D
iabetes is a condition where the amount of glucose in your blood is too high because the body cannot use it properly. This is because your pancreas does not produce enough or no insulin, to help glucose enter your body’s cells - or the insulin that is produced does not work properly (known as insulin resistance). Insulin is the hormone produced by the pancreas that allows glucose to enter the body’s cells, where it is used as fuel for energy so we can work, play and generally live our lives. It is vital for life. Glucose comes from digesting carbohydrate and is also produced by the liver. Carbohydrate comes from many different kinds of foods and drink, including starchy foods such as bread, potatoes and chapatis, fruit, some dairy products, sugar and other sweet foods. If you have diabetes, your body cannot make proper use of this glucose so it builds up in the blood and isn’t able to use glucose as fuel. ¾¾ What is Type 1 Diabetes? Type 1 diabetes develops when the insulinproducing cells in the body have been destroyed and the body is unable to produce any insulin. Insulin is the key that unlocks the door to the body’s cells. Once the door is unlocked glucose can enter the cells where it is used as fuel. In Type 1 diabetes the body is unable to produce any insulin so there is no key to unlock the door and the glucose builds up in the blood. Nobody knows for sure why these insulinproducing cells have been destroyed but the most likely cause is the body having an abnormal reaction to the cells. This may be triggered by a virus or other infection. Type 1 diabetes can develop at any age but usually appears before the age of 40, and especially in childhood. Type 1 diabetes accounts for less than 10% of all people with diabetes and is treated by daily insulin injections, a healthy diet and regular physical activity. ¾¾ What is Type 2 Diabetes? Type 2 diabetes develops when the body can still make some insulin, but not enough, or when the insulin that is produced does not work properly (known as insulin resistance). Insulin acts as a key unlocking the cells, so if there is not enough insulin, or it is not working properly, the cells are only partially unlocked (or

not at all) and glucose builds up in the blood. Type 2 diabetes usually appears in people over the age of 40, though in South Asian and black people, who are at greater risk, it often appears from the age of 25. It is also increasingly becoming more common in obese children, adolescents and young people of all ethnicities. Type 2 diabetes accounts for between 85 and 95 per cent of all people with diabetes and is treated with a healthy diet and increased physical activity. In addition to this, medication and/or insulin are often required. ¾¾ What is Pre Diabetes? Pre diabetes occurs when glucose levels are higher than normal but not high enough to be diagnosed as diabetes. As the name suggests, people with pre diabetes have an increased risk of developing Type 2 diabetes. Having pre diabetes has also been shown to put a person at risk of having heart disease. The important thing to remember is that if you have been diagnosed with pre diabetes, steps can be taken to prevent or delay the onset of Type 2 diabetes ....pg 6

Vasectomy Birth control for men
Vasectomy is a good birth control choice for men who are certain they don’t want to father a child.
V
asectomy is a form of male birth control that cuts the supply of sperm to your semen. It’s done by cutting and sealing the tubes that carry sperm. Vasectomy is straightforward and has a low risk of problems. Before getting a vasectomy, however, you need to be certain you don’t want to father a child in the future. Vasectomy is considered a permanent form of male birth control. ¾¾ Why it’s done Vasectomy is a good birth control choice for men who are certain they don’t want to father a child. ªª Vasectomy is nearly 100 percent effective in preventing pregnancy. ªª Vasectomy is an outpatient surgery with a low risk of complications or side effects. ªª The cost of a vasectomy is far less than the cost of female sterilization (tubal ligation) or the longterm cost of birth control medications for women. ªª You won’t need to take birth control steps before sex, such as putting on a condom. Keep in mind, however, that vasectomy only prevents pregnancy — it offers no protection from sexually transmitted diseases (STDs). ¾¾ Risks For most men, a vasectomy doesn’t cause any noticeable side effects, and serious complications are rare. ¾¾ Side effects right after surgery can include: ªª Swelling ªª Bruising of your scrotum ªª Bleeding or a blood clot (hematoma) inside the scrotum ªª Blood in your semen ªª Infection of the surgery site ¾¾ Delayed complications can include: ªª Fluid buildup in the testicle, which can cause a dull ache that gets worse with ejaculation ªª Inflammation caused by leaking sperm (granuloma) ªª Pregnancy, in the event that your vasectomy fails (rare) ªª Chronic pain (rare) ¾¾ Unfounded concerns Many men worry that a vasectomy could cause serious problems — but these fears are unfounded. For example, a vasectomy won’t: ¾¾ Affect your sexual performance. A vasectomy won’t affect your sex drive or your masculinity in any way other than preventing you from fathering a child.

