The canyon is consisted of volcanic rocks. Its splendid structure extends to Uludağ crests, 6 km long in total.

Oylat Cave Oylat Cave is located 17 km southeast of İnegöl, within the borders of Hilmiye Village, and 1 km from the village. The cave begins where Oylat Canyon ends. The cave is accessed through the right side of the road leading to Oylat Thermal Spring Resort and is 700 meters long. The cave is a touristic spot and a cafe is located at the main entrance. The cave is formed of two interconnected levels. The clearance

between the beginning gallery and the end of the cave is 93 meters. The cave above is 3-15 meters high, the temperature varies between 14°C and 18°C, and the humidity is between 45% and 90%. Oylat Cave, where stalactites and dickites are illuminated, also holds stalactite pools with a depth of 3-4 meters.

Ayvaini Cave Ayvaini Cave starts from Doğanalan Village in Mustafakemalpaşa district, ends in Ayva Village in Nilüfer district, and has a total length of 5,5 km. The cave, which is accessible from both sides, is closed to tourism due to its steep entrances and preferred by professional mountaineers and cavers only. Karadonlu Creek runs through the cave and forms more than 30 large and small ponds across the cave. Also, the stalactites, dickites and travertines inside are wonders of nature.

Bayındır Cave Bayındır Cave is located near Bayındır Village in Büyükorhan district of Bursa. It’s about 1 km long. Since the air circulation inside is good, the cave offers a comfortable tour without any gears. The fascinating stalactites and dickites inside the cave remind Damlataş Cave.

The Necessity Of “Stroke Rehabılitation” For Paralytic Patients’ Quality Of Life By Dr. Metin YURDAKOŞ Physical Therapy and Rehabilitation Specialist

H

emiplegia, or stroke, as commonly referred to, is a syndrome that occurs as a result of changes in cerebral (brain) circulation, and it is characterized by functional problems on one of side of the body. Hemiplegic patients may experience problems in perceptual, motor, and sensory functions, and speech and mental abilities.Cerebrovascular events may occur due to reasons such as tumors, inflammation of the blood vessels, collagen vascular disorders and endocarditis. The most common cause of hemiplegia is a bleed in the brain or when the brain cannot get enough blood. Despite being described fundamentally as the loss of function on one side of the body, hemiplegia refers to the collection of all the clinical symptoms. The clinical symptoms may include manifestations of motor aphasia, shoulder subluxation, vision problems, incontinence, difficulties with memory, loss of sensory perception, and Horner syndrome. The symptoms of any one patient may include different combinations of the above dependingon the affected brain vessels and the way they were affected.

“The most common risk factors for stroke include high blood pressure, diabetes and cardiovascular diseases.Obesity accompanied by high cholesterol, smoking, alcohol use, a sedentary lifestyle, and some medications contribute to the risk factors. Dr. Metin YURDAKOŞ from Bursa ROMMER Physical Therapy and Rehabilitation Center focused on stroke, related hemiplegia risks and hemiplegia rehabilitation. Physical Therapy and Rehabilitation Specialist Dr. Metin YURDAKOŞ explained that our brain has two hemispheres, and the right hemisphere controls the movements of the left arm and leg while the left hemisphere is responsible from the movement of the right arm and leg in addition to our speech ability; and emphasized that any dysfunction that occurs in the blood circulation of these areas results in stroke which may affect half of the body, arm and leg. Affirming that it is possible to categorize vessel events that cause stroke in two groups, Specialist Dr. Metin YURDAKOŞ said: “Ischemic events, meaning relating to blockage of vessels: These occur when brain tissue

in a certain region is damaged due to an embolism in that region. The cause of stroke in 80% of patients is an embolism. Those that are due to bleeding usually occur as a sudden aneurysm in patients with high blood pressure or patients with vasodilation in the form of a little bubble-like structure.” Indicating that the symptoms vary depending on which vessel in the brain is affected to what degree, Dr. YURDAKOŞ said:

Cervical hernia or myalgia (muscle Pain)? You are suffering from a neck pain nowadays. Does this pain radiate to your arm, cause numbness in your hand and/or impair your quality of life? This article will be helpful for you. We interviewed Assoc. Prof. Melih Çekinmez, M.D., the Director of Neurosurgery Department in Bursa Medicalpark Hospital, about neck pain and cervical hernia.

Neck pain is a condition which every individual face with at any period of life. These pains may result from simple muscle spasm, mechanical strain and sprain, arthritis (joints and spine), soft tissue diseases and strains, inflammations-infections of spine and soft tissue, rheumatoid diseases, tumor diseases and cervical hernias. Neurosurgeons first take a detailed anamnesis of patient in order to understand exact etiology of the pain. We classify the pains according to the location, onset time, duration, severity, spreading, type (burning, stinging, sharp), response to resting and movement and whether it wakes up the patient at night or not.

