Fred H. Olin, MD

Golden Aesculapius Award In 1980, the Bexar County Medical Society (BCMS) established the Golden Aesculapius Award as its highest honor to recognize a lifetime of distinguished service by a BCMS member to patients and the profession. Rarely has it been earned by a doctor whose patients included the residents of the San Antonio Zoo as well as hundreds, if not thousands, of Homo sapiens, serving the Alamo City as an orthopaedic surgeon.

Said Fred H. Olin, MD, “I knew I wanted to be a veterinarian by the time I was 12 years old.” Achieving his DVM in 1961 from the University of Illinois, he went on to serve in the US Army Veterinary Corps, stationed in Philadelphia, Pennsylvania.

“Our duty was largely that of sanitary inspection of food processing plants that sold to the military. Even beyond passing Department of Agriculture standards, they had to be inspected by a military vet. With the Philadelphia Navy Yard, Fort Dix, NJ and a few other installations in the area, we were kept busy assuring the quality of food processing.”

After his release from active duty in 1963, Dr. Olin joined a small animal veterinary practice, Broadway Animal Hospital, here in San Antonio. He and his employer, the late Thomas E. Vice, DVM, were offered the opportunity to serve as veterinarians to the San Antonio Zoo a year later.

After they left the zoo, Fred decided to expand his practice by one species, and earned his MD from the University of Texas Medical School at San Antonio in 1973. Following graduation, he did a straight surgery internship at Bexar County Hospital, now called University Hospital.

Leaving the Lone Star State to do his Residency at the University of Arkansas for Medical Sciences in Orthopaedic Surgery was rough, but it had the unmatched benefit of introducing him to a master teacher, Carl L. Nelson, Jr., MD, who arrived as chair shortly after Fred began his residency. “Carl was constantly in an instructive mode… he employed the Socratic Method, with a question ready for any occasion, from walking up stairs to scrubbing for surgery.”

“He was a great teacher,” said Dr. Olin.

“Hippocrates mandates that we honor our teachers as the parents of our profession. I was very fortunate to have teachers in both veterinary and medical school whose voices – and questions! – I can still hear resonating today.”

Specializing in orthopaedics came naturally to the eternal optimist. “We very rarely lose patients; there are nearly immediate results and instant gratification. At heart, I’m a tool-using person and orthopedic devices are in a continual cycle of test and improvement.”

With such professional devotion, Dr. Olin found himself serving as Chairman, Department of Orthopaedic Surgery at what was then St. Luke’s Lutheran Hospital, before chairing the Department of Orthopaedic Surgery of Southwest Texas Methodist Hospital, and eventually the Surgical Board at Methodist Hospital, where he also served on the Orthopaedic Quality Improvement Committee for nearly two decades.

Readers of the San Antonio Medicine magazine are familiar with his diverse writings; his byline has appeared on articles ranging from book reviews to professional commentary. He has served as a member of the Bexar County Medical Society Publications Committee, from 2005 to the present, chairing the committee during 2014-2015.

“But his greatest leadership role isn’t likely to end up in a CV,” said Jennifer Olin, RN, elder of his two daughters and an OR nurse herself. “Dad promoted, encouraged and advanced women professionally throughout his entire career.

He supported women pursuing their goals, even in deeply male-dominated fields. Dad was ‘woke’ before ‘woke’ itself got woke,” Jennifer concluded.

For service to his patients, his profession and his community, the Bexar County Medical Society is proud to number Dr. Fred H. Olin among the recipients of the 2020 Golden Aesculapius Award.

Gerardo Ortega, MD, FACS Golden Aesculapius Award The Golden Aesculapius Award recognizes a lifetime of distinguished service by a BCMS member to patients, organized medicine and the profession. “Lifetime of service” only scratches the surface of contributions made by Gerardo Ortega, MD, FACS. Serving as President of Baptist Health System Medical Staff and member of Baptist Health System Board of Trustees from 20172019 and currently serving on the Baptist Health System Medical Executive Board, Dr. Ortega has been a member of the Bexar County Medical Society continuously for more than 38 years, becoming President of the Bexar County Medical Society in 2001.

