WELCOME Our lead is about the mTOR inhibitors; they have become a hot topic in antiaging medicine, ever since rapamycin was discovered in the soils of Easter Island (which explains the cover). So we invited Dr. Marios Kyriazis, one of Britain’s leading antiaging physicians to detail this subject for us. A new class of anti-aging drugs discovered A brand new class of anti-aging drugs has been identified in a study from The Scripps Research Institute and the Mayo Clinic. So far the group have reported that the drugs, which the researchers have dubbed “senolytics,” have reduced frailty, improved heart function and extended the healthy lifespan of mice. Senolytics work by causing senescent cells to die, senescent cells are cells that have stopped dividing or have experienced DNA damage. These senescent cells include normal differentiated cells and stem cells. If left these cells secrete harmful substances that damage other cells, including stem cells, said study contributor Laura J. Niedernhofer, a Scripps Research scientist. Since senescent cells show some biochemical similarity to cancer cells, except for cell division, the researchers looked for cancer drugs that could selectively cause senescent cells to die.

So far the substances with these properties that have been identified are dasatinib, sold under the brand name Sprycel; and quercetin, the nutritional supplement with antihistamine and anti-inflammatory properties. Dasatinib killed senescent progenitor human fat cells, while quercetin was more effective against other cell types in humans and mice. Giving both drugs at the same time caused more senescent cells to die in cell cultures, and stronger anti-aging effects in the mice. Perhaps equally exciting is the fact that since these substances are already approved for human use, researchers say clinical trials should be relatively quick and easy to arrange. Now that this line of research has been identified the team are also confident that even stronger drugs may be discovered.

FOREFRONT Fluoride in drinking water linked to hypothyroidism A new study from England has found that fluoride added to drinking water is associated with a 30% higher rate of diagnosed hypothyroidism. The actual rate of under-active thyroid may be much higher since this study was only limited to people who were diagnosed. Many more may suffer with impaired thyroid function and not receive necessary treatment. In the study researchers looked at 2012 levels of

fluoride in the drinking water supply in England. About 10% of the English population or six million people live in areas with naturally or artificially fluoridated water containing 1 milligram of fluoride per liter of drinking water. They found that in areas with fluoride levels above 0.3 mg of fluoride per liter of water, rates of hypothyroidism were 30% higher.

Hypothyroidism or low thyroid function causes fatigue, weight gain, constipation, aches, dry skin, thinning hair, and intolerance to cold. It can also cause slowed heart rate, depression, and impaired memory. In addition to hypothyroidism, other studies have suggested fluoride has links to other serious health conditions such as heart disease, low IQs and cancer.

Garlic protects from more than just vampires Researchers from The University of Missouri have identified a nutrient in garlic which offers the brain protection against ageing and disease. They report it could even be used to prevent Alzheimer’s and Parkinson’s. The University of Missouri researchers found a carbohydrate celled FruArg is the key. ‘Garlic is one of the most

widely consumed dietary supplements,’ said Zezong Gu, associate professor of pathology and anatomical sciences at the MU School of Medicine and lead author of the study. ‘Most people think of it as a ‘superfood,’ because garlic’s sulfur-containing compounds are known as an excellent source of antioxidant and anti-inflammatory protection.

The team created a cell model of neurological stress and monitored microglial cell function, in order to study FruArg’s contribution to brain health. ‘When stress was applied to the model, there was an expected increase in microglial cells and their byproduct, nitric oxide,’ Gu said ‘However, once we applied FruArg, the

microglial cells adapted to the stress by reducing the amount of nitric oxide they produced. ‘This helps us understand how garlic benefits the brain by making it more resilient to the stress and inflammation associated with neurological diseases and aging.’

When it comes to regulating aging, an important molecule which has widespread effects is mTOR. mTOR is the acronym for the Mechanistic (previously also called ‘mammalian’) Target of Rapamycin... Rapamycin (also known as sirolimus) is a protein kinase anti-fungal agent that was first discovered in the soils of Easter Island in the 1970s, which has been found to regulate the proliferation and development of cells, and to act as an immunosuppressant. It has been used to dampen the immune response following transplants. Rapamycin also has a positive effect on the heart and brain, particularly in age-related neurodegenerative diseases. mTOR acts as a central controller in signalling pathways which regulate the survival and function of the cell. It can affect cell division processes, the response to stress, and general cell and protein tasks. Figure 1: The impact of mTOR on various aspects of aging.

These signalling pathways are very complicated and depend on many feedback loops, energy supply, and a great variety of other molecules or signals. During certain diseases and aging, the function of mTOR can become deregulated and result in clinical manifestations, [see figure 1]. Despite talking about mTOR as if it is just one molecule, it functions, in fact, as two separate complexes called mTORC1 and mTORC2. (1) Several compounds have now been developed (or discovered) which inhibit some of the actions of mTOR and so improve certain health parameters. Rapamycin and its analogues inhibit the immune response by blocking the proliferation of T-cells and by reducing the impact of interleukin inflammato-

ry compounds. They also block the formation of new blood vessels (angiogenesis) which results in cancer cells being deprived of their blood supply. Other mTOR inhibitors have been developed, but most of these are based on the basic rapamycin molecular structure. For example, the rapamycin derivative temsirolimus is a soluble agent which has a reduced risk of side effects compared to rapamycin. Since it is water soluble, it can be administered by intravenous infusion. It is approved by the FDA for the treatment of kidney tumours. Other derivative molecules (called rapalogs, i.e. ‘rapamycin analogues’) are: • Everolimus (approved initially for the treatment of kidney tumours, and more recently for the treatment of breast and pancreatic tumours). • Ridaforolimus (also known as Deforolimus) is used for breast and other cancers.

These drugs bind to the same sites as rapamycin, and are considered to be generally better inhibitors. They can be used orally, as well as by intravenous injection. They work by stopping the growth of some cancers, but are not very effective in all cancers, particularly if used alone. One important mechanism of action of these rapalogs is through competition for the ATP molecule, thus blocking energy supply to tumour cells. Many rapalogs also interfere with apoptosis (loss of damaged cells). This results in increased apoptosis of cancer cells, thereby reducing the size of the tumour. These drugs inhibit both mTORC1 and mTORC2 elements, [see fig 2].

