Health news

Health camp Vydehi hospital recently conducted a Mega General Health Camp at Lakkur (Mallur Tq) in liason with HAL (Composite Manufacturing Division). The camp attracted as many as 1620 patients in all age groups from the neighboring towns & villages. The camp was indeed a huge success as it was a very well coordinated team effort of all VIMS & RC doctors, paramedics & other staffs.

Thus when we talk of “Suncare”, we mean protection against the immediate and long term side effects of UV A and UV B. A word of caution. Ozone is present in the stratospheric layer of the environment, existing approximately 15-35 kilometers above the surface of the earth. Because of the depletion of the ozone layer by industrial wastes, pesticides etc. it leads to increased UV rays entering the earth and causing more disorders especially skin cancers. Certain factors increase the deleterious effects of UV rays. Dark scanty clothing, outdoor jobs, high altitude, outdoor recreational activity, visits to “sunny” places cigarette smoking all lead to increase damage by the sunlight. Certain atmospheric factors like heat, wind humidity pollutants, cloud cover, summer season, time of the day (Between 10 am-3 pm, the UV rays enter the earth nmore) all worsen sunlight damage on the skin.

It is these ultraviolet rays which are responsible for all the skin problems that the man suffers from. This is further subdivided into UV A(320400nm); UV B(280-320nm) and UV C (200-280nm). Of these, UV C is absorbed byb the ozone layer. Only UV A and UV B reach the skin surface. Of these UV A comprises 95-98% and UV B 2-5 percent. It is these two fractions, which are mainly responsible for all the sunlight related problems. They act on collagen. Elastin and other proteins present on the skin and induce damage to the skin. The damage caused by the UV rays can be seen immediately as in sunburn , sun tanning or certain allergic disorders. The main culprit in all these problems is the sunlight. It is important we understand why sunlight causes the problems and how we can overcome it.

Principles of protection: To avoid or minimize harmful effects occurring due to sun exposure, certain protective measures can be adopted: ªªAvoid going out in sunlight between 10 am and 3 pm, the period when UV rays are at its maximum intensity. This also applies during rainy season. ªªClothing should be tightly woven, light coloured. White cloth, loosely fitting, which cover maximum area of body surface. Eg. Kurta, pyjama, salwar, full sleeve shirts, long pants etc. ªªUse of wide brimmed hats, gloves and broad sun glasses. ªª Protection with umbrella is also very effectiuve when there is no availability of shade. ªª Adequate and proper use of sunscreens applied 20 minutes before going out in sunlight helps minimize sunlight damages. A sunscreen with Sun Protection Factor (SPF) 15 is ideally recommended for sun protection. A good sunscreen is one which should block UV A and UV B and also water resistant (i.e SPF of a sunscreen stays unchanged after 40 minutes of contact with water). This is useful especially for outdoor workers like agriculture workers, salesman, and outdoor personnel. A liberal quantity (2 mg/cm) appeared evenly all over the face will alter protection for 4 hours. Very rarely sunscreens itself can induce allergic reactions because of its chemical nature and other ingredients. To conclude, though sunlight is protective and nourishes the skin, the harmful effects of sunlight exposure cannot be ignored. Effective protection will help prevent a large number of skin problems and thus minimize the cost of treatment. Patients are advised to consult dermatologists if they do not improve or develop skin problems, at the earliest.

Certain drugs especially pain killers, antibiotics, drugs used in BP control are also known to interact with sunlight and cause harmful effect on the skin. Certain cosmetics like soaps, perfumes and other facial cosmetics can induce skin allergies on exposure to sunlight. Also contact with certain plants are also known to worsen skin problems. In our country because of its geographical variants like mountains, deserts as well as sea, rivers, lakes etc, offers vast areas for reflection of sunrays, thus act as indirect source for UV radiation. A detailed examination is done including pelvic examination to look for local causes for the bleeding like polyp and infection which are easily made out during the examination. Also a pap smear is taken which is the surface scarping of mouth of the womb with a wooden or plastic spatula and sent for microscopy. EGG FREEZING & IVF IN CANCER PATIENTS

