Hypertension or high blood pressure

Hypertension or high blood pressure is a very common disease. Various surveys suggest that 10% of population above 20 years of age have hypertension. In people who have more tension in their work -example, executives - it may reach 20%. High blood pressure must be recognised early and treated because it is a silent killer. It is a major risk factor for “Heart Attack”. Heart attack or coronary artery disease is twice as common in patients with high blood pressure and paralytic strokes are 8 times more common. Proper control of blood pressure reduces these risks. ¾¾ CAUSES OF HYPERTENSION In majority of people the exact cause of high blood pressure is not known. ªª The high blood pressure occurring in middle age is likely to be due to heredity. Contents have high blood pressure there is 25% chance of developing high blood pressure in their children. ªª Those who consume more salt in their diet are likely to develop high blood pressure. The salt accumulates in the blood vessels and makes it more contractile and hence causes high blood pressure. Hence those who have high blood pressure should reduce salt in the diet. ªª Mental stress, emotional upsets, anxiety have all been implicated in the causation of high blood pressure. ªª It has been found that those who have high blood pressure are more often irritable, get angry for trivial things, emotional, and try to adhere to strict discipline. ªª There are a small number of people with high blood pressure where definite cause can be found. There may be disease of Kidney, renal artery stenosis, coarctation of aorta, disease of endocrine glands and some neurological disorders. ªª In some cases the cause of high blood pressure may be due to long-standing use of oral contraceptive pills. ªª It is important to recognise these secondary causes of hypertension because if treated there can be a ‘Cure’ for high blood pressure. ¾¾ CAN WE PREVENT HIGH BLOOD PRESSURE? In majority of patients high blood pressure is produced by interaction of hereditary and environmental factors. As we cannot alter our heredity, attempts should be made to minimise enviromental factors. These measures are weight reduction, reducing the salt in diet, avoiding smoking and drinking alcohol. To some extent Yogasanas, meditation and relaxation techniques help to control the blood pressure. ªª Early detection and early treatment of high blood pressure prevents complications; ªª If untreated, high blood pressure may damage vital organs like brain, heart, kidney, eyes as well as blood vessels. ªª Since in most people there are no symptoms and there are no specific symptoms of high blood pressure, all adults, especially middle aged individuals should get blood pressure checked regularly and routinely. ªª Salt intake reduction, weight reduction, avoiding smoking and alcohol are all important measures in controlling high blood pressure. ªª In most people with high blood pressure medication is needed lifelong. ªª Some Yogasanas and meditation are additional measures for control of high blood pressure.

TYPES OF CAUSES: Shock can be divided into two types’ Nervous shock and True shock. ªª NERVOUS SHOCK - is due to strong emotional upset, eg fear, pain, or bad news, and not necessarily due to a serious injury. (Does not need treatment) ªª TRUE SHOCK-is due to: Severe Bleeding, Severe Burns, Heart Attacks, Abdominal Emergency , Crush Injuries , Loss of Body Fluid & Bacterial Infection ¾¾ SIGNS & SYMPTOMS ªª Casualty feels faint or giddy ªª Complains of blurring of vision ªª Feels cold with clammy (damp & cold) skin. ªª Face and lips look pale. ªª Pulse may become slow at first, but later always beats faster, but gets feeble. ªª Vomits (in many cases). ªª Becomes unconscious in later stages of shock. ¾¾ MANAGEMENT ªª Reassure the casualty (when conscious) ªª Put him comfortably on his back. Except in cases of injury of the head, of the chest or of the abdomen, lower the head slightly and turn it to a side. In cases of vomiting, place in the three quartet back-up position. ªª Loosen tight clothing, but do not remove clothing. ªª Wrap in light bedsheet or thin blanket (rug). ªª Never use hot water bottles or very warm rugs. Do not rub any part of the body with anything. ªª In cases of injuries to chest or abdomen, nothing should be given by mouth as he may later need an operation or blood transfusion. ªª Observe all the above quickly as even minutes delayed may mean death. If there is no chest or abdominal injury, and the patient is conscious, give sips of water, hot tea or coffee or tender coconut water (Never give any alcoholic drinks). MOST IMPORTANT: SHIFT THE CASUALTY TO THE HOSPITAL ON TOP PRIORITY. ¾¾ TREATMENT ªª Do not allow people to crowd around, as fresh air is essential. ªª Stop bleeding, if possible. ªª Reassure the casualty. ªª Keep the casualty warm. ªª Moisten the lips if they are dry. ªª If due to dehydration, give plenty of oral fluids. ªª Place casualty in “head-low” position, if there are no head or spinal injuries. ªª Arrange for the transportation of the casualty to the care of a doctor or hospital as soon as possible.

Myths and facts

Mental Retardation Come out of Myth and know the facts For details Vydhe hospital Rehabilitation center Whitefield, Bangalore – 66 Ph: 080-28413381/2/3/4 www.vims.ac.in

Myth: Mental retardation is a hereditary problem. Fact: Only a few causes of mental retardation are hereditary, i.e. passed on from parents to children. Mental retardation is often caused by external influences, some of which can be prevented. Myth: Bad deeds in the previous life of parent’s cause’ mental retardation. Fact: This is completely false. Such beliefs add to the suffering of the families who are already overburdened with caring for their special children. Some communities perpetuate the myth that if one tries to remedy the illness or take treatment, the suffering will be repeated in one’s next life. This results in added suffering to the patient from lack of proper treatment. Myth: Mental retardation is caused by pregnant and lactating women not following restrictions on food. Fact: Pregnant and lactating women must maintain good nutrition for their own health and also for the health of the unborn or newly-born child. There is absolutely no basis for restricting food to pregnant and lactatittg women. However, .some medications, if taken during pregnancy, may lead to malformations m the unborn child. Medication should be taken only on the prescription of a doctor. When consulting a doctor for an illness, women should always inform the doctor about being pregnant Myth: Mental retardation is infectious. Fact: This is completely false. Mental retardation cannot be spread by touching a patient. Children with mental retardation must be cuddled and loved just as much as normal healthy children. Myth: Tonics/vitamins/medicines can cure mental retardation. Fact: If mental retardation is caused by a treatable condition, appropriate treatment will cure it. However, there are no “brain tonics” which can stimulate a damaged brain. Many unscrupulous healers and manufacturers market such substances with popular and misleading names, which imply that if these substances are taken, the child will become normal. This is particularly common in rural areas, where quacks market some mixtures, guaranteeing parents a cure. These substances frequently contain a substance called ‘steroids’. These medications make the child plumper and perhaps happier temporarily, which makes the parents feel good. But the basic condition of mental retardation is not cured. In fact, steroids are

september-2013 AYURPHARMA

harmful if taken for long durations. Myth: Brain operations can cure mental retardation. Fact: There are very few conditions leading to mental retardation which can be cured by surgery. Myth: Marriage can cure mental retardation. Fact: This is completely false. In fact, a mentally retarded person should be married only with the full consent and knowledge of the partner. Myth: Children with mental retardation become completely normal when they grow up to be adults. Fact: Children can make substantial progress as they grow up. However, it is unlikely that they will become completely normal. Each case must be assessed individually. Myth: Mentally retarded adults have poor sexual control and pose a danger to others. Fact: In fact, adults with mental retardation are sexually more inhibited than their normal counterparts. On the contrary, many such people are victims of sexual abuse. Myth: Mentally retarded children are incapable of learning anything and so everything has to be done for” them. Fact: These children are capable of learning, although how much they learn and at what speed they learn may vary. The harder we work with them, the more they will learn and more independent they can become. There i» •» better solution to their development than working hard with them. Myth: Mentally retarded children should not be made to cry for any reason or should not be disciplined in any fashion. Fact: All children need to be disciplined. Every effort should be made to teach children with mental retardation what is right and what is wrong, recognizing their capacity for learning and taking into consideration factors beyond their control. Myth: Faith healers can cure mental retardation. Fact: This is completely untrue. There are many sad stories about parents selling all their valuables and their land on the advice of faith healers and giving this away in charity, frequently to the faith healer. Faith healers mislead the parents. There are many alternative systems of medicine practiced, some of which claim to have a ‘cure’ for mental retardation. However, considerable research is still needed before their exact efficacy and safety can be established. www.bangalorehealthtv.com

Women health

Pregnancy and nutrition Good nutrition during pregnancy is not just important for your health but also for the optimum growth and development of the baby

For details contact Vydehi Hospital Department of Gynecology Whitefield, Bangalore – 66 Ph: 080-28413381/2/3/4 www.vims.ac.in

