Scientists find a link between diabetes and Alzheimer’s Neurology researchers at Washington University School of Medicine in St. Louis, have demonstrated that raised glucose levels in the blood increase the production of amyloid beta, the proteins that make up the characteristic brain plaques in Alzheimer’s Disease. This research adds further evidence to the growing body of work that there is a connection between the causes of diabetes and the development of amyloid beta brain plaques that trigger the complex changes to the brain which result in Alzheimer’s Disease. To learn how elevated blood sugar might cause these plaques
to form, the team injected mice specially bred to have an Alzheimer’s like condition with diazoxide, a glucose-elevating drug commonly used to treat low blood sugar. In young healthy mice with no amyloid plaques, doubling glucose levels in the blood increased amyloid beta levels in the brain by 20 percent. When the scientists repeated the experiment in older mice that already had
developed brain plaques, amyloid beta levels rose by 40 percent. Taking the research further the team then identified a mechanism that directly linked glucose levels, the activity of neurons and amyloid beta levels – KATP channels. These channels on the exterior of brain cells close when glucose levels raise, this actions makes the cells more likely to fire. Excessive firing in certain areas of the brain can increase amyloid

beta production, which eventually can lead to more amyloid plaques and the development of Alzheimer’s disease. “Given that KATP channels are the way by which the pancreas secretes insulin in response to high blood sugar levels, it is interesting that we see a link between the activity of these channels in the brain and amyloid beta production,” said Shannon Macauley, PhD, the lead author and a postdoctoral research scholar.

and this increases our risk of developing cardiovascular diseases. New research by Kevin D. Monahan, from Pennsylvania State University College of Medicine, USA studied the effects of omega-3 supplementation on a
group of healthy men and women, aged between 60 and 80 years. Each of the participants took 2 omega-3 capsules twice a day for 12 weeks and their pulse wave velocity was assessed for signs of improvement. The study authors reported that: “these data indicate that 12

Telomere shortening pattern predicts cancer? Researchers from Northwestern University, Illinois, USA have discovered what they believe to be a telltale pattern for the development of cancer. Over a 13-year period the team took multiple telomere measurements
from 792 people, they discovered that subjects who experienced rapid telomere shortening, followed by a stabilization period of 3-4 years were eventually diagnosed with different types of cancer, including prostate, skin, lung, and leukaemia
amongst others. Telomeres are the end caps of chromosomes which we liken to aglets on shoe laces, they protect the DNA from deterioration during cell division. The researchers reported that telomere length in persons who went on
to develop cancer were as much as 15 years chronologically older than those who did not develop the disease. The lead investigator concluded that: “this pattern of telomere growth may mean it can be a predictive biomarker for cancer.”

“Vision may be the most precious of all 5 senses...” Vision may be the most precious of our five senses. Yet most of us may take it for granted...until it begins to deteriorate with age. As we get older we likely need greater amounts of light to perceive details clearly, and reading glasses to discern fine print which would otherwise be fuzzy. Night driving becomes difficult due to sensitivity from bright lights and glare, and it’s challenging to visually adapt to the darkness. These are common examples of “normal” ocular aging in otherwise disease-free eyes that will not severely impact quality of life in the senior years. But the problem is, with advancing years also comes a higher risk of developing age-related diseases, including eye disorders such as macular degeneration, cataract, and glaucoma, which could lead to vision loss and potentially blindness − a devastating consequence. The good news is, like many other degenerative aging processes, there is something (actually, many things) we can do about it. Let’s examine how to ensure lifelong healthy vision, or at least stop the progression of, and even reverse existing disorders.