W

hen a young woman is diagnosed with cancer, the rush to treatment is immediate. Sometimes the patient is aware her fertility could be affected, but is so overwhelmed and exhausted that she puts the “fertility issue” aside. With the advances in technology, pregnancy can be achieved with women’s own egg & IVF. Egg freezing is a form of fertility preservation available to young women diagnosed with cancer, or other serious medical condition, that requires treatment such as chemotherapy or radiotherapy which may harm her ovaries & damage her future fertility. IVF techniques are used to stimulate the ovaries & then retrieve, freeze & store eggs for possible future attempts to conceive, if the woman’s fertility & the quality of her eggs are reduced as a result of cancer therapy. Patients who are candidates for egg freezing are those young women recently diagnosed with cancer. Since most cancer treatments involve radiation, chemotherapy, or a combination of both, ovarian function is often compromised rendering the patient infertile. It is important that the patient be offered egg freezing as soon as possible after her cancer diagnosis so that there is time to proceed with egg collection and freezing prior to the initiation of cancer treatments. In addition, patients with non-malignant diseases, such as lupus and rheumatoid arthritis, may be on chronic medications that are detrimental to oocyte function. Such

patients may also benefit from oocyte cryopreservation. Egg freezing using either the slow freeze method or vitrification is a comparatively recent development in fertility treatment & fertility preservation. As with any medical procedures, egg freezing treatment carries some risks. Egg collection procedure is carried out under general anaesthesia & is classed as low risk procedure. A woman has to take ovarian stimulation drugs with a small risk of hyperstimulation. Egg freezing gives reassurance for motherhood to young women suffering from cancer. Eggs are stored until the patient wishes to try to conceive. Currently eggs may be stored for a maximum of 10 years, although this period may be extended if the women become prematurely infertile due to any reason, in which case they can be stored for 55 years from the date they were stored. Hundreds of babies have been born worldwide using both methods of egg freezing but as with all treatment; no clinic can guarantee that the procedure will result in a successful pregnancy or live birth. Research suggests that the risks of miscarriage, fetal abnormality or birth defects are no higher than IVF or ICSI. So egg freezing for the cancer patients gives a new ray of hope for future fertility.

Swine influenza is a disease in pigs. The virus currently transmitting among people is now generally referred to as swine flu, although the origin of the disease is still under investigation. There is no evidence of this strain of the disease circulating in pigs in the UK. There are regular outbreaks of swine influenza in pigs worldwide. It does not normally infect humans, although this occasionally does occur - usually in people who have had close contact with pigs. Swine influenza viruses are usually of the H1N1 subtype. The swine flu that has spread to humans is a version of this virus. Because the swine flu virus has mutated (changed) and is now able to infect humans and transmit between them.

How is swine flu infection diagnosed?

Is the new swine flu virus contagious?

Swine flu infection is confirmed when the person has a positive swab test result, after being assessed by their GP. But the NHS is now moving away from the laboratory confirmation of all cases to clinical diagnosis of some cases. This means that people who have been in close contact with a confirmed case and who have flu-like symptoms may be clinically diagnosed as having swine flu without the need for a swab test.

The Health Protection Agency (HPA) says the new swine flu virus is highly contagious and is spreading from person to person.

Swine flu spreads in the same way as ordinary colds and flu. The virus is spread through the droplets that come out of the nose or mouth when someone coughs or sneezes. If someone coughs or sneezes and they do not cover it, those droplets can spread about one metre (3ft). If you are very close to the person you might breathe them in. Or, if someone coughs or sneezes into their hand, those droplets and the virus within them are easily transferred to surfaces that the person touches, such as door handles, hand rails, telephones and keyboards. If you touch these surfaces and touch your face, the virus can enter your system, and you can become infected.

All suspected cases who have swine flu symptoms will have been asked to self-isolate at home and restrict their contact with people. In Sanskrit, mantra is defined as mananaat traayate iti mantrah that is the repetition of any akshara or an

alphabet, which has origin from Vedas and tantras. Among the the Vedas, Rigveda primarily involves the teaching of mantras like gayatri mantra which is practiced to invoke the sungod and the cosmic powers for their blessings. Mantras are also said as the combination of grouping of the sound vibrations. It is a tool for connecting us with the spiritual wisdom and the most important tool for attuning our bodies and minds to their proper resonance. It is considered as a vehicle for expansion of the mind, liberation of energy and also one of the easiest of yogic practices to perform unlike asana or pranayama that most people find difficult to do. Methods of Mantra chanting