Is surgical treatment necessary for every cervical hernia? Of course not. Cervical hernia accounts for only a small part of neck pains. Moreover, surgical management is indicated only in a very small part of cervical hernias.

The Goal of Rehabilitation

Assoc. Prof. Melih ÇEKİNMEZ, M.D.

Does cervical hernia surgery pose risk? Cervical hernia surgery poses risks that are already valid for all surgeries. However, risks specific to cervical hernia surgery include temporary-permanent hoarseness, difficulty swallowing and strength-sensory loss in hands and feet. Undoubtedly, these risks can be minimized with microscopic discectomy and experience of the surgeon. Patients can be discharged and resume daily life activities at postoperative day 1, if micro discectomy method is used.

With the Purpose of Rehabilitation, • During hemiplegia rehabilitation, various electrotherapy methods are used in addition to individualized neurophysiological exercises.

enthusiasm, and also that their family is aware of their treatment plan. At Bursa-based ROMMER Physical Therapy and Rehabilitation Center, every patient’s condition is evaluated independently, and a personalized treatment program is prepared. First of all, the goal is for the patient to regain enough mobility to be able to perform basic tasks of daily living.

• Also, technology, which has an important role in our lives, is put to use in hemiplegia rehabilitation as well. Robotic rehabilitation systems train the patient to walk correctly, and computerized balance systems play an important role in increasing the patient’s balance control and coordination.

Once the patient masters sitting and maintaining balance while standing, walk training begins. Walking may be delayed in some patients. It is possible to overcome obstacles with an experienced team and a good treatment program so that the patient can train to be able to initially walk with support and later, walk without support in a short amount of time.

• While occupational therapy assists the patient in adapting to their daily lives, the sensory integration training provided at the same time helps the

patient regain function for a more integrated treatment.

ROMMER achieves treatment of all kinds of hemiplegia disorders with computerized balance equipment, robotic rehabilitation systems and occupational therapy methods.

Dr. YURDAKOŞ underlined the importance of starting rehabilitation right away the next day in the case when stroke is caused by an embolism with these words: In patients with bleeding, the stroke is often accompanied by loss of consciousness and therefore waiting for a few days is necessary, and some patients may require surgery. In the meantime, the bleeding is observed with methods such as a CT or MRI, and the necessary drug treatment is prepared.Since the patient is required to stay in bed during this time, extra attention should be paid to their position on the bed, their diet, urination frequency and quality, and infections that may occur and interfere with treatment.”

Can you brief us about the nature of cervical hernia or myalgia?

On the second phase, we assess the limitation of neck motion, pain spot and spreading of pain in the neurological examination and we evaluate strength-sensory losses. In the next phase, if there is a suspicious neurological loss, we make diagnosis based on magnetic resonance imaging, computed tomography and radiographies. Therapeutic approach includes resting, medication therapy, physiotherapy (hot bag, TENS, traction), shortterm collar and surgery, if required.

“The most important clinical observation would be the paralysis of the arm and leg together, either on the right or the left side of the body.In addition to loss of movement, symptoms such as loss of consciousness, perceptual or sensory dysfunction, problems with speech, vision problems, and loss of memory may be observed.Obesity accompanied by high cholesterol, smoking, a sedentary lifestyle, alcohol and some medications increase the risk of stroke.Especially those that have high blood pressure, diabetes and cardiovascular diseases should pay close attention these risk factors.”

cific neurological disorders. For this reason, it is very important to start hemiplegia rehabilitation as early as possible after a stroke.

The goal of rehabilitation is to minimize the patient’s bodily losses and increase functional capacity, help them accomplish daily tasks independently and improve their quality of life. For certain patients, it is crucial that they are treated and rehabilitated at a rehabilitation center with proper equipment and a specialized team in order for them to be able to return to their jobs or get another job. With this goal in mind, a team consisting of the physical therapy specialist physician, physiotherapist, and occupational therapist work together. When necessary, other specialists such as a speech therapist, psychologist, and social services expert contribute a lot to the rehabilitation team. The return of function after the cerebrovascular event is correlated closely with etiology, severity, and localization.Factors that affect the patient’s recovery include the age of the patient, their motivation, family and socioeconomic status, and spe-

• People with brain lesions have trouble structuring and organizing information. The patient may not be able concentrate on planning an activity, pay attention during the activity or comply with the stages of action that an activity may require. For this reason, it is important to evaluate and improve sensory perception functioning of people with brain lesions. This is where occupational therapy comes into play. When hemiplegia patients are treated at well-equipped physical therapy and rehabilitation clinics with an experienced team, they improve faster, and are likely to join their communities and resume their daily activities and lives sooner.