He has held multiple leadership positions over the years including that of the Hispanic Medical Society President, San Antonio Vascular Surgery Society President, founding President of the University of Nuevo Leon Medical School Foundation, and currently both a Member of the BCMS Board of Directors and Chair of the Philanthropic Committee of the BCMS Library Association.

Building community comes naturally to Dr. Ortega. “I believe that organized medicine is the best way to make changes and improve health care. We can make a difference one leader at the time, but when all leaders get together, the impact is tremendous along with the potential for change.”

In the mid-1990s, Dr. Ortega found out that Baptist Medical Center was going to replace a hospital incinerator and he was instrumental in negotiating a donation of the older incinerator to the University Hospital Medical School in Monterrey, Mexico, his alma mater. Raising $50,000 for transportation of this large incinerator to Monterrey, Mexico, the incinerator continues to work now, providing services and income to the University Hospital.

This was the initial step to the foundation of an association of 500 alumnae of physicians in the US who graduated from Monterrey, Mexico, 80 of which were in San Antonio and Bexar County.

This organization, of which he became founding president, provided donations and resources to the medical school there.

This 2020 BCMS award reminds him that it takes a community of colleagues, friends and family to achieve accomplishments. Said Dr. Ortega, “From my fifth-grade teacher (who told me I’d never be a doctor!) to the current day, each step of the way has always relied upon people helping. From classmates, friends, my friend’s father who was a physician (with a wonderful library!), to professors I emulate to this day; and of course, my wife is critical to each and every accomplishment. Someone helped me take each step in the journey and this – this honor – is a great opportunity to review and appreciate them.”

Dr. Ortega has been dedicated to the education of the community, and in so doing has been Chair of the Baptist Vascular Update Symposium. This is an annual conference that attracts approximately 200-250 attendees and prominent physicians as speakers from around the country. The Baptist Wound Care Update was

started by Dr. Ortega and chaired by him for more than 17 years.

He has done a program on television, "Consulte a su Medico" (Consult Your Doctor) for three annual seasons. He established a oneyear fellowship program in conjunction with Peripheral Vascular Associates and Baptist Health System. (The first Fellow was Dr. Cesar Pena from Monterrey, Mexico in 1995.)

Born and raised in San Antonio, Delbert Chumley attended Central Catholic High School and has spent the last four decades here in private practice. After graduating from Texas A&M University, he received his medical degree from The University of Texas Medical Branch, Galveston where he remained for his post-graduate education serving as Chief Resident in Internal Medicine and completing a Fellowship in Gastroenterology in 1976. Afterwards, he moved back to his hometown and established Gastroenterology Consultants of San Antonio which has grown into a 26-physician group.

Dr. Chumley is a Fellow in the American College of Physicians and the American College of Gastroenterology and continues to maintain a clinical faculty appointment of Professor of Medicine at the UT Health San Antonio Long School of Medicine.

He has held several hospital and professional appointments and remains very active in organized medicine, legislative affairs and development of medical education programs. In recent years, Dr. Chumley has served as President of the Texas A&M Medical and Dental Society, President of the Texas Society of Gastroenterology and Endoscopy, President of the Bexar County Medical Society, and President of the American College of Gastroenterology.

He and his wife of 45 years, Louise, have remained active in community affairs supporting numerous nonprofit organizations including serving as co-chairs for the American Cancer Society Cattle Barons Gala. Dr. Chumley most recently served on the Board of Directors for BioMedSA which focuses on the growth of biomedical research in San Antonio and as Administrative Liaison for the American College of Gastroenterology Board of Trustees.

Dr. Chumley and Louise enjoy spending time at their second home in Rockport, fishing in Aransas Bay, attending Texas A&M home football games and supporting the Spurs. They obtain the greatest pleasure, however, enjoying their three adult children and their spouses and their four grandkids, (soon to be five).