¾¾ Permanently damage your sexual organs.
There’s very little risk that your testicles, penis or other parts of your reproductive system will be injured during surgery. ¾¾ Increase your risk of certain cancers. Although there have been some concerns about a possible link between vasectomy and prostate and testicular cancer in the past, there’s no proven link. ¾¾ Increase your risk of heart disease. As with cancer fears, there’s doesn’t appear to be any link between vasectomy and heart problems. ¾¾ Cause severe pain. You may feel minor pain and pulling or tugging during surgery, but severe pain is rare. Likewise, after surgery you may have some pain in the surgery area, but for most men it’s minor and goes away after a few days. ¾¾ Desire to father children after a vasectomy If you change your mind about having a child, it may be possible to reverse your vasectomy. However, there’s no guarantee a vasectomy reversal will work. The surgery is more complicated than the original vasectomy, can be expensive and is ineffective for many men. Before you get a vasectomy, you should be

certain you don’t want to father a child in the future. ¾¾ How you prepare Some family medicine or general practice doctors do vasectomies, but most are done by doctors who specialize in the male reproductive system (urologists). Insurance companies vary on their coverage of vasectomies, so check your policy ahead of time. Some doctors and insurance companies require a waiting period between your first meeting with the doctor and surgery. ¾¾ Talking to your doctor Deciding to get a vasectomy can be a tough decision, since you won’t be able to father a child in the future. Before doing a vasectomy, your doctor will want to meet with you to be sure it’s the right form of birth control for you. If you’re in a relationship, bring your partner to your initial doctor’s appointment. ¾¾ At the initial appointment (consultative visit) be prepared to discuss: ªª Whether there’s any chance at all you might want to father a child in the future ªª How your partner feels about the decision, if you’re in a relationship ªª Other methods of birth control available to you ªª What vasectomy surgery and recovery involves, and possible complications ¾¾ Before surgery Your doctor will give you instructions on how to prepare. You may need to: ¾¾ Stop taking aspirin or other blood-thinning medications several days before your surgery. These can include warfarin (Coumadin), heparin, and aspirin and other over-the-counter pain medications such as ibuprofen (Advil, Motrin, others). ¾¾ Wash your genital area thoroughly the day of your surgery. Your doctor may ask you to shave the surgery area. ¾¾ Take medication to relax you 30 minutes to an hour before surgery, if your doctor prescribed it. ¾¾ Arrange for a ride home to avoid movement and pressure on the surgery area caused by driving. ¾¾ What you can expect A vasectomy is usually done at a doctor’s office or surgery center under local anesthesia, which means you’ll be awake and have medicine to numb the surgery area. ¾¾ During surgery Vasectomy surgery usually takes about 20 to 30 minutes. To perform a vasectomy, your doctor will: ¾¾ Numb the surgery area by injecting a local anesthetic into the skin of your scrotum with a small needle. ¾¾ Make a small cut (incision) in the upper part of your scrotum once the surgery area is numb. Or, with the “no-scalpel” technique, a small puncture is made in the scrotum instead of an incision. ¾¾ Locate the tube that carries semen from your testicle (vas deferens) on either the right or left side. ¾¾ Withdraw part of the vas deferens through the incision or puncture. ¾¾ Cut the vas deferens where it has been pulled out of the scrotum.

Seal the vas deferens by tying it, sealing it with heat (cauterizing) or using surgical clips. Then your doctor will return the ends of the vas deferens to the scrotum. ¾¾ Stitch up the incision at the surgery area. Stiches aren’t necessary if your doctor has used the noscalpel technique. ¾¾ Repeat the procedure on the other side. ¾¾ After surgery Following a vasectomy, you’ll have some bruising, swelling and pain. It usually gets better within a few days. Your doctor will give you instructions for recovery. Your doctor may tell you to: ¾¾ Support your scrotum with a bandage and tightfitting underwear for at least 48 hours after your vasectomy. ¾¾ Limit activity after surgery. You’ll need to rest for 24 hours after surgery. You can probably do light activity after two or three days, but you’ll need to avoid sports, lifting and heavy work for a week or so. Overdoing it could cause pain or bleeding inside the scrotum. ¾¾ Use an ice pack periodically to help with swelling and pain for one to three days after surgery. ¾¾ Avoid blood-thinning medications for three to seven days after surgery. These medications can increase your risk of bleeding. ¾¾ Refrain from bathing or swimming for a few days following surgery. ¾¾ Avoid any sexual activity for a week or so. If you do ejaculate, you may feel pain or notice blood in your semen. Don’t have sexual intercourse until your doctor says it’s okay, unless you use another form of birth control. It takes several weeks or longer before sperm are no longer present in your semen. ¾¾ Call right away if you have signs of infection, such as pus or blood oozing from the surgery site, fever, or worsening pain or swelling. ¾¾ Results A vasectomy doesn’t provide immediate protection against pregnancy. Use an alternative form of birth control until your doctor confirms there are no sperm in your semen. Before having unprotected sex, you’ll need to wait up to eight weeks or longer and ejaculate 20 times or more to clear any sperm from your semen. Most doctors do post-surgery checks at six and eight weeks following surgery to be certain that no sperm are present. You’ll need to give your doctor sperm samples to examine. To produce a sperm sample, your doctor will have you masturbate into a container or ask you to have intercourse using a special condom to collect your semen. Your semen is then examined under a microscope to see whether sperm are present. If you’re not in a committed relationship, use condoms during sex — even after you have a vasectomy. Vasectomy is an effective form of birth control, but it won’t protect you or your partner from sexually transmitted diseases (STDs), such as chlamydia or HIV/AIDS.

