The region is cool in the summer and mild in the winter. The first impression of the city might be that of a large, concrete-ridden modern metropolis that is betraying its largely recognized Turkish epithet of Yeşil Bursa (“Green Bursa”), and its historical prominency as being the cradle of the Ottoman state; however, upon a closer look you will see it really lives up to its name, by proudly displaying its Ottoman heritage in the shape of many mosques, tombs, and lovely quarters of old houses, as well as by harboring many pleasant parks, which fill the city with fresh air, and provide the weary traveller some shade to rest in—and even if you haven’t found the parks sufficient enough to have this city earned the honorific of “green”, then the lush woodlands of Uludag Mountain is just above that steep hill. Bursa offers a series of challenging and diverse range of opportunities for health tourism
Associate Professor Ömer Tontuş, General Director, Health Improvement Unit of Turkish Health Ministry
BURSA- Bursa hosted “2nd Bursa Tourism Summit” on December 18-19 at Merinos Congress and Culture Centre. The Summit was organized by the Governorship of Bursa and the Metropolitan Municipality of Bursa by cooperating with some leading Non-governmental Organizations. On the occasion of the Summit, Associate Professor Ömer Tontuş, General Director, Health Improvement Unit of Turkish Health Ministry, pointed out, “ Bursa offers a series of challenging and diverse range of opportunities for health tourism. Health tourism has an important opportunity considering the potential of our country in parallel with the gradually increase of receiving medical treatment, the use of thermal sources, rehabilitation services and services that allows people to feel good about themselves and healthy.” “Health tourism is to go to another country in order to get the medical treatment people need, use thermal sources, get rehabilitation services or allow them to feel good and healthy. Three sub-components of health tourism according to the classification generally accepted in the literature are thermal tourism, medical tourism, and the care and rehabilitation services for the elderly.” He said, “In recent years, especially in developed countries gradually aging of the world population and increase of the weight of chronic diseases in disease burden and consequently increased treatment costs cause the people go to countries with lower costs in order to be treated. Long waiting times for treatment in their respective countries in addition to the costs also prompt the people to receive services abroad. Therefore, the increase in demand to get rid of long waiting times and to receive services in lower cost causes medical tourism increasingly gain
Insulin therapy in diabetes mellitus Diabetes is a chronic and progressive disease which occurs with total deficiency of insulin secretion and/or the decrease in the biological activity of insulin, resulting with hyperglycemia (high blood glucose ) and accompanied with dysfunction of the carbohydrate, lipid, protein metabolisms. It is classified as Type I, Type II, specific types and gestational diabetes mellitus. Type I diabetes mellitus accounts for approximately 10% of the all cases. It occurs before the age of 20 in many of the patients and the patients certainly need insulin to maintain their life. The risk of Type I diabetes for the people in who has history of diabetes in their family are; 3% if their mother has Type I diabetes, 6% if their father has Type I diabetes, 6% if their siblings have Type I diabetes. The biggest risk is in identical twins which is between 30-40%. The 90% of diabetic patients have Type 2. It can be seen at all ages but it is more common above the age of 30. 90% of the individuals who have Type 2 diabetes are obese (overweight). Obesity is especially located at abdominal region. There are also Type I diabetes cases which occur at advanced ages and Type 2 diabetes cases at early ages. The diagnosis of diabetes mellitus is made with plasma glucose tests. The fasting plasma glucose amount higher than 126mg/dl in the blood which is drawn after minimum 8 hours of fasting, Higher 2nd hour plasma glucose level than 200mg/dl following the 75 g oral glucose loading or higher plasma glucose level than 200mg/dl in the blood samples obtained from an adult who has common symptoms of classical diabetes mellitus such as drinking high amount of water, urinary frequency, weight loss, without taking fasting-posprandial situations into account, results with evident diabetes mellitus diagnosis.
importance.” “On the other hand, to be able to learn about hospitals abroad easily due to the increase of cross-country transportation facilities and the rapid development of communication technology makes it easier for people to go abroad to receive health care” he said. Associate Professor Ömer Tontuş said, “With this program, it is aimed to increase the competitiveness of Turkey in general, Bursa in particular, increasing service quality medical tourism, thermal tourism and elderly-disabled tourism areas that are in emerging markets position in the world in parallel with developments in the health sector in recent years.” As for the major targets of the Program of SATURKK- Health Tourism Coordination Council of Turkish Health Ministry, Associate Professor Ömer Tontuş underlined the following: - Establishing 100.000 bed capacity in health tourism - Supplying service to 1,500,000 (600,000 treatment purposes) foreign tourists in thermal tourism - Obtaining 3 billion dollars revenue in thermal tourism - Being among top 5 global destinations in medical tourism - Providing medical treatment to 750,000 foreign patients - Obtaining 5.6 billion dollars revenue in medical tourism - Reaching 150,000 foreign visitors in elderly tourism - Obtaining 750 million dollars revenue in elderly tourism According to data from the Association of Turkish Travel Agencies (TÜRSAB), Turkey has hosted nearly 500,000 visitors from abroad in 2015 for medical treatment, including in sub-sectors such as thermal therapy centers used mostly by the elderly. The main specialties in Turkey drawing medical tourists were treatment of eye diseases, hair transplantation, dentistry, and plastic surgery in 2014,
Complications such as microvascular complications like nephropathy (kidney disease), retinopathy (eye disease), neuropathy (nerve fibers disease) and macrovascular complications like coronary artery disease (cardiovascular disease), cerebrovascular disease (cerebral vessels disease), peripheral vessel disease develop. These complications not only shorten the life time of the diabetic patients but also decrease their life quality.
