ith summer approaching and schedules lightening up a bit, now is the time many of us finally follow up and stop putting off doctor appointments and procedures. We finally have a moments break to stop and take care of ourselves. We start looking at our calendars and start planning when we are going to take the time to take action on our healthcare. The question is, where do we start? Let’s face it, everyone wants to know more about the surgeons they are considering. While personal recommendations and physician generated referrals are certainly valuable, most people want to know more about a surgeon’s credentials and experience. That’s where we at NJ Top Docs come in. For the last seven years, NJ Top Docs has been featuring healthcare providers across 120 specialties in all 21 New Jersey counties. We provide potential patients with the opportunity to essentially meet their surgeon, doctor and/or dentist on our site or in our magazines before they even schedule
their appointment. Our online profiles answer the most frequently asked patient questions, all in one convenient resource. To be featured in the magazine and/or profiled on the site, each surgeon, doctor and dentist must qualify and is extensively screened and approved. Below are the questions to ask your healthcare professionals before you or a loved one has surgery, while also providing the key reasons for asking these questions. Remember to always speak up when you don’t understand something, and never hesitate to voice questions and concerns to your doctor. Communication can be the cornerstone of good health care.

What are the risks involved?
Every medical procedure involves some type of risk. But the most important thing to keep in mind is whether the expected benefits outweigh the possible risks associated with the procedure, according to the American Cancer Society. Today, advances in surgical techniques enable surgeons to greatly limit the potential of life-threatening risks; however, there’s always a degree of risk involved during surgery. It’s important to know the possible complications that may be caused by the surgery itself and other factors such as anesthesia or infection. always speak up when you don’t understand something and never hesitate to voice concerns to your doctor.

What kind of anesthesia will I need?
Anesthesia is an important part of most surgical operations. A patient is given either local, regional or general anesthesia so that the surgery can be carried out without pain. Though safe for most patients, it’s advised to meet with the specialized doctor (anesthesiologist) or nurse (nurse anesthetist) prior to the operation, according to the Agency for Healthcare Research and Quality (AHRQ). The AHRQ recommends telling him or her what kind of medical issues you have, and if you’re currently taking any prescription or over-the-counter medicines, as they might affect your response to the anesthesia.

Why is this procedure being recommended? Are there alternatives?
Your physician should explain the surgical procedure clearly, using illustrative or informative examples when possible. A good question to ask during this time is if there are different treatments or methods for performing this operation, according to the American Academy of Orthopedic Surgeons (AAOS). In some cases there are alternative methods for performing an operation. Your physician should be clear as to why he or she favors one way over another. Other times, nonsurgical alternatives may exist that could be as helpful in improving a condition as surgery would be. By asking your physician to specify why this procedure is being recommended, you are making sure how this operation compares to other medicinal or nonsurgical treatments in improving your current medical condition.

What are the costs of this operation?
Knowing your health care plan prior to surgery is an important part of staying well informed. To better assess what costs you will be responsible for, The Johns Hopkins Hospital reminds patients to determine the total costs of the procedure prior to surgery. That way you’ll know what portion of the costs you’ll be responsible for paying. These may include, but aren’t limited to, the surgeon’s fee for surgery, hospital fees (if required) and separate billing for services such as assisting surgeons, anesthesiologists and other medical consultants.

What are the surgeon’s qualifications, and where can I get a second opinion?
You should feel confident that the physician who’ll be performing your surgery is qualified to perform this operation. According to the Columbia University Department of Surgery, you should always seek the consultation of a surgeon who’s a qualified surgical specialist. To ensure that this procedure is your best option, you may want to ask another physician or surgeon for a second opinion. If you decide to get a second opinion, be sure to check with your health plan to see if it’s covered. Requesting names of qualified physicians from your primary care doctor or hospital is a good place to start.