Mantras can be chanted in 3 ways such as 1. Vaikhari chanting- chanting the mantra louder or the one which is audible. 2. Upanshu chanting- chanting in low voice or just whispering with the movements of lips or the tongue. 3. Manasika chanting- this is one of the subtle form of chanting commonly used by advance practitioners to achieve the higher state of awareness. This is silently chanted in the mind with no movements of the lips or the tongue. When the sound or mantra is produced in these 3 different forms, it results in production of different types of waves. When the mantra is chanted louder or in a whisper, it creates a standing wave and when the same is chanted silently mentally, it produces a resonant wave. Practise of Mantra chanting

In todays world, chanting or doing a bhajan has become the most important and rapidly growing practice. Even the bhajans are based upon the repetition of mantric sounds, names or phrases. It is found that the modern day yoga guru’s also give importance to mantra, where the mantra om has become the main word and sound of yoga. For instance, the transcendental meditation for decades has promoted the mantra meditation as its main technique and found significant reduction in blood pressure, stress etc disorders. The concept of yoga previously understood only as asana or the posture, now has expanded the thinking and has created awareness as a science including asanas, pranayama etc and not just the asanas. In this process, a new popular usage of mantras is arising. Just as each asana puts the body into an energetic posture in which higher forces can be released, so too each mantra puts the mind into a certain pose in which it can become a conduit for a higher flow of energy and grace. Each mantra like asana has its intention, form and means of adaptation. Much like each asana, each mantra has its signature energy that brings about a specific effect upon the mind and can be understood according to its sound, meaning and application. It protects heart muscle, stimulates brain action and helps prevent constipation. It has a mild diuretic effect. The dry powder with hot water or honey can be used as adjuvant therapy along with other medicines in epilepsy. The paste if applied to the piles mass relieves pain and irritation and reduces swelling. Two drops of the fresh juice put into the ears relieves ear-pain. The powder of vacha, nagakeshara and pippali induces the labour pain during delivery when labour is delayed because of less uterine contractions. The decoction helps to remove the urinary calculi. The smoke emitted by burning vacha is a very good mosquito repellent. Hence can be used during evenings. The mixture of vacha, pippali and honey, can be used as nasal medicine during partial headache. The powder with honey if taken before food controls hyperacidity. What is dry eye?

Normally, the eye constantly bathes itself in tears. By producing tears at a slow and steady rate, the eye stays moist and comfortable. Sometimes people do not produce enough tears or the appropriate quality of tears to keep their eyes healthy and comfortable. This condition is known as dry eye. The eye uses two different methods to produce tears; it can make tears at a slow, steady rate to maintain normal eye lubrication. It can also produce large quantities of tears in response to eye irritation or emotion, when a foreign body or dryness irritates the eye, or when a person cries, excessive tearing occurs.

What are the symptoms of dry eye?

The usual symptoms include: stinging or burning eyes; scratchiness; stringy mucus in or around the eyes; excessive eye irritation from smoke or wind; excess tearing; discomfort when wearing contact lenses. Excess tearing from “dry eye” may sound illogical, but it can be understood as the eye’s response to discomfort. If the tears responsible for maintaining lubrication do not keep the eye wet enough, the eye becomes irritated. Eye irritation prompts the gland that makes tears (called the lacrimal gland) to release a large volume of tears, overwhelming the tear drainage system. These excess tears then overflow from your eye.

What is the tear film?

When you blink, a film of tears spreads over the eye, making the surface of the eye smooth and clear. WEith out this tear film, good vision would not be possible. The tear film consists of three layers: An oily layer; a watery layer; a layer of mucus Each layer has its own purpose. The oily layer produced by the meibomian glads, froms the outermost surface of the tear film. Its main purpose is to smooth the tear surface and reduce evaporation of tears. The middle watery layer makes up most of what we ordinarily think of as tears. This layer, produced by the lacrimal gland, cleanses the eye and washes away foreign particles or irritants. The inner layer consists of mucus produced by the conjunctiva. Mucus allows the watery layer to spread

What causes dry eye?

