A lot of people reading this are I am sure, aware of the ‘power’ of growth hormone (GH); a hormone that is secreted by the pituitary gland (see figure 1). GH is popular by those interested in antiaging medicine, especially since Dr. Daniel Rudman’s landmark published article in 1990 in which it was stated; “growth hormone reversed aspects of aging in some individuals by as much as 20-years.” (1) What Rudman discovered was that by injecting his patients, (aged between 61 and 81) with recumbent human GH, (which remember in the late 1980s had only just recently become commercially available via newly developed production methods), that there were many reversed ‘consequences’ of aging. Benefits included more elastic and thicker skin, greater hair growth rate and quality, enhanced feelings of wellbeing and of course, perhaps the most famous attributes- decreased body fat levels and more lean muscle mass. 4

But as Dr. Richard Walker discussed with me in his interview regarding sermorelin last year (2); today we know that there are some ‘issues’ surrounding GH- when it is used in its recombinant form via subcutaneous injections; these include: • GH needs to be injected to be effective- not everyone wants to inject daily. • GH is somewhat temperature sensitive and needs to be stored in the fridge.

• GH creates a negative feedback upon the pituitary gland, which means that if levels are not administered or monitored properly, then it could shut down/ reduce one’s own endogenous production of GH. • GH administered as a bolus injection does not mimic the natural pulsate release of GH, but rather produces a large bell curve in blood after administration. • GH injections remain relatively expensive. • GH has become a controlled substance in many countries. This means that its possession without a prescription can be considered to be a felony, and laws regarding import and export require import certificates to be supplied before it can be exported. On top of all this, MasterCard and Visa are now refusing to accept credit card payments for controlled substances, (and some other non-controlled medicines too!) Thus, in this last section, I have explained to you why GH has disappeared from most pharmacy sites. But not all hope is lost, since there are genuine GH precursors known as Growth Hormone Releasing Peptides (GHRPs) that are real-world alternatives.
IGF-1 Just before we move onto the GHRPs, mention should be made of IGF-1 (insulin like growth factor), which has been documented by Dr. Richard Lippman in his articles on the IAS website (3). IGF-1 is considered to be the ‘active’ agent that instructs cells to start ‘burning’ fat reserves. Indeed, the laboratory measure for determining the blood level of GH actually ‘looks’ for the levels of IGF-1. When IGF-1 levels are high then so have been GH and vice versa. In Dr. Ronald Klatz’s book; ‘Grow young with HGH’ (4) -published in 1998, I was quoted in it as saying of IGF-1; “that it will be the GH of choice in a few years’ time.” My thinking was that as IGF-1 became commercially available its price would begin to drop dramatically, (as had been the case with GH). Well, we’re now 15-years on from that statement and today there are a few commercially available IGF-1 products on the market, but unfortunately whilst their price has declined significantly, they remain expensive, so alas IGF-1 still remains the domain of the dedicated elite. Figure 2: Dr. Ronald Klatz’s book, Grow Young with HGH, released in 1998. So what else has recently become commercially available that looks promising to improve GH secretion?
GHRPs to the rescue! Ghrelin, a name derived from ‘growth hormone release inducing’ is a substance that stimulates growth hormone stimulatory receptors or GHSR, (5, 9). Ghrelin is produced primarily in the stomach and upper intestines (6) – but note that whilst GHSR’s are concentrated in the pituitary and hypothalamus, they are also widely distributed in central and peripheral tissues, thus helping to explain the diverse action of GH on the body (6, 10). But the problem with ghrelin is that it makes you hungry! In fact, it has been dubbed ‘the hunger hormone’ (7) - and there are many blogs on the internet that report this ‘side effect.’ This effect has also been published as occurring in both humans and animals (8). Ghrelin levels are high before eating increasing appetite and low after eating (see figure three). Ghrelin stimulates the appetite with the purpose of increasing the intake of food and promoting the storage of fat. So when ghrelin levels are high, we feel hungry. After we eat, ghrelin levels fall and we feel satisfied. Thus ghrelin may not be the supplement of choice for those wishing to lose weight, although it may be the one for those who want the desire to eat more! However an analogue of ghrelin, known as GHRP2 can realise the GH benefits of ghrelin without triggering any significant hunger pains (11).

GHRP2 clinically studied One placebo controlled study with GHRP2 (12) on 94 healthy individuals (male and female) aged between 40 and 70 years of age took various measures including waist and hips and ratios, total body composition (fat, water, bone and muscle mass), and forced lung capacity at its commencement. The subjects received either 10 ml of GHRP2 or a placebo each morning for 90 consecutive days. At the end of the study it was shown that those using GHRP2 improved their IGF-1 levels, from an average (pre-study) of 103.54 ng/ml to an average of 120.47 ng/ml (post study), thus clearly indicating that GH levels had been improved; however those using placebo had no significant changes in their IGF-1 (hence GH) levels. These improved GH levels did, (as expected) in the GHRP2