Elevated blood glucose in diabetics, or even prolonged exposure to normal levels of glucose over time in aging non-diabetics, sows the seeds for the consequences of increased rates of glycation throughout the body. Structures within the eye have a high metabolic rate and are particularly susceptible to glycation and oxidation, processes which underlie the development of cataracts, macular degeneration, and glaucoma, among others. Although in this article we’ll mainly focus on strategies to combat the generation of ROS and AGE’s, other mechanisms, such as inflammation, vascularization (the abnormal formation of blood vessels), and mitochondrial dysfunction can further complicate and worsen eye damage, leading to vision loss.
OXIDATION AND GLYCATION: DOUBLE THREAT TO AGING EYES
So we now know that oxidative stress and glycation are implicated in the development and progression of age-related eye diseases. Cataracts are formed when oxidation of lipids and cross-linking of proteins in the lens of the eye cause the normally transparent lens to become opaque or cloudy, impairing vision. Cataracts left untreated are a major cause of blindness worldwide. Another serious disease associated with oxidative damage that leads to vision loss is age-related macular degeneration (AMD), which is characterized by damage to the macula, a pigmented yellow spot near the retina that is responsible for central vision. There are two forms of the disease, wet (in which newly formed blood vessels leak blood or fluid) and dry (in which cells atrophy or die). AMD impairs the ability to read, drive, and perform other activities that require fine, sharp, forward-directed vision. In glaucoma, another disease linked with oxidative stress, peripheral vision is hindered due to damage to the optic nerve from an increase in eye pressure.
Two deleterious processes exacerbated by aging that affect the body as a whole, including the eyes, are oxidation and glycation. Oxidative stress occurs when the over-activity of reactive oxygen species (ROS) such as free radicals, by-products of normal cellular metabolism, exceeds the body’s capacity to neutralize them. As their name implies, ROS are highly reactive, and can damage proteins, lipids, and DNA, and destroy cells and tissues if not kept in check by the body’s production of endogenous antioxidants (i.e., SOD, catalase, glutathione). Disease and aging can alter the oxidant/antioxidant balance, resulting in an excess of ROS that overwhelm the body’s waning antioxidant defenses. This damaging process is often compounded by another mechanism known as glycation, in which glucose or other molecules form irregular bonds with proteins, modifying their structure and function. Glycated proteins can then go on to chemically combine with other proteins in a process called “cross-linking.” Glycation and cross-linking result in the formation of toxic products, including advanced glycation endprod-

STRATEGIES TO MAINTAIN EYE HEALTH OR IMPROVE EXISTING CONDITIONS It’s clear that in order to protect our vision, and our health in general, the goal is to reduce the impact of oxidation and glycation. Thankfully, we can take steps, including lifestyle changes, targeted nutritional supplements, and drug therapies when needed, to prevent, slow, or even reverse age-related eye diseases.
• Antioxidant Vitamin/Mineral/ Carotenoid Support A good foundation of antioxidant support to neutralize oxidative damage, provided through the diet and supplements, is vital to the health of the body and eyes. A diet of fresh fruits and vegetables, legumes, lean meats, dairy, fish, and nuts supplies a range of antioxidant vitamins and minerals. (We’ll see more on optimizing diet later.) For further antioxidant support, a comprehensive multi-vitamin/mineral supplement and carotenoid formula is recommended; antioxidant vitamins A, C, E and minerals such as zinc have all been shown, individually and combined, to block oxidative damage and protect vision, as have carotenoids (phytochemical antioxidants found in dark green or yellow fruits and vegetables), including β-carotene, lutein, zeaxanthin and meso-zeaxanthin. Let’s look at one of these antioxidants, vitamin C, for example. Several studies have shown that greater intake and higher blood levels of vitamin C, which is found in high concentrations in healthy eye tissues, are linked with lower rates of cataracts, (1) and deficiencies have been reported in glaucoma and AMD patients. (2) Other studies have shown that combined nutrients act synergistically. A large-scale long-term placebo-controlled trial known as the Age-Related Eye Disease Study, sponsored by the National Eye Institute, evaluated the effects of an antioxidant mix on disease progression in 3640 participants with AMD, aged 55-80 years. Researchers found a statistically significant reduction in the risk of developing advanced macular degeneration in patients receiving a combination of vitamin C (500 mg), vitamin E (400 IU), beta carotene (15 mg), zinc (80 mg), and copper (2 mg), compared with placebo. (3)
• B- Complex Vitamins B vitamins are important to vision as well as cardiovascular health; they prevent high levels of homocysteine, a toxic amino acid metabolite that contributes to increased risk of heart disease and AMD. A seven year-long, double-blind, placebo-controlled trial at Harvard Medical School on 5442 female health professionals over 40 years old with preexisting cardiovascular disease, or at risk for the disease, examined the effect of B-vitamin therapy on lowering homocysteine and reducing the incidence of AMD. Researchers found that a daily combination of folic acid (2.5 mg), B6 (50 mg), and B12 (1 mg) reduced the risk of developing AMD by 33%. (4)
• Omega-3 Fatty Acids Omega-3 fatty acids are long-chain essential fatty acids (EFA’s), derived from marine and vegetable sources. EFA’s are classified as “essential” because they cannot be synthesized in the body and must be obtained from the diet or supplements. Important omega-3’s are the parent compound, alpha-linolenic acid (ALA), and its metabolites, eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), which are derived from cold water oily fish such as salmon, sardines and albacore tuna. Omega-3’s have been shown over the past few decades to be crucial to various aspects of health, including vision; they protect against oxidative stress, inflammation, and vascularization. Several studies have shown that higher intake is associated with lower risk of AMD. In another large-scale study by Harvard researchers on over 38,000 women (average age of 55) who completed a food-frequency questionnaire, regular consumption of DHA, EPA, and fish was associated with a significantly decreased risk of AMD. (5). Omega-3’s may also lower the risk of progression of macular degeneration to late stage, (6) and protect against cataract formation or progression. (7)
• Acetyl-l-carnitine (ALC) ALC is the acetylated ester of the amino acid derivative l-carnitine. Like l-carnitine, ALC shuttles long-chain fatty acids across the inner mitochondrial membrane to produce energy in the form of ATP. Besides its role in enhancing mitochondrial energy metabolism, which

