emorrhoids (HEM-uh-roids), also called piles, are swollen and inflamed veins in your anus and lower rectum. Hemorrhoids may result from straining during bowel movements or from the increased pressure on these veins during pregnancy, among other causes. Hemorrhoids may be located inside the rectum (internal hemorrhoids), or they may develop under the skin around the anus (external hemorrhoids). Hemorrhoids are common ailments. By age 50, about half of adults have had to deal with the itching, discomfort and bleeding that can signal the presence of hemorrhoids. Fortunately, many effective options are available to treat hemorrhoids. Most people can get relief from symptoms using home treatments and lifestyle changes. ¾¾ Signs and symptoms of hemorrhoids may include: ªª Painless bleeding during bowel movements — you might notice small amounts of bright red blood on your toilet tissue or in the toilet bowl ªª Itching or irritation in your anal region ªª Pain or discomfort ªª Swelling around your anus ªª A lump near your anus, which may be sensitive or painful ªª Leakage of feces Hemorrhoid symptoms usually depend on the location. Internal hemorrhoids lie inside the rectum. You usually can’t see or feel these hemorrhoids, and they usually don’t cause discomfort. But straining or irritation when passing stool can damage a hemorrhoid’s delicate surface and cause it to bleed. Occasionally, straining can push an internal hemorrhoid through the anal opening. This is known as a protruding or prolapsed hemorrhoid and can cause pain and irritation. External hemorrhoids are under the skin around your anus. When irritated, external hemorrhoids can itch or bleed. Sometimes blood may pool in an external hemorrhoid and form a clot (thrombus), resulting in severe pain, swelling and inflammation. ¾¾ When to see a doctor Bleeding during bowel movements is the most common sign of hemorrhoids. But rectal bleeding can occur with other

Involvement in community works helps elderly persons not only share their experience with the youth but also help to remain fit and active.

T

he Indian family has traditionally provided natural social security to the old people. However, in recent times, with joint families breaking up, elderly increasingly find themselves having no space in a nuclear family. Several face destitution to fend themselves as their children don’t live with them. Some faces destitution due to neglect and property disputes. Lonely families is become a common factor in cities. There are ways where aged or elderly people can live happily by actively involving in numerous activities. Elderly people can share their long-cherished career experience with young people in several ways. This will also help aged people to remain active and healthy. ¾¾ Tips to remain active The awareness campaigns become successful with the involvement of the senior citizens. The young people who are residing in apartments can rope in the services of senior citizens in various association activities of the apartment during the festival or normal days. Nominating or electing them as the president or officebearer of the association will not only bring in their experience but also helps them remain active and fit. Meanwhile, they can also motivate younger ones to involve more in such activities. Offer praise and support when the elderly person completes the activity. Acknowledgement is important to everyone and the elderly are no different. Praise can go a long way to helping a senior feel motivated to continue the activity in the future. ¾¾ Other tips that will help elderly to remain active are: ªª Involving in community/voluntary activities of a school, club, association, temple civic body or NGO. For example a elderly doctor can offer subsidized or free medical services in a locality ªª Supporting their children in various activities without disturbing latter’s work. ªª Reading books, writing to newspapers, attending lectures and cultural activities ªª Regular walking and excise and traveling Old age homes should be banned so that children are forced to take care of their aged parents. If you have signs and symptoms of hemorrhoids, make an appointment with your regular doctor. Depending on your signs and symptoms, your doctor may refer you to one or more specialists - including a doctor with expertise in the digestive system (gastroenterologist) or a colon and rectal surgeon — for evaluation and treatment. You can help your doctor by being prepared with as much information as possible. Here are some suggestions to help you get ready for your appointment. ¾¾ What you can do ªª Be aware of any preappointment restrictions. At the time you make the appointment, ask if there’s anything you need to do in advance. ªª Write down any symptoms you’re experiencing and how long you’ve noticed them. ªª Write down key personal information, including typical bowel habits and diet, especially your fiber intake. ªª Make a list of all medications, as well as any vitamins or supplements, that you’re taking. ªª Write down questions to ask your doctor. Your time with your doctor is often limited, so preparing a list of questions will help you make the most of your time together. For hemorrhoids, some questions you might want to ask your doctor include: ªª What’s the likely cause of my symptoms? ªª Is my condition likely to be temporary or permanent? ªª Am I at risk of complications related to this condition? ªª What treatment approach do you recommend? ªª If treatments we try first don’t work, what will you recommend next? ªª Am I a candidate for surgery? Why or why not? ªª Are there any additional self-care steps that might help? ªª I have other medical problems. How can I manage these along with hemorrhoids? In addition to the questions that you’ve prepared, don’t hesitate to ask questions during your appointment at any time that you don’t understand something. ¾¾ What to expect from your doctor Your doctor is likely to ask you a number of questions. Being ready to answer them may reserve time to go over points you want to spend more time on. Your doctor may ask: ªª When did your symptoms first occur? ªª How uncomfortable are your symptoms? ªª What are your typical bowel habits? ªª How much fiber does your diet contain? ªª What, if anything, seems to improve your symptoms? ªª What, if anything, appears to worsen your symptoms? ªª Has anyone in your family ever had hemorrhoids or cancer of the colon, rectum or anus? ªª Have you had a change in your bowel habits?

diseases, including colorectal cancer and anal cancer. Don’t assume that bleeding is coming from hemorrhoids without consulting a doctor. Your doctor can do a physical examination and perform other tests to diagnose hemorrhoids and rule out more serious conditions or diseases. Also consider seeking medical advice if your hemorrhoids cause pain, bleed frequently or excessively, or don’t improve with home remedies. If your hemorrhoid symptoms began along with a marked change in bowel habits or if you’re passing black, tarry or maroon stools, blood clots or blood mixed in with the stool, consult your doctor immediately. These types of stools can signal more extensive bleeding elsewhere in your digestive tract. Seek emergency care if you experience large amounts of rectal bleeding, lightheadedness, dizziness or faintness. ¾¾ Causes The veins around your anus tend to stretch under pressure and may bulge or swell. Swollen veins — hemorrhoids — can develop from an increase in pressure in the lower rectum. Factors that might cause increased pressure include: ªª Straining during bowel movements ªª Sitting for long periods of time on the toilet ªª Chronic diarrhea or constipation ªª Obesity ªª Pregnancy ªª Anal intercourse It’s also possible to inherit a tendency to develop hemorrhoids. Hemorrhoids are more likely as you get older because the tissues that support the veins in your rectum and anus can weaken and stretch with aging. ¾¾ Complications Complications of hemorrhoids are rare but include: ªª Anemia. Chronic blood loss from hemorrhoids may cause anemia, in which you don’t have enough healthy red blood cells to carry oxygen to your cells, resulting in fatigue and weakness. ªª Strangulated hemorrhoid. If blood supply to an internal hemorrhoid is cut off, the hemorrhoid may be “strangulated,” which can cause extreme pain and lead to tissue death (gangrene). ¾¾ Preparing for your appointment

For persistent bleeding or painful hemorrhoids, your doctor may recommend another minimally invasive procedure. These treatments can be done in your doctor’s office or other outpatient setting. ªª Rubber band ligation. Your doctor places one or two tiny rubber bands around the base of an internal hemorrhoid to cut off its circulation. The hemorrhoid withers and falls off within a week. This procedure — called rubber band ligation — is effective for many people. Hemorrhoid banding can be uncomfortable and may cause bleeding, which might begin two to four days after the procedure but is rarely severe. ªª Injection (sclerotherapy). In this procedure, your doctor injects a chemical solution into the hemorrhoid tissue to shrink it. While the injection causes little or no pain, it may be less effective than rubber band ligation. ªª Coagulation (infrared, laser or bipolar). Coagulation techniques use laser or infrared light or heat. They cause small, bleeding, internal hemorrhoids to harden and shrivel. While coagulation has few side effects, it’s associated with a higher rate of hemorrhoids coming back (recurrence) than is the rubber band treatment. ¾¾ Surgical procedures If other procedures haven’t been successful or you have large hemorrhoids, your doctor may recommend a surgical procedure. Surgery can be performed on an outpatient basis or you may need to stay in the hospital overnight. ªª Hemorrhoid removal. During a hemorrhoidectomy, your surgeon removes excessive tissue that causes bleeding. Various techniques may be used. The surgery may be done with a local anesthetic combined with sedation, a spinal anesthetic or a general anesthetic. Hemorrhoidectomy is the most effective and complete way to remove hemorrhoids, but it also has the highest rate of complications. These may include temporary difficulty emptying your bladder and urinary tract infections related to this problem. Most people experience some pain after the procedure. Medications can relieve your pain. Soaking in a warm bath also may help. ªª Hemorrhoid stapling. This procedure, called stapled hemorrhoidectomy or stapled hemorrhoidopexy, blocks blood flow to hemorrhoidal tissue. Stapling generally involves less pain than hemorrhoidectomy and allows an earlier return to work. Compared with hemorrhoidectomy, however, stapling has been associated with a greater risk of recurrence and rectal prolapse, in which part of the rectum protrudes from the anus. Talk with your doctor about what might be the best option for you. ¾¾ Lifestyle and home remedies You can often relieve the mild pain, swelling and inflammation of hemorrhoids with home treatments. Often these are the only treatments needed. ªª Use topical treatments. Apply an over-thecounter hemorrhoid cream or suppository containing hydrocortisone, or use pads containing witch hazel or a numbing agent. ªª Soak regularly in a warm bath or sitz bath.

