks before you’re able to fully resume your normal activities. ªª Laparoscopy. In laparoscopic surgery, your surgeon uses several small incisions rather than one large one. A fiber-optic tube with a tiny camera is inserted into your abdomen through one incision, and miniature instruments are inserted through the other incisions. Your surgeon then performs the operation using the video camera as a guide. In laparoscopic surgery, synthetic mesh is always used to repair the hernia (hernioplasty). ªª Advantages of laparoscopic repair include less discomfort and scarring after surgery and a quicker return to normal activities — most

ENDOMETRIOSIS ¾¾

What is endometriosis? The Word “endometriosis” (Say: “end-oh-metree-oh-sis”) means a problem with the lining of the uterus. The lining is called the endometrium. Normally, every month when you have your periods, the lining (which is also called endometrial tissue) comes out in menstrual flow. Endometrial tissue is usually only inside your uterus. However, if you have endometriosis, tissue from the lining of the uterus moves through the fallopian tubes and gets on your ovaries, in your pelvis, on your bladder or in the other areas. When you have your periods, this tissue swells and bleeds, just like the lining of your uterus. This is often painful, and scar tissue can form in your pelvic area. ¾¾ Is endometriosis harmful to me? Endometriosis may hurt, but it doesn’t cause cancer or affect your overall health, although it might make it harder to get pregnant. ¾¾ What will be my symptoms be if I have endometriosis? If you have painful periods, severe cramping and pain during sex, low back pain or rectal pain, you may have endometriosis. The only way for your doctor to be sure is to a laparoscopy (say:”lap-ah-ross-ca-pee”). This is away of looking inside your uterus by making a small cut in your skin and putting a thin tube inside. If your doctor decides to perform this procedure, you will be given medicine so you won’t feel pain. ¾¾ How can we treat endometriosis? A number of medicines might help with your pain. Sometimes it helps to take birth control pills.

Another medicine that might helps is along - acting progestin (this is a hormone that comes in a shot; it’s also used for birth control). Other medicine used is a monthly shot of a hormone called a GnRH analog. ¾¾ How do these medicines work? These medicines stop your periods. They may cause side effects like the ones women have at menopause. These include hot flashes and vaginal dryness. Danazol might cause acne and unwanted facial hair. Sometimes the side effect of the GnRH analog goes away if you also take an estrogen pill every day. ¾¾ What is the duration of the treatment? Birth control pills, danazol and GnRH analogs are taken for 4 to 6 months. Pain is usually relieved during the period of medication. After you stop taking the medicine, the pain may come back. ¾¾ How can it be diagnosed?

Why cervical carcinoma? Uterine cervical cancer is the second most common cancer in women worldwide and induced by sexually transmitted agents. Early detection and treatment results in a lower rate of advanced cancers and cervical cancer deaths. ¾¾ What causes cervical cancer? HPV is the primary cause of cervical cancer. More than 80 HPV types have been identified and more than 35 distinct types are known to infect the genital mucosal epithelium. According to the World Health Organization (WHO), the offending strains of HPV are mainly types 16, 18, 31 and 33. Among these strains of HPV, the most clinically important, causing the majority of cervical carcinomas are HPV-16 and 18. ¾¾ How does HPV cause cervical carcinoma? In cervical cells, HPV can occur in episomal form, integrated form, or both. Viral integration in the human genome often happens in the viral E1 or E2 region and can result in the loss of negative feedback control of oncogene expression by the regulatory E2 protein. ¾¾ How would one identify cervical carcinoma? The current standard of care for the early diagnosis of cervical cancer by screen uses the cytology method (Pap smear). However, this method has important limitations, false negatives being the most significant. The establishment of HPV as central and necessary cause of cervical cancer was exploited for the development of molecular technologies for viral detection to overcome limitations linked to cytological cervical screening. ¾¾ Why prefer DNA based method (genotyping)? HPV cannot be grown in conventional cell cultures, and serological assays have only limited accuracy. Accurate HPV genotyping is essential for adequate classification of patients into low-risk or high-risk groups and HPV viral load may also be a valuable predictor of disease although currently accurate quantitative viral load measurements are technically difficult in clinical samples. ¾¾ What is the advantage with molecular diagnosis? Molecular detection of HPV DNA or RNA is currently the gold standard for identification of HPV viruses since HPV DNA has been detected in 99.7% of studied cervical cancer cases and it has been shown to have higher sensitivities (84–100%) than the conventional Pap test. The extremely high sensitivity and specificity of PCR assays and the need of only small amounts of DNA template (tissue sample), makes it the support of several other highquality assays.