“Melatonin may combat aged macular degeneration by stimulation of telomerase activity.” is altered in age-related diseases, ALC has demonstrated powerful antioxidant and anti-glycation properties, and so is a useful agent in preventing or treating ocular diseases. ALC guards against cataract development by preventing depletion of endogenous antioxidant enzymes and inhibiting lipid peroxidation within the lens, (8) and reduces glycation of lens proteins known as crystallins. (9) And in patients with AMD, ALC was shown to improve visual function. (10)
• Coenzyme-Q10 (CoQ10) Like ALC, CoQ10 is a necessary cofactor in mitochondrial energy production and a potent free radical scavenger, blocking oxidative damage of DNA, lipids, and proteins. Its production wanes progressively with age, which increases the risk of degenerative diseases and accelerated aging. One study found a significant decline in CoQ10 concentrations in the retinas of donors over 80 years old compared to donors under 30. Researchers believe that the decrease in both antioxidant capacity and ATP synthesis in the retina resulting from a CoQ10 deficiency may be linked to the progression of macular degeneration. (11)
• Pyrroloquinoline quinone (PQQ) PQQ is another mitochondrial energizer and potent antioxidant that may have potential in supporting eye health.
• Combination nutrients: ALC, Omega-3’s, and CoQ10 In another placebo-controlled clinical trial on the efficacy of synergistic nutrients in improving vision, the combination of ALC, omega-3 fatty acids, and CoQ10 was administered to 106 patients with early AMD. After 12 months, patients receiving this “cocktail” showed significant improvement in four parameters of visual functions, with only 2% of cases worsening, compared to 17% of cases worsening in placebo group; in other words, in the majority of treated patients, the disease did not progress to late stage, but instead, partially reversed! Researchers concluded that an appropriate combination of nutrients which enhance mitochondrial metabolism may improve and stabilize vision in patients with early AMD. (12)
• Can-C Eye Drops Can-C eye drops is a major innovation in cataract treatment and prevention, and vision health in general. The drops contain the substance n-acetylcarnosine (NAC), a naturally occurring precursor to carnosine, a potent inhibitor of glycation and oxidative processes. (Note: carnosine is not to be confused with l-carnitine; they are two distinct substances.) The acetyl group on the precursor NAC molecule allows it to penetrate the cornea and enter the aqueous environment of the eye, where it is then de-acetylated into carnosine. Once in the eye, carnosine accumulates in the lens, where it combats free radical activity while preventing the cross-linking of lens proteins, both of which, as we’ve seen, are implicated in cataract development. Several clinical studies on cataract patients using the drops have demonstrated its efficacy and value as a non-invasive alternative to cataract surgery, which involves the removal and replacement of the lens. In a placebo-controlled study on 49 patients with cataracts (average age of 65), 90% of those receiving Can-C (NAC 1%) eye drops twice daily for 6 months showed improvement in visual acuity and 89% showed improvement in glare sensitivity. (Note: visual acuity is a measure of clarity or sharpness of vision, determined by the ability to read the Snellen chart, the chart in the eye doctor’s office with letters that progressively decrease in size; for example, a result of 20/20 is considered “normal” vision.) After 24 months, the treated group exhibited no further cataract development, in contrast to the control group in which vision deteriorated. Tolerability of the drops was good, with no reported adverse events. (13) In another placebo-controlled study on 137 older adult automobile drivers, 4 months of eye drop use improved visual acuity and glare sensitivity in those with cataracts, also with good tolerability. (14) This study has important implications for improving both vision and driving safety in older adults. According to Russian scientist Dr. Mark Babizhayev, the developer of the drops, Can-C eye drops “help the aging eye to recover by improving clarity, glare sensitivity, color perception, and overall vision,” (15) and

