How do you make each procedure personal?
Spinal problems are often complex and can cause debilitating pain, reduced mobility and loss of function. The team of nationally recognized, board certified spinal surgeons at The New Jersey Spine Specialists takes the time necessary with each patient to help determine the source of their symptoms and come up with a comprehensive treatment
plan. Each patient’s individual goals are considered and whenever possible, non-surgical treatment methods are pursued. By taking a careful history, performing a thorough physical exam and utilizing the most advanced imaging techniques, Drs. Drzala and Reiter are able to make an accurate diagnosis. identified, many spinal disorders can be treated by nonsurgical methods, such as exercises to relieve pressure on spinal nerves, physical therapy, medication and spinal injections. “The human spine has a remarkable ability to heal when proper treatments are applied,” says Mitchell Reiter, MD, the current president of the New Jersey Orthopaedic Society. For some, however, surgery is the most reasonable treatment option. “Spinal surgery — or any surgery for that matter — should not be taken lightly and should only be considered after noninvasive therapies have been exhausted,” says Mark Drzala, MD. The good news is that in addition to traditional procedures, there are new, state-of-the-art, minimally invasive techniques for treating a variety of spinal problems. While using the latest techniques whenever appropriate, Drs. Drzala and Reiter strike a balance between adopting newly introduced technologies while always ensuring that there is enough supporting data from reputable scientific studies demonstrating a procedure’s safety and effectiveness. That being said, the New Jersey Spine Specialists are at the forefront of their profession. The perfect example of their commitment to
excellence is the cervical disk replacement procedure. Traditional treatment for a cervical disc herniation that is causing pressure on a person’s spinal cord or nerve roots frequently involved a spinal fusion surgery which forces two bones to grow together. A cervical fusion results in a permanent loss of motion in the neck and places additional stress on a person’s remaining discs. Cervical disc replacement is an FDA-approved procedure that preserves motion in the cervical spine and has been shown to be an effective alternative to cervical fusion. While Drs. Reiter and Drzala waited until high-level data was available showing the safety and effectiveness of cervical disc replacement, there are few surgeons in the region with as much experience as The New Jersey Spine Specialists with this technique. “Cervical disc replacement preserves motion while avoiding many of the downsides of cervical fusion surgery including the need to use cadaver bone grafts, plates and screws, as well as the risk of the fusion not healing,” says Dr. Reiter. What are the catalysts behind continually posting such high success rates in your programs?
At the IVF New Jersey Fertility Center, a team of specialized, board certified physicians provide personalized service for each of their patients. In 1990, when co-directors Susan Treiser, MD, Ph.D, and Dr. Michael Darder, MD, founded the Center, it was their vision to establish an infertility practice that combined state-of-theart medicine with the individualized care of a private setting. Today, their In Vitro Fertilization and egg donation facilities combine to form one of the largest and most successful centers in the U.S. But compassionate, tailored care still stands as a top priority. To that end, physicians are constantly communicating with their patients, and each patient or donor is assigned a personal nurse concierge to assist them through the entire process. Billing counselors also help couples and future mothers through the pitfalls often associated with coverage. These positions are part of a larger, caring team that provides information and support at every
step along the way, with the overall goal of making the patient feel empowered. Educating patients and screening donors have been the underlying principles since day one. To date, their egg donor program’s 71 percent success rate stands well over the national average and continues to put them in the 5 percent of fertility clinics nationwide. Dr. Treiser also emphasizes fertility preservation, guiding young women to freeze their eggs as an insurance policy, and letting older patients know there’s no right or wrong way to have a family. “There are so many options available right now,” says Dr. Treiser, “It doesn’t matter how you conceive, just like it doesn’t matter how you give birth.” “Just the other day I was in the hospital and I saw a patient that we helped get pregnant 18 years ago. What types of procedures do you offer your patients, specifically those undergoing breast cancer surgery?
“For women who have been diagnosed with breast cancer, it represents a very stressful time in their lives,” says Dr. Matthew Kaufman, MD, FACS. “As a plastic surgeon, I’m often one of the first people they will see after diagnosis, and it’s tremendously rewarding to be a part of their recovery.” A board certified plastic surgeon at The Plastic Surgery Center’s Institute for Advanced Reconstruction, Dr. Kaufman takes an individualized approach to each patient. While he has expertise in all types of reconstructive procedures, his primary goal is to achieve the patient’s expectations through individualized care. “While it may be very unnerving to anticipate the bodily changes that occur after mastectomy, we have the capability to recreate form in a very natural and elegant manner. Achieving optimal results using advanced reconstructive methods can help alleviate the psychological and emotional trauma that
accompanies breast cancer,” says Dr.Kaufman. Though breast reconstruction surgery is his most common procedure, Dr. Kaufman is also a board certified OtolaryngologistHead and Neck surgeon. This unique skillset allows him to perform complex cosmetic and reconstructive operations on a range of issues concerning the head, face and neck; many of which in the past have gone undertreated. For example, he has pioneered a revolutionary surgical treatment for diaphragm paralysis — paralysis of the respiratory muscles. In the U.S., approximately 15 to 20 percent of married couples will experience some degree of infertility. When breast cancer is found, does that mean a woman will always need a mastectomy?
“Believe it or not, it is actually uncommon for a woman to require a mastectomy. The survival rate for women who choose breast preservation and radiation is the same as those who have a mastectomy. Additionally, the rate of reoccurrence is also similar. If you test positive for the BRCA gene mutation, having mastectomies will decrease your chances of getting cancer. Your survival rate, however, will be no different than that of a woman who chooses close surveillance. A mastectomy may be recommended if a patient is elderly or ill and can’t be compliant with radiation, if radiation is not available and/or if the woman has had previous radiation for breast cancer or another condition. It also is indicated if there’s a simultaneous diagnosis of two breast cancers far away from each other”, says Drs. Jan Huston, Michelle O’Shea and Tamica White. Mammograms and Breast Health By Simon Murray
O
ther than skin cancer, breast cancer is the most common cancer among women in the United States. It’s estimated that every year more than 220,000 women in the United States are diagnosed with breast cancer, and more than 40,000 of those diagnosed will die from this terrible disease. Statistically, one in eight women will be diagnosed with breast cancer in her lifetime, which is why many of us have friends and family members who count themselves as breast cancer survivors. Because of the worldwide prevalence of breast cancer, the World Health Organization (WHO) and