Although initially promising, these rapalogs may be more effective if used in association with other tumour-suppressing agents. However, it remains unclear as to what specific tumours to use these agents on, because it all depends on what markers each tumour carries and how sensitive these markers are. Nevertheless, research is continuing at a quick pace. Apart from synthetic inhibitors of mTOR, there are certain natural ones. The following are some examples which are also used for their antiaging benefits and effects.

Epigallocatechin gallate: Also known as EGCG, this is a well-known natural compound (a component of green tea), with multiple benefits. It can disrupt the mTOR pathway and is effective in inducing apoptosis of human pancreatic cells in the laboratory.(2) The researchers stated: “Our results revealed that EGCG dramatically inhibited the proliferation of (pancreatic cancer) cells and induced apoptosis simultaneously. Furthermore, it upregulated (improved) mRNA and protein expression levels, as well as downregulated the expression of phospho-AKT and phospho-mTOR”.

Caffeine: Caffeine can induce apoptosis of cancer cells through modulation of the p53 element which takes active part in apoptosis. Caffeine can also

induce autophagy. This can be achieved through inhibition of the mTOR pathway, so a possible benefit associated with this is an anti-cancer activity.(3) In this regard, caffeine can be used in addition to chemotherapy in bone and soft tissue tumours. Caffeine affects mTOR and other protein kinases, and reduces inflammation markers.(4)

Curcumin: This ingredient of turmeric can be used with other established chemotherapy drugs in order to enhance their action, particularly in difficult-to-treat tumours such as glioblastoma in the brain. The mechanism is rooted in the ability of curcumin to block the mTOR pathway and thus enhance apoptosis of cancerous cells. (5) Curcumin down-regulates the mTOR signalling pathway, reduces the accumulation of beta-amyloid (the aggregate found in Alzheimer’s dementia) and thus improves cognitive function in mice. This effect is also due to curcumin’s ability to induce autophagy, i.e. degradation of abnormal protein accumulation. It can therefore be used as a neuroprotective agent during aging. (6)

Resveratrol: The case of resveratrol is particularly interesting. Resveratrol can inhibit certain cancers, and recent research has confirmed that one of the mechanisms involved is

mTOR inhibition, as well as modulation of apoptosis. The same mechanism may underlie resveratrol’s antiaging effects. (9) Resveratrol and rapamycin can enhance each other’s actions. At a very high dose, resveratrol can suppress aging through its mTOR inhibition mechanism alone. It is well known that apart from its effects on cancer, mTOR also has several effects on the aging process. (10) This may explain resveratrol’s multiple actions and benefits.

Other phytochemicals: All of the following have been found to inhibit the mTOR pathway: (7) timosaponin; genistein; diosgenin; gallic acid; and pomegranate. In a review paper published in 2011, it was reported that these compounds, through an inhibition of the mTOR pathway, enhance reprogramming of pluripotent stem cells. This shows that there are common pathways in cell regulation which can be affected by modulation of mTOR and can eventually affect aging itself. (8) Several experiments in mice and other animals have shown that the mTOR molecule can affect both the average and maximum lifespan. (11) [See figure 3].

Oxaloacetate - a most promising compound Oxaloacetate is an organic compound which takes part in several metabolic processes such as synthesis of amino acids and fatty acids; it also regulates mitochondrial and brain function. Among its other benefits, it is an antioxidant, brain function-enhancer, and a modulator of certain age-related genes such as FOXO. Plus, oxaloacetate is also an effective mTOR modulator. In fact, oxaloacetate may also be considered to be an mTOR mimetic, which means that it has benefits similar to other mTOR inhibitors (such as modulation of apoptosis and reduction of glutamate).

As mentioned above, agents such as rapamycin work by improving signalling between the cells. An example of these pathways is the regulation of the Growth Hormone/insulin/IGF-1 pathway, inactivation of which has been shown to increase lifespan in the laboratory. (12) In a systematic review of the top 10 antiaging interventions being studied today, mTOR interventions were classed second in importance, with calorie restriction being first, and nutrients such as resveratrol being third. However, it has additional benefits that originate from mechanisms other than mTOR inhibition. (14) Oxaloacetate is a wellknown calorie restriction mimetic, and can even be considered as an exercise-mimetic [Ed. - we have asked Dr. Kyriazis to write about exercise mimetics for a future issue of the Aging Matters™ magazine]. This is because it can help increase production of energy and improve muscle endurance. Supplementation with oxaloacetate increases lifespan in C. elegans worms. This is dependent on the transcription factor FOXO, as mentioned above, but also on protein kinases which depend

on adenosine monophosphate (AMP)--which is involved in energy production. (15) Perhaps an interesting situation here is to realize that mechanisms for aging regulation, energy production and mTOR regulation are all interdependent, and one compound may have multiple effects across many regulatory pathways. Carnosine was another good example of this--but now research supports oxaloacetate as another pluripotent agent.

Summary It is clear that aging is very dependent upon mTOR-regulated pathways and processes, and that several agents acting on these pathways have direct

or indirect benefits. Several pathways involved in cancer can be regulated by mTOR inhibitors, and many of these pathways are also involved in aging. It is logical to assume that mTOR inhibitors have a positive benefit on aging. Some antiaging physicians

and scientists are recommending the more natural molecules involved in mTOR inhibition (such as resveratrol, oxaloacetate or curcumin) be added to our anti-aging regimens as a way of addressing age-related health problems.

