SURGEON-IN-CHIEF
TODD J. ALBERT, MD
Surgeon-in-Chief
I look forward to attending this month’s AAOS conference and convening with HSS colleagues and alumni.
Last fall’s Annual Alumni Meeting was my first activity with the association and it was impressive to see nearly 200 alumni return to the HSS campus to participate in a variety of offerings during the three-day span. HSS SPINE SERVICE As you may have heard during the meeting, the HSS Spine and Scoliosis Services have been combined into one Spine Service. Frank P. Cammisa, MD, has been named chief of this merged spine team. I thank Matthew E. Cunningham, MD, for his service as Interim Chief of the Scoliosis Service. The deformity expertise he and others have exhibited will help continue to solidify HSS’s differentiation as the leading spine program in the world. MEDICAL STAFF CONFERENCE GUEST SPEAKER SESSIONS AVAILABLE ONLINE TO HSS ALUMNI We have secured a series of renowned guest speakers for a number of our monthly Medical Staff Conferences. Most recently, Gerald Hickson, MD, was the keynote speaker for our annual culture of safety program. He is a national expert in quality improvement and patient safety with particular interest in the area of individual behaviors that can weaken a culture of safety. Michael Chernew, PhD, and Steve
Udvarhelyi, MD, brought a collective wealth of knowledge and expertise in the forces influencing healthcare during this unprecedented time of change in the industry. I am pleased to let you know that these three lectures are now available for exclusive alumni viewing by logging into www.hss.edu/eacademy and searching keyword “MedStaff.” I am certain you will find the sessions thought-provoking on pressing topics facing our industry. You will be notified of future presentations through email and a posted notice on the for[MD] portal. HSS STAMFORD OUTPATIENT CENTER A significant milestone for HSS is the recent opening of a new location in Stamford, CT. See page 18 of this issue for additional details about the facility. Our expansion in this region strengthens our presence outside of New York City and meets patient needs closer to where they live and work. ANNUAL ALUMNI MEETING FALL 2015 Plans for the 97th Annual Alumni Meeting are well underway and will include cutting-edge content and relevant guest and alumni speakers. Daniel J. Berry, MD, is confirmed as Surgeon-in-Chief Pro Tempore. Mark
your calendars for October 22–24, 2015, and please plan to attend. This year, we are encouraging increased reunion class participation, and are working to expand recognition to resident class years at the notable anniversaries of 5, 10, 15, 20, 25 and 30 years. In addition, we will combine fellows in 10-year class/specialty increments. Participation as reunion classes gives us all an invaluable opportunity to network and connect. Discounts for reunion class tables at the annual benefit are in the works to assist with creating opportunities to easily connect. WITH APPRECIATION I am grateful to inform you that the 2014 Autumn Benefit was a tremendous success, thanks to your ongoing support and the dedicated efforts of my wife, committee chair Barbara Albert, and co-chairs Anne Altchek, Renee Flug, Ann Roberts and the Benefit Committee. The event at Guastavino’s raised over $750,000 in support of medical education, the HSS Journal® and alumni initiatives. Please reserve the date of October 23, 2015, for the benefit this fall. I look forward to seeing many of you at the AAOS Annual Meeting.

FROM THE OFFICE OF
ALUMNI AFFAIRS Our 12th Surgeon-in-Chief,Todd J. Albert, MD, hit the ground running when he took over from Thomas P. Sculco, MD, in July 2014, less than a year ago. One of his first challenges was to review HSS Orthopaedic Specialty Services and their chiefs for possible change. We like to keep alumni all over the world apprised of who the chiefs of services are at HSS, so that you can contact one directly. For that reason, we are listing the specialty services with their current chief/ co-chiefs as of February 1, 2015. We believe that you, as HSS alumni, are VIPs and if you wish to refer a patient, you may want to contact a member of the HSS staff personally either by phone or email. In addition, you should be aware that in the past year, under the leadership of Daniel S. Rich, MD (Resident 1982), Martha O’Brasky, MPA, Administrative Director, Alumni Affairs and Colleen O’Shea, MPA, Manager, Alumni Affairs, the new password-controlled alumni social networking platform, for[MD], allows you to post cases securely for discussion, questions and answers. Clicking on the link www.hss.edu/ alumniservices will bring you to a for[MD] log-in and also lists a market basket filled with services of great potential. Try it – you’ll like it, guaranteed! GAME CHANGERS Traditionally, service chiefs are appointed by the surgeon-in-chief. For many years dating back to the 1900s, a chief could remain in the position for years. There

were no rules set in stone regarding service chief tenure. I was Chief of the Scoliosis Service for 28 years, from 1967 to 1995, when I retired from patient care. I served in that position under four surgeons-in-chief (Robert Lee Patterson Jr., MD, Philip D. Wilson Jr., MD, Andrew J. Weiland, MD, and Russell F. Warren, MD). Last October, Oheneba BoachieAdjei, MD, who replaced me in 1995, moved back to Ghana after serving as Scoliosis Chief for 20 years. But now, as we are halfway into the second decade of this 21st century, the game is changing.