patient group alter various physical parameters. For example, total body fat decreased on average by 9.14% with visceral fat levels decreasing on average by 14.27% (with no significant changes in the placebo group).
fibers were denser even 12 weeks after surgery in those who used GH. It was stated that progressive insufficiency of GH during aging may contribute to periodontal disease and tooth loss.
At the same time, muscle mass (as lean body mass) increased on average by 5.37% in the GHRP2 group; plus, forced lung capacity was increased on average by 16.61% (and again there no significant changes in the placebo group).
Therefore, GHRP2 supplementation could benefit the maladaptive effects of sarcopenia on an individual’s body composition and extend good dental health.
All these positive changes to these aging biomarkers corroborate findings from the studies that utilized GH injections.
GHRP2 synergy
Side effects Few side effects were noted in the study (12), with just some soft stools and some upset stomachs recorded. There was also an increase in the total blood platelet count, but the values remained within the normal clinical range. Animal studies regarding the mutagenicity of GHRP2, as well as its LD50 and various genotoxicity tests have been undertaken (13, 14, 15, 16, 17). None of these studies have shown any reason for concern, despite doses having been administered as high as 2000 mg per Kg bodyweight for 2-days and at 50 mg per Kg bodyweight for 90 consecutive days. Compare this with a typical recommended dose for humans at 4 mg (total bodyweight) per day! It is worth noting that one quite unusual study administered GHRP2 to children for 8 consecutive month; its outcome lacked any significant side-effects or toxicities (18). Another children reference also showed safety after 12 months of GHRP2 administration BID, (J Pediatr Endocrinol Metab. 2003 Sep; 16(7):981-5).
Uses GHRP2 is clearly a useful adjunct to boost GH levels. Therefore, perhaps its most direct application is to assist in the agerelated disorder of sarcopenia. Sarcopenia is the loss of lean body mass. Surprisingly, muscle loss can begin early in life, even by 25-years of age. Clinical studies that measure the excretion of creatinine, (a measure of muscle content) note decreases of 50% between the ages of 25 and 70. This fact highlights that the degradation of muscle exceeds its synthesis past young adulthood. Clearly, maintaining muscle mass is essential to supporting whole-body protein metabolism, wound healing, physical strength, organ function, skin integrity and immunity. In the elderly, the loss of lean body mass manifests itself as frailty, reduced ability to walk, climb stairs, rise from a chair etc. This loss of independence can not only lead to disability but depression too. As a side note, some fairly recent studies suggest that patients with growth hormone deficiency have a higher risk of developing dental diseases (19). Another dental study utilised GH around implants (20), it recorded that bone repair and collagen 6
Sermorelin is another very fascinating GH peptide and was described by Dr. Richard Walker in issue 2 of the 2012 Aging Matters™ magazine. Sermorelin acts as a secretagogue to encourage the release of GH from the pituitary gland into the blood and it has been shown to be efficacious when taken sublingually, and it is stable at room temperature. Dr. Walker told me that when GHRP2 is taken in combination with sermorelin that the total effects of GH release can be enhanced by up to five-fold! Studies have confirmed this combination synergy, (21, 22, 23, 24, 25, 26). I worked with Professor Ana Aslan for 25 years, from 1963, first as a researcher, then as chief physician and afterwards as the Director of the National Institute of Gerontology and Geriatrics in Bucharest, Romania, between 1978 and 1990. In the last 3 years of her life, Aslan chose me as her personal physician and three months before her death she asked me to do some personal things, including to write a book about her life and her work. So I took notes at her bedside as a moral testament. Ana Aslan’s life can be seen in her work. She battled courageously, all for the service of good, to make man’s dream to live with dignity for as long as possible. For 35 years she led the first Institute of Gerontology in the world, she was an Ambassador of Gerontology and a brilliant woman. 8

