I have 20 plus years of experience in personal injury litigation and trial work. I represent adults and children who have suffered serious injuries caused by car, truck, motorcycle, and bicycle collisions, stair and railing collapse, explosions, slip and falls, and other dangerous conditions caused by the negligence of someone else. I have represented individuals suffering from serious injuries to the brain and body including but not limited to brain injuries, neck and spinal injuries, eye injuries, burn

The HCG Story... The story of HCG began in the Fifties when, after having done research on weight loss for 40 years and research on HCG specifically for twenty years, Dr. Simeons reported that patients taking small amounts of HCG, in conjunction Kathryn Azhar, Owner with a very low calorie and her staff diet, could lose weight safely and quickly without exercise, muscle loss or hunger. But all HCG is not created equal. The SuperVision Weight Loss program starts with pharmaceutical HCG. Unfortunately, there has been a lot of confusion created by products being sold without guidance that call themselves “HCG” but do not contain any in their ingredient list. HCG is one of 67 hormones naturally produced in the human body, so if a product states that it is hormone free, it's not HCG.

Cindy's Story

For years, I've had trouble losing the excess weight that I put on. I've tried several plans and fad diets, only to put the weight back on. I came to Super Vision Weight Loss when my doctor told me that I was pre-diabetic. I found the program easy to follow. i was glad to have the support of the staff through the entire transition. I am so happy with my results and my new lease on life.

Gary's Story

My doctor was on my case about my blood pressure, and I know that the extra weight I was carrying wasn’t helping that situation. My knees hurt and I just wanted to feel better. I gave Super Vision Weight Loss a chance to help me get the weight off. They were really great!! I lost 38 lbs. in 31 days! My girlfriend is thrilled. I travel for a living and I was able to stay true to the diet protocol even on the road. I’m so glad I did this diet with Super Vision Weight Loss. I feel YEARS younger and I know I’ll never have that belly again.

HCG does not produce weight loss by itself. However, in concert with a very specific balanced diet, and our help in avoiding unexpected environmental or behavioral deviations, HCG will allow the body to naturally pull the nutrients and calories needed from stored fat reserves into the blood stream at the rate of 1500 to 4000 calories per day. We are sure that those who follow our protocol WILL lose 20-30 pounds in 40 days. These fantastic results ARE typical with our full service packages. SuperVision Weight Loss program is not your run of the mill “eat less and exercise more” diet for a slow loss of 1 to 2 pounds a week that seems to take the rest of your life. Our protocol tricks Mother Nature a little, maybe, however only natural processes are at work. Per Dr. Simeons, it is safe and so very effective.

What Word Would Change Your Life? It's that time of year again when you do a self-assessment and look at the things that you would like to change. Most people look at January as an opportunity for a fresh start. Now you have to be 100% ready and committed to evoke change. If you aren't committed to a goal it will never materialize. I promote the New Year as a great time to add a "word" or "concept" to your life that will alter your behavior. Words can dramatically shape your awareness and attitude and thereby change your reality. Instead of lamenting that you want to lose weight, get up earlier, exercise more, give up sugar, or any of the other common January resolutions, consider this: Pick a word that would alter the way you live if implemented into your life. The process affects people on multiple levels. I ask my clients to self-reflect to determine their needs. Then they are to pick a word that represents or symbolizes the change they would like in their life. This requires thinking about what behavior one might want to eliminate or add to promote better mental health. For instance, a workaholic father picked the word balance because he wanted to have more family time. As he increased his time at home to lead more of a balanced lifestyle, he realized how much more freedom he felt to pursue other activities like church, neighborhood progressive dinners, and monthly date nights with his wife. A perfectionistic mother who also worked outside the home chose acceptance as her word. She said she was going to work on accepting her imperfections as opposed to beating herself up for them. The result was that she was able to calm the anxiety inside her because she no longer felt driven to do it all.