Tear production normally decreases as we age. Although dry eye can occur in both men and women at any age, women are most often affected. This is especially true after menopause. Dry eye can also be associated with other problems. For example, people with dry eyes, dry mouth, and arthritis are said to have Sjogren’s syndrome. A wide variety of common medications—both prescription and over-the-counter—can cause dry eye by reducing tear secretion. Be sure to tell your ophthalmologist (Eye M.D.) the names of all the medications you are taking, especially if you are using: ªª diuretics; m beta-blockers; ªª antihistamines; ªª sleeping pills; ªª medications for “nerves”; or ªª Pain relievers. Since these medications are often necessary, the dry eye condition may have to be tolerated or treated with eye drops called artificial tears. People with dry eye are often more prone to the toxic side effects of eye medications, including artificial tears. For example, the preservatives in certain eye drops and artificial tear preparations can irritate the eye. These people may need special preservative-free artificial tears.

How is dry eye diagnosed?

An ophthalmologist is usually able to diagnose dry eye by examining the eyes. Sometimes tests that measure tear production are necessary. One test, called the Schirmer tear test, involves placing filter-paper strips under the lower eyelids to measure the rate of tear production under various conditions. Another test uses a diagnostic drop (called fluorescein or rose bengal) to look for certain patterns of dryness on the surface of the eye.

How is dry eye treated?

Eye drops called artificial tears are similar to your own tears. They lubricate the eyes and help maintain moisture. Artificial tears are available.

The transferred hairs are not “rejected” as they are patient’s own hairs and are not foreign tissue. The hair in the back of the scalp is insensitive to the androgen mediated balding process and hence the transplanted hairs will last a lifetime –this is the basis for transplantation. Thus, hair transplantation is the only permanent method of treating baldness. Any person with significant baldness can undergo the procedure. Women can also under go the procedure in selected cases. However it is important to understand that pattern hairloss is a continuous process and hence frequently combination with drug therapy is also required. Very young people are not suitable candidates as they are not emotionally mature and their pattern of hairloss is yet to be established. Like wise, elderly people with extensive baldness are also not good candidates. As in any surgical procedure, history of serious underlying medical illness is a contraindication for hair restoration.

How is hair transplantation carried out? Previously, hair transplantation used to be done by a simple method called punch transplantation in which instruments called punches were used to transplant groups of 8-12 hairs. This method though simple, used to give an artificial, ugly “doll “look. This method has been abandoned in favour of a new method called Follicular unit hair transplantation.

Are there any side effects? When will be the results seen? Since the procedure is performed under local anesthesia, there are no serious side effects. After the surgery, small scabs will form on the scalp at the graft sites, which disappear in 4-7days. A cap will have to be won to hide these scabs. Shampooing can be resumed 24 hours after surgery. Mild swelling of fore head can result on 3-4 days, which is usually mild and subsides in a day. The suture (stitch) used in the donor area is undetectable as it is completely covered by the existing hair and is removed two weeks after surgery. Typically, the grafted hair will shed in 2-4 weeks. New growth will begin in 3-4 months and length will increase approximately 1/2 inch per month. Full results are seen in 8-9 months.

What is laser Hair transplantation? Can artificial hairs be used?

The procedure is performed under local anaesthesia, as follows:

Laser was used to create recipient holes and it was claimed that the use of laser caused less bleeding. However, laser also leads to destruction of tissue which can affect growth of hairs. Hence use of laser is not favoured. Artificial hairs are poyproplylene fibres which look like hairs; these are fixed to the inner most layer of scalp called galea in a knot in a simple procedure. However, these fibres a re on the banned list of FDA in USA and have a history of leading to frequent problems such as infections, allergy, frequent loss of fibres, cysts etc. Hence their use is not advocated. It is important to understand that many treatments are advertised in the lay press; often these treatments are costly and do not have sufficient proof. Hence patients have to be careful and seek proper information before resorting to such treatments.