her research results materialised a different product with a new formula that was named Gerovital-H3®. In the new formula, the addition of benzoic acid to the procaine induced a greater access of the procaine into the hydrophobic cellular compartments and modified the pharmacological action of it. It should be noted that between Gerovital-H3® and hydrochloric procaine there is a difference of pharmacological action. For example, procaine by itself has a stability of 6-months, whereas the Aslan product has a much increased stability of two and half years. Once introduced into the human body, the procaine molecule is hydrolysed into two metabolical fractions; specifically paraaminobenzic acid (PABA) and diethylaminoethanol (DEAE); however the absorption of the two metabolites is better when they result from the in-vivo hydrolysis of GerovitalH3® than when they are administered separately. The procaine from GerovitalH3® has a greater capacity of wadding the medium (pH), due to benzoic acid, reducing the degradation speed of the product. Interestingly, in the case of Gerovital-H3®, the chromatographic techniques showed that there are intact procaine molecules in the blood and heart of experimental animals 6 hours after the product is administered. t is considered that procaine is metabolised differently depending on the pH of the solution. At a pH of 7, the procaine is permeated ‘en mass’ into the blood, but at a pH of 3.3 the procaine is gradually set free from the blood. An acid solution will decrease the sudden release of the substance, a quality that Gerovital-H3® possesses. (1, 2, 3) Therefore, GerovitalH3® acts upon the human body both under the form of an intact molecule and through the hydrolysis products PABA and DEAE, which participate in the regulation of the intermediary metabolism. Gerovital-H3® favors the acetylcholine synthesis and it is a source of folic acid. Research suggests that procaine, by means of PABA, stimulates the intestinal flora and the production of folic acid, vitamin K and tyramine. Pharmacological studies (4) summarise its basic mechanisms as: • Gerovital-H3® competitively and reversibly inhibits monoamine oxidase (MAO). • Gerovital-H3® acts as an anti-depressive through the modification of the MAO level in the brain. • Gerovital-H3® is considered to play a role in maintaining the physiological status of the nervous cell membrane, restoring the equilibrium between the processes of excitation and inhibition at the level of the cortical and subcortical systems. • Gerovital-H3® exerts an important regulatory action upon the nervous vegetative centers. • Further experiments reveal procaine’s anabolic action. • Studies on Infusoria (Colpidium colpoda and Vorticella) show the proliferation of cells (6).
anabolic affects especially improving the quality of the hair (7, 9). Gerovital-H3’s® action upon the lipid mechanism is reflected through several mechanisms: • Gerovital-H3® diminishes the level of plasmatic lipoproteins and lipids. • Gerovital-H3® exerts an effect on the erythrocyte membrane. • Gerovital-H3® has an anti-oxidant mechanism that reduces the oxidative stress exerted on the membrane structure.. It has been shown that Gerovital-H3® action on the lipid metabolism results in positive modifications in improving the serum total of cholesterol, changes to the lipoprotein fractions ratio and also changes to the unsaturated fatty acids content (11).
The international confirmations Mention must be made of the fact that the research concerning Gerovital-H3® therapy has been simulated in more than 500 medical and scientific publications. The experiments that utilized the original Aslan product and method confirm the efficiency and efficacy of Gerovital-H3®. Special interest was generated by the mechanism of GerovitalH3®’s action by several American scientists who presented papers of double-blind placebo controlled trials. Among them was Professor William Zung from Duke University, North Carolina who in his study applied the treatment for 28 days on his patients who were suffering from depression (12). One group of patients aged 60 were submitted before, during and after the treatment to a battery of psychological tests. Professor Zung proved the Gerovital-H3 ® efficiency in the treatment of depression. Furthermore, the American authors, M. Kurland and M. Hayman from Palm Springs, California, presented the double-blind results performed with Gerovital-H3® on 63 patients suffering from depression aged 45 to 80 (33 using Gerovital-H3® and 30 using placebo). Under observation there were several types of depression; manic-depression, reactive depression, organic cerebral depression, chronic reactive depression and alcoholic depression. The results proved that Gerovital-H3® efficiency in all the tests applied, the differences between the two groups showed a great statistical significance. Recognition has come from the world-wide known gerontologist, Alex Comfort of England, who in an article published in the magazine ‘Mechanism of Ageing and Development’, made positive remarks upon Gerovital-H3® and Aslan’s method and on the occasion of the international meeting ‘Medizinischewoche’ in Baden Baden, Germany, whilst concluding the Gerontology and Geriatrics section, Professor Paul Luth said; “The Aslan treatment method represent a most efficient therapeutic procedure in geriatrics.” (15)

Aslan’s method of prophylactic and curative treatment with Gerovital-H3® Ana Aslan declared of Gerovital-H3®; “After one injection in the femoral artery given to a patient with embolism at the level of the inferior extremity, I noticed the almost instantaneous disappearance of pain. Then, for the first time, I had the idea of applying the same method in certain diseases with acute pain, which produce immobilization and thus long-lasting work incapacity.” Like other great discoveries, the clinical observation facts did not fail Aslan, as they represented the beginning of a period of original and fundamental studies which contributed to the prophylaxis and treatment of aging and chronic disease. Thus, in that period, another clinical observation was pointed out by the patients who stated that after the injections, (given into the artery of the extremities) pain was relieved in all the body. Injectable Gerovital-H3® was first experimented with at the Institute of Gerontology and Geriatrics in Bucharest in 1953 and then in 1957 Aslan started comparative investigations to establish the effectiveness of the oral treatment. It was found that in order to achieve the same results like in the parental treatment, the oral dose had to be doubled