Hair fall, chipping of nails, swelling in the feet especially around the ankles, leg cramps, loss of weight. excessive fatigue, any kind of dizziness, Lack of feeling of the baby’s movements. ¾¾ Morning sickness ªª In the first months of pregnancy, when morning sickness is usually at its worst, your tiny fetus requires minimum nutrition. So eat whatever you can retain. ªª If you are vomiting, keep yourself hydrated by drinking at least 2 liters a day milkshakes, buttermilk, juices, soups and water. ªª If you feel nauseated, try chewing on mint, cardamom, clove or sucking on ginger or lemon,. Avoid supari (beetle nut) and other packaged mouth fresheners. ¾¾ Balanced diet during pregnancy The diet during pregnancy should contain larger amounts of ‘tissue building’ and ‘protective foods’ without any great increase in the ‘energy yielding’ foods. The need for energy in a pregnant woman is raised not much above her pre-pregnancy requirements. On the

Pregnancy is a very exciting period of expectations in a woman’s life. It is a unique natural gift of the women folk. It is very important that you stay healthy and happy during the period. One of the major factors that promote health & well being of the mother & the baby in the womb is wholesome nutrition.. Your diet has direct influence on the birth weight of the baby. Pregnant women who do not eat well might not gain adequate weight during pregnancy which might result in giving birth of a low birth weight baby (less than 2.5 Kg). Low birth weight babies are at risk of impaired physical growth, mental development and deficiency diseases. During pregnancy the woman creates a new life. The mother also grows during pregnancy. There is an increase in the overall size of her baby. There is an enlargement of the child bearing organs to accommodate the developing fetus, an increase in the breast tissue to meet the subsequent demands of lactation and an increase in the amount of circulating blood to facilitate the supply of nourishment of the newly built tissues. In order to provide for this rapid growth which includes formation of bone, production of blood and building of muscles, brain and other tissues the fetus needs ready- made nutrients. These are solely derived from the mothers body through her blood stream. Therefore, the mother has to pay special attention of the kind & quantity of food she eats, since this will determine the heath of her baby. ¾¾ Warning signs of food deficiency

other hand, the woman’s need for body building and protective foods particularly during the latter part of pregnancy is increased to nearly double that of her usual requirements. After a careful examination of the needs of a pregnant woman, nutrition experts have suggested a ‘balanced diet’ for pregnancy. ¾¾ Eat a well balanced diet ªª Cereals provide energy hence they should form the major portion of the diet. Fats and oils also provide energy but should be consumed sparingly. It is traditionally believed that consumption of ghee (especially in the 3rd trimester) will help ease delivery. Nutritionist’s advice is that excessive consumption of ghee will only make you fat without giving any helping hand during delivery. ªª Milk curd & cheese -are the very good sources of protein. 2-3 servings in the form of 2 cups of milk and 1 -2 katories of curd. Meat, poultry, fish, dry beans and dhals, eggs and nuts (Badam, pista, cashew nuts). These are also the good sources of protein required for the growth of the fetus.- 2-3 servings a day. ªª c. Vegetables and fruits are the sources of minerals like calcium and iron and vitamins like vitamin A and vitamin C. Hence they should be eaten in plenty. Green leafy vegetables are rich sources of all vitamins and minerals. Citrus fruits are the rich sources of vitamin C. Consume 3-5 katories of vegetables a day. Vary your vegetable choice between dark green and leafy (spinach) deep yellow or orange (Carrots, sweet potatoes) and starchy (potato) . Fruits - 2-4 servings like 2 bananas, 1 Apple and 1 orange in a day. ªª Bread, rice, chapattis and potatoes- 6-11 servings. It is not dangerous to eat chocolates, ice- creams or cakes during pregnancy, providing you don’t consume excessive amounts. Aerated drinks only add calories without giving any kind of nutrition. Hence such foods can be eaten in small quantity and occasionally. ¾¾ Eat 3 or more nutritious meals a day and do not skip breakfast. “You are what you eat” is a phrase that has lot of merit, especially when you are pregnant .Most pregnant mothers get hungry easily and need to eat more regularly as their need for energy (an extra 300 calories a day) and most nutrients increases. Eating 2 regular meals and 2-3 light snacks a day will not only satisfy your hunger pangs but also meet all your nutritional need. ¾¾ Eat more of “Nutrient dense” and less of “empty calorie” foods. Recommended dietary allowance (RDA) refers to the amount of energy (calories) and nutrient which will maintain good state of nutrition in healthy people of all ages .The allowance include a margin of safety to cover individual differences. Opt for “nutrient dense” foods (packed with essential nutrients) that are prepared with less oil/fat and are less sweet. ¾¾ Increase food intake ªª Whole grains, pulses. legumes, sprouted pulses, leafy vegetables, jaggery, dates, groundnuts, are foods of plant origin having good iron content. Include more number of these in the daily diet.

september-2013 AYURPHARMA

ªª

Include green leafy vegetables in daily diet right from the beginning as all foliage provide ‘folic acid’ much needed during early months. ªª Consume vegetables in plenty. And one seasonal fruit daily. ªª Milk, curd, buttermilk, egg, meat, fish are helpful foods during pregnancy. ªª Some foods like eggs, papaya etc. causes heat during pregnancy is a misconcept. ªª Iodized salt should be consumed as pregnant women require sufficient iodine for brain development of the child in the womb. ªª Take plenty of fluids/water. Drink at least 8 cups of fluids a day. Water, milk, fruit juice and vegetable soups are excellent choices. ªª Take small and frequent meals. ¾¾ Rest: Heavy work should be avoided throughout the pregnancy. Rest (in lying down position) during third trimester is important to enable adequate flow of nutrients from mother to the child. ¾¾ Avoid: ªª Artificial sweeteners are strictly prohibited. ªª Monosodium glutamate (Ajinomoto) found in Chinese foods should be avoided. ªª Do not drink tea or coffee immediately after lunch or dinner, as it will reduce the absorption of iron from your food. ªª Try and avoid papads, pickles, salted snacks, potato chips, chocolates, cakes, pastries and aerated/ carbonated beverages. ªª Avoid drugs like painkillers or antibiotics without your doctor’s consent. ªª Don’t drink alcohol or smoke cigarette. ªª Note that cough medicines include alcohol. ¾¾ Eat folate and iron rich foods: Pregnant women need more of folic acid that helps to increase birth weight and reduce congenital anomalies. Green leafy vegetables, legumes, nuts and liver are good source of folic acid. Iron is needed for hemoglobin synthesis, mental function and body defense against diseases/ infections. Deficiency of iron leads to anemia maternal mortality and low birth weight infants. Plant foods like legumes, dried fruits and green leafy vegetables contain iron. It can be obtained better through meat, fish and poultry products. Iron & folic acid tablets should be consumed throughout the pregnancy. ¾¾ Immunization: Immunization of the pregnant woman with tetanus toxoid 5th & 6th months of pregnancy in two doses at an interval of 4 weeks is essential. ¾¾ Pregnancy and Diabetes There are some women who are potentially diabetic which is not a bar for having a healthy baby. But always consult your doctor before planning pregnancy and control your blood sugar very rigidly. Have a regular diet, exercise and insulin if necessary for a healthy baby. Visit your doctor and obstetrician whenever you are asked to visit. Your baby has no greater risk of developing diabetes because you had diabetes during pregnancy. It is always advisable to have one or maximum two children at an early age. www.bangalorehealthtv.com

EYE DONATION

EYE DONATION IS THE MOST NOBLE DEED ONE “ CAN DO” “COME LETS DO A NOBLE DEED TODAY AND LIGHT THE LIFE OF TWO BLIND PEOPLE” Fill colour in One’s Life! DONATE EYES

Eye Donation means donating the eyes of a person after death for transplantation with the family consent. There are millions of people who are blind because of problems affecting the carnea transparent front part of the eye. In India we have an estimated 4.6 million people with carneal blindness that is curable through carneal transplantation (so called eye transplant). But the rate of eye donation is for less than the need. Therefore it is an appeal to everyone to pledge to donate the eyes on their death. We should know that our eyes live even after death and can light the life of two bind persons. Magnitude of the problem of blindness in India We have an estimated 4.6 million people with corneal blindness that is curable through corneal transplantation with eyes received by us as donations. Out of this, an estimated 4.6 million are 90% of the people with corneal blindness are below the age of 45 years including some 4.5 million below the tender age of 12. More than 90% of the corneal transplantation is carried out successfully in restoring vision to people

with corneal blindness. Including infants born with cloudy cornea, corneal transplantation can make a big difference to the rest of their lives. Your beloved ones thus live and light up the lives of others even after death. The relatives of the deceased person can donate the eyes of their beloved ones immediately after the death. The eyes need to be collected within six hours of death. ¾¾ What is corneal transplantation? It is an operation that replaces opaque cornea with clear cornea obtained form human donor after death. Living person cannot donate his eyes ¾¾ Who can be an eye donox? There is no age limit, people with spectacles, diabetes hypertension, persons blind with retinal diseases provided the cornea is clear. All one needs to do is to pledge one’s eyes to donate after death. The support of relatives or friends to carry out the wishes of eye donation after death. One needs to call the eye bank immediately as the eye needs to be removed with in 6wks of death.

