umpectomy (lum-PEK-tuh-me) is surgery to remove cancer or other abnormal tissue from your breast. Lumpectomy is also called breast-conserving or breast-sparing surgery because — unlike a mastectomy — only a portion of the breast is removed. Doctors may also refer to lumpectomy as a wide local excision. During lumpectomy, a small amount of normal tissue around the lump (clean margins) also is taken to help ensure that all the cancer or other abnormal tissue is removed. Lumpectomy helps confirm a diagnosis of cancer or rule it out. Lumpectomy is also a first treatment option for some women with early-stage breast cancer. In cases where cancer is found, lumpectomy usually is followed by radiation therapy to reduce the chances of cancer returning. Lumpectomy is a surgical procedure that carries a risk of side effects, including

How you prepare
You’ll meet with your surgeon a few days before your lumpectomy. Bring a list of questions to remind you to cover everything you want to know. Make sure you understand the procedure and its risks. You’ll be given instructions about pre surgical restrictions and other things you need to know. In most cases, the surgery will be done as an outpatient procedure, so you can go home the same day. ªª Tell your doctor about any medications, vitamins or supplements you’re taking in case something could interfere with the surgery. In general, to prepare for your lumpectomy, it’s recommended that you: ªª Stop taking aspirin or other blood-thinning medication. Your doctor may ask you to stop taking it a week or longer before the surgery to reduce your risk of bleeding. ªª Check with your insurance company to determine whether the procedure is covered and if there are restrictions on where you can have it done.

your radiologist may have placed a tiny marker or clip in your breast during the biopsy. If this is the case, a thin wire may be inserted in your breast and passed down to the marker or clip. Your surgeon can use the wire as a guide to the precise area that needs to be removed during surgery. If you have a lump or mass in your breast that can be easily felt through the skin, the wire procedure isn’t necessary because the surgeon can more easily find the abnormal area to be removed.

Results
The results of your pathology report should be available in a week or two. At the follow-up visit after your surgery, your doctor will explain the report. If you need more treatment, your doctor may recommend: ªª More surgery if the margins around your tumor weren’t cancer-free ªª A medical oncologist to discuss other forms of treatment after the operation, such as hormone therapy if your cancer is sensitive to hormones or chemotherapy or both ªª A radiation oncologist to discuss radiation treatments, which are typically recommended after lumpectomy ªª A counselor or support group to help you cope with having breast cancer
Preparing for lymph node removal
Lymph nodes are often removed during surgery to determine whether cancer has spread beyond the breast. In one lymph node procedure called an axillary node dissection, the surgeon removes a number of nodes from your armpit on the side of the tumor. In another lymph node procedure called a sentinel lymph node biopsy, your surgeon removes only the first one or two nodes into which a tumor drains (sentinel nodes). These are then tested for cancer. If no cancer is present, no further lymph nodes need to be removed. If cancer is present, the surgeon will remove more lymph nodes in the armpit. If you’re having sentinel node biopsy, before your surgery a radioactive substance or blue dye or both will be injected into the area around the tumor or the skin above the tumor. The dye will travel to the sentinel node or nodes, allowing your doctor to see where they are and remove them during surgery.
Why it’s done
The goal of lumpectomy is to remove cancer or other abnormal tissue while maintaining the appearance of your breast. Studies indicate that lumpectomy is as effective a treatment as removal of the entire breast (mastectomy) for women with early-stage breast cancer. Your doctor may recommend lumpectomy if a biopsy has shown that you have cancer and that the cancer is believed to be small and early stage. Lumpectomy may also be used to remove certain noncancerous or precancerous breast abnormalities.
During the procedure
A lumpectomy is usually performed using anesthesia, which will make you unaware during the procedure. Your surgeon will make an incision over the tumor or over the area that contains the wire, remove the tumor and some surrounding tissue, and send it to the lab for analysis. He or she will do the same for the sentinel lymph node or nodes if you’re having a sentinel node biopsy or the axillary lymph nodes if you’re having an axillary node dissection. The surgeon will then close the incisions with attention to preserving the appearance of your breast, using stitches (sutures) that will either dissolve on their own or be removed later by your doctor.