During bowel movements, have you noticed blood on your toilet paper, dripping into the toilet or mixed into your stools? ¾¾ What you can do in the meantime In the time before your appointment, take steps to soften your stools. Eat more high-fiber foods, such as fruits, vegetables and whole grains, and consider trying an over-the-counter fiber supplement, such as Metamucil and Citrucel. Drinking six to eight glasses of water a day also may help soften your stools and relieve your symptoms. ¾¾ Tests and diagnosis Your doctor may be able to see if you have external hemorrhoids simply by looking. Tests and procedures to diagnose internal hemorrhoids may include: ªª Examination of your anal canal and rectum for abnormalities. During a digital rectal exam (DRE), your doctor inserts a gloved, lubricated finger into your rectum. He or she feels for anything unusual, such as growths. The exam can give your doctor an indication of what further testing might be appropriate. ªª Visual inspection of your anal canal and rectum. Because internal hemorrhoids are often too soft to be felt in a rectal examination, your doctor may also examine the lower portion of your colon and rectum with an anoscope, proctoscope or sigmoidoscope. These are scopes that allow your doctor to see into your anus and rectum. Your doctor may want to do a more extensive examination of your entire colon using colonoscopy. This might be recommended if: ªª Your signs and symptoms suggest you might have another digestive system disease ªª You have risk factors for colorectal cancer ªª You’re over age 50 (age 45 if you’re black) and haven’t had colonoscopy within 10 years ¾¾ Treatments and drugs To remove a hemorrhoid using rubber band ligation, your surgeon inserts a small tool called a ligator through a lighted tube (scope) in the anal canal and grasps the hemorrhoid with forceps. Sliding the ligator’s cylinder upward releases rubber bands around the base of the hemorrhoid. Rubber bands cut off the hemorrhoid’s blood supply, causing it to wither and drop off. Most of the time, treatment for hemorrhoids involves steps that you can take on your own, such as lifestyle modifications. But sometimes medications or surgical procedures are necessary. ¾¾ Medications if your hemorrhoids produce only mild discomfort, your doctor may suggest over-thecounter creams, ointments, suppositories or pads. These products contain ingredients, such as witch hazel or hydrocortisone that can relieve pain and itching, at least temporarily. Don’t use an over-the-counter cream or other product for more than a week unless directed by your doctor. These products can cause side effects, such as skin rash, inflammation and skin thinning. ¾¾ Minimally invasive procedures If a blood clot has formed within an external hemorrhoid, your doctor can remove the clot with a simple incision, which may provide prompt relief.

Soak your anal area in plain warm water 10 to 15 minutes two to three times a day. A sitz bath fits over the toilet. You can get one at most drugstores. ªª Keep the anal area clean. Bathe (preferably) or shower daily to cleanse the skin around your anus gently with warm water. Soap isn’t necessary and may aggravate the problem. Gently dry the area with a hair dryer after bathing. ªª Don’t use dry toilet paper. To help keep the anal area clean after a bowel movement, use moist towelettes or wet toilet paper that doesn’t contain perfume or alcohol. ªª Apply cold. Apply ice packs or cold compresses on your anus to relieve swelling. ªª Take oral pain relievers. You can use acetaminophen (Tylenol, others), aspirin or ibuprofen (Advil, Motrin, others) temporarily to help relieve your discomfort. With these treatments, hemorrhoid symptoms often go away in a few days.See your doctor if you don’t get relief in a few days, or sooner if you have severe pain or bleeding. ¾¾ Prevention The best way to prevent hemorrhoids is to keep your stools soft, so they pass easily. To prevent hemorrhoids and reduce symptoms of hemorrhoids, follow these tips: ªª Eat high-fiber foods. Eat more fruits, vegetables and whole grains. Doing so softens the stool and increases its bulk, which will help you avoid the straining that can cause hemorrhoids or worsen symptoms from existing hemorrhoids. Add fiber to your diet slowly to avoid problems with gas. ªª Drink plenty of fluids. Drink six to eight glasses of water and other liquids (not alcohol) each day to help keep stools soft. ªª Consider fiber supplements. Most people don’t get enough of the recommended amount of fiber — 20 to 35 grams a day — in their diet. Studies have shown that over-the-counter fiber supplements, such as Metamucil and Citrucel, improve overall symptoms and bleeding from hemorrhoids. These products help keep stools soft and regular. If you use fiber supplements, be sure to drink at least eight glasses of water or other fluids every day. Otherwise, the supplements can cause constipation or make constipation worse. ªª Don’t strain. Straining and holding your breath when trying to pass a stool creates greater pressure in the veins in the lower rectum. ªª Go as soon as you feel the urge. If you wait to pass a bowel movement and the urge goes away, your stool could become dry and be harder to pass. ªª Exercise. Stay active to help prevent constipation and to reduce pressure on veins, which can occur with long periods of standing or sitting. Exercise can also help you lose excess weight that may be contributing to your hemorrhoids. ªª Avoid long periods of standing or sitting. Sitting too long, particularly on the toilet, can increase the pressure on the veins in the anus.

Am I pregnant? Many women don’t suspect they’re pregnant until they’ve missed a period, but there may be other indications ªª Sickness and/or feelings of nausea ªª Breast tenderness, tingling and enlargement ªª The small bumps - ‘Montgomery’s tubercules’ - on your nipples becoming more obvious ªª Stomach pains ªª Tiredness ªª Needing to urinate more often ªª Going off certain foods, such as coffee and fatty foods ªª Increased vaginal discharge ¾¾ When should I do a test? Home pregnancy tests can be done from the fifth day of your missed period, and some very sensitive tests can be used sooner than this. The tests work by detecting the hormone human chorionic gonadotrophin (hCG) in your urine. A chemical in the plastic stick changes colour or a card shows 2 dark lines when it comes into contact with this hormone It’s possible to have a false negative, when the test says you aren’t pregnant but you are. If your period still doesn’t arrive, repeat the test or check with your obstetrician ¾¾ CONFIRM THAT THE PATIENT IS PREGNANT BEFORE BEGINNING ANTENATAL CARE Antenatal care monitors your health during pregnancy, as well as the health and development of your baby. It can help predict possible problems with your pregnancy or the birth, so action can be taken to avoid or treat them. Your first antenatal check-up This is likely to be the longest one. It will take place between the eighth and 12th week of pregnancy, and you’ll be examined by a doctor ¾¾ WHAT INFORMATION IS REQUIRED AT THE FIRST VISIT? ªªThe first day of the last normal menstrual period must be determined as accurately as possible. ªªAny medical or pregnancy problems which the patient has had since the start of this pregnancy, for example:  fever (such as influenza) with or without skin rashes.  Symptoms of a urinary tract infection.  Any vaginal bleeding.  Attention must be given to minor symptoms which u may experience during your present pregnancy, for example:  Nausea and vomiting.  Heartburn.  Constipation.  Swelling of the ankles and hands. ªªIs the pregnancy planned and wanted, and was there a period of infertility before u became

pregnant? If you are already in the third trimester of her pregnancy9the last 3 mths), attention must be given to the condition of the fetus. ¾¾ When am I due? The length of your pregnancy is dated from the first day of your last period, even though you actually conceive two weeks or so after this date (depending on the length of your cycle). Your baby is likely to be born two weeks either side of the due date. ¾¾ GIVE A PROPER MEDICAL HISTORY Some medical conditions may become worse during pregnancy, e.g. a women with heart valve disease may go into cardiac failure while a hypertensive woman is at high risk of developing pregnancy induced hypertension. A. Tell the doctor if u have any of the following: 1. H ypertension. 2. Diabetes mellitus. 3. Rheumatic or other heart disease. 4. Epilepsy. 5. Asthma. 6. Tuberculosis. 7. Psychiatric illness. 8. Any other major illness. B. Mention Any Medication Taken. 1. C ertain drugs can be teratogenic (damage the fetus) during the first trimester of pregnancy, e.g. retinoids which are used for acne. 2 . Some drugs can be dangerous to the fetus if they are taken close to term, e.g. Warfarin C. Give previous operations details 1. O perations on the urogenital tract, e.g. caesarean section, myomectomy, a cone biopsy of the cervix, operations for stress incontinence and vesicovaginal fistula repair. 2. Cardiac surgery, e.g. heart valve replacement D. Explain The Family History Close family members with a condition such as diabetes, multiple pregnancy, a bleeding tendencies

trumpet) or a Doppler, which uses ultrasound Questions about your baby’s movements Urine tests Checking for any swelling in your legs, arms or face ªª Questions about how you feel ¾¾ When you need to take your injections? Two tetanus toxoid injections are recommended at intervals of 4-6 wks starting of from the 14th week ¾¾ GRADING THE RISK On completion on their second antenatal visit, all patients must be graded into a risk category . A few high risk patients would have already been identified at the first antenatal visit. There may be Low (average) risk and Intermediate risk patients also.A low risk women has no risk factors for the mother or the baby present. An intermediate risk lady has a problem which requires some, but not continuous, additional care. For example, a grande multipar ( a woman who is pregnant for the 5th time or more) should be assessed at her first or second visit for medical disorders, at 34 weeks for an abnormal lie, and again requires additional care during labour and postpartum. She, therefore, is at an increased risk of problems only during part of her pregnancy, labour and puerperium. A high risk woman has a problem which requires continuous additional care. For example, a lady with heart valve disease or one with a multiple pregnancy. These women usually require more care. If a complication develops, the risk grading will change. This change must be clearly recorded on the woman’s antenatal card. Subsequent visits will now be more frequent, depending on the nature of the risk factor. ¾¾ THE VISIT AT 28 WEEKS ªªWHAT IMPORTANT COMPLICATIONS OF PREGNANCY SHOULD BE LOOKED FOR? 1. Antepartum haemorrhage (bleeding in pregnancy) becomes a very important high risk factor from 28 weeks. 2. Early signs of pre-eclampsia (hypertension in pregnancy) may now be present for the first time, as it is a problem which develops in the second half of pregnancy. Therefore, the woman must be assessed for protein in urine and a rise in the blood pressure. 3. Cervical changes in a woman who is at high risk for preterm labour(a woman who goes into labour before she starts her 9th mth), e.g. multiple pregnancy, a history of previous preterm labour. 4. If the symphysis-fundal height measurement is below the 10th centile, assess the lady for causes of poor fundal growth. 5. If the symphysis-fundal height measurement is above the 90th centile, assess her for the causes of a uterus larger than dates.