Insect bites viz, honey bee; juice can be applied locally. Rheumatism: The paste of the leaf is applied locally. Urinary calculi: Can be controlled if the juice is taken regularly. Headache: paste of the leaf if applied to the forehead reduces ache in 15 minutes.

Seasoning with mustard is optional. ¾¾ Chutney Grind the cut leaves along with pepper, red chillies, dry copra, tamarind juice, and salt. It is a very good remedy for cough & fever. ¾¾ Bajji: Basin, Rice flour, Chilli powder, Salt to taste, Ghee 1 spoon Mix all the ingredients into a thick consistency. Dip the washed borage leaf in this mix. Deep fry till it is golden brown. ¾¾ Borage Rice: ªª Borage leaves 15 ªª Onion 1 ªª Blackgram dhal 2 spoons ªª Mustard ½ spoon ªª Coriander ªª Grated coconut ªª Salt to taste ªª Rice 1 cup ªª Green chillies 5 ªª Bengalgram dhal 2 spoons ªª Curry leaves ªª Groundnut ªª Lemon ªª Ghee/oil Prepare the rice. Season it with mustard, dhals and chillies. Roast cut pieces of onion and borage leaves in little oil or ghee. Mix with rice along with lemon juice, salt, coriander, curry leaves and grated coconut. ¾¾ Thokku: (preservable chutney) Borage leaves 4 handsful , Red chillies 10, Tamarind, Garlic flakes, Mustard, Asafoetida, Cumin seeds, Oil, Jaggery, Salt, Ginger Wash the leaves and dry them with a clean cloth. Grind all the ingredients to a paste. Season with mustard cumin seeds and asafoetida. Heat the contents till all the water content evaporates. Cool and store in a clean bottle.

For this place the eye cup with water and open your eyelids wide rotate the eye ball to ensure thorough bathing of the eye. .Another method to get dirt out of your eye is to plunge your face into a basin containing clear water. Hold your breath and open your eyes wide .Move your face around the water to help dislodge any bits of grit.

A cataract is a clouding of the lens in the eye, which can affect vision.

¾¾

What is Lens? The human lens is a clear structure that is located behind the iris & pupil and has an important function in vision. It works much like a camera lens. In a normal eye, it focuses rays of light into the retina to allow image formation. The retina is the light sensitive tissue at the back of the eye. The lens is able to change its shape, and hence it can focus objects at different distances, letting us see things clearly both up close and far away. Loss of transparency of the lens and resultant opacity leads to blurring of vision. ¾¾ What is cataract? A cataract is a clouding of the lens in the eye, which can affect vision. Cataract can occur in either or both eyes. It cannot spread from one eye to other. ¾¾ What are the types of cataract in children? 1. Childhood cataract: Some babies are born with cataracts (called as congenital cataract) or develop them in childhood (developmental cataract) often in both eyes. These cataracts may be so small that they do not affect vision. If they do, they may need to be removed. 2. Secondary cataract: Cataracts also can develop in children who have other health problems, such as rubella, diabetes, etc., It could occur with ocular problems like microophthalmos (small eye), etc. 3. Traumatic cataract: Cataracts can develop after an eye injury, sometimes years later. ¾¾ What causes cataracts in children? Any disruption of the normal structure of the lens can result in clouding. This could be either caused by: Accumulation of abnormal breakdown products (metabolites) with retention of water within the lens or Interruption of normal development of lens by a genetic error of following infection with viruses, radiation and drugs. In our country infection of pregnant ladies with Rubella virus & consanguineous marriages (marriages within relatives) are the most common causes. The chances of having congenital cataract are particularly high if there is a family history of cataract. ¾¾ How can cataract affect my child’s vision? In the case of a newborn infant, a cataract causes the immature visual system to be deprived of the stimulation needed for normal development. If left untreated, permanent visual loss (Lazy Eye) may occur. ¾¾ What are the effects of cataract? If the cataract is small, there may be only