By Dr. Richard Lippman Mark had a medical problem, which he embarrassingly referred to as a ‘senior slump,’ but he refused to visit a doctor because he considered them to be quacks. Thus, he continued to suffer poor erections despite self-dosing with copious amounts of Viagra® and Cialis. Eventually, Mark’s wife convinced him to see me and I meticulously questioned and examined him. Mark’s blood pressure was low and he was dehydrated. He urinated frequently at night. I eventually learned that Mark suffered a three centimeter laceration of a finger due to an accident with a power mower, and unexpectedly bled profusely from what appeared to be a minor

injury. His blood simply would not coagulate. Mark was often thirsty at night, and felt compelled to urinate frequently. Worse still, he had memory problems and complained about not ‘thinking straight’ anymore. In my office, his blood pressure (BP) measured 95 over 60—which is fine for a highly trained athlete, but a bit low for a deconditioned 47 year-old man. Along with low BP, he

felt that he had lost energy compared to his younger self.

Antiaging medicine to the rescue Both vasopressin (ADH) and oxytocin are hormones made in parvoventricular and supraoptic nuclei cells in the hypothalamus, a small gland located in the brain (Fig. 1). Both hormones are transported inside axons, long nerve

endings of the neurons, to the posterior lobe of the pituitary gland where they are secreted into the bloodstream. (1) Vasopressin is essential for maintaining normal blood pressure, and insuring a sufficient amount of blood in an erect penis or an aroused clitoris. In his aging body, Mark appeared to be deficient in vasopressin as well as other hormones such as thyroid and oxytocin. I reasoned

that he was probably lacking oxytocin since the same cells in the hypothalamus produce both vasopressin and oxytocin, and a deficiency of one usually means a deficiency of the other. My meticulous examination revealed the following signs and symptoms of hypothyroidism: loss of the outer third of the eyebrow; swollen eyelids; dry, thick, brittle hair; swollen lips and tongue; dry, rough skin; cold hands and feet; excessive laxity of the fingers; brittle, slow-growing nails; constipation and abdominal bloating; thick, swollen calves and especially ankles; sluggish and slow thinking and moving; attention deficit; bradycardia and faint heart sounds; and slow Achilles tendon reflexes (2). I recommended that Mark start consuming 30 mg Armour® thyroid every morning without any caffeine. According to Dr. Thierry Hertoghe, caffeine interferes with Armour’s action (3). I further recommended that he add 10 units (one spray each) of vasopressin (Vaso-Pro™) and one 10 IU spray of oxytocin (Oxy-Pro™) at bedtime. I referred Mark to a well-known urologist for a digital rectal exam in order to ascertain if Mark’s frequent bathroom visits might be caused by BPH (benign prostate hyperplasia or enlarged prostate gland). BPH is an all-too-common problem in older men who produce

excessive amounts of the essentially female hormone estradiol, without balancing it with natural progesterone (Progesterone-Pro™). His urologist reported that Mark’s prostate was slightly enlarged but shouldn’t cause problems for many years to come. Thus, Mark began consuming three vital and deficient hormones (Armour®, vasopressin and oxytocin) daily.

Aftermath Most middle-aged or older people should maintain their aging bodies just as they maintain their aging cars. This was certainly the case with Mark, who only needed a little confidence in a competent antiaging-trained physician, who could readily diagnose a patient and arrive at needed remedies. In addition to the vasopressin and oxytocin, high on the list of needed remedies

applies a numbing cream (lidocaine) to the face and hands and waits an hour. Then, the physician will move a DermaPen™ pen over the surface of the skin. This electric pen contains 40 or 50 tiny needles that are often adjusted to penetrate the skin to a depth of 0.50 mm. On sensitive tissues such as the forehead and eyelids, the depth of the needles is set to only 0.25 millimeters. This pen punctures and opens the skin down to the dermal layers and allows improved blood flow to underlying tissues. During this process, slight bleeding and shortterm inflammation will occur. The body’s own stem cells are attracted to the site of injury, which, in turn, encourages new collagen growth. The Power of the Pineal Peptide By Leslie J. Farer

Back in the 1980’s, a colonel in the Soviet army Medical Corps was assigned a seemingly impossible, though intriguing task: to come up with a way to protect troops against the risks of modern military weapons, (such as a blinding battlefield laser or radiation exposure) while at the same time regenerate damaged organs and tissues in the event of injury. What he discovered, through experiments supported by the generous funding of the Soviet military, was that certain naturally-produced substances, later termed ‘peptide bioregulators,’ (that were isolated from various animal tissues and organs), have the capacity to act at the genetic level to regenerate tissue and restore the function of the corresponding organ(s) in other animals or in humans. As a byproduct of their rejuvenating properties, these biologically active short chains of amino acids were also discovered to act as so-called ‘geroprotectors’ (therapeutics that target the root causes of aging and age-related diseases), with impressive longevity-promoting potential. Once a military secret in the 1980’s, a wealth of subsequently-published research has demonstrated that peptide bioregulators restore physiological balance, prevent age-related diseases, increase life span and more. One in particular, the pineal bioregulator peptide called Epithalamin, is considered to be the most promising in its class of geroprotectors.

Let’s first examine the properties and actions of peptide bioregulators in general, and then the particular benefits of pineal peptide Epithalamin, in stemming the onset of aging and age-related diseases. EACH PEPTIDE RESTORES THE FUNCTION OF A SPECIFIC ORGAN In the early days of his research, the then Soviet colonel, Dr. Vladimir Khavinson, (who is currently the president of the International Association of Gerontology and Geriatrics and Head of the St. Petersburg Institute of Bioregulation Professor Vladimir and Gerontology), developed Khavinson a method for the isolation, purification and fractionation of low-molecular-weight peptides from extracts of different bovine-sourced organs and tissues such as the pineal gland, thymus, hypothalamus, retina, endocrine glands and liver etc. These amino-acid chains are endogenously produced in the cells of healthy tissues and play a regulatory role, from the molecular and cellular levels to the overall biochemical and physiological functioning of the particular organ in which they are manufactured (hence, the designation ‘peptide bioregulator’). Professor Khavinson analyzed over 15,000 genes

in mouse heart and brain tissue before and after the injection of various peptide extracts, (including the pineal peptide Epithalamin) and identified the mechanism of ‘peptide regulation of genetic activity’ whereby each peptide interacts with DNA, transferring information encoded in its amino acid sequence to regulate particular genes in a particular tissue. (1,2)