NASAL RINSE WITH SALINE WATER BENEFICIAL TO KEEP YOUR SINUSES CLEAN Just as we wash our face to freshen up after a long stressful day , several studies have shown that Nasal rinsing with salt water have a surprising change in maintenance of the health of the sinuses .The polluted atmosphere tends to clog them with irritants and dust participles along with thickened mucus. Periodic washing them down with Luke warm salt water through a special pot called the neti pot keeps the nasal sinuses fit and reduces those agonizing headaches. If you are congested use the prescribed entire 8 ounces of salt water otherwise use 4 ounces only.Jala neti Pot specially designed for this purpose has a nozzle which can fix into the nostril .The pot must be filled with the warm saline water. To make the salt water mix half tea spoon full of uniodised salt in 8 ounce glass of warm water Uniodised salt is used because iodized salt may be irritating when used over a periods of time Stand squarely legs a part body at evenly distributed two feet and knee forward in front of a basin. Tilt the head to the right side and place the nozzle in the left nostril. Open the mouth slightly and breathe through the mouth .Keep the whole body relaxed and let the water pass out through the right nostril .When half of the water is used remove the pot remain bending forward let the water run out .Close the right nostril with a finger and blow gently through the left nostril so that the remaining water comes out. Practice in the same way passing the water in the right nostril. Through out the whole process breathe through the mouth. Don’t attempt through the nose. Don’t blow hard. It is best to do Bastrika and Kapalabathi Pranayama after this treatment to help the nose spin dry the excess water and maintain warmth in the nostril.

The same treatment can be taken to reduce cough. ªª A decoction of mango shoots should be taken thrice a day to cure diarrhoea and dysentery. ªª Bum dry mango leaves to ashes. Press the ashes on bleeding cuts and wounds to stop bleeding. They will gradually heal. No other medication may be necessary.

Heat to simmer, stirring, and cook for 1 minute. Peel 4 bananas, slice and add to saucepan. Add 1 cup diced mangos. Heat fruit mixture and simmer, uncovered, 10 minutes, stirring occasionally until mixture thickens. Slice and add remaining bananas to chutney. Add remaining diced mangos and raisins. Stir to mix well, remove from heat, cover and let stand 30 minutes. Serve hot or cold with curries or as a condiment with meat, fish or poultry dishes.

ajor study shows that tobacco-related cancers and cervical cancer are important causes of death among working-age people in India New research analyses cancer death rates across India, and shows that oral, stomach, and lung cancers are important causes of death in Indian men, while cervical, stomach, and breast cancers cause the most cancer deaths among Indian women. The Article, published Online First by The Lancet, is by Professor Prabhat Jha, Centre for Global Health Research, St. Michael’s Hospital and University of Toronto, ON, Canada, and colleagues across India and worldwide. About three-quarters of Indians live in rural areas. Yet mortality for specific cancers has previously been estimated mostly with data from India’s 24 urban population-based cancer registries, with only two registries representing rural areas. In this new study the authors assessed cancer mortality in the Million Death Study (MDS), which is led by the Office of the Registrar General of India, and is one of the few large, nationally representative studies (including rural areas) of the causes of death in any low-income or middleincome country. The authors focus on the geographical and social variation in specific cancers, and the degree to which these cancers might be avoidable by controlling their risk factors or causative agents. The authors found that 7137 of 122 429 study deaths were due to cancer, corresponding to 556 400 cancer deaths across the whole of India in 2010. Some 71% (395 000) of these deaths occurred in people aged 30-69 years (200 100 men and 195 300 women). Cancer deaths accounted for 6% of deaths across all ages, but among the 30-69 years age group this rose to 8% of the 2-5 million total male deaths and 12% of the 1-6 million total female deaths. At 30-69 years, the three most common fatal cancers in men were: oral (including lip and pharynx, 45 800 [23%]), stomach (25 200 [13%]), and lung (including trachea and larynx, 22 900 [11%]. For women, the leading causes of cancer death were cervical (33 400 [17%]), stomach (27 500 [14%]), and breast (19 900 [10%]) cancers in women. Tobacco-related cancers represented 42-0% (84 000) of male and 18-3% (35 700) of female cancer deaths at ages 30-69 years and there were twice as many deaths from oral cancers as lung cancers, in part due to common use of chewing tobacco in men and women. The authors say: “The number of oral cancers was more than twice the number of lung cancers in individuals aged 30-69 years, indicating that the range of fatal cancers caused by tobacco in India differs substantially from that in high-income countries.” Age-standardized cancer mortality rates per 100 000 were similar in rural areas (men 96, women 97) and urban areas (men 102, and women 91, but varied greatly between the states. Mortality rates were two times higher in the least educated than in the most educated adults: men, illiterate 107 vs. most educated 46; and