slight blurring of vision with near normal visual development. Frequently, though, the impairment is more severe and can lead to permanent amblyopia (lazy eye). Without adequate stimulation central vision can be permanently impaired. Some peripheral vision usually remains and the eye does not usually lose the ability to distinguish light and dark. Congenital cataracts can affect one eye only or both eyes. In bilateral cases the cataract can be denser in one eye than other. ¾¾ How do I know that my child has cataract? As cataract is an opacification of the lens, there is a white reflex seen in the centre that is in the papillary region. This could be either seen by the paediatrician or child’s parents. Infect this reflex may also be picked up in photographs. The absence of an eye contact is an important sign that should alarm the parents. If the child does not respond to visual stimuli like picking up objects, or recognizing faces or has constantly wandering or moving eyeballs after first 2-3 months, the parents should immediately consult a paediatric ophthalmologist. ¾¾ What is the Treatment? The aim of the surgery to remove the cloudy lens and clear the visual axis (the central part of the eye) so that the light rays can easily enter the eye and produce a crisp image on the retina. Intraocular lenses are becoming more common in children. Putting an intraocular or artificial lens will depend on the age of the child, status of the eye, presence of any ocular problem. However, they cannot be implanted in very young children as the eyeball is small in size and still growing. Even if the lens is not put at the time of cataract surgery. It can definitely be

considered as an option when the child grows older. The surgery is performed under general anesthesia. To serve the purpose of improving vision of child, optical correction with a contact lens and or glasses is necessary. Cataract Surgery in a child is technically more difficult in an adult &should be performed only by a surgeon with adequate experience in pediatric cataract surgeries if surgery to only one eye is required, the issue of amblyopia therapy becomes very important for the final visual outcome. ¾¾ What is the ideal time for performing cataract Surgery? Because a clear visual axis is essential for early visual development, a diagnosis within the first few weeks of life is important. Surgery has to be done as early as possible to improve the chances of maximum visual recovery. It can be done as early as 2 days after birth provided there is no risk for general anesthesia due to systemic problems.

Is surgery the end of all the problems? No, though surgery does clear the visual axis, the child may develop lazy eye, misalignment of eyes, shaking of eyes (nystagmus), development of refractive errors, and late complications of surgery. Lazy eye (amblyopia) may be seen in one or both eyes and would require treatment for the same depending on the case, like patching, eye drop instillation and even vision stimulation exercise. Misalignment of eyes may require surgical intervention later in life. Late complications of surgery that can occur include membrane formation in pupillary axis, increase in intraocular pressures (glaucoma) with subsequent optic nerve damage and retinal detachment. However, with newer surgical techniques, the incidence of these complications has significantly dropped. Hence following surgery monitoring of the vision, refractive error, development of complications etc., is mandatory and repeated follow-ups are warranted in an attempt to achieve good visual rehabilitation. In short, it is not the end but the beginning, requiring a long association between the pediatric ophthalmologist and the child along with the parents. ¾¾ What is the final prognosis? The eventual outcome is very much dependent on the type of cataract, age of the child & the timing of surgery. Since timely management results in significant visual improvement and hence a near normal child, it is important that the parents should be highly motivated and act fast. The important point to remember is that early diagnosis & treatment is the key to successful visual rehabilitation in childhood cataracts.