or mental retardation increases the risk of these conditions in the patient and her unborn infant ¾¾ Blood tests Normally, a small sample of your blood is taken at your first antenatal appointment. You may also be asked to give a sample in later pregnancy. The first test can: ªª Identify your blood group ªª See whether your blood is rhesus positive or negative ªª Check for conditions that could affect your health or your baby’s (this may include HIV) ªª Check for anaemia The blood test shouldn’t be painful and should only take a minute or so Blood tests can also be used to estimate the risk of Down’s syndrome. A blood sample is taken at about 16 weeks to measure three substances: alpha-fetoprotein (AFP), unconjugated oestriol and human chorionic gonadotrophin. Together with the mother’s age, these give an estimate of risk. The level of AFP can also be used to assess the risk of a neural tube defect, such as spina bifida. ¾¾ When you’ll have a scan This differs according to your own needs. You might be offered a scan at any of these following times: ªª About six to eight weeks to confirm/date the pregnancy, see if it’s ectopic (developing in the fallopian tubes, not the uterus) and check the foetus is alive by looking for a heartbeat ªª About ten to 14 weeks to confirm/date the pregnancy, see if you’re expecting twins, or more, and when offered alongside a nuchal scan (which looks at a pad of skin at the back on your baby’s neck) assess the risk of Down’s syndrome or other chromosomal conditions ªª About 20 to 23 weeks to check for spina bifida and other possible abnormalities, look in detail at your baby’s major organs and skeleton, check the health of your placenta and monitor your baby’s growth Later scans monitor your baby’s growth and check the position of the placenta and your baby How often do you need to visit your doctor? Not counting appointments for scans or other hospital-based tests, you can expect to have appointments every four weeks after week 12 upto 28 weeks, every two weeks till week 36, and every week during the last three or four weeks. Routine checks at appointments are likely to include: ªª Blood pressure ªª Weight ªª Palpation - feeling your tummy ªª Listening to your baby’s heart - this can be done with a stethoscope, a Pinard (looks like an ear

3. Women who have had a previous caesarean section must be assessed with a view to the safest method of delivery. 4. The woman’s breasts must be examined again for flat or inverted nipples, or eczema of the areolae which may impair breast feeding. These abnormalities should be treated. ¾¾THE VISIT AT 42 WEEKS ªª WHY IS THE VISIT AT 42 WEEKS IMPORTANT? A patient, whose pregnancy extends beyond 42 weeks, has an increased risk of developing the following complications: 1. Intrapartum fetal distress. 2. Meconium aspiration. FOLLOWING SYMPTOMS OR SIGNS, INDICATES THE PRESENCE OF SERIOUS COMPLICATIONS, AND MUST BE DISCUSSED WITH THE DOCTOR. 1. Symptoms and signs that suggest abruption placenta: a. Vaginal bleeding. b. Persistent, severe abdominal pain. c. (iii) Decreased fetal movements. 1. Symptoms and signs that suggest pre-eclampsia: a. Persistent headache. b. Flashes before the eyes. c. Sudden swelling of the hands, feet or face. 1. Symptoms and signs that suggest preterm labour: a. Rupture of the membranes. b. Regular uterine contractions before the expected date of delivery ¾¾ What’s rhesus negative blood? If the mother is rhesus negative and the father is rhesus positive, it could lead to problems Most people have rhesus positive blood; the remainder have rhesus negative. If both partners have the same rhesus factor, there’s no problem. If the mother is rhesus positive and the father is rhesus negative, there are no concerns either. But if the mother is rhesus negative and the father is rhesus positive, it could lead to problems if the baby is also rhesus positive. During a first pregnancy this may have no obvious effect, as the two bloodstreams don’t usually meet until birth or if there’s a miscarriage. However, at either of these times some of the baby’s cells can get into the mother’s bloodstream, sensitising her to the rhesus factor. In a subsequent pregnancy, antibodies are produced that may cause a potentially serious blood disorder in the baby Treatment consists of injections of an immunoglobulin after the first birth (or miscarriage) to help the mother get rid of the positive blood cells before she starts producing antibodies in her next pregnancy.

Anaemia in pregnancy may be detected for the first time. Diabetes in pregnancy may present now with glycosuria. If so, a random blood glucose concentration must be measured.

¾¾

OW SHOULD YOU MONITOR THE FETAL H CONDITION? 1. All women should be asked about the frequency of fetal movements and warned that they must report immediately if the movements suddenly decrease or stop. 2. If a patient has possible intra-uterine growth retardation(baby is smaller than the required size for that period of pregnancy) or a history of a previous fetal death, then she should count fetal movements once a day from 30 weeks and record them on a fetal movement chart. . ¾¾ THE VISIT AT 34 WEEKS ªª WHY IS THE 34 WEEKS VISIT IMPORTANT? 1. All the risk factors of importance at 28 weeks (except for preterm labour) are still important and must be excluded. 2. The lie of the fetus (whether the baby’s head is down or not) is now very important and must be determined. If the presenting part is not the head, then an external cephalic versio (turning the baby around) must be attempted at 36 weeks if there are no contra-indications. A grande multipara who goes into labour with an abnormal lie is at high risk of rupturing her uterus.

Young cancer patients of reproductive age now pose a new problem after successful treatment with sophisticated chemo and radio therapy.

Fertility preservation in young cancer survivors T

he prevalence of cancer cases in India is estimated to be around 2.5 million, with diagnosis of over 8,00,000 new cases and 5,50,000 deaths occurring each year due to this disease. The impact of cancer in younger population causes immense emotional trauma to the suffers and their parents 70% of the cases report for diagnostic and treatment services in advanced stages of the disease with poor survival and high mortality rate. Of late with the knowledge of cancer disease as a whole and advent of facilities for early diagnosis and complete treatment, cancer survivors have an extended lease of life. But young cancer patients of reproductive age now pose a new problem after successful treatment with sophisticated chemo and radio therapy. They look forward to basic rights of a normal human being like issues of marriage and raising a family . Many of the drugs in treatment of cancer have adverse lasting effects on gonadal function (function of testis and ovary) and majority of

Some of the cancers like leukaemia, lymphoma and bone cancers occur in young adults, pre pubertal age and even in childhood. Breast cancers can occur in women of reproductive age at anytime between 18 - 40 years. Early detection is most important not only to expect a complete cure of cancer but also to save the essential requirement of quality life like for eg. conservation of fertility potentia. Patients and their guardians should be aware of what topics they should discuss with their doctors before subjecting their wards for treatment. Surgery, radiation, and chemotherapy can all impair reproductive function. Cancer involving genital organs in lower abdomen often requires surgical procedures resulting in removal or irreversible damage to gonads ( ovaries and testes). Radiation causes partial or total premature ovarian failure. In women damage to uterus can be caused by radiation resulting in shrinkage of Uterine volume with increased risks of miscarriage, preterm labor and

length of time. After the cancer treatment is over and the patient is well enough to consider a pregnancy these eggs can be then thawed and fertilized with the husband’s sperm and the resultant embryos can be transferred into the uterus. Strips of ovarian tissues can be removed from the periphery of the ovary by laparoscopy and these can be frozen in liquid nitrogen. The strips can be re - implanted in the pelvis at the same place where the evary is normally situated (Orthotopic graft) or in other places like the muscle tissue, the subcutaneous tissue of the thigh or abdominal wall (Heterotopic graft). Ovarian strip cryopreservation is still considered to be experimental. As on today, only a few babies are born out of this technology. Moreover, the patients and even doctors are not aware of these advances in creation of embryos outside the body with cryopreserved ova and sperm. In women undergoing radiotherapy, whole ovary and tube can be transplanted to a distance away from the site of cancer to which a maximum dose is delivered. With the tube still attached to the uterus in the original place, one can expect even a spontaneous pregnancy to occur after the patient has finished her treatment. In view of this, the parents, the young adolescents, men and women who are likely to be subjected for the treatment should be able to discuss these topics and find out as to how the doctors can help them. After all, once the cancer is cured and the patient finds out that there is afresh lease of life, he or she wants to live like peers. However, the research is going on in feverish speed in all developed countries and India should not be left alone waiting for their results to give us the knowledge. The technology is here but the awareness amongst the patient should spread. Please ask your doctor on fertility preserving options.

intrauterine retardation of growth. Chemotherapy impairs reproductive function depending upon patient’s age, nature of drugs given and their dosage. Increasing age is associated with decreasing fertility after chemotherapy. Many patients get into stoppage of menses which may be permanent or temporary. Return of menses does not always mean that germ cells (ova and spermatozoa) have arrived for creation of a baby. ¾¾ In Married Women / Women with Life - Partners: When a particular treatment is likely to destroy the gonads, embryos can be made by In-Vitro Fertilization (creation of embryo in laboratory with the couple’s eggs and spermatozoa) before starting treatment for cancer. The resulting embryos are frozen in liquid nitrogen for future use. When the patient is cleared of cancer by the clinicians, these embryos can be released, thawed and transferred and one can expect a pregnancy rate in the range of 30 - 40 % per transfer. Embryo cryopreservation is not always feasible in every case because it involves 4-6 weeks of preparation to be able to get a reasonable number of good quality mature eggs from the females. In view of this, the cancer therapy has to be postponed for this length of time and secondly the number of embryos that one expects to create in one cycle may not be more than 8-10 in numbers. This number of embroys cannot be used for more than 2-3 cycles .This delay of 4-6 weeks for production of embryos may not be acceptable as cancer therapy has to be started as soon as possible without wasting time. Another point to be considered is that some of the cancers thrive on gonadal hormones and stimulating secretion of these types of hormones for the purpose of IVF may not be allowed in such cancers like for e.g. those occurring in breast and uterus. ¾¾ In Single Women: There is no partner available for making embryos. Such women can be subjected to collection of immature eggs before starting the treatment for cancer. These immature eggs can be preserved in liquid Nitrogen at a temperature of -187 C for any

Age-related macular degeneration (ARMD)

show the findings of this disease process. To assess the condition in detail, certain other tests are done:

Age-related macular degeneration (ARMD) is the most common cause of vision loss in those over 50s and its prevalence increases with age. It is caused by degeneration of the macula, the central and most sensitive part of the retina at the back of the eye. ARMD affects mainly the macula which is the part of the retina that allows the eye to see fine details and also the center of the field of vision. Degeneration results from a partial breakdown of the retinal pigment epithelium (RPE) which is the insulating layer between the retina and the choroid (the layer of blood vessels behind the retina). The RPE acts as a selective filter to determine what nutrients reach the retina from the choroid. Many components of blood are harmful to the retina and are kept away from the retina by normal RPE. Breakdown of the RPE interferes with the metabolism of the retina, causing thinning of the retina (the “dry” phase of macular degeneration). These harmful elements may also promote new blood vessel formation and fluid leakage (the “wet” phase of macular degeneration). This disorder results in the loss of central vision only peripheral fields are always maintained. The risk factors for ARMD are hypertension, smoking and family history of ARMD. ¾¾ What are the symptoms of ARMD? Distortions in objects which are looked at directly Bulges or curved appearance of a straight door Distorted print lines in book Adark or blank spot in the center of vision Fading of colors especially blue ¾¾ How to detect ARMD? As it is a silent disease, mostly it is picked up in a routine examination by an ophthalmologist. The retinal examination done by an ophthalmoscope will

to destroy the fragile leaking blood vessels. ªª Photodynamic therapy A drug called Verteporfm is injected into the arm and it reaches the blood circulation and gets collected in the abnormal new vessels present in the eye, it tends to stick to the surface of new abnormal blood vessels, which is then exposed to a laser. The drug gets activated and destroys the new blood vessels leading to slower decline in vision. This drug does not destroy the surrounding normal tissue and hence is better then laser treatment which was the mode of treatment in the past. However, treatment results often are temporary and may require re-treatment. Anti VEGF - most recent advancement in managing any form active wet ARMD. There are drugs available which should be taken with the consultation of doctors Low vision aids and lighting These are Special optical devices eg. Magnifiers (hand held, desktops) which along with bright light make reading more comfortable in patients with less severe form of disease. There may not be of great help in severe form of ARMD

What are the treatment options for ARMD? There is no permanent cure for most type of ARMD. In some cases ARMD may be active and then slow down or even stop progressing for many years. The aim is to keep a vigilant check on progression of disease and take measures to improve functional capability of the patient. ªª Nutrition: Eat fresh fruits, dark green leafy vegetables containing antioxidants are found beneficial in patients with already in one eye with severe form of ARMD, ªª Sunlight: Blue rays of the spectrum seem to accelerate macular degeneration. Sunglasses with good UV filters for outdoor activities are recommended. ªª Smoking: Quit smoking, as this accelerates the process of wet ARMD ªª Early detection: Weekly monitoring of vision by Amsler gridreport if any change noticed (the development of wet type may need urgent treatment with laser or injections directly into the eye) ªª Laser treatment: Focal laser has the highest success rate in typical cases which are away from the center of the fovea. In this procedure a high-energy laser beam is used

Does your body have a ability to do large muscle work .If your answer is yes it says that cardiovascular endurance is perfect and your heart is well conditioned A well conditioned heart conserves energy and can supply oxygen rich blood to the rest of the body .. It also decreases the risk of important diseases that shorten your life such as heart disease stroke and high blood pressure... ªª Walking is the best form of exercises that increases your heart rate and breathing for an extended period of life and helps in building up your endurance gradually. Muscular endurance is the ability to resist fatigue and continue to exercise over along period of time. Strength training is needed to maintain muscle strength. Muscular endurance is the ability of muscles to continue working for long hours without rest... ªª Keep using your muscles by keeping active throughout the day. When muscles aren’t used they waste away at any age, Muscles increase your metabolism allowing you to burn more calories during the day when your body is at rest. . Stretching can help your flexibility when your muscles have warmed up .Flexibility is the ability of the joints and muscles to achieve a full range of motion. This prevents injuries and helps keep your body comfortable after any work. Get at least 30 minutes of activity that makes you breathe harder. of energy all day long. As day goes on you may get busier and leave you no time for exercise. This way you have no excuses for skipping. Morning hours have a cooler temperature (benefits especially in summer), Air pollution is lowest in the morning and body adjusts to your exercise time for you have fewer distractions and schedule interruptions in the morning. It smartens you up to boldly face interviews ,important business meeting or an examination for morning exercises gets the blood flowing to your brain circulation and precious nutrients and oxygen rich blood to your various organs . More oxygen to your brain gives you mental boost, and keeps you focused all day long you will look great and feel awesome. Your body will thank you and so will your mirror. Good for your heart and boost for your ego. But always remember any time you can find to exercise is the best time if your morning hours are preoccupied. ¾¾ What should be the intensity of your work outs and length of your sessions? Ayurvedic Experts recommend full dose of exercises in spring and cold seasons but advises exercising to half of one’s strength in other seasons. Appearance of sweat over the forehead, nose, body joints and armpits and when your mouth becomes dry is an indication of putting a stop to exercises. Over exercising may make you prone to cough, fever, vomiting, vertigo, thirst, wasting, asthma, bleeding disorders. ¾¾ Do not exercise after a meal and if illness strikes. During exercise blood is directed to the working muscles supplying oxygen and other nutrients to these tissues .The stomach then may receive 20% of its usual blood flow. As a result exercising soon after food slows digestion, Food remains in the stomach longer causes discomfort and cramping and this makes a regular workout quiet unpleasant. Avoid exercises soon after a heavy meal. Avoid exercising when you have fever because the body temperature rises during exercise and it could lead to a heat stroke. Our body fights an infection during fever so store energy in the form of rest to recover faster. Do not exercise when you have a cough .Exercising when you have cough weakens your lung capacity , makes breathing difficult and your cough more resistant. Stick to the recommended Pranayama ---breathing exercises recommended by your physician. During episodes of diarrhea and vomiting your body looses water content .Avoid exercising till your body is hydrated well. When you have a chronic illness or a serious one be sure to get your physicians permission before you continue any type of exercises. Regular exercise makes your body light , improves your level of performance in all the work , reduces obesity , maintains a normal physique pacifies aggravated bioenergies ,promotes metabolism ,relieves tightness and tensions in your muscles leading to muscular relaxation and total body relaxation.

P

remenstrual syndrome (PMS) is a combination of emotional, physical, psychological, and mood disturbances that occur after a woman’s ovulation and typically ending with the onset of her menstrual flow. The premenstrual syndrome also refers to a variety of symptoms which recur in the same phase of the menstrual cycle. These generally make their appearance two to seven days before the onset of menstruation and are relieved once the menses start. Approximately, 40 per cent of menstruating women suffer from premenstrual tension and it occurs mostly in women over 30 years of age. In some women, the onset of symptoms seems to coincide with ovulation and may then persist until menstruation commences. In some rare cases, relief from the premenstrual syndrome may be obtained only with the cessation of the menstrual flow. A more severe form of PMS, known as premenstrual dysphoric disorder (PMDD), also known as late luteal phase dysphoric disorder) occurs in a smaller number of women and leads to significant loss of function because of unusually severe

AYURPHARMA

symptoms. The mood changes surrounding this condition have been described as early as the time of the ancient Greeks. However, it was not until 1931 that this disorder was officially recognized by the medical community. The term “premenstrual syndrome” was coined in 1953. Symptoms: The onset of this syndrome is abrupt , generally with a headache which is often accompanied by vomiting. The most common mood – related symptoms are irritability, depression, crying, oversensitivity, and mood swings with alternating sadness and anger. The most common physical symptoms are fatigue, bloating, breast tenderness (mastalgia), acne, and appetite changes with food cravings. A general feeling of depression and irritability permeate the entire experience. What is worse, these symptoms intensify progressively, making the last day of the PMS the worst. Tension headaches are common during this period, but in some cases, migraines attacks occur with severe pain and vomiting. The patient suffers from breast tenderness, which is sometimes so severe that it is almost unbearable. There may also be abdominal bloating , accompanied in some cases, by oedema of the ankles and hands. Some women resort to dieting to get rid of the abdominal bloating but this only leads to fatigue and depression. Others may experience a craving for sweet foods. Some of the less common symptoms are exacerbation of epilepsy dizziness, backache, hoarse voice, greasy hair , acne and allergic reactions. Patients suffering from premenstrual tension may show a gain of weight of one kg or more in the latter part of the menstrual cycle due to salt and water retention. The retention of fluid is partly due to ovarian steroids, but there is also an increased output of anti diuretic hormone from the posterior pituitary gland. Diagnosis: There is no specific laboratory diagnosis of the premenstrual syndrome. The problem can be diagnosed on the basis of past history showing a clear, recurrent relationship between a stage of the menstrual cycle and the onset of symptoms as well as the coincidence of relief with the start or cessation of menstruation. The patient may maintain a personal diary about

24

SEPTEMBER

her symptoms and feelings during those days. The record should be kept for at least three cycles. Causes: PMS remains an enigma because of the wide – ranging symptoms and the difficulty in making a firm diagnosis. Several theories have been advanced to explain the cause of PMS. None of these theories have been proven, and specific treatment for PMS still largely lacks a solid scientific basis. Most evidence suggests that PMS results from the alterations in or interactions between the levels of sex hormones and brain chemicals known as neurotransmitters. PMS does not appear to be specifically associated with any personality factors or specific personality types. Likewise, a number of studies have shown that psychological stress is not related to the severity of PMS. Premenstrual syndrome involves a variety of physical, mental, and behavioural symptoms tied to a woman’s menstrual cycle. By definition, symptoms occur during the days before a woman’s period starts. They usually go away after the first day or two of flow. PMS is a complex health concern. Up to 80% of women experience some symptoms of PMS. Scientists cannot agree on its cause or the best way to treat PMS. PMS usually occurs in women in their fourth and fifth decades of life (aged 30 – 49 years). For a small number of women, it can be severely incapacitating. A woman who has had a hysterectomy (removal of the uterus) may still experience PMS if at least one ovary remains. Because many different processes may contribute to PMS, methods of treatment vary widely and can include medical and alternative approaches. Surgery is a last resort. As many as 3 – 8% of women may have a more severe condition called Premenstrual dysphoric disorder (PMDD): PMS and PMDD are not the same. Women with PMDD become seriously depressed for a week or more before their periods. PMS is shorter, usually milder, and involves more physical symptoms. The two may occur together, or a woman may have one and not the other. Treatment: Treatment depends on the severity of the symptoms. Where only mild symptoms are experienced, the problem can be elevated by a change of routine. Extra work and stressful situation should be avoided. Fluids should be moderately restricted and care should be taken not to add extra salt to the food. The patient’s partner and family members should be educated about all the facts of the PMS. The patient should not take any oral contraceptives as these may cause fluid retention and lowering the plasma levels. Hormonal imbalance and infections of the uterus can be helped by a natural diet regimen. As most women feel tension arising from chronic constipation it is essential to treat this condition