The Scoliosis Service has now been combined into the Spine Service. This was the oldest scoliosis service in the country, starting back with Walter Truslow in 1899. John Cobb made it into one of the most famous, sought-after places to visit or train for young doctors interested in studying scoliosis.
Hand and Upper Extremity Service
The Chair in Scoliosis, established in 2006, is now fully funded for over $2 million by many dedicated donors, grateful patients and families and friends of the hospital. As the Scoliosis Research Society celebrates its 50th year in Minneapolis this September, it has continued to place the name “scoliosis” at the forefront of orthopaedics in global healthcare and keep it there. I expect HSS to follow suit.

HSS Class Repre Forging Bonds, Fostering Professional Excellence Five years ago, the Class Representative program was launched to further invigorate the already dynamic HSS alumni network by improving communications, increasing the engagement of alumni and attendance at events and identifying potential avenues for alumni collaboration. In this short time, Class Representatives have championed an array of alumni programs and services into existence. Today the group totals 74 and includes representatives from residents and fellows in each specialty, for most years since 1977. International Alumni Ambassadors work with the Class Representatives and the Office of Alumni Affairs to boost HSS’s worldwide reach and reputation (see sidebar). Each year, two professional meetings provide an opportunity for Class Representatives and International Alumni Ambassadors to convene: the Annual HSS Alumni Meeting and the American Academy of Orthopaedic Surgeons Annual Meeting (AAOS, upcoming, March 24–28 in Las Vegas). “With the busy professional lives that we lead, it is hard to keep up with all of our classmates and alma maters,” says Gregory S. DiFelice, MD (Resident 1999). But I feel that each one of us has a special fondness of our time at HSS and the maturation that we all went through together. The Class Representative program has been a great way to stay updated on each others’ whereabouts and accomplishments and help foster initiatives of importance and relevance to the alumni community.” Communication lies at the core of the

Class Representatives’ agenda, whether through face-to-face social gatherings, opportunities for mentoring and teaching, or using social media to compare notes on cases. By maintaining contact with class members and the HSS community, and promoting alumni activities and events, Class Representatives create and maintain a vibrant network essential to the success of the Alumni Association. They also work to enhance connection and community by boosting HSS alumni attendance at specialty meetings. Supported by the Office of Alumni Affairs, HSS Class Representatives have spearheaded the following programs:
AAOS Hospitality Suite Need a place to retreat from the bustle of the AAOS conference? Look for the HSS Hospitality Suite – and thank the HSS Class Representatives for initiating this benefit, which provides a place to relax and reconnect with colleagues while grabbing a bite to eat. This year’s suite will be at the AquaKnox Aqua Room, in the Venetian Restaurant Row.
Bioskills Education Laboratory (BSEL) Visiting Alumni Program This program gives alumni who work outside of HSS an opportunity to return as visiting instructors for residents and fellows in HSS’s nationally accredited simulated surgical training facility. These sessions build relationships between trainees and alumni, foster lifelong learning and promote the exchange of diversified surgical knowl-
edge and strategies. Scheduling visits to coincide with the Visiting Professor Lecture Series and Resident Journal Club provides additional opportunities for participation. Visiting alumni also can observe in the OR. “I spent an afternoon working with the residents at the Bioskills Lab, performing ACL reconstructions,” says Domenick J. Sisto, MD (Resident 1984). The facility is first-rate and the enthusiasm of the residents was infectious – it was an exhilarating day. I strongly recommend each alumnus to volunteer to teach!” Watch for the upcoming announcement of BSEL visiting alumni for 2015. To date, the following alumni have participated: 2014: Benjamin A. McArthur, MD (Resident 2013), Charles L. Nelson, MD (Fellow 1998), and Christopher S. Smith, MD (Fellow 2012) 2013: Asheesh Bedi, MD (Fellow 2009), Domenick J. Sisto, MD (Resident 1984), Robert C. Klapper, MD (Resident 1988), and Christopher Wahl, MD (Fellow 2002) 2012: Sommer Hammoud, MD (Resident 2011) In addition to conducting the BSEL session, several alumni took part in additional opportunities while visiting HSS. Dr. Bedi conducted a Journal Club. Dr. Klapper observed surgery of current attending Chitranjan Ranawat, MD (Fellow 1969). Dr. Wahl observed surgeries of current attendings David S. Wellman, MD (Fellow 2011), and Dean G. Lorich, MD (Fellow 1996).