An overweight client who lost 70 lbs. identified moderation as her meaningful word. After she picked the word, she felt inspired to eat Carol Juergensen Sheets differently for the first time in her life. Instead of eliminating food choices she cut them in half, which resulted in a consistent weight loss. Her nighttime routine was to come home by 6:30 and watch TV until bedtime. She used the word moderation to cut back on her TV time and added beading to her evening routine. Her jewelry-making substituted for TV and eating. This process was simple and yet changed her life! Sometimes the word reflects a mental shift. My word for 2016 is gratitude. I am picking the word because I know that I have the choice to embrace all that has been given to me or to focus on what is lacking in my life. I truly believe that by focusing on gratitude I will be more content, I will appreciate the present, and I will be more fulfilled as a result. Now I challenge you to give up the typical New Year's resolution formula and pick a word to integrate into 2016. You'll know when you have the right word, because it will resonate inside you and it will reflect your need for self-improvement. Write a paragraph on how you will use the word to alter your day, your routine and your life. Then place Post-It notes in several places to visually remind you of your choice. It's a very simple formula, yet it's incredibly powerful. It motivates you both consciously and unconsciously and it can enhance your life forever.

Happy New Year!

Motivation for Weight Loss in the New Year by Despite good health and a reasonable level of fitness, our bodies can develop disproportionate contours or stubborn localized fat deposits. These problem areas can be frustrating and demotivate us from sticking to our fitness regimen. While there is no substitute for a healthy diet and consistent exercise, treatments exist that compliment and enhance our existing programs. New non-surgical therapies and technologies using ultrasound, bio-stimulation, and radio frequency shrink fat cells, smooth out the appearance of cellulite, and tighten skin. These treatments combined with regular exercise routines can act as motivation to stick with weight loss and fitness goals. For the last 20-years I have researched and provided clinical testing for many medical laser and ultrasound technology companies. This experience helps provide insight into treatment effectiveness. I’ve often heard from my patients that they are overwhelmed by the many choices that exist. To assist, I have complied some of the common questions asked.

with a noticeable reduction. The ultrasound energy penetrates and the tissue to the optimal temperature for neocollagensis. This can improve the tone and quality of the tissue. • Bio-Stimulation uses high intensity pulses to activate muscles to burn up fat. It’s the same mechanism as several hours of intense exercise. • Lymphatic Stimulation uses precise heating, suction, and specifically designed rollers to smooth out the skin to facilitate safe and efficient heat energy delivery. The procedure increases the metabolism of stored fat, and promotes lymphatic drainage. • Fat Dissolving Injections use enzymes and proteins to open the fat cell membrane and release stored fat.

Q&A

What options exist for non-surgical fat and cellulite reduction? • Laser therapy uses light based energy to reduce the size of fat cells and fat chambers. • Monopolar & Bipolar Radio Frequency use (RF) waves that pass through the top layers of the skin, into the lower dermal collagen layer to tighten the skin, and into the subcutaneous fatty layer to reduce cellulite. • Ultrasound is a new technique that effectively targets stored fat cells. This allows for optimal penetration such that the fat cell walls can be disrupted, causing shrinkage and damage to the fat, 8

What treatments are most effective? Patients obtain maximum benefit when the treatment is matched with their specific concern and the patient follows the treatment plan. For example, some nonsurgical treatments require consecutive sessions performed weekly. If a patient does not believe he or she can follow this plan, I will suggest an alternative option.

How do you determine what option is best for each patient? As a result of researching many systems, I have been able to see first hand what works best for certain types of issues. For example, we have found that cellulite is best treated with bipolar radio frequency combined with lymphatic stimulation and drainage.

What causes cellulite? The skin's connective tissue surrounds fat cells, which are arranged in chambers. The margins of these chambers contain septae. Over time, the fat cells increase in size and number. This phenomenon together with accumulation of localized edema; push the skin's surface between the septae in an upward direction. This is what creates the lumpy and dimpled skin appearance, also known as the 'cottage cheese' appearance, associated with cellulite.

Do cellulite creams work? In my experience, cellulite creams have limited impact on reducing cellulite but can be beneficial for maintaining skin tone. Three ingredients that help skin tone are retinol, aminophylline and caffeine.