When asked what he enjoys most from his 40-plus years of practice, Dr. Chumley responds, “caring for my patients whose trust and friendship I truly cherish.”

He also has been and continues to be amazed at all the advancements in gastroenterology, including the development of new drugs to treat duodenal ulcers, to inflammatory bowel disease, to hepatitis C and how technology has helped to improve diagnosis. However, he emphasizes that attentively listening to a patient and performing a complete and careful physical examination still remain the foundation of quality healthcare.

David G. Schulman, MD Golden Aesculapius Award David G. Shulman, MD is a board-certified ophthalmologist whose career is a devotion to improving Texans’ eyesight; little wonder he was nominated to receive the “2020” Golden Aesculapius Award. Practicing in the San Antonio area for over 30 years, he graduated from the Creighton University School of Medicine, completing his internship and residency in ophthalmology in Temple, Texas.

Dr. Shulman has a special interest in cataract and refractive surgery, having performed refractive surgery procedures since 1985.

He is certified in the Catalys procedures, which is the latest technology in cataract removal for his patients. He has participated in clinical research concerning ophthalmic drugs for the treatment of glaucoma, eye infections and allergic conditions of the eye.

A team of investigators, led by Dr. Shulman, conducted a phase 3 extension study showing similar efficacy in reducing intraocular pressure of BAK-free latanoprost compared to latanoprost containing BAK. In this follow-up study, investigators sought to determine the long-term safety of use without the preservative.

Dr. Shulman is a clinical professor of ophthalmology at the UT Health San Antonio Long School of Medicine. He has served his medical community by being President of the Texas Opthalmol-

ogy Association in 1993, President of the Bexar County Medical Society in 1995, President of the BCMS Foundation in 2003 and President of the San Antonio Society of Ophthalmology in 1997.

He has had the honor of sitting on the Board of Trustees of the American Academy of Ophthalmology from 2000-2003.

“Dr. Shulman has been very active in the Bexar County Medical Society, having served many leadership positions and contributing vastly to our legislative/patient advocacy agenda” said co-award-recipient Dr. Delbert Chumley. “He also is a highly respected and admired ophthalmologist still practicing and contributing to the healthcare of our city and still supporting our Medical Society.”

David N. Henkes, MD Distinguished Service Award The Bexar County Medical Society (BCMS) Distinguished Service Award recognizes doctors who have provided distinguished service to the physicians or patients of Bexar County, or to the medical profession in general. It is with pleasure that Dr. David Henkes is named a 2020 recipient of the Distinguished Service Award, an award for specific acts of service and accomplishments. Dr. Henkes has been a strong advocate for physicians and patients for over 30 years.

Dr. Henkes said, “Early in my career, a mentor told me that as a professional we are expected to not only do a good job but to also give back to the profession. Being involved in organized medicine, from the time that I was a medical student until now, is my attempt to follow that advice and the results have been very rewarding to me.”

He has served the Bexar County Medical Society as President, Vice President, Treasurer, and as an at-large Board member. He also has served as Chair of many BCMS committees, including the legislative, BCMS Delegation to Texas Medical Association, and finance committees. He is currently the Chair of the BCMS Leadership Development Committee.

In the Texas Medical Association (TMA), Dr. Henkes has served 22

on the Socioeconomics and Legislative Councils, on the TMA Board of Trustees for nine years, and as Vice Chair and as Chair of the TMA Board of Trustees. He currently is the Chair of the Texas Delegation to the American Medical Association.

He is also currently the President of the Christus Santa Rosa Medical Staff which oversees the medical staff of four hospitals in this area and has been Chief of Staff at the Christus Santa Rosa Medical Center Hospital two times.

Dr. Henkes is a past president of the Texas Society of Pathologists and served on advocacy councils and committees for the College of American Pathologists. He also previously chaired the Texas Medical Association Political Action Committee (TexPAC).