What is bipolar mood disorder? Bipolar mood disorder, also known as manicdepressive psychosis (MDP), is a type of mood disorder characterised by mood swings, many people experience normal mood swings from “Monday morning blues” to mild euphoria, in bipolar mood disorder, there are swings from extreme depression to great elation or mania and hyperactivity, occasionally at times losing touch with reality. However, there are also long periods of complete normality. Manic depressives suffer from an extreme of mood change over a more prolonged period and are regarded as being clinically ill during these attacks. A doctor or psychiatrist will look at whether an attack of mania is accompanied by depression before diagnosing bipolar mood disorder. Bipolar mood disorder typically begins in adolescence or early adulthood and continues throughout life, ft is often not recognised as an illness and people who have it may suffer needlessly for years or even decades. ¾¾ What causes bipolar mood disorder? Much research is being carried out as to the causes of bipolar mood disorder but there are no definite conclusions yet. There may be a genetic predisposition to the illness such that other family members may also suffer from this illness. It could also be triggered by major stress events or hormonal changes such as those occurring after childbirth. It may also be due to a chemical imbalance affecting that area of the brain responsible for “moods”, thus causing changes in feelings and behaviour.

This is a state in which the person shows a high level of energy, excessive moodiness or irritability, and impulsive or reckless behaviour, Hypomania may feel good to the person who experiences this and the sufferer seldom thinks that anything is wrong and may object quite violently if anyone tries to point this out to him or her. With experience, it is possible for some people to recognise their condition but often in an extreme state, all understanding is lost, H is not unusual for someone suffering an attack to break the law perhaps

during a flight of fancy, about of excessive spending, committing a fraud, or fortunately, very rarely, of committing a physical assault. 3. Mixed States A few people sometimes experience a combination of symptoms from both mania and depression. For example, a high level of activity may be combined with a low level of concentration and a depressed attitude towards life. This can lead to an increased risk of suicide attempt. 4. Normal phase After a period of depression or of mania, the sufferer usually return to a normal state, back to one’s usual personality, thinking capabilities and energy levels without any physiological damage. However, the sufferer may start regretting the damage done to relationships or finances. There may be a loss of confidence and also embarrassment. ¾¾ How frequent are the attacks The frequency of attacks varies not only from one individual to another but also within the same individual. Some have a regular cyclic pattern: daily, monthly, yearly or every few years. Other people go for years free from the problem and then have a series of random attacks. ¾¾ Why is treatment necessary? If untreated, sufferers will probably recover by themselves in time but often with devastating effect to their lives, their families and considerable unnecessary suffering. Wherever possible, early admission to hospital either for mania or severe depression does facilitate a quicker recovery. In addition, in-patient treatment also helps to protect the patient from the consequences of the illness, such as rash judgments in mania or suicide attempts in severe depression. ¾¾ How is Bipolar mood disorder treated? Mood swings in many cases (60-80%) can be controlled or reduced by medications such as mood stabilisers. The most frequently used medication is lithium carbonate, carbamazepine and divalproex sodium. At times other drugs are also used to treat either the depression or the mania. Sometimes, more than one drug is required. These drugs Include antidepressants, antipsychoticsand benzodiazepines. ror every drug, there is a “side effect”. These drugs are used only hasten a return to normality and it is seldom that they have o be continuously given. Another treatment sometimes used in cases of