It’s a crippling disease that can spoil the pleasures of retirement and lead to an uncomfortable, inactive old age. As people live longer, osteoporosis will become more of a problem for both the elderly and for those who take care of them. Based on 2001 census, approximately 163 million Indians are above the age of 50 and this is expected to increase by 230 million by 2015. According to the World Health Organization, the prevalence of osteoporosis among U.S. white women past menopause is estimated to be 14% in those between 50-59years of age, 22%in those between 60-69years of age, 39%in those between 7079years of age and 70% in those around 80 years of age and even older and white and Asian racial groups are said to be at greatest risk. In India, osteoporosis is likely to emerge as an important health problem in decade or so with trends of rapid urbanization, new occupational fields like industries and detrimental lifestyle habits and the osteoporotic risk fractures are

said to occur 10 to 15 years before than the western counter parts. Because osteoporosis causes bone degeneration which leads to loss of height over time and also back pain, hence, early on prevention is to be kept in mind as the treatment. Osteoporosis is a systemic skeletal disease characterized by low bone mass and micro

architectural deterioration of bone tissue, with a consequent increase in bone fragility and susceptibility to fractures. As the name implies, the bone becomes abnormally porous and thin. The reduced mass weakens the mechanical strength of the bone, thus making it much more likely to break often with little or no trauma. Although, considered to be a disease of the ageing, it starts very early particularly in women. Types There are 3 types of osteoporosis as Type I (Postmenopausal osteoporosis) – This

is characterized by the acceleration of bone loss beginning with the onset of menopause. This accelerated bone loss may continue for up to 20yrs after the onset of menopause. In this condition, the trabecular bone is lost three times faster than the cortical bone. This results in vertebral crush fractures, forearm and ankle fractures subsequently. Here, estrogen deficiency plays a major role in the acceleration of bone loss due to menopause as the estrogen receptors are present on bone and are

directly involved in controlling the rate of bone remodeling. Type II (Senile osteoporosis) – This is mainly due to the effect of ageing. Here, bone loss occurs in both trabecular and cortical bone to the same extent. Though the rate of bone loss is average 1% per year with an equal rate of loss between both the sexes, women are at a higher risk of developing type II osteoporosis than men. Type III (Secondary osteoporosis) – This is a condition that develops as a secondary manifestation of another disease. Endocrine abnormalities such as diabetes mellitus, hyperthyroidism etc, Malignant conditions such as multiple myeloma, lymphoma etc, Genetic abnormalities, Hepatic disorders, Nutritional deficiencies like calcium and protein deficiencies and Rheumatoid arthritis are some of the diseases that leads to secondary osteoporosis. This results due to an increase in the rate of bone resorption or a decrease in bone formation or a combination of both the factors. ¾¾ Risk factors for Osteoporosis As the age advances, our bones tend to become thinner and porous leading to osteoporosis. But there are certain factors that may accelerate or decelerate this process and therefore it is important to become aware of those factors and take a step towards its prevention. These are as follows: 1. Menopause- In the post menopausal period, ovaries stop producing the estrogen that is said to be very important for the maintenance of bone strength. However, early menopause before the age of 45yrs will deny the bones from receiving important benefits of estrogen much earlier than usual and hence increases the risk of developing osteoporosis. 2. Family history- The affected person with parents having or had osteoporosis will be at greater risk. Genetics play a vital role in determining the susceptibility to osteoporosis. 3. Medical history- As discussed earlier, some diseases leads to secondary osteoporosis. Medications used for longtime also have a detrimental effect on the bones including the steroids. 4. Diet- Calcium is the major mineral related to bone health and its requirement has to be fulfilled by consuming calcium rich foods. Person with calcium deficiency will be at increased risk of developing osteoporosis. 5. Lack of Physical activity- Bones need a certain amount of exercise to stay strong and healthy. People with inactive lifestyle are at a higher risk than who are moderate to regular