Death due to unknown cause, death due to infections like rabies, syphilis, Hepatitis B, AIDS, Septicmia, Malignancy ¾¾ Instructions to be followed after death:ªª Inform eye bank immediately ªª Keep both eye closed and cover with moist cotton. ªª Switch off the ceiling fan if any directly over the deceased person. ªª Raise the head of the deceased by about 6” if possible to reduce the chances of bleeding at the time of removal of eyes. ªª If possible apply antibiotics eye drops periodically into the deceased person’s eyes. ¾¾ Is there any religious / social conflict? Certainly not more over eye donation gives a gift of sight light. As such it is consistent with beliefs and attitudes of all major religious and ethical traditions. ¾¾ Will eye donation cause delay in funeral arrangement and disfiguring? No it takes only half hour or less by the trained person & there is no disfiguring of face. Artificial glass eyes can be used to fall up the eye select and improve appearance. ¾¾ Is there any payment? No payment from donor as well as recipient however the recipient may have to bear the medical expenses for surgery. use as these are used for research and education. Cornea is the clear tissue covering the front of the eye. If the cornea becomes cloudy from disease, injury, infection or poor nutrition, vision is dramatically reduced or lost. Corneal blindness can be treated by replacing the damaged cornea by a healthy donated human cornea. The human cornea can be procured through Eye Donation only. Corneal blindness affects mainly children and young adults who have a long life ahead of them. Though we have trained, highly qualified surgeons and hospital facilities to do the corneal transplantation, we do not have enough eyes donated to do the surgery. Approximately 70 lakh blind population of our country are waiting for corneal transplantation and approximately 40,000 new cases of this treatable blindness are being added every year. However, only 20,000 eyes are donated every year in our country. Leaving behind a huge backlog of avoidable blindness. One eye donation can give sight to two corneally blind persons. Before Eye Donation Eyes should be donated with in 6 hrs of death. Anyone can be a donor, irrespective of age, sex, blood group or religion. Anyone with cataract or spectacles can also donate eyes. Persons suffering from hypertension, diabetes can also donate eyes. Total procedure takes 15-20 minutes. There is no disfigurement of the face of the donor after eye donation. Eyes can be donated even if the deceased had not formally pledged their eyes during their lifetime. Eye Bank team will rush over to the donor’s home or any other place where the body is available after death. This is free service in public interest. Following pledging your eyes please inform your family about your wish so that they can fulfill your wish.

THYROID DISORDER The thyroid gland is the one most important of the endocrine glands in the body. It is a butterfly shaped gland situated in the anterior part of the neck below the skin and muscle. It plays an important role in the life of every individual. The hormone secreted by it is required by every single cell in the body. 2% of the people suffer from thyroid disorder. Women suffer from the disorder more than the men ¾¾ Anatomy of the thyroid gland The thyroid is a small gland located below the skin and muscles at the front of the neck, with left and right halves (called lobes) that look like a butterfly’s wings. It weighs about 30grams.. Fig1: Picture showing Thyroid gland in the neck region.

¾¾

Hormones produced by thyroid gland The Thyroid gland produce two hormones namely ªª Thyroxine (also known as T4) ªª Triiodotyronine (also known as T3) The thyroid gland stores these hormones and releases them as they are needed. Iodine is a fundamental requirement for the production of T3 and T4 and is therefore essential to life itself. T3 and T4 are responsible for controlling the basic activity of every cell in the human body. ¾¾ Functions of thyroid hormones The function of the thyroid is to secrete hormones which control metabolic pathways and thereby control various physiological functions. Thyroid hormones are essential to proper development and differentiation of all cells of the human body. These hormones also regulate protein, fat, and carbohydrate metabolism

Fig2: Picture showing secretion of thyroid hormones ¾¾ Control of thyroid hormone production A properly functioning thyroid will maintain the right amount of hormones needed to keep the body’s

12

metabolism functioning at a satisfactory rate. As the hormones are used, the thyroid creates replacements. The quantity of thyroid hormones in the bloodstream is monitored and controlled by the pituitary gland which is located in the center of the skull below the brain, senses either a lack of thyroid hormones or a high level of thyroid hormones, it will adjust its own hormone (TSH) and send it to the thyroid to tell it what to do. The hypothalamus releases thyrotropin-releasing hormone (TRH), which stimulates the pituitary gland to release thyroid-stimulating hormone (TSH). ¾¾ Change in thyroid hormone production There are two types of changes in the thyroid hormone production-one excess production is called HYPERTHYROIDISM and another low production called as HYPOTHYROIDISM. ¾¾ HYPERTHYROIDISM. When the thyroid produces too much hormone, the body uses energy faster than it should. This condition is called hyperthyroidism. ¾¾ Causes of hyperthyroidism The most common cause of hyperthyroidism is Graves’ disease. This occurs when the body’s immune system over stimulates the thyroid. The following conditions also cause hyperthyroidism: ªª A single or multiple nodules in the thyroid gland may produce more and more hormones. ªª Thyroiditis, a disorder that can be painful or painless, can also release hormones that was stored in the thyroid gland causing hyperthyroidism for a few weeks or months. ¾¾ Symptoms of hyperthyroidism The following are symptoms for hyperthyroidism: ªª Irritability/nervousness ªª Muscle weakness ªª Infrequent, scant menstrual periods, infertility ªª Weight loss ªª Sleep disturbances ªª Enlarged thyroid gland ªª Vision problems or eye irritation ªª Heat sensitivity

¾¾

Diagnosis of hyperthyroidism Diagnosis of hyperthyroidism is usually done by ªª Hormone Studies: Study of T3, T4, TSH, free T3 and free T4. ªª Thyroid Scanning

investigated since a small proportion of them are cancerous.

¾¾

Treatment of hyperthyroidism Several methods of treatment are available for hyperthyroidism. ªª Radioactive iodine scan: Because the thyroid is the only organ in the body that takes up and uses iodine, when a dose of radioactive iodine is given the radiation enters the thyroid and destroys part of it. ªª Medications (Neomercazole) can also be used for treatment. These drugs slow down the working of thyroid and restore normal levels. ªª Surgical removal of part of the thyroid is occasionally used as a treatment. ¾¾ HYPOTHYROIDISM When the thyroid doesn’t produce enough hormone, the body uses energy slower than it should. This condition is called hypothyroidism. ¾¾ Causes of hypothyroidism The most common cause of hypothyroidism is Hashimoto’s thyroiditis. In this condition, the body’s immune system mistakenly attacks the thyroid gland. There are several different causes of thyroid disease. The following conditions cause hypothyroidism: ªª Thyroiditis is an inflammation of the thyroid gland. This can lower the amount of hormones produced. ªª Hashimoto’s thyroiditis is a painless disease of the immune system that is hereditary. ªª Postpartum thyroiditis occurs in 5 percent to 9 percent of women after giving birth. It is usually a temporary condition. ªª Iodine deficiency ¾¾ Symptoms of hypothyroidism The following are symptoms for hypothyroidism: ªª Fatigue ªª Frequent, heavy menstrual periods, infertility ªª Forgetfulness ªª Weight gain ªª Dry, coarse skin and hair ªª Hoarse voice ªª Intolerance to cold ¾¾ Diagnosis of hypothyroidism Diagnosis of hypothyroidism is usually done by ªª Hormone Studies: Study of T3, T4, TSH, free T3 and free T4. ¾¾ Treatment of hypothyroidism Treatment of hypothyroidism consists of taking thyroid hormone in pill form on a daily basis. Adjustment of the dose of thyroid pills is based upon measurements of serum TSH. Most patients need to take thyroid hormone for life. Diseases of Thyroid ¾¾ Goitre Enlargement of thyroid gland is called as Goitre. The enlargement is caused by the deficiency of Iodine. ¾¾ Thyroid Nodules Thyroid nodules are lumps which commonly arise within normal thyroid gland. Thyroid nodules are common and treatable but should always be

Thyroid cancer Thyroid cancer is a disease in which cancer (malignant) cells are found in the tissues of the thyroid gland. It is although uncommon, but in the majority of cases it is successfully treated. ¾¾ Thyroiditis Thyroiditis is an inflammation of the thyroid gland. This can lower the amount of hormones produced. It is an autoimmune disorder, which is caused by antibodies or immune cells to self antigens. ¾¾ Prevention ªª Use of more iodized salts. ªª Avoid radiations which can leads to cancer. ªª Improved public awareness and understanding of thyroid disorders will enable patients and their families to cope more effectively with the sometimes disturbing course of thyroid illness. ¾¾ Conclusion: Thyroid hormones are extremely important and have diverse actions. As long as thyroid releases the proper amounts of hormones, body system functions normally. But sometimes thyroid doesn’t produce enough hormones, upsetting the balance of chemical reactions in the body. The disease is not life threatening, but those affected could be physically weak for a fairly long time. So proper medications & regular follow up should be done.