The main component of a diabetes mellitus treatment is a well-balanced and nutritious diet. Foods should be consumed as three main and three snacks according to height, weight and daily activity of every single individual. Diabetes training and regular physical activity are integrated parts of therapy. Subcutaneous or intravenous insulin therapies are used along with the pharmacologic oral anti-diabetic agents to hold the plasma glucose levels within the normal ranges. Insulin is a protein-structured hormone which is secreted from pancreas and decreases blood glucose level. It was first isolated from dog pancreas by Banting and Best in 1921 and applied to a diabetic patient in 1922. The commercial production of insulin started in years following the fist application. While the insulin from cattle and pig pancreas was used firstly, human insulin was commercially produced in 1980. Nowadays, human insulin that is produced from E.Coli bacteria or yeast cell with DNA technology is used. Insulins are classified as very short acting (insulin lispro and insulin aspart), short acting (crystalline insulin and semilente insulin), intermediate acting (NPH insulin and lente insulin) and long acting (glargine insulin, insulin detemir, ultralente insulin) according to their acting durations. The acting of very short acting insulins starts in 5-15 minutes and lasts 3-5 hours, the acting of short acting insulins starts in 30 minutes and lasts 6-8 hours, the acting of intermediate acting insulins starts in 2-4 hours and lasts 14-18 hours and the acting of long acting insulins starts in 2 hours and lasts 24 hours on average. The ready-mixture insulins are obtained by mixing short and very short acting insulins with intermediate acting

insulins and commonly used in therapy. Insulin therapy can be applied as daily single dose, double dose, multiple doses and as continuous subcutaneous infusion. Insulin combinations can be made with oral anti-diabetic agents for Type 2 diabetes mellitus patients. Generally, it is preferred to apply long acting and single dose insulin with oral anti-diabetic agents. In insulin pump application, very short acting or short acting insulin is continuously given through the reservoir inside the topically-existing pump to the abdominal region with plastic catheter. The advantages of this are good blood glucose regulation, non-existence of serious hypoglycemia (reduced blood glucose level), the easy keeping up with the life conditions, indecency of daily injection. The complications such as hypoglycemia, lipoatrophy (local loss of subcutaneous fat tissue in injection regions), lipohypertrophy (local swelling of subcutaneous fat tissue in injection regions), insulin edema, antibody development against insulin, insulin allergy, insulin resistance, weight gain (up to 2-1 kg) can be seen with insulin therapy. Appropriate arrangement of insulin doses, compliance with diet, using injection regions by turns, using purified human insulins can reduce this risks. Insulin does not lead to addiction. The discourse “When you use insulin once, you cannot give up.” is not true. It can be possible to use in pregnancy period, diabetic comas, the pre-operative periods, in situations that blood glucose level elevates due to infection and then switch to other therapies. Continuous insulin therapy is needed for Type I diabetes mellitus patients, Type II diabetes mellitus patients that do not benefit from oral anti-diabetic therapy, patients whose pancreas is extracted surgically, patients with chronic degenerative disorders. In short, insulin therapy is planned individually.

It is clearly seen that health tourism investments are gradually increasing worldwide and health tourism becomes an important sector. Bursa is an important city which is attractive for health tourism investments due to natural substructure, serious thermal potential and especially historical and cultural relations for Arabic and African countries and Turkic Republics. It is a candidate region to be the capital of health tourism in Turkey with the new and modern facilities that will be built in health tourism by both public and private sectors. Extremely modern and well-equipped Public Hospitals are built at the city center and districts. All efforts are rapidly ongoing for large-scale city hospital, which is planned to be constructed in Bursa. Although these hospitals are planned for local patients, they represent important potentials for health tourism in subsequent period. There are many hospitals, which are actively operated, about to be commission or still constructed, in Bursa. We know that these hospitals that are projected by both public and private sectors and put into the service or ready to be put into the service can compete with renowned hospitals worldwide. Thus, we can undoubtedly say that Bursa is ready for the health tourism. As the process goes forward, the existing charming opportunities for the new investors in this field will increase. From the perspective of thermal health tourism, Bursa had been the capital of thermal springs for centuries, but it failed keeping pace with modern transformation. Recently, serious thermal investments, pioneered by the public sector, have started to arrive Bursa. There are many untouched, available areas for thermal investments that can be integrated to ecotourism, nature tourism, history tourism, culture tourism, winter and summer tourism in Bursa which is one of the most important centers in the world by means of the quality of hot waters. These opportunities can be found in central districts and all districts especially in Inegöl, Mustafa Kemal
Pasa, Harmancık, Orhaneli, Orhangazi, Karacabey. When we look from this aspect, the works to make the hydrothermal area which is under the investments of Metropolitan Municipality a thermal cure center with rearrangements and new facilities. With this work, the congress center, city park and a very important area within the historical view in the city center is saved for the health tourism. Here, the areas for both public and the private sectors that want to make investments will be opened. When we see the situation by means of the districts, we can see that new investment areas are being formed rapidly. What is especially expected from the local authorities is their forming appropriate and inexpensive investment areas with a ready substructure for the private sector investors instead of making investments by themselves. We know that the local authorities of Bursa see the issue in this way and do important works in this field. We are aware of the fact that in many of our districts, there are very important areas for this subject and they should be utilized. After Bursa was accepted to EHTA (European Historical Thermal Towns Association) the awareness has increased more and our local authorities started to consider this issue in a more sensitive manner. We will see that the big investments of the important investors from the country and also from abroad who we know are interested in Bursa will start to be realized. Within the next decade, it will not be a surprise to see Bursa as one of the most important Health Tourism investment areas of the world. It is clear that we will begin to experience a process in which the other parallel investments in third age tourism, healthy aging and the other aspects of health tourism will simultaneously take place. In conclusion, Bursa is waiting for its investors as an important investment for both national and international investors.