Administering method for Gerovital-H3® The long term treatment with Gerovital-H3 ® has been extensively established by Aslan. In the beginning, Gerovital-H3 ® was administered only as injections. The tolerance has always been tested before starting the treatment; one subcutaneous injection of 1 ml on the first day, followed by an intramuscular injection of 2 ml the next day.
If no local or general reactions occur, proper treatment can be started. In the experience of more than 300,000 patients that completed this treatment in Romania, intolerance only occurred in 1 in 7000 cases.
Preventative treatment Treatment of chronic diseases and aging consists of 4 courses of 12 injections and 4 courses of 24 pills, i.e., one course of 12 injections over 4 weeks (i.e., 3 injections per week), a 4-week break then one course of 24 pills over 12 days (one pill twice daily between meals), a 2-week break and then the cycle is resumed. Starting from the age of 40 years, the prophylactic treatment with pills only is recommended in a series of 25 tablets during the first 12 days, with an interval of 2 months; 1st day, 1 tablet/ day, 2-hours after breakfast, increasing to the 12th day when 2 tablets per day are taken, (again 2-hours after meals, for example one at 10 AM and another at 4 PM). There should a series of 5 treatment courses in the year, which should be increased to 6 per year for persons over the age of 65.
Curative treatment Chronic diseases require a 6 course of 12 injections, and 5 courses of 24 pills, yearly, i.e., one course of 12 injections over 4 weeks, a 2-week break, one course of 24 pills over 12 days (one pill twice daily between the meals); 2 week break. The cycle is then resumed. Depending upon the outcomes the physician and patients can either shorten or extend the breaks. The first and second course of injections can be administered daily in order to study the individual reactivity. Gerovital-H3® treatment can be individualised according to the disease/ diseases accompanying the aging and the patient’s biological age.
Contraindications Gerovital-H3® should be avoided by anyone suffering or utilising the following; • Allergy or sensitivity to Gerovital-H3® (or Novocaine). • Gerovital-H3® cannot be used together with eserine or prostigmine. • Gerovital-H3® cannot be used at the same time as sulphamides. • Gerovital-H3® should not be used with an antibacterial treatment.
Side effects Gerovital-H3® side effects are uncommon. With injections minor side effects consist of a heating sensation and metallic taste; these effects disappear during the treatment. Other side effects are related to the skin; macular eruption, rash and itch-
ing which determine the interruption of the treatment and the remake of the tolerance test (1 ml injected id.) after the eruption has disappeared. In spite of its monoamine oxidase (MAO) inhibitor character, Gerovital-H3® does not interfere, as the conventional MAO inhibitors do, with the tyramine from food (commonly called the cheese affect), which means there is no known incompatibility with Gerovital-H3® and food.

The Hair Growth Factors By Leslie J. Farer
The goal of an antiaging regimen is to increase life-span by promoting a youthful physiology on the inside, but for most, a vibrant outward appearance is equally important. Avoiding excess weight, keeping in shape, and discouraging wrinkles by maintaining skin quality are all essential for a years-younger exterior, and, as we will discuss in this article, so is reducing or eliminating hair loss. Hair ─ its style, color, condition, and amount ─ is a central component to one’s overall appearance and even one’s self-esteem. Thinning hair and a balding head do not have to be inevitable consequences of aging! Shedding up to a few dozen hairs a day is normal, but excessive loss, known as alopecia, is a cause for concern. There are many types of hair loss, the most common of which is androgenetic alopecia, which exhibits as male-pattern baldness in men (a receding hairline at the temples and balding at the top of the head) and female-pattern hair loss in women (a general diffuse thinning of the hair). As the name implies, androgenetic alopecia is androgen-dependent and heriditary, resulting in the atrophy or miniaturization of predisposed hair follicles and progressive decline in hair density. (1, 2) So let’s examine the micro-anatomy of the hair follicle and its key component, the dermal papilla.
Dermal papilla controls hair growth by producing cytokines in response to androgens The dermis, the second layer of the skin just under the outer layer, the epidermis, contains tens of thousands of tiny and complex structures, the hair follicles, which enclose the hair
root. At the base of each follicle is a bulb-like projection, the dermal papilla, surrounded by the hair matrix, a group of cells which divide to form the major structures of the hair fiber and root. The dermal papilla regulates the growth of hair, which in humans occurs in unsynchronized cycles, with each hair entering a phase of the cycle at a different time. Anagen is the stage of active hair growth; catagen is a short resting phase until telogen, when the hair falls out. Under normal circumstances, the cycle then repeats. During the anagen growth phase, stem cells migrate to the hair matrix and divide and differentiate in a process controlled by numerous cytokines produced by the dermal papillae. (3) (Cytokines are regulatory proteins involved in intercellular communication.) The synthesis and release of these cytokines is influenced by androgens which bind to receptors of the dermal papillae. (1, 3) Most released cytokines stimulate cell proliferation and hair growth; however, in response to particular androgens, and in certain areas, such as the sides and top of the head, some cytokines induce follicle atrophy, leading to hair thinning and eventual loss. (3) Of all androgens, the one that most negatively influences dermal papilla cells to produce follicle-shrinking cytokines is 5-α-dihydrotestosterone (DHT), which is synthesized from testosterone by the enzyme 5-α-reductase. (1, 3) The FDA-approved drug finasteride, (brand names Propecia® or Proscar®) inhibits a form of this enzyme, type 2 5-α-reductase, and prevents the conversion of testosterone to DHT. Daily oral administration of finasteride has been shown to be effective in the prevention and treatment of hair loss in men. (1, 3-5)