At the national level, he frequently attends the American Medical Society National Advocacy Conference and visits legislators in Washington to ensure that Texas medicine and the doctor-patient relationship are protected from harmful legislation.

Throughout his service to the physicians and patients of San Antonio and Texas, Dr. Henkes has maintained a reputation for being a trusted authority on medicine and is frequently called upon to talk with news media on emerging issues.

William L. Henrich, MD, MACP Distinguished Service Award William L. Henrich, MD, MACP, a specialist in kidney diseases, has served as the president of The University of Texas Health Science Center at San Antonio (now called UT Health San Antonio) since 2009. UT Health San Antonio is one of six health institutions under the umbrella of The University of Texas System.

He is the inaugural holder of the John P. Howe, III, MD, Distinguished Chair in Health Policy.

He is the author of over 300 original articles and chapters, and the founding editor of the popular dialysis textbook, “Henrich’s Principles and Practice of Dialysis”. His current areas of interest are in improving dialysis and vascular renal disease.

An elected member of several prominent research societies, Henrich also served as President of the American Society of Nephrology.

He is passionate about mentoring students, residents and young physicians and has received teaching accolades in every institution in which he has served. A devotee of literature and opera, he is also committed to staying fit by running or swimming most days so that he can continue to enjoy life to the fullest with his wife of 50 years, Mary, son and daughter, and five grandchildren.

Louis J. Goodman, PhD Lifetime Service Award The Award for Lifetime Service to an Organization is designated for those who have shown exceptional leadership in the development of our professional community. Louis J. Goodman, PhD, the Executive Vice President and CEO of the Texas Medical Association (TMA), served in that capacity from 1997 through 2019.

Dr. Goodman is an economist with a decades-long perspective on the American health care delivery system. He is an astute leader who inspires his team to set and reach ever-higher goals in their work on behalf of US physicians. He melds a deep appreciation of the health care marketplace, politics, organized medicine, and human behavior.

As CEO, Dr. Goodman was responsible for the strategic, legislative, and operational direction of the TMA with more than 51,000 physician and student members.

Since his appointment as CEO, membership increased 47-percent. During his 31 years of experience at the TMA, Dr. Goodman leveraged his extensive expertise on such critical health care issues as physician shortages, tort reform, and the legislative, economic, and political determinants of health reform, on behalf of his member physicians.

Highlights and accomplishments for TMA under Dr. Goodman’s direction include the return of millions of dollars to Texas physicians as a result of the passage of an amendment nullifying the re-

coupment in the Omnibus Budget Reconciliation Act, historic state medical liability reform legislation that became the model for the entire country, and the repeal of the Medicare Sustainable Growth Rate formula.

Dr. Goodman has held numerous leadership positions at renowned medical societies and organizations, including president of the American Association of Medical Society Executives, chairman of the Texas Society of Association Executives, and honorary member of the American Medical Association.

He held the appointment of Adjunct Professor of Public Health at The University of Texas Health Science Center at Houston. He was a board member and former president of The Physicians Foundation and chair of its Medical Practice Committee, and was the treasurer of the Physician Advocacy Institute.

In addition to his leadership roles, Dr. Goodman has been recognized for his outstanding work in health care with the American Medical Association’s “Medical Executive Lifetime Achievement Award” and the Texas Society of Association Executives “Distinguished Executive Award.”

Dr. Goodman has a master’s and a doctoral degree in public policy and medical economics from New York University. He has published more than 70 articles on health care.

ONE STATE'S RESPONSE TO THE OPIOID EPIDEMIC & How Other States Might Not Be Ready for their Own

By Dylan Zayne Erwin Most people have felt the joy of holding a newborn. It is often visceral: the innocence, sounds, and smell can make us smile like few other things. This smile fades when we look down at what appears to be a perfectly healthy newborn, writhing in pain as their nerve endings fire in response to every sensation. This is the reality for thousands of newborns across the country fighting gestational opioid exposure, leading to Neonatal Abstinence Syndrome (NAS).