the Centers for Disease Control and Prevention (CDC) have labeled breast cancer a “global burden” that “claims the lives of hundreds of thousands of women each year, while affecting countries at all levels of modernization.” But enough of the scary facts. Breast cancer can be stopped, and doing so begins with awareness. According to the CDC, death rates from breast cancer have been on the decline since about 1990, due in part to early detection, increased awareness of breast health and the continual improvement of treatment options. To promote breast health, women should have regular mammograms, which may lower the risk of dying from breast cancer. Treatment has the highest chance of success when
cancer is found at its earliest stages, so do ctors urge patients to go early in life and get regular screening. The CDC advises women 50 to 74 to have a mammogram every two years. For women ages 40 to 49, it’s imperative that they speak to their doctors about when and how often to get a mammogram. While the CDC is working to increase breast cancer awareness across all ages, it’s worth noting that about 11 percent of all new cases of breast cancer in the United States are found in women younger than 45 years of age. The Reality of a SURVIVOR By Simon Murray

n October 17, 2011, Giuliana Rancic followed through on a difficult decision. The face of E! News, and an upbeat analyst on Fashion Police, E! Entertainment’s pullno-punches, celebrity haute-couture litmus test, Rancic appeared on the Today show. Ostensibly, she was there to talk about the cliff-hanging season finale of her reality show, Giuliana & Bill, which chronicles the lives of Giuliana and her husband, Bill, leading to speculation that she was about to gush over her newfound pregnancy. But instead of revealing the success of her in vitro fertilization (IVF) treatments, Rancic revealed much more somber news: The star had been diagnosed with breast cancer. Rancic, who patrols the red carpet