(19 Besides cytokines, growth factors also among bioactive Providing for the health of the molecules involved in hair cycle new hair regulation Other ingredients, while not directly influencing hair growth Three polypeptide growth factors are crucial to the regulation of the hair cycle: these are insulin-like growth factor 1 (IGF-1), vascular endothelial growth factor (VEGF), and basic fibroblast growth factor (bFGF). Both bFGF and IGF-1 potentiate the growth of hair follicles. (3, 6-10) In addition, IGF-1 prevents cell death (apoptosis) in many cell types, which may be important in promoting the anagen growth phase and delaying the catagen (apoptosisdriven) phase. (6) In a few fascinating studies, hair growth in humans with alopecia was promoted by inducing IGF-1 production. (7, 11) Interestingly, another way DHT impacts hair loss besides stimulating the production of alopeciainducing cytokines is by inhibiting IGF-1 production at the dermal papillae. In an animal study, mice administered DHT exhibited reduced expression of IGF-1, which resulted in a lower number of proliferating cells in hair follicles and decreased hair growth. (12) A third growth factor, VEGF, is essential for establishing a highly developed blood vessel network around the hair follicle dermal papillae during the anagen growth phase. Research has demonstrated that improved follicle vascularization, or formation of blood vessels, induces hair growth by increasing the size of follicles and hair shafts. (13-15) In fact, stimulation of VEGF expression to promote follicle vascularization is a likely mechanism of action of the hair-growth drug minoxidil. (15)
Additional novel ingredients: a potassium channel activator and an energy-enhancing amino acid Another truly innovative approach to youthful, radiant sAdditional agents such as niacin and L-carnitine stimulate potassium ion channels within the dermal papillae of hair follicles to induce hair growth. Potassium channels are poreforming proteins that control the flow of potassium ions across the cell membrane and are essential to the follicle’s function. Niacin (nicotinic acid) bears a structural resemblance to minoxidil, and another mechanism by which minoxidil positively impacts follicles besides stimulating VEGF production, as we have just seen, is through activation of their potassium channels. (2, 15-18) The amino acid L-carnitine transports long-chain fatty acids across the mitochondrial membrane where they undergo oxidation to produce energy. Researchers have shown that L-carnitine promotes hair growth in humans by increasing energy production in proliferating anagen hair matrix cells and 14

HA has the remarkable ability to bind large quantities of water, forming a thick, gelatinous substance that lubricates movable parts of the body, while also keeping skin and follicle-containing dermal layer of the scalp hydrated. A nourished, moisturized scalp encourages the production of healthy, lustrous hair.
• Dexpanthenol: The alcohol analog of the B vitamin pantothenic acid, is important in stimulating skin regeneration. When used topically, dexpanthenol has been shown to penetrate the skin, reduce water loss, moisturize, and maintain skin softness and elasticity. Dear Dr Dean… Q. I am very interested in using the Can-C™ eye-drops to help fight my cataract- but how soon might I see positive results and what may I do in addition to speed things along? - R.M.F., Scotland A: In my experience, 3-6 months is the minimum time to see improvement with Can-C™, although I have known people who have persisted, and did not see improvement until over one year had elapsed. Of course, the time required depends on the person’s diligence with the drops, and the degree of progression of the cataract at the time the treatment, (2-drops twice a day) was begun. The formulators of Can-C™ eyedrops also designed Can-C Plus capsules, as an oral adjunct to the eye-drops. Note: Dr. Babizhayev, the principal Can-C™ researcher, doesn’t recommend lutein (found in many ‘eye’ formulas), as lutein competes with same receptors as N-acetyl carnosine, and could down-regulate efficacy of the drops. An alternative to Can-C™ capsules, would be to take L-carnosine itself, one of the principal (and I believe, most active) ingredients in Can-C™ Plus capsules. Additionally, you might consider the AGE-breaking aminoguanidine. Several Japanese research teams have demonstrated the ability of aminoguanidine to inhibit or completely prevent the development of cataracts in two diabetes-prone rat strains. The first team used the Shumiya cataract rat (SCR). SCR is a hereditary cataract model in which lens opacity appears spontaneously in the nuclear and perinuclear portions at 11-12 weeks of age. The researchers found that oral administration of aminoguanidine (AG)
strongly inhibited the development of lens opacification in SCR rats. In another very recent study, the researchers used another strain, Spontaneously Diabetic Torii (SDT) rats. SDT rats have marked hyperglycemia and severe ocular complications. They evaluated the effect of the anti-AGE agent, aminoguanidine, on the development of diabetic retinopathy (DR) and cataract. Five SDT rats were treated with aminoguanidine (0.5 g/L in drinking water). During the duration of the study, ALL of the control rats developed cataracts, whereas NONE of the aminoguanidine-treated rats developed cataracts. These results were dramatically illustrated in the accompanying photographs (Figure 1). The researchers concluded that aminoguanidine had a strong inhibitory effect on diabetic ocular complications in SDT rats. Although I have unfortunately not found any human studies to corroborate these findings in rats, aminoguanidine has been shown to be safe and effective in humans for a number of conditions, and I would expect it to have the same cataractinhibiting effect in humans as it has in rats. 1. was awarded in 1998 for its discovery. Unfortunately as we age, we lose our ability to produce NO. This puts us at risk for a host of conditions, including hypertension, atherosclerosis, myocardial infarction (i.e., heart attacks), stroke, Alzheimer’s disease, and erectile dysfunction. One way to increase NO is to consume the nitric oxideproducing amino acid, arginine. The effect of oral arginine intake to improve erectile response was first investigated in animal models. Older rats supplemented with arginine resulted in increased nitric oxide synthase activity and increased nitric oxide levels, which caused improved erectile response. The authors of the study suggested that dietary supplementation with arginine might lead to improved sexual performance in humans, as well. The first evidence that oral supplementation with arginine might lead to improvement in human sexual function came from a short-term study in 1994. The study showed a positive correlation between arginine supplementation and sexual performance and satisfaction.