Asthma makes breathing difficult for more than 300 million people worldwide according to the WHO. It affects people living in the developing as well as developed countries and affects all age groups. It is the most common chronic disease in children and even with all the advances in treatment, the prevalence has only risen in the past few years. Asthma is primarily a chronic disease (a respiratory allergy) that presents as recurrent attacks of breathlessness and wheezing. There is a vast variation in the frequency and severity

of symptoms of asthma from person to person. This is where the individualized treatment with Homeopathy can give significant relief to patients of asthma and help them lead an easy life. ¾¾ What is Asthma? Running out of breath… The term ‘asthma’ comes from the Greek word ‘aazein’ which means ‘to exhale with open mouth, to pant’. This has been derived from the typical feeling in asthma which is that of running out of breath. Asthma (also called Bronchial Asthma) is

a chronic disease that affects the tubes (bronchi) that carry air in and out of lungs. Whenever these tubes become inflamed due to any reason, three things happen: ªª The muscles of these tubes contract ªª There is excessive mucus production inside the tubes ªª Constriction of the tubes occurs due to the above two reasons This ultimately causes difficulty in the passage of air in and out of the tubes leading to the feeling of breathlessness. When asthma is diagnosed after 18 years of age, it is known as adult onset asthma and is most commonly due to allergies (respiratory allergy). Adult onset asthma is more frequent in women as compared to men. Childhood asthma on the other hand is almost 3 times more prevalent in boys as compared to girls. Around puberty, the prevalence is more or less the same in girls and boys. Developed countries all over the world experience higher incidence of asthma (breathlessness, respiratory allergy, wheeze) than developing countries. The ratio between these groups is however reversed for deaths due to asthma. In people who are prone to this condition, the air tubes are very sensitive and react strongly to allergic or irritating things. The reaction (allergy) involves the above three steps and results in cough, wheezing and chest tightness. This state may persist in a milder form at most times; however it may worsen under certain conditions and at such times is called an ‘asthma attack’. Severity of asthma may range from minor wheezing to life-threatening attacks. Long term medication is generally needed to control the disease and prevent the acute attacks. ¾¾ Causes of Asthma ¾¾ Why am I so breathless? The primary reason, why asthma (breathlessness, respiratory allergy, wheeze) develops in some people and not in others, is not well understood. However, certain risk factors have been identified that make a person more prone to develop this condition. Conversely, it must be noted that not all cases develop asthma in spite of the presence of risk factors. One of the strongest risk factors for asthma is a combination of genetic tendency and exposure to allergens / irritants. The allergens or irritants form a large group and some of the common ones are mentioned here: ªª Pollen and moulds ªª House dust mites ªª Air pollution ªª Pet dander ªª Tobacco smoke ªª Chemical irritants in the workplace ªª Strong perfumes ªª Presence of any other kind of allergy ¾¾ Apart from allergens/irritants, there are other things that can trigger an asthmatic attack:

september-2013 AYURPHARMA

ªª ªª ªª

Chilly air Intense emotions like anger, fear, etc. Physical exercise (known as Exercise-induced Asthma, this is triggered by rigorous physical exercise) ªª Certain drugs like aspirin, non-steroidal antiinflammatory drugs (NSAIDs), Beta-blockers, etc. ªª Viral infections ªª Certain foods ªª Changes in weather ªª Hormonal changes such as those occurring in pregnancy, menopause, thyroid disorders, etc. ªª Obesity ªª Gastro-esophageal reflux disease (GERD) – the reflux of the acid from the stomach into the food pipe (esophagus) can trigger asthma in sensitive individuals ªª Heredity ¾¾ Symptoms of Asthma Breathing becomes an effort… The difficulty in breathing (asthmatic attack, respiratory allergy, wheeze) experienced by an asthmatic person can have numerous expressions. Some people feel their throat is being strangled during the wheeze, others remark that they feel as if they are breathing though a sponge, yet others may feel trapped as if by a plastic sleeve. As many the people, so many the expressions! However, there are certain patterns that can be commonly seen such as:  Perennial or seasonal asthma  Continuous or intermittent asthma  Daytime or night time asthma ¾¾ The symptoms that are seen amongst the patients can have any combination from the following set: ªª Cough – this is mostly dry cough (not accompanied by phlegm) ªª Breathlessness / dyspnoea ªª Wheezing (wheeze) - An audible whistling when exhaling ªª Chest tightness or pain ªª Difficulty in lying down due to the breathlessness/dyspnoea ªª Bouts of coughing or wheezing (wheeze) that www.bangalorehealthtv.com

are worsened by a respiratory virus such as a cold or the flu ªª Restless sleep, tired feeling on waking ªª Easily getting tired with activities that normally would not affect anyone so much ªª Tightening of neck muscles, rapidity of breathing ªª Difficulty in talking at times ªª Excessive sweating (especially during an acute asthma attack) The symptoms of asthma change over time in most people and these must be tracked regularly. The patient must periodically visit his physician to get his condition evaluated. ¾¾ The physician evaluates asthma based on certain key features and classifies them into:  Mild intermittent asthma- Mild symptoms up to two days a week and up to two nights a month  Mild persistent asthma- Symptoms more than twice a week, but no more than once in a single day  Moderate persistent asthma- Symptoms once a day and more than one night a week  Severe persistent asthma- Symptoms throughout the day on most days and frequently at night It must be noted that first three types of asthma (respiratory allergy / wheeze) respond very well to Homeopathy; however in the last type the treatment tends to be more palliative in nature. ¾¾ Diagnosis of Asthma Seize that wheeze in time Once your doctor has taken your history and has examined you thoroughly, he will be fairly sure of the diagnosis. This is one of those conditions for which history and clinical findings are sufficient to diagnose. However, your physician may perform certain tests to confirm the same:  Spirometry (Lung function tests)  Peak flow rate  X-ray chest Some other diagnostic tests can also be done to confirm asthma; however these are not very commonly performed. These include:  Methacholine bronchial challenge  Nitric oxide test In case of children, it can be hard to differentiate asthma from a wheezy bronchitis or pneumonia and

16

the diagnosis should be very carefully established. ¾¾ Living with Asthma Take control of your asthma… NOW Managing asthma may not be all that easy for a patient but effective steps taken in time can help them deal better with this chronic illness. Here are some tips that you can incorporate in your life to make a difference: ªªDrink plenty of water to loosen the secretions ªªAvoid exposure to allergens / irritants as far as possible – Effective steps to prevent the exposure to known allergens/irritants goes a long way in keeping good health ªªAvoid a sedentary life and start exercising regularly to keep your body fit. Exercising helps to improve the lung functions and also conditions the muscles. Contact your health care provider for details on the exercise that are good for you. ªª Learn stress management techniques to effectively deal with stress ªª Green tea which contains antioxidants may give some relief in asthma symptoms ªª Many people report improvement in symptoms after including ginger in their diet ªª Stop smoking ¾¾ Myths and Facts Myth–I have asthma only when I have trouble in breathing and the rest of the time I am free from asthma. Fact-This sort of thinking is the single most important reason for poor patient compliance in asthmatics. It is important to understand that asthma has episodes of acute attack during which its symptoms of breathlessness, cough and wheezing are seen. In between those attacks, the body still retains its asthmatic tendencies though they are not expressed outwardly. Therefore, to treat asthma, it is not only important to take medications during the attack, but it’s also essential to take medication regularly in between the attacks to provide effective long term control of this disease. Myth - Children tend to outgrow asthma and do not require long term medication. Fact - While it is true that a small percentage of children do outgrow asthma, latest studies have shown that this is valid in a small percentage of cases. Many children show a reduction in their symptoms around the age of 5-7 years. However, a lot of these children report recurrence by the age of 13. On the other hand, there is significant percentage of children, who experience no reduction in their suffering with age. For such children, unless active medical intervention is done, asthma becomes very debilitating. Myth - If you are suffering from asthma, exertion is strictly to be avoided. Fact - If we carefully look around us, we will notice that there have been many famous athletes with asthma – people who have proven themselves in the area of sport, never letting their asthma to stop them from becoming the best. History is testimony to the fact that many professional basketball players, Olympic medalists, marathon runners, etc. have been asthmatic. If their asthma did not stop