sever depression and occasionally mania is ElectroConvulsive Therapy (ECU When dministered, the patient is given a muscle relaxant and a short acting general anaesthetic. A strictly controlled electrical current is assed across the brain from electrodes placed against the head. There may be a loss of memory for the period of treatment but there is no evidence of long-term effects on memory. Generally severe depression can be successfully improved by ETC. ¾¾ How to cope with the illness? The effects of the mood swings put a great strain on marriages and children (or parents), and on employers and friends as well as the individual sufferer. In a number of cases this leads to isolation, loss of job, or loss of home, making for further strains and increasing the loneliness of the sufferer. Hence this illness affects the sufferer, the family and society in different ways, ¾¾ The Sufferer Sufferers need to admit that they are ill and this is not easy. He or she may have to take mood stabiliser, in one form or another, for an appreciable length of time, possibly even for the rest of their lives. Try to recognise symptoms early as it is sometimes possible to stop a mood change from becoming more intense either by early drug treatment or by avoiding difficult situations. A change in sleep pattern is often a useful indicator of early relapse. Prevent complications by not abusing alcohol and caffeine products (tea, coffee, coke) and maintaining a balanced diet. As the illness can be stress-related, learn what causes you stress and plan your life to try and avoid unnecessary stress. Never make major decisions when ill. Should you have a tendency to spend money when “high”, then consider putting your affairs into a trust or appoint attorney to act on your behalf. Recognise that those close to /ou are trying to help. ¾¾ Relatives and friends First of all, loved ones and relatives should recognise that the strange behaviour, violent or abusive language or prolonged silences, which characterise certain phases of the illness, is not a previously hidden facet of the personality but a symptom of the mood disorder. Understand that you may have to lock away or hide household poisons or tablets on certain occasions or at other times go out and find the sufferer. You may also have to encourage the person to visit the psychiatrist, or ask for urgent help whenever the sufferer is so “high” that he or she is no longer aware that anything is wrong. The patient is made to lie in the lithotomy position, the pile mass is identified and is ligated with the thread. The procedure is completed in 10-15 minutes with very mild pain and with no bleeding. The mass falls in 5-7 days. The patient can resume routine work from the day of treatment and is on medication for a week. Fistula is a blind tract connecting the anal canal to the exterior. During treatment the tract is identified with the help of a probe and the thread is inserted and knotted at the end. It has to be changed once in a week for about 3-7 weeks depending upon the length of the tract. It heals on its own without relapse. In ayurveda, the treatment is based on the digestive capacity or agni of the patient. Impairment in the digestive capacity aggravates the piles. The medication also aims to improve the digestive capacity if in the primary stage, the disease can be treated with medicines. Buttermilk with salt, light easily digestible food and carminatives like ginger, pepper, leafy-vegetables, green vegetables, warm water intake are recommended. Sits bath, cotton seats and cotton mattresses are recommended. Spicy. heavy, non-vegetarian food and curd should be avoided. Riding, travelling long distance, sitting over foam, sponge seats and mental strain must be avoided.

In a woman who has healthy reproductive organs but has blocked or damaged tubes it is responsible for approximately 25% to 35% of all female factor infertility. What are Fallopian tubes? Fallopian tubes are tube like hollow structures connected to each side of the uterus and extending to the surface of each ovary, The end of each tube adjacent to the ovary is flared open and consists of many fin like delicate finger structuresknown as fimbriae. When the ovary releases the egg these finger like structures pick up the egg and direct it to the uterus via the tube. The tube lining provides a very comfortable andhospitable atmospherefor the sperm on its voyage to fertilise the egg.
The internal wall or lining of the tube is also important for the fertilisation which usually happens in the portion farthest from the uterus called the distal portion of the tube. The tube is vital for the survival of the fertilized egg which moves through the fallopian tube for up to 5 days before it passes into the uterus and implants on the walls of the uterine cavity . ¾¾ What are the Necessary functions of the

Fallopian tube plays an important role in the mechanical transportation and physiological sustenance of early conceptus. Damage to the fimbriae may reduce the ability to pick up the egg and direct it to the tube. Damage to the cells lining the tube may prevent or greatly reduce the chances of fertilisation. Blockage in the tube can prevent the fertilised egg from moving to the uterus, increases the chances of ectopic pregnancy. Tubal factor is an anatomic abnormality that prevents the union of sperm and egg from reaching the distal portion of the fallopian tube where fertilisation normally occurs. Distal tube disease can range from mild fimbrial adhesions to severe complete occlusion .If blockage occurs only at the distal end then the secretions of the fallopian tube will not be able drain out of the end of the tube .The resulting accumulation of fluid in the tube has a very negative affect on fertility .Proximal obstruction uterotubal obstruction occurs in 10-20%of woman with tubal disease and is due to muscular spasm , stromal oedema , amorphous debris , mucosal agglutination , viscous secretions others include corneal polyps , chronic salpingitis , endometriosis , intra uterine synechiae , parasite infection. ¾¾ How do tubes get blocked? Tubes can get blocked when they are not able to perform their mission /assignment, because of their inactivity – a functional problem.Pelvic inflammatory disorders e.g. pelvic infection like gonorrhoea, Chlamydia can cause inflammation and infection in the tubes. These infections can damage the cilia the tiny hairs lining the fallopian tube that are responsible for transporting egg. When the cilia fail to perform their action they often prevent the sperm and egg from meeting or if an egg by chance isfertilised (the meeting of a sperm and egg resulting in pregnancy), because of inability to travel to the uterus the fertilised egg starts growing instead in the wall of the fallopian tube. emon grass is the source of ‘Lemon grass Oil’ obtained from the leaves and shoot of the plant. The lemon grass oil is mainly used in the manufacture of perfume for soaps, hair oils, scents and medicines. It also has the antibacterial properties. Lonone prepared from the citral present in lemon grass oil is one of the most important raw materials for the preparation of vitamin ‘A’. In addition to its use in perfumery, lonone is used in certain kinds of confectionery and liquors. lonone can be prepared either directly from the lemon grass oil or from the citral obtained from the oil. The oil can be used to improve the flavour of some fish and can be used to flavour wines, sauces, etc. It can also be used for headaches and toothaches and for baths and fomentations, particularly in female complaints. It is used as a diuretic agent and as an insect repellent. ¾¾ PARTS USED Leaf and the oil of the grass ¾¾ CHEMICAL COMPOSITION Citral is present in the grass ¾¾ MEDICINAL USES ªª If small piece of the grass is added when the tea is prepared it increases appetite and even good for cough and cold. ªª Decoction of the grass taken with milk and sugar controls pain due to dyspepsia. Dose: 1-3 years.— 1 spoon three times a day. 3-6 years.— 2-3 spoons three times a day. ªª Decoction of the grass mixed with V* spoon of ginger juice and sugar cures fever. Dose: 1-3 years — One spoon 3- 6 years — 2-3 spoons ªª In abdominal pain during menstruation decoction