SEPTEMBER

first. In constipation, the putrefying faecal matter may be reabsorbed into the blood stead, and the same blood, if supplied to the brain, will cause gradual enervation. Constipation can be relieved by a lukewarm water enema and liberal intake of seasonal fruits and vegetables and simple fibrous meals. Other treatment for the PMS include regular cold hip baths for 10 to 15 minutes twice a day. This will congestion and inflammation of the uterus and connected organs. Tension will also be dissipated with this treatment. Hot foot baths followed by a cold compress to the lower abdomen and the inner surfaces of the thighs also help to relieve uterine congestion and tension. If the cold hip bath is not practicable, a wet girdle pack applied twice a day on empty stomach is very beneficial for clearing up uterine congestion and improving bowel function. All these statements should be suspended during the menstrual flow. Diet pays a significant role in preventing premenstrual syndrome. The patient should avoid refined carbohydrates, sugars, coffee, tea, tobacco, other stimulants, oily, fried or spicy food and all meats. A regular practice of yogasanas, especially those recommended for strengthening the genitourinary system will be very useful in overcoming premenstrual syndrome. These asanas are bhujangasana, shalabhasana, vajrasana, paschimotanasana, ardhmatsyendrasana and trikonasana. Other helpful measures are brisk walks and abdominal exercises which are good for strengthening the abdominal muscles and pelvic organs. Great relief can also be obtained by manipulating the tender points gently, on the big as well as other toes of the feet. Manipulation on the middle portion of the leg foot which relates the uterus and vagina will help to correct the disorder of the uterus. Mental poise is an important factor. Negative mental attitudes like fear, worry, anger, jealousy , tension and inferiority complex should be eliminated by positive thinking, meditation and good company.

(Ig E variety) ªª To detect: To which substance he is Allergic ªª Planned symptom based and clinical information based approach to make the testing more rational and cost effective. ªª Expressed in units which help to interpret the importance of the lab value. Allergy affects more than 4% of population severely. If milder episodes are considered it accounts for more than 15% of population at any point of time. The clinical manifestation of allergy may vary from a simple skin rash to most disturbing anaphylactic shock. The manifestation of severe nature needs to be treated at the earliest since the quality of life is significantly impaired by allergy, though in majority it is not life threatening. Often allergy treatment is frustrating to both the physician as well as the patient since attempts to find out the responsible antigen and pattern of allergy is not possible. Not all clinicians can perform the skin prick test and frequently the patient is also unwilling to go through this painful procedure. In- vitro IgE specific assay is a useful alternative to this test. We at CDL in association with Phadia have launched In-vitro allergen test for detection of the allergen and taken support of specialists in field of immune therapy for allergy to combat this problem. CDL in association with CRICR, with it’s vast knowledge base of previous epidemiological work and clinical experience have launched customized approach to allergy at a much affordable cost. ªª The most common clinical problems of allergy are allergic rhinitis, asthma, skin rash and eczema.

“We have nothing to fear but fear itself - Winston Churchill. What is fear? Fear is a feeling of alarm. It is an emotional feeling resulting from the anticipation of danger, pain or disaster. So it’s clear that the expectation or anticipation of the danger is the culprit. The dangerous thing is that when you develop one fear, two or more other fears simultaneously develop. If nothing is done at this stage of early development, ultimately it will paralyse you. The greatest mistake which people usually make is the fear of making mistakes. In fact everybody in the world is afraid sometime somewhere. But a successful person is one who performs well in spite of his fear. “Fear anger and envy are the triple impediments that will never allow a man to rise to eminence” - Sri Ramakrishna Paramhamsa. Never fear to interact with anybody but never interact out of fear. Never allow any fear to come. When you think it is quite impossible now, it will soon very much be possible with your dedication, determination and will power. Fear robs a major amount of pleasure from our life. For a person it is a tremendous loss. The fear of socializing is the most common fear while other elements like rejection and failure follow. It is the culprit which spoils a person’s peace of mind and comfortable living. The more we brood over fear, it will get strengthened to disturb us more and more, making our life hell. It squeezes out the vital energy necessary for normal living. Learn to see slight change or little gain in everything and every work so that you will not be affected by

fear. Don’t be afraid if you are not very perfect at your first attempt As per Albert Ellis “It is more important to do than to do well’. You do not have to wait until your performance is perfect before you engage in an activity. If a person has been afraid for a long time, it is difficult to overcome the problem in a short span of time. Then you must develop patience and welcome whatever progress you are making. If you take risks frequently, the chances of being successful are definitely bright. But if you never try, then you won’t be successful in your life. Swami Vivekananda said “Both in this world and in the world of religion, it’s true that fear is the

sure cause of degradation. And what causes fear? It is ignorance of our own nature”. The reason for most of the failures is constantly dwelling upon the possibility of failures. If you allow more fear sometimes you may not be able to find out the actual cause of its development. According to the opinion of many psychologists, set backs and failures are the result of our own unconscious wishes. In spite of us trying very hard to accomplish a certain thing, we may not succeed since deep within us is the feeling that we do not deserve and we don’t have those qualities to attain the target and thus lead us to failure. General George Patton said, “Fear Kills more people than death. Death kills us only once and we don’t feel it. But fear kills us over and over again”. Repeatedly tell yourself, “I can do it! I can do it too! Till the time your fear vanishes. The secret of success is to get the things done which you fear most. We can’t eliminate danger, but we can eliminate fear. Now if you are not able to do the work boldly, better do it now otherwise fear will catch hold of you. “Fear always springs from ignorance”- Ralph Waldo Emerson. Never be afraid to take calculated risks. Be innovative, spot the opportunities and capitalize it. Your mind is filled with doubts and fear. Your prior programming to achieve success is to remove all the garbage from the mental warehouse of your mind. The opposite of fear is courage. Courage is not the absence of fear, but opposed to fear and is the mastery of fear. As Cherie Carter Scott rightly said, “Courage is finding the inner strength and bravery required when confronting danger, difficulty or opposition. It is the energy current behind all great actions and the spark that ignites the initial baby steps of growth. It resides deep within each of us, ready to be accessed those moments when you need to forge ahead to break through seemingly insurmountable barriers”.

Overcome Worry Never worry about any past incident. What is past is always past. Charge up and experience the present because so many problems are always there in the present and in the future. So why look back? Worry is simply a misuse of the imagination. Worrying about the present destroys the future, worrying about the future destroys the present, worrying about the past destroys both the present and the future. It can’t change the past, but it has strong power to destroy the present as well as the future. As William Shakespeare said, “The worst is not so long as we can say - this is the worst”. It’s unwise to worry about an incomplete task. Better try hard to get it done and stop worrying. Worry is like a rocking horse. It keeps you moving to and fro. Beryl Pfizer rightly said, “If I spent as much time doing the things I worry about getting done as 1 do worrying about doing them, I would not have anything to worry about”. Never worry about changing things beyond your control. Don’t ever get worried about what other people might think, other people may not think about you at all. According to Robert G. Le Tourneau “Worry and trust cannot live in the same house. When worry is allowed to come in through one door, trust walks out through the back door and worry stays until trust is invited again, whereupon worry walks out”. When you are intelligent and know very well how to tackle different situations, what are you worrying for? Take life as bathing on a seabeach. You know beforehand waves are there. Sometimes you may go up, other time you may go down and the waves will pass smoothly. “Worry does not empty tomorrow of its sorrow, it empties today of its strength” - Corrie Ten Boom. Don’t ever get worried about what other people

might think, other people may not think about you at all. It is the interest paid on trouble before it becomes due. It is nothing but waste of human energy. It merely underscores your ability and capability to face trouble. Take and accept the fact that problems are normal to a person, more to those who are very ambitious and want to do so many things in life. Face your problems courageously boldly and objectively but never worry for anything. According to Dale Carnegie, “Instead of worrying about what people say of you, why not spend your time trying to accomplish something they will admire”. Now you are accustomed to using your imagination for only one thing: worrying. All your energy for visualization is channeled into colourful pictures of what could go wrong. So stop worrying from now and live a peaceful life. Overcome Tension Try to keep up an easy relaxed manner with reduced tension. When you are feeling low, go to some religious place and have a good cry to release you tension. When you clearly know which problem is currently giving your trouble, you will immediately know the source of your tension. Swami Vivekananda said, “Once we are conscious of our innate divine nature and can manifest it, then power, strength, purity and all that is excellent comes to us. But if we are not conscious of the great power, which at the back of our limited physical frame, we shall naturally continue to be in a sort of neutral mental gear. The calmer we are, the less disturbed our nerves, the more shall we learn, the better will our world be”. Expert’s opinion: 90% of our tension is created by 10% of our carelessness and laziness. If we reduce the tension, we lessen the fear and if the fear is lessened, our muscles become relaxed. As per Denning and Philips, “Tension is the natural prelude to purposeful action but when that action is delayed or inhibited, tension builds up excessively and may be only partially released. Such accumulated tension locks up energies that are needed for purposeful mental tension”. Do the following to avoid mental tension –