“Trials using peptide preparations obtained from other organs (i.e. prostate, cerebral cortex, adrenal gland, liver, etc.) have all demonstrated beneficial effects as evidenced by improvements in the condition of the respective organ.” Activation of these genes stimulates protein synthesis, (1,3,4) and the manufacture of proteins according to the encoded genetic information, (a crucial physiological process which we’ll examine further below). Short-chain peptides also have the capacity to induce cell proliferation and differentiation, (1,2,4) resulting in tissue repair and restoration of organ function. When Professor Khavinson administered peptide extracts to animals (and later humans), he found that they did indeed stimulate protein synthesis and produced regenerative effects in cells and tissues of the organ from which the particular peptide had originally been derived. (2) For example, retinal peptides (example, Visoluten®) initiate the formation of retinal and pigment cells in patients with retinal degeneration, (1,2) thymic peptides stimulate immunity, (1,5) and pineal peptides induce the aging pineal gland to manufacture melatonin. (1,5)

PEPTIDES INTERACT WITH GENES’ DNA TO ACTIVATE PROTEIN SYNTHESIS

The molecular model Professor Khavinson proposed, which was later demonstrated experimentally, involves ‘complementary binding’ or complexing of the amino acid sequence of a shortchain peptide with the nucleotide sequence on a gene’s DNA. (1,6,7) DNA is a long polymer chain composed of repeating units called nucleotides which, by their combination of nitrogen bases, create a ‘code’ or blueprint for the manufacture of proteins; each ‘code’ is specific for one of thousands of different proteins. examples of the ability of the pineal peptide- Epithalamin to ‘switch on’ genes to synthesize these essential substances to provide overall restorative effects. COUNTERING PEPTIDE DEFICIENCIES ASSOCIATED WITH AGING In a nutshell, the main point to digest here is that these biologically-active peptides act at the genetic level and possess the ability to recover functions of tissues and organs undergoing age-related deterioration. Aging is characterized by a peptide-dependent downward spiral: changes in gene expression accompanied by decreased endogenous peptide production lead to disorders in protein synthesis, in turn causing structural and functional deterioration of various organs (pineal gland, liver, pancreas, thymus, retina, nervous, immune and endocrine systems) that result in aging, and age-related diseases. (1,2,4,5,6,8) Thankfully, this negative cascade can be ameliorated, or even reversed, through the delivery of the appropriate peptide(s) which have been shown to exhibit geroprotective, or antiaging properties. (Professor Khavinson refers to the process of peptide administration to

animals or humans the ‘epigenetic regulation of aging.’ (6) STUDIES ON THE PINEAL PEPTIDE SHOW MULTIFACETED REGENERATIVE EFFECTS In the late 1990’s Professor Khavinson began manufacturing bioregulators synthetically, based upon the amino acid composition of peptide extracts isolated from animal tissues. These laboratory-formulated peptides, consisting of two to four amino acids, were shown to possess equivalent biological activity and safety to the animal-derived extracts. (1-3) Epithalamin is expressed as Epithalon®, a synthetic tetrapeptide- Ala-Glu-AspGly (alanine-glutamic acid-aspartic acid-glycine), modeled on the amino acid sequence of Epithalamin, a polypeptide complex isolated from the bovine pineal gland and also as Endoluten®, (see figure 2 for explanation). Epithalamin been shown to regulate pineal gland function and increase melatonin production, improve immunological parameters, stimulate antioxidant and anti-cancer defenses, restore reproductive function, increase lifespan in animals and provide geroprotective effects in humans. gland to restore youthful melatonin production, (figure 2) but has many other restorative effects on age-related endocrine dysfunction. For example, in aging rhesus monkeys receiving Epithalon® injections, night-time melatonin production increased and the normal daily pattern of cortisol secretion was restored, for an overall normalizing effect on circadian rhythms. (8) In another experiment on old rhesus monkeys, Epithalon® administration decreased glucose and insulin levels (both of which typically rise with age due to altered function of the pancreas) and as in the previous trial, increased night melatonin levels. (9) (See figure 3). Interestingly, Epithalamin also has positive effects on vision: it protects the functional integrity of the retina in rats (10) and improves visual function in patients with retinal degeneration in an impressive 90% of cases! (5,11) The restorative effects on both the pineal gland and retina probably reflect the common embryological origin of the two structures. Epithalamin also has positive effects on the age-related decline of the immune system by activating the proliferation of lymphocytes (a type of white blood cell) in the thymus. (12) It has also been shown to complex with the nucleotide sequence of the interferon gamma gene, activating interferon gamma production. (12) This is an excellent example of the molecular model of peptide-DNA bonding we reviewed earlier that results in the manufacture of a critical protein- in this case, in-

terferon gamma, which is a cytokine, or chemical messenger, involved in the regulation of nearly all phases of immune and inflammatory responses. Several trials have demonstrated the anti-carcinogenic effects of Epithalamin in countering genetic mutations and the development of malignant tumors. For example, Epithalon® treatment decreased the incidence of chromosome aberrations which can lead to tumors in senescence-accelerated mice, (mice which are bred as models of rapid aging), (13) and suppressed breast cancer in mice (14) and colon cancer in rats. (15) THE PINEAL PEPTIDE EXTENDS TELOMERE LENGTH Perhaps even more compelling, Epithalamin activates the telomerase gene and lengthens telomeres - (see figure 3) further evidence of its longevity-promoting potential. Aging is associated with a decrease in the proliferative capacity of cells (i.e., the ability of cells to divide, necessary for the construction and repair of tissues) due to the progressive shortening of structures called telomeres. Telomeres are the protective ‘caps’ at the ends of chromosomes that maintain their structural integrity and telomerase is an enzyme that adds repetitive DNA sequences to the telomeric ends of chromosomes each time a cell divides (during cell division, portions of DNA are lost due to an inherent problem of incomplete end-replication; telomerase ensures the complete copying of DNA repeats).