(4-6 spoons) mixed with a pinch of pepper powder gives good relief. Decoction mixed with mint juice, pepper powder/ dried ginger, and sugar controls abdominal pain, burning sensation. This is to be given often with a gap of 2-3 hours. 3 - 6 drops of the oil with half glass of water and sugar controls abdominal pain and vomiting. The oil mixed with coconut oil can be used for massage in joint-pain or in rheumatism. Massage the face with the oil once a week and take fomentation. This improves facial complexion. Ten drops of oil mixed in hot water used for bath keeps body fresh and skin cracks will not appear in winter. Few drops of the oil sprayed to the pillow keeps fresh and good against Insomnia, 3-4 drops of oil in the handkerchief, and smelling the same often gives good relief for cold and cough. RECIPE Tea: While preparing the regular tea the lemon grass can be added along with the tea leaf/ powder. It gives a soothing effect and is good for cold and cough. It is said to increase the digestive power. Thambuli: The grass blade is cut into small pieces. It is roasted along with cumin seeds. Grind the roasted items along with grated coconut and salt. Add the paste to buttermilk or curds.

Schizophrenia, Bipolar Disorder and Major Depression are serious mental disorders which take a toll on both patients and their caregivers. They persist for prolonged periods and create serious disability, interpersonal and work problems, and lead to cognitive decline. While medicines have remarkable and quick ameliorative effects, they cannot prevent cognitive disability due to these illnesses. Thus interventions to improve cognitive capacity are necessary. Cognitive enhancement therapies have emerged to treat cognitive impairment. Different cognitive training programs help improve sustained attention and language processing, planning and executing activities, affect recognition, verbal memory, working memory, processing speed and social problem solving.

Yoga -the holistic science enhancing powers of the mind as well as the body- should be investigated as a economical, effective and culturally acceptable cognitive enhancement program. Yoga can have a profound influence on physical, mental, emotional, behavioral, social and spiritual aspect of life. Yoga facilitates in managing or controlling anxiety, improves muscle-tone, strength, reduces stress and tension, improves concentration and creativity, and creates a sense of well being and calm. Several studies have proved the positive effects of yoga in various psychiatric and cardiac disorders. The present project, for which this protocol was developed, was carried out to determine the effects of yoga on various aspects including cognitive functions, on persons suffering from major mental disorders and on their caregivers. For comparison, stable cardiac patients participated in the same set of yoga practices. Hence a special protocol of yoga practices needed to be developed. These yoga practices could be performed by patients with impaired attention, concentration and poor physical stamina. In addition older cardiac patients, and older caregivers of patients, could also perform these exercises.

The present yoga protocol, which does NOT represent the full complexity of yoga practices, was specifically developed by yoga experts for this purpose. It is simple, does not take too long and can be carried out at home. Most of the asanas used benefit nerves, mind and concentration. If both patient and caregiver perform these practices daily we are confident that they will benefit greatly. ¾¾ Dos and Don’ts ªª Arrive early so that you can settle in and get into the proper frame of mind. ªª Remove your shoes outside the yoga Hall. ªª Do not bring your mobile phone to yoga class. ªª Wear neat and clean loose clothes so that asanas can be performed easily and comfortably. ªª Do not speak or disturb others, be calm and peaceful to derive full benefits. ªª Have a bath before yoga class, and bring a towel if you sweat a lot. ªª Don’t eat for two or three hours before class. If you practice yoga on a full stomach, you might experience cramps, nausea, or vomiting. Digesting food also takes energy that can make you lethargic and decrease your performance. ªª Your bowel should be empty before yogic exercises. ªª Don’t enter class late or leave early as it will disturb others. ªª Please let the trainer know beforehand, about injuries or conditions that may affect your practice. She/he will help you to simplify or avoid asanas you find difficult. 1 Don’t try to be perfect the first time, especially if you have never practiced asanas before. Instead of trying to go as deeply or completely into a pose as others might be able to do, do what you can without straining

or injuring yourself. Breathe only through the nose, because by doing so the air which you take in is filtered. Nasal respiration keeps the temperature of the Nadis (Vessels) -’Ida’, Pingala and ‘Sukshma’ even. It also prevents foreign and harmful objects from getting into lungs. Don’t miss Shavasana which helps your muscles to relax after the hard work of asanas. Go to the Bathroom during Resting Poses only. It is permissible to leave class for a few minutes to go to the bathroom quietly and without disturbing others. There is no need to ask for permission. Take some time afterwards to remember what you did in class, so you can retain what you learned. Review the poses you practiced, and note down any instructions that are more useful for you. Even if you remember just one thing from each class, it will provide a lot of information that can improve your own practice.