exercises. 6. Smoking/Alcohol- In addition to the health risks associated with smoking and high alcohol, they also increase the rate of bone loss making susceptible to osteoporosis. 7. Aerated/soft drinks- This contains the phosphoric acid that makes the blood acidic and the body to release bone calcium as a buffer and leads to osteoporosis. 8. Low body weight- Women who are unusually slender may beat an increased risk of osteoporosis as their skeletal frame will be small. Once they cross the age of menopause, they reach a stage where they fracture their bones more easily than women who have a normal build. ¾¾ Fracture sites due to Osteoporosis In this condition, the trabecular bone will be affected the most and this type of bone is found in the Spine, Bones of the Hip joint, Wrists and Ribs. ¾¾ Investigations As this is a silent disease, its signs and symptoms go unnoticed and most of the time only after a break, it gets noticed. Bone densitometry (BMD) is a measure that is used to measure the bone mineral density or the mineral content over an area of the bone. The densitometry predicts the risk only and not the fracture. There are several methods of measuring BMD such as 1. Dual-Energy X-ray Absorptiometry (DEXA) – This is the most accurate method to measure BMD and is considered as the gold standard in the diagnosis of osteoporosis. Osteoporosis is diagnosed when the BMD is less than or equal to 2.5 standard deviations below that of a young adult reference population. This is translated as T-score. 2. Peripheral dual energy X-ray

Absorptiometry (P- DEXA) 3. Quantitative Ultrasound (QUS) 4. Bio-chemical test- These are the test’s done to support the bone mineral density test and they are serum calcium, serum phosphorus, alkaline phosphatase and vitamin D test.

Management The aim of osteoporosis treatment is the prevention of bone fractures by controlling bone loss and by increasing the bone density. There are several modes of treatment for osteoporosis such as ªª Estrogen Replacement Therapy (ERT): This can be taken either as a pill or a skin patch. Estrogen given alone can increase the risk of developing endometrial cancer and therefore is usually given in combination with progesterone. This helps in reducing the bone loss and increase bone density in spine and hip joints. ªª Calcitonin: This is a hormone of thyroid gland which regulates the calcium in bones. This is generally indicated for women who cannot go for ERT. This can be administered either orally, as an injection or the nasal spray. ªª Bi-phosphates: These are widely used for the treatment and prevention of osteoporosis in post menopausal women. Alendronate is a biphosphate that has been approved by the FDA for the treatment of osteoporosis in menopausal women. ªª Calcium supplementation: Calcium supplements are given in the form of calcium carbonate, calcium citrate and calcium phosphate. Calcium carbonate taken along with the food is best absorbed. Normally, 500mg of calcium is advised to be taken twice a day.