Most of the body’s cells (excluding reproductive cells) can divide only a finite number of times according to a programmed genetic clock that results in decreased telomerase production with increasing age, leading to shortened telomeres and arrested cell division. This process, known as replicative senescence, ultimately leads to aging and age-related diseases. Khavinson found that the addition of Epithalamin to cultured human cells activated the telomerase gene to synthesize telomerase, resulting in lengthened telomeres and an increased number of cell divisions. (16, 17) This is another example of the molecular mechanism of peptide-DNA complexing which results in the synthesis of a protein, in this case the ‘immortalizing’ enzyme telomerase. According to Professor Khavinson; “these results indicate the possibility of prolonging the life span

of a cell population and of the whole organism” (16) - an intriguing and exciting prospect. All of these mechanisms - normalization of endocrine function, immune enhancement, cancer inhibition and telomerase activation - underlie the pineal peptide’s pronounced geroprotective, or antiaging effects. Studies specifically on longevity demonstrate increased life span in drosophila, (2,5) mice, (5) and rats (16) when administered Epithalamin and it follows that this effect is mimicked in humans. Undoubtedly, the peptide bioregulator is one of the most innovative and effective antiaging therapies developed in the past few decades. We can now take advantage of these fascinating physiologically-active short peptides - including the pineal version - to rejuvenate age-damaged organs and extend the quality and quantity of our senior years.

A. Dear Mr. A.M., One cause of foggy thinking and poor concentration can be subclinical (or overt) hypothyroidism. See the attached list of hypothyroid symptoms in figure 1, (adapted from Dr. Broda Barnes’ book, Hypothyroidism, the Unsuspected Illness). (1) If many of the symptoms apply,

you might consider treatment with Armour® thyroid, as outlined by Dr. Richard Wilkinson in his article; The Hypothyroid Epidemic, in issue #4, 2014 of the Aging Matters™ Magazine. A second suggestion is phenytoin (Dilantin®). Although phenytoin is a time-honored anti-seizure medication, one of its lesser-known properties is its ability to restore focus and attention. A series of studies by different investigators in the 1970s demonstrated the ability of phenytoin to enhance concentration and focus. In a double-blind study of 47 “retardates,” Goldburg and Kurland reported strong improvement in ability to maintain attention and concentration, using a dose of 100 mg of phenytoin twice daily.(2) Another study, using the same dose, reported phenytoin was useful for symptoms of lack of attention

and concentration.(3) A third double-blind study of the effects of 100 mg of phenytoin twice daily on the mental functions of elderly persons in the “normal-bright normal in the “middle and lower-upper socio-economic group” resulted in “significant improvement in long-term memory and social comprehension, increased ability to learn new material and increased speed in visual-motor coordination, improved general mental functioning and marked improvements in concentration.” (4) You might also consider one of the eugorics, or “alerting drugs.” Eugeroic means good arousal. Drugs in this class include modafinil (Provigil®), armodafinil (Nuvigil®), or the less expensive prototype, adrafinil (Adra-Pro®). These drugs are psychostimulants with a waking effect, prescribed to shift work-

ers and patients suffering from narcolepsy with cataplexy. Adrafinil was the first of this class, developed in France and still available in Europe, but which has never been approved in the U.S. Modafinil and armodafinil are analogs of adrafinil and are both available by prescription in the U.S. All of these drugs have mood-elevating and memory-enhancing effects, in addition to maintaining mental alertness. They are used “off label” to treat memory loss due to demen-

Q. Dear Dr. Dean, May I please have your opinion on the relevance of piracetam as a smart drug? Do you feel that it still ‘stands up’ despite being many decades old, and have any of the new nootropics surpassed it in anyway? S.F., California

A. Dear SF, That’s a very good question. As you said, piracetam has been around for a long time - since 1972. I first learned about piracetam in Korea, (where I graduated from medical school in 1978). At that time, carbon monoxide poisoning was a common occurrence, due to leakages from the Korean in-floor heating systems. The standard method of treatment was a combination of intravenous piracetam and hyperbaric oxygenation. Although piracetam has been available generically for many years, research into its properties for a wide range of clinical conditions has continued. In Hungary in 2000, a Phase IV clinical study of 104 patients with mild cognitive impairment-dementia participated in a double-blind

study of piracetam, 4800 mg/day for 4 weeks, followed by 2400 mg/day thereafter. Five factors of the modified Mini-Mental State examination were evaluated. Nearly all factors significantly improved with piracetam- especially memory, concentration and psychomotor speed. The cognitive enhancement effects appeared in a few weeks, and were most pronounced in patients with depressive symptoms. (1) Belgian scientists in 2002 conducted a meta-analysis of nineteen double-blind studies of patients suffering from dementia or age-related cognitive impairment treated with piracetam. The authors concluded that the treatment demonstrated meaningful clinical improvement, with compelling evidence for the global efficacy of piracetam. (2) In 2005, Polish scientists reviewed the documented benefits of piracetam, affirming that it improves alertness, learning, memory, brain metabolism and capacity. They reported that piracetam alters plasma membrane properties by increasing membrane fluidity and protects the cell against hypoxia, increases red cell deformability and normalizes hyperactive platelet aggregation, resulting in anti-thrombotic, neuroprotective and rheological properties. The scientists theorized that these properties resulted in the efficacy of piracetam in various disorders including dementia, vertigo, sickle cell anemia, myoclonus and stroke, improves reading comprehension and accuracy in dyslexic children, socialization and IQ in elderly psychiatric patients, and is even used to treat alcoholism. (3) Also in 2005, piracetam’s properties were reviewed by a scientist from the Karolinska Institute in Sweden, who echoed the above review from Poland.