(16) (Note: Carnosine may also be taken as an oral supplement to inhibit glycation and oxidative reactions throughout the body; Can-C eye drops produce targeted tissue-specific effects.)
• Can-C Plus Can-C Plus is an add-on product to be used in conjunction with Can-C eye drops in more advanced stages of cataract development. The oral tablets contain N-acetylcysteine (another powerful antioxidant and enhancer of endogenous glutathione production), carnosine (in an oral form rather than drops), vitamin E, zinc, and other ingredients.
• Resveratrol Resveratrol, an antioxidant phytochemical found in grapes and other sources, has gained popularity over the past few years for its widespread beneficial effects on health and longevity. It has been shown to protect various organs against aging by activating SIRT1, one of the sirtuins (SIRT’s), a family of enzymes with key roles in metabolic homeostasis and life span regulation. Upregulation of SIRT1 in animals has been shown to protect against several eye diseases related to oxidative stress, including cataract, retinal degeneration, and glaucoma. (17) Specifically, resveratrol counters free radical activity in lens epithelial cells and suppresses cataract formation, (18-19) protects against oxidation-induced glaucoma, (20) and controls neovascularization processes in AMD. (21) In a small case study on octogenarians with macular degeneration, improvements in retinal structure and visual function were obtained with daily administration of a resveratrol-based supplement. (22)
• Melatonin Melatonin is a hormone produced by the pineal gland and within the retina with diverse physiological functions. Besides its role in regulating circadian rhythms such as sleep, the hormone has been shown to prevent premature aging and increase life span, with strong 10
antioxidant and anticarcinogenic actions. The decline in production of melatonin with age is one of the leading mechanisms of aging and age-related diseases. In the eye, melatonin is synthesized by photoreceptors in the retina, with high levels at night and lower levels during the day; it acts as a scavenger of free radicals generated in ocular tissues. Dozens of studies have demonstrated melatonin’s favorable effects on eye health. In rats, it was shown to increase antioxidant activity and decrease the incidence of cataracts (23) and retinopathy, (24) a pathology similar to AMD. These results suggest that melatonin may prevent or treat cataracts and AMD in humans. A clinical study did indeed show that 3 mg melatonin, given orally at bedtime to 55 AMD patients for 6 months, stabilized visual acuity, protected the retina, and delayed further macular degeneration, and without side effects. (25) Besides cataracts and AMD, melatonin also shows promise in the treatment and prevention of glaucoma. (26-27) In addition to scavenging reactive oxygens within eye tissues, melatonin may combat macular degeneration by another mechanism: stimulation of telomerase activity in the retinal pigment epithelium, a pigmented layer of the retina. (28) Telomerase is the enzyme that enables chromosomes to maintain their length and structural integrity at their ends (called telomeres) when cells divide. of physiological functions, from retarding aging to protecting vision, melatonin should be included in any comprehensive longevity regimen.