Treatment of NAS involves a hospitalization and a painful withdrawal from opioids in the first months of life.

I first grew interested in the opioid crisis while volunteering during college at a Dallas neonatal intensive care unit, where I witnessed infants with NAS daily. The United States Department of Health and Human Services has declared opioid usage and overdose a public health emergency1. Consequently, studies have shown an over five-fold increase in NAS rates in the last decade (Figure 1), leading to hundreds of millions of dollars in increased healthcare costs2.

Recently, I traveled to Huntington, West Virginia, considered by many to be the epicenter of the opioid crisis, to spend a few weeks FIGURE 1

learning more about the issue. West Virginia has the highest ageadjusted opioid overdose rate in the country (Figure 2), but the West Virginian healthcare system is rising to the meet this challenge2.

Throughout my visit, I met countless healthcare professionals dedicating their lives to confronting the epidemic. West Virginia has become a leader in opioid abuse treatment and detoxification. The state has championed residential and medication assisted treatment, along with unique outpatient programs aimed at treating NAS3.

A large amount of the funding for the advancement and innovation occurring in West Virginia is from Medicaid expansion4. West Virginia is one of the 36 states (plus Washington, DC) to expand Medicaid, and it has experienced one of the largest increases in Medicaid enrollment5,6. Since expanding, over 150,000 additional West Virginians have enrolled, contributing to a 56% reduction in the state’s number of uninsured. It is predicted that over 20,000 people within the expansion population have received opioid addiction treatment as a direct result of the expansion4.

With my own experiences in Dallas, and now seeing patients with opioid addiction in San Antonio, I have seen that Texas is not immune to the epidemic plaguing the rest of the country. However, statistically this might not appear to be the case, as Texas has one of the lowest opiate overdose rates in the country. Although, a deeper look shows that Texas has seen a twofold and threefold increase in heroin and fentanyl containing opiates in the last decade2. Texas ranks top-five in the U.S. in number of opioid related deaths, second highest in opioid related healthcare costs and in the bottom five for mental health expenditures. Specifically, the

San Antonio metroplex is FIGURE 2 one of the most severely affected regions in Texas. San Antonio accounts for onethird of all Texas cases of NAS, and the mortality rate for opioid-related overdose is almost double the national average7.

Politically, West Virginia and Texas appear similar.

But as Texas is one of the 14 states to not expand Medicaid, this added source of funding in West Virginia is not available in Texas. Texas has not experienced the rates of opioid usage seen in the mid-East, but as seen above, the statistics are cause for alarm. Since expanded Medicaid funding is not available, Texas should be proactively planning for unique funding opportunities to address this worsening problem.

Traveling to West Virginia also illustrated that the overall attitude towards opioid treatment is very different across the country and even from city to city in the same state. Local attitudes towards medication assisted treatment and needle exchange programs vary drastically. For example, local political debate and protests to treatment centers has led to increased tensions between the West Virginian cities of Charleston and Huntington, which are just an hour apart.

States that have not started to address the opioid crisis will struggle not only finding funding to manage the crisis, but also with de-stigmatizing treatment. Limited sources of funding, societal opinion regarding treatment, and a large at-risk population prime Texas and many other states to be potentially vulnerable to the worsening opioid epidemic. Currently, programs like the Texas Targeted Opioid Response are proactive measures underway in Texas and are a significant start, but continued work is needed to keep pace with increasing opioid usage8.

2020 Alfa Romeo Stelvio Quadrifoglio

AUTO REVIEW For many years there have been almost no bad cars for sale in the U.S. That doesn’t mean bad cars don’t exist, because they do.

It just means that if you wander into any car dealer and buy a new vehicle at random, that vehicle will almost certainly be good – it will start every morning regardless of the weather, run smoothly and trouble free day after day, and not leak or rattle. For 200,000 miles or more, in all likelihood.

Again, I’m not saying there aren’t lemons, because there are.