AYURPHARMA

Be determined to love the work given to you. zz Don’t think that the entire responsibility of the world is on your shoulders. zz Don’t try to do everything at the same time, do it on a piece meal basis. zz Create a positive mental attitude zz Increase your efficiency in whatever you do zz Plan your work in advance and follow the plan strictly. zz Practice to be relaxed all the time. ¾¾ Overcome Anxiety Anxiety is the fear of anticipation of danger which does not really exist. When a person experiences recurrent physical and psychological manifestations of anxiety without any provocative stimulus, they are likely to have an anxiety disorder. Stafford Clark defines an anxiety disorder as “A state of continual irrational anxiety and apprehension, sometimes flaring up into acute fear amounting to panic, accompanied by symptoms of autonomic disturbance; with secondary effects on such other mental functions as concentration, attention, memory and judgment”. The physical repercussions of anxiety are palpitations, sweating, dryness of the mouth, headache, dizziness and tightness in the chest. ¾¾ Anxiety can be controlled by the following ways ªª It is a fact that the more you hide and avoid, you increase your inner anxiety and complicate the whole problem which otherwise is not so complicated. Sometimes it has no cause. People are unnecessarily anxious about something over which they have no control. Avoid this situation. ªª Practice to control anxiety - producing situations until you know that you are the master and can skip it altogether. If anxiety comes, try to avoid excessive resting and being unoccupied. The unseen fear of danger makes you more anxious. Try to occupy yourself with some constructive work. ªª Never put into your mind the thoughts which heighten your anxiety. When you feel relaxed you are physically much better and mentally more stable. In this sound state of mind, you can handle anxiety better. If you think that the ultimate worst may be anything and be mentally prepared to accept it, anxiety can be reduced. ªª According to Dr. Burns “Anxiety results from repressed negative feelings. When you confront the problems the anxiety will gradually diminish or disappear. This can often include becoming more assertive. Assertive people communicate effectively what they think and feel. They can say ‘No’. By allowing their needs and problems to be known, they confront conflicts, not in an aggressive way but in a firm and polite manner. The rights of others as well as their own are respected. They take responsibility for their own wants”. ªª Living in the present - Don’t impose too many conditions to become happy. Enjoy your life by considering each day as a gift, starting from the time you wake until you go to bed at night. Forget the past, it is over. Forget the future, it is yet to happen.

Stay in the present and enjoy every moment of what you are doing. ªª Add more laughter in your life - It helps you to free yourself from negative emotions. You can challenge your anxiety situations with humour. Listen and crack jokes and watch humorous TV programmes and films, ªª Watch your diet - Avoid sweets, dairy products, alcohol and caffeine as far as possible. Eat a balanced nutritional diet and space your meals at intervals, without skipping any meal. Food enriched with vitamin B (complex), vitamin C & E, magnesium, potassium, calcium, zinc and phosphorus are said to be relieving you of anxiety. Reduce your sodium intake. ¾¾ Overcome Panic Panic is a normal response to fear and feelings of panic are harmless initially. Elaine Sheehan defines “A panic attack involves the inappropriate experience of a heightened state of arousal which the body would normally produce in preparation for confronting or fleeing from real danger. Panic attacks can occur in response to particular objects, activities or situations in phobic disorders”. Researchers have found that panic attacks are among the most frightening symptoms of stress and commonly occurs in one out of ten adults. ¾¾ How to cope up with a panic attack ªª Stay in the present - Try to understand your feelings what is happening inside. Slow down a bit and tell yourself “Be cool! Be calm! Be relaxed ! Relax all your muscles deliberately to make yourself a panic-free person. ªª Deep breathing exercise - Inhale through the nose slowly and smoothly and slowly exhale. Repeat the process a number of times. While doing this exercise sit straight, look straight and be in a relaxed posture. Feel the air expanding your lungs and deflating it. ªª Challenge your negative self-talk - Monitor your thoughts, don’t allow negative thoughts to nurture and grow and disturb you emotionally and mentally. When a person is emotionally weak, those negative thoughts get a free - flow entry to attack. ªª Analyze your panic attack - Look out for the specific factors for which you are having panic attack. Try to eliminate the situation or be alert enough not to be overwhelmed by the situation. ªª Avoid irrational thinking - Change your irrational thinking pattern to practical thinking. Do not be bowed down and become panic stricken with thoughts which may have no connection with the situation. ªª Taking care of yourself - As per Dr. Denise F. Beckfield, Ph.D “Remind yourself that you are doing something ultimately healthy and positive for your life, a means of turning the tide and transforming a life filled with some very difficult times into one filled with more positive ones. You should compliment yourself for your courage so far and for taking the important steps of tackling all the dimensions of your panic attack. ªª Avoiding tendency would be harmful - Don’t avoid, show courage to stay in the situation and

SEPTEMBER

wait patiently for the panic to fade away. Distract yourself - When you experience that panicky thoughts are starting to attack, distract your mind to anything entertaining or deliberately do some other activity so that the panic disorder won’t get a chance to surface in you. ¾¾ Overcome Shyness Shyness is basically a personal problem. Now the society is moving very fast and modern, some of the people cannot cope up with the speed and trend of modern society and shyness results. Now it has become a very big social problem. Peter Desberg defines shyness as “Like all forms of stage fright, shyness is brought about by predictions of unpleasant events or outcome of the fear of negative evaluation”. Arnold Buss points out that the fear-provoking, negative thoughts of many shy people center around low self-esteem. Phillip Zimbardo also reported that his research showed a very strong relationship between shyness and situations producing low self-esteem. Some of the causes of shyness are uncomfortable feeling with new people or under new situations, fear of making mistakes, pre-anticipation of the fear of people, poor self esteem and a perceived lack of social skills. The consequences of shyness are loneliness, depression, strained social relations, tendency to hold back, average occupational achievement, lower marital stability, and low confidence level. Since shyness is a personal problem, one can conquer shyness by initiating sincere steps to get over it, some practical approach to combat shyness are:

Change your anxious thoughts and expectations - Negative emotions are the result of negative interpretations and beliefs and interpret a situation as threatening or dangerous. But most of the time they are exaggerated or inaccurate. Try to think more positively and realistically with the thinking that real situation is not as bad as you expect. zz Goal setting - Make a list of five things which you could not accomplish because of shyness or your inhibitive thinking prevents you from doing. Now try to achieve them one by one starting from easy to moderate to little tough (as per your assessment). Goal setting and accomplishment gives you a feeling of happiness and worthiness. zz Areas of Strength - If you continue to focus on small mistakes or little flaws in your personality or appearance, you are sure to experience anxiety feeling. But concentrate on your areas of strength and make it strong and develop a positive feeling about those feel good factors. zz Overcome catastrophic thinking - Resist yourself from concentrating on the immediate consequences of some negative experience. In reality, people will forget very easily about your initial discomfort and assess yourself on overall achievement or performance. zz Self consciousness - The most distinguishing feature of the shy people are that they are very much conscious of people’s reaction towards them -what people think of me? Am I doing anything wrong? Avoid being too much conscious. zz Lack of proper skills - It happens in some matters or some situations as person is not matched with the necessary skills required for that purpose, automatically shyness plays the role of a villain. Cultivate the required skills, it is the proper solution to this problem. zz Failure to express - Publicly shy people cannot communicate their grievance, fear points of disagreement, uncertainties, good qualities, demands and desires to appropriate to the right person at the right time. By keeping one under the shell in these non-returnable self-containers they don’t come out from the shell. Many celebrity orators in the world were shy in the beginning. It shows a great prospect for you also. zz Don’t bring yourself down - Don’t allow yourself to indulge in guilt and shame feelings. Never put yourself down by criticizing and feeling yourself to be a failure and worthless. ¾¾ Overcome Nervousness Sri Sri Paramahansa Yogananda has given an explanation of nervousness. He said, “The disturbance of mental equilibrium, which results in nervous disorders, is caused by continuous state of excitement or excessive stimulation of the senses. Indulgence in constant thoughts of fear anger melancholy remorse envy sorrow hatred discontent or worry and lack of the

AYURPHARMA

necessities for normal and happy living, such as right food, proper exercise fresh air, sunshine, agreeable work and a purpose in life, all are causes of nervous disease”. A nervous person has to face lot of problems such as lack of confidence and courage to face the world, low self esteem and it affects-physically mentally and emotionally. Some of the easy practical methods to overcome nervousness are: ªª Try to find out the causes of your nervousness and remove those causes from your mind. Take a deep breathing from the stomach through the nose and exhale it from your nose slowly. Repeat it for three times. It will release your tension. Think of yourself as a relaxed person at that particular time. When inhaling, think of the fresh energy enthusiasm going inside the body and while exhaling, think that all the mental and psychological debris are going out from the storehouse of mind. ªª Never think anytime that you are nervous. You must not tell or disclose others also that you are nervous. Nervousness lies in the mind. If you keep your mental toughness, nothing can disturb you and your smooth sailing life. ªª Don’t let your emotions rule your life. Take proper charge of yourself and control everything. Ask for reasoning against any irrational and nervous thoughts coming into your life. ªª Associate more with people who are calm and wise to banish nervousness and realize your true potential. Better avoid people who are also suffering from nervousness. Bold and courageous people can lift you up better from the stage of nervous debility. ªª Sometimes act as if you are very bold confident and outspoken person, pretend having that type of personality for some time till your acting coincides with naturality. In this manner, you can be a nervous-free person. ªª You must always try to be relaxed confident and unafraid and must recall that nervousness escalates your problem. ªª Never allow any ‘thought of nervousness’ into your mind while talking to anybody or while doing any job.