AYURPHARMA september-2013

them from excelling in whatever they pursued, why should yours? Myth - If you are asthmatic and pregnant, you can develop complications. Fact- On the contrary 30% women experience reduction in their asthma symptoms during pregnancy. 30% report no change and 30% get slightly more breathless. Controlled asthma has no bearing on the development of baby in the mother’s womb. However asthma is known to be hereditary. If a single parent has asthma, the child stands a 30% chance of developing it. If both parents have asthma, the risk goes up to 75%. ¾¾ Breathe easy with Homeopathy In a research conducted by professors at the University of Glasgow, Europe’s largest medical school, double-blind, placebo-controlled trials showed that over 80% patients given a homeopathic remedy showed improvement, while only 38% of patients given a placebo experienced a similar degree of relief. Isn’t it time you took your step towards getting treated with Homeopathy? Chest Physician carefully evaluates the cases and regularly tracks as well as documents the improvement. With the help of the Spirometer, the asthmatic state of each patient is very closely monitored. Thus every case is given special attention with regards to the treatment. Reduced frequency and severity of attacks, shorter duration of the attacks and overall improved resistance are some of the prime benefits of homeopathic treatment as reported by most of our patients. It must be borne in mind that the treatment period may vary from case to case and depends on certain factors such as the age of patient, duration of the illness, frequency and severity of complaints, medication taken in the past, etc. Presence of other systemic illnesses in the patient may prolong the

september-2013 AYURPHARMA

duration of treatment. An interesting aspect about the homeopathic treatment is that the drugs are not the same for all people with asthma. Case taking is done in-depth and the remedy may vary from person to person since it depends on many factors, few of which are as follows: ªª The triggers of the asthmatic attack ªª Seasonal worsening of the illness ªª Effect of moon phases (full moon, new moon, etc.) on the condition ªª Time of the day when the asthma is better or worse ªª Whether condition is better or worse from lying down, sitting, walking, stooping, etc ªª The effect of emotions, foods, etc. on the attack After taking all these points (and many more) into consideration, the physician arrives at a constitutional remedy that heals the condition from the root level without causing any suppression. In cases that are not very severe, Homeopathy can also help to reduce the need for conventional medications as well as reduce their dosage. There is reduced dependency on bronchodilators and steroid inhalers with continued homeopathic treatment. People who regularly use steroid inhalers (and large doses of the same for a long time) must watch out for side-effects of the same such as: ªª Irritability, mood swings, aggression, depression and insomnia ªª Osteoporosis ªª Stunted growth in children ªª Steroid psychosis in severe cases ªª Oral thrush or candidiasis In severe cases of asthma, the role of homeopathic medications is largely palliative. At any rate, homeopathy is strongly suggested for anyone who asthma, whatever is the stage of the illness. www.bangalorehealthtv.com

Good Reading Habits Take regular breaks while working on computers. Do not lie on the floor and read. Reading material should be placed 12-14 inches away. When reading the light source should be behind you and direct the light on the page. If reading on a desk, use a shaded light. The shade will keep light from shining directly into the eyes. Adjust you monitor or book 20-28 inches from your eyes. Place it at an arm’s length. The screen should be 20 degrees below the eye level. Place your keyboard directly in front of the monitor. Placing it at an angle puts undue stress on your eyes. Reduce glare: adjust the contrast and brightness of your computer to a level that is soothing to the eyes. Keep your reference material close by. This shall prevent eyes from frequent readjusting and tiring. Keep your spectacles clean as dust can reduce contrast and contribute to glare. First Aid During festival time like Diwali, burst crackers from a distance. Always wash hands with soap and water after bursting the crackers. Don’t burst crackers in your hand. This can lead to serious face and eye injuries. If a spark enters your eye wash immediately with water. If you have reduced vision, wear spectacles while bursting crackers. If you are injured with any sharp object do not try to remove the object and consult doctor immediately. DID YOU KNOW? You can use your eyes as much as you wish - they do not wear out.

According to Charakaacharya the herpes which is limited to skin and underlying tissues (bahya margashrita) can be treated. The herpes which affects skin, blood and lymph can be treated successfully. But the herpes which involves the internal organs (abhyantara margashrita) like brain cannot be treated. But before knowing about the treatment a brief introduction about causes, symptoms and precipitating factors for herpes, as described in ayurveda are given here. Pathology: When a person indulges in above mentioned activities the three doshas of body Vata (the element which controls nerves, neurons and related physiology), Pitta (the element which regulates and controls the body and cell metabolism. Also present in skin) and and Kapha (the element which binds the tissues and gives strength to body) get imbalanced. The imbalanced doshas vitiate or spoil ¾¾ Causes for Decreased Libido (according to charaka) ªª Blood ( rakta), ªª Lymph( lasika), ªª Skin (twacha) and ªª Muscle (mamsa) ªª Treatment(according to ayurveda) Balancing Imbalanced Doshas and Vitiated Tissues: When body is suffering from a disease the balance between three doshas will be disturbed. The first step of treatment is to balance doshas. This is achieved by administering medicines which balance the vitiated doshas of the disease .In genital herpes all three doshas get imbalanced and vitiate skin, blood and lymph. The skin, blood and lymph are purified through herbal preparations which metabolize the toxin AAMA accumulated in blood and lymph formed due to imbalanced doshas. The herbal preparations detoxify blood and lymph and expel the toxins and vitiated doshas from blood and lymph. When blood and lymph get purified the skin and underlying tissues also get rejuvenated. This reduces the inflammation, burning and tingling sensation. Clinical trials have shown that after the administration of these preparations the frequency of attacks are reduced considerably.

the precipitating factors for Genital herpes are Illness Stress Poor diet Immune disorders Friction Sunburn Physical trauma Surgery Menstruation Sex Of eleven reasons mentioned above the first five can be definitely avoided by adopting ayurveda treatment methods. ªª Illness: Falling ill frequently is mainly due to decreased immunity of body. Due to decreased immunity body can not resist the diseases. Many ayurvedic preparations which are called as Rasayanas are specially formulated by our ancient sages to boost the body immunity ªª Stress: Stress is efficiently managed through YOGA and MEDITATION ªª Poor Diet: The nutritional deficiencies due to poor diet are effectively compensated by Ayurvedic herbal Rasayanas ªª Immune Disorders: The immune disorders can be effectively controlled with Ayurveda herbal rasayana preparations ªª Friction: In genital herpes the skin of affected part of genital organ will be very sensitive .Even a little friction during intercourse cause injury to that part. The sensitivity of the affected skin to friction can be reduced by applying herbal pastes locally. These herbs work wonders on affected parts of skin by rejuvenating skin cells to resist the frictional force. A Complete Treatment : According to ayurveda treatment of genital herpes will be successful when all the above mentioned treatment methods are adopted.

Boosting Body Immunity and Rejuvenation of Body: According to ayurveda a disease not only affects a particular organ but affects the whole body. Hence the treatment is given to the entire body. After balancing doshas of the body , the body should be rejuvenated. By rejuvenation the body immunity gets boosted to fight any recurrent attacks of diseases. Many ayurveda preparations are available for body rejuvenation when body is suffering from herpes. The body immunity is boosted using Rasayanas. Rasayanas are specifically mentioned as herbal preparations which give body a strength to resist diseases. Rasayana preparations balance vitiated doshas in skin and blood. When vitiated doshas of skin are balanced the frequency of attacks reduces. The duration between the attacks are extended considerably. These preparations also boost the body immunity and also rejuvenate the body tissues by nourishing the body. Local Treatment for Affected Skin Parts: Along with the process of balancing doshas and rejuvenation of body, the treatment to the particular organ which is mainly affected by the disease should be treated. This is called local treatment (sthanika chikitsa). In genital herpes this is achieved through local application of herbal paste, washing the affected area with herbal decoction etc. Rejuvenation of the affected local skin is achieved through local application of herbal pastes. These preparations rejuvenate the skin and restores the structure -color of skin. These preparations also reduce local inflammation of the skin and skin gets a very good resistance to subsequent attacks. The tenderness of the skin vanishes and skin discoloration fades to get back the natural color. Diet and Lifestyle Modifications:These Help to rejuvenate body and mind there by increasing resistance to attacks. These modifications also reduce frequency of attacks by controlling precipitating factors like mental stress and emotions. Psychiatric disorders are on the rise due to enormous distress caused by present day’s competitive, insecure and demanding life. These disorders appear in the form of anxiety, depression, fear, panic attacks, schizophrenia as well as headache, body pain, weakness and poor work efficiency. Persons of all ages including children get affected by such problems. Children exhibit unruly behavior, lack of concentration, poor study performance, eating disorder and hyperactivity. ¾¾ Factors causing psychiatric disorders are: ªª Genetic – factor which has been passed down by ancestors. ªª Regular or over use of alcohol and recreational drugs. ªª Stressful work atmosphere – overwork without recreation, lack of appreciation and incentive for good work. ªª Emotional stress due to strained relationship with spouse, parents, children or near relative bereavement due to loss of love object and failure to achieve success. ªª Physical illness and accident. ªª Irresponsible and reckless life style. These factors many of which are known as stressors effect adversely on the nerve cells and alter the neurochemistry of central nervous system. The fine balance of neurochemicals acting through various pathways in brain gets altered and produce psychiatric disorder. One or more of these factors lead to nervous