n addition to being the founder of Summit Breast Care and the Medical Director of The Breast Health Center at Hackensack UMC Mountainside, Jan Huston, MD, has devoted 25 years to the diagnosis and treatment of breast disease. She provides kind, calm care that begins on day one and continues through treatment and any follow-ups with her patients. Dr. Huston is an evidence-based practitioner. She points out that there is no evidence to support the use of screening ultrasounds or MRIs. They do not increase survival from breast cancer even in the instance of a new diagnosis of breast cancer. At this
time, clinical exams and mammograms are the only screening tests that have been proven effective. Dr. Huston is a big advocate of breast preservation. The survival and local recurrence rates are equal between mastectomy and lumpectomy with radiation. There are 40 years of randomized trials proving these results. There is no survival benefit in removing the unaffected breast. “In 93 percent of the breast cancer patients treated by myself at Hackensack UMC Mountainside, I was able to preserve their breasts,” Dr. Huston says. “I’m a skilled surgeon and take great pride in the techniques I’ve learned and adapted that make it nearly impossible for anyone to know that my patients have had surgery.” More specifically, Dr. Huston makes a small incision around the nipple, raises the skin to remove the lump, and then slides the two sides back together before closing the opening. The surgery is very cosmetic; the breast is only slightly lifted or smaller than the other, with no dent or defect. This result is achieved even after removing a significant amount of breast tissue. Dr. Huston has been recognized by her peers locally, regionally and nationally. She also received the 2012 Distinguished Alumni award from Western Michigan University.

proper fitting of the artificial joints. At the same time, he also cautions against always jumping to procedures that are touted as the latest innovations. Sometimes “new” innovations, such as hip resurfacing, are actually procedures that have been around for years and are just being reintroduced. Similarly, some new procedures aren’t always the best option for all patients. A case in point is partial knee replacements, which might sound enticing, but is usually a temporary fix for a younger patient who will eventually need the entire joint replaced. Dr. Alexander and his caring staff—most of whom have been with him for many years—try to demystify the joint replacement process and put patients at ease. fter finishing her general surgery training, Michelle O’Shea, MD, decided to focus her surgical practice on breast care because she values the relationships built with her patients. “Breast surgery is a tough process to go through,” says Dr. O’Shea. “My heart stops when I have to tell a woman she has breast cancer. I spend a lot of time with each patient so I can take care of her needs, not just her disease. urgery is the unification of medicine and technology, which means, in broad terms, when one advances, so does the other. Though these days we may think of laboratory engineered organ transplants and Nanorobotics as the frontier in this field, surgical practices have always involved some element of radical thinking that has given rise to the standards we expect from our surgeons today. As it becomes easier to invade people’s bodies to cure ailing organs, it’s also unquestionably becoming safer and more routine. The amount of operations being performed in the United States in the last decade alone has skyrocketed. According to a recent study published in The Journal of the American College of Surgeons, Americans can expect to undergo an average of 9.2 surgical procedures in their lifetime. While this may seem like a lot: Sports trauma, cardiac disease and obstructed labor (to name only a few) require surgery for effectively treating such conditions. Part 1:
Minimally Invasive Surgery The future of surgery is in many ways already here, and it’s hardly invasive. By using state-of-the-art technology, surgeons have at their disposal the potential to dramatically reduce tissue damage unavoidable in most conventional surgical procedures due to the necessity of large, open incisions. That means, above all else, less postoperative pain, quicker healing and a faster return to normal. It also means fewer noticeable scars after healing. Welcome to the world of minimal access surgery, where less is truly more.
General Surgery As a result of the past decade, the field of minimally invasive surgery has expanded becoming the standard of care for many different types of surgery. Minimal access, also known as laparoscopic surgery, is conducted by inserting a long, thin tube called a laparoscope through small incisions in the abdomen—just a few millimeters in length, in some cases. Attached to the tube is a miniature camera that projects a clear, magnified image of the surgical area inside the body. By passing other special surgical instruments through other openings, surgeons are able to perform complex