(4) In 2014, a team of Chinese scientists conducted a meta-analysis of three studies of piracetam used preventively prior to coronary artery bypass grafting (CABG). (5) Cognitive dysfunction of varying degrees is an unfortunately too-common side effect of this procedure. Between 25 and 80% of patients who undergo CABG surgery develop postoperative cognitive dysfunction (POCD), ranging from slight to pronounced disturbances that may persist for several weeks up to several years. Three studies were identifiedtwo in Germany and one in Romania - in which piracetam was administered prior to surgery to prevent the resultant cognitive dysfunction. In the first trial in 2003 in Germany, physicians administered 12 gm piracetam (or saline, placebo) via IV infusion to 64 patients 3 hours prior to undergoing bypass surgery. The authors reported that the piracetam-treated patients produced a better level of performance in short-term memory and attention 3-days after surgery, compared to the placebo group. (6) In 2006, Romanian physicians administered piracetam or placebo intravenously (150 mg/ kg daily; 300 mg/kg on the day of surgery) to 98 patients, from the day before surgery to 6-days after surgery, then orally (12 g/ day) up to 6 weeks after surgery. The authors reported that six weeks after CABG, cognition was significantly improved in those treated with piracetam compared to placebo. (7) In 2008, another team of German physicians performed a third study of piracetam’s ability to prevent cognitive dysfunction due to bypass surgery. This study was the largest, comprised of 120 patients, each of whom

received 12 grams of piracetam or placebo just prior to the surgery. Again, the piracetam patients performed significantly better compared to the placebo patients after the operation, and had less decline of overall cognitive function. (8) From 2010-2013, a series of papers was produced by an international team including scientists from Germany, Italy, France, Switzerland, China, Australia and the U.S., who investigated the mitochondrial resuscitating mechanisms of piracetam as a further explanation of its neuroprotective properties. In the first paper in this series, the authors showed that piracetam prevented decreases in mitochondrial membrane potential (MMP) and fall in ATP production associated with β-amyloid accumulation, suggesting that improving mitochondrial function protected against the neurodegeneration induced by β-amyloid (Aβ) (involved in Alzheimer’s disease). The brains of animals treated with piracetam showed markedly decreased loads of Aβ. Aβ-induced mitochondrial dysfunction also causes impairment of neuritogenesis and the Aβ-induced reduction of neurite growth was substantially improved by piracetam. The authors concluded that piracetam’s ability to improve mitochondrial function was a valid approach to ameliorate the detrimental effects of Aβ on brain function. (9) In the second paper, the authors reviewed piracetam’s ability to improve interhemispheric transfer, enhance cerebral resistance to hypoxia, (as I observed in Korea during treatment of carbon monoxide poisoning) and most importantly, to facilitate learning and other cognitive functions. The authors reported that the cognition-improving properties represent the

“...a combination of piracetam with a Russian cognitive-enhancer named Cerebral resulted in an increase in the lifespan and a delay in the appearance of brain degenerative processes...” most common therapeutic use of piracetam all over the world i.e., memory disorders, impaired cognitive functions after head injuries, and vertigo. In over four decades of piracetam’s clinical use, the cognition-improving properties have been found to be more pronounced when brain function is impaired, by aging, hypoxia, cerebral injuries, or Aß load in Alzheimer’s disease. The authors concluded that after more than 40 years’ use of piracetam, the new findings of mitochondrial protection provide a new perspective for this old nootropic and explain its continued use in many countries around the world. (10) In a third paper, the scientists again demonstrated that piracetam improved MMP (mitochondrial membrane potential), elevated mitochondrial ATP production, enhanced neurite outgrowth and reduced Aβ levels in several cell models (in vitro) as well as in mice (in vivo). They concluded that piracetam, (which they referred to as a “metabolic enhancer”) improves mitochondrial function following mitochondrial impairment typical for brain aging and early stages of AD. (11)

Piracetam has never been approved for clinical use in the U.S. Thus, it’s not surprising that all of the above-cited papers were from elsewhere. Nevertheless, in 2010, scientists from NovoMed Consulting, in Silver Spring, Maryland, aware of the increasing interest in nootropic drugs for the treatment of CNS disorders, undertook an analysis of evidence-based clinical investigations of piracetam performed during the previous ten years. They conducted a literature search of the following therapeutic categories of CNS disorders: (i) cognition/memory; (ii) epilepsy and seizure; (iii) neurodegenerative diseases; (iv) stroke/ ischemia; and (v) stress and anxiety. The scientists were unable to confirm that piracetam exhibited long-term benefits for mild cognitive impairments, but they affirmed the studies demonstrating piracetam’s neuroprotective effect during coronary bypass surgery and its efficacy in treating cognitive disorders of cerebrovascular and traumatic origin; and they found piracetam’s overall effect on lowering depression and anxiety was even higher than improving memory. As an add-

on therapy, piracetam appeared to benefit individuals with myoclonus, epilepsy and tardive dyskinesia and when used in combination with a vasodilator drug, piracetam appeared to have an additive beneficial effect on various cognitive disabilities. (12) Piracetam’s sales continue to rise. The most recent data I could find (from 2010-2011), indicated world-wide sales in 2011 were in excess of $120 million, with an approximate increase in sales of about 8% from 2010 (Figure 2). Although piracetam has been around for over 40 years, research into its mechanisms and clinical effects continues. It has a safety record equaled by few pharmaceuticals and the recent identification of its mitochondria-resuscitating properties and ability to restore cell and mito-

SPOTLIGHT: GHRPS A R E AL ‘OR AL’ ALTER NATIVE TO G H INJEC TION S Ever since Dr. Rudman’s work in the 1980s and then the release of Dr. Klatz’s book ‘grow young with HGH’ in the 1990s, there has been a strong interest in the use of growth hormone (GH) in antiaging medicine. Dr. Rudman’s research concluded that after injecting his elderly patients with GH, many of them had reversals of their biological age markers by as much as 20-years; specifically having noted improved skin, hair, muscle mass, decreased fat levels and enhanced levels of stamina, strength and well-being. It’s not entirely surprising given the multi-faceted role of growth hormone, plus as its name suggests it is involved in the growth and repair of tissues, but unfortunately blood levels of it decline dramatically past the age of 35 (see figure 1), despite the fact that there is evidence that the pituitary gland continues making significant amounts of it.