breakdown. Children suffer mostly when their parents vitiate home atmosphere by frequent quarrels with each other, at times leading to their separation. When an individual gets mentally disturbed, it gets reflected in his behavior which becomes disorganized. He does not feel at peace with himself as well as with others. He becomes a burden for others. His vital functions like sleep, eating habit, emotional response, sexual activity and work efficiency get affected and tell upon his performance. His coping mechanism and stress adaptability become weak. As a result he feels east down and contemplates suicide. Most suicides are the result of such disturbance. Psychiatric diseases are like any other diseases as diabetics and blood pressure. Where disturbance in different specific set of body chemicals cause them. Therefore psychiatric illness need treatment as any other illness. Poor awareness prevents to seen for proper psychiatric help. People suffer silently using mechanism of denial to project that every thing is well with them though it is on the contrary. Moreover psychiatric illness is seen as a taboo in society. The fear of being labeled as a psycho, crack or mental inhibits motivation for seeking proper psychiatric assistance. Hence it shall be wise to understand these illnesses as any other illness shun inhibition and seek for proper professional help which shall not only improve quality of life but also bring about better functioning in day to day responsibility. ----- To be continued about common psychiatric illness.

Menopause

Dr. Grace G & G Multi-specialty Clinic, No.29, 1st floor, 1st cross, 3rd main, Ramamurthy Nagar, Bangalore-16. Mob: 8951544239 ¾¾

What is menopause? Menopause is the ending of a woman’s monthly menstrual periods. Periods stop and it’s no longer possible to get pregnant. Menopause may occur from 35 to 60. But most women stop menstruating in their midforties to early fifties. ¾¾ What happens during menopause? Ovaries -the body’s main producers of female sex hormones (oestrogen progesterone) gradually stop working, and the hormone levels drop dramatically. This process begins 3 to 5 years before the final menstrual period. ¾¾ How will I know I have menopause? Your periods become irregular, or longer or shorter. Menstrual flow lessens, but often a light period is followed by a heavy one, or “flooding”. Sometimes, there is a gap of months between periods. You may think it is stopped—then be surprised by another. The reduction in the amount of female hormones in the body can have wide

ranging effects — from short-term symptoms such as hot flushes, night sweats, and vaginal dryness, to an increased risk of osteoporosis (brittle bones) and heart diseases. ¾¾ Does every woman undergo these problems? Menopause is an individualised experience. Some women notice little difference in their bodies or moods, while others find the change extremely bothersome and disruptive. Few women simply stop having periods without any trouble, but this is rare. ¾¾ What are hot flushes? Hot flushes are uncomfortable waves of heat, which are the body’s response to declining hormone (oestroien) levels. Hot flushes last only for a few months in some women, may persist for years or never occur at all for others. You may get just one hot flush a day, or several an hour. During a hot flush, which typically lasts from 1 to 5 minutes, the heart beats faster, and blood vessels dilate causing a flush, a feeling of heat. Women

may sweat or suffer a wave of anxiety. Wearing loose clothes and avoiding synthetic fabrics, which trap perspiration, may help you feel more comfortable. ªª Keep your rooms well-ventilated, especially your bedroom at night. ¾¾ Does menopause affect a woman’s sex life? There is no reason for your sex life to decline during menopause. Declining hormone levels may cause your vagina to become dry, making intercourse painful, and sex uncomfortable. Generally, the more sexually active you are, the fewer problems you are likely to encounter. ¾¾ Can menopause bring any emotional problems? Women can become irritable when hot flushes rob them of a good night’s sleep, or when they feel they are getting older. Fluctuating hormonal levels affect the part of the brain that controls moods and feelings. Some signs of depression observed during this period include insomnia or sleeping too much, loss of appetite or eating too much, and feeling helpless, hopeless, or worthless. ªª Relaxation exercises like yoga and meditation help cope with mood swings and feelings.

Bedsores (pressure sores)

G & G Multi-specialty Clinic, No.29, 1st floor, 1st cross, 3rd main, Ramamurthy Nagar, Bangalore-16. Mob: 8951544239

Bedsores, more accurately called pressure sores or pressure ulcers, are areas of damaged skin and tissue that develop when sustained pressure — usually from a bed or wheelchair — cuts off circulation to vulnerable parts of your body, especially the skin on your buttocks, hips and heels. Without adequate blood flow, the affected tissue dies. Although people living with paralysis are especially at risk, anyone who is bedridden, uses a wheelchair or is unable to change positions without help can develop bedsores. Bedsores can develop quickly, progress rapidly and are often difficult to heal. Yet health experts say many of these wounds don’t have to occur. Key preventive measures can maintain the skin’s integrity and encourage healing of bedsores. ¾¾ When to seek medical advice Carefully inspect your skin at least once a day, paying particular attention to your spine, tailbone and other bony places. Because the early signs of skin damage aren’t always clear, it’s best to also feel your skin, noting any spots that are unusually firm, spongy or warm to the touch. If you can’t look for bedsores yourself, a family member, nurse or caregiver should do it for you. If an area of your skin is red or discolored but not broken, keep pressure off the sore, wash it gently with mild soap and water, dry thoroughly, and apply a protective wound dressing. Contact your doctor right away if you notice any broken skin or open sores. Get immediate medical care if you have signs of infection: fever, drainage from the sore, a foul odor, or increased heat and redness in the surrounding skin. ¾¾ Tests and diagnosis Bedsores are usually unmistakable, even in the initial stages, but your doctor is likely to order blood tests to check your nutritional status and overall health. Depending on the circumstances, you may have other tests, including: ÂÂ Urine analysis and culture. ÂÂ Stool culture. ÂÂ Biopsy. ¾¾ Treatments and drugs Treating bedsores is challenging. Open wounds are slow to close, and because skin and other tissues have already been damaged or destroyed, healing is never perfect. Addressing the many aspects of wound care, including those that are emotional and social, requires a multidisciplinary approach. You’re likely to receive care from nurses and your primary care physician, along with help from a social worker and physical therapist. When incontinence is an issue, you may see a urologist or gastroenterologist. And if a wound requires surgical repair, a neurosurgeon, orthopedic surgeon and plastic surgeon may be involved in your care. ¾¾ Conservative treatment Although it may take some time, most stage I and stage II sores will heal with conservative measures. But stage III and stage IV wounds, which are less likely to

resolve on their own, may require surgery. The first step in treating a sore at any stage is relieving the pressure that caused it. You can reduce pressure by: ¾¾ Changing positions often. Carefully follow your schedule for turning and repositioning — approximately every 15 minutes if you’re in a wheelchair and at least once every two hours when you’re in bed. If you’re unable to change position on your own, a family member or other caregiver must be able to help. Using sheepskin or other padding over the wound can help prevent friction when you move. ¾¾ Using support surfaces. These are special cushions, pads, mattresses and beds that relieve pressure on an existing sore and help protect vulnerable areas from further breakdown. The most effective support depends on many factors, including your level of mobility, your body build and the severity of your wound. No one support surface is appropriate for all people or all situations. In general, protective padding such as sheepskin isn’t thick enough to reduce pressure, but it’s helpful for separating parts of your body and preventing friction damage. ¾¾ Other nonsurgical treatments of pressure sores include: ªª Cleaning. It’s essential to keep wounds clean to prevent infection. A stage I wound can be gently washed with water and mild soap, but open sores should be cleaned with a saltwater (saline) solution each time the dressing is changed. Saline solution is available at the drugstore, or you can make it at home by boiling 1 teaspoon of salt in 1 quart of water for five minutes. Store the solution in a sterile container and cool before using. Avoid antiseptics such as hydrogen peroxide and iodine, which can damage sensitive tissue and delay healing. Controlling incontinence as far as possible is crucial to helping sores heal. If you’re experiencing bladder or bowel problems, you may be helped by lifestyle changes, behavioral programs, incontinence pads or medications. ªª Removal of damaged tissue (debridement). ªª Dressings. ªª Hydrotherapy. ªª Healthy diet. ªª Muscle spasm relief. ¾¾ Surgical repair: Even with the best medical care, bedsores can quickly reach a point where they require surgical intervention. The goals of surgery include improving the hygiene and appearance of the sore, preventing or treating infection, reducing fluid loss through the wound, and lowering the risk of future cancer. ¾¾ Other treatment options: Researchers are searching for more effective bedsore treatments. Under investigation are hyperbaric oxygen, electrotherapy