The wife of a cardiologist friend of mine just traded in her four year old Mercedes GLS after innumerable break downs, and yet consumer websites don’t flag the GLS as any more problematic than its competitors (and another friend’s wife loves her GLS, has had zero problems, and can’t wait to buy a new one).

What I’m saying is that, while you can point to the occasional problem car here or there, you can’t legitimately say, “don’t buy a 2020 model X because most of them are bad,” while you could have said that about a 1983 Chevy Citation.

On the other hand, it’s much harder in 2020 than it was in 1983 to say that a certain vehicle is interesting. Back then we had Saab 900 Turbos, Volkswagen GTIs, and Plymouth Volares. By today’s standards all of those cars were unreliable and unsafe, but they were certainly interesting. Name a 2020 vehicle that’s actually interesting. That’s Ok, I’ll wait…

Oh, the McLaren Senna? Great, a million-dollar car. It had better be interesting, hadn’t it? I actually found one for a lot less money, the Alfa Romeo Stelvio Quadrifoglio. It’s a mid-size Audi

SQ5/BMW X3 M competitor that just brings it, and wow is it interesting.

For starters, it uses the same platform as the enthusiast fave, the RWD Alfa Guilia (FYI – all Stelvios are AWD), and then adds the joyous engine from the Guilia Quadrifoglio. That engine would happen to be a 2.9L 505HP twin-turbo V6 that’s essentially ¾ of a Ferrari V8, and I can definitely vouch for its awesomeness.

It’s loud and fast, and I found myself shifting down often using the huge Ferrari-like paddles – that don’t move with the steering wheel, as god and Enzo Ferrari intended – just so I could listen to this glorious internal combustion wonder. This engine is SO much more interesting than anything BMW, Mercedes, Audi, or Lexus have right now. Yes, I’m aware of all those AMG V8s, M turbo in-line sixes, and RS what-have-yous. This is better.

It’s a cliché to say that so-and-so crossover or SUV drives like a car, and mostly it’s true. But the Stelvio Quadrifoglio drives better than a car, at least most cars. Through any corner, turning is

quick and decisive, body lean is minimal, and balance tilts toward oversteer. In fact, driving this SUV is a joy except for low speed wheel hop when you turn the wheel too much and slightly excessive road noise at high speeds.

For the record, 0 – 60MPH takes just 3.4 seconds(!), and top speed is 177MPH. Not surprisingly, fuel economy is not a strong suit with EPA numbers that would certainly make Greta Thunberg frown: 17MPG city/23MPG highway. And while more pedestrian Stelvios go for $41,000 and change to start, the base price of a Stelvio Quadrifoglio is just over $80,000, a big number to be sure.

In fact, the Stelvio you want is more than $10,000 more than a BMW X3 Competition and, gulp, $25,000 more than an Audi SQ5 (neither of which is as interesting or fun as this inspired vehicle).

I would predict that BCMS’ Phil Hornbeak would be able to do much better than that price, as he generally does for BCMS members no matter which vehicle we’re shopping.

As you’d expect for a vehicle from Italy, the exterior design of the Stelvio is attractive and contemporary. And the interior is different from the usual Audi/Lexus/Cadillac fare. I especially enjoyed the Ferrari-esque red engine start button on the steering wheel and the large central tachometer, but mostly I liked that the overall feel inside the Stelvio Quadrifoglio is special and different from anything else on the market.

Regrettably, numerous reviewers have encountered quality glitches with both Guilias and Stelvios, and those problems are certainly of concern. I had no issues of any kind during my week

with the Stelvio Quadrifoglio, and Alfa Romeo spokespeople reassure me that fixes have been and will continue to be made. But that reality will need to be weighed against the fun/interesting/fast pluses that this vehicle brings in spades.

After decades of incredible progress, the automotive industry has brought us an amazing array of, umm, similar vehicles that are mostly devoid of personality and boring. Thank you, Alfa Romeo, for giving us vehicles that are interesting and fun. I salute you.