Is there a way to permanently remove fat nonsurgically? The only method to permanently remove fat is surgical body contouring, which has made dramatic advances over the years. Our center uses tumescent ultrasound assisted liposculpting that uses laser and ultrasound technology to melt fat and tighten skin. The procedures use high frequency ultrasonic waves to break down the fat and allow the fat cells to be removed. Laser technology directly contracts collagen and tightens the skin. This is an effective treatment for the body, neck, or face. Before any of my patients consider fat reducing treatments, I remind them that the treatments are not a replacement for regular exercise and proper nutrition. Individuals with stubborn areas of fat who exercise regularly are the best candidates for these procedures. The best plan is to use the body sculpting procedures as a reward for successfully sticking with your fitness regimen.

Months after the delivery of her big bundle of joy, Laura continued to have pain when sitting and during intercourse. Things were not the same. Frustrated by the results of her outcome, she looked for answers on the internet where she found Dr. Brillhart’s web site. She discovered and consulted with the Laser Vaginal Rejuvenation Institute of Indianapolis. Dr. Brillhart’s surgical option uses a laser to bloodlessly tear down the scarred layers and reassembles them correctly. Laura opted for this procedure and just days later things had already significantly improved and that nagging pain was gone. Erin loved to ride her bicycle but she was tired of the pinch that would suddenly come from sitting the wrong way on her bike seat. Besides that, her favorite swimsuit bottom never fit or looked quite right. She consulted Dr. Brillhart at LVRI of Indianapolis and scheduled a laser labial reduction. Soon she was back on her bike and riding without the pinch. After four babies, Melissa knew sexual relations were just not the same. She 10

couldn’t feel anything anymore and was afraid her husband might be experiencing the same. The birth canal was no longer holding him as snugly as it did before her first baby. James Brillhart, MD Leaking urine when laughing with friends or exercising at the gym was getting old fast. Dr. Brillhart’s Laser Vaginal Rejuvenation (LVR) surgery brought her hope. Melissa was thrilled with her results after surgery. Mini pads are now a thing of the past and she and her husband are enjoying that “hold me tight” feeling they both missed. Indiana Women In Need Foundation Hosts 12th Annual The 12th annual Pink Pajama Party hosted by the Indiana Women In Need Foundation will be held on February 19th and February 20th, 2016 at the Indianapolis Marriott North at Keystone at the Crossing. This annual signature fundraising event provides a night of pampering to breast cancer survivors, friends, family and women throughout the state. This overnight event includes complimentary spa and salon services Friday evening in addition to an exciting silent auction, dinner and dancing to the sounds of Living Proof. Guests are invited to join this party in their favorite pink pajamas and wake up the next day to a delicious breakfast and wrap-up activities. An annual sold out event, guests are encouraged to register on-line at www.iwinfoundation.org. This year’s leading Pink Pajama sponsors are Defender, CNO Financial, Roche, Kroger, Tricia’s Hope, Community Hospital Oncology Physicians, Turkle & Assoc. and Pile CPA’s. This year’s Master of Ceremonies is Anne Marie Tiernon. Funds raised from the Pink Pajama Party will support I.W.I.N. Foundation’s grant program that provides women the strength to endure breast cancer treatment by providing financial assistance for individualized services and support such as house cleaning, gas and groceries, child care, massage, yoga, lawn care, and snow removal. “I can’t believe you’re calling. I was just crying in my pantry wondering how I was going to pay for groceries.” Comments like these are heard every day at the I.W.I.N. Foundation. Receiving treatment for breast cancer is exhausting… financially, physically and emotionally! Combination treatment regimens of surgery, chemotherapy and radiation, often span the course of several months to one year. Daily trips to radiation therapy are 18

costly. Making meals or cleaning the house is tiring. Working full days is challenging. Too often the physical and financial strain of treatment causes women to give up their fight thereby losing their opportunity to receive treatment and their only true chance at a cure.

treatment.