• More on Reducing Glycation So far, we’ve discussed a variety of vision-supporting nutrients and supplements, many of them antioxidants, though we’ve covered two that protect against both oxidation and glycation. Now, let’s focus specifically on combatting glycation. As with antioxidant support to neutralize free radicals, we must also establish a strong physiological foundation that limits the formation of destructive AGE’s. This can be accomplished by maintaining blood glucose levels within the normal range for non-diabetics and within an acceptable range for diabetics receiving treatment (there’s controversy over what the “normal” range is, but let’s say a fasting blood glucose level less than 100 mg/dL, and optimally less than 85 mg/dL). Hemoglobin A1C, a blood test that measures levels of glycation in the body, specifically glycated hemoglobin, should also be monitored. In diabetes, elevated glucose in the blood speeds up the formation of AGE’s (since excess glucose is available to form higher levels of glycated proteins), leading to diabetic complications throughout the body, including the eyes. In fact, diabetes is a form of accelerated aging because this destructive process happens faster than it ordinarily would; but even in those with normal blood sugar, the rate of glycation increases with age due to prolonged glucose exposure over many years. A nutritional strategy to control blood sugar that should be followed by diabetics and non-diabetics alike is a low glycemic index diet that replaces simple carbohydrates such as super-sized sugary drinks, white bread, and sweets, with complex carbohydrates from whole grains, fruits and vegetables. This low-glycemic diet coupled with the antioxidant-rich diet discussed earlier is a solid preventive nutritional base. In addition, avoid food preparation that triggers the production of dietary AGE’s (yes, they are even formed during cooking, and then ingested, further adding to the body’s toxic burden!). Frying and grilling foods at high temperatures should be replaced by lower temperature methods that also involve liquid, such as boiling, poaching, or stewing, which produce significantly fewer glycotoxins. Following these measures, coupled with the use of appropriate nutrients, supplements, and drugs when necessary, will help protect against glycation-induced damage. • Novel Agents Up to now, the nutrients, supplements, and drugs we’ve reviewed promote eye health by combatting pathological aging processes, including oxidative stress, glycation, and mitochondrial dysfunction. Now, let’s look at agents that support vision by unique mechanisms. Vinpocetine, a derivative of the periwinkle plant, is a nootropic and vasodilator that improves blood flow in the brain. Since blood vessels of the eye are in direct connection with the cerebral circulation, vinpocetine also increases microcirculation in the eye, which is beneficial in treating various eye disorders. In a placebo-controlled trial on 40 patients with AMD, aged 40 to 65, those who received 10 mg vinpocetine for 2 months, in addition to conventional therapy, demonstrated better visual acuity, improved retinal function, and increased ocular blood flow. (29) Another nootropic, picamilon, also increases circulation in the brain. The structure of picamilon consists of the vitamin niacin bonded to the neurotransmitter GABA. The molecule can cross the blood-brain barrier into the brain where is broken down into its components GABA, an anxiolytic, and niacin, a vasodilator. And again, since the circulation of the brain is in close proximity to that of the eyes, the vasodilation action has a positive effect on vision. In Russia, picamilon is used to increase cerebral blood flow and treat glaucoma, among other applications. The last novel agent we’ll discuss is the peptide bioregulator visoluten. A major innovation in anti-aging therapies over the past few decades was the development of peptide bioregulators − short chains of amino acids (peptides) isolated from various animal tissues and organs. administered to other animals or humans, these peptides act at the genetic level to regenerate age-damaged tissues and restore the function of a particular organ. For example, retinal peptides initiate the formation of retinal and pigment cells in patients with retinal degeneration, thymic peptides stimulate immunity, and pineal peptides induce the aging pineal gland to manufacture melatonin. The mechanism behind these rejuvenating effects is complex: each short-chain peptide interacts with a gene’s DNA, transferring information encoded in its amino acid sequence and stimulating a process called protein synthesis, the manufacture of proteins according to the encoded genetic information. By “switching” on genes to synthesize specific proteins, peptide bioregulators can recover functions of tissues and organs undergoing age-related deterioration. What are stem cells? Stem cells are unspecialized ‘primary’ cells from which all other cell types in the body descend. They have three important features: (1) as mentioned, they are undifferentiated (i.e., unspecialized); (2) under the right conditions and with certain enablers they are capable of activating and dividing for long periods; and (3) they develop, or differentiate, into specialized cell types such as muscle, blood, or nerve cells. Two broad classifications of stem cells are embryonic stem cells and adult stem cells. There is often controversy attached to embryonic stem cells as they are derived from an embryo. Whereas adult stem cells (also called somatic stem cells), are located in tissues, muscles and organs throughout the body and present in grown-ups and newborns alike. Note that adult stem cells are distinct from embryonic stem cells and not associated with any of the ethical, moral, or political issues surrounding embryonic stem cells.