6. How is it best treated & instructions while on treatment? Specific antiviral drug is given according to Kg body weight ranging from 5-10 days depending upon the severity of disease. - Patient should not take bathing for 10 days. - Should be on bland diet for 10 days. - Should be on plenty of water, Juices, for 10 days. 7. Why bathing avoided in chickenpox? Maintaining hygienic is always good but while taking bathing chances of rupture of vesicles is more & incidence of scarring is more at that time. One bad thing about chickenpox is scarring, so to avoid bad scar, we generally advise not have bath till the lesion fall off. 8. Is there any vaccine for chickenpox? Yes, there is a live attenuated vaccine presently available. Children of 1 to 1 ½ years -1 dose is enough For the adolescents aged 13 yrs or older -2 doses at an interval of 4-8 weeks Each vaccine can give a protective immunity for 10 years. 9. What is the controversy with chickenpox vaccine? If we look at the disease process, chickenpox at an younger age group normally ends up with no complication. Whereas chickenpox in the adult population many times ends up with complication. So giving chickenpox vaccine at an younger age develops a cohort of group of patients seen with chickenpox in the adult age group. So better to suffer from chickenpox in the childhood & getting lifelong immunity against virus. So problems of taking booster dose once in 10 years will not be there. 10. Whom actually requires chickenpox vaccine? It is the immunocompromised patients like kidney transplant patients, leukemic, lymphoma patients requires vaccines. Chickenpox in them can cause lot of complications. 11. What systemic complication can arise due to chickenpox? Pneumonia - Commonly seen on 4-5 days Encephalitis - 1 in 1000 cases Thrombocytopenic purpura - 5th to 10th day & persists for 3-6 months Very rarely hepatitis, endocarditis keratitis occurs. 12. Chickenpox & pregnancy: Chickenpox & complications in pregnancy depends on the trimester.

But in reality it is a disease caused by a virus by name “Varicella”. It is the winter season, where we see more number of cases. 1. Common age to see: Most of the patients are below 5-7 years, many below 10 years & few in adult age group. 2. Most Common route of Spread? Through air i.e., by nasal droplets from infected cases. 3. How does it Presents? It normally preceedes with prodromal symptoms of fever, malaise, running nose 3-4 days prior to onset of rash. The rash typically starts from face followed by involvement of trunk, limbs and oral mucosa. The characteristic of chickenpox is presence of polymorphic lesions means lesion of different morphology like raised red lesions, fluid filled lesions & pustules 4. Does it require isolation? Yes. Absolutely. Because most common route of spread is droplet infection. 5. How many days of isolation is required? Strict isolation is required 1-2 days before the onset of rash and 4-5 days after the onset of rash.

7. 7. What is PHN? Any persisting pain or onset of pain in the local dermatome either 1 month or 3 months after the onset of rash is called PHN. 8. Why do the doctors check for HIV test during Herpes Zorter infection? Because, it is considered as one early markers of HIV. Herpes Zoster
It is being called by layman term as serpentine rash. The characteristics presentation of rash is eruption of fluid filled lesion along the line of dermatome in a serpentine pattern. Some people still believe it is a type of curse from snake god. But in reality it is a disease caused by reactivation of latent virus by name “Varicella”. 1. How does it start? Pain; It I s the pain or hyperesthesia in a particular dermatome alarms the patient before the rash. 2. How does the lesions look? Lesions are typically fluid filled ones erupted along the line of dermatome in a serpentine pattern. It is always unilateral. 3. What are predisposing factors? - Old age - Trauma to local area - Stress - Post radiation - Immuno suppression - drug - Induced -diseases like malignancies, HIV 4. Any relationship with chickenpox? Yes. Absolutely. Both are caused by same virus & both are treated by same drug with little bit changes. Chickenpox is a primary infection, whereas Herpes Zorter is a reactivation of latent virus which persists in the sensory ganglia. 5. 5. Is it requires isolation like chickenpox? No. Absolutely not. It is a reactivation of old virus & mode of transmission is not nasal droplets. 6. 6. What are the common complication of this infection? Secondary infection of local lesions, gangrene, nerve palsies & most commonly post Herpetic neuralgias(PHN) if the patient is in older age group.

Does Herpes Zoster can be transmitted to others? Definitely. But not through nasal droplets. It is through skin lesion & whoever who have not had chickenpox previously can develop chickenpox & not Herpes Zoster. 13. What is the role of steroids in Herper Zoster? It is being indicated in older patients to decrease the incidence of PHN & in ophthalmic Zoster to reduce the severity. To conclude, Herper Zoster is very well known for its pain. So patient should be made comfortable in terms of pain as early as possible. This can be achieved only when they have consultation with nearby dermatologist without missing golden hour of treatment.