With for[MD], alumni can create profiles, connect, receive event updates, and securely discuss cases in a HIPAAcompliant platform. Internal and external alumni from the Foot and Ankle Service are active users of this platform, and Ogonna (Kenny) Nwaka, MD, is leading radiology alumni to encourage discussion of cases. Ultimately, the aim is for each service to have an outlet for bringing together alumni at HSS and around the world.

WELCOME TO OUR LATEST
INTERNATIONAL ALUMNI AMBASSADOR JAMES LINKLATER, MD
Radiology & Imaging Fellow, Class of 1997 linklj@bigpond.com
Upon completing his MRI fellowship in 1997, James Linklater, MD, returned to Sydney, Australia, where he works in private practice, providing comprehensive sports imaging services to a number of professional sporting teams and elite sportsmen. Dr. Linklater is actively involved in research and teaching and is immediate past president of the Australasian Musculoskeletal Imaging Group (AMSIG). He lives in Sydney with his wife and two children and enjoys surfing and skiing. Our HSS International Alumni Ambasadors have completed their graduate medical education at HSS and have been nominated. We welcome Dr. Linklater as the newest representative. The program now numbers 10 ambassadors, representing 7 different countries. A major goal of the program is to engage ambassadors to participate in hosting an HSS resident during his/her travel elective. The ambassadors work with the Alumni Association to identify new opportunities worldwide for future partnerships in integrating best practices, sharing research, and creating educational programming. With the current international climate and the globalization of medicine, we are excited for the future and what this program can accomplish.
“It is a great honor to be appointed an HSS International Alumni Ambassador. I am grateful to HSS and to Dr. Potter in particular for teaching me a commitment to excellence and academic enquiry. I look forward to working with my fellow alumni.” ENGAGING LEADERS. CONNECTING PEERS.

96TH ANNUAL
ALUMNI MEETING The Alumni Association conducted another outstanding annual meeting from Thursday, October 30, through Saturday, November 1, 2014. The 7th Annual HSS Education Leadership Program kicked off the CME-accredited program. Anne E. Van Heest, MD, facilitated a seminar focused on measuring performances and innovations in education that are necessary to develop effective teaching skills. After the program, alumni donned festive attire and headed to Guastavino’s for the Annual HSS Autumn Benefit, which raised over $750,000 in support of medical education initiatives. The meeting resumed early Friday morning with resident case presentations and a lecture by Georg Schett, MD, Physician-in-Chief Pro-Tempore. The day continued with several symposiums on scoliosis, bone remodeling, the role of vitamin D in sports medicine and new techniques in joint replacement. Debate sessions examined outpatient vs. inpatient total knee replacement, the use of rhBMP-2 in spinal surgery and the use of tranexamic acid for joint replacement surgery. Alumni Association President Bernard N. Stulberg, MD, presided over an informative Annual Business Meeting. JOHN J. CALLAGHAN, MD A LIFETIME OF LEADERSHIP Dr. Callaghan credits his HSS mentor Philip D. Wilson Jr., MD, with teaching him about leadership. In addition to many other positions, Dr. Callaghan has served as president of the American Association of Hip and Knee Surgeons, The Hip Society, and other organizations. In 2010 he became president of the American Academy of Orthopaedic Surgeons, focusing his efforts on making U.S. orthopaedics a model for quality and value-driven interventions.
From left to right: Son, Patrick, wife, Kim, and daughter Katie, with Dr. Callaghan.
In recognition of his leadership and his active participation in alumni affairs, HSS presented the Distinguished Alumnus Award to John J. Callaghan, MD, at the 96th Annual Alumni Meeting last November.
Callaghan. “In New York, medicine was already dynamic in the 1980s. To see the model of academic medicine, and that concept of quality, taught me what kind of medicine I wanted to practice.”
Dr. Callaghan is The Lawrence and Marilyn Dorr Chair in Hip and Knee Reconstruction as well as professor in the Department of Bioengineering, at the University of Iowa College of Medicine in Iowa City.
From HSS, Dr. Callaghan went on to serve in the U.S. Army on the faculty of the Uniformed Services University of the Health Sciences, where he remains assistant professor. He then moved to Duke University before returning to Iowa in 1990.
He graduated cum laude from the University of Notre Dame, earned the MD at Loyola University Stritch School of Medicine, in Chicago, and completed a residency in orthopaedics at the University of Iowa before arriving at HSS as a fellow in 1983. A CULTURE OF EXCELLENCE “My experience at HSS jump-started both my clinical and research careers,” says Dr.
Among many honors, Dr. Callaghan has received numerous awards from the Hip Society and the Knee Society. In 2014 the British Orthopaedic Associaton named him an honorary fellow. He also has authored or co-authored more than 350 peer-reviewed publications.
In 2014, Dr. Callaghan took over as president of the International Hip Society. He is the first HSS alumnus and only the sixth American to serve in this role in the 40-year history of the organization. LOOKING AHEAD Dr. Callaghan continues to shape the future of orthopaedics as chairman of the Orthopaedic Research and Education Foundation, and he maintains an active research program focused on issues of quality in healthcare. He also continues to collaborate on new knee designs with long-time HSS mentor, colleague and friend Chitranjan S. Ranawat, MD. Dr. Callaghan’s lifelong ties to the “tremendous family” of HSS fellows and faculty are personal as well as professional – he and his wife, Kim, and their children, Patrick and Katie, still consider New York to be a second home. These relationships also form the basis of his advice to current fellows: “Find mentors. Over the course of a career, mentors become colleagues and collaborators – and friends. That cycle is so important.”