procedures such as cholecystectomies and procedures to correct GERD (gastro esophageal reflux disease), while reducing scar tissue resulting from longer incisions.
Neurosurgery In the film Fantastic Voyage, doctors aboard a miniaturized submarine enter a man’s brain in an attempt to save his life by removing a dangerous blood clot. While modern day science has yet to catch up entirely with Hollywood’s depictions and isn’t quite able to deploy tiny doctors into the bloodstream, a technique is being used by neurosurgeons that is in many ways microsurgery. To remove pituitary tumors without the need of a speculum—a specially designed clamp with mirrors— neurosurgeons are navigating an endoscopic instrument through the natural passageway of the nostrils to operate on the brain. The outfitted camera at the end of the utensil provides the viewer with a direct, high definition visual of the surgical site. In doing so, doctors can operate on brain tumors more precisely by illuminating and magnifying the area where the tools are being applied. With the use of this endoscopic surgical equipment neurosurgeons are now

able to perform life-changing surgeries in some areas of the brain that were inoperable less than ten years ago.
Cardiac Surgery In the past century a common belief among the medical community was that operating on the heart, or even touching the heart, was an act fraught with peril. Nowadays of course an entire branch of surgery is devoted to performing complex and life-saving operations on the cardiac muscle. For example, when it comes to valvular heart disease, cardiac surgeons are using a minimally invasive technique to repair or replace heart valves. Instead of opening the chest with a large incision then breaking the breastbone— a procedure typically required in conventional cardiac surgery—access to the heart is being provided through small incisions between the ribs. Like Alice in Wonderland, in some cases, these two-inch openings allow cardiac surgeons to operate through the proverbial keyhole, minimizing long hospital stays and unnecessary trauma.
Cosmetic Surgery Just as surgical procedures in different fields have shifted from open surgery to the use of less invasive techniques, cosmetic surgery is now done with the use of viewing scopes, specially designed surgical instruments and lasers that trim recovery time and shorten visits. Today’s plastic surgeons have a variety of tools and techniques available that can make procedures such as facelifts and eyelifts more accessible, while laser surgery is part of a front line of techniques being used to improve wrinkles.

Part 2: Robotics
Since leaving the medieval barbershop and entering the modern day operating room, surgery has advanced in head-spinning leaps and bounds. But nothing compares to the shift this branch of medicine has undergone in the past decade. Robotic technology, once reserved for flights of fantasy in science fiction adventures, is actually commonplace today. Remote-controlled robotic arms, high-tech surgical systems and robotic helpers are now a fixture in certain operations and operating rooms across the country.
The da Vinci Surgical System In Ridgewood, the Valley Hospital’s Institute for Robotic and Minimally Invasive Surgery, was the first medical center in the US to perform the Whipple procedure—a highly advanced surgery for treating pancreatic cancer— with a multi-armed robot named after Leonardo da Vinci. It’s not as out-there as it sounds. What started as a military funded program to research the possibility of remote surgery in battlefield triage situations is now common equipment in operating rooms right now. The da Vinci Si System is the latest generation of mechanical surgeon consoles from Intuitive Surgical. Some 13 years ago the FDA cleared the first da Vinci for laparoscopic surgery making it the first “telesurgical device” allowed in operating rooms. Since then, literally hundreds of thousands of operations have been performed using Intuitive Surgical’s futuristic technologies. To date, more than 2,585 da Vinci Systems have been installed in more than 2,025
In the US, the Whipple procedure was first performed here in New Jersey using the da Vinci Surgical System hospitals worldwide, with at least 42 of those systems residing in medical centers across New Jersey. While a “telepresent surgical system” may conjure up thoughts of long-distance doctors performing operations from behind computer screens thousands of miles away, reality is much more localized. Instead of outsourced physicians, with the da Vinci System, the surgeon is seated at a console only a few feet from the operating table—directly controlling the motions of the robot “arms” that grip tiny instruments too difficult to handle with the human hand. Depending on the model, the surgical unit may include three or four arms that the surgeon uses to make small incisions in the patient’s abdomen. These entry points allow complex procedures to be carried out through incisions the diameter of a
pencil. This means the potential for shorter hospital stays, faster recoveries and less scarring—not to mention improved access and precision for surgeons. In doing so, the arms insert stainless-steel rods with endoscopic cameras, which provide the surgeon with a computer-enhanced visualization of the site where the micro operating tips are working. Now, using the cutting-edge da Vinci Si System, two surgeons can perform a range of operations to treat conditions including cancers of the bladder, kidney, lungs, prostate and throat as well as coronary artery disease, obesity and gynecologic cancer. Utilizing two sets of controls allows 35 unprecedented collaboration, as two surgeons from different specialties can operate in tandem, while training processes are streamlined through advanced procedural techniques.