nsomnia or sleeplessness has assumed alarming proportions in present times, especially among the upper classes in urban areas. This is evident from the wide range of medication for this condition prescribed by physicians and sold by chemists. Instances of persons taking an overdose of sleeping pills with fatal results are quite frequent. Insomnia deprives a person of mental rest and thereby interferes with his activities in the daytime. It constitutes a severe health hazard when it becomes a habit. Sleep is a periodic state of rest for the body which is absolutely essential for its efficient functioning. Sleep gives relief from tension, rests the brain and body and a person wakes up in the morning fresh and relaxed after sleep. The amount of sleep, however, varies within very wide limits from individual to individual. Normally, seven to eight hours of sleep every night is adequate for most people. Some, however, do well with four to five hours because their sleep is deeper and more refreshing. Insomnia is common among the elderly for a variety of reasons. The sleep of the elderly is often punctuated by brief periods of wakefulness during the night. In such cases it is the quality rather than the quantity which is most affected. With age, there is gradual reduction of periods of deep sleep. The older person, therefore, gets roused easier. Sleep requirements

also diminish with ageing. From nine hours of sleep per night at the age of 12 the average sleep needs decrease to eight hours at the age of 20 , seven hours at 40 , six and half hours at 60 and six hours at 80. Symptoms: The signs of pathological insomnia are dramatic changes in the duration and quality of sleep, persistent changes in sleep patterns, lapses of memory and lack of concentration during the day. Other symptoms are emotional instability, loss of coordination, confusion and a lingering feeling of indifference. Causes: The most common cause of sleeplessness is mental tension brought about by anxiety, worries, overwork and overexcitement. Suppressed feelings of resentment, anger and bitterness may also cause insomnia. Constipation, dyspepsia, over – eating at night, excessive intake of tea or coffee and going to bed hungry are among the other causes. Smoking is another unsuspected cause of insomnia as it irritates the nervous system, especially the nerves of the digestive system. Often, worrying about falling asleep is enough to keep one awake. The Cure: Sleeping pills are no remedy for sleeplessness. They are habit forming and become less effective when taken continuously. They lower the I.Q., dull the brain and can prove fatal if taken in excess or before or after alcohol. The side effects of sleeping pills include indigestion, skin rashes, lowered resistance to infection, circulatory and respiratory problems, poor appetite, high blood pressure, kidney and liver problems and mental confusion. To overcome the problem, one should adhere to a regular sleeping schedule, going to bed at a fixed time each night and getting up at a fixed time each morning. Early to bed and early to rise is a good rule. Two hours of sleep before midnight are more beneficial than four after. It is sheer folly for students, at examination times, to keep awake till long after midnight, drinking one cup of tea after another, as that is only apt to cause blackness and inability to concentrate in the examination hall. Research has shown that people with chronic insomnia almost invariably marked deficiencies of such key nutrients as B complex vitamins, and vitamin C and D as also calcium, magnesium, manganese, potassium and zinc. The sleep mechanism is unable to function efficiently unless each of these nutrients is present in adequate amounts in the diet. A balanced diet with simple modifications in the eating pattern will go a long way in the treatment and cure of insomnia. Such a diet should exclude white flour products, sugar and its products, tea , coffee, chocolate, cola drinks, alcohol, fatty foods, fried foods, foods containing additives, that is chemicals for preserving, colouring and flavouring, excessive use of salt and strong condiments. In the modified eating pattern, breakfast should consist of fresh and dried fruits, whole cereals, seeds and yogurt. Of the two main meals, one should consist of a large mixed salad and the other should be protein based. A cup of milk