A Gene LINKS MALARIA
AND RHEUMATIC DISEASE
Translational research takes former HSS Fellow Richard Bucala, MD, PhD, from his Yale University laboratory to a hospital in Zambia Back in 1988, when Richard Bucala arrived at HSS for subspecialty training in rheumatology under Charles Christian, MD, he never imagined that he would start a rheumatology training program in Zambia. But today he credits his experience at HSS for the success of this endeavor. “HSS had such a great cadre of mentors and amazing clinical teachers,” says Dr. Bucala. “It imparted in me a notion of what excellence was in clinical care and the importance of a multidisciplinary approach.” Dr. Bucala earned his MD and PhD through the Weill Cornell–Rockefeller University program and completed internship and residency training in internal medicine at Harvard's Brigham & Womens Hospital. In addition to his HSS fellowship, returning to New York gave him the opportunity to do postdoctoral research at Rockefeller. “We were trying to identify new cytokines,” he says. That’s when he succeeded in cloning macrophage migration inhibitory factor (MIF), a cytokine that regulates steroid responsiveness. Little did he know that
the research would launch his career and eventually take him halfway around the world. From the beginning, Dr. Bucala and others thought MIF had potential as a therapeutic. Nine years later, when his laboratory at Yale sequenced the human MIF gene, Dr. Bucala gained insight into how MIF works. He discovered short repeating DNA sequences that influence how much MIF protein is produced during an immune response – the more repeats, the more MIF. Sequencing MIF in different human populations showed a tendency for fewer repeats among populations in Africa, where malaria is common. And that led him to wonder whether, similar to sickle cell trait, reduced MIF production confered protection against malaria’s lethal complications. To test this idea, Dr. Bucala led a genetic epidemiology study in collaboration with the Macha Mission Hospital in Zambia. He also developed a fast, low-cost biochip for characterizing an individual’s MIF allele. The
research confirmed the protective effect of low-level MIF against severe malaria. A practical result of this work is that children with malaria who are susceptible to a high MIF response can be identified, and monitored and treated more aggressively. Dr. Bucala’s population studies also revealed a role for MIF in autoimmune disease. People with rheumatoid arthritis, for example, who have fewer MIF repeats, tend to have less severe disease. With more repeats, this and other autoimmune diseases such as lupus are more severe. Through his work in Zambia, Dr. Bucala learned that this country of 13 million people had only two physicians working part time in rheumatology. With support from the International League of Associations for Rheumatology in 2012 and 2013, he spearheaded a project to advance training and professional development at the University Teaching Hospital in Lusaka, establish rheumatic disease registers and increase public awareness. Today Dr. Bucala is bringing MIF-based therapies to fruition. At Yale University School of Medicine, where he is Professor of Medicine, Pathology, and Epidemiology & Public Health, his laboratory is developing such therapies to target autoimmune diseases, and for use as immune adjuvents, among other applications. Dr. Bucala also is the newly elected editor-in-chief of Arthritis & Rheumatology, closing the circle established by his mentors Charles Christian, and more recently Michael Lockshin, who have both served as prior editors-in-chief.