TELEVISION nip/tuck: DR. SEAN McNAMARA (Dylan Walsh); DR. CHRISTIAN TROY (Julian McMahon) The only thing Dr. Sean McNamara, a co-owner of a plastic surgery center, has under control is his successful career. The craniofacial surgeon faces many troubles at home with his wife Julia and son Matt due to his wandering eye. Since beautiful women at work constantly seduce McNamara, the couple endures much distress, leading them to divorce, remarry, and divorce again. Dr. Christian Troy, a fellow plastic surgeon and McNamara’s business partner, takes the opposite approach with women; whereas McNamara wants to repel women, Troy excels at enticing them with his charm. He’s the epitome of a self-centered womanizer—he drives luxurious cars, wears Gucci suits and hangs out at Miami’s best nightclubs. Troy’s lack of remorse further aids him in having affairs with many of the series’ female characters, including Julia. Consequently, Troy is the biological father of Matt, which was initially unbeknownst to McNamara.
M*A*S*H: DR. “HAWKEYE” PIERCE (Alan Alda)
Medical Center: DR. JOE GANNON (Chad Everett) Dr. Joe Gannon, a successful surgeon, has the skill of soothing his patients, their families, and coworkers with his soft-spoken voice. Gannon also works as the head of the Student Health Department at the Medical Center, a Los Angeles teaching hospital. However, the debate of youth versus experience rises at the core of the series between Dr. Lochner and Gannon.
Dr. Hawkeye Pierce is known for making a name of himself from his football career at Androscoggin College to his medical residency in Boston. With a turn of events, Pierce is drafted into the US Army Medical Corps where he serves at the 4077th Mobile Army Surgical Hospital (M*A*S*H). Even between critical sessions of treating wounded patients, Pierce makes the Army camp a memorable experience with heavy drinking, carousing and pulling pranks on the most rigid people, such as Major Frank Burns and Major Margaret “Hot Lips” Houlihan.