Rehabilitation Process After the patient comes to the rehabilitation center, a detailed assessment is performed, and treatment begins. It is important that the patient participates in the program voluntarily and with

At the end of the rehabilitation process, a majority of the patients are able to return to their social and professional lives in addition to having regained their walking ability.

Preventative Measures Against Stroke We should remember that, in order to prevent vascular diseases like stroke, simple precautions such as: • Reserving some “me time” every day and resting, • Following the principles of an orderly life and a balanced diet, • Avoiding obesity and a sedentary lifestyle, • A proper amount of exercise, • Avoiding smoking and alcohol, • Getting a physical check-up regularly every year; would protect us from big risks down the line.

Bursa Thermal Springs

Bursa is one of the richest cities in the world especially in terms of thermal springs and Hamams. The earliest information about Bursa Thermal Springs can be seen in the speeches of Dion in year 82. While repairing the Hamams left over from the Byzantines, the Ottomans were building new hot springs and thermal Hamams at the same time. Bursa’s thermal hot springs come out from the Bademli Bahce and Cekirge regions, west of the city. As the chemical analysis of the thermal waters is different in each region, it is also different with the ones that come out of the same region. So, it can be accepted that there are no connection between the sources. Waters in Cekirge are called steely waters and the waters in the Bademli Bahce are sulfurous waters. Water at the Kara Mustafa Thermal Springs is totally different. Water from the stone depot located in the Vakif Bahce (Vakif Garden) is distributed to 32 places. Among these areHamams, hotels and residential homes. Eski Kaplica (Old Thermal Spring) is Bursa’s oldest and the largest Thermal Spring. You can relax your body that got fatigued from the intense pace and revive and renew your worn-out soul and health at the thermal springs where the healthy and healing waters are. Bayezid, Yeni Kaplica (New Thermal Spring) built by Rustem Pasha, the vizier of Sultan Suleyman the Magnificent and the Kara Mustafa Pasha Thermal Springs which was built in his own name. Outside the center of Bursa, there is Oylat Thermal Springs and Citli Mineral Water in Inegol, Tumbuldek Thermal Springs in Akarca Village under Mustafakemalpasa, Terme Thermal Springs on the highway of Umurbey Village in Gemlik and Agachisar and Sadag Thermal Springs in Orhaneli and they all have continued to deliver healing for centuries.

B Bursa puts emphasis on far reaching project on silk and silk fabric production

ursa Metropolitan Municipality launched a new training project to produce silk as it was the case in the past. Within this framework, educational and training sets were prepared and distributed to the students. In order to feed silk worms, mulberry leaves are procured from Çaltılıbük quarter of former Kirmasti. Traditionally, Bursa was famous for being the largest center of silk trade in the Byzantine and later the Ottoman empires, during the period of the lucrative Silk Road. The city is still a major center for textiles in Turkey. Once famous for its silk, Bursa is now using silk as a nostalgic way to convey the message of Bursa. Bursa was a final stop on the fabled Silk Road, with the precise combination of soil and climate needed for the cultivation of mulberry trees. Mulberry leaves are the food of choice for silkworms, so the industry grew quickly. Bursa silk soon became a luxury.Until recently, every village around Bursa had at least a

handful of houses where the worms were cultivated and their gossamer cocoons spun into silken thread. Women wove it, artists designed endlessly varied patterns to be printed on it and men fanned out through Asia Minor, North Africa and Europe to sell it to eager customers. Bursa rose with the reputation of its silk, its name becoming a synonym for wealth as well as for delicacy and beauty. As recently as a generation ago, the silk industry here was thriving. It was still based on centuries-old methods practiced by villagers who had learned them from parents and grandparents. Bursa Metropolitan Municipality puts emphasis on reviving the silk production. Bursa was the main textile manufacturing centre of the Ottoman Empire. In the early 1850s Bursa had fourteen textile mills equipped with steam driven machinery like their counterparts in Europe, and there were a further two in Mudanya. In Bursa there were around 150 to 200 looms

weaving tulle, and pure and mixed silk fabrics. The traditional looms used in Bursa were extremely simple, consisting of a rectangular wooden frame on which the weft threads were stretched, and two cylinders for rolling up the fabric as it came off the loom. Lead weights kept the threads balanced and in tension as the alternate threads were pulled forward by a foot pedal for the shuttle to cross between them. Apart from the weights every part of the loom was made of wood. Bursa fabrics were celebrated far beyond the borders of the Ottoman Empire. They were exported as far afield as China, and filled the markets of Hungary, Poland, Italy and the Balkan countries. In the sixteenth century rich fabrics woven in Bursa from silk, wool and silver and gold thread were used for clothing worn by the Ottoman sultans and princes. Bursa fabrics included velvets, the velvet brocade known as çatma-which was also woven in Bilecik and Üsküdar, diba- a brocade incorporating gold thread, and a fine taffeta known as canfes.