sweetened with honey at bedtime is helpful as the amino - acid tryptophan contained in milk induces sleep. Sleep is often elusive. Any attempt to force it only drives it further away. It is better to divert the mind with soft music or light reading. While going to bed, visualise a blank black wall occupying the entire field of vision. Turn your thoughts to light and cheerful matters. Use light bed clothes and relax. Do not lie on your back, put on your side with one or both knees brought well up and the head and shoulders slightly forward. During the night, the position of the arms and legs should be changed frequently and a healthy sleeper usually shifts from one side to the other several times in the course of the night. Controlled breathing is also a great help in inducing sleep. The method is to lie on your side in bed, and then take three deep breaths expanding the abdomen completely. Then hold your breath as long as you can. Next, take three more breaths and repeat the breath – holding . While you hold your breath, carbon dioxide accumulates in the body and induces natural sleep. Regular, active exercising during the day and mild exercise at bedtime enhances the quantity and the quality of sleep. Exercise stimulates the elimination of lactic acid from the body which correlates with stress and muscular tension. Regular exercise also produces hormonal changes which are beneficial to the body and to the sleep pattern. Walking, jogging, skipping, swimming are all ideal exercises. Vigorous exercise should, however, be avoided at night as this can be over stimulating. Yogasanas: Yoga helps a majority of cases of insomnia in two ways. Firstly, yoga treatment helps tone up the glandular, respiratory and nervous system. Secondly, yoga also gives physical and mental relaxation as a safety value for one’s disturbing problems. The traditional yogasanas which are effective for insomnia patients are shirsana, sarvangasana, paschimottanasana, uttanasan, viparitakarni and shavasana. Hydrotherapy is also effective in treatment of insomnia. Application of hot packs to the spine before retiring, hot fomentation to the spine, hot foot bath or an alternate hot and cold foot bath at bedtime are all time – tested methods. The cold hip bath with the feet in hot water and the prolonged neutral immersion bath (92 o to 96 o F) at bed time, when one’s nerves are usually irritable, are also effective measures. Along with the various measures for the treatment of insomnia, all efforts should be made to eliminate as many stress factors as possible. The steps in this direction should include regular practice of any relaxation method or meditation technique, cultivating the art of doing things slowly (particularly activities like eating, walking and talking) limiting the working day to nine to ten hours and five and a half days weekly, cultivating a creative hobby and spending some time daily on this, avoiding working against unrealistic targets and completing one task before starting another.