The cord blood that remains in the placenta after birth is routinely discarded as waste despite the fact that it has a sufficient quantity of stem cells capable of curing diseases. Your baby’s umbilical cord blood is a valuable source of stem cells, which are genetically unique to your baby and family. This blood is rich in haemopoetic stem cells-proliferate cells that have the capacity to develop into blood cells. There are also indications that these cells may have the capacity to develop into other kinds of lineages, such as nerve cells or cardiac tissue. To date, umbilical cord blood has been used in more than 8,000 transplants for children and adults. In many cases, the cord blood was used by the baby’s sibling. Other transplants have occurred for the newborn himself, the newborn’s mother, father, and the newborn’s cousin. With the number of births in India running into several million a year, the potential to fight disease and save lives is immense even if the cord blood from a small fraction of new born can be collected and preserved. This life saving material is literally being disposed of as medical waste in labor and delivery units across the country day and night. There are compelling reasons for saving cord bloodthe avoidance of the need for a perfect tissue match required in bone marrow transplants, the low rate of viral contamination, and ready availability. The scope for using the cord blood later in the event of the child suffering from certain diseases is also a major attraction. So what does all this have to do with prospective parents? Known source of stem cells are adult bone marrow, peripheral blood to some extent, embryos and umbilical cord blood. The use of human embryos for therapeutic or research purpose is fraught with ethical and legal problems. The availability of adult bone marrow is dependent on voluntary donations and the possibilities of tissue matching are very low. Besides, it has been found that cord blood is some 10 times richer than bone marrow in its proportion of haemopoetic stem cells. holesterol is a fat - like substance in the blood. It is a soft waxy substance found among the lipid (fatty acids) in the bloodstream. It is not in itself harmful and the body needs a certain amount for proper functioning. When the levels rise, cholesterol begins to clog the arteries resulting in heart disease or heart attack. If it affects the arteries that carry blood to the brain, a stroke may occur. However it is produced in the body in plenty and about 15 % - 20 % is derived form the food sources we eat. Cholesterol attaches itself to a protein in the blood to carry to the cells. Lipo - protein is the name of this combination. Out of many lipo - proteins two are most important - HDL (High Density Lipo Protein) good cholesterol and LDL (Low Density Lipo Protein) bad one. HDL is very good for the heart and LDL is bad in the sense that it carries cholesterol towards the heart. A lipid test of the blood sample determines cholesterol levels. atigue refers to a feeling of tiredness or weariness. It can be temporary or chronic. Almost every person has to work overtime on certain occasions, sacrificing rest and sleep, which may cause temporary fatigue. This condition can be remedied by adequate rest. Chronic or continuous fatigue is, however, a serious problem which requires a comprehensive plan of treatment. Chronic fatigue can result from a variety of factors. A specific character trait, compulsiveness, can lead to continuous fatigue. Many persons constantly feel that they cannot take rest until they finish everything that needs to be done at one time. These persons are usually perfectionists, tense and cannot relax unless they complete the whole job, no matter how tired they may be.
¾¾ Causes ªª The chief cause of fatigue is lowered vitality
or lack of energy due to wrong feeding habits. Fatigue is an indication that the cells of the body are not getting sufficient live atoms in the food to furnish them with a constant flow of needed energy. The habitual use of refined foods such as white sugar, refined cereals and white flour products as well as processed, tinned and preserved foods have a very bad effect on the system in general. Foods ‘denatured’ in this way are deprived, to a very great extent, of their invaluable vitamins and minerals. Such foods lead to nervousness, tiredness, obesity and a host of other complaints prevalent today. ªª Certain physical conditions can cause fatigue. Anaemia is a very common ailment leading to tiredness. It is known as ‘tired blood’ disturbance. In anaemia, very little oxygen reaches the tissues with the result energy cannot be produced normally. This causes constant tiredness and mental depression. Anaemia usually results from deficiencies of iron and vitamin B12. Sometime deficiencies of vitamin B6 and folic acid are also involved. Marsh recommends fresh raw nuts taken directly from the shell and root vegetables. He says, “Any seed or root vegetable that will grow again will renew human vitality” . The patient should take an optimum diet made up of (i) seeds, nuts and grains, (ii) vegetables, and (iii) fruits. Roughly, each food group should supply the bulk of one of the three meals. ªª Sprouting is an excellent way to eat seeds, beans and grains in raw form. Sprouting increases the nutritional value of foods and many new vitamins are created or multiplied in seeds during sprouting. The patient should supplement the three health – building food groups with special protective foods such as milk, high quality cold – pressed unrefined vegetable oil and honey. The patient should also take natural vitamin and mineral supplements as an effective assurance against nutritional deficiencies, as such deficiencies have been found to be a factor in fatigue. Lack of pantothenic acid, vitamin B in particular, leads to extreme fatigue as deficiency of this vitamin is associated with exhaustion of the adrenal glands. In fact the entire B-complex protect nerves and increases energy by helping to nourish and regulate glands. The vegetarian foods rich in vitamin B are wheat and other whole grain cereals, green leafy vegetables, rice polishing, milk, nuts, banana, yeast, pulses and peas. ªª Minerals are also important. Potassium is especially needed for protection against fatigue. Raw green vegetables are rich in this mineral. Calcium is essential for relaxation and is beneficial in cases of insomnia and tension both of which can lead to fatigue. Sodium and zinc are also beneficial in the treatment of fatigue. Raw vegetable juices, especially carrot juice, taken separately or in combination with juices of beets and cucumbers, is highly valuable in overcoming fatigue. The formula proportions considered helpful in the combination juice of 500 ml are carrot 300 ml and beet and cucumber 100 ml. each. The patient should avoid depending for an energy lift, on crutches such as taking aspirin, tranquilizers and other drugs, drinking coffee or alcohol, smoking , eating some sugar or sweets. They give only a temporary boost and this is soon followed by a downward plunge of energy, leaving a person worse than before.
ªª Intestinal parasites can also lead to fatigue as they rob the body of good nourishment and gorge themselves on rich red blood. ªª Other ailments which can cause fatigue are low blood pressure, low blood sugar, any kind of infection in the body, liver damage, a sluggish thyroid and allergy in foods and drugs caused by additives including artificial flavours, colours and preservatives. ªª Mental tension is one of the major causes of fatigue. A person who is tense and cannot relax has all the muscles of his body more or less contracted. This leads to needless waste of unusually large amounts of energy. Food is continuously burnt, lactic acid accumulates more rapidly than it can be carried to liver for conversion to body starch. ªª Persons who are high - strung, nervous and irritable usually suffer from this type of fatigue.
¾¾ Treatment ªª Nutritional measures are most vital in the
treatment of fatigue. Studies reveal that people who eat small mid – meals suffer less from fatigue and nervousness, think more clearly and are more efficient than those who eat only three meals daily. These mid – meals should consist of fresh or dried fruits, fresh fruit or vegetable juices, raw vegetables or small sandwich of whole grain bread. The mid – meal should be small and less food should be consumed at regular meals. They should be taken at specified time such as 11 a.m. , 4 p.m. and before retiring to bed. The patient should eat health foods which supply energy to the body. ªª Charles De Coti Marsh of London in his book ‘Prescription for Energy’ prescribes foods to relieve fatigue and gain energy. He says, “Regenerating must begin with foods..... They must be taken in their natural state. These cereals are corn seeds, wheat seeds, rye seeds, maize seeds, barley seeds and oat seeds. They must be freshly milled. In uncooked cereals, we do have one perfect food for perfect health which contains essential vitamins and energy creators”. In addition to cereal seeds,