and the topical use of human growth factors. Growth factors — proteins that stimulate cell growth — have been approved for the treatment of diabetic ulcers but not, as yet, for pressure sores. ¾¾ Prevention Bedsores are easier to prevent than to treat, but that doesn’t mean the process is easy or uncomplicated. Still, although wounds can develop in spite of the most scrupulous care, it’s possible to prevent them in many cases. The first step is to work with your nurses and doctor to develop a plan that you and your caregiver(s) can follow. The cornerstones of such a plan include position changes along with supportive devices, daily skin inspections and a maximally nutritious diet. ¾¾ Position changes Changing your position frequently and consistently is crucial to preventing bedsores. It takes just a few hours of immobility in a wheelchair or bed for a pressure sore to begin to form. For that reason, experts advise shifting position every 15 to 30 minutes that you’re in a wheelchair and at least once every two hours, even during the night, if you spend most of your time in bed. If you can’t move on your own, a family member or caregiver must be available to help you. A physical therapist can advise you on the best ways to position yourself in bed, but here are some general guidelines: ªª Avoid lying directly on your hipbones. ªª Support your legs correctly. ªª Keep your knees and ankles from touching. ªª Avoid raising the head of the bed more than 30 degrees. ªª Use a pressure-reducing mattress or bed. ¾¾ Skin inspection Daily skin inspections for pressure sores are an integral part of prevention. Inspect your skin thoroughly at least once a day, using a mirror if necessary. Some experts suggest examining your skin twice a day or with every repositioning. A family member or caregiver can

help if you’re not able to do it yourself. If you’re confined to bed, pay special attention to your hips, spine and lower back, shoulder blades, elbows and heels. When you’re in a wheelchair, look especially for sores on your buttocks and tailbone, lower back, legs, heels and feet. If you see skin damage or any sign of infection such as drainage from a sore, a foul odor, and increased tenderness, redness and warmth in the surrounding skin, get medical help immediately. Nutrition A healthy diet is important in preventing skin breakdown and in aiding wound healing. Unfortunately, the people most likely to develop pressure sores are also often the most malnourished. If you’re ill, recovering from surgery or living with paralysis, you may have little appetite and eating may be physically difficult. These measures also may help: ªª Try smaller meals. ªª Take advantage of the times when you feel your best. ªª Limit fluids during meals. ªª Consider pureed or liquid meals. ªª Consider protein alternatives. ªª Find a comfortable position. ªª Don’t rush. ¾¾ Lifestyle changes Although you may need assistance with many aspects of your care, you can take control of some important preventive measures, including: ªª Quitting smoking. ªª Exercise. ªª Support. ¾¾ For the family If you have a loved one in a nursing home, hospital or other care facility, check that person’s skin condition, weight and general care every time you visit. If you notice any sign of pressure sores or neglect, alert the nursing staff and attending physician or nursing home director immediately. You and other family members are in the best position to monitor and assess a loved one’s condition.

Self motivate and believe your beliefs Have faith in your ability, determination, willpower and to march ahead till you achieve your goal. Motivation is a powerful and complex force that causes you to act in the right direction with the external results and fruits in mind. Motives are desires that originate deep inside you and incentives from it propel you into action. Sang H Kirn famous motivator in US has summarized some key ideas that are essential to understanding motivation; ªª Motivation is a force, positive or negative, that causes us to act ªª Understanding the underlying motive behind the action is the key to motivating anyone, including yourself. ªª Every motive we have for action arises from a need and out of desire to satisfy it, ªª Motives come in many forms and change throughout our lives. ªª Motives can change during an activity. ªª To create a strong incentive, your reward for achieving a behaviour must be directly tied to the motive for action. ªª Motivation is often the result of social conditioning and experience. ªª Motivation is the act of encouraging someone to do something they need to do, but may not be able to initiate on their own. Manipulation is the act of deceiving someone into doing something they might not otherwise do. ¾¾ Some more guidelines to motivate yourself: ªª Fix your goal but the road map i.e., the guidelines must be flexible. ªª Always set a deadline to finish any task. ªª Follow the principle of ‘Divide and Conquer’. Don’t look into the goal as one big one, but divide

it into steps and accomplish it step by step. Motivation to succeed is a more powerful tool than your knowledge or training. World famous motivational writer Napoleon Hill said “The great master key is nothing more or less than the self discipline necessary to help you take full possession of your own mind. Remember, it is profoundly significant that the only thing over which you have complete control is your own mental attitude”. You should have some fixed plan, if you don’t know where to go, how will you reach your destination. According to Walt Disney of Disneyland fame “If you can dream it. You can do it”. Don’t try to live upto other’s expectations but live upto your expectations. If you don’t get the opportunities, just create them. Remember: knowing and doing are as different as sleeping and acting. Think that there is no obstacle you can’t overcome. Only intention is not enough, your action counts much more. Focus on what you actually want and you will become so, focus on being a motivated person and that is what you will become. Identify the most common de-motivators: uncertainty, fear of failure, anger, ficklemindedness, lack of enthusiasm and bad in decision making. Steve Chandler said “Only challenge causes

growth. Only challenge will test our skills and make us better. Only challenge and the self motivation to face the challenge will transform us. And every challenge we face creates a more skillful self. ¾¾ Believe your beliefs Whatever you trust and believe about yourself from the soul or the heart becomes your reality. The only measure of whether you sincerely overcome your difficulties is how firmly you believe in yourself. Accept yourself to be a valuable and worthwhile person in all respects. When you absolutely believe in yourself and your ability to succeed, nothing can prevent you from accomplishing your cherished wish. As Swami Vivekananda said, “Stand up, be bold and strong. Have faith in yourself. Know that you are the creator of your own destiny. All the strength and succor you need is within you. All powers are within you; you can do anything and everything. The most important thing for you is to believe in that. We are the children of the Almighty; we are sparks of the infinite divine fire. How can we be nothing? The idea of faith in ourselves is of the greatest help to us. Throughout the history of mankind, great men and women have had their faith in themselves”. The man who has practiced control over himself cannot be acted upon by anything from outside us, there is no more slavery for him. Belief in ourself is the greatest source of strength. To accomplish great things, we must not only act but also dream, not only plan but also believe. Abraham Lincoln said, “Let us have faith that, right makes might and in the faith let us, to the end, dare to do our duty as we understand it”. Do not attempt to do a thing unless you are sure of yourself, but do not relinquish it simply because someone else is not sure of you. Have faith in the God above us, faith in the little, faith in the infinite soul within us, faith in life and in our fellow soul. Without faith, the plus quality, we cannot really live. Mahatma Gandhi said, “Faith is not a delicate flower which would wither under the slightest stormy weather. Faith is like the Himalayan Mountains, which cannot possibly change. No storm can possibly remove the Himalayan mountains from their foundation”. Faith will turn any course, light any path, relieve any distress, bring joy

out of sorrows, peace out of strife, friendship out of enemity, heaven out of hell. That man alone acquires strength of body and soul and attains happiness whose heart is free from suspicion and is filled with faith. Faith is the only key to the Universe. The final meaning of human existence and the answers to the questions on which all our happiness depends cannot be found in any other way. As per Sir William Osier, “Nothing in life is more wonderful than faith - the one great moving force which we can neither weigh in the balance nor test in the crucible”. William Shakespeare said “There are no tricks in plain and simple faith”. It’s amazing what you can achieve if you believe you can achieve it. Those who do not have any hope to achieve has already lost the race. Try to understand yourself - it helps to know your attitudes and to assess your strengths and recognize your weakness. As per Charles Dickens, “Youth is the period of building up of habits, hopes and faiths”. To accomplish great things you must not only act but also dream, not only plan but also believe. To think a thing impossible is to make it so. Courage is victory, timidity is defeat. All things are possible to him that believeth. Never give up. It is wise and always better to hope than despair. Let faith and hope be the steering and gear of your vehicle of success. To be failure proof, you must possess an unconquerable faith, wholly consistent towards the goal, a will surrounded to a cause greater than yourself. When you imagine yourself accomplish a goal in your dream, you begin to believe that you can accomplish it practically in real life too. Believe and it can be done. Believing in a solution paves the way to solution. Trust the sun; don’t trust the shadows. Believe in your dreams; don’t believe in thoughts of despair. Have faith in your faith; yet doubt your doubts. In fact, it is through your beliefs that you create the very world you live in. Beliefs are latent, but powerful forces buried deep in the confines of your mind. Faith works like a magic. It looks beyond all limitations, crosses all boundaries, crosses any hurdle and helps you to reach your goal. So have faith in your ability, determination, willpower and to march ahead till you achieve your goal.