Adult stem cells - the body’s endogenous repair system Adult stem cells are located in specific areas of tissues and organs called stem cell niches. They multiply by cell division to replenish damaged, deteriorated, or dead cells and regenerate tissues − their primary biological role. Ongoing tissue and organ repair is a critical function and relies on effective activation and differentiation of stem cells within these specialized niches, in particular the bone marrow. Besides the bone marrow, adult stem cells have been studied and identified in a variety of organs and tissues including peripheral blood, cord blood, brain, liver, muscle, heart, gut, and skin. Depending on their source and the particular cells into which they develop, adult stem cells are further classified into types, including mesenchymal, hematopoietic, CD3+, stromal, etc.
The versatility of adult stem cells
cell types of the particular tissue or organ in which they reside, without the universality, or pluripotency of embryonic stem cells, which can develop into any cell type in the body. For example, a hematopoietic (blood forming) stem cell found in bone marrow was typically thought to form one of the many types of blood and immune cells, but not to develop into a brain or muscle cell. But research over the past few years has challenged this belief and shown that, although they are not by definition pluripotent, certain adult stem cells found in particular tissues can differentiate into cell types found in other organs or tissues in a process called transdifferentiation. This means that an adult stem cell found in one tissue has the ability to migrate to a completely different tissue, transdifferentiate, proliferate, and repair damage there − an intriguing finding with important implications for treating degenerative diseases and impeding aging.
Adult stem cells have widespread applications in medicine Given their capability to generate new tissue, adult hematopoietic stem cells have routinely been used in stem cell transplants (sometimes called bone marrow transplants) after high doses of chemotherapy and/or radiation are used to treat blood cancers such as leukemia and lymphoma wipe out a patient’s own bone marrow. Apoptotic Modulators in aging By Marios Kyriazis MD Apoptosis (a Greek word, meaning ‘falling off’) is a crucial event during aging. It is the orderly death of cells which are no longer functional or which are beyond repair. Apoptosis (also called ‘programmed cell death’) was first described in 1972 by Kerr, Wyllie and Currie (1) and it has been attracting substantial research interest since. The reason is that a better understanding of apoptosis will help explain some age-related changes AND suggest directions for cancer treatments. During apoptosis cells die following a set pattern and are methodically eliminated by the organism. Apoptosis is different from necrosis (when cells die at random). In necrosis, healthy and unhealthy cells may die indiscriminately and the process generates toxic material which kills other cells down the line. Instead, in apoptosis, cells are eliminated in a controlled manner which minimizes the damage to other cells. It is important to have at least a general idea of the complexity of the process, before a discussion about
modulating it can take place (FIGURE 1). There are three phases during apoptosis. These are: A. The Induction phase: This is influenced by signals which stimulate pro-apoptotic cascades. Initiating signals may be released by free radical damage, UV radiation and DNA mutations. There are several mechanisms which converge into a single event: the cell’s commitment to die, and this depends on the caspases. Caspases (Cysteine-dependent aspartate-directed proteases) are proteases which are classified into several sub groups, having a role in different stages of apoptosis.
Caspases must be pre-activated before they become effective. One way of activating the caspases are via Apafs (Apoptotic Protease Activating Factors, which are the results of an earlier DNA damage). Their activation depends on the balance between pro and anti-apoptotic factors. Once activated, the caspases activate other caspases all of which cleave cell proteins leading to apoptosis. B. The Effector phase: In this second phase, the cell becomes committed to die. This may happen by activation of death receptors which, again, lead to the activation of pro-apoptotic caspases. C. The Degradation Phase: When the