Vitamin D supplementation Prevention Once you are screened for osteoporosis and said to have either osteopenia or osteoporosis, medication alone will not help. A proper diet rich in calcium along with alkaline foods, a good exercise regime, exposure to sunlight for a minimum of 15 minutes, Ayurvedic treatment and modified therapeutic yoga will be a holistic and a healthy approach. Early detection and treatment can reduce the risk of osteoporosis related fractures. However, it is not possible to completely cure osteoporosis; therefore prevention is always a better option. A few changes in your daily routine can considerably bring down the risk of osteoporosis at any point of life. ªª Good Dietary plan: This is very important throughout the life to prevent osteoporosis and other health disorders. Calcium plays a significant role in strengthening the bones and thereby decreasing the bone loss and the risk of fractures after the age of 30yrs. Foods rich in calcium include milk, milk products, green leafy vegetables, fish, ragi, soya etc which has to be consumed daily. Vitamin D is a key to open the door to deposit the calcium in bone. Vitamin D in conjunction with calcium plays a vital role in maintaining the bone mass and strength. Food sources of vitamin D include fish (Salmon, Mackerel, Sardine and Tuna) eggs, mushrooms and fish liver oils. ªª Physical activity: Experts recommend regular physical activity such as exercise to do for at least 30 mins three times a week. This helps to maintain the ideal body weight thereby reducing the overload on our skeletal system. Physical activity reduces the risk of hip fractures in older women as a result of increased muscle strength. Weight bearing exercises improve bone mass in premenopausal women and helps to maintain BMD after menopause. ªª Exposure to sunlight: Getting enough exposure to sunlight daily can help maintain healthy bone density and strength. ªª Smoking: As per the research studies, in order to prevent osteoporosis, it is strongly recommended to quit the smoking as early as possible, as the smoking is associated with early menopause and a high risk of osteoporosis. ªª Alcohol consumption: Consumption of excessive alcohol leads to loss of calcium and thereby causes early osteoporosis. ªª Ayurveda: Ayurveda, a 5000yrs old ancient science has a vast description of rasayana’s and herbal preparations that can supply the body with sufficient nutrients and prevent the early ageing and thereby the early osteoporosis which has been proved by the research studies. ªª Yoga: According to several research studies published on yoga and osteoporosis, yoga has shown significant improvement on pain and improved the quality of life of the patients affected with osteoporosis. Sleep as much as needed t3 feel refreshed during the following day (usually 8 hours). Sleeping too much is as detrimental as sleeping too little. Get up at the same time each day, seven days a week. (A regular wake-up time in the morning leads to regular times of sleep onset.) Avoid caffeinated beverages in the evening. No coffee or tea after 6pm, switch to decaffeinated soft drinks. Avoid alcohol in the evening. Although alcohol helps tense people fall asleep more easily, the second half of the night is much disrupted. Evening alcohol also increases snoring. A steady amount of activity during the day is conducive to sleep at night. Insulate your bedroom against random outside noises, block out light, and avoid drastic changes in room temperature. Excessively warm rooms will disturb sleep; keep the room temperature normal. Hunger may disturb sleep. A light snack at bedtime may help sleep. Avoid liquids during the two hour period prior to bedtime, in order to minimize the need for night time trips to the bathroom.

Keep your lipsticks and make up pencils in the fridge, this prevents them from breaking. Apply foundation on lip before applying lipstick, this way lipsticks stay for a longer time. Using a lip liner can also make the lipstick stay for long. Apply a tooth paste containing calcium carbonate on pimples. Matte lipstick will stay for a longer period of time as compared to gloss lipstick. Rubbing ice over face before applying foundation makes make up stay for longer. Alternatively try washing the face with cold water. The right time to tweeze eye brows is immediately after a bath when hair follicles are open. This makes plucking less painful and easy. Never share personal toiletries or products. Do not apply deo, roll - on stick, etc. on wet skin because it will get sticky and less effective. Apply coconut oil with orange peel powder all over the body before taking bath. Apply a paste of fresh fenugreek (methi) leaves for 10-15 minutes and wash with warm water. Patient-doctor support group programme for kidney transplant patients

On the ocassion of Columbia Asia Referral Hospital, Yeshwanthpur’ – Renal Transplant Patient Support Group meeting, (From left to right: His Royal Highness Shri Kanta Datta Narasimha Raaja Wadiyar, Maharaja of Mysore, Dr Nandakumar Jairam, Chairman and Medical Director, Columbia Asia Hospital, Padma Shri, Dr Ramana Rao, Physician and Cardiologist) lives. Mr. Chetan was able to walk a couple of days after the surgery and is now ready for discharge just a week after undergoing a major surgery. His family and he are extremely relieved that he has done so well after the surgery. Spinal deformities are often missed in children due to lack of awareness. Even if they are detected, they tend to be neglected till the situation worsens for the fear of complications. Results of treatment are far better if the patient comes to medical attention early. A study on pregnancy by Bangalore Assisted Conception Centre (BACC)

shown that if a woman is isolated from all other men except her husband for three full days after embryo transfer following in vitro fertilization (fVF) or intracytoplasrmc sperm injection (ICSI), the chances of establishment of pregnancy increase by 50% - that is, from 30% to 45%. From this study, it can be extrapolated that in the case of normal sexual intercourse, the chances of successful pregnancy would increase if the woman is kept isolated as above for six days, which is the time required for implantation of the egg of the