Live life the eco-friendly way and benefit yourself by helping the nature: Use energy-saving lights and dimmers and reduce electricity bills. Carry your own shopping bags and always use jute or fabric bags. Buy the daily groceries in bulk (but not to waste) to cut down on the packaging and is economical too. Use herbal toiletteries and medications rather than the synthetic chemicals sold. Maintain a garden in the terrace or in the balcony. Use some indoor plants to clean up the air. The myth of the holy basil is worthwhile as it purifies the air. Plant a tree to commemorate a landmark event or a celebration in your life and keep track of its age with pride as a memoir. Use rechargeable batteries as the alkaline ones emit harmful chemicals. Keeps the machines and electrical appliances in good working order and wellserviced for efficient usage. Avoid using chlorine in the swimming pools. Instead use a UV system to kill bacteria and filter yhe water. Use solar batteries in street and garden lights. Transform your room to look higher by experimenting with vertical hued stripes and

wider by experimenting with horizontal hued stripes. It also brightens up the atmosphere. Float some flowers and scented candles in a medium sized bowl or /OML HI’S vessel for giving that aesthetic touch and freshen up your room. Sparkle tiles overnight by rubbing them with bleaching powder and water and then rinsing in the morning. Always start dusting from top to bottom so those things that have been already dusted need not be done again. While washing or cleaning walls take the bottom route upwards as it is easy to clean the drips from washed walls. A dry cotton towel sprayed with the cleaning formulation does better than paper towels or newspapers in cleaning glass surfaces. While washing windows , wiping horizontally on the inside and vertically on the outside helps us know which direction the marks are. While cleaning go from edges to the centre to avoid the dust from spreading again. A weak ammonia solution with a Vz teaspoon of borax can be used for removing tough stains. Sprinkling a little boric acid on bathroom tiles before washing gives sparkling results. Hard water stains from bathroom fixtures can be removed by applying a paste of vinegar and baking soda. Drape them with a soft towel and leave on for an hour. Then rinse off and dry properly.

Polycystic Ovary Syndrome (PCOS) Polycystic ovary syndrome (PCOS) is common condition which consists of irregular or absent periods, excessive body hair, obesity and slightly enlarged ovaries.

Gynecologic problems

Understanding ovaries and ovulation The ovaries are a pair of glands that lie on either side of the uterus. Each ovary is about the size of a large marble. The ovaries make eggs and various hormones. Ovulation (release of egg) normally occurs once a month. Before an ovum is released, it develops in a little swelling of the ovary called a follicle. Each month several follicles start to develop, but normally just one fully develops and goes on to ovulate. The main hormones that are made in the ovaries are estrogen and progestogen - the main ‘female hormones’, and are the main controllers of the menstrual cycle. The ovaries also normally make small amounts of ‘male1 hormones (androgens) such as testosterone. PCOS is a syndrome where at least two of the following occur, and often all three: At least 8-12 follicles (tiny cysts) develop in your ovaries. (Polycystic means ‘many cysts’) You do not ovulate each month . Some women with PCOS do not ovulate at all.

(In PCOS, although the ovaries usually have many follicles they do not devevlop fully and so ovulation often does not occur). If you do not ovulate then you do not have a period. The balance of hormones that you make in the ovaries is altered. In particular, your ovaries make more testosterone (male hormone)than normal. Why do I develop PCOS? The exact cause is not clear. Several factors probably play a part. These include the following. Insulin: A raised level of in the bloodstream is thought to be the main underlying reason why PCOS develops. Insulin is a hormone that you make in your pancreas. The main role of insulin is to control your blood sugar level. Insulin acts mainly on fat and muscle cells which take in sugar (glucose) when your blood sugar level rises. Women with PCOS have ‘insulin resistance’ .This means that cells in the body are resistant to the effect of a normal level of insulin. Therefore, the level of insulin in the blood needs to be higher than normal to control the blood sugar level. Luteinising / hormone (LH): This hormone is also probably involved in causing PCOS. This hormone is secreted from pituitary gland. LH stimulates the ovaries to ovulate. It is assumed that a high level of insulin and a high level of LH work together to cause more testosterone than normal to be made in the ovaries. Hereditary factors: Your genetic makeup is probably important. One or more genes may make you more prone to develop PCOS. PCOS is not strictly inherited from parents to children, but it may run in some families Weight: Being overweight or obese is not underlying cause of PCOS. However, if you are overweight or obese it may make things worse, excess fat can make insulin resistance worse, which may than cause the level of insulin to rise even further. It also causes changes in female hormonal level so that you do not ovulate. However, PCOS may also be a contributing cause of overweight and obesity. What would one experience with PCOS? ªª Period problems -You may have irregular or light periods, or no periods at all. ªª Fertility problems- You may not ovulate each month, and some women with PCOS do not ovulate at all. PCOS is one of the commonest causes of infertility. PCOS women who do manage to become pregnant tend to have increased frequency of miscarriage. ªª Hair growth (Hirsutism) occurs in more than half of women with PCOS. It is mainly on the face, lower abdomen and chest. This is the only symptom in some cases. ªª Acne may persist beyond the normal teenage years. ªª Thinning of scalp hair: occurs in some patients. ªª Weight gain - women with PCOS are overweight or obese. ªª Depression or poor self esteem may develop as a result of the other symptoms. Symptoms typically begin in the late teens or early 20s. Not all symptoms occur in all women with PCOS. Symptoms can vary

40

www.bangalorehealthtv.com

Possible long-term problem of PCOS: If you have PCOS, over time you have an increased risk of developing diabetes, a high cholesterol level, and possibly high blood pressure. These problems in turn may also increase your risk of having a stroke and heart disease in later life, these increased health risks are due to the long-term insulin resistance (and also being overweight which is common in women with PCOS). PCOS are at higher risk of developing endometrial cancer. Tests needed: Tests may be advised to clarify the diagnosis, and to rule out other hormone conditions. Blood tests may be taken to measure certain hormones - FSH and LH (High LH) an ultrasound scan of the ovaries may be advised. This can detect the typical appearance of PCOS with the many follicles (small cysts) in slightly enlarged ovaries with increased stromal thickness. Serum insulin (high) and oral glucose tolerance test (altered) Treatment for PCOS ? There is no cure for PCOS. Treatment is basically tailored to the symptoms of the patient, You should aim to lose weight if you are overweight: Losing weight helps to reduce the high insulin level that occurs in PCOS. This then improves the chance of you ovulating, which improves any period problems, fertility and may also help to reduce hair growth and acne. The increased risk of long-term problems such as diabetes, high blood pressure, etc., is also reduced. Losing weight can be difficult. A combination of eating less and exercising more is best. Advice from a dietician and physiatrist may increase your chance of losing weight; we have a physiatrist next door for the same. Even a moderate amount of weight loss can help. Treating hair growth: Drugs to treat hair growth take 3-9 month to work fully and for their effect to be noticed. You need then to carry on taking them otherwise hair growth will recur. Things like waxing, shaving, electrolysis and laser might also help. Treating acne: The treatments used for acne in women with PCOS are no different to the usual treatments for acne. Treating period problem: Women with PCOS are advised to take the contraceptive pill as it causes regular ‘withdrawal bleeds’ similar to periods. If this is not suitable, another option is to take progestogen hormone for several days every month which will cause a monthly bleed like a period. ‘-• Fertility issues: The chance of becoming pregnant depends on how often you ovulate. Some women with PCOS ovulate now and then, others not at all. If you do not ovulate but want to become pregnant, then fertility treatments have a high chance of success. You are much less likely to become pregnant if you are obese. If you are obese or overweight then losing weight is mandatory in addition to other fertility treatments. Fertility chances can be increased by doing laparoscopic ovarian drilling, which is a day care procedure. Preventing long term problems: A healthy lifestyle is important to help prevent the ‘long-term problems’. Eat a healthy diet, exercise regularly and lose weight if you are overweight or obese.