Q. I am a 48 year old male and over the last couple of years have noticed that my erections are becoming weaker, to the point where, although not diagnosed, I am sure that I have erectile dysfunction. I don’t really want to become reliant on taking a Viagra® pill every time my wife and I have sex, what can you recommend Dr. Dean and what may address the cause of this problem rather than just the symptoms? - S.S.K., Canada A: First, I suggest that you ask your physician to check your testosterone levels. If they are in the low or low-normal range, you might ask him to prescribe testosterone. I prefer injectable testosterone (cypionate or enanthate) usually 200 mg injected intramuscularly every 2-3 weeks. Alternatively, the topical creams or gels may also help, although I believe topical testosterone is not as effective as the injectable route. That may solve your problem.
These intriguing findings were supported by a more recent, larger study conducted at the Department of Urology and Nephrology at Tel Aviv University in Israel. Fifty men with confirmed erectile dysfunction were administered either a placebo or 5 g/day arginine for six weeks. Thirty-one percent of the subjects receiving arginine reported improvement and satisfaction in their sexual performance. In the placebo group, only 12% of the men noted improvement in sexual function. Importantly, at the 5 gram daily dose, no significant side effects occurred in any of the patients throughout the entire duration of the study. The percentage of patients reporting improved performance (31%) may not sound like an impressive number, but keep in mind that up to 50% of Viagra® users report no benefit from taking this prescription medication. As a source of arginine, IAS offers Nitric-Pro™ powder, which contains a hefty 3500 mg of L-arginine plus magnesium (to help relax the smooth muscle layer of the endothelium), per serving. Unfortunately, the intestinal enzyme, arginase reduces the effectiveness of the conversion of oral arginine to NO by metabolizing arginine, reducing the amount that is available for conversion to NO. Citrulline may offer a solution. Citrulline is a semi-essential amino acid that is not found in the diet- it is synthesized in the body via a mechanism called the urea cycle. In this metabolic pathway, arginine is catalyzed by the enzyme arginase to urea and ornithine. Ornithine, in turn, is

converted to citrulline, which is converted to argininosuccinate, which is converted back to arginine, completing the cycle. It has recently been shown that oral citrulline supplementation increases the plasma arginine concentration and augments NOdependent signaling in a dose-dependent manner. This provides the rationale for using oral citrulline supplementation as a donor for the arginine/NO pathway in those with ED. An article in Urology described a pilot trial aimed to determine whether oral citrulline supplementation could improve erection hardness in patients with mild ED. In the Urology study, 24 men (mean age of 56.5 ± 9.8 years) with mild ED (erection hardness score [EHS] of 3) received a placebo for 1 month. They were then given citrulline, 1.5 g/day split into two doses, for another month. At the end of the study, the erection hardness score, number of intercourses per month, treatment satisfaction, and adverse events were recorded.
In the first group (n = 20), all patients regained spontaneous and sufficient erections after six months of oral GBE treatment. Arterial flow rates were actually improved after three months and continued to improve at six months. Rigidities at the penile tip and base were significantly improved after six months and remained constant during the nine month duration of the study. In the second group (n = 30) (this was the group that did not respond to high-dose papaverine injections—i.e., a very severe state of ED), improved arterial penile flow rates and rigidities were noted at six and nine months. Nineteen patients, responded to intracavernous PGE1 following GBE treatment. Of these 19 patients, 9 required a minimal dose of 5 mcg PGE1, while the other 11 required a maximal dose of 20 mcg. The remaining 11 patients remained impotent. Thus, it appears that Ginkgo biloba extract at a dose of 240 mg per day may also greatly assist those with ED.
The improvement in the EHS from 3 (mild ED) to 4 (normal erectile function) occurred in 2 (8.3%) of the 24 men when taking placebo during the first half of the study. But when the subjects were switched to citrulline, 12 (50%) of the 24 men reported achieving an EHS of 4. An EHS of 3 is considered “hard enough for penetration but not completely hard,” while an EHS 4 is “completely hard and fully rigid.” citrulline increased hardness by 50%. At the same time, the mean number of intercourses per month increased from 1.37 at baseline to 1.53 at the end of the placebo phase (about 12% greater frequency) and 2.3 at the end of the treatment phase (about a 68% increase) for citrulline. Citrulline increased successful intercourse connects by 56% more than placebo, and all patients reporting an EHS improvement from 3 to 4 reported being very satisfied.
In summary, I recommend evaluation of testosterone levels, and replacement, if appropriate, or using Andro-Pro™, arginine (3-5 grams per day) or the arginine-containing NitricPro™, citrulline (1.5 grams per day), Neo40® (1 tab twice daily), and Ginkgo biloba (240 mg per day). I don’t recommend starting everything at one time—rather, begin with one nutrient at a time, and add other nutrients in sequence so you can tell what is (and is not) working.
Perhaps the most effective way to increase nitric oxide has recently become available. It is a patent-pending combination of natural products, developed at the University of Texas Health Science Center, called Neo40®. Neo40® is primarily a combination of nitrate-rich beet root, and nitrate reductase activity-enhancing Hawthorn Berry extract, plus a relatively small amount of citrulline. Preliminary clinical studies at the Houston Institute for Clinical Research indicate that Neo40® has demonstrated clinically relevant increases in NO and serum nitrate concentration, improvement in triglyceride levels, and normalization of blood pressure in hypertensive patients. Anecdotal reports from my patients who have used Neo40® also include rapid relief from angina attacks, and Viagra-like enhancement of sexual performance. Another approach to improving erectile dysfunction is the timehonored multi-purpose herb, Ginkgo biloba extract (GBE). In a 1991 study, fifty patients with proven arterial erectile impotence were treated with 240 mg of GBE daily for nine months. The patients were divided into two groups. The first group had achieved sufficient erections with intracavernous papaverine injections before beginning treatment with GBE. The second group had not achieved sufficient erections with high-dose papaverine injections.