Most People Sleep without Resolving Their Tensions, This Is Termed Nidra Nidra Means Sleep, No Matter What or Why, But Yoga Nidra Means Sleep After Throwing Off the Burdens, It Is A Blissful, Higher Quality Altogether

Y

oga Nidra is a deep relaxation technique through which relaxing the body and mind reduces the effects of stress. It is a simple yet profound practice that induces deep relaxation into the whole body-mind system in a conscious way. Yoga nidra, derived from the tantras, is a powerful technique in which one learns to relax consciously. It’s also called as the state of dynamic sleep. Yoga nidra is a systematic method of inducing complete physical, mental and emotional relaxation. The term yoga nidra is derived from two Sanskrit words, yoga meaning union or one-pointed awareness and nidra means sleep. During the practice of yoga nidra, one appears to be asleep, but the consciousness will be functioning at a deeper level of awareness. For this reason, yoga nidra is often referred to as psychic sleep or deep relaxation with inner awareness. ¾¾ Aim of Yoga Nidra Yoga nidra is also said as the art of relaxation. Within the last century or two, diseases have sprung up with new dimensions, expressions, manifestations and this has reached a peak in the last few decades. Medical science has put an end to the great plagues of the past, but we are now faced with a new epidemic of stress-related disorders caused by our inability to adapt to the highly competitive pace of modern life. Psychosomatic illness such as diabetes, hypertension, obesity, thyroid disorders, migraine, asthma, ulcers,

digestive and skin disorders are said to arise from the tensions of body and mind. The leading causes of death in developed countries like cancer and heart disease also stem from tension. Though Modern medical science has been trying to tackle these problems in many ways, but have failed to deliver the good necessary health to man. This is because the real problem does not lie in the body it originates from the man’s changing ideals, his way of thinking and feeling. The International problem today is not hunger, poverty, drugs or fear of war. It is tension, hypertension and the total tension. One who knows to free from tension and is able to balance the tensions control the emotions and anger will be able to control high blood pressure, heart disease etc diseases. Whether one thinks too much or don’t think at all, he accumulates tension. In the same way, whether one works or doesn’t work, sleep too much or doesn’t sleep at all, whether take a heavy protein diet, carbohydrate diet or a vegetarian diet, he accumulates tension. These tensions amass in the different layers of the human personality. They accumulate in the muscular, emotional and mental systems. In yoga, we deal with the problems of tension with a wide periscope. We realize that if mind is tense, the stomach will also be tense. And if the stomach is tense, the whole circulatory system is also tense. It is a vicious circle of events. Therefore in yoga, relaxation from tension is one of the chief concerns and thus by the practice of yoga nidra, the nature of mind can be changed, diseases can be cured and the creative genius can also be restored. ¾¾Mechanism of Yoganidra The precise mechanism by which yoga nidra acts centrally upon the brain to induce relaxation throughout the nervous system and to booster the resistance levels of the physiological systems has become an important area of research. Various research groups have defined this relaxed state of awareness in different terms including the hypnagogic state, the creative surrender and the relaxation response. Each is referring to the same process of yoga nidra- a state of profound psychophysiological relaxation and metabolic rest in which a fundamental release and readjustment in the controlling mechanisms of the autonomic

nervous system appear to occur. Many of the physiological parameters of this experience have now been identified in scientific experiments. They included altered frequency of the brain’s electrical rhythms, decreased heart rate and systolic and diastolic blood pressures, altered levels of circulating ‘stress hormones’ including adrenaline and cortisol from the adrenal glands and decreased sympathetic nervous activity as reflected in increased galvanic skin resistance (GSR). ¾¾ Therapeutic Applications Yoga nidra has widespread application in the management of diseases of all kinds and its effect both as a palliative and curative has been investigated in many research centres around with extremely favorable results. Yoga nidra can be utilized either alone or in conjunction with other conventional forms of medical therapy. It has been found useful in both acute and chronic conditions especially in degenerative and stress- related conditions such as hypertension, coronary heart disease and arthritis. ªª Psychiatric diseases: Yoga nidra is an effective adjunct to traditional forms of psychiatric treatment especially in cases which are not responding to routine psychotherapy. The practice of yogic relaxation has been found to effectively reduce tension and improve the psychological well-being of sufferers from anxiety. ªª Psychosomatic diseases: Yogic relaxation therapy has been effectively prescribed in conjunction with other medical and yogic procedures in the management of severe psychosomatic diseases including cancer, bronchial asthma, colitis and peptic ulcer. ªª Problems of Children: Yoga nidra relaxation and visualization are also excellent therapeutic techniques for children with behavioral problems and maladaptive social and environmental responses. ªª Drug addiction: Yoga nidra is also being used in psychiatric admission units, enabling patients to reduce dependency on tranquilizing and hypnotic drugs and to increase their self-reliance, thereby reducing their hospital stay. Yoga nidra is also being taught as a suicide preventive to highly disturbed and depressed patients. ªª Insomnia: It is also a successful treatment for insomnia and has produced definite results

Addiction of Tobacco, alcohol: In response to elevated levels of stress in modern life, addiction to tobacco, alcohol and other stimulants and depressants has become a big problem. Many people think that smoking or drinking helps to relax. By reducing the deep seated conflicts and tensions, yoga nidra promotes a general feeling of wellbeing, which helps to curb excessive consumption of coffee, cigarettes and alcohol as well as aiding in the management of drug abuse and addiction. Degenerative diseases: The influence of yoga nidra on chronic degenerative diseases has been investigated in several countries. In one study conducted with patients suffering from major long term disabilities, at the University of California Medical Centre at Davis (USA), researchers concluded that yoga nidra is applicable for bedridden, incapacitated and chronically afflicted patients of all degrees. Pain: The ability of yoga nidra to control migraine pain was investigated in a study at the Presbyterian University College, Hospital, Pittsburgh (USA), in which the need for analgesic medications was eliminated or markedly reduced among the 54 patients participating in the study and after follow up of 6 weeks of yoga nidra therapy, patients reported an average of 81% effective pain relief. Arthritis: Yoga nidra has also been used successfully in the management of chronic arthritic pain Pregnancy and Menstrual problems: Yoga nidra also has an important role in the techniques of natural childbirth, in which deep physical relaxation and spontaneous breath awareness form the basis for drug free labour and delivery. In a study conducted at East Texas University (USA), researchers concluded that yogic relaxation training should be prescribed more frequently as an adjunct or alternative to conventional drug therapy for menstrual pain and disorders. Geriatrics: Yogic relaxation helps elderly people overcome many of the specific psychological difficulties related to old age, according to a recent study conducted in a French Geriatric hospital. Asthma: Highly significant reduction in

frequency and severity of attacks has been reported in asthmatic patients who practice yoga nidra. The yoga nidra state has also been used by a Texas pediatrician to cure severe asthma in adolescents. Colon diseases: A Swedish study has reported remission of symptoms of colonic irritability following 3 weeks of relaxation training, while severe and long standing ulcerative colitis has been cured or effectively controlled by a daily program of specific asanas, including shavasana, together with Ayurvedic treatment and dietary modification in a controlled study in the Department of Ayurvedic medicine, Banaras hindu university of India. Cancer : Yoga nidra is specially recommended in conjunction with meditation therapy to treat cancer. The work of Australian psychiatrist Dr.Ainslie Meares stands as the most convincing research evidence to date. He has demonstrated clear regression of cancer of the rectum following meditation and the remission of metastatic cancers developing from a primary cancer in the lung. Cardiovascular diseases: Yoga nidra seems to operate via the same relaxation response as simple japa yoga, which has been found to relieve the fundamental cause of cardiac strain and heart attack by reducing the load of environmental and intrapsychic stress being relayed from the hypothalamus into the electrical conducting fibres of the heart via the sympathetic nervous system. In an important study at the University of Tel Aviv (Israel), yoga nidra significantly lowered the levels of serum cholesterol in cardiac patients. Hypertension : Yoga nidra therapy adopted either alone or as an adjunct therapy has been found to reduce systolic readings by an average of 15-20mmHg, and diastolic readings by 10mmHg after 3 weeks or more of daily guided practice. Study conducted at the Langley Porter Neuropsychiatric Institute in California found that reductions in blood pressure and anxiety levels in hypertensive patients continued for 12 months after yoga nidra training. Stress: A survey conducted by the Australian Federal Health Department in 1980 revealed that stress is the aspect of modern life that concerned most people. Yoga nidra is now prescribed by doctors in many countries both as a preventive and curative therapy in the first three stages of stress-related disease. In addition, it is also a most effective adjunct in the management of psychosomatic disorders that have progressed to a severe or structurally irreversible stage. Here yoga nidra provides effective symptomatic relief, alleviates depression and long standing anxiety.

orn is a low – fat complex carbohydrate that deserves a regular place on any healthy table. Unfortunately, as with many other naturally low – fat foods, the American tendency is to smother corn – on – the – cob with butter. But these high – fiber, fat – fighting kernels of goodness are better served with seasoning. Because corn is hearty and satisfying, it can curb your appetite. The health benefits of sweet corn are more far – reaching than you might imagine at first. Corn is incredibly versatile, and it’s used in all kinds of dishes. You can find corn at every country worldwide, although corn is only produced in certain countries, such as Peru, Ecuador, Bolivia and South Mexico. Corn has a composition of 80 % carbohydrate , 10 %

protein , 4.5 % fat , 3.5% fiber, and 2% micro – nutrient. It’s a good source of fiber, and regular consumption is great for the colon. We create all kinds of products from corn, such as corn flour, corn oil, cornflake, corn syrup, popcorn, rice corn and corn soup. It is believed that it was first domesticated between 9,000 and 8,000 BC and by 2,000 to 1,500 BC it had become a staple food in the diets of the Olmecs and Mayans, who held it in great reverence, so much so that it became part of their daily rituals and took on religious significance. It also featured in their art. Native Americans also valued corn and used it as both a food and medicine as well as for other purposes, such as weaving the fibres from the plant into sleeping mats, moccasins, baskets and other items. Corn husk dolls were also made after using the edible kernels. Let’s take a look at all these great benefits of corn products : Health benefits: This popular food is high in fiber. In fact, it’s notoriously hard to digest. But its insoluble fiber is tops at tackling common digestive ailments (like constipation and hemorrhoids) by absorbing water, which swells the stool and speeds its movement. Corn is a surprising source of several vitamins, including folic acid,

38

niacin and vitamin C. The folic acid in corn is now known to be a important factor in preventing neural – tube birth defects. It’s just as important in preventing heart disease, according to studies that show folic acid can prevent a build – up of homocysteine, an amino acid, in the body. Long – term elevation of homocysteine has been linked to higher rates of heart disease; folic acid helps break it down.