Apoptosis in Aging I will mention a few representative examples of apoptosis during age-related degeneration. Example 1 In Alzheimer’s disease there is an increased activity of amyloid beta proteins, which results in accumulation of aggregates of damaged proteins. This increased amount of amyloid beta stimulates apoptosis, resulting in loss of function due to too many neurons being eliminated. Oestrogens may reduce the rate of apoptosis in the brain. This may be due to the ability of oestrogens to induce the expression of an anti-apoptotic protein Bcl-XL which restricts the activity of amyloid beta
and thus, ultimately, reduces the risk of dementia. Example 2 Aging causes apoptotic loss of cardiac muscle cells. Cells may also be lost through ischaemic events which cause necrosis. Drugs which have an action on the heart may reduce apoptosis. For example, enalapril was found to reduce cardiomyocyte apoptosis in animals (2). Whilst writing, I will also mention that enalapril was found to be effective in protecting neurons against apoptosis and it may thus be useful in dementia (3).

In conclusion, this is a very promising area of research that needs our attention, as there are several agents and compounds that can be used in achieving positive results. This is not to say that the currently available or future agents will be fully effective, but at least we have some information and some facts which we can work on.

Dear SW, Good Questions! Frankly, although there’s a lot of unsubstantiated claims that 7-keto DHEA is superior to DHEA, in that it is purportedly more stable and does not convert as readily in women into testosterone (as DHEA does)—I’m not convinced. There is more hype about 7-keto than good science. I’ve searched it on PubMed for comparative studies and there’s not much there. Most of the solid research has been done with DHEA, NOT 7-keto. If 7-keto were so superior, I believe there would be a lot more studies out there. Pregnenolone levels, like DHEA, drop predictably with age. I recommend pregnenolone at 25-50 mg, taken first thing in the morning

(1-4) However this remedy escaped the attention of ophthalmologists in the US until 1995, when reported by Dr. Herschell Boyd. (5) Dr. Boyd administered doses of ascorbic acid (averaging 10 grams per day) to 30 patients with elevated intraocular pressure. Average pressure reduction was about 5 mm Hg, although one patient experienced a drop of 13 mm Hg, and seven patients experienced reductions of 10-12 mm Hg. Dr. Mark Babizhayev at IVP, the inventors of Can-C™ eye-drops reported that N-acetyl-carnosine is a useful adjunct in open angle glaucoma. He believes that the anti-glycosylation properties of Can-C™ reduce the accumulation of proteins from around the Schlemm canal (the valve in the eye), which over time reduces

Dear Dr. Dean, I have been a customer of yours for about 8 or 9 years. I would like to try again one of the thyroid medications and some of your smart drugs but find when I do within 4 or 5 days I fall into a drugged-like state. I even experience this with things like CoQ10 and the like. I have been diagnosed with low adrenal reserve and am on prednisolone 2.5mg in the morning and 1mg in the afternoon. I asked my endocrinologist about this ‘drugged feeling’ but he did not have an answer. It is very frustrating because I’m sure I have an underfunctioning thyroid despite my blood levels returning a low but not deficient result. My mind is very foggy and my concentration terrible. I have other symptoms corresponding to low thyroid all of which I’ve had for about 10 years (since a complete hysterectomy - I am female 53 years). Do you have any suggestions? Many of my patients have come to me out of desperation due to their physicians’ failing to appreciate their hypothyroid state. As I’ve said repeatedly, thyroid blood tests (as interpreted by current standards) are largely a waste of blood. I provide a questionnaire which gives a good indication of one’s thyroid status. Although all of the signs/ symptoms listed can be caused by other conditions, when an abundance of the responses are positive, there is a good likelihood that the underlying cause is an under-functioning thyroid. I start such patients on a low dose of desiccated porcine thyroid, (Armour®, Nature-Throid® or ERFA®) and gradually titrate the dose to a dose that alleviates many or most of the symptoms, but does not cause an abnormal elevation in the patient’s heart rate or body temperature. Usually, the response is gratifying to both physician and patient. Metformin is best known for its insulin-receptor-sensitizing properties. However, less well known is that it is a multi-hormone receptor sensitizer peripherally, as well as a central hypothalamic
receptor sensitizer. Consequently, metformin helps your body respond to all of the hormones in a more efficient manner—including thyroid and adrenal hormones. It is important to also consider the adrenals, as you are doing. The BEST remedy for adrenal dysfunction was the old pharmaceutical Adrenal Cortical Extract, which was knocked off the formulary of US physicians in the late 1970s. I’ve been searching for a replacement ever since. Adrenal Cortical Extract was an oil or water soluble preparation for injection, comprised of bovine adrenal cortex, containing a range of all adrenal cortical factors in low concentrations—hardly enough to do anything, by today’s pharmaceutical standards. Nevertheless, it somehow “reset” the adrenals, and enabled those suffering from adrenal exhaustion to function normally. It was GREAT stuff. You are doing the best you can with what we’ve got available. As you’ve found, prednisolone is the BIG gun, but must be judiciously used. Hydrocortisone is more forgiving—but chronic use can result in problems, as well.
I think a combination of the Russian peptides (Thyreogen® and Glandokort®) and metformin, in conjunction with low doses of hydrocortisone and appropriate doses of porcine thyroid will help your adrenals to recover. In addition, I’d consider pregnenolone (25 mg in the morning) and DHEA (also 25 mg in the morning). In view of your hysterectomy, you might also consider topical progesterone cream. SPOTLIGHT: THYROID SUPPORT FOR THE HYPOTHYROID EPIDEMIC Dr. Broda Barnes in the 1970s estimated that 40% of the adult population was deficient in thyroid hormones; he published this statement in his excellent book‘hypothyroidism, the unsuspected epidemic.’ Since then, pupils of Dr. Barnes, such as Dr. Richard Wilkinson, have suggested that this figure could be even greater now! The importance of the thyroid gland The hormones produced by the thyroid control the body’s metabolism- the rate at which it burns calories for energy. General cognitive benefits Classifying the precise benefits of the various smart-drugs can be tricky. Most individuals often simply refer to their ailing cognitive facilities as “memory loss.” However, a quick breakdown of that statement requires further evaluation in order to determine the precise nature of the problem. Professor Imre Zs.-Nagy says; “Centrophenoxine has shown many facets to improve conditions related to my membrane hypothesis of aging. Lipofuscin Lipofuscin is a waste material that accumulates in aging cells, especially those in the brain, heart, lungs and skin; indeed, in skin cells, lipofuscin can form part of the pigmented spots that are often referred to as ‘age’ or ‘liver’ spots Lipofuscin accumulation can be very troublesome for the cell because it inhibits proper functioning
Perhaps it is therefore centrophenoxine’s primary mode of action to help remove lipofuscin deposits. It does it so well, that it is currently believed to be the best tool commercially available to do so and so well, that in patients with visible ‘age’ or ‘liver’ spots in the skin; it is possible over several weeks of centrophenoxine supplementation to see them fade or even disappear. Such patients can also benefit from the knowledge, that at the same, lipofuscin deposits are being reduced in their heart, lungs and brain etc.
Centrophenoxine is best suited to the last problem. If your speech appears to be full of “ums” and “ers” - whilst your brain tries to catch up with your mouth, then it is likely that centrophenoxine will be an aid for you. In our experience, centrophenoxine should not only be considered for those concerned with the development of Alzheimer’s, but when utilized correctly it can help many people, especially those aged over 40, to hasten their recall speed, bringing clarity and order to both speech and thought.