I am a 55 year old man. - F.M., Missouri A: Although there are several nutrients that may lower blood pressure, the combination of high blood pressure with increased heart rate is best treated with a beta blocking antihypertensive medication. Propranolol (Inderal®) is good, although cardio-selective beta blockers like Atenolol®, Labetalol®, Carvedolol® or Metoprolol® are also excellent. These medications act to slow the heart rate, (I prefer a resting rate of 65-75 BPM), while simultaneously normalizing blood pressure. Start with low doses, and gradually increase the dose until the target heart rate is obtained. Naturally, please discuss these recommendations with your physician. I recommend first normalizing your pressure (and heart rate) with one of these medications. You may also add a combination of potassium and magnesium for longer term use, which may enable you to wean yourself from the medication. I often prescribe magnesium-potassium aspartate as a source for these two minerals. The dose can be gradually titrated up to bowel tolerance (magnesium in high enough doses will cause GI distress and diarrhea)—but before that point is reached, blood pressure will usually return to normal. In addition to monitoring your blood pressure, another useful parameter to follow is your Arterial Stiffness Index (ASI). ASI is one of the best biomarkers of biological aging, as documented in my book, Biological Aging Measurement— Clinical Applications. At the time the book was published (1988), only the most sophisticated cardiovascular testing labs had the expensive equipment required to conduct this test. In recent years, the folks at IAS discovered a compact, computerized unit that was affordable for Doctors’ offices (BioClip™, $799.99), and have now added a personal unit suitable for home use (Bio-Cuff™-$124.99). Q. As I have gotten older I find that my sleep quality has appeared to have worsened. Now as a 70-year old woman I find that it’s not just the quality of the sleep but the time also. I am now at the point where I seem to be sleeping on average about 4 hours a night. What procedures can I follow to improve this and are there any sleeping drugs you recommend? - G.M., Australia A: In Aging Matters™, Issue #1, 2013, I answered a similar question, and suggested the anti-seizure drug, gabapentin (Neurontin®), as a powerful sleep aid. Gabapentin is an analog of the ‘notorious’ GHB. GHB is actually the safest, most nontoxic, non-habit forming sleep inducing agent there is. It is a natural substance, existing in every cell in our bodies in small amounts. When taken in adequate doses, it induces sound, restful sleep—enhancing all four phases of sleep—and is metabolized into harmless carbon dioxide and water. My experience with gabapentin as a sleep inducing agent is excellent. Although gabapentin doesn’t work for everyone, most of my patients gratifyingly report that it causes sound, restful sleep, without frequent early-morning awakenings. When taken in an appropriate dose, at the proper time, there 20
is little to no ‘hangover’ effect. If you experience a hangover, take it an hour or so earlier, or reduce the dosage. Dosages of gabapentin must be individualized. Some very sensitive people find as little as 300 mg to be adequate. Usually, however, 600800 mgs are effective and some highly resistant patients require 900-1200 mg. It is a very safe, non-habit forming medication. IAS now stocks gabapentin. Q. I am a 40-year old woman, in otherwise good health and fit. But I have recurring skin problems, specifically acne and boils which not only appear on my face, but other parts of my body such as my back. Obviously this is not a pleasant experience for me, I have tried using fiber and vitamin C to help detox myself with some limited success, but I would welcome your advice. - N.M., England A: Of course, I’m at a disadvantage in not being able to ‘eyeball’ your lesions. Skin lesions as you describe could be due to Rosacea, acne, or even by infectious microbes like staphylococcal bacteria. With that caveat, here are some general suggestions. Vitamin D is essential for healthy skin, and a healthy immune system. Most of my patients are deficient in this essential vitamin. I like to see levels in the range of 60-70 ng/ml. To reach these levels usually requires supplementation of 5,00010,000 IU per day. IAS has a very potent form of Vitamin D, D3-Pro, in a dose of 50,000 IU, these capsules should be taken once per week. [Ed- IAS now also stocks D3-5000 which are 5000 IU capsules). Tetracycline or doxycycline may help. The usual dose of doxycycline is 100 mg twice per day. You may have to continue to take smaller ‘suppressive’ doses on a chronic basis, if you find that this will control your outbreaks. Another suggestion is to purchase a bottle of ‘hand sanitizer’—basically an ethyl alcohol gel, and apply it topically to the skin several times daily. This will kill bacteria on the skin, and should help to ‘dry up’ your lesions. If you have acne, you may apply Retin-A® topically, as well—although if you are truly having ‘boils’ (i.e., abscesses), Retin-A® will probably not be effective. Finally, have you had your hormone levels tested? Excess testosterone (as can occur if you are taking DHEA—women very efficiently convert DHEA to testosterone) is known to cause acne in women. If you are taking DHEA, I’d suggest reducing your dose.