leep is a periodic rest of the body which is absolutely essential for its efficient functioning. It has been called “most cheering restorative of tired bodies”. Sleep is the indispensable condition to the recuperation of energy. We go to bed fatigued and get up refreshed. Sleep repairs the wear and tear of the body and mind incurred during waking hours. Nothing is so restorative to the nerves as sound and uninterrupted sleep. Sleep is thus a vital element in a total way of life. It is a basic need in man’s mental as well as physical life. During sleep most of the functions of the body are carried on at the lowest level possible in health. Heat production is from 10 to 15 percent below the basal level. The mechanism regulating the body temperature are less sensitive than in the waking state and are depressed. The rate of the heart is reduced by 10 to 30 beats per minute and a decline in blood pressure of about 20mm occurs in quiet restful sleep. The urine volume is considerably reduced but its concentration in solids is increased. The tone of all the skeletal muscles is lessened. The eyes are usually rolled upward and the pupils constricted. Loss of sleep exerts seriously detrimental effects upon the nervous system. Long periods of wakefulness may cause profound psychological changes such as loss of memory, irritability, hallucination and even

Theories of sleep: Many theories of sleep have been advanced to explain the temporary loss of consciousness which we know as sleep. The oldest theory is that sleep is induced by a reduction in the blood supply to the brain or at least to conscious centres. This is known as ischemic theory. Even the ancient Greek physicians were aware that the carotid artery was in a way concerned with the onset of sleep. The name itself expresses this belief. The Greek word ‘ Karotides’ for carotid arteries is derived from karoo which means ‘put to sleep’. In modern times, the drowsiness after a meal, presumably due to the diversion of blood from the brain to the digestive organs, is cited in support of the ischemic theory. Another important theory about sleep is the chemical theory. As a result of experiments in the metabolism of sleeping subjects, it is considered that the fatigue inducing sleep may be a mild form of blood poisoning or toxaemia. This “ poisoning” is believed to be brought on by the expenditure of energy during the waking hours. According to this theory, every contraction of a muscle and every impulse passing through the brain or the nerves breaks down a certain amount of tissue. The debris from broken down tissue is then thrown into the bloodstream. In the waking state, much of the waste from broken down tissue is got rid of through the natural