The Indiana Women In Need Foundation (I.W.I.N.) fills a critical gap in the breast cancer support network and is the link in the chain that keeps these women in

The I.W.I.N. Foundation was founded as a 501(c)(3) by Nancy Shepard, R.N. in 2000. Nancy’s dedication to the fight against breast cancer began several years ago when the diagnosis of breast cancer came to her close friend, Ellen- a nursing colleague and working mother of two children under the age of 5. Nancy saw first hand how the fatigue of treatment made the simplist things more difficult. Add to that, the financial burden of treatment and the journey becomes daunting. Having assisted 3 women in the first year, I.W.I.N. now serves an average of 500 recipients statewide per year and more recently opened their doors to men with breast cancer. I.W.I.N. has delivered over $1.3 million dollars in services to individuals in all 92 Indiana counties.

Intimacy Restored

Using the MonaLisa Touch laser, Gynecologist Dr. Michelle Jones Singer is helping women find relief from vaginal dryness, itching, burning and painful intercourse. Here’s how she can help you, too. Since introducing the MonaLisa Touch laser to the Indianapolis area three months ago, Dr. Michelle Jones Singer has treated about 30 patients seeking relief from vaginal dryness, itching, burning and painful intercourse. “This restorative, minimally invasive CO2 laser therapy, which was FDA- cleared last September, has produced far better than hoped for results in 95 percent of my patients treated,” she reports. MonaLisa Touch means real relief for patients dealing with painful intercourse, vaginal dryness and menopausal issues who don’t want to take hormones.

It was well worth it!” Fifty something K.M. of Noblesville with a history of stress and urge urinary incontinence says “I have much less loss of urine with coughing or sneezing. I am not getting up all night long to go to the bathroom.”

MonaLisa Touch Laser is an office procedure that works painlessly, takes about three to five minutes to activate vaginal tissues to produce collagen. There is no anesthesia and no downtime. Three treatments are necessary to restore vaginal health. Although insurance does not cover this procedure, Dr. Jones Singer notes that the cost of $900 per treatment for 3 treatments is similar over time to the other treatments such as creams and hormone therapy. How many embryos should be transferred? by Jennifer Hostetter There has been a push to convince Intended Parents (IPs) in surrogacy agreements, to agree to the transfer of only one embryo in in vitro fertilization (IVF) cycles in order to reduce the possibility of multiples during pregnancy. Often, IPs will attempt to implant more than one embryo for various reasons, including but not limited to the quality of the embryos, eggs, or sperms, success rates of the clinics, and other financial considerations. Sometimes, even if IPs agree to transfer one embryo, an egg can split, resulting in twins. Unfortunately, multiple gestations can lead to increased complications for the fetuses and carrier and the IPs and the carrier can be faced with a difficult decision, multi-fetal pregnancy reduction. Before this procedure can even be considered, the IPs and carrier have to be in agreement. Although you may discuss this in depth before entering into the agreement, any provisions on multi-fetal pregnancy reduction may not be enforced in Indiana. In Indiana, a surrogacy agreement that requires a woman to consent to undergo or undergo an abortion is void. There are multiple scenarios where this can raise several problems for the parties. For example, if the carrier was carrying multiples and there were no medical complications but the IPs want the carrier to have a multi-fetal pregnancy 30

reduction due to other reasons, and the contract was either silent on the issue or indicated that the carrier would agree to undergo the procedure in the event of a life or death situation, the end result may be unknown. There is no clear answer and unfortunately there a few cases across the United States dealing with similar sets of circumstances with unknown outcomes. It would be impossible to predict every outcome for a surrogacy arrangement, but it is crucial to attempt to consider all of them and take them into consideration when selecting your carrier or IPs, drafting your contract, and deciding how many embryos will be transferred during IVF. While twins can be a result of even a single embryo transfer, you are still significantly reducing the odds of multiple gestations that may lead to the very difficult decisions. Also, while deciding on the number of embryos to transfer, consider the additional expenses of caring for multiple children from gestation through college and beyond. The first time you consider those expenses and decisions should not be when you find out that you’re going to be having children. That should be one of the most exciting times of your life. ** This is my own personal legal opinion. You should always consult with your ART team, including but not limited to clinic/agency, medical providers, and attorneys.