With a win / win solution, all parties feel good about the decision and feel committed to the action plan. Win / Win see life as a cooperative, not a competitive area. One alternative to win / win is win / lose, the authoritarian approach. Most of us are deeply influenced by this win / lose scripting. Some of us programmed the other way-lose / win. Lose / win is worse than win / lose because it has no standards, no demands, no expectations, no vision. People who think lose / win are usually work to please or applause.
Then there is another version of that provokes a condition of lose / lose. When two win / lose people get together-that is when two ego-invested individuals interact-the result will be lose / lose. Both will lose. Another common alternative is simply to think win. People with win /win mentality don’t necessarily want someone else to lose. The Institute of Public Health (IPH), Bangalore and the Institute of Tropical Medicine (ITM), Antwerp has jointly organized a national conference on health in collaboration with the Karnataka State Health Systems Resource Centre (KHSRC), Bangalore (Government of Karnataka) and the National Health Systems Resource Centre, (NHSRC) New Delhi (Government of India). The theme of the conference was “Strengthening health systems to achieve universal health coverage”. Aravind Limbavali, Minister for Health and Family Welfare, Government of Karnataka inaugurated National Conference. Over 300 delegates including leading researchers, practitioners, and policymakers in public health discussed on the Indian health system. The Minister said “The Karnataka Government is committed to move towards ensuring universal health coverage for all our people, especially the poor and needy who suffer from the difficulties of paying for healthcare. There is also the challenge of healthcare in cities and towns. Cities and towns are expanding in many parts of our state with the problems of solid waste management, urban housing etc. Although, many private hospitals, nursing homes and clinics are doing a good job, we need to ensure that they are also providing good quality and affordable services. In this respect, in Karnataka, we have the Karnataka Private Health Establishment Act, which will help us regulate private sector. At the same time, the urban primary health centres system needs to be strengthened. Again, this is very important for the huge number of poor people who live in urban areas He also released a book “Towards Universal Health Coverage: An Operational Manual for States in India” developed by Institute of Public Health. Medical and technological advancements have made treatment of spinal fractures very effective with the help of procedures like Kyphoplasty or Vertebroplasty. Between 25% and 60% of women, aged over 60 years develops spinal compression fractures. There are two interesting features about the Osteoporosis in India the high incidence among men d the lower age of peak incidence as compared to Western countries. The incidence of Osteoporotic hip fractures is 1 woman: 1 man in India, while in the Western world, it is 3 women: 1 man. And in most Western countries, While the peak incidence of Osteoporosis occurs at about 70-80 years of age, in India it afflicts those at age 50-60.” Speaking on the occasion, to mark WORLD OETEOPOROSIS DAY at jain hospital Dr. Upoendra, Spine Surgeon,Jain Institute of Spine Care & Research said, “Spinal fractures caused by osteoporosis are all too often dismissed as simple back pain or arthritis hence and so often do not come to clinical attention, thereby remaining undiagnosed and untreated. Even when referred to a doctor, only about 40% of older women with spinal fractures visible on X-ray are tested for osteoporosis and the figure is even lower in men (less than 20%). It is essential that spinal fractures are identified and treated before further fractures occur. Unless treated, as many as one in five women with a spinal fracture will sustain another within twelve months.” He further added, “Data suggests that even amongst patients who are recommended a surgical intervention for spinal fracture, very little percentage actually receive the right treatment. One of the major reasons for patients dropping out of surgical procedure is ‘fear’ of surgery. Unfortunately, despite
the marked improvement that medical science has made over the decades, Indian patients suffer from a psycho fear and prefer leading their life in pain rather than getting the right treatment. Procedures like Balloon Kyphoplasty, Vertebroplasty etc. when performed on right patients by trained physician can lead to miraculous improvement in patient’s quality of life. Dr. Upendra further said, “Spinal fractures represent a significant socioeconomic burden. It is estimated that around the world, one spinal fracture occurs every 22 seconds. Conservative estimates suggest that India will have 36 million Osteoporotic patients by 2013. Therefore, there will be a marked rise in Osteoporotic fractures and associated costs in decades to come. Hence, prevention of all Osteoporotic fractures, early diagnosis and treatment must be a key public health goal. This will help reduced disease burden along with significant improvement in quality of life of those who suffer from Osteoporotic Spinal fractures. Open spinal surgery is an invasive procedure that is generally considered if neurological damage is present. A less invasive surgery technique called KYPHON® Balloon Kyphoplasty can treat pain and improve quality of life faster and better than conventional non-surgical treatment and has proven safety. Balloon Kyphoplasty is a minimally invasive procedure performed by a spine specialist. It can be performed using either a local or general anesthesia. It typically takes about one hour to treat each fracture and may require an overnight hospital stay. Balloon Kyphoplasty gently restores the height of the vertebra and corrects deformity by inflating two balloons in the fractured vertebra through small incisions in the skin. Once height is restored, the balloons are removed and the cavity they create is filled with thick bone cement at low pressure. egarding increasing number of suspected dengue fever cases in Bangalore city, BBMP health department has started intensive dengue awareness campaign. Nursing College, Medical College, and NSS students totalling to about 1200 members are participating in IEC activities. They
are Identifying and eradicating mosquito breeding places and also creating awareness about control of dengue fever by source reduction and mosquito bite protection methods. BBMP has provided caps and I’d cards to all volunteers for identification houses. More than 1 lakh houses are surveyed, 1800 houses and buildings have been found to have mosquito breeding places. Each teams educated all house owners regarding dengue fevers. Thousands of rag pickers were given free pre exposure prophylaxis against rabies Human Biologicals Institute (HBI), a division of Indian Immunologicals Ltd (IIL), in association with Bruhat Bengaluru Mahanagara Palike (BBMP) organized a free pre exposure prophylaxis camp against rabies for rag pickers recently. Dr S N Madhusudhana, Professor Neurovirology, NIMHANS, Bangalore inaugurated the free vaccination camp at Rag pickers colony, Sarjapur road, Speaking on the occasion, Dr Parviz Ahmed Piran, Joint Director Animal Husbandry Dept of BBMP said “Rag pickers, Municipal workers, Animal Handlers, veterinarians etc are at a high risk of rabies as they are continuously exposed to the animals. This gesture of HBI in immunizing nearly around 1000 rag pickers would help in protecting them from Rabies” he said. Pre exposure prophylaxis comprises of 3 doses of anti rabies vaccine on day 0, 7 & 28 to the high risk groups in order to protect them from in inadvertent exposure to rabies. They require annual booster doses to maintain immunity levels. Stating on the occasion, Mr. N Anand, General Manager – Human Health, HBI, said “We have been conducting free vaccination camps against rabies for the street dogs & pet for the past several years in various locations in India with the help of respective state governments. This is for the first time we are conducting free pre exposure prophylaxis camp against rabies for rag pickers. Ranka Steels conducts free Eye Camp
The people of the villages surrounding Chickballapur thronged Jain Bhavan for the free eye camp that was set up in this tiny village, courtesy Ranka Steels with the support of Project Dhristi recently. Over 400 people were screened for various eye ailments. Around 82 surgeries were conducted. Approximately 160 of these patients were provided with muchneeded spectacles. Along with this eye camp, a small dental camp was conducted alongside. Most of the people screened on the day had their teeth cleaned. About 40 needed treatment and this was conducted free of cost. This camp was a success thanks to the 20 nurses and doctors. For those with eye ailments and who needed immediate attention, it was provided on the spot. Each and every one of the surgeries scheduled involved having the patients picked up from their homes with their families. Everything related to their care – accommodation, food, pre- and post-operative care as well as transportation back home for all, were provided free of cost. “What is important to note is that we make no distinction in eye ailments that we handle. We consider everything from cataracts to more complicated procedures,” said Mr Ashok Ranka, Director, Ranka Steels. Patients are provided free transportation, food and accommodation as well as medication to complete the cycle of care. Almost every one of the cataract surgeries are suture-less along with IOL or Intra Ocular Lenses being implanted as well. Post-operative care is also taken care. Bone Marrow is responsible for the production of Red Blood Corpuscles, White Blood Corpuscles and platelets. It involves the following steps: 1. Stem cells are mobilized from the Bone Marrow into the peripheral blood by giving the donor Granulocyte Stimulating Factors as subcutaneous injections. 2. These stem cells are harvested from the donor with the help of a cell separator over 3 days. 3. Two days later a high dose of chemotherapy is given to the patient. 4. Then the cryopreserved stem cells are thawed and reinfused into the patient intravenously.