Cervical cancer was around 40% less common in Muslim than in Hindu women, probably due to high circumcision rates among Muslim men having a protective effect against human papillomavirus (HPV) infection, a causative agent in cervical cancer. In terms of geographical variation, the authors noted that a 30-year old man in northeastern India had the highest chance (11 -2%) of dying from cancer before 70 years of age. By contrast, the risk was less than 3% for men in the adjacent states of Bihar, Jharkhand, and Odissa in eastern India. For women, the highest risk (6-0%) of dying from cancer before 70 years of age was in the northeastern states. Rates of cancer deaths in India are about 40% lower in adult men and 30% lower in women than in men and women in the USA or UK (table 1). However, cancer death rates are expected to rise, particularly with increases in age-specific rates of tobacco smoking. The authors conclude: “Prevention of tobacco-related and cervical cancers and earlier detection of treatable cancers would reduce cancer deaths in India, particularly in the rural areas that are underserved by cancer services. Many people experience some time or the other a bussing sound in their ears, probably after a long tedious period of work. It needs careful attention and detailed investigations from an ENT specialist. as prevailing in big industries. This is supposed to be due to the discharge of nerve impulses in fibers of auditory nerve, a kind of irritative after effective of the intense stimulation. A detailed investigation has to be carried out to ascertain if it is due to lesion in the auditory path. Such tests not only determine the degree of hearing loss in the ears but are also one of the important diagnostic tools to establish the underlying pathological reasons for the occurrence of Tinnitus in an individual. Some of the theories say that a particular point or portion on the basilar membrane of the organ of corti in cochlea of the inner ear. In order to give a relief to the patient if he is suffering with severe Tinnitus affecting even his sleep. Others may in the nerve endings. Some people might even taken tranquillisers in order to get relieved from the annoyance caused by Tinnitus. A few have also suggested ignoring the presence of Tinnitus and not paying any attention to that. But it might be very difficult task for a person to do so. Some may get slight relief if he does valsalva (closing noise and mouth to blow air to the ears). A doctor has to take an elaborate history of Tinnitus in the patient followed by various investigations to arrive at a specific line of treatment. Even then the patient might not get any relief. He may have to live with persisting sound in the ears. Therefore sound in the ear, cannot be ignored by anyone though* treatment is really a challenging one to an ENT specialist in particular and to the doctors in general. But even this is not properly understood. These effects which occur apparently in ear mechanisms are not in the higher analytical level of Brain.

It may occur occasionally or a person might not have given any attention thought it has persisted for a longer time. This sound has greater clinical significance from a diagnostic point of view in the sense that it could either be physiological or psychological. Tinnitus may take various forms. It may be of a rushing or hissing nature or a noise of constant frequency like that obtained from a tuning fork. Jt may very day by day or some times may be less in the morning times than at nights or evenings. It may go on increasing in intensity from morning tili evening. In some persons it may be constant throughout the day or intermittent in nature occurring either in one or both the ears. If he is asked to explain clearly the nature of sound a person may compare it to the humming sound of a bee or to engines whistling noise or to the sound produced by a rotating fan, or to the cracking sound of dried leaves. Persons may even feel as if the sound is approaching him from outside. So each person gives different examples. These subjective example help a doctor to understand whether the sound is high frequency or of low frequency in nature. Tinnitus may also be present along with attacks of dizziness, nausea, vomiting and loss of hearing (as in meniere’s disease) It can also be the result of exposure to high and sudden noise levels like crackers, gun fire or continuous

loud sound develop Tinnitus which even disturb his sleep, if it sleep, if it persists for a long time. Stress and strains due to emotional factors may also give rise to the buzzing sound in ears. Due to ageing process also one may develop Tinnitus. This may be due to the degeneration of nerves in the auditory path. A man who has a strong dosage of antibiotics is likely to get tinnitus. It may appear after One has suffered an attack of typhoid or small pox or chicken pox. Severe injuries to the head or blow to the ears due to accidents may cause tinnitus. If a person is subjected to sudden charges in the atmospheric pressure (as inn the case of fighter and bomber pilots) he may develop tinnitus along with other symptoms.