with unflappable gusto, even when interviewing the most eccentric Hollywood characters, appeared visibly shaken. But she was also resolute as she recounted the circumstances that saved her life. Giuliana Rancic was born Giuliana Depandi on August 17, 1975, in Naples, Italy. At the age of seven, Rancic moved with her family to Bethesda, Maryland. After arriving in the United States with a limited English vocabulary, Rancic spent hours in front of the mirror, practicing her newly adopted language as she pretended to be local NBC anchorwoman Barbara Harrison. Rancic’s desire to become a news anchor led her to overcome the language barrier and eventually earn an undergraduate degree in journalism from the University of Maryland and a master’s degree from the American
University School of Communication. Through her career at E! News, Giuliana met Bill Rancic, the original winner of The Apprentice, Donald Trump’s “You’re fired!” series. Bill was hired by Trump at the show’s conclusion, becoming the first reality star to be hired on national television. Following his success on the show, Giuliana interviewed Bill. Sparks grew to flames, and the two were married in September 2007, during a televised ceremony. When it came to conceiving a baby, the road was rocky for the couple. And it was far from private. Every twist and turn in attempting to have a child was documented on Giuliana & Bill, which first aired on the Style Network but has since moved to Tuesday nights at 8/7 Central on E!. Two IVF treatments, one of which resulted in a miscarriage at eight

weeks, led the couple to a third attempt to get pregnant. But before going forward, Rancic was diagnosed with the unthinkable: breast cancer. Used to working and living in the spotlight, it was only natural that Rancic would take her battle with breast cancer public. If her doctor hadn’t been adamant about screening for breast cancer, Rancic is certain she wouldn’t have had a mammogram until she was 40 years old, believing that was the customary age to begin screening. She was 36 and about to go for another round of IVF, which would have accelerated the cancer, had it been left undiagnosed. Rancic now preaches a proactive approach to maintaining good health and stresses the importance of early screenings, self-detection and communication with your doctor. And she didn’t give up on that dream of having a baby. Following a double lumpectomy, radiation and a double mastectomy—something she calls “one of the toughest, if not the toughest, decisions I’ve had to make in my life”—a revitalized Giuliana joined Bill on the Today show on April 23, 2012, to tell the world some good news for a change: With the help of a gestational carrier, the couple was finally expecting with Edward Duke Rancic, who celebrates his second birthday in August of this year. An egg retrieval before undergoing radiation has given Giuliana the ability to realize her wish of a big, happy family.

I’m a big snacker, so it’s really helpful to keep only healthy snacks around. I love KIND Bars—they’re great because they’re made with all-natural ingredients. It’s really important to me to avoid artificial ingredients in my food. And I always keep a bag of raw almonds in my purse for days when I am on long shoots or traveling. Being a mom to Duke is the greatest joy I have experienced in my entire life. Every smile, laugh and even cry melts my heart, and I am just so grateful to be his mom. Bill and I always joke that Duke is a real scenestealer, but it’s true—he loves the camera. You’ll also see us building our dream home in Chicago, balancing our work and family life and planning our future. It’s our best season yet, in my opinion.

Q: YOU’RE AN INSPIRATION NOT ONLY TO WOMEN UNDERGOING BREAST CANCER TREATMENT BUT ALSO TO THOSE BATTLING FERTILITY ISSUES. WHAT’S IT LIKE TO BE CELEBRATED FOR YOUR PERSONAL LIFE AS MUCH AS FOR YOUR PROFESSIONAL LIFE? A: It’s incredible! It’s the greatest feeling in the world when someone comes up to me and tells me that they got their first mammogram because they heard my story and felt compelled to be proactive about their health. Or when a woman or couple tells me that they, too, struggled with IVF and thanks me for giving this trying circumstance a voice. My decision to go public with my health issues was not an easy one, but I decided that if I could help even one person by sharing my story, then it would be worth it.
Q:
SINCE BEING DIAGNOSED WITH BREAST CANCER, HAVE YOU CHANGED YOUR DAY-TO-DAY LIFESTYLE IN ANY WAY? A: I’ve always tried to live a very healthy lifestyle, with both my diet and exercise, so nothing has really changed. Of course, now, with the little guy in our lives, Bill and I want to be as healthy as possible so we can be around for a very, very long time.

he food triangle has had a difficult run. Since 1992, the USDA’s Food Guide Pyramid has been hopelessly controversial. The compartmentalized, spatially challenged pyramid has been the cause of continuous debates by government officials, nutritionists and the food industry at large. But, the average citizen took it as gospel, FOOD GUIDE PYRAMID, 1992 The controversy started with the Food Guide Pyramid’s first failing: it appeared as if you were supposed to eat more foods from the bottom than from top, which caused infighting at the highest levels and prompted interest groups to attack the merit of the design. At the bottom of the pyramid, the section that included bread, cereal, rice and pasta recommended a whopping 6-11