eliminating processes of lungs, kidneys, bowels and skin. But there comes a saturation point when there is such an accumulation of waste that it cannot be disposed of by these processes and it then invades the grey matter of the brain. In such an eventuality, mental and physical alertness are impaired. It is nature’s warning that the waste product must be reduced to replenish the lost energy. So we get tired and the urge to get sleep becomes irresistible. During sleep, the cells and tissues that break down to produce toxic waste become less active and the production of toxic waste is vastly reduced. Simultaneously, constructive activities take place within the body during sleep, which rebuild the broken down tissue. Another theory places a sleeping centre in the hypothalamus. Many of the bodily changes in sleep such as constriction of pupils, reduced frequency of heart beat, increased gastric tone and secretion are manifestations of the activity of hypothalamus nuclei, especially parasympathetic centres. Perhaps some of the sleeping pills affect this centre in the brain. Although the various theories have certain amount of experimental evidence to support them, none has really solved what is the most mysterious process in our lives. All we know is that sleep substitutes constructive measures for the destructive processes of our waking hours. We cannot live without sleep. Duration: Another mystery about sleep is that no
a little while. According to Dr. Lindlahr, two hours before and two hours after midnight are the most valuable for sleep of all the twenty - four hours of the day. In these four hours, mental and physical vigour are at their lowest ebb and sleep is soundest and most natural. It is believed that three-quarters of our sleep consists of what is called ‘ slow wave sleep.’ The restorative processes occur during this time. The remaining quarter is taken by what is called ‘rapid eye movement (REM) sleep’. It is also called paradoxical or dreaming sleep and it comes in episodes of about 20 minutes duration about five times in a night. It involves dreaming, irregular heart rates, raised blood pressure and erection of the penis. It is in this phase of sleep that normal healthy young men may have wet dreams. Both forms of sleep are considered equally important, being normal sleeping rhythms.
Sleeping positions: There are many theories about good and bad sleeping positions. Practically everyone changes positions several times during sleep. Hence how one starts out is of no consequence. It is a good thing we do turn about in our beds. If we did not, we would awake in the morning stiff, having maintained the same position all night. For proper sleep, however, one should not sleep on one’s back but on the side with one or both legs brought well up and the head and the shoulder slightly forward. Sleeping pills are no remedy for sleeplessness. They are habitforming and become less effective when taken continuously. They lower the I.Q. dull the brain and can prove fatal if taken in excess or before or after alcohol.

The amount of sleep needed for an individual’s well - being, is determined by what he feels he needs, not by what other people, including the doctor, think is reasonable. On the whole, women sleep from 45 minutes to one hour more than men. The amount of sleep required varies at different ages as follows :
The side-effect of sleeping pills include indigestion, skin rashes, lowered resistance to infection, circulatory and respiratory problems, poor appetite, high blood pressure, kidney and liver problems and mental confusion. Sleeping well is an art. It needs a perfect blend of healthy habits and control of mind. A clean body and mind, relaxed mood, physical exercises, and perfect dietary control are some of the basic sleep-inducing methods. Unpleasant situations at bed time such as arguments, quarrels, watching a horror movie, listening to loud music which would create anxiety, fear, excitement and worries should be avoided. Such situations stimulate the cerebral cortex and tend to keep one awake. The sleeping place should be well ventilated, with balanced temperature and free from noises. The bed should be neither too hard nor too soft, but comfortable. The pillow should not be too hard or too high. The bed clothes should be loose-fitting and light coloured. Another important rule is not to have heavy food shortly before bed time.

ye irritation is often accompanied by redness of the eye. Redness of the eye can be a sign of the many conditions and diseases that our eyes may contract. The three major causes of red eyes are dry eyes, infections caused due to bacteria or virus, or allergies. There are at least a dozen different types of infections. Also, an allergy itself can be of various types like Seasonal Allergic Conjunctivitis, Perennial Allergic Conjunctivitis, Vernal Keratoconjunctivitis, Atopic Keratoconjunctivitis, Giant Papillary Conjunctivitis, Irritative Conjunctivitis, and many others. But people often ignore redness and irritation. When discomfort increases people often use old prescriptions and self medicate. Some people also use home remedies, over-the-counter eye drops or other medicines based on own assumptions or advice from other people. All this, in order to save the effort of consulting a doctor, which most often aggravates the problem rather than treating it. Because, unknown to them, old prescriptions may contain steroids or other medications that are simply not meant for the condition they’re trying to treat. Use of steroids does provide immediate relief, but it’s temporary in nature. All symptoms can reappear the moment steroids are discontinued. Extensive use of steroids without medical advice may lead to serious side effects including loss of vision.