Deprenyl (selegiline) Parkinson’s disease is characterised by the loss of the brain neurotransmitter dopamine and deprenyl helps to prevent this loss by acting on the specific enzyme MAO-b. Professor Knoll also believes that deprenyl very significantly increases PEA (phenylethylamine) and that this may have an even more significant function that the MAO-b inhibition, (for more details on this please read Professor Knoll’s excellent book on the subject- ‘the brain and its self’). Hydergine (ergoloid mesylate) Hydergine is an extract of a fungus that grows on rye called ergoloid mesylate and its approved uses have been to aid persons with memory disturbances, Alzheimer’s disease and even for persons who have suffered from shock related to electric, drowning or suffocation. This is because hydergine is an excellent provider of stabilising the levels of oxygen in the brain and has been shown to improve levels when they are too low and even decrease levels when they are too high. The result of this optimised oxygen availability in the brain is an ability to do more mental workload without becoming tired. Accordingly, hydergine can be a good tool to battle through boring paperwork or a long drive home in the dark. A fascinating animal experiment showed a special ability for hydergine to improve mitochondrial condition. The mitochondria are vital organelles found in every cell of our body responsible 26
for the generation of ATP, the universal energy molecule. Unfortunately mitochondrial efficiency declines as we age and this has been linked to disrupted repair mechanisms. However an Italian study highlighted those old animal mitochondria can be improved with hydergine supplementation. Hydergine has also been attributed to stimulate the growth of dendrite nerve fibers, since these connective strands are known to decline in aging this is another antiaging feature of hydergine. Some have even linked the density and numbers of dendrite fibers to the level of I.Q. Melatonin (MZS™) Melatonin is one of nature’s remarkable substances and made in response to darkness by the pineal gland. Its benefits to help overcome jet lag and shift work have become quite well known and indeed it is useful for this purpose since adequate melatonin availability enables our bodies to be correctly in tune with our circadian rhythms, these rhythms dictate our hormonal cyclicity and this cyclicity ultimately determine our aging and our level of immunity. Unfortunately melatonin secretions decline with aging. Professor Pierpaoli has published widely on melatonin, principally through the New York Academy of Science and at the meetings of the Stromboli conferences on aging and cancer- which IAS is delighted to support. What is important to know about melatonin is that Professor Pierpaoli advocates a 3mg dose to (his words) “put the pineal to sleep.” In addition, it is also vital to use a melatonin that produces a night peak of melatonin that is the same as the peak produced by the pineal itself. Here are some before and after Fundus photos showing the benefit of Melatonin Zn Se. They highlight dramatic improvements to both wet and dry forms of macular degeneration. Naltrexone The results of low dose naltrexone, (widely known as LDN) therapy have been remarkable. Originally, the use of naltrexone was in the management of dependence on opioid and alcohol. But it was Dr. Bernard Bihari in the 1980’s who first used low dose naltrexone for the treatment of HIV and then for a number of other autoimmune diseases such as multiple sclerosis, Crohn’s disease and many immunological related diseases, and especially in the treatment of MS, where patients have reported an improved quality of life. A leading study has established that by maintaining regular nightly LDN, less than 1% of the MS sufferers experience fresh MS attacks. IAS supplies the LDN dose of 4.5 mg.

Progesterone Progesterone is often the ‘missing’ hormone in the treatment of female menopause. What we mean by this is that whilst estrogen treatments, (see Esnatri™ for details) are often considered as a primary replacement hormone for women of that certain age, progesterone is often forgotten about. The best known action of progesterone is its ability to help build bone mass and since progesterone levels fall rapidly in menopause it contributes to osteoporosis in elderly women. Progesterone is strongly advocated to be taken in combination with any estrogen replacement program.

Sermorelin

Sermorelin is a breakthrough development in the field of growth hormone (GH) replacement, for sermorelin is the bioidentical molecule used by the pituitary gland to help stimulate production and release of GH into the bloodstream. Sermorelin is not GH; GH is a complicated molecule consisting of 191 amino acids, but rather sermorelin is a 29 chain of aminoacids, but importantly it is the first 29 aminoacids of GH, the part responsible for activation.