headaches. Corn is also used as a diuretic to get rid of excess fluids in the body. The corn husks would be burned and parts of the body which had sores, ulcers, or other skin problems would be held over the smoke, to cure them. Corn has wound healing properties due partly to the presence of allantoin which is often used in herbal remedies. Corn contains some of the B - complex vitamins including B1 (thiamine), B2 (niacin), B3 (riboflavin), B5 (pantothenic acid) and B6, making corn good for hair, skin, the digestion, heart and brain. It also contains vitamins C, A and K along with amino acids, flavonoids, and large amounts of beta – carotene and a fair amount of selenium which improves the functions of the thyroid gland and plays a role in the proper functioning of the immune system. Beta – carotene is also found in tomatoes, papaya, pumpkin and red peppers. Corn therefore possesses potent antioxidant properties which help to protect the body from the ravages of free radicals which can damage the cells and cause cancer. Corn also contains fibre which is essential to our diet. It also helps with production of sex and stress – related hormones and is good for our sexual health especially that of men as niacin can help with erectile dysfunctions. It is believed that it can help with the symptoms of arthritis and rheumatism as the B – complex vitamins can improve joint mobility. It seems that sweet corn has some very beneficial properties although some of the traditional uses have yet to be proved, such as its use in the treatment of kidney stones and gravel. In China, it is used as a diuretic and to help in cases of jaundice. It is believed that it can help in the treatment of diabetes mellitus, although this has not been proved without doubt. It does seem to help to reduce blood sugar levels and blood pressure, however. It goes well with tuna fish and can be used in many ways, however it is best to steam or grill corn on the cob to get its full nutritional benefits. Street sellers cook it in hot sand in covered metal trays, making a delicious snack on the hoof. Selection and storage: End – of – summer corn is by far the best year in town. Although you can find a good – tasting corn year – round, many out – of – season years aren’t worth eating. When buying fresh corn, be sure it was delivered in cold – storage, as temperatures rise, the natural sugar in corn turns to starch, and the corn loses some sweetness. Corn is best eaten within a day or two of picking. Corn husks should be green and have visible kernels that are plump and tightly packed on the cob. To test freshness, pop a kernel with your finger nail. The liquid that spurts out should be milky coloured. If not, the corn is either immature or over ripe. Once home, refrigerate corn immediately.

Dandruff ªª ªª ªª

¾¾ ªª

ªª ªª

¾¾ ªª

Dandruff is the single most common problem and the number one enemy of healthy hair. It is the result of excessive flaking of dead skin cells in your scalp. Dandruff results when oil and loose cells cling to each other and stick to the scalp. There are two forms of dandruff— oily and dry. Dry dandruff is most common and appears as loose white flakes, and the scalp itches a great deal. Oily dandruff is sticky and yellow in colour, and the scalp smells bad with oily dandruff. The oily form is found most among adolescents and adults with an excessively oily skin and scalp. Causes of Dandruff Dandruff is generally caused by an excessive growth of microbes (bacteria) on the scalp. This could be the result of a faulty diet, emotional tension and stress, hormonal disturbances, infection due to disease, injury to the scalp, unwise or excessive use of hair cosmetics and dry weather. Androgens (male hormones) which cause baldness in men are also responsible for greasy hair and dandruff. The actual cause of dandruff is a mystery even for doctors, and most people face this problem at some time. Dandruff does not cause permanent baldness, though in extreme cases it might result in severe hair-loss. How to Get Rid of Dandruff? The first step is to keep the hair clean. Wash your hair and scalp frequently: it could be daily or every other day depending upon how stubborn

your dandruff is. Use a good anti-dandruff shampoo. Massaging and brushing your hair frequently is also extremely helpful in treating dandruff. They invigorate the blood circulation to the scalp, promote oil secretion and dislodge the dead skin cells sticking to your scalp. Avoid hair sprays and lotions that can build up on your hair. Use an anti-dandruff or medicated shampoo, containing zinc pyri-thione or selenium sulphide or corticosteroid hormones. To start with, use it once a week and as the dandruff condition improves, reduce the usage to once a month. If the situation persists, a dermatologist should be consulted. Treatment with herbal extracts has been found to be extremely effective. Herbs like henna, shikakai, reetha and amla are antiseptic ingredients provided by nature and have a powerful cleansing action, without any harmful effects like those of detergents. Pure coconut oil or olive oil can be heated and applied on the scalp It should be left overnight. Next day, apply the juice of a lemon on your scalp. Shampoo your hair after an hour. This is a good treatment for dandruff. Applying henna on the hair also helps in controlling dandruff. Where dandruff is concerned, it is vital to keep the hair, combs, towels and pillow cases absolutely clean, as there are chances of reinfection.

Hair care

Tips for Different Types of Hair One should know the prakurthi or constitution of there hair and then start treating them, because different hair needs different care. Here are some different tips for different types of hair Before we get down to hair care, it is important to determine what kind of hair you have. Different types of hair need different hair care. The use of wrong products is detrimental to the hair. Always use products made by reputed companies, check the date of manufacturing and never use products more than a year old. Dry Hair The lack of sebum and oil causes dry hair. Flakiness of the scalp and dandruff are a direct result of dryness. If these is dryness the hair looks limp, tends to look thin and rough, becomes less elastic, loses its body and bounce. Dry hair is susceptible to breakage and damage. Tips for Dry Hair Your hair care routine should aim at replenishing the oil and the moisture in your hair, as excessive dryness can cause your hair to become brittle. Use a good hair oil to massage your hair. It is a good idea to steam the scalp before a hot oil massage. With fingers planted firmly on the head, work the fingers in small, circular strokes, first clockwise and then anti¬clockwise, to massage your scalp. If the hair is extremely dry, rub a little oil right into the roots before steaming. This is even more beneficial, as the subsequent steaming and a second massage with oil encourage better and deeper absorption. Frequent shampooing is harmful — don’t use a strong shampoo. Never comb, brush or massage vigorously. Normal Hair The hair is healthy, shiny and silky without overdry ends or over-oily roots. It has a good body and bounce. It is easy to cope with and care for normal hair. Tips for Normal Hair To maintain the balance of neither too oily nor too dry, condition your hair regularly. Use a good hair oil to massage your hair.

SEPTEMBER

Frequent shampooing is harmful — don’t use a strong shampoo. Never comb, brush or massage vigorously. Oily Hair When the oil (sebaceous) glands in the scalp secrete excess oil, it travels down the hair shaft, causing excessive oiliness of the scalp and hair. Oily hair therefore appears lanky, dark and coarse. Tips for Oily Hair The hair care routine should aim at removing the excess oil and the dead skin cells which clog up and suffocate the hair follicles in the scalp. Regular cleansing helps wash away the excess oil from your hair and scalp and removes the dead skin cells. Wash your hair every third or fourth day, with a good quality herbal shampoo. Toning helps you to keep your hair healthy. Toning of the scalp and hair is achieved by gently massaging the head with your fingers, which helps in stimulating and invigorating the blood circula¬tion required for the healthy growth of hair. Do not use hair oil, instead, use a good quality toning lotion. Excessive oiliness, when neglected, often leads to falling hair and premature baldness in men. So it should not be neglected, for prevention is always better than cure.

Body care

Unwanted hair removal Every one has to face the hair removal process from time to time. While it’s just simple routine for some, others have to cope with a number of problems while dealing with it.

AYURPHARMA

Hair removal may be classified by different criterions, for example, location (facial, body, bikini), removal type (waxing, shaving, plucking) or by type of hair (vellum or terminal). Knowing what type of hair you will be removing will help you choose the way of performing the procedure. Vellum hair is soft, fine and short. This type of hair usually appears on women back, chest or face and they are so small that the removal is only optional, unless you have a darker complexion, which can make them more visible. Terminal hair is coarser, darker and longer. It’s the hair that grows on your head, legs, armpits and bikini zone. Laser hair removal, plucking or waxing is the most recommended methods for vellum hair removal. It might be more painful, but the effect will last longer and hair won’t get thicker (the effect shaving causes). Terminal hair can be removed by using such methods as shaving, plucking, depilatories, waxing, electrolysis, laser and prescription treatment. The choice of methods depends on your ability to deal with pain, area you want to clean and your financial capabilities. Shaving is cheap and won’t cause you any pain. Yet the effect won’t last so long (1 to 3 days) and you’ll have to do it again and again. It is very comfortable in a way that you do not have to make any appointments with anybody. The major drawback is that you can’t shave them until they reach a certain length. Plucking lasts long (from 3 to 8 weeks), yet it’s really painful and takes time. As far as you can pluck only one hair at a time removing hair from your legs would probably take a whole day. Plucking is recommended on small areas such as eyebrows, upper lip and chin. Depilatories are inexpensive and won’t cause you any pain. The effect lasts from several days to two weeks. They can be used on any area, yet you have to be very careful when purchasing one, because it usually has a lot of chemical components. It’s not recommended for people who have sensitive skin. Waxing is probably the most painful method of hair removal, however, the effect is long lasting (from 3 to 6 weeks). For some people it may cause skin redness, inflammation and bumps, yet these effects will not take long to disappear, leaving smooth skin. Electrolysis is permanent, yet it takes time for you to see the actual effect and you have to make appointments for that. It’s usually used only for small areas such as upper lip, eyebrows and underarms. The procedure is painful and may cause dry skin, scabs, scarring and inflammation. Also, electrolysis is expensive, being the only permanent hair removal method. Laser hair removal is expensive and painless, with the effect lasting for about 6 months. The thing with laser hair removal is that your skin has to be light and the hair dark if you want to use this method.

DR JOHN EBNEZAR CREATING A NEW WORLD RECORD IN BOOK WRITING

and 15 September 2008. Two World Records in Writing : He was the first orthopedic surgeon in the world to have created a Guinness World Record in the field of book writing in the year 2010. This was bettered by Ryuho Okawa of Japan in the year 2011 and now I have bettered his record and have created a new world record in book writing for the year 2012 with 108 books. Thus he has become the first orthopedic surgeon in the world to create two world records in the field of book writing. This is a matter of great pride to our country and Karnataka for never before any orthopedic surgeon has gone on to create two world records that too in the field of writing. First of its kind in the world in health education for common man And moreover these 108 books are meant for common public and no doctor in any field of medicine or surgery has ever attempted to bring so many books for public education.