Her curiosity raises the attention of Chief of Surgery Dr. George Harris and Wheeler’s boyfriend, Dr. Mark Bellows. Despite the risk of losing her job, Wheeler continues the investigation. She discovers an overabundance of anesthesia-induced comas that all take place in the same operating room. Harris tries to stop Wheeler from revealing the truth and attempts to leave her brain dead. Bellows quickly realizes Harris’ scheme and saves Wheeler from the procedure. Harris awaits his arrest in the OR, knowing that he has been defeated.
Lieutenant General Yevgraf Andreyevich Zhivago tells his supposed niece the story of her father, Dr. Yuri Andreyevich Zhivago, in hopes to reveal the truth of her biological parents. Zhivago, a Russian surgeon and poet, was orphaned after the death of his parents. He was then sent to live in Moscow with family friends and their daughter Tonya, whom he eventually marries. During the Bolshevik Revolution, Zhivago falls in love with Lara, a political activist’s wife. Despite Lara’s pregnancy, the two never meet again. Years later, Zhivago sees Lara walking down the street and runs after her, but in his attempt, he dies of a heart attack.
Doc Hollywood: DR. BENJAMIN STONE (Michael J. Fox) Dr. Benjamin Stone, a young, skilled and money-hungry plastic surgeon, journeys in his Porsche from Washington, DC to a hospital in Beverly Hills to interview for a position at a thriving practice. To his dismay, his plans are delayed in Grady, SC when he plows into the fence of local Judge Evans and is sentenced to community service at an understaffed hospital. Stone finds love with his patients and the ambulance driver Lou, but after being pardoned from community service and given an opportunity with his dream job, he’s forced to choose between two different lives. We won’t give away the ending, but surely you can guess.
The Fugitive: DR. RICHARD KIMBLE (Harrison Ford) When Dr. Richard Kimble, a prominent vascular surgeon, is wrongly convicted for the murder of his wife Helen he must set out to prove his innocence, though evidence against him suggests otherwise. After escaping from federal custody—thus becoming a fugitive— Kimble is relentless to bring those who were responsible to justice while being pursued by a team of US Marshals, led by Deputy Samuel Gerard. Ultimately exonerated of the crime, the ordeal finds Kimble far from his sanctuary in the operating room.

hen the first laser beam was invented in 1960, it was given the infamous— and now defunct—description of “a solution looking for a problem.” From pointers to printers to anti-ballistic tactical defense systems, the laser has since found its way into ubiquity. While this universal nature is currently applied across the board, providing fields and industries with an impressive amount of solutions; the laser now offers humankind a solution to arguably the most important field of all: Medicine. The term “laser” is actually an acronym that stands for Light Amplification by Stimulated Emission of Radiation. Its distinctive quality is found in the ability to produce an intense, highly concentrated beam of a single wavelength. Thus, only a single color of light is emitted. While many types of lasers exist for different purposes, the ones typically used in medical clinics include the carbon dioxide (CO2) laser, the YAG (yttrium aluminum garnet) laser, alexandrite, KTP and the pulsed dye laser. The different colors produced by these beams are reportedly directly related to the type of surgery that’s being performed.
One such operation is called softtissue laser surgery, and it’s applied to a variety of medical fields. In general surgery, for example, a surgeon uses a highly focused CO2 laser beam to

create tiny incisions in tissue. Due to the high water content found in living tissue, the CO2 laser ablates—or vaporizes—unwanted growths such as tumors, while safely and effectively cauterizing capillaries, small blood
vessels and nerve endings. By doing so, the surgeon can limit the patient’s postoperative bleeding, swelling and unnecessary pain. And since a laser scalpel makes no physical contact with the surgical site, it naturally sterilizes, leading to a marked decrease in bacterial, viral or fungal infection. This unique interaction lasers have with tissue makes them standard procedure now in many exciting surgical operations. When it comes to LASIK—or laser eye surgery—lasers are used as a corrective procedure, reshaping the cornea to correct near and far-sighted vision. With hard tissue laser surgery, teeth can be cleaned and bones can be sealed. Even when it comes to neurology, with the help of MRI-guided thermal imaging, lasers are being used to destroy brain lesions that cause epilepsy and seizures. Today, these luminescent lights have come to dominate the surgical scene, offering a wide variety of effective solutions for the world of medicine. From here, it’s only a question 41 of experimentation, application and refinement, as this technology becomes an important tool for many surgeons alike.

the timeline: SURGERY THROUGHOUT HISTORY

urgery began when the first humans used their tools to manipulate the flesh of other humans. Since this branch of medicine is deeply rooted in tools, it should be no surprise then that the word “surgery,” which stems from Greek, actually means “work done by hand.” By today’s standards, surgical operations as recent as the 19th century were somewhat barbaric. Throughout the ages, patients would expect to have holes drilled in their heads, their blood let out by leeches and receive very little anesthesia—if at all. Modern medicine has allowed surgeons to become the rock stars of the medical world, but this progressive form of healing was not always so.