HSS in brief HSS Launches Two New Outpatient Centers in Connecticut and Northern New Jersey HSS continues to strengthen the main campus while also developing locations to bring services and medical treatment closer to patients outside of New York City. As the demand for the excellent care we provide continues to grow, it is essential that the Hospital grows along with it. The HSS Stamford office has 10 physician offices for full-time and part-time physicians, seating for 50 in the lobby, a conference room, 20 exam rooms, an MRI, three x-ray units and a special procedures unit. It is co-located with Chelsea Piers and NBC Sports. You can find a list of the service lines and physicians who will be practicing at the HSS Stamford office at right.
Doors opened at HSS Stamford Outpatient Center in February Last year, HSS announced that the office in Greenwich, CT, would be moving to a larger site in Stamford. The HSS Stamford Outpatient Center opened its doors at Chelsea Piers Connecticut on February 2, 2015. The new office, which is 18,000 square feet, offers musculoskeletal care focused on surgical and
non-surgical treatment, with physicians representing the range of orthopedic subspecialties represented at the HSS main campus. Jo A. Hannafin, MD, PhD (Fellow 1991, 1992), has been appointed the facility’s medical director. The HSS Stamford office will offer community education and outreach programs to meet the musculoskeletal health and lifestyle needs of the surrounding community, says Hannafin.

HSS Welcomed Travelling Fellows HSS welcomed the 2014 John N. Insall Travelling Fellows last fall for a two-day program hosted by Steven B. Haas, MD, Chief of the Knee Service and John N. Insall Chair. The Insall Fellowship is awarded to four candidates internationally and provides the opportunity for those individuals to travel to prominent joint replacement and knee surgery centers. Two physicians came from the United States, one from the United Kingdom and one from Japan. The fellows spent one day at HSS observing in the operating room and one focused on an educational program that included a welcome address by Surgeon-in-Chief Todd J. Albert, MD, and presentations by HSS alumni, including Drs. Mathias P. Bostrom, Mark P. Figgie, Russell E. Windsor, and David J. Mayman. HSS also welcomed two travelling fellows from the European Society for Surgery of the Shoulder and the Elbow (SECEC/ESSSE) and the American Shoulder and Elbow Surgeons Society (ASES) last month. Ogonna (Kenny) Nwawka, MD, has been a leader in implementing the for[MD] platform for Radiology Alumni. Back in September, Dr. Nwawka initiated sharing “What’s the Diagnosis” and MSK Ultrasound cases with the HSS Radiology Alumni and Musculoskeletal Radiology for[MD] communities. These cases test orthopaedic, rheumatologic and radiology/imaging knowledge. When each new case is added, the for[MD] community is welcomed and encouraged to engage in a discussion.
AMERICAN SOCIETY OF ANESTHESIOLOGISTS MEETING Several HSS alumni organized a group dinner at a local restaurant during the American Society of Anesthesiologists Meeting in New Orleans. The tight alumni community takes advantage of national meetings to stay in touch and reconnect with each other. FOCUSED ASSESSED TRANSTHORACIC ECHOCARDIOGRAPHY (FATE) COURSE Dr. Stephen C. Haskins (Fellow 2013) returned as activity director for the second officially sponsored North American Course in Focused Assessed Transthoracic Echocardiography (FATE), which took place on January 31 at HSS. Other alumni participating as faculty were Dr. Enrique Goytizolo (1997–1998 Fellow and Attending Anesthesiologist) and Dr. James Beckman (1996–1997 Fellow and HSS Assistant Attending Anesthesiologist).
Han Jo Kim, MD, has also been highly involved in for[MD] and recently participated in a session from the Orthopaedic Education Initiative (OEI). The process here is that prominent faculty from various institutions present a de-identified patient case with a medical history, images and other posts of discussion. Dr. Kim’s unusual case generated a series of questions and a thought-provoking discussion on different techniques and treatment approaches.
We invite all alumni to sign up and participate in discussions to help make this HIPAA-compliant platform become an active network and a productive vehicle. All alumni with email addresses on file received a personal activation link to sign up. If you have not yet activated your membership, please contact Colleen O’Shea, MPA, at osheac@hss.