Steroid anti-inflammatory eye drops are very effective, but they are reserved for severe symptoms or advanced stages as complementary therapy because a single anti-allergy drug may not be enough to provide quick relief. However, doctors use them with lot of caution because of the potential side effects of the long-term use of steroids. Side effects of steroids include elevated pressure in the eyes and cataracts. he Pediatric Ophthalmology service of Narayana Nethralaya has started with a noble aim of bringing back a smile to a child’s face. Over the years the department has grown to be a major referral centre for pediatric eye disorders. At present, this service involves not only diagnosis and management of various facets of pediatric eye disorders like refractive error, childhood cataract, glaucoma, amblyopia, squint, retinal disorders, congenital anomalies, pediatric eye cancer etc.. but also adult patients with squint/ double vision problems. The department provides appropriate therapeutic intervention for all stages of retinopathy of prematurity, a blinding disease of the premature newborn. Currently, the department is actively involved in various community ophthalmology projects like Pavagada Pediatric eye disease study, Smart School Screening, Slum 10K project, KIDROP study to help the weaker section addressing eye related issues of their child. Today we see nearly 100 pediatric patients per day and perform close to 50 eye surgeries every month.

WHY WE HAVE TO PROTECT OUR MAIN DOOR? According to vaastu, the main door or entrance has been accorded prime importance as it lets in the vital life-giving force that promotes health, wealth and luck. It is called the mouth of the house.
The main door of the house should not be below ground level. There should not be an underground tank, septic tank, or canal under the main entrance. There should not be any abandoned, wrecked buildings, tall trees, light poles in front of the main entrance. Such situations are highly inauspicious. But we have remedies with pyramids for all these things. It is good to decorate a house’s entrance with pictures or statues of Laksmi, Ganesh or auspicious signs such as OM or the Swastika. Inauspicious pictures should not be on the door. Doors should have thresholds preferably made in marble or wood. Display two pair of auspicious Laksmi steps on both the side of threshold to invite Lakshmiji, a goddess of wealth. Shoes should not be kept in front of the door, but to the side or in the shoe box. Anything connected with your religion, such as a Yantra, a Ganesh idol, a cross or scriptures from the Quran should be placed above the front door. It is auspicious to have main entrance in the north or east. Wherever the entrance of any home or building is in the east, the residents enjoy better health, increasing prosperity, and harmonious relations with others; but wherever houses are facing south or south-west, the opposite is the result.
Urja (energy) enters the house through doors and windows. However, the front door of your house is where the major part of energy enters and therefore this door plays an important role in determining the flow of vibrations experienced by the household. The door should open completely. Due to the shoe rack or furniture behind the door in some of the houses, the entrance is not able to open completely. In such a situation the energy is not able to enter in to the house completely. Such house will have short supply of the required energy, which will lead to a limited opportunities and poor luck to the occupants. Make sure that your house open at 90 degree paving way to the energy to circulate in without hindrance. The cracking sounds from hinges on opening of the main door should be avoided. It is “Swar Vedha” according to vaastu. The entrance area should be neat and clean and avoid having a dustbin in the entrance. Sun Shades The slopes of all sunshades constructed above doors and windows should be towards the north-east, so that the rain water must flow from south-west to north-east. The windows and doors in your home should not close automatically as these may affect the health of your children. What is yoga?

Asana means the posture of the body. These are firm and comfortable poses of the body. One should follow Yama and Niyama before practicing Yogasanas. Yama means improving moral standards and Niyama means safeguarding health through good physical and mental actions. Asanas keep the body healthy, strong light and in a balanced state. Yogasana is based on scientific study of the structure of human body (Anatomy). Through Yogasana every part of the body becomes lighter and energetic making the body healthy. adhu smitha (name changed) comes from a family of Ayurvedic physicians. Her father, grandfather, great grandfather all healed their people with Ayurvedic medicines. So when it was time for entering into a marriage relationship Madhu’s father took extra care and importance in finalising the relationship of his daughter’s marriage. He took the Ayurvedic view point into consideration as marriage or matrimonial relation is not chiefly aimed for enjoying sexual pleasure but carry’s an aim of keeping the thread of excellent progeny -family tradition, in continuity, emphasising on production, introduction and providing progeny with best qualities and good culture. It is a common practice in preparation for Hindu marriage to inquire about the kula-gotra (meaning clan lineage) of the bride and groom before approving the marriage. In almost all Hindu families, marriage within the same gotra is prohibited, since people with same gotra are considered to be siblings and chances of inheriting genetic disorders carry’s a high risk. So marriage within the same lineage is never encouraged. Even today Hindu families follow this tradition. Madhu Smitha’s dad - a renowned Ayurvedic physician agrees that cousin couples have a higher incident of birth defects than non-related couples. Two flowers of the same vine have different sizes and shapes. Nature is full of variety and we are no exception. We exhibit diversity both in appearance and personality. This diversity makes us different from each other. Ayurveda plays a supportive

