A Letter from a Parent at St. Joseph Institute for the Deaf We thought we were prepared when we adopted our daughter, however we didn’t quite see her profound bilateral sensorineural hearing loss diagnosis coming. We didn’t know anyone who was deaf/Deaf. Should we stick to ASL or only focus on spoken language? Do both? Cochlear implants? What brand? No doubt this was a confusing time. Fast forward to today, almost two years after our daughter’s cochlear implant surgery – Knowing we wanted our daughter to attend public school in the future, we knew we had a big language gap to close – which is where St. Joseph Institute for the Deaf has stepped in. Our daughter receives daily speech therapy and is in a small classroom setting that enables her to further develop those skills. Her speech therapist is constantly researching different techniques to help our daughter develop speech, and she attends her mapping appointments with us to ensure she has full access to sounds. We are so grateful to the team at St. Joseph Institute for seeing the potential in our daughter and for coming alongside us. At only a “listening age” of not even two years old, our daughter, who entered St. Joseph with no spoken language, now sings Twinkle Twinkle Little Star and she tells us the Goldilocks and the Three Bears story. We’re amazed at where she has been, and are so very excited for where she’ll be able to go in the future. 2

Assertiveness Teaches Clear Communication. Kids can be put in situations where they are emotionally stifled, intimidated or bullied. It can happen at their school, their neighborhood or in their homes. These kids may not be able to assert themselves because it is not safe to share their feelings but they need to learn assertiveness so they can share their feelings with people who are safe and who can validate their experiences with them. I was recently working with 2 kids who are going back and forth from mom and dad's house and unfortunately, they are witnessing a lot of abuse at dad's house. The abuse involves watching their dad fight with their stepmother both verbally and physically. It also spills over to their relationship with dad because if they don't agree with him, he verbally attacks them and intimidates them into agreeing with him. Although the situation has been investigated it has not resulted in any actions that would improve the situation and reduce the put downs. The children complain that when they

share their feelings with their father he teases and belittles them. As a counselor, what I know to be true is that when kids are in psychologically unhealthy situations it may not be safe to express themselves Carol Juergensen Sheets because it will increase the psychological harassment. Because these kids still need the skills to share their thoughts, feelings and beliefs, we spend time talking about how important assertiveness is in daily functioning. Even if they can't do this at home, we talk about the need for assertiveness with other people who want to know how the boys feel. In our session, I have taught them to use an assertiveness statement that describes the behavior, their feelings, and the message it conveys.

When kids use assertiveness, it does not ensure that they will get what they want but it does clarify how one feels and what one would like. The assertiveness statement goes like this: (Name)_____________,when you _____________(Describe the behavior, I feel ________________(the feeling) because ____________________________. A child who can use assertiveness with a healthy person may say:

never done anything to make you not trust me.

Mom, when you ask me to go my brother’s soccer game and to do my homework at the same time, I feel frustrated because it is hard to concentrate when I am at a game.

Mrs. Silvers, when you want me to tell you who was talking, I feel scared, because I don't want the other kids to be mad at me.

Dad, when you don't trust me to hang out with my friends, I feel angry because I have

Tommy, when you tease me about my hair, I feel upset, because it makes me feel ugly and stupid.

Diagnosing ASD Diagnosing autism spectrum disorder (ASD) can be difficult, as there is no medical test – like a blood test – to diagnose the disorders. Clinicians assess a child’s behavior and development to make a diagnosis. Autism can be diagnosed at a very young age; however, many children do not receive a final diagnosis until much older. This delay means that children with ASD might not get the help they need. Diagnostic evaluations of ASD often include two steps: developmental screening and comprehensive diagnostic evaluation.

Developmental Screening A developmental screening is a brief test to tell if children are learning basic skills when they should, or if they might have delays. During a developmental screening the family doctor might ask the parent some questions or talk and play with the child during an exam to see how he/ she listens, speaks, behaves, and moves. A delay in any of these areas could be a sign of a problem. It is recommended that all children should be screened for developmental delays and disabilities during regular well-child doctor visits at 9 months, 18 months, and 24 or 30 months. Additional screening might be needed if a child is at high risk for developmental problems due to preterm birth, low birth weight or other reasons. Most children are also screened specifically for ASD during regular well-child doctor visits at 18 and 24 months. Additional screening might be needed if a child is at high risk for ASD (e.g., having a sister, brother or other family member with an ASD) or if behaviors sometimes associated with ASD are present

It is important for doctors and pediatricians to screen all children for developmental delays, but especially to monitor those who are at a higher risk for developmental problems due to preterm birth, low birth weight, or having a brother or sister with an ASD. If the doctor sees any signs of a problem, a comprehensive diagnostic evaluation should be recommended.

Comprehensive Diagnostic Evaluation The second step of diagnosis is a comprehensive evaluation. This more thorough review includes observing the child’s behavior and development as well as interviewing the parents. It may also include a hearing and vision screening, genetic testing, neurological testing, and other medical testing to rule out other factors. In some cases, the primary care doctor might choose to refer the child and family to a specialist for further assessment and diagnosis. There are multiple evaluation tools practitioners can use to diagnose autism, including the ADOS-2, Vineland-3, M-CHAT, CARS, and GARS. The ultimate goal is to make a determination of diagnosis and move forward with treatment as quickly as possible.