By Simon Murray eating their way to bad health under the guidance of the government. “For no-nonsense, rock-solid nutrition information, people often look to the Food Guide Pyramid,” wrote Dr. Walter Willet, MD, professor of epidemiology and nutrition chair in the department of nutrition at Harvard University, in his book, Eat, Drink, and Be Healthy The Harvard Medical School Guide to Healthy Eating. “That’s a shame, because the USDA
Pyramid is wrong. It was built on shaky scientific ground back in 1992. Since then it has been steadily eroded by new research from all parts of the globe. Scores of large and small research projects have chipped away at the foundation (carbohydrates), the middle (meat and milk), and the apex (fats).” In June 2011 the triangle was reshaped into a new food icon: MyPlate. The plate shape promotes core servings per day. Nutritionists were up in arms, claiming that eating too many servings of grains encouraged obesity. The pyramid was reconstructed on a new foundation. MY PYRAMID, 2005 The MyPyramid that replaced the 1992 Food Guide Pyramid was even worse. Doing away with the hierarchical design, the USDA came up with a spectrum of colorful food categories that were difficult to decipher without going online. (Its minor,
food groups: fruits, grains, vegetables, protein and a small side of diary, and it does so in proportion to the space they should occupy in our basic diet. It is simplified and easy to understand. But Dr. Willet and a team of experts at the Harvard School of Public Health and Harvard Medical School argue that the plate still doesn’t offer the complete picture underrated success was in implementing stairs on the side of the pyramid with an exercise component, adding, “Sixty to 90 minutes of daily physical activity may be needed to prevent weight gain or sustain weight loss”). But this pyramid, too, was deemed unsound and criticism was unabated. MYPLATE, 2011 The colorful, modern-day iteration of the food guide was turned into a plate offering a clear breakdown of what
when it comes to basic nutrition advice. Instead, they have come up with their own version, the Healthy Eating Plate. This update on the update is based on the best available science unhindered by political or commercial pressure from food industry lobbyists. “MyPlate does not give adequate guidance about the most important choices we make on a daily basis,”
Dr. Willett wrote to Healthy Living in an email. “Our plate gives more specific information about the optimal types of grains, protein sources, vegetables, fruits and fats. It also emphasizes water over milk.” For more on Dr. Willett’s revised food guide and a history of the USDA’s Food Pyramids, check out the sidebar. an average plate should look like. However, critics argue that it still doesn’t clearly indicate what types of nutritional choices should be made, and doesn’t distinguish between how much to eat of certain foods (e.g.: red meat or potatoes). MyPlate also lacks an activity message and is silent about fat, an omission that could steer consumers towards an unhealthful low-fat, high-carb diet.

A BALANCED BREAKFAST, LUNCH AND DINNER Dr. Willet the co-author of Thinfluence, a new book about the external factors that influence our weight, says there is “no single change we can make that will lead to successful weight control; this involves putting together multiple elements of a healthy diet and incorporating physical activity into our daily lives.” However, the single most important thing we as consumers can do is to limit or avoid soda and other sugar-sweetened beverages; for some people, this alone can make a great difference.
DR. WILLET’S HEALTHY EATING PLATE TIPS: > Make vegetables and fruits a significant part of your meal — ½ of your plate: Aim for color and variety, and remember that potatoes, because of their negative impact on blood sugar, don’t count as vegetables on the Healthy Eating Plate.
> G o for whole grains — ¼ of your plate: Whole and intact grains—whole wheat, barley, wheat berries, quinoa, oats, brown rice and foods made with them, such as whole wheat pasta —have a milder effect on blood sugar and insulin than white bread, white rice and other refined grains do.
> Protein up with power — ¼ of your plate: Fish, chicken, beans and nuts are all healthy, versatile protein sources—they can be mixed into salads, and pair well with vegetables on a plate. Limit red meat, and avoid processed meats such as bacon and sausage.
> I nclude healthy plant oils — in moderation: Choose healthy vegetable oils such as olive, canola, soy, corn, sunflower and peanut, and avoid partially hydrogenated oils, which contain unhealthful trans fats. Remember that low fat does not necessarily mean “healthy.”
> Drink water, coffee, or tea: Skip sugary drinks, limit milk and dairy products to one to two servings per day, and limit juice to a small glass per day.
> S tay active: The red figure running across the Healthy Eating Plate’s placemat is a reminder that staying active is also important in weight control.