A typical starting dose is 2 mg, start from day one (of what would have been the start of your menstrual cycle) and continue until day 25. Progesterone Progesterone is the counterbalance to estrogens. Indeed, whilst women can significantly decline in estrogen levels during menopause- they rarely reach zero production levels, whereas progesterone can

It is also the low of progesterone that most significantly impacts bone strength, leading onto osteoporosis, so there are numerous reasons to ensure that progesterone is also taken alongside an estrogen therapy. IAS provides a 5% strength natural progesterone cream. Typical doses are 25 mg to 30 mg of progesterone applied on day 10 and continuing to 25. The start date varies according to the usual timing of your ovulation. S U PPLYING AND BAL ANCING OMEGA S NATU R ALLY Most people are familiar with the benefits of omega oils for their health and in particular to reduce inflammation. Indeed cod-liver oil supplements are the world’s most popular supplement, but recently Professor Brian Peskin’s research which is all contained in his new book – the PEO solution - is causing quite a stir and may make many folks to rethink and change their stance.

We are referring to his discovery that the body naturally uses plant oils to make its own omega 3 and 6, thereby satisfying a precise balance according to needproviding the ‘raw materials’ are available. Professor Peskin has termed the phrase PEO as the acronym for ‘parent essential oils.’ Professor Peskin states openly that he has found this information hidden in the scientific literature and never once found it referred to in the medical literature. Why does it matter? Because as he says, it means that physicians are unaware of these facts and therefore they continue to advocate super-physiological doses of omegas from fish oil, which may be actually doing more harm than good! It’s a lot to swallow, (no pun
intended) and we can’t do his research justice on a single page, which is why we recommend his book, or take a look at the Aging Matters™ magazine No1, 2015 in which Professor Peskin’s article is the lead story. Let’s summarise the advantages of the PEOs: • The plant oils live in our environment, (typically around 50-60°F); hence they do not go rancid at room temperature. Cold water fish live in a cold environment (typically around 30-40°F), therefore PEOs represent a much more stable product. • The PEOs are obviously a nonanimal source and therefore can be considered more suitable for those wanting to avoid animal based products. • The PEOs represent a sustainable source, since there is already too much ‘pressure’ on the seas to provide both fish and even krill stocks. • The PEOs enable the body to produce its own internal essential fatty acids (EFAs) and correct its own balance. This has not been shown to be the case with fish oil supplements.