role in assessment of ability of a prospective couple to regenerate. For the purpose of judging matrimonial compatibility the Prakriti of both the boy and girl are considered. When the boy and the girl have same dominant doshas bioenergy, there is biological imbalance .So when they unite the foetus inherits the same Doshas in a greater proportion .This would be in great disharmony with the nature and the foetus is put into tremendous struggle right from the time of conception stage. Depending upon the intensity of the doshas there is either no conception at all or it may result into a miscarriage or they may give birth to a very sickly child. If we extend the Ayurvedic notion of doshas we know that basic Prakriti of a person does not remain static throughout. It gets modified with the changes in the season, food habits, temperament of the person and his life style as a whole. Any Vaidya will be able to suggest different sets of food etc. to counter the doshas according to the season. Similarly with the practice of Pranayama and Yogasanas it is quite possible to bring in the desired changes in the body system. This will modify the basic Prakriti of a person to a considerable degree .It is therefore wise to consult an expert Vaidya when a pair with otherwise good compatibility factors is rejected on the basis of doshas alone. If a couple having same doshas bioenergy Prakriti constitution have got married and find incompatibility with each other they should take advantage of all wisdom of an Ayurvedic Vaidya to ascertain whether there is acute incompatibility in terms of Prakritietc. in them and if present they can put conscious effort to remove the negativity through correct diet and life style. The things a mother eats /does not eat, or uses it on her body, during pregnancy and breast feeding can have a profound effect on the intellect, memory, mental problems etc. of their baby. Early childhood problems like ADD, ADHD ,brain cancer , autism , etc. may primarily be caused by the things a mother eats /does not eat , does/does not do during pregnancy . If a prospective mother to be is under extreme and constant stress she can induce the same stressful state in her unborn child. Whenever the mother becomes upset her body releases stress hormones into her blood stream. They are absorbed by the embryo at the conception stage itself. That is why a woman who encounter acute or chronic stress increase the chances of babies who are premature, lower than average weight, hyper active irritable and colicky . Some babies are even born with thumbs sucked raw or even with ulcers because of constant stress to the mother. The womb establishes the child’s expectations .If it has been a warm loving environment the child is likely to expect the outside world to be the same .This produces a predisposition towards trust, openness, extroversion, self-confidence. If the environment has been hostile the child will anticipate that his new world will be equally uninviting. to your partner who can then infect you creating a vicious cycle that’s hard to break. Moreover it’s difficult to vouch the health of the sperm or the ova when we are in a diseased state. On the days of fast or religious activity discipline we are deprived of necessary nourishing food which is necessary for the development of the embryo. Hence avoiding intercourse on those days is recommended. It’s not advised to have intercourse on the first three days of the menstrual cycle. The uterine cavity has a rich network of blood vessels and forms a very special covering after the menstruation in anticipation of pregnancy. This covering has the ability to allow an early embryo/baby, to become implanted/rest on its bed and further nourishes and provides support during the entire period of gestation. Every month if the egg has not fertilised then this bed which has an amazing quality of nourishing an embryo/baby undergoes further maturation, becomes necrotic tissue, separates from the viable parts of the uterus/ surrounding living tissue as in a wound, sore or an inflammation and flows out as menstruation. The uterus flushes out the debris in the form of menstrual blood leaving just the residual basal layer which regenerates again after the menstrual cycle. Under normal conditions naturally produced in the body the internal lining grows about 1 to 2 mm every other day after the third day of menstrual period, and at the time of ovulation it is about 8mm-10mmthicker. If there is failure in the development of normal uterine lining the chances of fertilised egg becoming implanted is threatened. If there is inadequate stimulation of the lining due to lack of nourishment the endometrium remains thin and inactive. In human being this condition leads to amenorrhoea –no menstrual flow. Its formation is affected by seasons, climate, stress, and other factors like food and lifestyle. Ayurvedic acharyas discourage having excess food, having intercourse, or doing excess work on first three days of menstrual cycle because these activities decrease the metabolic activity of the uterus. Decreased metabolism will fail to clear the entire debris from uterine cavity. The priming of the bed of the uterus can be successfully accomplished if the previous layer is mopped up clean. Ayurvedic acharyas say that a woman can become pregnant from having sexual intercourse during menstruation. It is possible that sperms that enter her body during menstruation may survive long enough to fertilise an egg that is released latter. But the pregnancy could result in, abortion, intra uterine death of the baby or a baby born with deficient or defective body parts, short lived and transmission of diseases that can be transmitted sexually and spreading of blood borne infections between the partners become easy on the first three days of menstruation. Hence couple is discouraged having sexual union on these days. This becomes an act of

