Scholarships Health Scholarships of Georgia, LLC, is a nonprofit organization affiliated with Community Health Foundation. Health Scholarships of Georgia, LLC gives scholarships to nursing students who work for the System of which Community Health Foundation is a part. Although Health Scholarships of Georgia, LLC was formally organized in October 2003, its roots are much deeper, as the first scholarship program was organized in 1989. Since that time, Health Scholarships has funded over 150 scholarships to deserving students committed to health care careers.

HEARTS TO HANDS The Community Health Foundation Hearts to Hands program exists to enable caring individuals within our family of organizations to provide meaningful financial assistance from their Hearts into the Hands of fellow employees who are in crisis. Community Health Foundation has already helped many employees during difficult times by providing financial support, emotional support, ministry and counseling. Gifts to the Hearts to Hands fund by our employees allow this program to grow so that the Foundation can reach more people in need.

COMMUNITY INVOLVEMENT The Annual Fund of Community Health Foundation also helps with projects and special needs for individuals and groups outside our organization. Some projects are done exclusively by the Foundation. At other times, the Foundation partners with groups like Habitat for Humanity, the Alzheimer’s Association, the Advocacy Resource Center or other local community organizations to help them accomplish their missions.

Eligibility for scholarships includes the following criteria: enrollment in a school approved by the U.S. Department of Education; maintenance of a cumulative C average; and, an employee of an organization that is part of the Community Health Systems family. Scholarship funds are paid directly to the school and may be used for tuition, matriculation, activity and lab fees, books and uniforms. Scholarships are used to pay the difference between any other grants or scholarships and the applicant’s tuition and fees. Our Mission Community Health Foundation is a non-profit member of Community Health Systems, whose mission is to serve others through positive change by: 1) serving as the organization’s primary vehicle for ministry and support; 2) facilitating individual involvement in community projects; and 3) helping to shape and model our corporate culture.

Our Fund Accounts Community Health Foundation Annual Fund: Funds in this account are provided by caring individuals to assist communities and non-employees in times of crisis. One-hundred percent of the funds placed into this account are distributed by the Foundation to make a positive difference in the lives of people outside our organization.

Community Health Foundation Hearts To Hands Fund: Funds in this account are voluntary contributions made by our employees for the purpose of assisting other employees during times of significant crisis. Like the Annual Fund, 100% of the funds in this account go directly to serving others through positive change.

Community Health Foundation Beneficiary Fund: Funds in this account are voluntary contributions designated for specific individuals or projects and can only be used for the benefit of the specified beneficiary. The Foundation reserves the right of approval for all such specified individuals or projects prior to the acceptance of any funds into the Beneficiary Account.