GH injections

The issue with injecting GH, (brand names include Genotropin®, Saizen® and Zomacton®), other than its expense, is that it does have to be injected to be effective, this is because as a 191 chain amino-acid it simply can’t be absorbed via any other route, thus daily injections can become a chore. Furthermore, many countries have decided that GH injections be classified as a controlled substance, partly because of its anabolic actions. Controlled substances often require special import and export licenses; this is overand-above the requirement for a prescription. Furthermore, the research of Dr. Richard Walker has highlighted that bolus injections of GH are not

bioidentical and that as they induce spikes of GH into the blood they could end up damaging the pituitary gland, leading to a down-regulation of its own production of GH, or even to stop GH production altogether.

GHRPs

But meanwhile, Dr. Walker’s research has shown that the use of GHRPs, (growth hormone releasing peptides) have a much safer profi le whilst enjoying the same benefitseven if they provide them a little more slowly. We would recommend that you read his extensive article in the Aging Matters™ magazine, No3, 2014 to understand fully how they operate. What we can say is that GHRPs, (GHRP2, GHRP6 and sermorelin) have the following benefits: • They can be sublingually, intra-nasally and even orally, passing into blood and thus avoiding the need for needles. • Their feedback loop means that they cannot cause the pituitary to down-regulate. • GHRPs are not controlled substances. • Rather than inducing a spike of GH in the blood, GHRPs augment (improve) each release of GH naturally into the blood, for which there are several peaks daily, (although the rising from bed peak is the highest one) - see figure 2.

Synergy

Sermorelin is actually the precursor to GH, being the first 29 amino acids and is

applied via the sublingual route. Sermorelin’s function may be to release existing stores of GH from the pituitary- rather than encourage more production as a pure agonist would. Dr. Walker has highlighted that combining sermorelin with GHRP2 or GHRP6 has a highly synergistic effect, in some cases eliciting up to a 5x greater quantity of GH into blood, an action that can be equivocated to using injectable GH itself. Differences

There is no definitive answer yet as to the specific differences between GHRP2 and GHRP6; our own experiences, including information received from physicians and patients who use these products suggests the following: • GHRP6 may induce more hunger feelings than GHRP2 and could improve levels of IGF-1 more. SPOTLIGHT: SMART DRUGS AND NUTRIENTS DEPR ENYL FOR FOCU S AND CONCENTR ATION Deprenyl is also known as selegiline, it was created in the 1960s by Professor Joseph Knoll from Hungary, principally as an aid to Parkinson’s patients - because deprenyl has a significant benefit to improve dopamine levels in the brain. Dopamine is the neurotransmitter most affected in Parkinson’s disease, in fact, deprenyl still remains a front line treatment for that disease.

Professor Knoll’s experiments with rats produced some of the most incredible longevity benefits that have ever been seen. When they were fed deprenyl in their food, they lived so much longer than those that were not, so much so, that even after the last non-treated rat died, the first of the deprenyl treated rats was yet to die! These results are shown in figure one: Note; interestingly and importantly, these results were verified independently in another study not conducted by Professor Knoll. Based on this research, Dean, Fowkes and Morgenthaler, (as published in

For a long time deprenyl has been expressed as a MAO-b inhibitor, that it to say that is prevents the enzyme monoamine-oxidase type-b from destroying dopamine, ergo leading to its greater availability in the brain. The inhibition of the more common MAO-a can be problematic, leading to something called ‘the cheese effect,’ therefore this is not a side effect of deprenyl, although it should be noted that dopamine can inhibit type-a, but usually only at very high doses of 20mg. In more recent times, Professor Knoll has noted that there is another significant action of deprenyl and this is the raising, (albeit briefly) of PEA levels. PEA is a catecholamine activity enhancer that raises norepinephrine levels; this is

a significant attention agent that is behind the primary mechanism of the famous Eugeroic drug- modafinil (Provigil®). To learn a great deal more about dopamine and deprenyl, we would recommend Professor Knoll’s books; ‘the brain and its self’, or ‘how selegiline/ deprenyl slows brain aging.’

Typical patient responses In patients who have mild cognitive impairment, or age related minor cognitive dysfunction, the most common report is of a significant improvement in their focus and concentration. Persons with higher dopamine levels often appear more ‘driven’ and ‘dedicated.’ Avoid overuse since it can lead to what may appear to be an oppressive behavior, as others around you are not so focused and ‘on the ball’ as you! This is why we recommend occasional breaks from deprenyl use, some advocate one week off in the month and others use it during the weekdays but not at the weekends.

Dosing Doses are as normal, based upon need and age. Whilst Parkinson’s patient will require large doses, a person wanting to improve their cognitive performance may want to typically consider 1mg to 3mg per day, with occasional breaks. Note: These doses do not take into account synergy with other dopamine enhancing agents and persons using antidepressants should consult with their physician beforehand. SPOTLIGHT: BIOIDENTICAL HORMONES OX Y TOCIN FOR PA SS ION AND S E X IAS carries a wide range of bioidentical hormones - a term that means ‘natural to and in the body’. In this featured section we are focusing on oxytocin. What is oxytocin? Oxytocin is a hormone produced by the hypothalamus, but excreted via the pituitary gland. Its orthodox medicine role is to help women give birth, since the large dose that’s injected helps relaxes the uterus and alleviates the passage of the child into the world for the mother.

However, as we will discover and as been highlighted by Dr. Thierry Hertoghe’s book; ‘passion, sex and longevity, the oxytocin adventure’ -it has many other roles to play too.