unrighteousness too according to Ayurvedic scriptures and husband becomes a prey for shortened life span, with the decline of eye sight and glory. These three days a woman should dedicate to get complete rest in order to restore the circulation, completely heal and renew the activity going on in the uterus, strengthen the digestion, restore vitality and energy. In the 2005 NIH study Wilcox and his colleague studied the menstrual cycle of 213 women most between the ages of 25 to 35 when a woman’s cycle is most likely to be regular. They found that a woman’s fertile period can extend to the day when menstruation is predicted to begin .In addition they found that a woman can become fertile as early as Day 4 of her cycle and that 17%are fertile by day 7.Therefore menstruation is no guarantee that a woman will not become pregnant when engaged in sexual intercourse. A wife is appreciated as most affectionate and dependable friend just like a sibling brother. She is highly credited of possessing qualities like tolerance, cordiality, love, affection, soft sweet talk, good serving and nursing ability, passionate and .other salient features which make her perfect, effective and favourite life partner of husband in real and practical sense .The wife is not merely a sex partner but she plays significant roles in family and social life and for this major task she is given various responsibilities and a commanding position in the family. If the wife is not passionate, desirous, interested and exited, is frigid for sexual association, or has illicit relationship with other men, such a woman is considered unfit for sexual union. What is the correct position of having intercourse? There are two purposes of performing Sexual intercourse by man with woman. ªª Toprocreate. ªª For sexual pleasure. If it is with the intention of procreation then husband should have union with his wife during her fertile period. During cohabitation a woman should not take a prone posture nor should she be on sides. If she assumes prone posture then vata – wind bioenergy gets aggravated and afflicts her uterus .If she remains on the right side then kapha on that side gets displaced and blocks the uterus. If pressure is put on the left side during cohabitation then the pitta bioenergy burns the ovum and sperm due to its heat. Therefore the woman should receive seeds while lying on her back. In this posture the doshas remain in their respective seats. To recline is divine i.e. after intercourse it is preferable for the woman to lie down on her back for at least 15 minutes with her hips elevated .This practise will allow the sperms some time to begin their journey to meet the egg without having to fight gravity. To elevate your hips you can put a pillow underneath your bottom. On the other hand the couple is socially culturally pledged to live together executing all the duties of a house hold order in physical, mental and sexual association. They can engage in sexual relationship for the purpose of enjoying sexual pleasure which helps strengthen the bond of love and passion psychologically, and physically. When

Fertile days are the days when a woman is likely to get pregnant if she is sexually active with her husband. There is always a bit of guess work involved in accessing when a woman releases an egg for conception. Ayurvedic classics recommend that couple trying to have a baby should have intercourse between day 8 and day 20 of a woman’s menstrual cycle. Having intercourse every other day or every third day works effective to produce the desired result. Sperm can live in a woman’s body for 3to 5 days. A released egg survives for 4 to 12 hrs. .So it’s wise not to wait for ovulation to occur, just go ahead with the intercourse when you are planning for a baby. Being observant helps a woman to predict the most fertile period when she is able to conceive. Key to this method is experience. Her body gives signals and clues to her in the form of sticky discharge which is clear like a raw egg white .She can take a sample of this discharge from the outer lip of the external genitals .Take the sample, see, feel it and stretch it between two of your fingers –thumb and index finger to test for consistency. After menstrual bleeding stops we can call this phase 1; Where the growth of the follicles which contain eggs start .This happens by production of cells by multiplication of parts, proliferatory period. During this period the vagina will be dry and you cannot appreciate any mucus.

He empties his bowels at unusually frequent intervals, spits up half his feed and generally makes a nuisance of himself. He is to all intents a neurotic infant when he is born a result of an unsatisfactory environment. Resorting to anger makes the off spring fiercely deceitful and jealous. ªª Constantly given to sleep makes the offspring drowsy, dull and deficient in digestive power.