The love hormone Oxytocin has been dubbed ‘the love hormone’, why would this be? Principally because oxytocin can induce feelings of bonding and care and not just between

individuals, but even with animals too! Oxytocin measurements have been taken between lovers, friends, relatives, parents and their children etc. From those results, it has been noted that oxytocin levels are higher when they are in their presence. Mothers naturally bond with their children, but even men, (especially those who experience the live birth), express their emotions as wanting to care and protect their offspring, these effects may be attributable to the release of oxytocin hence triggering the bond. On the other side of the coin, psychopaths are notoriously low in their oxytocin levels, which may be a cause of their uncaring feelings towards other humans.

The pain and orgasm connection Fibromyalgia can be a very debilitating disorder with a lot of pain, sometimes constant for those who suffer with it. In women it was noted that when they were experiencing an orgasm they felt no pain at all. Later, it transpired that women

undergo a burst of oxytocin during orgasm. Trials were undertaken to see if oxytocin supplementation could alleviate the pain of fibromyalgia, there was some success, but the side-effect noted was that those women now enjoyed multiple orgasms! This was a fact picked up on by the popular press and is probably singularly the action most responsible for bringing oxytocin into the public gaze.

Synergy Dr. Hertoghe has explained that some folks will not feel the effects of oxytocin. This is principally because of two reasons, (if we consider that the dose is correct for that individual). Firstly, that some people are ‘low’ in their own principal sex-hormone, so if a man is low testosterone, or if a woman is low estrogen, it is possible that oxytocin will not elicit its full potential in those persons. The other issue could be low vasopressin; vasopressin

Dosing As might be expected doses are very dependent upon its use. However for social or sexual enhancement, one can consider 5 IU to 10 IU a ‘typical’ dose. In fact, Dr. Hertoghe has somewhat reduced the doses that he recommends in his book, (transmitted via personal conversation to me). Currently IAS is providing Oxy-Sub™ in 20 IU trouches (a soft sublingual tablet), these can therefore be cut into half or quarter for a dose of 5 or 10 IU and should be placed under the tongue and allowed to melt. The other option is Oxy-Pro™ which is applied intranasally delivering 10 IU per spray.

SPOTLIGHT: SMART DRUGS AND NUTRIENTS PIR ACE TAM, THE ORIG INAL NOOTROPIC Smart drugs and nutrients, or to give them their correct medical terminologynootropics, are agents that can not only improve conditions of senile dementias, but in recent times have become popular for older individuals to improve their mental and cognitive processes. It was Ward Dean, M.D. who highlighted these facts through his very popular ‘Smart Drug’ series of books in the 1980s, since then the term ‘smart drugs’ has become mainstream.

Piracetam, the original nootropic The smart-drug we focus on here was in fact the first, developed as it was by Dr. Giurgea for UCB laboratories in Belgium in the 1960s. Originally it was designed to assist with travel and altitude sickness, but shortly afterward individuals realised that piracetam had positive cognitive enhancement effects.

What can piracetam do for me? Piracetam is a cognition agent that has been used successfully to treat a wide

range of conditions, for example it has been shown to increase a person’s attention levels and improve memory and intelligence. Piracetam can help to slow down ‘senile involution’, dementia and Alzheimer’s disease. In tests and trials, piracetam induces significant improvement to memory consolidation and recall in those suffering from ‘age-associated memory impairment’. Piracetam has also been used to improve patient’s recovery from strokes, particularly improving post stroke speech impairment (aphasia). Another use has been in cases of acute and chronic cerebral ischaemia, (decreased blood flow to the brain). Using piracetam has restored speech and the use of limbs in these patients; it has also increased neuronal activity in the brain when measured with EEG. For regular individuals, piracetam has been shown

to enhance idea creation and the ability to ‘see things through,’ in other words to have ideas and being them to fruition. The level of clarity piracetam creates is often described/ perceived as; “the fog has lifted.”

How does piracetam work? Piracetam’s key and unique method of action is upon the Corpus Callosum, the region of the brain that links the two hemispheres. It is this that most experts believe is the key that gives piracetam users the ability to channel greater brain potential by connecting the logical side of the brain with the creative side more effectively, Yin and Yang if you will.

What are the doses of piracetam? A common dose is 800mg tablets three times a day, then lowering to 800 mg twice a day after the first month. Note: The effects of piracetam can be enhanced if taken concurrently with centrophenoxine or Hydergine®. Side effects are minimal and seldom experienced, but should you experience nausea or headache then it is usually caused by an overdose, so in which case reduce the dose and build up more slowly, (if it is necessary). SPOTLIGHT: BIOIDENTICAL HORMONES NATU R AL ESTROG EN S AND PROG ESTERONE FOR WOMEN IAS carries a wide range of bioidentical hormones - a term that means ‘natural to and in the body’. Yet the industry is stuck in this old loop, despite the fact that natural (bioidentical) estrogens can be easily produced now. Some people believe that the known side-effects from ‘traditional HRT’ are due to the fact that the hormones given are not correct.

Esnatri™ use The Esnatri™ cream can be applied by daily rotation to your neck, upper chest, breasts and behind the knees, or inner thighs. A typical starting dose is 2 mg, start from day one (of what would have been the start of your menstrual cycle) and continue until day 25. Then you should stop for five days, before repeating the application at the start of the next menstrual cycle. During these last few days, the estrogen receptors are being allowed to ‘rest’ as they have been accustomed.

Progesterone Progesterone is the counterbalance to estrogens. Indeed, whilst women can significantly decline in estrogen levels during menopause- they rarely reach zero production levels, whereas progesterone can

sometimes not be measured at all in elderly women. It is also the low of progesterone that most significantly impacts bone strength, leading onto osteoporosis, so there are numerous reasons to ensure that progesterone is also taken alongside an estrogen therapy. IAS provides a 5% strength natural progesterone cream. Typical doses are 25 mg to 30 mg of progesterone applied on day 10 and continuing to 25. The start date varies according to the usual timing of your ovulation. Note: As with the Esnatri™ cream, stop for the last five days of your cycle so that the estrogen receptors have their accustomed ‘rest’ period. Remember, your hormone replacement therapy should be overseen by a physician and should not be undertaken if you have undergone cancer treatment.
